This study aimed to assess the utility of echocardiography-measured epicardial adipose tissue (EAT) thickness (EATT) as an independent predictor for coronary artery disease (CAD), examining its correlation with oxidative stress levels in epicardial tissue and the complexity of the disease in patients undergoing open-heart surgery. This study included a total of 25 patients referred for cardiac surgery with 14 in the CAD group and 11 in the non-CAD group. Epicardial fat was sampled from patients subjected to open-heart surgery. EATT was higher in the CAD group compared to the non-CAD group (8.15 ± 2.09 mm vs. 5.12 ± 1.8 mm, p = 0.001). The epicardial reactive oxygen species level was higher in the CAD group compared to the non-CAD group (21.4 ± 2.47 nmol H
2 O2 /g tisssue/h vs. 15.7 ± 1.55 nmol H2 O2 /g tisssue/h, p < 0.001). EATT greater than 6.05 mm was associated with CAD, with a sensitivity of 86% and specificity of 73%. Echocardiographically measured EATT is a significant, independent predictor of CAD. Its relationship with increased EAT oxidative stress levels suggests a potential mechanistic link between EATT and CAD pathogenesis. These findings highlight the importance of EATT as a diagnostic tool in assessing the complexity of CAD in patients undergoing cardiac surgery. [ABSTRACT FROM AUTHOR]- Published

9. Risk stratification in patients with heart failure: the value of considering both global longitudinal left ventricular strain and mechanical dispersion

In previous studies, mechanical dispersion (MD) predicted ventricular arrhythmias independently of left ventricular ejection fraction (LVEF). Moreover, the combination of MD and global longitudinal strain (GLS) increased the prediction of arrhythmic events. We investigated the prognostic value of a new 2-dimensional strain index, GLS/MD, in patients with heart failure (HF). We analyzed 340 consecutive HF outpatients in sinus rhythm. Echocardiography was performed at 1.6 [+ or -] 0.4 months after hospital discharge. The end point included sudden cardiac death, ventricular fibrillation, and sustained ventricular tachycardia (SCD/VA). During the follow-up period (36 [+ or -] 9 months), SCD/VA occurred in 48 patients (14.1%). A multivariate Cox regression analysis, which included LVEF, early diastolic transmitral / mitral annular velocity ratio (E/E'), GLS, MD, and GLS/MD in the model, revealed that GLS/MD was the best independent predictor of SCD/VA (HR = 3.22, 95% confidence interval = 1.72-6.15, p = 0.03). Separate inclusion of LVEF, systolic mitral annular velocity, E/E', GLS, and MD together with GLS/MD showed that GLS/MD remained the best predictor of SCD/VA (each p Key words: heart failure, malignant ventricular arrhythmia, cardiovascular risk stratification, 2D strain imaging, global longitudinal strain, mechanical dispersion, GLS/MD ratio. Dans des etudes anterieures, la dispersion mecanique (DM) etait un facteur predictif des arythmies ventriculaires independant de la fraction d'ejection ventriculaire gauche (FEVG). De plus, l'association de la DM avec le strain longitudinal global (SLG) augmentait la predictibilite des evenements arythmiques. Nous avons etudie la valeur pronostique du ratio SLG/DM, un nouvel indice de strain bidimensionnel, chez des patients atteints d'insuffisance cardiaque (IC). Nous avons etudie en consultations externes 340 patients atteints d'IC consecutifs, qui affichaient un rythme sinusal. Nous avons effectue une echocardiographie 1,6 [+ or -] 0,4 mois apres le conge de l'hopital. Le parametre devaluation (MSC/AV) etait compose des elements suivants : mort subite cardiaque, fibrillation ventriculaire ou tachycardie ventriculaire soutenue. Pendant la periode de suivi (36 [+ or -] 9 mois) le MSC/AVest survenu chez 48 patients (14,1 %). Al'aide d'une analyse de regression multivariee de Cox comprenant dans le modele la FEVG, le rapport des vitesses diastoliques precoces transmitrale/anneau mitral (E/E'), le SLG, la DM et le ratio SLG/DM, le ratio SLG/DM etait le meilleur facteur predictif independant pour le SCD/VA (rapport de risques instantanes (RRI) de 3,22 avec un intervalle de confiance a 95 % de 1,72 a 6,15, p = 0,03). L'inclusion de la FEVG, de la vitesse annulaire transmitrale systolique, du rapport E/E', du SLG et de la DM separement avec le ratio SLG/DM a montre que le ratio SLG/DM demeure le meilleur facteur predictif de MSC/AV (chacun p < 0,05). La valeur predictive limite optimale du ratio SLG/DM pour le MSC/AV etait de -0,20 %/ms (sensibilite de 80 %, specificite de 76 %). Sans egard a la FEVG, la survie sans evenement etait nettement meilleure chez les patients avec un ratio SLG/DM [less than or equal to] -0,2 %/ms (log-rank, p < 0,001). Anthracyclines are important anticancer drugs, but their use is limited by cardiotoxicity. Left ventricular ejection fraction (LVEF) is often inadequate to detect myocardial disease induced by chemotherapy. Tissue Doppler and bidimensionalstrain imaging could detect LV myocardial dysfunction earlier than LVEF. In drug-induced cardiotoxicity, torsional and longitudinal LV deformations [LV twist (LVtw), radial strain (GRS), global longitudinal strain (GLS)] are damaged. We assessed whether a new index, GLSxLVtw, could predict future anthracycline-induced cardiomyopathy. Echocardiography, troponin, and natriuretic peptide determination were prospectively performed in 74 patients before and after 6, 12, 24, and 52 weeks of anthracycline treatment. These patients were treated for breast cancer, Hodgkin's or non-Hodgkin's lymphoma, acute lymphoblastic or myeloblastic leukaemia, or osteosarcoma. At 6 weeks after initiation of chemotherapy, isovolumic relaxation time, systolic mitral annular velocity, LVGLS, LVGRS, LV apical rotation, LVtw, and GLSxLVtw deteriorated, and troponin levels became elevated (all p < 0.05 by ANOVA) before LVEF decreased. The receiver operating characteristic curves identified early deterioration of GLSxLVtw as the best predictor of later cardiotoxicity (area under curve = 0.93), followed by GLS (0.84) and LV apical rotation (0.81) deterioration. In conclusion, early change in the GLSxLVtw index seems to be a good predictor of future development of anthracycline-induced cardiotoxicity. Les anthracyclines sont d'importants agents anticancereux, mais leur utilisation est limitee par leur cardiotoxicite. La fraction d'ejection ventriculaire gauche (FEVG) est souvent inadequate pour detecter un dommage au myocarde induit par la chimiotherapie. L'imagerie Doppler tissulaire et l'imagerie de strain bidimensionnelle pourraient detecter la dysfonction myocardique precoce du VG. En situation de cardiotoxicite provoquee par le medicament, les deformations de torsion et longitudinales du VG [torsion du VG (VGT), le strain radial (GRS) et le strain global longitudinal (GLS)] sont affectees. Nous avons evalue si un nouvel indice, GLSxVGT, pourrait predire une cardiomyopathie eventuelle provoquee par les anthracyclines. L'echocardiographie et la determination de la troponine et des peptides natriuretiques ont ete realisees de maniere prospective chez 74 patients avant et apres 6,12, 24 et 52 semaines de traitement aux anthracyclines. Ces patients etaient traites pour un cancer du sein, un lymphome Hodgkinien et non-Hodgkinien, une leucemie lymphoblastique ou myeloblastique, ou un osteosarcome. Six semaines apres le debut de la chimiotherapie, le temps de relaxation isovolumique, la velocite systolique annulaire mitrale, le GLS, le GRS, la rotation apicale et la torsion du VG de meme que l'indice GLSxVGT se deterioraient, et les niveaux de troponine s'elevaient (p < 0,05 en ANOVA pour tous les parametres), avant que la FEVG ne decroisse. Les courbes des caracteristiques receveur-operateur ont identifie la deterioration precoce du GLSxVGT comme le meilleur predicteur d'une cardiotoxicite eventuelle (aire sous la courbe = 0,93), suivi de la deterioration du GLS (0,84) et de la rotation apicale du VG (0,81). En conclusion, le changement precoce du nouvel indice GLSxVGT semble etre un bon predicteur du developpement futur d'une cardiotoxicite provoquee par les anthracyclines. The role of systematic exercise testing (ET) in these patients