The participants were 91 patients with pressure ulcers in the Home Health Care Unit and 91 caregivers. The data collection tool, consisted of questions regarding sociodemographic data prepared by researchers and questioning the knowledge of caregivers’ knowledge about pressure ulcer. The Barthel Index, Pressure Ulcer Scale for Healing, Braden Risk Assessment Scale, and Care Assessment Inventory provided by family members were used to assess the level of dependence in patients’ daily living activities.Results: The patients’ average age was 77.21± 13.26 years and 58.2% of them were women. The caregivers’ average age was 53.32± 11.45 years and 83.5% of them were women. A statistically significant increase was observed between knowledge scores before and after training (p< 0.001). Indicating that the training positively affects the caregivers’ level of knowledge. Factors such as a private room, caregiver support, caregiver competence, and caregiving difficulties were significantly related to changes in knowledge scores.Conclusion: Training programs for caregivers on pressure ulcer handling in home health care patients are effective and should be planned from a broader perspective to include caregivers within the family.Keywords: home care services, pressure sore, patient care, health education

Background: Emerging carbapenem-resistant Klebsiella pneumoniae (K. pneumoniae) (CRKP) bacteremias are presenting significant public health risks due to limited treatment options and increased mortality. K. pneumoniae isolates exhibit carbapenem resistance rates that vary from 25% to 50% throughout the European continent, including our country. Aims: To assess the characteristics of CRKP bacteremia, a condition that has recently demonstrated an increasing prevalence in our center. We sought to ascertain the resistance rates of isolated strains to antibiotics other than carbapenems, identify the responsible carbapenemase genes, evaluate the efficacy of antibiotics, determine mortality rates, explore clonality among strains, and investigate the influence of the COVID-19 pandemic on all these factors. Study Design: Retrospective observational study. Methods: This study included patients aged 18 and older who had experienced meropenem-resistant K. pneumoniae bacteremia. Meropenem resistance was confirmed by employing the Kirby-Bauer disk diffusion method. Meropenem minimum inhibitory concentration (MIC) levels were determined using the gradient test, while colistin MIC levels were ascertained using the disk elution technique. Carbapenemase genes were evaluated via colony polymerase chain reaction (PCR), and clonality analysis was performed using the arbitrarily primed PCR technique. Results: The study comprised 230 patients, with a mean age of 63.1 ± 15.9 years, of whom 58.7% were male. Oxacillinase-48 (OXA-48) was detected in 74.8% of the patients, New Delhi metallo-beta-lactamase (NDM) in 12.6%, OXA-48 + NDM in 7.8%, and KPC in 4.8%. The 14-day and 30-day mortality rates were 57% and 69.6%, respectively. Multivariate analysis of the 30-day mortality revealed several crucial factors, including bacteremia development in the intensive care unit, the occurrence of bacteremia during the COVID-19 pandemic, polymicrobial bacteremia, the use of indwelling intravenous catheters, a platelet count of ≤ 140,000/µl, procalcitonin levels of ≥ 6 µg/l, and a Charlson comorbidity score ≥ 3. Notably, the OXA-48 and KPC genes were upregulated significantly during the COVID-19 pandemic, while the NDM gene groups were downregulated. Additionally, both 14-day and 30-day mortality rates increased significantly. Conclusion: In this study, the most prevalent carbapenemase gene was OXA-48; however, there has been a recent increase in KPC genes. No dominant epidemic strain was identified through clonality analysis. The clustering rate was 68% before the pandemic, increasing to 85.7% during the pandemic. The significance of infection control measures is underscored by the rise in both clustering and mortality rates during the COVID-19 pandemic.

With the intensifying challenge of global warming driven largely by anthropogenic activities, effective greenhouse gas capture techniques are critical. This paper focuses on the role of deep eutectic solvents (DES) as promising agents for such capture at the source. We review the key DES-based methods for greenhouse gas capture, drawing conclusions from a thorough analysis of the existing literature. In particular, we examine the effect of DES structure on gas solubilities and explore the mechanism of gas solubility in DES through molecular simulation. We present a synthesis of state-of-the-art results in this area, assessing the potential of DES as an alternative to current industrial gas capture methods. Furthermore, we propose future research directions for the design of novel DES tailored to more specific applications.

Abstract Switching of light polarization on the sub-picosecond timescale is a crucial functionality for applications in a variety of contexts, including telecommunications, biology and chemistry. The ability to control polarization at ultrafast speed would pave the way for the development of unprecedented free-space optical links and of novel techniques for probing dynamical processes in complex systems, as chiral molecules. Such high switching speeds can only be reached with an all-optical paradigm, i.e., engineering active platforms capable of controlling light polarization via ultrashort laser pulses. Here we demonstrate giant modulation of dichroism and birefringence in an all-dielectric metasurface, achieved at low fluences of the optical control beam. This performance, which leverages the many degrees of freedom offered by all-dielectric active metasurfaces, is obtained by combining a high-quality factor nonlocal resonance with the giant third-order optical nonlinearity dictated by photogenerated hot carriers at the semiconductor band edge.

Abstract Background The increasing prevalence of depression among older adults is a growing concern. Chronic health conditions, cognitive impairments, and hospitalizations amplify emotional distress and depression levels in this population. Assessing the quality of life is crucial for the well-being of older adults. Aims Our study aimed to examine how comorbidities affect depression and quality of life in geriatric patients in both outpatient and hospital settings. Methods 100 patients (50 from internal medicine outpatient clinic and 50 from internal medicine ward) were included in the study according to inclusion and exclusion criteria. Patients were classified into different age groups (65–74 years, 75–84 years and ≥ 85 years). Data on patients’ location of application, age, sex, living alone or with family status, number of comorbid diseases, types of accompanying diseases were recorded and WHOQOL-OLD and Geriatric Depression Scale (GDS) questionnaires were administered. Results were evaluated using SPSS. Results The WHOQOL-OLD questionnaire score was higher in the 65–74 age group compared to other groups, but there was no significant difference between outpatient group and hospitalized group. Patients with comorbid diseases had lower WHOQOL-OLD questionnaire scores compared to those without comorbid diseases. In the 75–84 and ≥ 85 age groups, the GDS scores were higher compared to the 65–74 age group. In hospitalized group, GDS scores were higher than outpatient clinic group. In patients with comorbid diseases, GDS scores were higher than the ones without comorbid diseases. Discussion Our findings indicate that quality of life is higher among those aged 65–74, with lower incidence of depression compared to other age groups. Hospitalization correlates with higher depression rates but not quality of life. As number of comorbid diseases increases in older adults, the frequency of depression rises and the quality of life declines. Conclusions Early detection and intervention for depression are crucial for enhancing older adults’ well-being.

