Patients were randomly divided into two groups: Group one and two received low (5 mg) and usual (10 mg) dose of Hyoscine, respectively. Cardiovascular service consultation was performed for all patients before entering the study and performing ERCP. Hyoscine was injected intravenously, and the spasmolytic effect of the drug was assessed while the papilla was in a completely enface view. The time interval between cessation of peristalsis and its further onset was recorded by the chronometer. Also, patient’s heart rate and blood pressure were monitored during ERCP by digital monitoring.Results: The results showed no statistically significant differences in the mean duration of peristalsis, the duration of the antispasmodic activity and the time required to increase the heart rate between two groups (P=0.38, P=0.48, P=0.32, respectively). No significant differences were observed regarding the average of heart rate and mean arterial blood pressure (MAP) before drug administration between the two groups (P=0.182 and P=0.29, respectively), but after the drug administration, tachycardia and hypotension were significantly higher in the second group (P=0.007 and P=0.001, respectively). There was no statistically significant difference in the frequency of arrhythmia between two groups (P=0.08). The results also showed that tachycardia and hypotension occurred more frequently in men and elderly patients (P < 0.05).Conclusion: A low dose of Hyoscine is as effective as the usual dose and its side effects such as alteration in blood pressure and heart rate are much fewer, especially in men and elderly patients.Keywords: hyoscine, ERCP, peristalsis, spasmolytic effect, cardiovascular abnormalities

Since then, COVID-19 has spread to more than 200 countries in the world, and a global pandemic has been declared by the World Health Organization (WHO). At present, no vaccines or therapeutic regimens with proven efficacy are available for the management of COVID-19. Hydroxychloroquine/chloroquine, lopinavir/ritonavir, ribavirin, interferons, umifenovir, remdesivir, and interleukin antagonists, such as tocilizumab, have been recommended as potential treatment options in COVID-19. Transplant patients receiving immunosuppressant medications are at the highest risk of severe illness from COVID-19. At the same time, with regard to receiving polypharmacy and immunosuppressants, treatment options should be chosen with more attention in this population. Considering drug–drug interactions and adverse effects of medications used for the treatment of COVID-19, such as QT prolongation, the dose reduction of some immunosuppressants or avoidance is recommended in transplant recipients with COVID-19. Thus, this narrative review describes clinically important considerations about the treatment of COVID-19 and immunosuppressive regimens regarding modifications, side effects, and interactions in adult kidney or liver allograft recipients.Keywords: SARS-CoV-2, COVID-19, liver transplant, kidney transplant, immunosuppressive, transplantation

Despite its high burden, the optimal treatment of PDN has remained a clinical challenge. To explain the emergence and maintenance of PDN, increasing attention has been focused on dimensions of inflammation and oxidative toxic stress (OTS). Accordingly, the aim of this study was to investigate the effects of oral N-acetylcysteine (NAC), an agent with known anti-oxidant and anti-inflammatory effects, as an adjunct therapy in patients suffering from PDN.Patients and methods: 113 eligible patients with type 2 diabetes suffering from PDN were randomly assigned to either the pregabalin + placebo or pregabalin + NAC group for 8 weeks (pregabalin at a dose of 150 mg per day, NAC and matched placebo at doses of 600 mg twice a day). Mean pain score was evaluated at baseline, week 1, 2, 4, 6, and 8 of the study based on the mean 24 hr average pain score, using an 11-point numeric rating scale (NRS). As secondary efficacy measures, mean sleep interference score (SIS) resulting from PDN, responder rates, Patient Global Impression of Change (PGIC), Clinical Global Impression of Change (CGIC), and safety were also assessed. Accumulating evidence shows a bidirectional association between inflammation and depression. Systemic inflammation can affect clinical outcomes after transplantation, such as time to engraftment. This study evaluated the effect of pre-transplantation depression and serum level of pro-inflammatory and anti-inflammatory cytokines on clinical outcomes such as neutrophil recovery time and mortality in patients undergoing HSCT.Patients and methods: In this cross-sectional study, we recruited 73 patients who were autologous or allogeneic HSCT candidates. Hospital Anxiety and Depression Scale questionnaire was used to assess depression in the first 2 days after their hospital admission. Serum levels of IL-10, IL-6, and hs-CRP were measured at the same time. Neutrophil engraftment time, length of hospitalization, rate of mortality, relapses, readmissions, and occurrence of major complications following HSCT in a 1-year follow-up period were recorded as comparative outcomes.Results: Thirty-five patients with depression and 38 patients without depression participated in the study. Among the related outcomes, time of engraftment (P=0.020) and mortality rate (P=0.059) were statistically different between the two groups. Depressed patients had longer neutrophil engraftment time and higher mortality rate 1 year following transplantation (14.14±3.95 days and 31.43%) in comparison to non-depressed patients (12.21±2.81 days and 13.16%). On the other hand, these patients are more prone to side effects and drug interactions associated with psychotropic drugs use. Benzodiazepines (BZDs), antidepressants, and antipsychotics are commonly used in critically ill patients. Therefore, their therapeutic effects and adverse events are discussed in this study. Different studies have shown that non-BZD drugs are preferred to BZDs for agitation and pain management, but antipsychotic agents are not recommended. Also, it is better not to start antidepressants until the patient has fully recovered. However, further investigations are required for the use of psychotropic drugs in ICUs. 7. Evaluation of aminoglycosides utilization in intensive care units of a teaching hospital in southern Iran

