A far rarer but well-established differential in patients with rapid cognitive deterioration, acute psychosis, abnormal movements and seizures is autoimmune encephalitis. Exemplified by our case we highlight clinical and economic problems arising in management of geriatric patients with cognitive decline and psychotic symptoms.Case Presentation: A 77-year-old female caucasian patient with an unremarkable medical history was hospitalized after a fall in association with diarrhea and hyponatremia. Upon adequate therapy, disorientation and troubled short-term memory persisted. Within a week the patient developed visual hallucinations. Basic blood and urine samples and imaging (cranial computed tomography and magnetic resonance imaging) were unremarkable. With progressive cognitive decline, amnestic impairment, word finding difficulty and general apathy, psychiatric and neurologic expertise was introduced. Advanced diagnostics did not resolve a final diagnosis; an electroencephalogram showed unspecific generalized slowing. Extended clinical observation revealed visual hallucinations and faciobrachial dystonic seizures. A treatment with anticonvulsants was initiated. Cerebrospinal fluid ultimately tested positive for voltage-gated potassium channel LGl1 (leucine-rich-inactivated-1) antibodies confirming diagnosis of autoimmune anti-LGI1 encephalitis. Immediate immunotherapy (high-dose glucocorticoids and administration of intravenous immunoglobulin G) led to a rapid improvement of the patient’s condition. After immunotherapy was tapered, the patient had one relapse and completely recovered with reintroduction of glucocorticoids and initiation of therapy with rituximab.Conclusion: Rapidly progressive dementia in geriatric patients demands a structured and multidisciplinary diagnostic approach. Accurate management and financially supportable care is a major issue in rare diseases such as anti-LGI1-encephalitis. 4. Purity in Chromatically Localized Algebraic K-theory

Abstract

We prove a purity property in telescopically localized algebraic K-theory of ring spectra: For n>1, the T(n)-localization of K(R) only depends on the T(0)+...+T(n)-localization of R. This complements a classical result of Waldhausen in rational K-theory. Combining our result with work of Clausen–Mathew–Naumann–Noel, one finds that LT(n)K(R) in fact only depends on the T(n-1)+T(n)-localization of R, again for n>1. As consequences, we deduce several vanishing results for telescopically localized K-theory, as well as an equivalence between K(R) and TC(T>0R) after T(n)-localization for n>2.

Abstract

The MuCap experiment at the Paul Scherrer Institute performed a high-precision measurement of the rate of the basic electroweak process of nuclear muon capture by the proton, $\mu^- + p \rightarrow n + \nu_\mu$. The experimental approach was based on the use of a time projection chamber (TPC) that operated in pure hydrogen gas at a pressure of 10 bar and functioned as an active muon stopping target. The TPC detected the tracks of individual muon arrivals in three dimensions, while the trajectories of outgoing decay (Michel) electrons were measured by two surrounding wire chambers and a plastic scintillation hodoscope. The muon and electron detectors together enabled a precise measurement of the $\mu p$ atom's lifetime, from which the nuclear muon capture rate was deduced. The TPC was also used to monitor the purity of the hydrogen gas by detecting the nuclear recoils that follow muon capture by elemental impurities. Abstract

We report on spatially resolved measurements of the spin-orbit effective magnetic field in a GaAs/InGaAs quantum-well. Biased gate electrodes lead to an electric-field distribution in which the quantum-well electrons move according to the local orientation and magnitude of the electric field. This motion induces Rashba and Dresselhaus effective magnetic fields. The projection of the sum of these fields onto an external magnetic field is monitored locally by measuring the electron spin-precession frequency using time-resolved Faraday rotation. Abstract

The magnetic stray field from Fe gates is used to modify the spin precession frequency of InGaAs/GaAs quantum-well electrons in an external magnetic field. By using an etching process to position the gates directly in the plane of the quantum well, the stray-field influence on the spin precession increases significantly compared with results from previous studies with top-gated structures. In line with numerical simulations, the stray-field-induced precession frequency increases as the gap between the ferromagnetic gates is reduced. Abstract

Time-resolved Faraday rotation is used to measure the coherent electron spin precession in a GaAs/InGaAs quantum well below an interdigitated magnetized Fe grating. We show that the electron spin precession frequency can be modified by applying a gate voltage of opposite polarity to neighboring bars. A tunability of the precession frequency of 0.5 GHz/V has been observed. Modulating the gate potential with a gigahertz frequency allows the electron spin precession to be controlled on a nanosecond timescale.