Abstract Study design Retrospective case series. Objectives We aimed to describe with a novel surgical approach for the removal of posterior thoracolumbar implant in patients with symptomatic failure of the implant and present our preliminary results with this method. Methods This retrospective, single-center study was performed in the neurosurgery department of a university hospital. Data were gathered from the medical files of 314 patients (243 women, 77.39%; 71 men, 22.61%) with symptomatic thoracolumbar implant failure that underwent implant removal operation using our novel technique between 2010 and 2020. Symptoms, radiological findings, intraoperative findings as well as clinical outcomes were evaluated. Results In our series, the average age was 46.5 years (range: 21–84) with a mean follow-up duration of 7 years (range: 3 months to 10 years). Preoperatively, the most common symptoms were leg pain and numbness of the lower extremity. Postoperatively, no major complications were noted. Clinical progression of symptoms was avoided by surgery in all patients, while we came across removal difficulties due to screw–screwdriver mismatch in 15 of 314 surgeries (4.78%). Our novel approach allowed successful screw removal including these challenging cases. Conclusions We suggest that our novel approach is a practical and effective for the removal of posterior thoracolumbar implant in cases with symptomatic failure attributed to screw–screwdriver mismatch. Further trials are warranted to assess the efficacy of this technique to overcome surgical problems associated with screw removal.

This study explores the synthesis and characterization of photochromic Polyvinylidenefluoride/Polyvinylpyrrolidone (PVDF/PVP)-based membranes, prepared through an in situ thiol-ene click reaction by incorporating viologen derivatives with different counter ions. Viologens are well-known for their light-sensitive properties and ability to change color, making them useful in various optoelectronic applications. The membranes developed in this study exhibit significant improvements in their interactions with light as a result of improved morphology and enhanced ionic conductivity (≈4 × 10−4 S cm–1) with higher porosity (Ra: 11.26–33.76 nm) compared to conventionally prepared membranes. These membranes show the ability to block almost all ultraviolet (UV) and a 90% of visible light after irradiation. Thanks to these properties, the membranes undergo visible color changes when exposed to sunlight, making them suitable for photochromic and thermochromic applications. The findings of this study could contribute to the development of innovative coating materials that enhance energy efficiency, potentially being applied to buildings, automotive windows, and other surfaces.

Digital games require high levels of attention, strategy, reflexes, and coordination, offering pathways to reach the pinnacle of competition. The benefits derived from digital games enhance the ability to perform effectively under stress, making them crucial for both physical and mental performance. This study investigates the effects of digital gaming on the physical performance, shoulder flexibility, muscle strength and reaction speed of female volleyball players. Abstract Background Although research on the mechanism and control of pain and inflammation in fish has increased in recent years, the use of analgesic drugs is limited due to the lack of pharmacological information about analgesic drugs. Tolfenamic acid is a non‐steroidal anti‐inflammatory drug and can be used in fish due to its low side effect profile and superior pharmacokinetic properties. Objectives The pharmacokinetics, bioavailability and plasma protein binding of tolfenamic acid were investigated following single intravascular (IV), intramuscular (IM) and oral administration of 2 mg/kg in rainbow trout at 13 ± 0.5°C. Methods The experiment was carried out on a total of 234 rainbow trout (Oncorhynchus mykiss). Tolfenamic acid was administered to fish via IV, IM and oral route at a dose of 2 mg/kg. Blood samples were taken at 13 different sampling times until the 72 h after drug administration. The plasma concentrations of tolfenamic acid were quantified using high pressure liquid chromatography–ultraviolet (UV) and pharmacokinetic parameters were assessed using non‐compartmental analysis. Results The elimination half‐life (t1/2ʎz) of tolfenamic acid for IV, IM and oral routes was 3.47, 6.75 and 9.19 h, respectively. For the IV route, the volume of distribution at a steady state and total body clearance of tolfenamic acid were 0.09 L/kg and 0.03 L/h/kg, respectively. The peak plasma concentration and bioavailability for IM and oral administration were 8.82 and 1.24 µg/mL, and 78.45% and 21.48%, respectively. The mean plasma protein binding ratio of tolfenamic acid in rainbow trout was 99.48% and was not concentration dependent. Conclusions While IM route, which exhibits both the high plasma concentration and bioavailability, can be used in rainbow trout, oral route is not recommended due to low plasma concentration and bioavailability. However, there is a need to demonstrate the pharmacodynamic activity of tolfenamic acid in rainbow trout.

Cycling has seen a remarkable rise, signifying a paradigmatic move towards sustainable, eco-friendly, and efficient commuting alternatives in the contemporary urban setting. Cities also encourage this trend by establishing cycle lanes, bike-sharing programs, and incentives for frequent riders. To enhance these motivations from an urbanistic perspective, it is essential to comprehend the influence of urban characteristics on cycling volume and to incorporate this understanding into data-driven decision-making processes.This research examines the Bicification project data from Istanbul with a spatial perspective. Utilising a comprehensive array of spatial big data, the study explores the impact of urban land use, transport services, land morphology, and sociodemographic factors on cycling volume through a Multi-scale Geographically Weighted Regression (MGWR). With an Adj R2 value of 0.68, the model demonstrates a strong relation between cycling volume and several factors, including biking park stations, park and ride points, pier stops, rail stops, transfer points, main roads, elevation, population, industrial facilities, health facilities, sports areas, and residential areas. The findings will serve to develop a data-driven strategic approach to promote cycling in Istanbul.

