Abstract

The present study was conducted to evaluate the anthelmintic activity of aqueous extracts of Azadirachta indica (Neem), Mallotus philippinensis (Kamala), Melia azedarach (Bakain), and their combination as herbal extracts against Ascaridia galli infection. To determine efficacy, egg hatch test (EHT) and adult motility assay (AMS) were performed using increasing concentrations of 12.5, 25, 50, and 100 mg/ml of aqueous plant extracts, in conjunction with 7.5 mg/ml Albendazole as the positive control and phosphate buffered saline (PBS) as the negative control for up to 15 h. There was 100% mortality achieved with the combined herbal extract (LC50 = 36.7 µg/ml), whereas mortality with extract from Mallotus 50 philippinensis (LC50 = 41.4 µg/ml), Melia azedarach (LC50 = 147 µg/ml), and 50 50 Azadirachta indica (LC50 = 189 µg/ml) were 87.5%, 62.5%, and 50% respectively, 50 at 15 hours post-exposure. These findings suggest that Azadirachta indica, Mallotus philippinensis, Melia azedarach herbal extracts, or their combination, should be further exploited to control Ascaridia galli infection. Abstract

Guiera senegalensis is a shrub that grows up to a height of 3 to 5 m and has tremendous medicinal properties. However, there is difficulty in the identification of medicinal plants leading to substitution of one plant for another. This problem can be addressed by establishing fingerprints of the medicinal plants. The aim of this research is to establish FTIR fingerprint of G. senegalensis leave. G. senegalensis leave were collected from the wild in Jigawa, Kaduna and Zamfara states, shade dried and size reduced. Samples (2 mg) were analysed in triplicate using Agilent technologies FTIR Carry 630 in the mid-IR region 4000-650 cm-1 at resolution 8 cm-1 with 16 scans. Abstract

Background The complexity of oesophageal surgery and the significant risk of morbidity necessitates that oesophagectomy is predominantly performed by a consultant surgeon, or a senior trainee under their supervision. The aim of this study was to determine the impact of trainee involvement in oesophagectomy on postoperative outcomes in an international multicentre setting. Methods Data from the multicentre Oesophago-Gastric Anastomosis Study Group (OGAA) cohort study were analysed, which comprised prospectively collected data from patients undergoing oesophagectomy for oesophageal cancer between April 2018 and December 2018. Procedures were grouped by the level of trainee involvement, and univariable and multivariable analyses were performed to compare patient outcomes across groups. Results Of 2232 oesophagectomies from 137 centres in 41 countries, trainees were involved in 29.1 per cent of them (n = 650), performing only the abdominal phase in 230, only the chest and/or neck phases in 130, and all phases in 315 procedures. For procedures with a chest anastomosis, those with trainee involvement had similar 90-day mortality, complication and reoperation rates to consultant-performed oesophagectomies (P = 0.451, P = 0.318, and P = 0.382, respectively), while anastomotic leak rates were significantly lower in the trainee groups (P = 0.030). Procedures with a neck anastomosis had equivalent complication, anastomotic leak, and reoperation rates (P = 0.150, P = 0.430, and P = 0.632, respectively) in trainee-involved versus consultant-performed oesophagectomies, with significantly lower 90-day mortality in the trainee groups (P = 0.005). Conclusion Trainee involvement was not found to be associated with significantly inferior postoperative outcomes for selected patients undergoing oesophagectomy. The results support continued supervised trainee involvement in oesophageal cancer surgery., The aim of this study was to determine the impact of trainee involvement in oesophagectomy on perioperative outcomes in the international multicentre Oesophago-Gastric Anastomosis Audit (OGAA). Analysis of 2232 oesophagectomies has shown that trainee involvement did not negatively impact perioperative outcomes.

