Abstract

Abstract Background Development of multidrug resistance in Uropathogenic Escherichia coli (UPEC) makes treatment of Urinary Tract Infections (UTIs) a major challenge. This study was conducted to investigate the effect of trans-resveratrol (t-RSV) at a subinhibitory concentration (sub-MIC-t-RSV) on phenotypic and genotypic expression of virulence factors of clinical isolates of UPEC and develop a nanoformulation of t-RSV. Fifty-five clinical UPEC strains were investigated for the presence of virulence factors by phenotypic methods and PCR detection of virulence genes. The effect of sub-MIC-t-RSV was studied on the phenotypic and genotypic expression of virulence factors. t-RSV-loaded nanoemulgel formulation was prepared and characterized. Results Out of the 55 tested isolates, 50.9% were biofilm producers, 23.6% showed both mannose-sensitive and mannose-resistant hemagglutination, 21.8% were serum-resistant, 18.2% were hemolysin producers, while 36.4% showed cytotoxic effect on HEp-2 cells. A total of 25.5% of the isolates harbor one or more of hly-A, cnf-1 and papC genes, while 54.5% were positive for one or more of fimH, iss and BssS genes. A concentration of 100 µg/mL of t-RSV effectively downregulates the phenotypic and genotypic expression of the virulence factors in positive isolates. A stable t-RSV-nanaoemulgel with droplet size of 180.3 nm and Zetapotential of -46.9 mV was obtained. Conclusion The study proves the effective role of t-RSV as an antivirulence agent against clinical UPEC isolates in vitro and develops a stable t-RSV-nanoemulgel formulation to be assessed in vivo. The promising antibacterial and antivirulence properties of t-RSV place this natural compound to be a better alternative in the treatment of persistent UTIs.

3. Role of Interleukin 1- Alpha in Pulmonary Toxic Effect among Silica Exposed Workers

Abstract

Background: Silicosis refers to a spectrum of pulmonary diseases attributed to inhalation of free crystalline silicon dioxide. It entails inflammatory response in which Interleukin-1 alpha is a key mediator. Objectives: to evaluate the role of interleukin-1α (IL-1α) as a biochemical marker for detection of pulmonary toxic effects of silica in relation to clinical and radiological manifestations among exposed workers. Methods: The study was conducted on 64 male workers exposed to silica dust assigned to El-Harareyat Factory of Alexandria. An interviewing questionnaire was used to collect data about personal, occupational and medical characteristics. Pulmonary function testing, plain chest radiographs and laboratory assessment of serum level of IL-1α were done for all workers. IL-1α was assessed also among a matched control group. Results: Prevalence of silicosis was 37.5%. Serum IL-1α level among silica-exposed workers was 2.8±1.8 pg/ml while it was undetected among a control group of healthy males. IL-1α was positively correlated with wheezes, negatively correlated with FVC, FEV1, MMEF, FEF50%, positively correlated with extent of nodular opacities in lung zones and presence of enlarged and calcified hilar lymph nodes. Conclusion: IL-1α can be a good monitoring marker for the earliest sign of silicosis; hilar lymphadenopathy.

Abstract

Carbopol is a good bio-adhesive polymer that increases the residence time in the eye. However, the effect of blinking and lacrimation still reduce the amount of polymer and the incorporated drug available for bioadhesion. Gel-core liposomes are advanced systems offering benefits making it a good tool for improved ocular drug delivery and residence time. Incorporation of carbopol in gel-core liposomes and their potential in ocular delivery have not so far been investigated. Fluconazole (FLZ) was selected as a challenging important ocular antifungal suffering from poor corneal permeation and short residence time. In this study, gel-core carbosomes have been elaborated as novel carbopol-based ophthalmic vehicles to solve ocular delivery obstacles of FLZ and to sustain its effect. Full in vitro appraisal was performed considering gel-core structure, entrapment efficiency, particle size and stability of the vesicles as quality attributes. Structure elucidation of the nanocarrier was performed using optical, polarizing and transmission electron microscopy before and after Triton-X100 addition. Ex-vivo ocular permeation and in vivo performance were investigated on male albino rabbits. Optimized formulation (CBS5) showed gel-core structure, nanosize (339.00 ± 5.50 nm) and not defined before (62.00% ± 1.73) entrapment efficiency. Cumulative amount of CBS5 permeated ex-vivo after 6 h, was 2.43 and 3.43 folds higher than that of conventional liposomes and FLZ suspension, respectively. In-vivo corneal permeation of CBS5 showed significantly higher AUC0-24 h (487.12 ± 74.80) compared to that of FLZ suspension (204.34 ± 7.46) with longer residence time in the eye lasts for more than 18 h. In conclusion, novel gel-core carbosomes could successfully be used as a promising delivery system for chronic ocular diseases.

Abstract

Fungal infections need long-term therapy with the proper antifungal agent. Despite effectiveness, Fluconazole (FLZ) ocular delivery is constrained by limited penetration, short residence time, in addition to the common barriers of the eye. Hyalugel-integrated liposomes were designed as novel ocular delivery systems integrating hyaluronic acid (HA) inside and surrounding vesicles by a simple preparation technique. The impact of combining HA hydrogel and liposomes was investigated in a series of different formulations. Full in-vitro optimization was performed regarding; HA and FLZ concentration, entrapment efficiency, particle size and stability to select the formula with the best characteristics. Structure elucidation of gel integration was done using polarizing and transmission electron microscopes before and after Triton-X100 addition. Corneal deposition and permeation were examined ex-vivo and in-vivo on male albino rabbits. Selected formulation (HYS7) showed gel-integrated structure, nanosize (218.50±4.50nm) and % EE 42.81%±1.66. Ex-vivo cumulative corneal permeation of FLZ after 6h from HYS7, was 2.99 and 4.18 folds higher than conventional liposomes and FLZ suspension, respectively. In-vivo corneal permeation of HYS7 showed unprecedented sustained effect of FLZ reaching 24h. In conclusion, novel hyalugel-integrated liposomes significantly enhanced corneal permeability compared to conventional liposomes and FLZ suspension. They would be promising alternates for eye drops; decreasing frequency of administration and increasing patients' compliance.