has not been established. This study was designed to assess clinical and therapeutic implications (device programming/drugs) of systematic ET in patients requiring fusion-pacing CRT without anright ventricle (RV) lead.Methods: Consecutive patients with a right atrium/LV-only dual-chamber (DDD) pacing system were included. Prospective data were obtained: device interrogation, ET, and echocardiography at every 6-month follow-up visit. CRT assessment during ET included maximal heart rate, beat-to-beat echocardiography analysis of LV fusion pacing, LV loss of capture, and improvement in exercise capacity. If LV loss of capture or unsatisfactory LV fusion pacing occurred, reprogramming was individualized for each patient and ET redone.Results: A total of 55 patients (29 male) aged 62±11 years were included. During follow-up (39±18 months), a total of 235 ETs were performed, with mean exercise load 6.4±1.3 metabolic equivalents of task (118±35 W, maximal heart rate 119±17 beats/min). Twenty patients (36%) had inadequate pacing or loss of LV capture during ET, due to exceeding the maximum tracking rate (11%), chronotropic incompetence (7%), and LV pacing outside the fusion-pacing band (18%), caused by physiological shortening of the PR interval or exagerated LV preexcitation during maximum exercise. Post-ET CRT-device optimization included reprogramming of rate-adaptive atrioventricular interval (total decrease 23±8 ms), individualized programming of maximum tracking rate, or rate-response function. Drug optimization was performed in 32% of patients, and ET redone in 36%.Conclusion: In one of three ETs, an intervention in device and medication optimization was done to ensure a better outcome. During follow-up (39±18 months), a total of 235 ETs were performed, with mean exercise load 6.4±1.3 metabolic equivalents of task (118±35 W, maximal heart rate 119±17 beats/min). We consider that elderly patients are the most severely affected by this disease that represents the final impact of cardiovascular disease continuum. Cardiovascular diseases are associated with serious morbidity and mortality, and considerable health care costs related to diagnosis and treatment. In this report we discuss some controversies regarding methods of heart failure evaluation as well as therapeutic steps and devices, including: reparatory therapeutic steps and initiation of therapy with loop diuretics, inconsistent dose titration for angiotensin-converting enzyme inhibitors/angiotensin receptor blocker and beta blockers, as well as novel therapies, such as angiotensin receptor neprilysin inhibitor and treatments that directly improve cardiomyocyte function. We conclude that, beyond technical progress, which is raising the cost of therapy for patients with heart failure, more careful monitoring of patient progress through clinical and paraclinical control visits, both at medical facilities and at home, would have greater impact and be more cost-effective. Physical therapy and promoting emotional and psychological wellbeing, to maintain a positive state of mind, contribute substantially to the quality of life and life expectancy, and are most important in elderly people who are most affected by dramatic reductions in wellbeing. Patients and methods: The study included 294 patients admitted for STEMI. They were divided into five groups according to the SBP at admission: group I, <105 mmHg; group II, 105–125 mmHg; group III, 126–140 mmHg; group IV, 141–158 mmHg; and group V, ≥159 mmHg. Increased HR was defined as ≥80 beats per minute (bpm). In-hospital death was defined as all-cause death during admission and classified into cardiac and noncardiac death. Results: Among the 294 patients admitted for STEMI, 218 (74%) were men. The mean age was 62±17 years. In-hospital mortality rate was 6% (n=18), with 11 (3.7%) deaths having cardiac causes. The highest mortality was registered in group I (n=9, 16%, P=0.018). Compared to the other groups, group I patients were older (P=0.033), more often smokers (P=0.026), and had a history of myocardial infarction (P=0.003), systemic hypertension (P=0.023), diabetes (P=0.041), or chronic kidney disease (P=0.0200). They more often had a HR ≥80 bpm (P=0.028) and a Killip class 3 or 4 at admission (P=0.020). The peak creatine phosphokinase-MB level was significantly higher in this group (P=0.005), while the angiographic findings more often identified as culprit lesions were the right coronary artery (P=0.005), the left main trunk (P=0.040)