Abstract

Background The Oesophago-Gastric Anastomosis Audit (OGAA) is an international collaborative group set up to study anastomotic leak outcomes after oesophagectomy for cancer. This Delphi study aimed to prioritize future research areas of unmet clinical need in RCTs to reduce anastomotic leaks. Methods A modified Delphi process was overseen by the OGAA committee, national leads, and engaged clinicians from high-income countries (HICs) and low/middle-income countries (LMICs). A three-stage iterative process was used to prioritize research topics, including a scoping systematic review (stage 1), and two rounds of anonymous electronic voting (stages 2 and 3) addressing research priority and ability to recruit. Stratified analyses were performed by country income. Results In stage 1, the steering committee proposed research topics across six domains: preoperative optimization, surgical oncology, technical approach, anastomotic technique, enhanced recovery and nutrition, and management of leaks. In stages 2 and stage 3, 192 and 171 respondents respectively participated in online voting. Prioritized research topics include prehabilitation, anastomotic technique, and timing of surgery after neoadjuvant chemo(radio)therapy. Stratified analyses by country income demonstrated no significant differences in research priorities between HICs and LMICs. However, for ability to recruit, there were significant differences between LMICs and HICs for themes related to the technical approach (minimally invasive, width of gastric tube, ischaemic preconditioning) and location of the anastomosis. Conclusion Several areas of research priority are consistent across LMICs and HICs, but discrepancies in ability to recruit by country income will inform future study design.

Abstract

Background Textbook outcome has been proposed as a tool for the assessment of oncological surgical care. However, an international assessment in patients undergoing oesophagectomy for oesophageal cancer has not been reported. This study aimed to assess textbook outcome in an international setting. Methods Patients undergoing curative resection for oesophageal cancer were identified from the international Oesophagogastric Anastomosis Audit (OGAA) from April 2018 to December 2018. Textbook outcome was defined as the percentage of patients who underwent a complete tumour resection with at least 15 lymph nodes in the resected specimen and an uneventful postoperative course, without hospital readmission. A multivariable binary logistic regression model was used to identify factors independently associated with textbook outcome, and results are presented as odds ratio (OR) and 95 per cent confidence intervals (95 per cent c.i.). Results Of 2159 patients with oesophageal cancer, 39.7 per cent achieved a textbook outcome. The outcome parameter ‘no major postoperative complication’ had the greatest negative impact on a textbook outcome for patients with oesophageal cancer, compared to other textbook outcome parameters. Multivariable analysis identified male gender and increasing Charlson comorbidity index with a significantly lower likelihood of textbook outcome. Presence of 24-hour on-call rota for oesophageal surgeons (OR 2.05, 95 per cent c.i. 1.30 to 3.22; P = 0.002) and radiology (OR 1.54, 95 per cent c.i. 1.05 to 2.24; P = 0.027), total minimally invasive oesophagectomies (OR 1.63, 95 per cent c.i. 1.27 to 2.08; P < 0.001), and chest anastomosis above azygous (OR 2.17, 95 per cent c.i. 1.58 to 2.98; P < 0.001) were independently associated with a significantly increased likelihood of textbook outcome. Conclusion Textbook outcome is achieved in less than 40 per cent of patients having oesophagectomy for cancer. Improvements in centralization, hospital resources, access to minimal access surgery, and adoption of newer techniques for improving lymph node yield could improve textbook outcome.

Abstract

Weeds incur significant losses to the crop plants by not only competing with crops for water, nutrients, sunlight, and space but also harbour insects and diseases. The herbicide resistant cotton offers farming community an opportunity to combat weeds by providing flexibility of herbicide application whenever needed. In the present study, the herbicide resistant cotton was evaluated to unravel the glyphosate dose response at different time intervals. Glyphosate was applied on advance transgenic cotton line VH-289 at the time intervals of 20, 25, 30 and 40 days after germination and was compared with manual weeding of its conventional non-transgenic counterpart as control. The weed control efficacy, types, mortality, and population of nineteen different weeds were calculated with reference to the time and dose of glyphosate spray [600 mL.acre -1 (242 mL.ha-1), 800 mL.acre -1 (324 mL.ha-1), 1000 mL.acre -1 (305 mL.ha-1), 1200 mL.acre -1 (486 mL.ha-1), 1400 mL.acre -1 (566 mL.ha-1), 1600 mL.acre -1 (647 mL.ha-1), 1800 mL.acre -1 (728 mL.ha-1) and 2000 mL.acre -1 (809 mL.ha-1)] and its impact on yield. The yield and contributing characters like boll weight and cotton yield were higher in lines where glyphosate was applied at 20 and 30 days after germination compared to other treatment (manually hoed). Importantly, weeds control efficiency was recorded up to 98% at a dose concentration of 1800 mL.acre -1 and 2000 mL.acre -1 after 15-30 days after glyphosate application (DAS). The average number of bolls per plant in glyphosate resistant line was recorded higher compared to non-transgenic manually hoed control. This study highlights that spraying 1800 and 2000 mL.acre -1 glyphosate can help to eradicate weeds in field at less cost and good yield return. Abstract