Introduction:Different anesthesia techniques were reported for the management of pilonidal sinus surgery, including local infiltration anesthesia (LIA), epidural anesthesia, spinal anesthesia (SA), and general anesthesia. However, even up to this date, the best anesthetic technique remains controversial. Here; we compared LIA SA for postoperative pain levels after pilonidal sinus disease surgery.Methods:We conducted a single-center prospective observational clinical trial between May 2022 and January 2023 in our tertiary education hospital. A total of 60 patients with a diagnosis of pilonidal sinus were included in the study Postoperative 0-hour, 1 hour, 8 hours, and 24-hour numeric rating scores (NRS) of the patients were evaluated. Results were evaluated using Statistical Package for the Social Sciences.Results:While 30 (50%) patients were operated under local anesthesia, 30 (50%) patients were operated under SA. 55 (91.7%) patients were male, 5 (8.3%) patients were female. The mean age of the patients operated under local anesthesia was 29.68±6.85 years, while the mean age of the patients operated under SA was 26.23±7.04 years. There was no statistically significant difference in age between the groups. When the comparison between postoperative NRS scores of SA and LIA was evaluated, there was no significant difference at postoperative 0 h and postoperative 1st hour. However, LIA NRS scores at 8th and 24th h were lower than SA scores. Recurrence was observed in 3 patients from both groups in the 3rd month outpatient controls. The difference between the groups was not statistically significant. All patients were discharged postoperatively on the 1st day of the surgery.Conclusion:As a result, we may suggest using LIA for pilonidal surgery for the anesthetic technique.

Graphene has become a prominent material in cancer research in recent years. Graphene and its derivatives also attract attention as carriers in drug delivery systems. In this study, we designed a graphene oxide (GO)-based methotrexate (MTX)-loaded and folic acid (FA)-linked drug delivery system. MTX and FA were bound to GO synthesized from graphite. MTX/FA/GO drug delivery system and system components were characterized using Fourier transform infrared spectroscopy (FTIR), differential calorimetric analysis (DSC), scanning electron microscopy (SEM), transmission electron microscopy (TEM), zeta potential analysis, and dimension measurement (DLS) studies. SEM and TEM images confirmed the nanosheet structure of GO synthesized from graphite, and it was shown that MTX/FA binding to GO transformed the two-dimensional GO into a three-dimensional structure. FTIR and DSC graphs confirmed that oxygen atoms were bound to GO with the formation of carboxylic, hydroxyl, epoxide, and carbonyl groups as a result of the oxidation of graphite, and GO was successfully synthesized. Additionally, these analyses showed that MTX and FA bind physicochemically to the structure of GO. The in vitro Franz diffusion test was performed as a release kinetic test. The release kinetics mathematical model and correlation coefficient (R2) of MTX-loaded GO/FA nanomaterials were found to be the Higuchi model and 0.9785, respectively. Stiffness analyses showed that adding FA to this release system facilitated the entry of the drug into the cell by directing the system to target cells. As a result of the stiffness analyses, the stiffness values of the control cell group, free MTX, and MTX/FA/GO applied cells were measured as 2.34 kPa, 1.87 kPa, and 1.56 kPa, respectively. According to these results, it was seen that MTX/FA/GO weakened the cancer cells. Combined use of the MTX/FA/GO drug delivery system had a higher cytotoxic effect than free MTX on the MDA-MB-231 breast cancer cell line. The results showed that the synthesized MTX/FA/GO material has promising potential in cancer cell-specific targeted therapy for MTX as a drug delivery system.

Introduction:To investigate fungal species in nasal polyps (NP) by microscopy, fungal culture, fungal DNA isolation, and sequencing.Methods:Twenty-four patients who applied to our outpatient clinic with complaints of chronic sinusitis and were found to have bilateral NP on clinical examination were included in our study. A control group was formed from 20 patients without NP who underwent septoplasty and endoscopic concha bullosa resection in our clinic. Samples from the participants were subjected to the same microbiological evaluations and the two groups were compared.Results:The mean age of the patients included in our study was 38.14±14.13 years (range from: 17 to 80). Nine of the participants were female and 35 were male. Direct microscopy and fungal culture positivity rates did not significantly differ between the groups (p>0.05). A significant (p0.05).Conclusion:We concluded that fungal colonization is not more frequent in patients with NP than in the normal population. We did not observe the superiority of PCR-based sequencing over conventional fungal isolation techniques. However, larger series using molecular methods are needed.

İşsizlik oranı, ülke ekonomilerinin durumlarını analiz etmede kullanılan önemli ve kritik göstergelerden birisidir. Histeri ise işsizliğe yönelik şokların kalıcı etkileri olduğunu belirten bir kavramdır. Bunun için geçici şokların işsizlik üzerindeki etkisi İşsizlik Histerisi Hipotezi ile analiz edilmektedir. Literatürde işsizlik histerisinin varlığı sıklıkla araştırılan konular arasında yer almaktadır. Bu çalışmanın literatürde az rastlanan değişkenleri kullanması ve uygulama metodu açısından literatüre katkı sağlayacağı düşünülmektedir. Çalışmada Türkiye’de 1988-2020 yılları arasındaki işsizlik verileri sadece toplam işsizlik için değerlendirilmemiş olup, kadın, erkek ve toplam işsizlik için ayrı ayrı değerlendirilmiştir. Literatürde çok fazla araştırma konusu olamayan bu değişkenler Genişletilmiş Dickey Fuller Testi, Phillips Perron Birim Kök Testi, Zivot Andrews Yapısal Kırılmalı Birim Kök Testi, Lumsdaine Papell Yapısal Kırılmalı Birim Kök Testi ve Lee Strazicich Yapısal Kırılmalı Birim Kök Testi kullanılarak ayrı ayrı analiz edilmiştir. Birden fazla birim kök testi ile yapılan çalışmanın sonucunda birim kök testlerinin çoğunda belirlenen dönem için Türkiye’de kadın, erkek ve toplam işsizlik üzerinde işsizlik histerisi hipotezinin geçerli olduğu görülmektedir.

Abstract Early detection of highly infectious respiratory diseases, such as COVID-19, can help curb their transmission. Consequently, there is demand for easy-to-use population-based screening tools, such as mobile health applications. Here, we describe a proof-of-concept development of a machine learning classifier for the prediction of a symptomatic respiratory disease, such as COVID-19, using smartphone-collected vital sign measurements. The Fenland App study followed 2199 UK participants that provided measurements of blood oxygen saturation, body temperature, and resting heart rate. Total of 77 positive and 6339 negative SARS-CoV-2 PCR tests were recorded. An optimal classifier to identify these positive cases was selected using an automated hyperparameter optimisation. The optimised model achieved an ROC AUC of 0.695 ± 0.045. The data collection window for determining each participant’s vital sign baseline was increased from 4 to 8 or 12 weeks with no significant difference in model performance (F(2) = 0.80, p = 0.472). We demonstrate that 4 weeks of intermittently collected vital sign measurements could be used to predict SARS-CoV-2 PCR positivity, with applicability to other diseases causing similar vital sign changes. This is the first example of an accessible, smartphone-based remote monitoring tool deployable in a public health setting to screen for potential infections.

