MIGRAINE , *SYNDROMES - Abstract
Temporary and persistent visual phenomena are a frequent reason for a neurological presentation. The diagnosis can usually be made with the help of a structured anamnesis with determination of the time of onset, the course and symptoms as well as the monocular vs. binocular manifestation. The visual aura in migraine is certainly the most frequent entity to be differentiated. In particular, persistent visual phenomena such as visual snow syndrome, hallucinogen persisting perception disorder and the Charles Bonnet syndrome (CBS) seem to be underdiagnosed in clinical practice for various reasons and are probably not that rare. Instrumental diagnostics are mostly used for exclusion diagnostics and are a component of a complete patient education, but can be indicative for certain questions (CBS, epilepsy). This article presents the most frequent visual phenomena from the neurological practice and their differential diagnoses, guided by a case history. [ABSTRACT FROM AUTHOR]

MIGRAINE - Abstract
Migraine is the most common neurological disorder and can be associated with a high degree of disability. In addition to non-pharmacological approaches to reduce migraine frequency, pharmacological migraine preventatives are available. Evidence-based guidelines from the German Migraine and Headache Society (DMKG), and German Society for Neurology (DGN), Austrian Headache Society (ÖKSG), and Swiss Headache Society (SKG) are available for indication and application. For therapy-relevant questions such as the duration of a pharmacological migraine prevention, no conclusions can be drawn from currently available study data. The aim of this review is to present a therapy consensus statement that integrates the current data situation and, where data are lacking, expert opinions. The resulting current recommendations on the duration of therapy for pharmacological migraine prophylaxis are shown here. [ABSTRACT FROM AUTHOR]

ACTION potentials , *MISSENSE mutation , *MIGRAINE , *GENETIC mutation , *STOICHIOMETRY , *POTASSIUM channels - Abstract
Voltage-gated potassium (Kv) channels are integral to cellular excitability, impacting the resting membrane potential, repolarization, and shaping action potentials in neurons and cardiac myocytes. Structurally, Kv channels are homo or heterotetramers comprising four α-subunits, each with six transmembrane segments (S1–S6). Silent Kv (KvS), includes Kv5.1, Kv6.1–6.4, Kv8.1–8.2, and Kv9.1–9.3, they do not form functional channels on their own but modulate the properties of heteromeric channels. Recent studies have identified the Kv6.4 subunit as a significant modulator within heteromeric channels, such as Kv2.16.4. The Kv2.16.4 heteromer exhibits altered biophysical properties, including a shift in voltage-dependent inactivation and a complex activation. Current genetic studies in migraine patients have revealed a single missense mutation in the Kv6.4 gene. The single missense mutation, L360P is in the highly conserved S4–S5 linker region. This study aims to demonstrate the biophysical effects of the L360P mutation in Kv2.1 6.4 channels with a fixed 2:2 stoichiometry, using monomeric (Kv2.1/6.4) and tandem dimer (Kv2.1_6.4) configurations. Our findings suggest that the L360P mutation significantly impacts the function and regulation of Kv2.1/6.4 channels, providing insights into the molecular mechanisms underlying channel dysfunction in migraine pathology. • Characterization of the biophysical properties of Kv6.4L360P using subunit concatenation. • Quantification of the impact of this mutation on channel gating. • L360P recovers the channel from pronounced closed-state inactivation. INTRACRANIAL hypertension , *VISUAL acuity , *INTRACRANIAL pressure , *OBESITY in women , *PAPILLEDEMA , *VISION , *MIGRAINE , *VISUAL fields , *CHILDBEARING age - Abstract
Idiopathic intracranial hypertension is a pressure-induced secondary headache disorder and optic neuropathy. It primarily affects obese women of childbearing age and poses an interdisciplinary challenge both diagnostically and therapeutically. The most common symptom of this disorder are headaches frequently accompanied by photo- and/or phonophobia, whose semiology often resembles that of migraine, followed by transient visual obscurations and pulsatile tinnitus. While protection of visual acuity and visual fields are of first therapeutical priority, adequate headache treatment also plays a key role. In the majority of cases, conservative treatment including weight loss and pharmacological therapy is sufficient. In case of a fulminant disease course or loss of visual function, interventional strategies can be applied additionally. Headache treatment is guided by the predominant semiology. Background: A patent foramen ovale (PFO) has been associated with embolic strokes and transient ischemic attacks (TIAs). Catheter closure of PFO is effective in preventing recurrent events. Residual shunts and procedure or device related complications can occur, including atrial fibrillation and thrombus formation. This study examines the initial experience with a new PFO closure device, the IrisFIT PFO-Occluder (Lifetech Scientific, Shenzhen, China). Methods: 95 patients with indications for PFO closure underwent percutaneous closure with the IrisFIT PFO-Occluder. The primary endpoint was the rate of accurate device placement with no/small residual shunt at 3 or 6 months follow-up. All patients underwent transoesophageal echocardiography (TEE) after 1 to 6 months. In case of a residual shunt, an additional TEE was performed after 12 months. Clinical follow-up was performed up to a mean of 33.1 ± 3.6 months. Results: The device was successfully implanted in 95 (100%) patients with no relevant procedural complications. At final TEE follow-up (7.6 ± 3.9 months) the effective closure rate was 96.8% with 1 moderate and 2 large residual shunts. There were 8 cases of new onset atrial fibrillation and 2 TIAs. There were no cases of device embolization or erosion. Conclusion: The IrisFIT occluder is a new PFO closure device with several advantages compared to other devices. In this small study cohort, technical success rate, closure rate and adverse event rate were comparable to other devices. The rate of new onset atrial fibrillation was higher in comparison to other studies and warrants further investigation. [ABSTRACT FROM AUTHOR]- Published