This research work incorporates experimental investigation and simulation to inspect the impact of influential parameters on the efficiency of a forced draft cooling tower. The number of packing ribs, the flow rate of water, the flow rate of air, the surface area, and the area of contact are the parameters investigated in this study. All of these parameters are altered and their effect on efficiency is explored. Two types of packing with varying rib numbers of packing and areas are utilized to observe their influence on the efficiency of the cooling tower. Modeling and CFD analysis of the aforementioned cooling tower is done on SOLIDWORKS and ANSYS Fluent respectively. Finally, the results from experimental data and CFD analysis are compared to inspect the effectiveness of the simulation. The results depicted that the efficiency of a forced draft cooling tower varies directly with the flow rate of air and inversely with the flow rate of water. Moreover, the performance of the cooling tower has a direct relationship with the area of contact, time of contact between air and water, and the number of ribs. The results obtained from CFD analysis are compared and validated with experimental results. Finally, percentage error analysis is also presented between simulation model efficiency and experimentation efficiency of the benchtop cooling tower and it exhibits good agreement with the experimental data. Abstract

Background: No evidence currently exists characterising global outcomes following major cancer surgery, including esophageal cancer. Therefore, this study aimed to characterise impact of high income countries (HIC) versus low and middle income countries (LMIC) on the outcomes following esophagectomy for esophageal cancer. Method: This international multi-center prospective study across 137 hospitals in 41 countries included patients who underwent an esophagectomy for esophageal cancer, with 90-day follow-up. The main explanatory variable was country income, defined according to the World Bank Data classification. The primary outcome was 90-day postoperative mortality, and secondary outcomes were composite leaks (anastomotic leak or conduit necrosis) and major complications (Clavien-Dindo Grade III-V). Multivariable generalized estimating equation models were used to produce adjusted odds ratios (ORs) and 95% confidence intervals (CI95%). Results: Between April 2018 to December 2018, 2247 patients were included. Patients from HIC were more significantly older, with higher ASA grade, and more advanced tumors. Patients from LMIC had almost three-fold increase in 90-day mortality, compared to HIC (9.4% vs 3.7%, p < 0.001). On adjusted analysis, LMIC were independently associated with higher 90-day mortality (OR: 2.31, CI95%: 1.17-4.55, p = 0.015). However, LMIC were not independently associated with higher rates of anastomotic leaks (OR: 1.06, CI95%: 0.57-1.99, p = 0.9) or major complications (OR: 0.85, CI95%: 0.54-1.32, p = 0.5), compared to HIC. Conclusion: Resections in LMIC were independently associated with higher 90-day postoperative mortality, likely reflecting a failure to rescue of these patients following esophagectomy, despite similar composite anastomotic leaks and major complication rates to HIC. These findings warrant further research, to identify potential issues and solutions to improve global outcomes following esophagectomy for cancer. Abstract

