Introduction In low? and middle?income settings, more than a third of HIV?positive individuals starting antiretroviral therapy (ART) present with advanced disease (CD4 ? 200 cells/mm[sup.3] ); over half of these [...], : Introduction: Many HIV?positive individuals in Africa have advanced disease when initiating antiretroviral therapy (ART) so have high risks of opportunistic infections and death. The REALITY trial found that an enhanced?prophylaxis package including fluconazole reduced mortality by 27% in individuals starting ART with CD4 Methods: The REALITY trial enrolled from June 2013 to April 2015. A decision?analytic model was developed to estimate the cost?effectiveness of six management strategies in individuals initiating ART in the REALITY trial countries. Strategies included standard?prophylaxis, enhanced?prophylaxis, standard?prophylaxis with fluconazole; and three CrAg testing strategies, the first stratifying individuals to enhanced?prophylaxis (CrAg?positive) or standard?prophylaxis (CrAg?negative), the second to enhanced?prophylaxis (CrAg?positive) or enhanced?prophylaxis without fluconazole (CrAg?negative) and the third to standard?prophylaxis with fluconazole (CrAg?positive) or without fluconazole (CrAg?negative). The model estimated costs, life?years and quality?adjusted life?years (QALY) over 48 weeks using three competing mortality risks: cryptococcal meningitis; tuberculosis, serious bacterial infection or other known cause; and unknown cause. Results: Enhanced?prophylaxis was cost?effective at cost?effectiveness thresholds of US$300 and US$500 per QALY with an incremental cost?effectiveness ratio (ICER) of US$157 per QALY in the CD4 Conclusions: The REALITY enhanced?prophylaxis package in individuals with advanced HIV starting ART reduces morbidity and mortality, is practical to administer and is cost?effective. Efforts should continue to ensure that components are accessed at lowest available prices.

Growth alterations have been described in patients operated on for oral clefts. The purpose of this work was to analyze the craniofacial and palate morphology and dimensions of young adults operated on for oral clefts in early childhood in Spain. Eighty-three patients from eight different hospitals were divided into four groups based on their type of cleft: cleft lip (CL, n = 6), unilateral cleft lip and palate (UCLP, n = 37), bilateral cleft lip and palate (BCLP, n = 16), and cleft palate only (CPO, n = 24). A control group was formed of 71 individuals. Three-dimensional (3D) digital models were obtained from all groups with an intraoral scanner, together with cephalometries and frontal, lateral, and submental facial photographs. Measurements were obtained and analyzed statistically. Our results showed craniofacial alterations in the BCLP, UCLP, and CPO groups with an influence on the palate, maxilla, and mandible and a direct impact on facial appearance. This effect was more severe in the BCLP group. Measurements in the CL group were similar to those in the control group. Cleft characteristics and cleft type seem to be the main determining factors of long-term craniofacial growth alterations in these patients. Background: Raising mucoperiosteal flaps in traditional palatoplasty impairs mid-facial growth. Hyaluronic acid-based hydrogels have been successfully tested for minimally invasive craniofacial bone generation in vivo as carriers of bone morphogenetic protein-2 (BMP-2). We aimed to develop a novel flapless technique for cleft palate repair by injecting a BMP-2 containing hydrogel., Material and Methods: Dog pups with congenital cleft palate were either non-treated (n=4) or treated with two-flap palatoplasty (n=6) or with the proposed injection/adhesion technique (n=5). The experimental approach was to inject a hyaluronic acid-based hydrogel containing hydroxyapatite and BMP-2 subperiosteally at the cleft palate margins of pups aged six weeks. At week ten, a thin strip of the medial edge mucosa was removed and the margins were closed directly. Occlusal photographs and computed tomography (CT) scans were obtained up to week 20., Results: Four weeks after the gel injection the cleft palate margins had reached the midline and engineered bone had enlarged the palatal bones. Removal of the medial edge mucosa and suturing allowed complete closure of the cleft. Compared to traditional palatoplasty, the injection/adhesion technique was easier, and the post-surgical recovery was faster. CT on week 20 revealed some overlapping or "bending" of palatal shelves in the two-flap repair group, which was not observed in the experimental nor control groups., Conclusion: A minimally invasive technique for cleft palate repair upon injectable scaffolds in a dog model of congenital cleft palate is feasible. Complications are infrequent after cochlear implant surgery but they might occur despite careful preoperative planning and meticulous surgical technique. Among the most commonly encountered problems are those associated with the postauricular flap. An exposed, and therefore contaminated, device requires immediate attention and intervention. Cochlear implantation revision surgery is justified by two main reasons, the high price of these devices and the difficulty of reimplantation, due to cochlear fibrosis and ossification after its removal. There are multiple options in cochlear implantation revision surgery with infected device. However, the temporoparietal fascia flap is highly vascularized and provides some advantages over other alternatives. We report a case of a 5 year old boy with bilateral sensor neural hearing loss, who suffered a device extrusion three years after its implantation.

