Left ventricular assist devices (LVADs) are used in end-stage heart failure. Inadequate positioning of the inflow cannula may necessitate replacement of the LVAD. We present the successful use of a three-dimensional printed model used to optimize surgical planning and allow for simulation and training for the LVAD exchange procedure.

Resumo Fundamento As últimas décadas têm assistido ao rápido desenvolvimento do tratamento invasivo de arritmias por procedimentos de ablação por cateter. Apesar da sua segurança e eficácia bem estabelecida em adultos, até o momento, há poucos dados nos cenários pediátricos. Uma das principais preocupações é a possível expansão da cicatriz do procedimento de ablação nessa população e suas consequências ao longo dos anos. Objetivos Este estudo teve como objetivo analisar o risco da progressão da lesão miocárdica após ablação por cateter de radiofrequência em pacientes pediátricos. Métodos Este é um estudo retrospectivo de 20 pacientes pediátricos com tratamento prévio de arritmia supraventricular com ablação, submetidos à ressonância magnética cardíaca e angiografia coronária para avaliação de fibrose miocárdica e da integridade das artérias coronárias durante o acompanhamento. Resultados A idade mediana no procedimento de ablação foi 15,1 anos (Q1 12,9, Q3 16,6) e 21 anos (Q1 20, Q3 23) quando a ressonância magnética cardíaca foi realizada. Quatorze dos pacientes eram mulheres. Taquicardia por reentrada nodal e síndrome de Wolf-Parkinson-White foram os principais diagnósticos (19 pacientes), com um paciente com taquicardia atrial. Três pacientes apresentaram fibrose miocárdica ventricular, mas com um volume inferior a 0,6 cm 3 . Nenhum deles desenvolveu disfunção ventricular e nenhum paciente apresentou lesões coronarianos na angiografia. Conclusão A ablação por cateter de radiofrequência não mostrou aumentar o risco de progressão de lesão miocárdica ou de lesões na artéria coronária.

Although not routinely used, cardioneuroablation or modulation of the cardiac autonomic nervous system has been proposed as an alternative approach to treat young individuals with enhanced vagal tone and significant atrioventricular (AV) disturbances.We report the case of a 42-year-old athlete with prolonged ventricular pauses associated with sinus bradycardia and paroxysmal episodes of AV block (maximum of 6.6 s) due to enhanced vagal tone who was admitted to our hospital for pacemaker implantation. Cardiac magnetic resonance and stress test were normal. Although he was asymptomatic, safety concerns regarding possible neurological damage and sudden cardiac death were raised, and he accordingly underwent electrophysiological study (EPS) and cardiac autonomic denervation. Mapping and ablation were anatomically guided and radiofrequency pulses were delivered at empirical sites of ganglionated plexi. Modulation of the parasympathetic system was confirmed through changes in heart rate and AV nodal conduction properties associated with a negative cardiac response to atropine administration.After a follow-up of nine months, follow-up 24-hour Holter revealed an increase in mean heart rate and no AV disturbances, with rare non-significant ventricular pauses, suggesting that this technique may become a safe and efficient procedure in this group of patients. Resumo: Ainda que raramente utilizada, a cardioneuroablação ou modulação do sistema nervoso autónomo cardíaco tem vindo a ser proposta como abordagem alternativa ao tratamento de indivíduos jovens com tónus vagal aumentado e, consequentemente, perturbações significativas da condução auriculoventricular (AV).É apresentado um caso de um homem, atleta de 42 anos, referenciado para implantação de pacemaker por bradicardia sinusal e bloqueio auriculoventricular (BAV) de predomínio noturno por provável hipertonia vagal. As pausas noturnas por bradicardia sinusal associada a BAV atingiam os 6,6 segundos. Exames de imagem e prova de esforço foram normais. O doente foi submetido a estudo eletrofisiológico (EP) e desenervação autonómica cardíaca. O mapeamento e a ablação foram guiados anatomicamente, tendo sido aplicada energia por radiofrequência, de forma empírica, em locais da aurícula esquerda e direita onde previamente tinha sido descrita a existência de plexos ganglionares. A modulação do sistema parassimpático foi confirmada através da variação da frequência cardíaca e das propriedades de condução nodal AV no final do procedimento e ainda através de resposta cardíaca negativa à administração de atropina.No seguimento de nove meses, o Holter de 24 horas evidenciou aumento da frequência cardíaca média e ausência de perturbações significativos da condução AV, sugerindo que essa técnica poderá vir a constituir uma alternativa à implantação de pacemaker neste subgrupo de doentes.