Background The complexity of oesophageal surgery and the significant risk of morbidity necessitates that oesophagectomy is predominantly performed by a consultant surgeon, or a senior trainee under their supervision. The aim of this study was to determine the impact of trainee involvement in oesophagectomy on postoperative outcomes in an international multicentre setting. Methods Data from the multicentre Oesophago-Gastric Anastomosis Study Group (OGAA) cohort study were analysed, which comprised prospectively collected data from patients undergoing oesophagectomy for oesophageal cancer between April 2018 and December 2018. Procedures were grouped by the level of trainee involvement, and univariable and multivariable analyses were performed to compare patient outcomes across groups. Results Of 2232 oesophagectomies from 137 centres in 41 countries, trainees were involved in 29.1 per cent of them (n = 650), performing only the abdominal phase in 230, only the chest and/or neck phases in 130, and all phases in 315 procedures. For procedures with a chest anastomosis, those with trainee involvement had similar 90-day mortality, complication and reoperation rates to consultant-performed oesophagectomies (P = 0.451, P = 0.318, and P = 0.382, respectively), while anastomotic leak rates were significantly lower in the trainee groups (P = 0.030). Procedures with a neck anastomosis had equivalent complication, anastomotic leak, and reoperation rates (P = 0.150, P = 0.430, and P = 0.632, respectively) in trainee-involved versus consultant-performed oesophagectomies, with significantly lower 90-day mortality in the trainee groups (P = 0.005). Conclusion Trainee involvement was not found to be associated with significantly inferior postoperative outcomes for selected patients undergoing oesophagectomy. The results support continued supervised trainee involvement in oesophageal cancer surgery. The aim of this study was to determine the impact of trainee involvement in oesophagectomy on perioperative outcomes in the international multicentre Oesophago-Gastric Anastomosis Audit (OGAA). Analysis of 2232 oesophagectomies has shown that trainee involvement did not negatively impact perioperative outcomes.

Abstract

Background: The complexity of oesophageal surgery and the significant risk of morbidity necessitates that oesophagectomy is predominantly performed by a consultant surgeon, or a senior trainee under their supervision. The aim of this study was to determine the impact of trainee involvement in oesophagectomy on postoperative outcomes in an international multicentre setting. Methods: Data from the multicentre Oesophago-Gastric Anastomosis Study Group (OGAA) cohort study were analysed, which comprised prospectively collected data from patients undergoing oesophagectomy for oesophageal cancer between April 2018 and December 2018. Procedures were grouped by the level of trainee involvement, and univariable and multivariable analyses were performed to compare patient outcomes across groups. Results: Of 2232 oesophagectomies from 137 centres in 41 countries, trainees were involved in 29.1 per cent of them (n = 650), performing only the abdominal phase in 230, only the chest and/or neck phases in 130, and all phases in 315 procedures. For procedures with a chest anastomosis, those with trainee involvement had similar 90-day mortality, complication and reoperation rates to consultant-performed oesophagectomies (P = 0.451, P = 0.318, and P = 0.382, respectively), while anastomotic leak rates were significantly lower in the trainee groups (P = 0.030). Procedures with a neck anastomosis had equivalent complication, anastomotic leak, and reoperation rates (P = 0.150, P = 0.430, and P = 0.632, respectively) in trainee-involved versus consultant-performed oesophagectomies, with significantly lower 90-day mortality in the trainee groups (P = 0.005). Conclusions: Trainee involvement was not found to be associated with significantly inferior postoperative outcomes for selected patients undergoing oesophagectomy. The results support continued supervised trainee involvement in oesophageal cancer surgery.

Abstract

Objective A hospital-wide difficult airway response team was developed in 2008 at The Johns Hopkins Hospital with three central pillars: operations, safety monitoring, and education. The objective of this study was to assess the outcomes of the educational pillar of the difficult airway response team program, known as the multidisciplinary difficult airway course (MDAC). Design The comprehensive, full-day MDAC involves trainees and staff from all provider groups who participate in airway management. The MDAC occurs within the Johns Hopkins Medicine Simulation Center approximately four times per year and uses a combination of didactic lectures, hands-on sessions, and high-fidelity simulation training. Participation in MDAC is the main intervention being investigated in this study. Data were collected prospectively using course evaluation survey with quantitative and qualitative components, and prepost course knowledge assessment multiple choice questions (MCQ). Outcomes include course evaluation scores and themes derived from qualitative assessments, and prepost course knowledge assessment MCQ scores. Setting Tertiary care academic hospital center Participants Students, residents, fellows, and practicing physicians from the departments of Surgery, Otolaryngology Head and Neck Surgery, Anesthesiology/Critical Care Medicine, and Emergency Medicine; advanced practice providers (nurse practitioners and physician assistants), nurse anesthetists, nurses, and respiratory therapists. Results Totally, 23 MDACs have been conducted, including 499 participants. Course evaluations were uniformly positive with mean score of 86.9 of 95 points. Qualitative responses suggest major value from high-fidelity simulation, the hands-on skill stations, and teamwork practice. MCQ scores demonstrated significant improvement: median (interquartile range) pre: 69% (60%-81%) vs post: 81% (72%-89%), p Conclusions Implementation of a MDAC successfully disseminated principles and protocols to all airway providers. Demonstrable improvement in prepost course knowledge assessment and overwhelmingly positive course evaluations (quantitative and qualitative) suggest a critical and ongoing role for the MDAC course.