First branchial arch cysts are uncommon. Therefore, together with its variable clinical and age presentation they are often misdiagnosed at first. The treatment is surgical, requiring a correct procedure to avoid future recurrences. In this paper we describe a typical case of first branchial arch cyst in which as described in other reports, we first made several misdiagnoses and therefore an inadequate treatment and lastly, with the correct diagnosis, we performed a meticulous complete excision under facial nerve monitoring.

In this work, we investigated the ability of injected recombinant human bone morphogenetic protein 2 (rhBMP-2) on brushite cement (a β-tricalcium phosphate-based biomaterial) and collagen gel as carriers to induce osteogenic differentiation in the palatal submucosa of 10-day-old rats. This was part of a broader study aiming to create bone in the palatal submucosa at cleft palate edges in the search for a minimally invasive treatment. Thirteen treated animals, 7 with rhBMP-2/brushite cement and 6 with rhBMP-2/collagen gel, were injected with 5 to 10 μL of each biomaterial in the right palatal submucosa at the level between the second and third rugae. The contralateral site was uninjected and served as the control. Six weeks after injection, both brushite cement and collagen gel were histologically unrecognizable in all treated animals. New bone structures such as ossicles of woven bone were not detected. However, an augmentation in the thickness of the palatal fibromucosa was observed at the injection site of all palates. In addition, immunolabeling for osteopontin, proliferating cell nuclear antigen, and TUNEL revealed intense osteogenic induction at the injection site with both constructs, which was negative in the control site from the same specimens; no differences regarding cell proliferation and death were observed. The present study confirms the feasibility of generating osteogenic cells in the palatal submucosa by injecting low doses of rhBMP-2 in these 2 biomaterials, together with their inability to form bone.

In humans, cleft palate (CP) is one of the most common malformations. Although surgeons use palatoplasty to close CP defects in children, its consequences for subsequent facial growth have prompted investigations into other novel surgical alternatives. The animal models of CP used to evaluate new surgical treatments are frequently obtained by creating surgically induced clefts in adult dogs. This procedure has been ethically criticized due to its severity and questionable value as an animal model for human CP. Dogs born with a congenital CP would be much better for this purpose, provided they developed CP at a sufficient rate and could be fed. Up until now, feeding these pups carried the risk of aspiration pneumonia, while impeding normal suckling and chewing, and thus compromising orofacial growth. We developed a technique for feeding dog pups with CP from birth to the time of surgery using two old Spanish pointer dog pups bearing a complete CP. This dog strain develops CP in 15-20% of the offspring spontaneously. Custom-made feeding teats and palatal prostheses adapted to the pups' palates were made from thermoplastic plates. This feeding technique allowed lactation, eating and drinking in the pups with CP, with only sporadic rhinitis. To determine whether the use of this palatal prosthesis interferes with palatal growth, the palates of three littermate German shorthaired pointer pups without CP, either wearing or not wearing (controls) the prosthesis, were measured. The results showed that the permanent use of this prosthesis does not impede palatal growth in the pups.

