Objective The factors affecting the transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antibodies from mother to newborn and the duration of seropositivity rates in these infants have not yet been clearly demonstrated. The objectives of this study were (1) to assess the levels of SARS-CoV-2 spike–specific immunoglobulin G (IgG) in women infected in the pregnancy period and newborns born to these women and (2) to search the transplacental transfer ratio of spike-specific IgG. Methods Seventy pregnant women with symptomatic SARS-CoV-2 infection and their newborns were prospectively followed. Anti–SARS-CoV-2 immunoassay was used for the detection of the in vitro quantitative determination of total antibodies to the SARS-CoV-2 spike protein. Discussion Spike-specific IgG was demonstrated in 89.1% (44 of 46) of pregnant women infected more than 14 days before delivery and in 92.6% (43 of 44) of their newborns. Median transfer ratio of spike-specific Ig was 0.87 (interquartile range [IQR], 0.34–0.90), 1.0 (IQR, 0.9–0.29), and 0.81 (IQR, 0.02–1.0) in first trimester (n = 4), second trimester (n = 14), and third trimester (n = 28) pregnant women, respectively. Antibody transfer ratio was correlated with time elapsed from infection (p Conclusion Timing from infection to delivery and severity of maternal infection are critical in assessing the antibody generation and transport. Higher antibody transfer ratio can be detected in neonates when SARS-CoV-2 infection is present for more than 60 days before birth. Maternally derived antibody can persist for 3 months after birth.

Purpose: There is a trend that causes an increase in newborn weights and macrosomy rates in many countries around the world. The aim of our study is to compare the mean newborn weights in 2012 and 2016, to investigate whether there is a tendency to increase in the mean newborn weight and macrosomia rates, and to evaluate the effects of neonatal gender, mode of birth, maternal age and the season of birth on newborn weight. Materials and Methods: The data of a total of 1573 newborns 2012 (n:687) and 2016 (n=886) between 37-40 weeks of gestation were evaluated retrospectively. The data of the newborns' birth weight, gender, gestationel age, maternal age, mode of birth and season of birth were evaluated statistically. Results: It was found that there was an increase in the rate of macrosomia (birth weight ≥4000 g) from 4.9% in 2012 to 8.1% in 2016. There is a weak positive correlation between maternal age and newborn weight for both years. There was a significant increase in terms of birth weight between babies delivered by cesarean section and babies delivered by normal route 2016. It was observed that the season of birth had no effect on birth weight. Conclusion: There was an increase macrosomia rates in 2016. It was observed that maternal age and neonatal gender affected the weight of the newborn, but the seasons have no effect on neonatal weight. Maternal age, sex of newborn and gestational age were considered as independent risk factors for macrosomia.

Objectives: To evaluate the perinatal outcomes of antenatally diagnosed omphalocele cases. Material and methods: This was a retrospective study conducted between July 2014 and February 2020 at the prenatal diagnosis center of a university clinic. Gestational week of diagnosis, associated anomalies, karyotype analysis results, complications during pregnancy, termination/delivery characteristics, and postnatal results were evaluated. Results: The analysis was performed on 58 patients. The median diagnosis time was 14.5 weeks of gestation. Thirty-three cases (57%) were defined in the first trimester. 33 (57%) of 58 patients had one or more concomitant anomalies, while 25 patients (43%) had isolated omphalocele. The most common associated anomaly was a cardiac anomaly which was observed in 17 fetuses (30% of all omphalocele cases). Karyotype analysis was performed in forty-five patients (41 in the prenatal period, 4 in the postnatal period). A normal karyotype was detected in 27 cases (60%). Trisomy 18 was the most common chromosomal anomaly (n = 15, 33.3%). Thirty of 58 patients (52%) requested termination of pregnancy (TOP) in the early pregnancy period. Thirteen of the cases died in-utero (22%). Fifteen pregnancies resulted in live births (26%), of those eight were lost in the first year of life (six of them had additional anomalies, while two of them had isolated omphalocele but the omphalocele pouch was containing the liver in those two babies). Conclusions: Most of the cases with an omphalocele can be diagnosed in the first trimester. Cardiac anomalies were the most common associated anomalies, while trisomy 18 is the most common chromosomal anomaly. Thus, earlier and effective counseling can be made about the prognosis of pregnancy.

We aimed to investigate whether transvaginal ultrasonography (TVUSG)-measured tumour size, pattern and location were significant predictors for lymph node metastasis in the uterus-confined endometrioid endometrial cancer (EEC) patients. A total of 213 patients with EEC were recruited and 73 of them were considered eligible and were analysed according to lymph node involvement. Tumour size, pattern and location measured by transvaginal ultrasound were recorded. Objective: Postoperative hematoma following abdominal surgery is relatively rare and mainly depends on the type of surgery. Specific treatment including surgery or interventional radiology is sometimes necessary. The aim of this study is to evaluate the cases of postoperative hematoma after gynecologic and obstetric surgery.Study Design: This is a retrospective cohort study of 30 patients with hematoma developed after gynecologic and obstetric surgery. We included the patients who hospitalized with the diagnosis of a postoperative hematoma between June 2017 and April 2019 at Gazi Yasargil Training and Research Hospital of Health Sciences University. Hematomas occurring after endoscopic surgery and episiotomy were not included. The diagnosed cases were divided into three groups as wound hematoma, rectus sheath hematoma and intra-abdominal hematoma (intraperitoneal and retroperitoneal). All cases were assessed by patient demographics and clinical findings, hematoma of characteristics, treatment methods and results.Results: A total of 30 patients were included in the study with a mean age of 33.0±8.6 years. Incidence of hematoma account for 0.2%. The mean c-reactive protein was 37.9±47.4 mg/dL at admission and 14.6±25.8 mg/dL at discharge, respectively. The decrease was statistically significant (p < 0.001). The mean hemoglobin was 10.6±2.1 g/dL at admission and 10.7±1.5 g/dL at discharge. Fever was detected in 7 (23.3%) patients. Only 12 patients (40%) were followed up by observation and symptom management. In 10 (33.3%) patients, antibiotics were included in the treatment due to infection. In addition, 4 patients (13.3%) had relaparotomy, 5 patients (16.7%) underwent percutaneous radiological drainage and 8 (26.7%) received blood transfusion. The mean time of resorption of the hematoma was 4.6 ± 2.0 days. The evaluation of the hematoma locations revealed that 14 patients (46.7%) had wound hematoma, 7 patients (23.3%) had rectus sheath hematoma (Type I: 2 cases, type II: 3 cases, type III: 2 cases), 8 patients (26.7%) had pelvic hematoma and 2 patients (6.7%) had a retroperitoneal hematoma. The mean hematoma size was 68.1±15.18 mm. Conclusions: In cases of hematoma resistant to antibiotic treatment and non-resorbable hematoma, we can consider percutaneous catheter drainage as an alternative to surgical intervention.