Abstract

Purpose Few guidelines exist regarding the selection of a particular type or size of tracheostomy tube. Although nonstandard tubes can be placed over the percutaneous kit dilator, clinicians often place standard tracheostomy tubes and change to nonstandard tubes only after problems arise. This practice risks early tracheostomy tube change, possible bleeding, or loss of the airway. We sought to identify predictors of nonstandard tracheostomy tubes. Materials and methods In this matched case-control study at an urban, academic, tertiary care medical center, we reviewed 1220 records of patients who received a tracheostomy. Seventy-seven patients received nonstandard tracheostomy tubes (cases), and 154 received standard tracheostomy tubes (controls). Results Sex, endotracheal tube size, severity of illness, and computed tomography scan measurement of the distance from the trachea to the skin at the level of the superior aspect of the anterior clavicle were significant predictors of nonstandard tracheostomy tubes. Specifically, trachea-to-skin distance > 4.4 cm and endotracheal tube sizes ≥ 8.0 were associated with nonstandard tracheostomy. Conclusions The findings suggest that clinicians should consider using nonstandard tracheostomy tubes as the first choice if the patient is male with an endotracheal tube size ≥ 8.0 and has a trachea-to-skin distance > 4.4 cm on the computed tomography scan.

Abstract

Background: The concurrent use of herbs and drugs for the treatment of various ailments is a common practice amongst patients; a practice that could result in drug-herb interaction.Objectives: This study is aimed at evaluating the effects of Hibiscus sabdariffa on in vitro availability of lisinopril.Method: The availability of lisinopril alone and in presence of Hibiscus sabdariffa calyxes water extract was determined using dissolution apparatus (BP, 2013) set at 50 rpm and 37 °C in 900 mL of three different dissolution media [0.1 M HCl (simulated gastric pH), phosphate buffers pH 6.8 (simulated intestinal pH) and phosphate buffers pH 7.4 (simulated blood pH)]. Samples (5 mL) were withdrawn from the media at 5, 10, 15, 30, 45, and 60 min and replaced immediately with the same medium. Each sample was analysed for the lisinopril content released using UV spectrophotometry at 215, 210 and 215 nm in 0.1 M HCl, phosphate buffers pH 6.8 and phosphate buffers pH 7.4 respectively.Results: Results showed that the media has no effect on the dissolution profile of lisinopril alone, however, it was observed that 89.40 (lisinopril alone) and 92.62 % (lisinopril in the presence of Hibiscus sabdariffa) was released in simulated gastric pH. The corresponding contents of lisinopril observed in simulated intestinal pH were 89.40 and 92.51 %, while in simulated blood pH 89.40 and 91.95 % of lisinopril was released. The presence of Hibiscus sabdariffa significantly (p < 0.05) increased the in vitro availability of lisinopril in all the media.Conclusion: The results of this study suggest that coadministration of lisinopril with Hibiscus sabdariffa could enhance its in vitro availability consequent to the increased dissolution of lisinopril in simulated gastric, intestinal and blood pH.Keywords: Lisinopril, Interaction, Hibiscus sabdariffa, dissolution