Resumo Fundamento A ablação da taquicardia atrial (TA) com local de ativação mais precoce próxima ao feixe de His é um desafio, devido ao risco de bloqueio de AV completo por sua proximidade ao sistema de His-Purkinje (SHP). Uma alternativa para minimizar esse risco é posicionar o cateter na cúspide não coronária (CNC), que é anatomicamente contígua à região para-Hissiana. A 57 year-old patient with dilated cardiomyopathy, long-standing persistent atrial fibrillation, heart failure and episodes of gastrointestinal bleeding underwent AF ablation with pulmonary vein isolation, homogeneization of septal scar, posterior wall isolation and also left atrial appendage isolation. Additionally, because of the high risk of embolism, underwent left atrial appendage occlusion.

The authors report the first catheter ablation of Brugada syndrome in the literature using the Rhythmia™ mapping system. Learning points include: (1) low voltage areas can be documented while mapping in some individuals, suggesting that Brugada syndrome may not be a pure ion channel disorder; (2) typical long fractionated potentials can also be identified in the endocardium, supporting the need to map the endocardium in all Brugada patients requiring ablation; (3) disappearance of the typical coved pattern following ablation does not necessarily predict cure, as the patient we present experienced ventricular fibrillation recurrence a few months later. Background: Ventricular and supraventricular premature complexes (PC) are frequent and usually symptomatic. According to a previous study, magnesium pidolate (MgP) administration to symptomatic patients can improve the PC density and symptoms. Objective: To assess the late follow-up of that clinical intervention in patients treated with MgP or placebo. Methods: In the first phase of the study, 90 symptomatic and consecutive patients with PC were randomized (double-blind) to receive either MgP or placebo for 30 days. Monthly follow-up visits were conducted for 15 months to assess symptoms and control electrolytes. 24-hour Holter was performed twice, regardless of symptoms, or whenever symptoms were present. In the second phase of the study, relapsing patients, who had received MgP or placebo (crossing-over) in the first phase, were treated with MgP according to the same protocol. Results: Of the 45 patients initially treated with MgP, 17 (37.8%) relapsed during the 15-month follow-up, and the relapse time varied. Relapsing patients treated again had a statistically significant reduction in the PC density of 138.25/hour (p < 0.001). The crossing-over patients reduced it by 247/hour (p < 0.001). Patients who did not relapse, had a low PC frequency (3 PC/hour). Retreated patients had a 76.5% improvement in symptom, and crossing-over patients, 71.4%. Conclusion: Some patients on MgP had relapse of symptoms and PC, indicating that MgP is neither a definitive nor a curative treatment for late follow-up. However, improvement in the PC frequency and symptoms was observed in the second phase of treatment, similar to the response in the first phase of treatment.