International audience; BACKGROUND: The timing of cardiac surgery after stroke in infective endocarditis (IE) remains controversial. We examined the relationship between the timing of surgery after stroke and the incidence of in-hospital and 1-year mortalities. METHODS: Data were obtained from the International Collaboration on Endocarditis-Prospective Cohort Study of 4794 patients with definite IE who were admitted to 64 centers from June 2000 through December 2006. Multivariate logistic regression and Cox regression analyses were performed to estimate the impact of early surgery on hospital and 1-year mortality after adjustments for other significant covariates. RESULTS: Of the 857 patients with IE complicated by ischemic stroke syndromes, 198 who underwent valve replacement surgery poststroke were available for analysis. Overall, 58 (29.3%) patients underwent early surgical treatment vs 140 (70.7%) patients who underwent late surgical treatment. After adjustment for other risk factors, early surgery was not significantly associated with increased in-hospital mortality rates (odds ratio, 2.308; 95% confidence interval [CI], .942-5.652). Overall, probability of death after 1-year follow-up did not differ between 2 treatment groups (27.1% in early surgery and 19.2% in late surgery group, P = .328; adjusted hazard ratio, 1.138; 95% CI, .802-1.650). CONCLUSIONS: There is no apparent survival benefit in delaying surgery when indicated in IE patients after ischemic stroke. Further observational analyses that include detailed pre- and postoperative clinical neurologic findings and advanced imaging data (eg, ischemic stroke size), may allow for more refined recommendations on the optimal timing of valvular surgery in patients with IE and recent stroke syndromes.

International audience; The HACEK organisms (Haemophilus species, Aggregatibacter species, Cardiobacterium hominis, Eikenella corrodens, and Kingella species) are rare causes of infective endocarditis (IE). The objective of this study is to describe the clinical characteristics and outcomes of patients with HACEK endocarditis (HE) in a large multi-national cohort. Patients hospitalized with definite or possible infective endocarditis by the International Collaboration on Endocarditis Prospective Cohort Study in 64 hospitals from 28 countries were included and characteristics of HE patients compared with IE due to other pathogens. Of 5591 patients enrolled, 77 (1.4%) had HE. HE was associated with a younger age (47 vs. 61 years; p < 0.001), a higher prevalence of immunologic/vascular manifestations (32% vs. 20%; p < 0.008) and stroke (25% vs. 17% p = 0.05) but a lower prevalence of congestive heart failure (15% vs. 30%; p = 0.004), death in-hospital (4% vs. 18%; p = 0.001) or after 1 year follow-up (6% vs. 20%; p = 0.01) than IE due to other pathogens (n = 5514). On multivariable analysis, stroke was associated with mitral valve vegetations (OR 3.60; CI 1.34-9.65; p < 0.01) and younger age (OR 0.62; CI 0.49-0.90; p < 0.01). The overall outcome of HE was excellent with the in-hospital mortality (4%) significantly better than for non-HE (18%; p < 0.001). Prosthetic valve endocarditis was more common in HE (35%) than non-HE (24%). The outcome of prosthetic valve and native valve HE was excellent whether treated medically or with surgery. Current treatment is very successful for the management of both native valve prosthetic valve HE but further studies are needed to determine why HE has a predilection for younger people and to cause stroke. The small number of patients and observational design limit inferences on treatment strategies. Self selection of study sites limits epidemiological inferences.