Abstract

Background: Inappropriate use of antimicrobial agents is one of the most important factors in inducing resistance and prolonged hospitalization as well as increase in patient mortality rate. Objective: The aim of this study was to evaluate aminoglycosides (AGs) usage pattern at intensive care units (ICUs) of Nemazee hospital Shiraz, Iran. Methods: In this cross-sectional study, the usage pattern of AGs was evaluated during 32 months. Guidelines for AGs usage were approved by the drug and therapeutic committee of the hospital, and criteria were developed to assess 11 parameters involving AGs therapy, such as proper indication for the use of the drug, dosage and duration of therapy. Clinical parameters, such as microbial culture and sensitivity, serum creatinine (SCr) and creatinine clearance, and white blood cell count were evaluated. Results: Ninety-five patients were recruited, 50 male and 45 females. In most patients (64%) the origin of infection was hospital and only in 36% of them, community was the source. Ventilator associated pneumonia (27%), central nervous system (25%) and urinary tract infection (10%) were the most important indications for AGs prescription. Scores of AGs usage at Nemazee hospital was calculated as 5.9 out of 11, which meant that in only 54% of cases AGs prescription was based on guideline proposed by the Department of Clinical Pharmacy of Nemazee Hospital. Conclusions: Non-adherence to the guidelines occurred frequently in the ICUs of Nemazee hospital. Prescription of loading dose, and AGs level measurement were not done and evaluating microbiological data was often neglected. Incorporating pharmacists in the health care team and holding training programs for physicians and nurses with the goal of raising awareness about the proposed guideline.

Abstract

Background: All the patients with Angina pectoris have not stenotic coronary artery. Syndrome x consists of patients with Angina pectoris and positive Exercise test along with normal coronary angiography. Syndrome x includes 25-30 percent of those who undergo diagnostic coronary arteriography at the different centers. In our study it was attempted to know the prevalence and characteristics of this special group in Afshar cath-lab. Materials and Methods: In a prospective cross sectional descriptive study 200 patients who were admitted for coronary angiography evaluated in terms of Risk factors: HTN, Family history Hyperlipidemia, DM, Cigarette smoking , clinical symptoms , response to TNG, different symptoms, ECG and Exercise test. data in two subgroups with normal and abnormal Coronary angioghraphy were compared. Six patients were excluded because of ambiguous Coronary angiography. Results: Among 194 patients 51 patients (26.3% )had normal coronary artery. 38.9% of female and 21.4% of males showed normal coronary angioghraphy (P = 0.013). The prevalence of risk factors among those with syndrome x and abnormal coronary Artery were as follow. HTN 37% vs 42% , Hyperlipidemia 50% vs 51% , smoking 25% vs 37% , DM 12% vs 28% , obesity 45% vs 47% Positive family history for CAD = 21.5% vs 47% respectively. Angina relieved suitably with TNG in 24% of those with syndrome x compared to 76% suitable response in those with abnormal coronary arteries.(P Value = 0.49 ). Regarding to functional class , 87% of patients with normal coronary artery belonged to class I and II. also 26% had atypical pain 50% had normal rest ECG and 51% achieved stage III or IV of Bruce protocol during Exercise test. Conclusion: Syndrome x was more prevalent among women ,also angina was less typical and less responsive to TNG. Most patients had not positive familial history for CAD. normal ECG was more prevalent among those with syndrome x .

25. Study on morphological diversity and parthenolide content of some feverfew (Tanacetum parthenium (L.) Schults-bip.) populations.