Objective: Congenital pes equinovarus (PEV) is the most common congenital deformity of the foot, characterized by plantar flexion with a frequency of 0.2-0.3%. It can be diagnosed from the 12th week of pregnancy. Non-isolated cases tend to be syndromic and complex. We aimed to evaluate the results of perinatally diagnosed isolated PEV. Materials and Methods: This was a retrospective cohort study conducted between March 2015-March 2020. Women who presented for fetal anomaly screening or were referred due to any suspected fetal anomaly were subjected to detailed fetal anomaly scans and checked for the presence of PEV. Karyotype analysis was discussed for patients with PEV. Pregnancy termination was recommended for those with chromosomal/life-threatening anomalies. The diagnosis was confirmed by postnatal examination/autopsy. Postnatal diagnosis was accepted as false-positive in those with no PEV. Results: One-hundred thirty-eight patients were found to have PEV, 41 (29.7%) of which were isolated. BACKGROUND: Among women who have suffered loss of pregnancy, the level of grief decreases gradually. Age, mental health status and childlessness are the factors known to mostly affect women’s levels of grief. OBJECTIVES: To assess the levels of grief among women who experienced perinatal loss and the changes in their ruminative thought styles over the first year after their loss. DESIGN AND SETTING: One-year follow-up study carried out in a university hospital in Turkey. METHODS: The study population included 70 women who experienced loss of pregnancy in the hospital. The sample size was calculated using G*Power V3.1. Data were collected at 48 hours, at the third month, at the sixth month and at one year after pregnancy loss, between June 2018 and June 2019. A personal information form, the Perinatal Grief Scale and the Ruminative Thought Style Questionnaire were used for data collection. RESULTS: The women’s highest levels of grief and ruminative thought style were in the first 48 hours. Their tendency towards grief and ruminative thought styles decreased over the repeated measurements during the follow-up. Women aged 20-29 years had the highest levels of grief at the third month after perinatal loss. CONCLUSIONS: Nursing assessments regarding grief and ruminative thought style over the first 48 hours after perinatal loss should be integrated into nursing care for these women. Grief follow-up programs for these women can be developed through nursing research.

Background and objectives: Limited data are present on safety and efficiency of epinephrine for the prophylaxis and treatment of spinal-hypotension. This was conducted to compare the effect of epinephrine with norepinephrine and phenylephrine on the treatment of spinal-hypotension and ephedrine requirement during cesarean delivery. Methods: 160 parturient with uncomplicated pregnancies undergoing elective cesarean delivery under spinal anesthesia were recruited. They were allocated randomly to receive norepinephrine 5 μg.mL-1 (n = 40), epinephrine 5 μg.mL-1 (n = 40), phenylephrine 100 μg.mL-1 (n = 40) or 0.9% saline infusions (n = 40) immediately after induction of spinal anesthesia. Whenever systolic blood pressure drops to less than 80% of baseline, 5 mg of IV ephedrine was administered as rescue vasopressor. The incidence of hypotension, total number of hypotension episodes, the number of patients requiring ephedrine, the mean amount of ephedrine consumption and side effects were recorded. Results: There was no statistically significant difference in incidence of maternal hypotension between groups. The number of patients requiring ephedrine was significantly greater in group saline than in group phenylephrine (p Conclusion: There is no statistically significant difference in incidence of hypotension and ephedrine consumption during spinal anesthesia for cesarean delivery with the use of epinephrine when compared to norepinephrine or phenylephrine. Epinephrine can be considered as an alternative agent for management of spinal hypotension.

Objective To evaluate sacrococcygeal teratoma (SCT) cases according to associated cardiac, extracardiac, and chromosomal anomalies in the prenatal period, and to review their outcomes. Materials and methods Data of pregnancies with a prenatal diagnosis of SCT between 2009 and 2019 were retrospectively reviewed. Results One ongoing pregnancy was excluded. There were five medically terminated cases, three due to severe heart failure and the remaining two due to additional congenital defects. Two infants who had heart failure due to hyperdynamic flow died in the neonatal period. Nine infants are well and alive at the time of writing. Conclusion When a lesion is detected in the sacrococcygeal region during fetal sonography, the differential diagnosis should be made with an appropriate evaluation with emphasis on a possible diagnosis of fetal SCT. Tumor growth and heart failure should be monitored with serial scans when SCT has been diagnosed prenatally.