Hemangiomas are frequent tumors that manifest in variable forms. Rapidly Involuting Congenital Hemangiomas (RICH) are a rare subtype, only recently described, which, in contrast to the typical infantile hemangioma, undergo accelerated involution, sometimes within days. Their different initial appearance and posterior evolution may cause confusion with other tumors of infancy or even with congenital malformations. We present a case of RICH in the upper lip that after rapid involution produced a morphologic sequel that resembled a microform cleft lip. The authors review the specific characteristics of this rare tumor and describe the technical detail of surgical correction in this patient.

Poland's syndrome is considered to be a polimalformative entity with different degrees of severity. We retrospectively reviewed our patients with Poland's syndrome diagnosis, treated between 2000 and 2006. The aim of our study is to assess the different choices of surgical treatment for mammary reconstruction according to the importance of malformation and taking into account our own experience and literature references. A total o f 13 patients (5 males and 8 females) were studied and only 6 of them (1 male and 5 females) underwent surgery. Surgical indication was based on Foucras's classification, considering 3 grades of severity. We propose a surgical approach adapted to each stage, as well as surgical refinements to improve the aesthetic aspect. Most of our patients were very satisfied with final results even though they underwent complex procedures with a considerable rate of complications.

Septal haematoma after nasal trauma is a complication that can lead to septal abscess if unrecognized or early intervention is not performed. It can cause compression and thereby necrosis that evolve to a septal abscess in which cultures reveal saprophyte bacteria. Cartilage necrosis and destruction can produce impaired breathing and aesthetic deformities with collapse of the dorsum and the tip of the nose. We present a 10 year old masculine infant, that suffered a nasal fracture with a septal haematoma that remained undiagnosed. The patient developed a septal abscess that required drainage and resulted in nasal sequelae with collapse of dorsum and cranial displacement of tip and columella. Functional and aesthetic reconstruction was performed using rib cartilage grafts. No complications occurred. Functional and aesthetic improvement was observed. Result after 2 months of follow-up is considered favourable.

Background: Palatoplasty has the undesired side effect of impaired mid-facial growth. To avoid this problem, we propose an alternative to palatoplasty. We hypothesize that if BMP-2 is injected together with a carrier into the periosteum of the cleft palate borders, border volume will increase and connective tissue cells will be activated to produce extra bone. Once these borders supported by bone reach the midline, extraction of their covering epithelia with trypsin will permit adhesion of the underlying tissues. We investigated in vitro the ability of cleft palate connective tissue cells to produce extra bone in the presence of BMP-2 and the possibility of using trypsin to remove the epithelium covering the cleft palate borders without impairing the underlying tissues' ability to adhere., Materials and Methods: We used the cleft palate presented by tgf-beta(3) null mice and small fragments of human cleft palate mucoperiosteum as models. Immunolabeling BMP-2-treated or untreated cultures with TUNEL and anti-osteocalcin or PCNA antibodies was performed. The epithelium of the cleft palate borders was removed with a trypsin solution, and the de-epithelialized tissues were cultured in apposition., Results: BMP-2 induces differentiation toward bone on cleft palate connective tissue cells without producing cell death or proliferation. Trypsin removal of the cleft palate margins' epithelium does not impair the underlying tissues' adhesion., Conclusion: It is possible to generate extra bone at the cleft palate margins and to chemically eliminate their covering epithelia without damaging the underlying tissues, which allows further investigation in vivo of this new approach for cleft palate closure.

Liposuction has become one of the most frequent aesthetic procedures. The "typical" patient is a young female with fat deposits in the trochanteric region. Gradually the safety of the technique together with the high satisfaction rate of the patients have contributed to the widening of its indications to other pathologies of the adipose tissue in different age populations. However, there is scarce literature regarding liposuction in childhood. The authors present their experience with liposuction as the treatment of congenital as well as acquired lipodystrophies in the paediatric age. Some particular details of the technique are described and the advantages of liposuction versus open surgery in these cases are analyzed.