Resumo: Introdução: Os circuitos de reentrada da taquicardia ventricular envolvem, por vezes, não só o endocárdio, mas também o epicárdio. A ablação de taquicardia ventricular por via epicárdica pode ser útil nessas situações. Objetivo: O objetivo do estudo consistiu em avaliar a eficácia, segurança e complicações de uma série de doentes consecutivos submetidos a ablação de arritmias ventriculares com mapeamento por via epicárdica. População e métodos: Foram incluídos no estudo todos os doentes submetidos a ablação de taquicardia ventricular com mapeamento por via epicárdica desde 2004 até 2012. De um total de 95 ablações, em nove doentes foi tentada a via epicárdica, oito do sexo masculino com 58 ± 12 anos. Todos os doentes tinham sido previamente submetidos ou realizaram concomitantemente mapeamento endocárdico. A etiologia da arritmia era não isquémica em oito doentes e isquémica num. Comparou‐se o número de eventos nos seis meses anteriores ao procedimento epicárdico com os seis meses seguintes. Resultados: O acesso epicárdico foi conseguido em oito doentes por via percutânea subxifoideia. Num caso não foi possível abordagem epicárdica por presença de aderências. Em nenhum dos doentes o procedimento foi repetido e não se verificaram complicações major no período intra‐hospitalar. Num seguimento médio de 3,5 ± 1,2 anos registou‐se um acidente vascular cerebral num doente, não se registando outras complicações a médio/longo prazo, tendo a maioria dos doentes diminuído o número de episódios de taquicardia ventricular após ablação. Conclusões: A ablação de taquicardia ventricular por via epicárdica foi utilizada eficazmente na diminuição da morbilidade em oito doentes com baixo risco de complicações a médio/longo prazo. Abstract: Introduction: Reentrant circuits of ventricular tachycardia may involve not only the endocardium but also the epicardium. Epicardial ablation can be useful in these situations. Objective: The aim of this study was to assess efficacy, safety and complications in a series of consecutive patients who underwent ablation of ventricular tachycardia with epicardial mapping. Methods: The study included all patients undergoing ventricular tachycardia ablation with epicardial mapping from 2004 to 2012. Of a total of 95 ablations, an epicardial approach was attempted in nine patients, eight male, mean age 58±12 years. Endocardial mapping was performed in all patients previously or simultaneously. The etiology of the arrhythmia was non‐ischemic in eight patients and ischemic in one. We compared the number of events in the six months prior to the epicardial procedure and six months after. Results: Percutaneous epicardial access was achieved in eight patients. In one case it was not possible due to the presence of adhesions. In none of the patients was the procedure repeated and there were no major complications during hospitalization. In a mean follow‐up of 3.5±1.2 years, one patient suffered stroke; there were no other medium‐to‐long‐term complications and the number of ventricular tachycardia episodes was reduced in all patients after ablation. Conclusions: Epicardial radiofrequency ablation of ventricular tachycardia was effective in reducing morbidity in eight patients, with a low risk of complications in the short and medium‐to‐long term. Resumo: Objetivos: Avaliar a eficácia e segurança do sistema de navegação magnética por controlo remoto Niobe II (Stereotaxis) na ablação de taquicardia ventricular ou extrassístoles ventriculares da câmara de saída do ventrículo direito e esquerdo. Métodos: Estudaram-se 32 doentes consecutivos, idade média 43 ± 11 anos, 24 mulheres referenciados para ablação. O mapeamento da arritmia foi efetuado com o sistema CARTO RMT, orientado por controlo remoto. A ablação foi realizada com radiofrequência no local de ativação mais precoce com pacemapping de pelo menos 11/12. O sucesso foi definido como a supressão da arritmia e a sua não inducibilidade sob isoprenalina.Após um seguimento mínimo de três meses avaliámos o sucesso clínico, definido como a ausência de sintomas e a supressão de arritmias no Holter, definido como

FUNDAMENTO: Diversos mecanismos podem estar envolvidos no desencadeamento da síncope em pacientes com cardiomiopatia hipertrófica (CMH), incluindo colapsos hemodinâmicos que podem estar relacionados a um desequilíbrio autonômico. OBJETIVO: Avaliar e comparar a função autonômica de pacientes que apresentam CMH com síncope inexplicada (SI) com os que não apresentam síncope. MÉTODOS: Trinta e sete pacientes foram incluídos, sendo 16 com SI e 21 sem síncope. Sua função autonômica foi avaliada por sensibilidade barorreflexa (SB) espontânea e induzida por fenilefrina, pela variabilidade da frequência cardíaca (VFC) no domínio do tempo durante o Holter de 24 horas e no domínio da frequência (análise espectral), ambos em decúbito dorsal e no teste de inclinação (TI) a 70º. RESULTADOS: A SB espontânea mostrou-se semelhante em ambos os grupos (16,46 ± 12,99 vs. 18,31 ± 9,88 ms/mmHg, p = 0,464), assim como a SB induzida por fenilefrina (18,33 ± 9,31 vs. 15,83 ± 15,48 ms/mmHg, p = 0,521). Não foram observadas diferenças no SDNN (137,69 ± 36,62 vs . 145,95 ± 38,07 ms, p = 0,389). O grupo com síncope apresentou um RMSSD significativamente menor (24,88 ± 10,03 vs. 35,58 ± 16,43 ms, p = 0,042) e tendência a menor pNN50 (4,51 ± 3,78 vs . 8,83 ± 7,98%, p =0,085) e a menores valores do componente de alta frequência da análise espectral da VFC em repouso (637,59 ± 1.295,53 vs. 782,65 ± 1.264,14 ms2 , p = 0,075). Nenhuma diferença significativa foi observada em resposta ao TI (p = 0,053). A sensibilidade, especificidade e acurácia do TI na identificação da etiologia da SI em pacientes com CMH foram 6%, 66% e 40%, respectivamente. CONCLUSÃO: Observou-se tônus parassimpático mais baixo em pacientes com CMH e SI, mas a relevância clínica deste achado ainda não está clara. O TI não é uma ferramenta vantajosa para avaliar a origem da síncope em tais doentes, principalmente por causa da sua baixa especificidade.