Objective:It was aimed to report the Acute Physiology and Chronic Health Evaluation-II (APACHE-II) score, Sequential Organ Failure Assessment (SOFA) score, Glasgow coma scale (GCS), 4C mortality score and the coronavirus disease-2019 (COVID-19) Reporting and Data System (CO-RADS) in predicting the outcome of critically ill COVID-19 patients.Materials and Methods:Patients with laboratory-confirmed COVID-19 infection or clinical and radiological confirmed COVID-19 infection who were admitted to adult intensive care unit (ICU) were included. Clinical characteristics, outcomes, APACHE-II score, SOFA score, International Severe Acute Respiratory and Emerging Infections Consortium/World Health Organization 4C mortality score and CO-RADS classification were reported at admission.Results:Two hundred seventy six patients were included in this study. The mean age was higher in non-survivor patients. The most common cause of hospitalization was respiratory failure (67%). The common co-morbidities were hypertension (51.8%), cardiac disease (43.4%) and diabetes (33.6%). Organ failure was present in 61.5% of the patients. The mean APACHE-II, SOFA, GCS and 4C mortality scores were higher in non-survivor patients. 4C mortality and SOFA scores showed higher predictive accuracy for mortality with an area under the curve 0.736 and 0.706, respectively. 4C mortality had sensitivity of 78.9% and specificity of 58.1% whereas of SOFA had a sensitivity of 78.9% and a specificity of 53.3%.Conclusion:4C mortality and SOFA scores could be a predictors of mortality in COVID-19 patients in the ICU.

Objective:Mayo Adhesive Probability score (MAP score) is a nephrometry system to predict surgical difficulty using radiologic image-based measurements and interpretations. MAP score is based on two main factors: Perinephric fat thickness at the level of the renal vein and perinephric fat stranding, which was defined as a linear area of soft tissue attenuation in the perinephric space. This study evaluated the efficacy of the MAP score on intraoperative parameters of laparoscopic donor nefrectomy and bench surgery.Materials and Methods:Four hundred twenty-one laparoscopic live-donor nephrectomies (LDN) and subsequent bench surgeries carried out between 2016 and 2022 have been included in this study. Preoperative computerized tomography images of donors were blindly scored for determination of MAP scores. Sex, age, hypertension, cigarette smoking, dyslipidemia, and body mass index (BMI) were evaluated as risk factors for high MAPS.Results:In females and males, the percentage of donors in the high MAPS group was 11.79% and 25.32%, respectively, and the difference between the two groups is statistically significant. Abstract Chemical modifications of porous materials almost always result in loss of structural integrity, porosity, solubility, or stability. Previous attempts, so far, have not allowed any promising trend to unravel, perhaps because of the complexity of porous network frameworks. But the soluble porous polymers, the polymers of intrinsic microporosity, provide an excellent platform to develop a universal strategy for effective modification of functional groups for current demands in advanced applications. Here, we report complete transformation of PIM-1 nitriles into four previously inaccessible functional groups – ketones, alcohols, imines, and hydrazones – in a single step using volatile reagents and through a counter-intuitive non-solvent approach that enables surface area preservation. The modifications are simple, scalable, reproducible, and give record surface areas for modified PIM-1s despite at times having to pass up to two consecutive post-synthetic transformations. This unconventional dual-mode strategy offers valuable directions for chemical modification of porous materials.

Abstract Background The periodontal status and distal bone level of the adjacent second molar can be negatively affected by the surgical extraction of an impacted lower third molar. Absorbable materials have some benefits, including enhancing primary wound coverage and promoting wound healing through isolation, clotting, wound stabilization and haemostasis. This study set out to compare primary and secondary healing and collagen-membrane-based primary healing after surgical removal of partially erupted impacted third molars (3Ms), evaluating the distal alveolar bone level (ABL) and periodontal status of the adjacent second molars (2Ms). Methods Patients who met the inclusion criteria were randomized into three groups: secondary healing (n = 28), primary healing (n = 27) and membrane-based primary healing (n = 29). Digital panoramic radiographs were obtained preoperatively (T1) and three months postoperatively (T2). Herein, the synthesis and characterization methods of natural deep eutectic solvents based on monoterpenoids have been presented. Low viscous fluids with suitable physicochemical properties are produced. The materials are non-toxic, biodegradable, and cost-effective. Thus, they can be used to develop sustainable solvents for various processes and can find their applications in various fields. A theoretical study based on quantum chemistry and classical molecular dynamics is used for the nanoscopic characterization of structure, dynamics, and hydrogen bonding. The reported results help analyze the properties of this new family of solvents. The required information for developing structure–property relationships for proper solvent design to form a sustainable chemistry framework is obtained.

Tympanic membrane (TM) perforations, primarily induced by middle ear infections, the introduction of foreign objects into the ear, and acoustic trauma, lead to hearing abnormalities and ear infections. We describe the design and fabrication of a novel composite patch containing photocrosslinkable gelatin methacryloyl (GelMA) and keratin methacryloyl (KerMA) hydrogels. GelMA-KerMA patches containing conical microneedles in their design were developed using the digital light processing (DLP) 3D printing approach. Following this, the patches were biofunctionalized by applying a coaxial coating with PVA nanoparticles loaded with gentamicin (GEN) and fibroblast growth factor (FGF-2) with the Electrohydrodynamic Atomization (EHDA) method. The developed nanoparticle-coated 3D-printed patches were evaluated in terms of their chemical, morphological, mechanical, swelling, and degradation behavior. In addition, the GEN and FGF-2 release profiles, antimicrobial properties, and biocompatibility of the patches were examined in vitro. The morphological assessment verified the successful fabrication and nanoparticle coating of the 3D-printed GelMA-KerMA patches. The outcomes of antibacterial tests demonstrated that GEN@PVA/GelMA-KerMA patches exhibited substantial antibacterial efficacy against Staphylococcus aureus, Pseudomonas aeruginosa, and Escherichia coli. Furthermore, cell culture studies revealed that GelMA-KerMA patches were biocompatible with human adipose-derived mesenchymal stem cells (hADMSC) and supported cell attachment and proliferation without any cytotoxicity. These findings indicated that biofunctional 3D-printed GelMA-KerMA patches have the potential to be a promising therapeutic approach for addressing TM perforations.