Recent data have demonstrated that a new tissue Doppler index, E/(E'×S'), index that includes the ratio between early diastolic transmitral and mitral annulus velocity (E/E'), as well as the systolic mitral annulus velocity (S'), has a good accuracy to predict cardiac death in patients with heart failure (HF). We analyzed the prognostic value of the E/(E'×S') ratio in patients with heart failure with reduced ejection fraction (HFrEF). Methods - Echocardiographic examination was performed in 166 patients hospitalized with the diagnosis of heart failure with reduced ejection fraction, before discharge as well as one month later. Worsening of E/(E'×S') was defined as a value greater than the previous value determined at discharge. The primary event consisted of cardiac death or hospital admission due to HF worsening. Results - During follow-up (34±8.8 months) cardiac events were recorded for 113 patients (68%). The optimal cut-off value for E/(E'×S') ratio was 1.96 with 84% sensitivity and 80% specificity. At discharge, 60 patients (36,2%) presented a value of the E/(E'×S') index =1.96 (group I) and 106 patients (63.8%) encountered a value of the E/(E'×S') index >1.96 (group II). The composite end point was significantly higher in group II than in group I (95 events, 89.6% versus 18 events, 30%, p<0.001). Patients with an E/(E'×S') ratio >1.96 at discharge and its worsening after one month presented the worst prognosis (p<0.05). Conclusions - In patients with with heart failure with reduced ejection fraction, a value of the E/(E'×S') index >1.96 at hospital discharge associated with its worsening after one month is an important predictor for future cardiac events. The aim of our study was to evaluate left ventricular (LV) systolic function in a team of elite rugby players using new echocardiographic techniques: Tissue Doppler, two-dimensional (2D) Speckle tracking and threedimensional (3D) echocardiography. Material and Method. Twenty six male athletes from the regional rugby players' team were evaluated. The team was divided in two groups of athletes based on the predominance of dynamic aerobic or anaerobic effort performed: thirteen of them (group 1) - anaerobe effort, the others thirteen (group 2) - aerobic effort. All athletes underwent a complete physical exam, complete 2D and 3D transthoracic echocardiographic exam at rest. Standard 2D measurements, Tissue Doppler parameters, 2D speckle tracking global longitudinal strain (2D GLS) and 3D global longitudinal strain (3D GLS) were evaluated. Results. There were statistically significant differences between the two groups of athletes. Those in group 1 had higher body mass index (BMI) (p=0.002), left atrial area (LAA) (p=0.006), 3D left ventricular enddiastolic and endsystolic volume (LVEDV3D and LVESV3D) (p=0.006 respective p=0.011), 3D left ventricular mass (LVMass3D) (p=0.002), pulse Doppler diastolic A'velocity of lateral mitral annulus (A'lat) (p=0.004), left atrial diameter (LAD) (p=0.001), pulse Doppler systolic velocity of medial mitral annulus (S'med) (p=0.01), mitral annular plane systolic excursion (MAPSE) (p=0.033) than those in group 2, who had higher diastolic E' velocity of lateral (E'lat) and medial (E'med) mitral annulus (p=0.047 respective p=0.021). There were no significant differences of 2D GLS respective 3D GLS between the two groups (p=0.59, respective p=0.27). Athletes who had greater LV hypertrophy and LV mass indexed higher had a better response in tests of anaerobic exercise capacity. There was a statistically significant association between the 3D ejection fraction (EF) and anaerobic and aerobic exercise capacity. Conclusions. Our study revealed interesting interrelations between 2D GLS, 3D GLS, and LV hypertrophy and test results of effort capacity in a group of elite rugby players. [ABSTRACT FROM AUTHOR]