Abstract

Feverfew (Tanacetum parthenium (L.) Schults-bip.) is a perennial medicinal plant from Asteraceae family and a rich source of sesquiterpene lactone parthenolide with anti-cancer, anti-inflammatory, anti-spasm, and especially anti-migraine properties. In this study, the aerial parts of five wild populations of this plant were harvested from Mazandaran (Marzanabad, Siah Bisheh, and Valiabad) and Tehran (Ahar and Gachsar) provinces at full flowering stage in 2020 to evaluate their morphological traits and parthenolide content. The results showed a significant difference at 1% probability level among the studied populations for all the traits. The highest plant height (130 cm), flower dry weight (8.60 g.plant-1), and parthenolide content (0.33 mg.g-1 dry weight) were observed in Gachsar, Siah Bisheh, and Marzanabad populations, respectively. Parthenolide content correlated with leaf length, number of lateral branches, and crown diameter negatively and with number of capitols positively and significantly at 5% probability level. Cluster analysis put the feverfew populations in two main groups. Principal coordinate analysis showed that the first three factors could explain 93.06% of the total variance. Overall, the results indicated the existence of high diversity in feverfew populations for use in breeding programs and cultivation and domestication of this species. [ABSTRACT FROM AUTHOR]

27. Comparison of important quality components of red-flesh kiwifruit (Actinidia chinensis) in different locations

Abstract

Important quality components of red-fleshed kiwifruit which was grown in three different regions in North of Iran were investigated in this research. Fruits produced in the region with higher altitude and cool summer temperatures showed higher ascorbic acid content (AAC), total phenolic content, and antioxidant activity than the two other regions both at harvest time and storage. It was observed that the 1,1-diphenyl-2-picrylhydrazyl was positively correlated with the AAC (r = 0.98), total phenolic content (r = 0.94), and the ferric reducing antioxidant power was positively correlated with the AAC (r = 0.98), TPC (r = 0.93). Fruits harvested in region 2 with cool environments, presented dark red flesh color with the highest values of anthocyanin content at harvest (2.78 mg CGE/100 g), with a gradually increasing trend during storage to reach 3.47 mg CGE/100 g at the end of storage. Total anthocyanin content correlated positively with soluble solids content (SSC; r = 0.82), total Soluble sugars (TSS; r = 0.94), pH (r = 0.94), SSC/titratable acidity (TA; r = 0.83) and negatively with TA (r = −0.77). Fruits grown in region 2 received the highest SCC and TSS scores after sensory test evaluations made by the panelists. During storage fruits firmness and total acidity of kiwifruit decreased while SSC, TSS, and anthocyanin content increased. Additionally, antioxidant compounds and taste-related quality of kiwifruit had a positive relationship with the product region. Results showed that the growing region and storage duration considerably influenced the antioxidant compounds and nutritional quality of red flesh kiwifruit.

Abstract

Objective: Patients’ complain regarding pharmaceutical services at community pharmacies is a fundamental issue as it can directly affect people’s service utilization. For the first time in Iran, this survey aimed to investigate the experience of people regarding declare a complaint against the pharmacy sectors as a community-based study. Methods: In this cross-sectional study, over 100 samples based on postal codes were randomly selected from the city of Shiraz in 2017– 2018. The data collection instrument was designed in two parts (demographic and social profile which record the complaint experiences against pharmacists, pharmacy services, etc.). The data were analyzed by SPSS. Findings: All 1035 eligible participants had a mean age of 45.54 ± 15.82 years (ranged from 14 to 91). Nearly 70% of the participants were female. Around 81.8% had a family physician coverage, whereas 7.4% of them had no medical insurance coverage. The frequency of complaints from the pharmacies was 35.6%. Nearly 55% of the complaints were related to governmental pharmacies. Homemakers were 1.36 times more likely to have experienced complaints in comparison with their employed female counterparts. Health status had an inverse association with complaints. Those participants who had received prescription medication were about two times more likely to have filed a complaint in comparison with those who received medication without a prescription. In addition, females aged 40–59 and above 60 and unemployed participants were more satisfied with respect to complaint follow-up process. Conclusion: Low level of satisfaction with respect to the complaint process is a concerning issue; hence, strategies are warranted to improve the quality of services provided in the pharmacies.