Purpose: The aim of this paper is to test the hypothesis that regional innovativeness is positively correlated with the two main building variables of smart specialization: relatedness density (a region’s potential to develop new technologies compatible with existing capabilities) and knowledge complexity (a region’s potential to develop unique and hard-to-imitate technologies).Methodology: Analysis are made over a panel regression model, using data from OECD-REGPAT database for the years 1978–2017 covering Türkiye’s NUTS-3 regions. The dependent variable is innovativeness, and the independent variables are the average relatedness density and knowledge complexity indices for each region. Per capita income, number of universities, number of technoparks, Turkish Patent Institute, Türkiye-EU Customs Union agreement, and lagged value of the dependent variable are the control variables.Findings: The results confirm that innovativeness and a region’s potential to develop new technologies that are compatible with existing technological portfolios (relatedness density) are correlated, while the variable that measures a region’s potential to develop unique technologies (knowledge complexity) is not statistically significant.Originality: The originality of this work is that we demonstrate relatedness to be one of the building blocks of smart specialization as correlated with regional innovativeness; complexity, the other building block, is not correlated.

The end of 2019 marked the beginning of a pandemic that shook of the whole world. This newly emerged viral agent was quickly identified with the help of molecular techniques and classified as a novel coronavirus severe acute respiratory syndrome-coronavirus-2. In the first part of this article, the general virological features and pathogenesis of this viral agent, which caused many deaths at the beginning and changed in the pathogenesis with different variants over time, were discussed. In the second part of the article, information is given about the diagnostic tests applied in pandemic conditions.

Abstract We aimed to describe the increased rate of Acinetobacter baumannii infections during the COVID-19 pandemic and define its significance within the last five years. This study was performed in a tertiary hospital with 280 beds and included all patients infected with A. baumannii in the intensive care unit between January 1, 2018, and June 30, 2022. A. baumannii-infected patients in the intensive care unit 27 months before the pandemic and 27 months during the pandemic were included. Pulsed-field gel electrophoresis was performed to assess clonal relatedness. The infection control measures were specified based on the findings and targeted elimination. In total, 5718 patients were admitted to the intensive care unit from January 1st, 2018, to June 30th, 2022. A. baumannii infection was detected in 81 patients. Compared to the pre-pandemic era, the rate of A. baumannii infection during the pandemic was 1.90 times higher (OR: 1.90, 95% CI: [1.197, 3.033]). Clonality assessment of multidrug-resistant A. baumannii samples revealed eight clusters with one main cluster comprising 14/27 isolates between 2021 and 2022. The case fatality rate of the pre-pandemic and pandemic era was not different statistically (83.33% vs. 81.48%, p = 0.835). Univariate analysis revealed the association of mechanical ventilation (p = 0.002) and bacterial growth in tracheal aspirate (p = 0.001) with fatality. During the COVID-19 pandemic, potential deficits in infection control measures may lead to persistent nosocomial outbreaks. In this study, the introduction of enhanced and customized infection control measures has resulted in the containment of an A. baumannii outbreak.

In soft robotics, a recent challenge is to decrease the number of rigid components used tocreate entirely soft robots. A common rigid component used in soft robots is the rigid encoder, which should be replaced with a soft counterpart if possible. In this work, we de-sign and manufacture a soft sensor, which is embedded into a C-shaped leg of a soft, legged, miniature robot. Our main goal is to show that we can embed a soft sensor into and receive contact feedback from a soft C-shaped leg of our soft miniature quadruped. We test various sensor parameters using custom test setups to analyze the soft sensor performance. Our soft sensor design is iterated by experimentally investigating several sensor shape options. For the C-leg of the soft miniature quadruped, optimal sensor geometry and position for the sensor implementation are found from a discrete design space as the outcome of this work. We received feedback from the soft sensor and compared commercial encoder data to the soft sensor embedded C-leg data. We managed to detect the rotation speed of the C-leg with the accuracy of 87.5% on a treadmill and with the accuracy of %86.7 under free rotation of the C-leg. However, if connection loss occurs in the miniature slipring mechanism, the error percentage in estimating the rotational speed increases significantly.