- Abstract
Many people suffer from headaches. Primary (idiopathic) headaches such as migraine and tension type headaches are most common. They may significantly affect occupational and social aspects of daily life, but are usually not dangerous. On the contrary, secondary (symptomatic) headaches may be a symptom for an underlying life-threatening disorder. In these cases, the primary care physician may need to organize certain diagnostic measures such as brain imaging, blood work or lumbar puncture. Sometimes, emergency hospital admission may be required. In this article we summarize the recommended steps for the assessment of new-onset headaches in patients visiting the primary care practice. - Abstract
Headaches are a common symptom of COVID-19 infections. Patients generally describe them as bilateral, predominantly frontal, squeezing and of moderate or severe intensity. Searching for "Red Flags" often allows distinction from primary headaches – usually fever, cough, and elevated inflammatory markers accompany COVID-19-associated headaches. Prospective studies did not confirm caveats against the use of ibuprofen as symptomatic treatment. While carrying facial masks often caused headaches, probably by compressing sensory nerves, many patients' migraine frequencies dropped during lockdown. Treatment of patients with primary headaches was complicated by quarantine and many centres offered online consultations. [ABSTRACT FROM AUTHOR]

31. Riboflavin: An Alternative Approach to Managing Migrane Attacks in the Adult Population

riboflavin ,vitamin B2 ,migrane ,headache ,treatment ,human. adult - Abstract
Riboflavin may serve as a clinically important nutritional supplement in treating migraine attacks. But despite its potential therapeutic utility, riboflavin’s effects on patients with migraines have only been documented in four studies. Based on the results of these studies, comparing riboflavin with placebo demonstrated statistically significant improvements in many variables, among which included reduction of attack frequency, severity, and duration. However, these improvements were only noted in the adult population (Age > 18). Studies conducted on the pediatric population showed the opposite trend, with no statistically significant differences found between the placebo and riboflavin groups. Thus, riboflavin appears to have therapeutic value only when administered to adult patients suffering from migraine attacks.