Abstract

Cotton plants are normally targeted by numerous challenges from which 75% are biotic. Among these biotic stresses, insects constitute major segment causing direct or indirect considerable yield loss. Bt cotton, corn and soybean are mostly resistant to chewing insects in the field, but their resistance to sap sucking pests has not yet achieved. Likewise Bt protein against chewing insect, there are some proteins characterized for their actions against sucking insects e.g. lectins. Major function of plant lectin is to provide anti-insect activity with other known and unknown function. Therefore, all necessary in-silico, in-vivo and plant transformation experimentation adopted to design a codon optimized (cotton targeted) Pinellia ternata agglutinin (pta) gene cassette under the influence of well-tested CaMV35S promoter in pCAMBIA2300 expression vector enabled by agrobacterium LB4404 plant infecting strain for stable delivery and integration of transgene in the recipient genome. Stable transformation of at least five different cotton plants (transformation efficiency 0.53%) took place successively, which was tested through PCR and Southern blotting in T0 and T1 generation. This work has provided genotypic bases and a candidate phenotype of commercial cotton crop resistant against sucking pests. [ABSTRACT FROM AUTHOR]

Abstract

Students pursuing a medical career in the US are subject to standardized testing at regular intervals. These standardized tests not only quantify the milestones students have already achieved, but also define the path for future achievements. The purpose of these examinations is to help students become self-directed, lifelong learners - an essential attribute of a medical professional. However, whether preparing for these examinations actually makes students such disciplined learners needs to be examined. Especially during residency training with its limited time and unpredictable exposure, trainees must learn in the most efficient way for their learning styles, and thus develop attributes that will be helpful to them in their medical career. In this review, we propose that a personalized, learner-centered approach tailored to residents' educational needs and preferences can not only fulfill learning interests and objectives but also serve as a time-efficient and cost-effective approach for graduate medical education.

Abstract

Background: The genetic cause of primary immunodeficiency disease (PID) carries prognostic information. Objective: We conducted a whole-genome sequencing study assessing a large proportion of the NIHR BioResource–Rare Diseases cohort. Methods: In the predominantly European study population of principally sporadic unrelated PID cases (n = 846), a novel Bayesian method identified nuclear factor κB subunit 1 (NFKB1) as one of the genes most strongly associated with PID, and the association was explained by 16 novel heterozygous truncating, missense, and gene deletion variants. This accounted for 4% of common variable immunodeficiency (CVID) cases (n = 390) in the cohort. Amino acid substitutions predicted to be pathogenic were assessed by means of analysis of structural protein data. Immunophenotyping, immunoblotting, and ex vivo stimulation of lymphocytes determined the functional effects of these variants. Detailed clinical and pedigree information was collected for genotype-phenotype cosegregation analyses. Results: Both sporadic and familial cases demonstrated evidence of the noninfective complications of CVID, including massive lymphadenopathy (24%), unexplained splenomegaly (48%), and autoimmune disease (48%), features prior studies correlated with worse clinical prognosis. Although partial penetrance of clinical symptoms was noted in certain pedigrees, all carriers have a deficiency in B-lymphocyte differentiation. Detailed assessment of B-lymphocyte numbers, phenotype, and function identifies the presence of an increased CD21lowB-cell population. Combined with identification of the disease-causing variant, this distinguishes between healthy subjects, asymptomatic carriers, and clinically affected cases. Conclusion: We show that heterozygous loss-of-function variants in NFKB1 are the most common known monogenic cause of CVID, which results in a temporally progressive defect in the formation of immunoglobulin-producing B cells.

Abstract

Guiera senegalensis (Sabara in Hausa) is a shrub with numerous medicinal values attributed to its secondary metabolites e.g. quercetin. The aim of this work is to develop and validate an RP-HPLC method for the identification and quantification quercetin in Guiera senegalensis. After several trials, the optimized condition comprises of a C18 column (250 x 4.6 mm), ternary mobile phase (Methanol: Water: acetonitrile in 55:5:45) containing 0.1 % orthophosphoric acid as additive, isocratic elution mode, 255 nm detection wavelength, flow rate of 1.2 mL/min, 10 μl injection volume, a runtime of 7 minutes and at ambient temperature. Quercetin was found to be eluted at 2.2 minute. The calibration curve was linear within the range of 10-50 μg/mL (r2 = 0.999) and was sensitive as indicated by a lower LOQ (3.15 μg/mL). The method was found to be precise, accurate and robust as indicated by their respective values which are within the acceptable limits.Key Words: Guiera senegalensis, Quercetin, RP-HPLC, isocratic elution, ternary mobile phase.