Purpose: The aim of this study was to evaluate the relationship between patient temperament and preference for invasive prenatal tests.Materials and Methods: This was a prospective study of 337 pregnant women who had increased risk of having a fetus with Down syndrome. Their temperament profile was determined by using the temperament evaluation of TEMPS-A. Women were grouped as accepting (study) or declining (control) to perform an invasive test. Results: 284 pregnancies were included in the final analyzes. The study group had more likely employed and had a higher level of education level. There was no predominant temperament in 247 (87%) women. Hyperthymic temperament had the highest scores (11.2 ± 4.1) among other four temperament types. Study group patients had lower scores for anxious and depressive temperaments compared with control groups. Conclusion: Women who had a less anxious or depressive personality were more likely to prefer an invasive prenatal test. WOS: 000462504300004 PubMed ID: 31019836 Objective: To evaluate persistent left superior vena cava (PLSVC) cases according to associated cardiac, extracardiac, and chromosomal anomalies in the prenatal period and to review their outcomes. Materials and Methods: The data of patients with a prenatal diagnosis of PLSVC between January 2013 and December 2017 were reviewed retrospectively. Results: Data of 32 cases were reviewed. Nineteen (60%) cases were associated with cardiac defects, 5 (15%) were associated with both cardiac and extracardiac defects, and 8 (25%) had no associated anomalies. Two fetuses had karyotype anomalies. All patients with isolated PLSVC survived. Among the cases associated with extracardiac anomalies, cardiac anomalies, and with both extracardiac and cardiac anomalies, the survival rate was 40%, 40%, and 25%, respectively. Outcome was more favorable in cases with isolated PLSVC (100% vs. 40%). Conclusion: Prenatally diagnosed PLSVC is associated with cardiac and extracardiac anomalies in the majority of cases. The prognosis is good in isolated cases, but worsens when accompanied by cardiac or extracardiac anomalies.

Objectives: Grade 3 endometrioid adenocarcinomas (G3 EAC), type two endometrial carcinomas (Type 2 EC), and also uterine carcinosarcomas (UCS) are considered as high-grade endometrial adenocarcinomas. The aim of this study was to compare the clinicopathologic features and survival of patients with UCS, G3 EAC, Type2 EC. Material and methods: We included two hundred and thirty-five patients in this study. Patients were divided into three groups according to the type of tumor as uterine G3 EAC (group 1, n = 62), Type 2 EC (serous, clear and mixed types; group 2, n = 93), and UCS (group 3, n = 80). We compared the groups according to age, initial symptom, surgical approach, stage, myometrial invasion (MI), lymph node invasion (LNI), lymphovascular space invasion (LVSI), adjuvant therapy, and survival. When comparing the survival outcomes the Kaplan-Meier analysis was performed. Results: The groups were similar according to age, menopausal status, nulliparity, initial symptoms, stage, LVSI, and LNI. Positive cytology was determined significantly more in group 3. There was a significant difference between the groups in terms of myometrial invasion degree. Optimal cytoreduction was similar among the groups. The primary adjuvant treatment was chemotherapy for UCS and Type2 EAC whereas radiotherapy was the main adjuvant treatment for G3 EAC. There were no significant differences among the groups according to overall survival (OS) (p = 0.290). Conclusions: Although the survival difference among the groups can not be revealed, these patients have different clinical and pathological features and they should be considered as different groups.

Clinic-pathologic features and survival in the MEC group (n=29) were compared to pure serous (n=42) and pure clear cell adenocarcinomas (n=13). Clinical features, operation characteristics, pathological findings, myometrial invasion degree (MI), lymph node involvement (LNI), lymphovascular space invasion (LVSI), adjuvant therapies, and follow-up data of the patients and their effects on survival were investigated. Results: Eighteen of the cases had endometrioid + serous, 7 had endometrioid + clear, 3 had endometrioid + serous, and 1 had clear + serous histopathology. Laparoscopic surgery was performed in 8 of the cases (27.6%) in the mixed group. Stage, the rate of LVSI, LNI, MI ≥50%, and omental metastasis were similar among the groups. There were no significant differences in the rates of receiving adjuvant therapy among the groups. Overall survive (OS) was similar among the groups. Conclusion: MECs are tumors that can be difficult to diagnose and manage. There was no difference between MEC and pure serous carcinoma (SC) and pure clear cell carcinoma (CC) in terms of clinicopathological features and prognosis. In addition to histopathological features, revealing and evaluating their molecular properties will help us to better understand this group of tumors., Amaç: Bu çalışmada miks endometrial karsinomun (MEK) klinik ve patolojik özelliklerini değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Ocak 2000 - Aralık 2019 tarihleri arasında kliniğimizde ameliyat edilen ve düzenli takip edilen 29 MEK hastasının klinik ve patolojik kayıtları gözden geçirildi. MEK grubundaki (n = 29) klinik-patolojik özellikler ve sağkalım, saf seröz (n = 42) ve saf berrak hücreli adenokarsinomlar (n = 13) ile karşılaştırıldı. Hastaların klinik bulguları, operasyon özellikleri, patolojik bulguları, miyometriyal invazyon derecesi (MI), lenf nodu tutulumu (LNI), lenfovasküler alan invazyonu (LVSI), adjuvan tedaviler ve takip verileri ve sağkalıma etkileri araştırıldı.Bulgular: Olguların 18'inde endometrioid + seröz, 7'sinde endometrioid + berrak hücreli, 3'ünde endometrioid + seröz ve 1'inde berrak hücreli+ seröz histopatoloji vardı. Hastaların ortalama yaşı 63,2 ± 12,1 yıldı. Mikst gruptaki olguların 8'ine (% 27,6) laparoskopik cerrahi uygulandı. Evre, LVSI, LNI, MI ≥% 50 ve omental metastaz oranları gruplar arasında benzerdi. Adjuvan tedavi alma oranları için gruplar arasında önemli bir fark yoktu. Genel olarak hayatta kalma süresi gruplar arasında benzerdi.Sonuç: MEK'ler, teşhis edilmesi ve yönetilmesi zor olabilen tümörlerdir. Klinikopatolojik özellikler ve prognoz açısından MEK ile saf seröz karsinom ve saf berrak hücreli karsinom arasında fark yoktu. Histopatolojik özelliklerinin yanı sıra moleküler özelliklerinin ortaya çıkarılması ve değerlendirilmesi bu grup tümörleri daha iyi anlamamıza yardımcı olacaktır.