Abstract

Electron spins in an InGaAs semiconductor quantum well are used as a magnetometer of magnetic stray-fields from patterned Fe stripes. Using time-resolved Faraday rotation, the coherent precession of quantum-well spins in the inhomogeneous field below the Fe stripes is measured for varying magnetic fields. Comparing with reference stripes made of Au, we find an enhancement of the spin precession frequency proportional to the Fe magnetization, in line with a decrease of the spin decay time, which is attributed to the inhomogeneous magnetic stray-field in the quantum well layer.

Abstract

Aharonov-Bohm oscillations are studied in the magnetoconductance of a micron-sized open quantum ring coupled capacitively to a Coulomb-blockaded quantum dot. As the plunger gate of the dot is modulated and tuned through a conductance resonance, the amplitude of the Aharonov-Bohm oscillations in the transconductance of the ring displays a minimum. We demonstrate that the effect is due to a single-electron screening effect, rather than to dephasing. Abstract

A modular computational framework for the identification of friction parameters in metal machining applications is presented. Numerical simulation of such processes using mesh-based techniques (usually) necessitates the re-meshing procedure, which is time-consuming and hard to parallelize. Therefore, the present framework synthesizes the advantages of mesh-free methods with GPU parallel computing, offering an efficient tool for the optimization procedures in thermomechanical modeling of cutting problems. The proposed approach employs an inverse method to determine the unknown coefficients of a temperature-dependent friction model in high-speed metal cutting. We found good agreement between the numerical results and experimental data through a quantitative-qualitative comparison.

Abstract

Objective. To evaluate patients with psoriatic arthritis (PsA) receiving etanercept (ETN) monotherapy or ETN plus conventional synthetic disease-modifying antirheumatic drugs (csDMARD) to determine the proportion achieving a clinically meaningful response in arthritis, psoriasis, and quality of life simultaneously. Methods. A prospective, multicenter, 52-week observational study in patients with active PsA evaluated treatment with ETN in clinical practice (ClinicalTrials.gov: NCT00293722). This analysis assessed simultaneous achievement of 3 treatment targets: low disease activity (LDA) based on 28-joint count Disease Activity Score (DAS28); body surface area (BSA) involvement = 3%; and a score > 45 on the Medical Outcomes Study Short Form-12 (SF-12) physical component summary. Results. Of 579 patients, 380 received ETN monotherapy and 199 received combination ETN plus csDMARD. At 52 weeks, data for all 3 disease domains were available for 251 patients receiving monotherapy and 151 receiving combination therapy. In the monotherapy and combination therapy groups, 61 (24.3%) and 37 (24.5%) patients, respectively, achieved all 3 treatment targets simultaneously. A significantly greater proportion of patients receiving monotherapy versus combination therapy achieved SF-12 > 45 (43.0% vs 31.8%; p < 0.05) and DAS28 LDA (72.5% vs 62.3%; p < 0.05). Conversely, BSA = 3% was reached by a significantly greater proportion receiving combination therapy (75.5% vs 56.6%; p < 0.001). However, baseline BSA involvement was higher for the monotherapy group. Conclusion. While nearly half the patients achieved arthritis and psoriasis treatment targets simultaneously and one-fourth reached all 3 treatment targets, combining ETN and csDMARD did not substantially improve clinical response compared with ETN monotherapy in this real-world PsA patient population.
