Introduction: Hypertension is a major public health problem due to its high proportion all around the globe. Hypertension is a major cause of premature death worldwide. An estimated 1.28 billion adults aged 30–79 years worldwide have hypertension, most (two-thirds) living in low and middle-income countries. Aims and Objectives: To estimate proportion of undiagnosed hypertensive cases in >30 years age group. Materials and Methods: A Pre-designed and semi-structured questionnaire was interviewed to the patients after taking their consent who were fulfilling the inclusion criteria, after that their blood pressure was measured using the appropriate tools. Data was entered in Microsoft excel spread sheet and analysed using Rsoftware. Chi square test and Kruskal Wallis H test was applied. Results: The proportion of hypertension was 32.9% (male: 40.9%, female: 26.0%).Mean systolic and diastolic BP were 124.25±15.05mmHg and 83.45±9.49mmHg, respectively. Higher odds of being hypertensive were found in male subjects, eldest age group, married subjects, subjects of upper socioeconomic status, illiterate subjects, and retired subjects. Tobacco and alcohol consumption, overweight, obesity, and abdominal obesity were also associated with hypertension Conclusion: Around one-third of the subjects were hypertensive and half of the study subjects were prehypertensive in this area. Unchecked inflammation is a hallmark of inflammatory tissue injury in diseases such as acute respiratory distress syndrome (ARDS). Yet the mechanisms of inflammatory lung injury remain largely unknown. Here we showed that bacterial endotoxin lipopolysaccharide (LPS) and cecal ligation and puncture-induced (CLP-induced) polymicrobial sepsis decreased the expression of transcription factor cAMP response element binding (CREB) in lung endothelial cells. We demonstrated that endothelial CREB was crucial for VE-cadherin transcription and the formation of the normal restrictive endothelial adherens junctions. The inflammatory cytokine IL-1[beta] reduced cAMP generation and CREB-mediated transcription of VEcadherin. Furthermore, endothelial cell-specific deletion of CREB induced lung vascular injury whereas ectopic expression of CREB in the endothelium prevented the injury. We also observed that rolipram, which inhibits type 4 cyclic nucleotide phosphodiesterase-mediated (PDE4-mediated) hydrolysis of cAMP, prevented endotoxemia-induced lung vascular injury since it preserved CREB-mediated VE-cadherin expression. These data demonstrate the fundamental role of the endothelial cAMP-CREB axis in promoting lung vascular integrity and suppressing inflammatory injury. Background: The Indian Ministry of Labour and Employment launched a social health protection program called Rashtriya Swasthya Bima Yojana (RSBY) in the year 2008 to provide financial protection from catastrophic health expenses to Below Poverty Line (BPL) households (HHs).[1,5]. Objectives: 1. To find out the awareness regarding Rashtriya Swasthya Bima Yojna among 100 BPL and unorganized workers in the urban population of Indore. 2. To find out the level of utilization of the scheme among card holders. Material and Methods: A community-based study using simple random sampling was conducted among 100 Below Poverty Line (BPL) families and unorganized workers residing in a slum area of Indore city. A pre-tested semi-structured questionnaire was used. Data was entered in excel sheet and analyzed using SPSS software 25.0 (trial version). Chi-square test was applied wherever necessary. Results: Among 100 study participants, 63% were males. Only 35% participants were literate. 79% were married, 20% were widow/widower, and 1% were unmarried. 45% were employed, 30% unemployed and 25% were retired. Majority (52 %) people were between 41-50 years' age. Only 25% were aware about RSBY. 95% felt it is very important and everyone should have it. 20% of the participants were card holders. 50% of the card holders had never utilized the card. When asked about changes desired in enrolment process, 50% people wanted it to be cashless and 35 % wanted it to be less time taking while 15% people wanted it to be paperless. Association between gender, employment status, literacy and awareness was found significant (p-value< 0.05). Conclusion: Awareness and utilization of RSBY in urban slum is low. Gastrointestinal tract duplications are rare congenital abnormalities. Majority are diagnosed in pediatric age group of less than two years. Lesions can be seen anywhere from the oral cavity to the anus, but the most commonly affected site is ileum (long and tubular) which is usually in the form of cystic masses. The clinical manifestation of Gastrointestinal duplicationin adults is variable, and they are rarely considered as part of differential diagnosis.