Objective: It was aimed to investigate the relationship of stereoacuity with retinal nerve fiber layer thickness (RNFLT) and P100 latency in multiple sclerosis (MS) patients with and without optic neuritis (ON). Materials and Methods: Twenty-nine patients diagnosed with clinically definite MS with and without a history of ON were included in this prospective study. Patients without ON were classified into relapsing-remitting MS (RRMS) and single-attack MS (SAMS) subgroups. There were 11 patients in the RRMS group with ON (ON-RRMS), 11 patients with RRMS and 7 patients with SAMS in the MS group without ON, and 16 healthy subjects in the control group. Stereoacuity was determined by the TNO and Titmus tests. RNFLT was measured by spectral domain-optical coherence tomography and P100 latency was measured in pattern visual evoked potential recordings. Objective:The coronavirus disease-2019 (COVID-19) pandemic effect diagnosis and treatment of certain conditions, including bladder cancer (BC). This study aimed to evaluate the effects of the COVID-19 pandemic on BC diagnosis and treatment.Materials and Methods:Following the approval of the ethics committee for the study, data of 869 patients who underwent surgery for BC in the 2-year period between March 1, 2019 and February 28, 2021 were analyzed retrospectively. The number of surgeries performed for BC, the time elapsed between symptoms and diagnosis, the treatments performed, and the operative pathologies were compared before and during the COVID-19 pandemic.Results:During the COVID-19 period, there was a decrease in the total number of BC surgeries compared to the pre-COVID-19 period (p=0.004). It was observed that this decrease was due to a decrease in patients newly diagnosed with BC (p=0.001) as well as the decrease in the number of primary transurethral resection for bladder tumor procedures performed. There was no difference in the tumor stages of the patients at diagnosis (p=0.9). Intracavitary Bacillus Calmette-Guérin therapy use in high-risk non-muscle invasive bladder cancers (NMIBC) patients also decreased (p=0.008) during the pandemic period. Günümüzün değişen iktisadi ve sosyal yapısı içerisinde rekabet avantajı elde etmek amacıyla işletmeler farklı metotlar incelemeye ve bu metotları kullanmaya başlamışlardır. Bu amaç doğrultusunda, işletmelerin başarıya ulaşabilmesi için kaynaklarını etkin ve verimli kullanması gerekmektedir. Bu noktada, işletmelerin verimliliklerini devam ettirebilmeleri için lojistik bilişim sistemlerine geçmeleri artık bir zorunluluk haline gelmiştir. Bu açıdan, araştırmanın temel konusu olan e-lojistik uygulamalarında e-hizmet kalitesinin, alışveriş memnuniyeti, yeniden satın alma niyeti ve ödeme seçenekleri üzerindeki etkisi bir model oluşturularak ölçülmeye çalışılmıştır. Çalışmanın ilk aşamasında; çalışmanın örnekleme tekniği ve veri toplama yöntemleri ile ilgili bir araştırma metodolojisi sunulmaktadır. Çalışmanın metodolojisi oluşturulduktan sonra araştırma ile ilgili istatistiksel analizler sunulmuş, teorik ve yönetsel çıkarımlar tartışılmıştır. Ayrıca çalışma kapsamında oluşturulan model, yapısal eşitlik modeli ile analiz edilmiş ve oluşturulan modelin geçerli bir model olduğu kanıtlanmaya çalışılmıştır. Modelin yapısal olarak uyumlu olduğu, veriler neticesinde anlaşıldıktan sonra çalışma bünyesinde oluşturulan hipotezler test edilmiştir. Çalışma kapsamında elde edilen hipotez sonuçları ve bulgulara göre; e-lojistik uygulamalarında hizmet kalitesinin alışveriş memnuniyetini olumlu ve anlamlı bir şekilde etkilediği tespit edilmiştir. Ayrıca alışveriş memnuniyeti ölçeği, yeniden satın alma ölçeğini olumlu ve anlamlı bir şekilde etkilemektedir. Buradan tüketicilerin gözünde hizmet kalitesinin artmasının alışveriş memnuniyetini beraberinde getirdiğini ve bunun sonucunda tüketicilerin ürünü veya hizmeti tekrar satın alma eğiliminde olduğunu varsaymak mümkündür.

Kentsel mekanlar bireylerin konforlu bir yaşam sürdürebilmeleri için elverişli boşluklardır. Konfor ise biyolojik, fizyolojik, psikolojik ve toplumsal ihtiyaçların karşılanmasıyla oluşan bir durumdur. Bir mekânın konforlu olması yanında algılanabilir ve yaşanabilir olması da büyük önem taşımaktadır. Algılama duyu organlarının varlığı sayesinde gerçekleşirken en büyük görevi ise görme olayını gerçekleştiren gözdür. Görsel algılamanın gerçekleşebilmesi için ışık ve renk en önemli iki faktördür. Işık yansımayla birlikte renkleri ortaya çıkararak tasarımı nesnelleştir. Renklerin algılanması ışık kaynaklarına bağlıdır ve bu nedenle farklı mekanlar için eylem özelliklerine uygun renkler ve ışık kaynakları seçilir. Yapılan bu çalışmada, ışık ve renk kullanımının mekânın görsel algılanmasına etkileri incelenmiş, ayrıca değişik ışık ve renk koşullarında insanlar üzerindeki algısal etkileri ortaya konulması amaçlanmıştır. Söz konusu amaçlara ulaşmak için ışık, renk, mekân algılama ve birbirleri ile ilgili ilişkileri açıklanmış ve elde edilen bilgiler ışığında Antalya kenti için büyük bir öneme sahip Cumhuriyet Meydanı çalışma alanı seçilerek, konuyla ilgili irdelemeler yapılmıştır.

Sıkılık ve esneklik sosyal psikolojide göreli olarak daha yakın ele alınmış bir kavram olarak öne çıkmaktadır. Kültürel sıkılık/esneklik boyutu toplulukları normların gücü ve normlara uymayanlara uygulanan yaptırımlar yönünden ayırmaktadır (Gelfand vd., 2011). Normlara dayalı bir yaklaşım, bireylerin sosyal çevrelerindeki paylaşılan beklentilerin yaratıcı davranışlarını ve performanslarını nasıl şekillendirdiğine odaklanır (Chua, 2014). Bu araştırmada sıkılık ve esneklik kültürel değerleri örgüt kültürünü irdelemek için kullanılmış ve Borsa İstanbul’a kote imalat ile hizmet şirketlerinde sıkılık/esnekliğin nasıl farklılaştığı incelenmiştir. Sonuçlar, müşteri ile doğrudan iletişim halinde olunan hizmet sektöründe disiplinin yani kuralların daha öne çıktığı (Singh, 1988), bu işletmelerin daha sıkı kültürlere sahip olduğu (Mattila ve Patterson, 2004a, 2004b ), imalat işletmelerinde ise sıkılığın daha düşük olduğu yönünde bulgulanmıştır

Nanofibers loaded with pharmaceutical agents for various medical purposes have become more important in recent years because of their advantages, such as control on release, gas permeability, high surface area, and lightweight matrices. In the present study, polylactic acid (PLA)-gelatin (Gel) nanofibers were successfully loaded with Ibuprofen-Na/Paracetamol (henceforth Ibu-Na and Par, respectively) by electrospinning. The nanofibers were characterized by scanning electron microscopy (SEM) and Fourier transform infrared (FTIR) spectroscopy. The Ibu-Na/Par content of the nanofibers was determined by using high-performance liquid chromatography (HPLC). Their antibacterial activities were tested against Enterococcus faecalis ATCC 29212, Staphylococcus aureus ATCC 25923, Escherichia coli ATCC 25922, and Pseudomonas aeruginosa PA01, opportunistic pathogenic bacteria which are frequently associated with infections. Moreover, their antibiofilm activities against P. aeruginosa and E. faecalis were also investigated. The Ibu-Na-containing nanofibers exhibited antibacterial activity against S. aureus, E. coli, and E. faecalis. The inhibition zone diameters of PLA-Gel-Ibu-Na 300 against E. faecalis, S. aureus, and E. coli were calculated to be 23.0 ± 2.1 mm, 18.0 ± 1.5 mm, and 12.0 ± 1.2 mm, respectively. It was found that PLA-Gel-Ibu-Na 300 and PLA-Gel-Par 300 nanofibers’ capacity to show biofilm formation inhibition originated remarkable effects on P. aeruginosa, which were found to be 48 % and 50.4 %, respectively. This study indicated that Ibu-Na/Par-loaded nanofibers are promising materials for wound healing applications.