Introduction/Background Mixed endometrial carcinoma (MEC) refers to a tumor that is comprised of two or more distinct histotypes. Each component histotype by definition has to represent more than 5% of the tumor. Although it is relatively rare, both diagnosis and management can be troublesome. Molecular and histopathologic features have become important in the identifcation and more importantly the precise management of the MEC’s. In our study, we aimed to evaluate the clinical and pathological characteristics of the MEC Methodology The clinical and pathological records of the 29 MEC patients who were operated on and regularly followed up in the clinic between January 2000-December 2019 were reviewed. Clinical features, operation characteristics, pathological findings, myometrial invasion degree (MI), lymph node involvement (LNI), lymphovascular space invasion (LVSI), adjuvant therapies, and follow-up data of the patients and their effects on survival were investigated. Results During the study period, 29 out of 1110 patients with endometrial cancer had MEC (2.6%). Eighteen of the cases had endometrioid + serous, 7 had endometrioid + clear, 3 had endometrioid + serous, and 1 had clear + serous histopathology. The mean age of the patients was 63.2±12.1. Laparoscopic surgery was performed in 8 of the cases (27.6%).Sixteen of the cases were in stage 1 (55.1%), 4 were in stage 2 (13.8%), and 9 were in stage 3 (31%). LVSI was positive in 17 cases (58.6%). LNI was detected in 7 cases (24.1%). Approximately 80 percent of cases received adjuvant therapy. While 80% of the cases received chemotherapy, this rate was 55% for radiotherapy. Conclusion MECs are tumors that can be difficult to diagnose and manage. Purpose: The aim of this study was to show the effect of early and late onset intrauterine growth retardation (IUGR) on perinatal outcomes and the differences between themMaterials and Methods: Our study is a retrospective study. 334 patients who were followed up with the diagnosis of IUGR between January 1, 2015 and January 1, 2019 were divided into subgroups as early and late onset IUGR. The type of growth retardation, the gestational week at diagnosis, the estimated fetal weight (EFW), the presence of preeclampsia, doppler findings, the birth weight, the delivery type, the birth week, the 5th minute Apgar score, the neonatal intensive care need were recorded and the relationship of all these parameters with the negative consequences of the newborn was discussed.Results: The average of the gestational week at diagnosis was 30.9 ± 4.5 (min-max: 18-39). The average of the gestational week at delivery was 34.4 ± 4.5 (min-max: 22-41). The patients were examined in four groups as 37 weeks according to the diagnosis time. Perinatal outcomes deteriorate with the severity of Doppler findings. Çalışmamızda 01 Ocak 2015- 01 Ocak 2019 tarihleri arasında İUGG tanısıyla takibe 334 hasta erken ve geç başlangıçlı IUGG olmak üzere alt gruplara ayrıldı. Gelişme geriliği tipi, tanı aldığı gebelik haftası, tahmini fetal ağırlığı (TFA), preeklampsi varlığı, Doppler bulguları, doğum kilosu, doğum şekli, doğum haftası, 5.dk Apgar skoru, yenidoğan yoğun bakım ihtiyacı kayıt edildi ve tüm bu parametrelerin yenidoğanın olumsuz sonuçlarıyla ilişkisi ele alındı.Bulgular: Tanı konulan gebelik haftası ortalaması 30,9±4,5 (min-maks:18-39)’dır. Amaç: Bu çalışmanın amacı agonist in-vitro fertilizasyon (IVF) / intrasitoplazmik sperm enjeksiyonu (ICSI) / embriyo transferi (ET) siklüslerinde, luteal faz desteği için progesterona ek olarak estrojen vermenin gebelik oranları üzerine etkisinin araştırılmasıdır. Gereç ve Yöntem: Bu çalışmaya toplamda IVF / ICSI endikasyonu olan 142 hasta çalışmaya alındı. Hastalar, oosit toplama işleminden (OPU) sonra luteal destek için progesteron (kontrol grubu n = 71) ve ek olarak transdermal östrojen (çalışma grubu n = 71) verilmek üzere randomize edildi. Östrojen seviyeleri, embriyonun transfer gününde ve transfer sonrası 12. günde ölçüldü. Çalışmanın esas sonucu devam eden gebelik oranıdır. Her iki grup indüksiyon özellikleri, östrojen seviyeleri, β-hCG pozitifliği ve düşük oranları açısından da karşılaştırıldı.Bulgular: Gruplar arasında yaş, infertilite süresi ve endikasyon açısından istatistiksel olarak anlamlı fark yoktu. Her iki grup indüksiyon özellikleri ve E2 düzeyleri açısından benzerdi. β-hCG pozitiflik oranı benzerdi ve devam eden gebelik oranı her iki grupta benzerdi. Biyokimyasal ve klinik abortus oranları açısından da her iki grup arasında anlamlı fark yoktu. 12. gün östrojen seviyesi gebelik elde edilen hastalarda, elde edilemeyenlere göre anlamlı düzeyde yüksekti.Sonuç: Çalışmamız; luteal destek amacı ile progesterona ek olarak östrojen eklenmesinin β-hCG pozitifliği, devam eden gebelik ve düşük oranları üzerinde olumlu bir etkisi olmadığını göstermiştir., Purpose: The aim of this study was to investigate the effect of transdermal estrogen on pregnancy rates in addition to progesterone for luteal phase support in agonist in-vitro fertilization (IVF) / intracytoplasmic sperm injection (ICSI) / embriyo transfer (ET) cycles. Material and Methods: This randomized controlled study (RCT) enrolled a total of 142 patients with an indication for IVF/ICSI. We randomized patients prospectively to administer either progesterone (control group n=71) and additionally transdermal estrogen patch (study group n=71) as luteal support after oocyte retrieval. The levels of estrogen was determined on the day of transfer of embryo and 12th day. The main outcome measure was the ongoing pregnancy rate. Both groups were also compared for the induction characteristics, estrogen levels, β-hCG positivity, and abortion rates. Results: There were no significant differences between the groups according to age, infertility time, and indications. Both groups were similar for induction characteristics and E2 levels. β-hCG positivity rate was similar and the ongoing pregnancy rate was similar in both groups. Biochemical and clinical abortion rates were also similar in both groups. On the 12th day, estrogen levels increased significantly in those who became pregnant group.Conclusion: Our study showed that adding supplementary estrogen to the progesterone has no positive effect on β-hCG positivity, ongoing pregnancy, and abortion rates.