Abstract

High blood cholesterol is typically considered a feature of wealthy western countries1,2. However, dietary and behavioural determinants of blood cholesterol are changing rapidly throughout the world³ and countries are using lipid-lowering medications at varying rates. These changes can have distinct effects on the levels of high-density lipoprotein (HDL) cholesterol and non-HDL cholesterol, which have different effects on human health4,5. However, the trends of HDL and non-HDL cholesterol levels over time have not been previously reported in a global analysis. Here we pooled 1,127 population-based studies that measured blood lipids in 102.6 million individuals aged 18 years and older to estimate trends from 1980 to 2018 in mean total, non-HDL and HDL cholesterol levels for 200 countries. Globally, there was little change in total or non-HDL cholesterol from 1980 to 2018. This was a net effect of increases in low- and middle-income countries, especially in east and southeast Asia, and decreases in high-income western countries, especially those in northwestern Europe, and in central and eastern Europe. As a result, countries with the highest level of non-HDL cholesterol—which is a marker of cardiovascular risk—changed from those in western Europe such as Belgium, Finland, Greenland, Iceland, Norway, Sweden, Switzerland and Malta in 1980 to those in Asia and the Pacific, such as Tokelau, Malaysia, The Philippines and Thailand. In 2017, high non-HDL cholesterol was responsible for an estimated 3.9 million (95% credible interval 3.7 million–4.2 million) worldwide deaths, half of which occurred in east, southeast and south Asia. The global repositioning of lipid-related risk, with non-optimal cholesterol shifting from a distinct feature of high-income countries in northwestern Europe, north America and Australasia to one that affects countries in east and southeast Asia and Oceania should motivate the use of population-based policies and personal interventions t

Abstract

Carbide coatings, due to their excellent anti-wear properties, are used to extend the life of molds exposed to abrasion forces. Various processes have been applied to produce carbide coatings. Thermo-reaction diffusion (TRD) using a molten salt bath could be considered as an economical method compared to other coating processes. In this study carbide-composite coatings using molten salt baths composed of oxides of carbide forming elements (chromium and vanadium) on SKD-11 and T10 tool steels at 1000 °C were formed. The results showed that the coatings included chromium carbide phases: CrC, Cr7C3, and Cr23C6 as well as vanadium carbide: VC, VC0.88, V6C5, V8C7, and a triple phase with the composition of Cr2C2V. The highest hardness (1890-2020 HV) and the lowest coefficient of friction (0.14) were achieved by the carbide coating of T10 steel with the second bath of vanadium oxide. [ABSTRACT FROM AUTHOR]

37. The Mechanism of the Reduction of Hematite-Magnetite Concentrate by Graphite-Calcium Carbonate Mixture in Hoganas Process.

Abstract

In this study, the mechanism of reduction of hematite-magnetite concentrate (HMC) by graphite-calcium carbonate reductant mixture at 1000 °C was investigated. The initial and middle stages of the reduction process (up to 63%) are mainly affected by the heat transfer inside the crucible. In the middle and final stages of the reduction process, the gaseous diffusion to the FeO-Fe reaction interface and the carbon gasification by CO2 jointly controlled this progress. The reduction rate was relatively slow below the temperature at which the carbon gasification by CO2 commences. According to the XRD analysis, prior to the carbon gasification, the metallic iron was not found. The reduction of the HMC hollow cylinder proceeded topochemically mainly from the outer surface. Abstract

The current research aims to investigate some eco-physiological factors of Ziziphora clinopodioides Lam. subsp. rigida (Bioss.) Rech. f. in different habitats. Studied parameters included topography, climate, and physiochemical properties of soil. Co-dominant plant, phenology, and vegetation variation were considered, too. In following, the effect of different climatic conditions on some physiological traits of Z. clinopodioides were investigated. For this purpose, five Z. clinopodioides populations were collected at two phenological stages (vegetative and full flowering). The highest density (0.37) and frequency (23.33%) were registered in Deh Bala and the lowest were in Damgahan and Tezerjan. Based on standard methods pH (7.5-8.0) and EC (0.633-1.470 dc/m) were calculated and the calculated data was different in various habitats. The content of chlorophyll a (9.73–13.18 mg/g), chlorophyll b (2.02–2.88 mg/g), total chlorophyll (11.76–15.98 mg/g), carotenoid (4.48–5.95 mg/g), proline (0.360–0.371 mg/g), and soluble sugar (0.18–0.23 mg/g) varied in the studied species. The phenological stages and climatic conditions significantly affected on the studied pigments except chlorophyll b. 42. Morphological traits variation of needle in males and females yew (Taxus baccata L.) in the Hyrcanian forests of Iran.