Progressive hemifacial Atrophy (PHA) or Romberg's Disease is a rare entity of unknown etiology, which most often affects only one side of the face and can include all tissue planes in different grades of severity. Of the several treatments proposed, pedicled or free flaps seem to achieve the best cosmetic results in severe cases. In mild to moderate cases, free autologous fat transplant may obtain optimal results with lower complexity, cost and morbidity. The presence of multiple adipoblasts might increase the possibilities of graft take, particularly in children. Furthermore, refinements in the processes of extraction, preparation and reintroduction of the fat have contributed to improve the results that may be long lasting. We present 2 cases of PHA in 2 girls (11 and 13 years old), that were treated by lipofilling. No complications occurred. In both cases we observed a small reduction in the volume of the initial graft. Thus the procedure was repeated 6 months later to improve facial symmetry. Results have been excellent in one year follow-up.

Nasal gliomas are tumors of extracranial neuroglial tissue. The incidence is very low, with little more than 300 cases described unliterature. It is important to make a correct diagnosis study of this lesion, to avoid the possible complications derived from an erroneous treatment. We review a case of one girl with midline nasal tumor, she was operated on with total extirpation of the tumor, we analyze the previous diagnosis study and their histopathology.

Vascular anomalies are extremely common but for many years have been poorly understood, partly due to the complex and colorful terminology used, which is full of descriptive terms tinged with folklore. While many lesions look the same, they have a very different prognosis and require very different treatment. Therefore, it is essential to be precise when using the nomenclature. This article reviews the current classification of vascular anomalies and describes recent advances in their diagnosis and management. Our aim is to replace the old confusing terminology and contribute to the unification of concepts and terms. At the same time we stress the importance of interdisciplinary collaboration in order to offer patients accessible and integral treatment.

We report a series of 21 consecutive patients seen at the Ophthalmology Department of the University Hospital Zurich, Switzerland, with the arteritic form of anterior ischemic optic neuropathy (AION). 19 patients had giant cell arteritis, one had periarteritis nodosa and one had cP-arteritis. They comprised 11 men and 10 women, ranging in age between 66 and 88 years. The median age was 80. We analyzed the course of events in each case before and after involvement of the first eye, as well as the frequency and possible causes of involvement of the second eye. The diagnosis was regarded as delayed when, despite typical signs, symptoms amd laboratory abnormalities, systemic vasculitis was not considered in the differential diagnosis. Treatment was considered inadequate if, following visual loss in one eye and diagnosis of a systemic vasculitis, a dose of 1 mg/kg prednisone or less was given, and/or the initial dose was reduced by more than 50% during the first month. Of 21 patients, 10 suffered bilateral visual loss. 8 of these 10 patients became legally blind. In 13 out of 21 cases there was no delay in diagnosis and treatment was adequately given. All 11 patients with unilateral involvement, who did not suffer a substantial loss in quality of life, belong to this subgroup. In 8 cases diagnosis was either delayed or treatment was inadequate. All of these patients had bilateral ocular involvement. In one patient, visual loss in the second eye could not be avoided despite correct diagnosis and treatment (M.A., No. 1). In this patient the interval between involvement of the first and second eye was very short (3 days). One patient had a mature cataract in the first affected eye and sought medical help only after his good eye became involved (K. F., No. 15). In this report we would like to draw attention to the extremely poor visual prognosis due to frequent bilateral ocular involvement in giant cell arteritis. Corticosteroid treatment cannot restore vision in the already affected eye, but it is, in the majority of cases, highly effective in preventing visual loss in the second eye. Thus, it is crucial to begin treatment immediately, to start with a high dose (preferably 1 g methyl-prednisolone i.v.), and to continue high-dose oral treatment long enough to prevent delayed visual loss in the second eye. The most vulnerable period appears to be the first month following involvement of the first eye. Caring for patients with giant cell arteritis who have lost vision in one eye is a challenge to all involved physicians. It resembles a "high-wire act" with the threat of blindness on the one hand and the dangers of long term corticosteroid treatment on the other. An interdisciplinary approach with ongoing communication between the family physician and the ophthalmologist is required.

