Simple Summary: The equine hindgut plays a central role in digesting forage by hosting a complex community of microorganisms responsible for fermenting and digesting plant fibers. Research has shown that the different compartments of the hindgut have unique microbial communities. The pelvic flexure, an anatomic structure that separates segments of the hindgut and prevents backflow, seems to help maintain these distinct communities. How it achieves this has yet to be fully understood. While much is known about the hindgut's anatomy and digestion, only a few studies have investigated gene expression patterns and differences in the tissues around the pelvic flexure. In this study, we used next-generation RNA sequencing to analyze and compare gene expression in the pelvic flexure and the ventral and dorsal segments of the large colon in horses. This knowledge can help researchers, veterinarians, and horse owners better understand the equine hindgut's physiology and how horses interact with the microbial communities there and could provide insights for managing healthy horses or treating horses with digestive issues. Previous research demonstrated the distribution of distinct microbial communities in the equine hindgut surrounding the pelvic flexure. The current study evaluated gene expression in epithelial tissues surrounding the pelvic flexure to characterize patterns that might correlate with microbial distribution. Gene expression was determined by analyzing RNA sequence data from the pelvic flexure, the left and right ventral colon, and the left and right dorsal colon. An average of 18,330 genes were expressed across the five tissues sampled. Most of the genes showed some level of expression in all five tissues. Tissue-restricted patterns of expression were also observed. Genes with restricted expression in the left ventral and left dorsal colons have communication, signaling, and regulatory functions that correlate with their known physiology. In contrast, genes expressed exclusively in the pelvic flexure have diverse functions. The ontology of genes differentially expressed between the pelvic flexure and the surrounding tissues was associated with immune functions and signaling processes. Despite being non-significant, these enrichment trends were reinforced by the functions of statistically significant expression differences between tissues of the hindgut. These results provide insight into the physiology of the equine hindgut epithelium that might influence the microbiota and its distribution. [ABSTRACT FROM AUTHOR]

Homeostasis is a complex physiological process that enables biological systems to maintain balanced internal environments in response to internal and external stimuli. In the equine gastrointestinal (GI) tract, homeostasis involves a network of factors that regulate gene expression, maintain the integrity of the mucosal barrier, and modulate immune responses to pathogenic microorganisms while supporting commensal bacteria that provide benefits to the host (Figure 1). These factors work together and respond to changes in diet, environment, external stress, and challenges by infectious agents or toxins. Proper gastrointestinal function is dependent on maintaining homeostasis in the system, and the dynamic interactions between the microbial population (the microbiota) and the equine host are central to this equilibrium. The regulation and interplay between the equine host and GI microbiota are evident in the distribution of distinct microbial communities within the various compartments of the equine hindgut. The contributions of each compartment to digestive physiology and the overlap with various stages of microbial fermentation in the equine hindgut are well-researched. What remains to be documented is gene expression in tissues of the equine GI and the underlying role this might play in the processes of digestion, physiology, and homeostasis in the GI. To better understand the roles that host gene expression plays in the interactions with the microbiota, it is critical that we investigate gene expression in the different tissues along the intestinal tract. Protein-coding genes are crucial in maintaining the health of the gastrointestinal tract. They direct the synthesis of various structural elements, molecular transporters, and digestive enzymes, which support digestion, nutrient absorption, and gut integrity. Non-coding genes also play an important role in regulating protein-coding gene expression, modulating immune responses, and maintaining gut barrier function. Moreover, there is evidence that non-coding transcripts regulate the composition of microbial communities in the GI tract. The presentation will focus on the current research and future directions for investigating protein-coding and non-coding gene expression to understand the mechanisms involved in the host-microbe interaction in the equine hindgut and their potential role in maintaining GI homeostasis. People working within mental health organisations or systems but not in dedicated lived experience roles might still make contributions as allies to mental health consumer movements. The current study explores the motivations of such allies, specifically in relation to collaborations with consumers and targeting systemic change when providing support. Semi-structured interviews were held with 11 participants identified as allies to consumer movements. Interview data were examined using a synthesis of inductive and deductive thematic analysis. The findings indicated the motivations of allies could be understood in relation to three themes: social justice values, witnessing support for consumers, and perceptions of valuable consumer contributions. Participants discussed no single primary motive, instead reporting a combination. The motives identified have implications for allies' involvement in and mobilisation with consumer movements. Allies using their power and influence to make opportunities for consumer empowerment might improve mental health policy, services, and academia. [ABSTRACT FROM AUTHOR]

Orthotic management is frequently part of the rehabilitation of various neurological conditions in adults such as stroke, peripheral neuropathies, spinal cord injuries and multiple sclerosis. Despite this, there are limited data available on the implementation of orthotic care in practice. The primary aim of this study is to establish the proportion of inpatients in a mixed neurorehabilitation cohort which receive orthotic assessment and treatment. The secondary aim was to document the overall care pathway. Existing data were reviewed retrospectively. Data gathered included patient demographics such as age, gender and diagnosis, type of orthoses prescribed, number of orthotic treatment sessions required and outcome measures used. The paper was prepared in accordance with the STROBE statement for observational studies. During a 6 month period, 54 patients were admitted as inpatients. Of these, 25 (46%) were referred to the orthotic service for assessment, with 23 (43%) receiving a lower limb orthosis and 19 (35%) subsequently being discharged using either an ankle-foot orthosis or knee-ankle-foot orthosis. The conditions most commonly seen were stroke (40%), incomplete spinal cord injuries (28%) and peripheral neuropathies (16%). Significant improvements were recorded at discharge using the Functional Independence Measure (FIM) and Functional Assessment Measure (FAM). Various aspects of the orthotic care process are described. This study suggests that orthotic treatment is needed with high frequency in a mixed inpatient neurorehabilitation cohort. In terms of service planning, these data support existing the recommendation that neurological rehabilitation centres should have good access to orthotic services. Custom orthoses such as AFOs and KAFOs are frequently needed by neurological rehabilitation inpatients. Orthoses may facilitate standing and walking at an earlier stage during rehabilitation. Those responsible for neurological rehabilitation services should ensure appropriate access to orthotic services. [ABSTRACT FROM AUTHOR]