Objectives Our primary aim was to evaluate the ability of various cerebroplacental ratio (CPR) reference values suggested by the Fetal Medicine Foundation to predict adverse neonatal outcomes in term fetuses exhibiting late-onset fetal growth restriction (LOFGR). Our secondary aim was to evaluate the effectiveness of other obstetric Doppler parameters used to assess fetal well-being in terms of predicting adverse neonatal outcomes. Methods This was a retrospective cohort study of 317 pregnant women diagnosed with LOFGR at 37–40 weeks of gestation between January 1, 2016, and September 1, 2019. Receiver operating characteristic (ROC) curves were drawn to determine the predictive performance of CPR 95th percentile in terms of predicting adverse neonatal outcomes. Results Pregnant women exhibiting LOFGR who gave birth in our clinic during the study period at a mean of 38 gestational weeks (minimum 37+0; maximum 40+6 weeks); the median CPR was 1.51 [interquartile range (IQR) 1.12–1.95] and median birthweight 2,350 g (IQR 2,125–2,575 g). The CPR Conclusions The CPR values below the 5th percentile better predicted adverse neonatal outcomes in pregnancies complicated by LOFGR than the UA PI and CPR

Objectives: We hoped to reveal the frequency of Aberrant Right Subclavian Artery (ARSA) and to find the relationship of isolated/non-isolated ARSA with chromosomal defects and other fetal congenital heart diseases (FCHD) in a heterogeneous population. Material and methods: This was a retrospective cohort study conducted between December 2015 to September 2018. Women admitted for routine ultrasound examination or referred to our hospital for a suspected fetal anomaly were underwent detailed fetal anomaly ultrasonography scan and tested for the presence of ARSA. Results: ARSA was detected in 27 patients and an isolated finding in 13 (48%) cases. Among 13 cases with isolated ARSA, trisomy 21 was diagnosed in 1 case. In the non-isolated group (n: 14, 52%), five cases presented with trisomy 21. There was no significant difference of trisomy 21 frequency between isolated and non-isolated groups (7.6% vs 35.7%, p = 0.08). In 3 patients, FCHD was diagnosed and 2 of them had trisomy 21. Conclusions: Our study shows that ARSA can be the only marker in trisomy 21. The examination of the subclavian artery must be a part of the fetal anomaly ultrasonography. Detecting an ARSA should increase the attentiveness of the sonographer to investigate for the other markers of trisomy 21. In the existence of other findings, invasive diagnostic procedures should be offered to the patients, whereas in cases that arsa is the only finding, other risk factors should be investigated to offer karyotyping or cell-free DNA analysis.

Background and objectives: Limited data are present on safety and efficiency of epinephrine for the prophylaxis and treatment of spinal-hypotension. This study was conducted to compare the effect of epinephrine with norepinephrine and phenylephrine on the treatment of spinal-hypotension and ephedrine requirement during cesarean delivery. Methods: One hundred and sixty parturients with uncomplicated pregnancies undergoing elective cesarean delivery under spinal anesthesia were recruited. They were allocated randomly to receive norepinephrine 5 µg.mL−1 (n = 40), epinephrine 5 µg.mL−1 (n = 40), phenylephrine 100 µg.mL−1 (n = 40) or 0.9% saline infusions (n = 40) immediately after induction of spinal anesthesia. Whenever systolic blood pressure drops to less than 80% of baseline, 5 mg of intravenous ephedrine was administered as rescue vasopressor. The incidence of hypotension, total number of hypotension episodes, the number of patients requiring ephedrine, the mean amount of ephedrine consumption and side effects were recorded. Results: There was no statistically significant difference in incidence of maternal hypotension between groups. The number of patients requiring ephedrine was significantly greater in group saline than in group phenylephrine (p< 0.001). However, it was similar between phenylephrine, norepinephrine, and epinephrine groups. The mean ephedrine consumption was significantly higher in group saline than in norepinephrine, epinephrine, phenylephrine groups (p= 0.001). Conclusion: There is no statistically significant difference in incidence of hypotension and ephedrine consumption during spinal anesthesia for cesarean delivery with the use of epinephrine when compared to norepinephrine or phenylephrine. Epinephrine can be considered an alternative agent for management of spinal hypotension. Resumo Justificativa e objetivos: Existem dados limitados sobre segurança e eficiência da epinefrina na profilaxia e tratamento da hipotensão arterial associada à raquianestesia. O presente estudo foi realizado para comparar o efeito da epinefrina com norepinefrina e fenilefrina no tratamento da hipotensão após raquianestesia e necessidade de efedrina durante o parto cesáreo. Método: Foram recrutadas 160 parturientes com gestações não complicadas, submetidas a cesariana eletiva sob raquianestesia. Elas foram alocadas aleatoriamente para receber norepinefrina 5 µg.mL-1 (n = 40), epinefrina 5 µg.mL-1 (n = 40), fenilefrina 100 µg.mL-1 (n = 40) ou infusão de solução fisiológica NaCl a 0,9% (n = 40) imediatamente após a indução da raquianestesia. Sempre que houvesse redução da pressão arterial sistólica para valor inferior a 80% da linha de base, 5 mg de efedrina iv eram administrados como vasopressor de resgate. A incidência de hipotensão, o número total de episódios de hipotensão, o número de pacientes que necessitaram de efedrina, o consumo médio de efedrina e os efeitos colaterais foram registrados. Resultados: Não houve diferença estatisticamente significante na incidência de hipotensão materna entre os grupos. O número de pacientes que necessitaram de efedrina foi significantemente maior no grupo solução fisiológica do que no grupo fenilefrina (p< 0,001). No entanto, foi semelhante entre os grupos fenilefrina, norepinefrina e epinefrina. O consumo médio de efedrina foi significantemente maior no grupo solução fisiológica do que nos grupos norepinefrina, epinefrina e fenilefrina (p = 0,001). Conclusão: Não houve diferença estatisticamente significante na incidência de hipotensão e consumo de efedrina durante raquianestesia para parto cesáreo com uso de epinefrina quando comparada à norepinefrina ou fenilefrina. A epinefrina pode ser considerada como agente alternativo para o tratamento da hipotensão após raquianestesia.

