Abstract

In a study involving 77 patients affected by lichen ruber lenticularis oralis, the relative clinical long-term healing rate was determined after application of a "Systemic Mouthmucosal Photochemotherapy" (SMPT). The results obtained showed that 78.5 percent of the patients were cured while the disease recurred in 21.5 percent of the patients. Against of 12 patients with erosive Lichen ruber oralis have had 9 patients an recidive post Re-SMPT.

Abstract

Immunohistochemical investigations on skin lesions of lichen ruber planus indicate HLA-DR-expression by keratinocytes, Langerhans cells (LC), and infiltrated mononuclear cells. In the early stage of this condition, the lymphocytic infiltrate consists of a considerable part of (CD8+) cytotoxic/suppressor- and (LGL+) natural killer cells. Evidently, the cytotoxic potential of these lymphocyte subpopulations is directed against basal keratinocytes and epidermal LC. As a result of cytotoxic/cytolytic activities, non-cell associated CD 1-reactivity accumulates in the dermoepidermal junction zone, thus indicating damaged LC.

In the group of perforating skin diseases with transepithelial elimination the hyperkeratosis follicularis et parafollicularis in cutem penetrans is very rare. By means of casuistics the clinical and histological features are discussed. Re-PUVA as a feasible therapy variant was performed.

Abstract

145 patients with lichen planus cutaneous were treated with PUVA. In a follow-up period of 1 to 4 years 6 of the patients (4.1%) showed a generalised recurrence and 32 patients (22%) a local recurrence. For the first time 3 morphological PUVA-resistance phenomena were observed. Recurrence- and resistance correlation are discussed.

Abstract

In a histological and immunofluorescence microscopic study on 145 patients suffering from lichen planus the PUVA-effects on the keratin bodies are observed. The formative "stop" of the keratin bodies in the basal layer is interpreted as a PUVA-induced immunohistological sign of cure and as primary antikeratinocytic and secondary anticytotoxic component of the 8-MOP-UVA effect.