- Abstract
The relationship between headache and multiple sclerosis (MS) has been a matter of controversy for over 60 years. Headaches are still rated as a "red flag", indicating alternative diagnoses to MS, although in the last few years numerous studies have shown a frequent association between headache and MS. In recent studies on MS patients, a link was found between lower age/shorter disease duration of MS and frequent headaches. A study of 50 patients manifesting MS for the first time showed the highest headache prevalence in MS of 78% reported so far. Headaches can also be a possible side effect of most disease-modifying MS drugs. In many cases, however, the headache appears to be a symptom of MS in terms of secondary headache. This is also supported by pathophysiological implications, for example, by detecting B cell follicles in the meninges of MS patients. Migraine is the most common type of headache in MS. In some cases, this is a comorbidity of two diseases with many similarities, but headaches caused by inflammatory MS lesions also appear to be phenomenologically very similar to classic migraines; thus, distinguishing between them is often only successful with the help of thorough differential diagnostics (cerebrospinal fluid, MRI etc.). The task of future studies must be to specify the phenomenology of headache in MS even more precisely, in order to, to gain knowledge in, among others, patients with radiologically isolated syndrome, who often suffer from headache, because in these patients a considerable differential diagnostic and therapeutic uncertainty exists. - Abstract
- Abstract
Ergebnisse: Von 1589 Jugendlichen (mittleres Alter 16.8 Jahre) hab, Introduction: Cardiovascular diseases are the foremost cause of death worldwide. First not (yet) clinically relevant vascular alterations are already detectable in childhood and characterize the concept of early vascular ageing (EVA). Therefore, it is important that cardiovascular risk prevention starts already in youth. Like cardiovascular diseases, headaches are a globally relevant disorder with a high burden of disease, and it is well established that migraine with aura is associated to a higher risk for cardiovascular diseases, especially stroke. EVA-Tyrol had the objective to ameliorate cardiovascular health in Tyrolean adolescents by assessing the effectiveness of a multi-layer health promotion program. Methods: EVA-Tyrol is a community-based non-randomized controlled trial. Recruitment took place in high schools and training companies in North- and East-Tyrol (Austria) and Bruneck in South-Tyrol (Italy). Participants of the 10th school grade (mainly aged 15 to 16 years) were appointed to an intervention (n = 1573) group with a two-year follow-up in the 12th school grade (mainly aged 17 to 18 years). A control (n = 529) group was recruited in the 12th school grade of different schools or companies that did not receive any intervention. In both groups participants underwent a medical examination with, among other variables, measurement of carotid intima media thickness by high resolution ultrasound, and of carotid-femoral pulse wave velocity. Headache history was part of the medical interview and was classified according to the International Classification of Headache Disorders. For the present analysis data from the follow-up examination of the intervention group, baseline information of participants in the intervention group without follow-up examination and of the control group was used (total n = 2102) if parameters of EVA as well as vascular risk factors and behaviours were available. Results: Of 1589 adolescents, with a mean age of 16.8 years, 43 (2.7%) r, submitted by Dr. med. univ.
Migraine is a chronically recurrent primary headache disorder, which affects more than one billion people each year, with an upward trend. Women are three times more likely to be affected than men. Therapeutic approaches are rarely personalized, which leads to a lot of patients being insufficiently treated by universally effective therapeutics. Trigger factors are potential elicitors of migraine attacks. Knowledge of individual trigger factors presents a possible approach to optimized therapy plans. Classification and clear distinction of migraine and other primary headache disorders provide an important basis for understanding the disease. Migraine etiology, pathogenesis and pathophysiology are not fully understood, but point to resulting mechanisms of action of potential trigger factors. Hormonal fluctuations, changes in eating and sleeping habits and stress are the most commonly reported trigger factors. Basic mechanisms of action and evidence-based data are diverse and highly varying. Despite patient-oriented benefits of trigger factor knowledge, extensive avoidance of them should be circumvented. Overall trigger factors offer an opportunity to better deal with migraine attacks. Context: Headaches not only concern adults but also many children and adolescents. Since pain can have a major impact on both a social and psychological level, it is important to recognise this issue. A debate about suitable treatment options is therefore relevant and physiotherapy can make a substantial contribution. Objectives: The aim is to answer the following research question: “Which impact can physical activity and relaxation techniques have on the duration, frequency and intensity of tension-type headache and migraine in children and adolescents aged between 6 and 18 years?” Methods: This thesis is a non-empirical research paper. As basis, a systematic literature search was carried out, using the online databases PubMed, CINHAL and PEDro. The qualitative assessment of the literature filtered was based on checklists for experimental studies and cohort studies published by the Specialist Unit for Review Evidence. Results: To answer the research question two RCTs, one effectiveness study and one cohort study were included in the literature review. In the three experimental studies, the treatments in the intervention groups led to a significant reduction in headache frequency. The cohort study examined the physical activity of participants with migraine in comparison to individuals with tension-type headache and participants without headache using pedometers. The results showed that the adolescents with migraine engaged in less physical activity than non-headache participants. Conclusion: According to current studies it can be said that many of the treatment approaches, already examined, have proven to be effective. Relaxation techniques have generally shown very positive results. The extent to which exercise affects migraine and tension-type headache has not yet been entirely clarified. The trend in the treatment of children and adolescents with headaches is currently towards combing different measures and thus achieving the best result within a multiprofessional team.

Context/Background: Migraine is considered one of the most common neurological diseases and is a great burden for those affected. It is also a burden on global health systems by increased inability of those affected to work and regular treatment costs. The pathophysiology of migraine and its background are not yet fully understood, but (chronic) inflammatory processes, along with genetic factors, are the focus of current research. Inflammatory processes and their systemic effects in the entire organism are an interesting field of activity for the integrative way of thinking. The use of integrative methods could be an ideal supplement to conventional methods due to the low rate of side effects. Previously unrecommended applications aiming at combating inflammation (as one of the main causes of migraine) are being searched. Objectives: The aim of this systematic literature review is to identify integrative methods that ideally counteract inflammation and support the treatment of migraine. The aim is to identify accepted holistic methods and to report on their quality. Methods: Studies on integrative methods in the treatment of migraine were identified through a systematic search, using the AMSTAR 2-tool to assess significance, and compared on their recommendations. Results: Eleven systematic reviews were included in the study, ten of which gave a positive statement on the method. Five papers were assessed as high quality, three papers as moderate quality and one paper as low quality. Thus, these data on certain diets, including the micronutrients melatonin, omega-3 fatty acids and curcumin, as well as on the cupping treatment, can provide new impulses in the treatment of migraine and motivate further research. For already known integrative therapy options such as magnesium, coenzyme Q10 and riboflavin, further supporting data could be collected. Conclusion: Integrative methods prove to be a potential support to conventional treatments for migraine and should be included in the treatment. Further research is needed in the future to improve the evidence level of the acquired knowledge.