Background: Monitoring the longevity of immunoglobulin G (IgG) responses following severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infections is vital to understanding the role of antibodies in preventing infection. Aims: To determine the quantitative IgG responses specific to the Spike-S1 (S1) receptor-binding domain (S1/RBD) region of the virus in serum samples taken between 4 weeks and 7 months after polymerase chain reaction (PCR) positivity in patients who are diagnosed with coronavirus disease-2019 (COVID-19). Study Design: A longitudinal study. Methods: This study included 113 patients with a clinical and molecular diagnosis of COVID-19. The first and second serum samples were taken 1 and 7 months, respectively, after the PCR positivity. S1/RBD-specific IgG antibody response was assayed using anti-SARS-CoV-2 QuantiVac ELISA (IgG) kit (Euroimmun, Lübeck, Germany). The neutralizing antibodies were investigated in 57 patients whose IgG test results were above the cut-off value. Results: In 57 patients with SARS-CoV-2 IgG, the anti-SARS-CoV-2 IgG quantitative antibody levels significantly decreased after 7 months (Z = −2.197, p = 0.028). A correlation was detected between the anti-SARS-CoV-2 IgG and nAb percent inhibition (IH%) levels detected in 1 month (rs = 0.496, p < 0.001), but without significant correlation in serum samples taken on 7 months. The nAb IH% levels of the first and second were compared for COVID-19 severity and revealed no statistical difference (p = 0.256). In the second serum sample, the nAb IH%s of patients with moderate COVID-19 showed a statistically significant difference from patients with mild COVID-19 (p = 0.018), but without significant differences between severe and moderate or mild COVID-19. Conclusion: SARS-CoV-2 quantitative IgG antibody titers are significantly reduced at long-term follow-up (> 6 months). Due to the limited information on seroconversion, comprehensive studies should be conducted for long-term follow-up of the immune response against SARS-CoV-2.

The aim of this study is to determine teachers' views on metaverse-based education. The research is qualitative and was conducted with a phenomenological design. The maximum diversity sampling technique, one of the purposeful sampling methods of the study group, was used. In this direction, data were collected from as many different teachers as possible, including their age, field of study, school type, and gender. The research was carried out with 32 teachers. The data were collected through face-to-face interviews using a semistructured interview consisting of four questions about personal information and 11 questions about the metaverse. The data were analyzed by a descriptive analysis method using MAXQDA software. Teachers stated that the integration of metaverse technology into education and training processes will increase the quality, efficiency and permanence of this process.

The Black Sea has been the scene of conflict between coastal states throughout history. Recently, emerging Russia-Ukraine tension continues. The Russian Federal Security Service ("FSB") destroyed and captured three Ukrainian Military Ships and detained 24 of its personnel on November 23, 2018. As the dispute continued, Ukraine finally raised the matter to the International Tribunals on Law of The Sea ("ITLOS"). This tension, which reaches a hot conflict point from time to time, causes legal changes in the Black Sea eventually. This article discusses the current legal status of Crimea, the Sea of Azov, and the Kerch Strait and aims to explain from the perspective of Russia, Ukraine, and Turkey. The study shows how the conflict environment in the region has a changing effect on the existing maritime borders and whether Russia’s act is a part of violation of the UNCLOS and International Law.

Background: Widespread and effective use of molecular diagnostic tests is indispensable for protecting public health and containing the severe respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic. More than 1 year into the pandemic, as resources have reached a point of depletion, grouping samples in pools of certain sizes appears to be a reasonable method to reduce both the costs and the processing time without necessitating additional training, equipment, or materials. Aims: To assess whether the pooling strategy that was used in past outbreaks and is used in blood tests prior to transfusion for screening large populations can also be used in SARS CoV-2 tests. Study Design: Diagnostic accuracy study. Methods: This prospective study was conducted with 2815 samples, sent to the coronavirus disease 2019 (COVID-19) Laboratory of our hospital between February 12 and 21, 2021, to be tested for the presence of SARS-CoV-2. The samples were examined individually and in pools of five 100 µl taken from each sequential sample, using 3 different SARS-CoV-2 reverse transcription-polymerase chain reaction (RT-PCR) kits, the Allplex™ 2019-nCoV Assay kit (Seegene, Republic of Korea), the GeneMAP™ 2019-nCoV detection V.3 kit (GenMark, Türkiye), and the Bio-Speedy™ SARS-CoV-2 Double Gene™ RT-qPCR kit (Bioeksen, Türkiye) on the BioRAD CFX96™ Touch (Bio-Rad Laboratories Inc., Hercules, CA, USA) platform available in our laboratory. Results: Following the extraction of serial dilutions prepared from the SARS-CoV-2 RNA positive (cycle of threshold: 20) sample, the standard curves of RT-PCR were analyzed. By evaluating the efficiency (E) values, all 3 kits showed high sensitivity and similar results; while the highest level was detected with the Allplex™ 2019-nCoV Assay kit in the nucleocapsid (N) gene (E: 124%), the lowest was detected with the Double Gene™ RT-qPCR kit in the N and ORF 1ab genes (E: 90%). Of the samples included in the study, only 1 positive sample with low viral load was found to be negative when studied by pooling. The total number of kits to be used in pooled tests and then to individually retest the 5 samples in positive pools was calculated as 827 and the savings rate as 69.91% (1968/2815). Conclusion: The pooling strategy is an effective approach to extend the impact of limited testing resources and reagents available in certain periods of the COVID-19 pandemic. Testing by pooling samples requires improvement of RNA extraction methods and careful monitoring of RT-PCR test sensitivity to avoid missing low-positive entities. Therefore, based on the prevalence of COVID-19 in their regions, laboratories should conduct their own validation of pooling studies for RNA extraction and amplification methods they use.