OBJETIVO: Avaliar a evolução clínica de pacientes com SNC após a suspensão do tratamento farmacológico e investigar as possíveis variáveis clínicas preditivas de recidiva. MÉTODOS: Trinta e sete pacientes (média de idade de 31 ± 16 anos) com SNC recidivante refratária, dos quais 19 eram mulheres, foram estudados prospectivamente. Todos os pacientes estavam assintomáticos e apresentaram teste de inclinação ortostática (TI) negativo após a introdução da terapia farmacológica. Um novo TI foi realizado um mês após a suspensão do tratamento. A probabilidade livre de sintomas foi analisada de acordo com sexo, idade, número de síncopes prévias ao tratamento, tempo de história clínica, tempo de tratamento, resultado do TI após interrupção do tratamento e período sem medicação. RESULTADOS: Vinte e dois pacientes (59%) apresentaram recidiva durante um acompanhamento médio de 21 ± 19,7 meses. As variáveis relacionadas com maior recidiva foram número de síncopes anteriores (p = 0,0248), TI positivo após a suspensão da terapia (p = 0,0002) e sexo feminino (p = 0,0131). CONCLUSÕES: A maior parte dos pacientes altamente sintomáticos com SNC apresenta recidiva após a supressão do tratamento. A realização do TI após a suspensão do tratamento pode identificar os pacientes com maior risco de recidiva, sobretudo durante o primeiro ano de acompanhamento.OBJECTIVE: To evaluate the outcome of patients with NCS after interruption of pharmacological therapy and to investigate the possible clinical variables predicting recurrence. METHODS: Thirty-seven patients (age 31±16 years) with refractory recurrent NCS being 19 females where prospectively studied. All patients became asymptomatic and had a negative tilt table test (TT) after pharmacological therapy. The treatment was interrupted and one month later, a new TT with no medication was carried out. The probability free of symptoms recurrence was analyzed according to sex, age, number of syncope episodes previously to the treatment, clinical history time, treatment time, drug free from treatment time and TT result. RESULTS: Twenty-two patients (59%) presented recurrence during a mean follow-up of 21±19.7 months. The variables related to greater recurrence were number of previous syncope (p=0.0248), positive TT after interruption of the therapy (p=0.0002) and female gender (p= 0.0131). CONCLUSIONS: Most of the very symptomatic patients with NCS present recurrence after the suppression of a specific therapy. A TT carried out after treatment discontinuation can identify patients with higher risk of recurrence, specially in the first year of follow-up.