PubMedID: 28647444 Objective The predictive value of spot urine protein-to-creatinine ratio (PCR) for estimating total 24-hour proteinuria in severe preeclampsia is unclear. This study aimed to assess the diagnostic accuracy of spot urine PCR for ascertaining the magnitude of proteinuria in women with preeclampsia of varying severity. Methods A total of 205 patients with prediagnosed preeclampsia were included in this prospective cohort study. Patients were allocated into one of the three groups categorized by severity of disease, as follows: gestational hypertension, group 1 (n = 41); preeclampsia, group 2 (n = 88); and severe preeclampsia, group 3 (n = 76). We assessed the spot urine PCRs to determine significant proteinuria and the magnitude of proteinuria in these groups. Results The spot urine PCR was 0.53, with 81% sensitivity and 93% specificity to detect significant proteinuria. A significant correlation was found between PCR and 24-hour total proteinuria in group 1 (r = 0.473, P = 0.002). There were also significant correlations in group 2 (r = 0.814, P < 0.001) and group 3 (r = 0.912, P < 0.001). The established formula using spot urine PCR to estimate 24-hour total proteinuria in severe preeclampsia was Y = 832.02X + 378.74 mg (r 2 = 0.8304). Conclusion Although 24-hour urine collection remains a merely reliable test to determine the degree of total proteinuria, our findings suggest that it is likely to assess the magnitude of proteinuria by the spot urine PCR, especially in severe preeclampsia. Background and aims There are limited data on safety and efficacy of epinephrine (E) for prophylaxis and treatment of spinal-hypotension. This study was conducted to compare the effect of epinephrine with norepinephrine and phenylephrine on treatment of spinal-hypotension and ephedrine requirement during cesarean deliveries. Methods They were allocated randomly to receive norepinephrine 5 μg/mL (group NE,n=40), epinephrine 5 μg/mL (group E,n=40), phenylephrine100 μg/mL (group PE,n=40) or 0.9% saline infusions (group S, n=40), immediately after induction of spinal anesthesia. Whenever systolic blood pressure fell to less than 90 mmHg, 5 mg ephedrine was administered. Hemodynamic variables were measured preoperatively, 1-minute intervals for 15 minutes and with 2.5-minutes intervals till end of surgery. the incidence of hypotension, total number of hypotension episodes, number of patients requiring ephedrine, mean amounts of ephedrine, atropine consumption was recorded. Results The incidence of bradycardia and mean atropine consumptions were similar between groups. There was no statistically significant difference in incidence of hypotension between the groups. the number of patients requiring ephedrine was significantly greater in group S than in group PE (p=0.001). However, it was similar between groups PE, NE, and E. There was no significant difference in number of hypotension episodes between the groups. the mean ephedrine consumption was significantly higher in group S than other groups(p=0.001). Conclusions Epinephrine infusion (150 µg/h) was as effective as norepinephrine (150 µg/h) infusion, with similar incidences of hypotension and ephedrine consumption, and can be considered as an alternative agent for management of hypotension. However, phenylephrine infusion (3 mg/h) was the most effective drug for management of spinal hypotension during cesarean.

Purpose: The aim of the study is examine the effect of oral contraceptives on the serum levels of adhesion molecules and identify if the synthetic oestrogens provide protection against atherosclerosis.Materials and Methods: The interleukin 6 (IL-6), Tumor necrosis factor alfa (TNF Alfa), Vascular cell adhesion molecule 1 (VCAM-1), Intercellular Adhesion Molecule (ICAM-1), E selectin, P selectin, C selectin, C Reactive Protein (CRP) levels and erythrocyte sedimentation rate (ESH) of patients between 18-45, without additional illness, using oral contraceptives for contraception at least for three years were compared to healthy volunteers, aged between 18-45, having regular menstruation and without any additional diseases.Results: There has been no meaningful difference between the VCAM-1, ICAM-1, E selectin, P selectin, TNF alfa, IL 6, ESH levels of the two groups. Serum CRP levels are meaningfully higher in the group using oral contraceptives.Conclusion: Oral contraceptives change the serum levels of adhesion molecules. The change in the serum CRP levels might be due to the liver first pass effect., Amaç: Bu çalışmanın amacı, oral kontraseptiflerin adezyon moleküllerinin serum seviyelerine olan etkisini araştırarak, sentetik östrojenlerin ateroskleroza karşı koruyucu etkisi olup olmadığı saptamaktır.Gereç ve Yöntem: Ek hastalığı bulunmayan, kontrasepsiyon amaçlı en az 3 aydır oral kontraseptif kullanan, 18-45 yaş aralığında olan hastaların serum interlökin 6 (İL-6), tümör nekrozis faktör alfa (TNF alfa), damarsal hücre yapışma molekülü 1 (VCAM-1), hücreler arası adezyon molekülü (ICAM-1), E selektin, P selektin, c reaktif protein (CRP) seviyeleri ve eritrosit sedimentasyon hızları (ESH); ek hastalığı bulunmayan, düzenli adet gören, 18-45 yaş aralığında olan sağlıklı gönüllülerle karşılaştırıldı.Bulgular: Her iki grubun VCAM-1, ICAM-1, E selektin, P selektin, TNF alfa, İL 6, ESH seviyeleri arasında anlamlı fark saptanmadı. Oral kontraseptif kullanan grupta serum CRP seviyeleri anlamlı olarak daha yüksek saptandı.Sonuç: Oral kontraseptiflerin adezyon moleküllerinin serum seviyelerini değiştirdiği gösterilemiştir. Serum crp seviyelerinin değişimleri ise karaciğer ilk geçiş etkisine bağlanmaktadır.