Abstract

In this study, the diversity of morphological traits of needle in male and female yew (Taxus baccata L.) along the topographic and longitude gradients in the Hyrcanian forests was studied. To this aim, a total of 580 needles from 58 trees were measured in three sites (Amlash, Gazu and Afratakhteh) along the longitudinal gradient of the Hyrcanian forests. A hierarchical or nested analysis of variance was used to assess influencing sex and site on the 6 measured morphological traits of needles. In addition, Duncan's test was used for multiple comparisons of means. Furthermore, principal component analysis (PCA) was used to determine the most important traits and environmental factors in differing sites. Results showed that variation of morphological traits of yew needles is only affected by site factor. However, the influence of sex was not significantly assessed. Results also showed that most morphological traits of yew needles except the form factor were highest in Gazu site that comprised moderate slopes, northern aspects, faced northern humid air and experienced higher growing season due to lower elevation. Generally, morphological traits of yew needles were lowest in Amlash site, which included steep slopes (loss of soil depth) and western aspects (decrease in relative humidity). Yew trees in Afratakhteh site showed intermediate values. Our results imply that sites with cool summers and higher relative air humidity (settling in the humid northern valleys) provide more favorable growth conditions to morphological traits of yew needles compared with sites with higher average annual rainfall across the Hyrcanian forests. Introduction Daily rhythm as one of the biological rhythms with a period of about 24 hours on various body systems and physiological responses affect on them. Body temperature is a fundamental variable as one of the specifications used daily rhythms The main purpose of this study was evaluating the effect of circadian rhythms on exercise performing and physiological variants of security system in swimmers. Abstract

The biological activity and composition of the essential oil of Sclerorhachis leptoclada Rech. f. an endemic species from northeast of Iran was studied. The essential oil was isolated from the aerial flowering parts of the plant and analyzed by GC and GC-MS. Fifty-four compounds accounting for 95.9% of the total oil were characterized. The main constituents were (E)-nerolidol (14.5%), terpinen-4-ol (13.3%), camphor (6.1%), 1,8-cineole (4.8%) and p-cymene (4.5%). The antimicrobial activity of the essential oil of S. leptoclada was tested against eight microbial strains and a fungi. The results of the bioassays showed that the Gram-positive bacteria, Bacillus subtilis and Staphylococcus epidermidis, were the most sensitive to the oil than others with the MIC value of 1.8 mg/mL. The tested fungi, Saccharomyces cerevisiae was highly inhibited by the oil of S. leptoclada with MIC value of 10 mg/mL. In the case of cytotoxicity, IC50 values estimated to be 312, 1250, 625 and 1250 μg oil/mL respectively, for the Vero, SW480, MCF7, and JET 3 cancer cell lines.

Abstract

In this work, the specimen of the fabrics (polyester/viscose blend) was prepared. At first, the samples were placed under microwave radiation at different times, and then the optimum condition of treated fabrics (8 min) was selected for treatment. The physical properties and surface morphology of Cu nanoparticle and multi-wall carbon nanotubes (MWCNTs) with different percentages were measured using dispersing agent, washing performance, stability, and physical properties of the fabric. The image of surface morphology's specimens was also photographed by scanning electron microscopy (SEM). Afterwards, we measured the specimens' electrical conductivity properties, according to AATCC 2005-76 standards, and subsequently, K/S, %R, and Lab value of specimens was analyzed using reflection spectrophotometer. In fact, the results indicated that optimum electrical resistivity, which was also the aim of the study, is 9% one weight of fabric (o.w.f.) nanoparticles on the fabric and that electrical resistivity for the values of 9% o.w.f. for CNT is slightly greater than Cu. Abstract

Rosmarinic acid (RA) is one of the most common and important caffeic esters in plant material. The effects of precursor feeding and carbon source concentration on biosynthesis of rosmarinic acid by S. khuzistanica cell suspension culture were evaluated. Different concentrations of sucrose (30, 45 and 60 g/l) and Lphenylalanine (0, 0.5, 1.5, 3 and 4.5 mM) were examined in two independent experiments. Feeding phenylalanine reduced growth of cultures and enhanced rosmarinic acid content of cells. Maximum rosmarinic acid accumulation (227.76 ± 3.36 mg RA g/DW) was observed with 3.0 mM phenylalanine at day 7th. Different concentrations of sucrose also influence growth and RA production of S. khuzistanica cell suspension culture. The highest concentration of sucrose (60 g/l) resulted in maximum DW (25.28 ± 0.07 g DW/l) after 21 days. A very high rosmarinic acid yield (about 4350 mg/l RA) was achieved when 45 or 60 g/l sucrose was added to the culture media. Results of this investigation revealed that altering of culture media components (carbohydrate and precursors) can improve RA yield in S. khuzistanica cell cultures and it can represent a promising biotechnological platform for commercial RA production. In general, 3 mM phenylalanine and 60 g/l sucrose concentrations are suggested to be used in the S. khuzistanica cell culture media for higher RA production. [ABSTRACT FROM AUTHOR]