OBJECTIVE: To evaluate the influences of circadian variations on tilt-table testing (TTT) results by comparing the positivity rate of the test performed during the morning with that of the test performed in the afternoon and to evaluate the reproducibility of the results in different periods of the day. METHODS: One hundred twenty-three patients with recurrent unexplained syncope or near-syncope referred for TTT were randomized into 2 groups. In group I, 68 patients, TTT was performed first in the afternoon and then in the morning. In group II, 55 patients, the test was performed first in the morning and then in the afternoon. RESULTS: The TTT protocol was the prolonged passive test, without drug sensitization. Twenty-nine (23.5%) patients had a positive result in at least one of the periods. The positivity rate for each period was similar: 20 (16.2%) patients in the afternoon and 19 (15.4%) in the morning (p=1.000). Total reproducibility (positive/positive and negative/negative) was observed in 49 (89%) patients in group I and in 55 (81%) in group II. Reproducibility of the results was obtained in 94 (90.4%) patients with first negative tests but in 10 (34%) patients with first positive tests. CONCLUSION: TTT could be performed during any period of the day, and even in the 2 periods to enhance positivity. Considering the low reproducibility rate of the positive tests, serial TTT to evaluate therapeutic efficacy should be performed during the same period of the day.

PURPOSE: Upright tilt-table testing (UTT) is an useful method for identifying patients with neurocardiogenic syncope, but its role in the evaluation of therapeutic efficacy is controversial. The aim of this study was to determine the correlation between negative UTT after therapy introduction (acute efficacy) and symptom recurrence during follow-up (chronic efficacy). METHODS: We studied 56 severely symptomatic patients (age 27±19 years) with recurrent (7±12 episodes) neurocardiogenic syncope (positive UTT). Once empirical pharmacological therapy was initiated, all patients underwent another UTT (therapeutic evaluation test - TET). Therapy was not modified after TET results. The probability of symptom recurrence was analyzed with the Kaplan-Meier method and compared by log-rank test in patients with negative and positive TET. OBJECTIVE - To assess the diagnostic value, the characteristics, and feasibility of tilt-table testing in children and adolescents. METHODS - From August 1991 to June 1997, we retrospectively assessed 94 patients under the age of 18 years who had a history of recurring syncope and presyncope of unknown origin and who were referred for tilt-table testing. These patients were divided into 2 groups: group I (children) - 36 patients with ages ranging from 3 to 12 (mean of 9.19±2.31) years; group II (adolescents) - 58 patients with ages ranging from 13 to 18 (mean of 16.05±1.40) years. We compared the positivity rate, the type of hemodynamic response, and the time period required for the test to become positive in the 2 groups. RESULTS - The positivity rates were 41.6 % and 50% for groups I and II, respectively. The pattern of positive hemodynamic response that predominated in both groups was the mixed response. The mean time period required for the test to become positive was shorter in group I (11.0±7.23 min) than in group II (18.44±7.83 min). No patient experienced technical difficulty or complications. CONCLUSION - No difference was observed in regard to feasibility, positivity rate, and pattern of positive response for the tilt-table test in children and adolescents. Pediatric patients had earlier positive responses.

A Taquicardia Ventricular (TV) subepicárdica pode ocorrer em pacientes isquêmicos, nao isquêmicos e com TVs idiopática. Verdadeiras TVs subepicárdicas podem ser ablacionadas preferencialmente pela superfície epicárdica. A técnica de punçao subxifóidea para abordar o epicárdio é fácil e segura para ser realizada em laboratórios de eletrofisiologia por eletrofisiologistas e pode melhorar os resultados do procedimento de ablaçao por cateter.

Resumo Fundamento O diagnóstico diferencial de taquicardia de QRS largo, entre taquicardia ventricular (TV) ou taquicardia supraventricular com condução aberrante (TSV-A) é algumas vezes difícil de ser feito na sala de emergência. Objetivo Avaliar a acurácia de um algoritmo novo e simples para a detecção de TV no eletrocardiograma (ECG) em pacientes com taquicardia de QRS largo. Métodos ECGs de 12 derivações para detecção de taquicardia de QRS largo foram obtidos prospectivamente de 120 pacientes durante estudo eletrofisiológico. Seis médicos com diferentes experiências analisaram os ECGs, e fizeram o diagnóstico com base no algoritmo D12V16, que envolve a análise da polaridade predominante do complexo QRS nas derivações I, II, V1 e V6. O diagnóstico foi comparado com os obtidos pelo algoritmo tradicional de Brugada e pelo estudo eletrofisiológico, o qual é considerado padrão ouro. Adotou-se um nível de significância de 5% (p