Presentation of results obtained when correction of congenital incurvation of the penis is done with the albuginea plication technique (with no resection of an albuginea wedge). Two left lateral, two ventral and one complex (ventral plus lateral) incurvations were performed. In four cases, curvature correction was achieved. In the case of complex curvature, a certain degree of lateral incurvation persisted, probably because the number of applications applied was insufficient. Partial necrosis of the penis skin in one case was attributed to simultaneous subcoronal and pubic approach. The simple plication of the albuginea is considered to be an adequate technique to resolve this type of congenital malformation.

We conducted an experimental study on 20 dogs that were subdivided into two groups: the control group comprised 5 dogs that were submitted to resection of 50% of the bladder; the other group comprised 15 dogs submitted to resection of 50% of the bladder and replacement with Gore-tex patch, which was removed 3 months thereafter in 10 dogs. CPK, creatinine, ions, and urinary pH values were determined and urine cultures were performed. The biomechanical parameters of the bladder wall were determined by cystometry. A histological study with hematoxylin-eosin of bladder dome specimens was done. The results showed no increased bladder capacity in the animals that received the Gore-tex implant versus the control animals, chiefly due to the necrosis produced by stiffness of the Gore-tex implant. This necrosis was associated with diminished elasticity (due to collagen) and substitution of the muscle fibers by other viscoelastic fibers with a higher elastic constant. Three months following removal of the Gore-tex implant, we observed fibrotic reaction, secondary calcification of the necrotic bladder wall and inflammatory phenomena that impeded muscle regeneration. The urinary pH increased significantly in the dogs that received the Gore-tex implant. The foregoing results show that although the Gore-tex patch is impermeable and resistant to infection, it does not have the appropriate biomechanical properties.

Presentation of 6 cases of congenitally short urethra seen in our Unit. The rarity of such malformation, as well as the difficulties to arrive to a presurgical diagnosis are emphasized. Also, review of surgical techniques described in the literature. Contribution of our experience in the use of the urethra's dissecting and elevating technique. We considered this the choice procedure except in extreme cases due to the good results obtained, fewer technical difficulties and lower risk of complications.

In the Canton of Berne, two working groups are currently developing a concept of school health education, in which the individual teacher assumes the principal role. In terms of documentation and availability of teaching aids, he is assisted by three addiction information centers, as well as other health institutions. Teaching goals are being formulated, socio-cultural conditions are being considered, and based on these, age specific lesson plans and exercises are being developed. On the other hand, the opportunity of a general reform of teacher training is being taken advantage of, in order to define main ideas, teaching goals and instructional contents for teacher training in human biology and health. The aim of teacher training in health education is twofold: on the one hand, it is to prepare the future teacher to include health education in his teaching skills; on the other hand, it is to enable the teacher to act appropriately, when faced with diseased or disabled pupils. The latter point becomes particularly important, as disabled children are being integrated into normal classes.

During the last ten years most cantonal educational departments in Switzerland have tackled the problem of sexual education. With the exception of the canton of Schaffhausen all the cantons have decided not to introduce compulsory sexual education. In almost all the cantons the consent of the parents is necessary. They usually have the right to have their children excused from sexual education. Theoretically speaking, sexual education is not confined to a certain age, but actually it is only given in the middle and upper classes. All the cantons accept that the parents should give their own children sexual education; school has only a complementary function. Usually the form master is responsible for sexual education. A close intercantonal cooperation is not the case, because sexual education is too much dependent on local conditions.