BackgroundThe COVID-19 pandemic accelerated the interest in implementing mobile health (mHealth) in population-based health studies, but evidence is lacking on engagement and adherence in studies. We conducted a fully remote study for ≥6 months tracking COVID-19 digital biomarkers and symptoms using a smartphone app nested within an existing cohort of adults. ObjectiveWe aimed to investigate participant characteristics associated with initial and sustained engagement in digital biomarker collection from a bespoke smartphone app and if engagement changed over time or because of COVID-19 factors and explore participants’ reasons for consenting to the smartphone substudy and experiences related to initial and continued engagement. MethodsParticipants in the Fenland COVID-19 study were invited to the app substudy from August 2020 to October 2020 until study closure (April 30, 2021). Participants were asked to complete digital biomarker modules (oxygen saturation, body temperature, and resting heart rate [RHR]) and possible COVID-19 symptoms in the app 3 times per week. Participants manually entered the measurements, except RHR that was measured using the smartphone camera. Engagement was categorized by median weekly frequency of completing the 3 digital biomarker modules (categories: 0, 1-2, and ≥3 times per week). Sociodemographic and health characteristics of those who did or did not consent to the substudy and by engagement category were explored. Semistructured interviews were conducted with 35 participants who were purposively sampled by sex, age, educational attainment, and engagement category, and data were analyzed thematically; 63% (22/35) of the participants consented to the app substudy, and 37% (13/35) of the participants did not consent. ResultsA total of 62.61% (2524/4031) of Fenland COVID-19 study participants consented to the app substudy. Of those, 90.21% (2277/2524) completed the app onboarding process. Median time in the app substudy was 34.5 weeks (IQR 34-37) with no change in engagement from 0 to 3 months or 3 to 6 months. Completion rates (≥1 per week) across the study between digital biomarkers were similar (RHR: 56,517/77,664, 72.77%; temperature: 56,742/77,664, 73.06%; oxygen saturation: 57,088/77,664, 73.51%). Older age groups and lower managerial and intermediate occupations were associated with higher engagement, whereas working, being a current smoker, being overweight or obese, and high perceived stress were associated with lower engagement. Continued engagement was facilitated through routine and personal motivation, and poor engagement was caused by user error and app or equipment malfunctions preventing data input. From these results, we developed key recommendations to improve engagement in population-based mHealth studies. ConclusionsThis mixed methods study demonstrated both high initial and sustained engagement in a large mHealth COVID-19 study over a ≥6-month period. Being nested in a known cohort study enabled the identification of participant characteristics and factors associated with engagement to inform future applications in population-based health research.

The importance of renewable energy (RE) shift increases as the instabilities around the world enhanced by the pandemic raise issues about energy security while the energy demand of countries continue to increase. This is especially the case for Türkiye, which is a member of OECD and among developing countries. In the next decade, onshore wind, and solar photovoltaics (PV) will be the focus of Türkiye's RE sector due to the rapid decrease in their costs. Hence, reasonable estimation of investments to the RE sector and cost projections of onshore wind and solar PV is crucial to plan, decide and set effective energy policies. In this study, three different investment scenarios are generated for RE of Türkiye. In addition, global and local cost projections are conducted for onshore wind and solar PV, again in three different scenarios. These scenarios are then utilized to estimate the annual installed capacity changes of Türkiye. Then, annual installed capacity amounts of Türkiye for onshore wind and solar PV are projected until 2030 in five novel scenarios: Economic, Average, Ambitious, Best-Case, and Worst-Case. According to the results, achievable and the most suitable 2030 targets for onshore wind and solar PV are determined as 25,000 MW and 60,000 MW for Türkiye, respectively. These results will assist the policymakers by elucidating the onshore wind and solar PV future of Türkiye, and the methodology may be useful for determination of RE targets for other countries.

Abstract Background Although open radical prostatectomy has been used in the treatment of localized prostate cancer for a long time, minimally invasive surgical approaches such as laparoscopic radical prostatectomy and robot-assisted radical prostatectomy have recently gained importance in order to improve postoperative results and shorten hospital stay. Although the enhanced recovery after surgery (ERAS) protocol was first defined for gastrointestinal surgeries in 2001, it has now been used in gynecological, orthopedic, thoracic and urological surgeries. In our study, we aimed to compare the results of the ERAS protocol with the conventional approach in patients who underwent laparoscopic radical prostatectomy. Methods There is a retrospective analysis of 70 patients who underwent laparoscopic radical prostatectomy at Kayseri City Hospital between May 2018 and January 2021. Data were analyzed using the Statistical Package for the Social Sciences (SPSS) for Windows, version 25.0 (IBM SPSS, Armonk, NY, USA). Seventy patients were included in the study. While 48 patients who underwent perioperative care with the traditional approach were included in the conventional group, 22 patients were included in the ERAS group. Age, comorbidities, preoperative PSA level, digital rectal examination findings, preoperative imaging, extra prostatic spread and lymph node involvement, location of tumors, time between biopsy and surgery, lymphadenectomy status, and histopathological findings of transrectal ultrasound biopsy and prostatectomy specimens were recorded for each patient. Initiation of enteral feeding, time to first defecation, duration of antibiotic use, ileus development rate and length of hospital stay (LOS) were compared for both groups. Results The mean age of 48 patients in the conventional group was 63.37 ± 7.01 years, while the mean age of 22 patients in the ERAS group was 66.36 ± 5.31 years (p = 0.080). Although the first defecation time was shorter in the ERAS group (4.75 ± 3.21 vs. 3.73 ± 2.12 days, p = 0.179), there was no statistically significant difference. Ileus developed in 10 (20.8%) patients in the conventional group and 2 (9.1%) in the ERAS group. Use of antibiotics in the postoperative period in the conventional group (5.83 ± 3.62 vs. 3.18 ± 2.42 days, p = 0.003) and LOS (7.92 ± 3.26 vs. 5.91 ± 2, 15 days, p = 0.011) were statistically significantly longer. Conclusion In summary, ERAS protocol is associated with short LOS, time to initiation of enteral feeding and duration of antibiotics use. There was no statistically significant difference in the rate of ileus and time to first defecation between the two groups. Randomized prospective studies on heterogeneous and larger patient groups are needed to confirm our findings.