The most frequent cause of thrombocytopenia during pregnancy is gestational thrombocytopenia. These patients usually do not have symptoms due to mild thrombocytopenia. The diagnosis is usually established by excluding connective tissue disorders, drug effects and obstetric causes. The etiology of thrombocytopenia during early pregnancy is most frequently due to autoimmune causes. Thrombocytopenia secondary to obstetric causes like preeclampsia, HELLP syndrome should always be kept in mind. Under these circumstances the thrombocytopenia will not resolve unless pregnancy is terminated. The clinician should be able to distinguish between thrombocytopenia that does not require treatment and the causes that require emergent and serious medical interventions., Trombositopeni gebeliklerin %7-10’unda saptanan, trombosit sayısının düşüklüğüyle karakterize bir durumdur. Gebelikte görülen trombositopenin en sık sebebi gestasyonel trombositopenidir. Bu hastalarda trombositopeni genelde hafif olduğundan semptom görülmez. Tanı çoğunlukla bağ dokusu hastalıklarının,ilaç etkilerinin, obstetriyle ilişkili sebeplerin dışlanmasıyla konulur. Erken gebelik haftalarında görülen trombositopenin etyolojisinde en fazla otoimmun sebepler rol alır. Preeklampsi,HELLP sendromu gibi obstetriyle ilişkili nedenlere ikincil gelişen trombositopeniler ise her zaman akılda tutulmalıdır. Bu durumların varlığında çoğu zaman doğum gerçekleşmeden trombositopeni düzelmeyecektir. Klinisyen; tedavi gerektirmeyen trombositopenilerle, annenin fetusun hayatını tehdit eden ve bu nedenle acil ve ciddi bakım gerektiren trombositopeni sebepleri arasında ayırımı iyi yapabilmelidir.

Aim: The Objective of our study was to evaluate the chromosomal analysis results that were obtained from amniocentesis, cordocentesis and chorionic villus sampling (CVS) inpatients whom had applied to the perinatology unit of Cukurova University Faculty of Medicine gynecology and obstetrics clinic with high risk in terms of chromosome anomaly according to Ultrasonography (USG).Material and Methods: Our study was conducted as a retrospective pattern. CVS, amniocentesis and cordosentesis were performed in 1298 pregnant women whom had applied to the Cukurova University Faculty of Medicine, Gynecology and Obstetrics Clinic, Perinatology Unit in the date interval between 1st December 2014-31st December 2016 with the indication of abnormal maternal serum screening tests, maternal request because of advanced maternal age, history of fetal anomaly with previous pregnancies, history of relatives with Trisomy 21, fetal abnormalities or signs of trisomy which were detected by ultrasonography and only depending on maternal request without any risk factors. Data obtained in the study were assessed using the SPSS (Statistical Package for Social Sciences) 22.0 package program. The relationships between categorical variables were determined by Chi-Square test. Relationship between normal distribution-matched, numerical data were assessed by ANOVA, Independent Sample t-test, and relationship between non-normal distributions of numerical data were assessed using Mann-Whitney U and Wilcoxon Test. Statistical significance level was determined as p 0.05.Results: Fetal anomalies were observed in 28.9% (n: 366) of the patients while 369 (28.4%) of the 1298 patients who had prenatal diagnosis had abnormalities in the maternal screening results. No chromosomal abnormalities were detected in 1120 (86.2%) of the 1298 patients who were taken into the study. 49 patients had Trisomy 21, 27 patients had Trisomy18 and 14 patients had Trisomy13. Turner syndrome was seen in 10 of thepatients. In our study, chromosomal abnormality rate of patients with more than one minor marker was found to be statistically significant (p: 0.01). Chromosomal anomaly was detected in 319 (8.4%) of 349 patients with combined test. Chromosomal anomaly was detected in 70 (19.1%) of the 366 patients who detected fetal anomaly. Chromosomal anomaly was detected in 27 (24.1%) of the 112 increased NT patients.Conclusion: In our study, 1298 invasive procedures are listed as follows; amniocentesis was performed in 841 (64.8%), cordocentesis in 57 patients (4.4%) and CVS in 400 patients (30.8%). As a result of karyotype analysis of the patients, nochromosomal anomaly was detected in 1120 patients (86.28%). In 178 patients, chromosomal anomaly (13.71%) was detected. This study aimed to determine the prevalence of fetal chromosomal anomaly in the Mediterranean region by determining the prevalence of invasive prenatal test indications and evaluating the results of invasive prenatal tests performed in our clinic in the 2-year period.

Twin reversed arterial perfusion syndrome is mostly seen in monochorionic diamniotic twin pregnancies with an estimated incidence of 1/9500-11000 pregnancies. One of the twins is acardiac with various abnormalities especially with upper part of the body, and mortality is 100%. The other twin functions as a pump twin and mostly has polyhydramnios and heart failure; and mortality rate is high due to prematurity and heart failure. Herein we report a TRAP syndrome and the prognosis of pump twin who was born at 30 weeks gestational age.

PubMedID: 29804024 Objective: Cancer-related inflammation is associated with poor long-term outcomes in various solid tumors. The aim of this study is to investigate the prognostic significance of pre-operative neutrophil lymphocyte ratio (NLR), monocyte count (MC), mean platelet volume (MPV), and platelet lymphocyte ratio (PLR) in endometrial cancer. Study Design: In this study, 763 patients with endometrial cancer were evaluated, who were followed between January 1996 and February 2016. Preoperative values of NLR, MC, MPV, and PLR were evaluated in terms of clinico-pathologic prognostic factors and overall survival (OS). Results: NLR, MC, and PLR were detected to be statistically significant with regard to advanced stage of the disease (p = 0.001, p = 0.02, and p = 0.001, respectively), but only MC was significant in terms of grade (p = 0.035). Higher NLR and PLR values were found to be associated with advanced stage of the disease, deep myometrial invasion, cervical involvement, lymphovascular space invasion (LVSI), and nodal involvement. When the cut-off value was considered as 3, sensitivity and specificity for NLR were found to be 68% and 69%, respectively, to predict lymph node metastasis. NLR was found as a prognostic factor for survival (p = 0.01). Age, the presence of comorbidity, stage, and cervical involvement were determined to be independent prognostic factors for OS in our cohort. Conclusion: NLR was detected to be a prognostic factor for survival. Moreover, NLR and PLR are a predictive value for lymph node involvement and also for cervical invasion in endometrial cancer. © 2018

Objective: Fetal structural malformations affect approximately 2-3% of all pregnancies. Only focusing on trisomy screening in first trimester and deferring the anatomic screening to second trimester may result with late detection of major anomalies that can be diagnosed earlier with careful examination. Material and Methods: This was a descriptive study of retrospective data that were obtained from all terminated single pregnancies due to ultrasonographic findings of major anomalies from 2011 to 2016 in our department. The study was based on a chart review and only abnormalities that were diagnosed before the 16th week were included. Results: Two hundred forty-four first trimester pregnancy terminations were performed. In total, 273 anomalies were detected in the 244 patients. Cranial NTD comprised 32% of all anomalies (n=89). Fifteen percent of anomalies (n=41) needed detailed anatomic scanning for early diagnosis. Conclusion: In this study, we presented the number and percentage of our early diagnosed anomalies by years, as well showed our diagnostic performance for specific anomalies such as atrioventricular septal defect during a 5-year period. The study provides valuable information for future studies in Turkey and shows the need for an anatomic scan protocol while performing aneuploidy screening during early gestation. © 2018 by the Turkish-German Gynecological Education and Research Foundation.

Amaç: Konvansiyonel portla yapilan total laparoskopik histerektomi (TLH) ile Laparoendoskopik transumbilikal tek giriş total histerektominin (LESS--TH) peri ve postoperatif sonuçlarının karşılaştırılması. Gereç ve Yöntemler: Tek merkezde Mart 2013 ve Mart 2014 tarihleri arasmda be-- nign uterin nedenler ve düşük risk faktörlü endometrial kanser vakalarında yapılan LESS--TH (n=24) ile TLH (n=47) olguları retrospektif olarak değerlendirilmiştir. LESS--TH, tek bir 2--3 cm'lik tran-- sumbilikal insizyona uygulanan multi--channel SILS TM port (Covidien®, Mansfield, MA, USA) ile yapılmıştır. Hastaların demografik özellikleri, peri ve postoperatif özellikleri her iki grupta değer-- lendirilmiş ve karşılaştırılmıştır. Bulgular: LESS--TH grubundan bir hastada konvansiyonel laparos-- kopiye, bir vakada ise laparotomiye dönülmüştür. Yaş, beden kitle indeksi (BKİ) gibi demografik özellikler ve endikasyonlar her iki grupta da benzerdir (p=0,497, 0,07, 0,594, sırasıyla). LESS--TH grubunda operasyon süresi TLH grubuna göre belirgin yüksekti (p=0,01). Uterusun eni, boyu, tah-- mini kan kaybı, postoperatif Hb seviyesinde düşme, postoperatif hastanede kahş süresi, analjezik te-- davisi gibi diğer peri ve postoperatif özellikler her iki grup arasında benzerdi. Her iki grupta da major komplikasyonumuz olmadı. Sonuç: Operasyon süresi dışında LESS--TH'in cerrahi sonuçları, konvan-- siyonel multiport TLH' ye benzerdir. Objective: To compare the peri— and post—operative outcomes between the laparoen— doscopic transumblical single—site total hysterectomy (LESS—TH) and conventional three—port total laparoscopic hysterectomy (TLH). Material and Methods: single institution retrospective review of patients operated on with LESS-TH (n= 24) and TLH (n=47) for benign uterine pathology or en— dometrial cancer with low—risk factors from March 2013 to March 2014. Patients underwent LESS— TH surgery through single 2—3 cm transumbilical incision with multi—channel SILS TM port (Covidien®, Mansfield, MA, USA). Patient demographics and peri-operative and post—operative variables were analyzed and compared between the groups. Results: There was one conversion to conventional laparoscopy and one conversion to laparotomy in the LESS—TH group, and no cases converted to laparotomy in the LTH group. Demographic characteristics such as age, body mass index (BMI), and indications were similar between the groups (p=0.497, 0.07, 0.594, respectively). Operation time was significantly longer in the LESS—TH group than in the TLH group (p=0.01). The other peri— and post—operative variables such as uterus length and width, estimated blood loss, post—operative Hb level drop, post-operative hospitalization time and analgesic management were found to be similar between the two groups. There were no major complications in either of the groups. Conclusion: With the exception of operative time, the surgical outcomes of the LESS—TH group were comparable to those of the conventional multiport TLH group.
