2. PACIENTE SIMULADO VERSUS SIMULADOR DE ALTA FIDELIDADE: SATISFAÇÃO, AUTOCONFIANÇA E CONHECIMENTO ENTRE ESTUDANTES DE ENFERMAGEM NO BRASIL

Objective: to assess and compare satisfaction, self-confidence and knowledge among Nursing students in the use of simulated patients and high-fidelity simulators. Method: a quasi-experimental study conducted in 2017 with 150 students from a private university The participants answered a pretest to assess knowledge and participated in a lecture and in skills training. After the simulation, the students answered the Satisfaction with Simulated Clinical Experiences scale and the Satisfaction and Self-confidence in Learning and Knowledge scale. The data were analyzed based on non-parametric tests. Results: the students showed high rates of satisfaction (93.5 and 92.5), self-confidence (4.3 and 4.4) and knowledge gain (6.2 and 6.4). There were no significant differences in the comparison between the simulated patient group and the high-fidelity simulator group regarding satisfaction, self-confidence and knowledge. Conclusion: the findings showed that well-prepared settings can be obtained with lower costs, and with an effective impact on the students’ learning.

Objective: To identify and evaluate the empathy of nursing students before and after a simulated clinical activity. Method: This is a quantitative and quasi-experimental study, carried out in the inland of the state of São Paulo. 107 nursing students participated in the study. A medium-fidelity clinical simulation scenario was built and validated. Empathy was evaluated through the Empathy Inventory before and after the simulated clinical activity. Results: There were positive changes after the simulated clinical activity in the cognitive components of empathy and there were no changes, in parts, in the affective component of empathy. Conclusion: The simulated clinical activity, well outlined in a validated clinical simulation scenario, was able to improve the cognitive component of empathy but was not susceptible to changes in the affective component of empathy, which transfigures into feelings of compassion and altruism.

RESUMO INTRODUÇÃO: a literatura demonstra que alunos que participam de atividades simuladas com experiência clínica prévia alcançam resultados menos positivos em simulação do que aqueles sem contato prévio com o campo clínico. OBJETIVO: identificar as implicações da prática clínica na satisfação e autoconfiança do estudante com atividades clínicas simuladas em medidas de conforto e higiene ao paciente usuário de fraldas descartáveis. MÉTODO: estudo quantitativo, quase-experimental realizado em uma universidade pública do interior do estado de São Paulo, no Brasil, utilizando dois instrumentos de avaliação da satisfação e autoconfiança do estudante com as práticas clínicas simuladas, a Escala de Satisfação e Autoconfiança no Aprendizado (ESAA) e a Escala de Satisfação com as Experiências Clínicas Simuladas (ESECS). RESULTADOS: participaram do estudo 100 estudantes de graduação em Enfermagem. Alunos com experiência clínica anterior tiveram valores mais significativos no item "autoconfiança"; estudantes sem experiência clínica para o item "realismo" das escalas. CONCLUSÃO: a simulação clínica é enriquecedora do ponto de vista do aprendizado, satisfação e autoconfiança do estudante, mas não dispensa a prática clínica real dos estudantes em campo clínico.

5. Emotional recognition for simulated clinical environment using unpleasant odors: quasi-experimental study

Objective: to compare the effect of exposure to unpleasant odors in a simulated clinical environment on the emotions of undergraduate nursing students. Method: quasi-experimental study. A total of 24 nursing students participated the study, divided into two groups, 12 in the intervention group with exposure to unpleasant odors, and 12 in the control group without exposure to unpleasant odors. To simulate the unpleasant vomiting odor in intervention group, fermented foods were used: boiled oats, curdled milk, spoiled Parmesan cheese, raw egg, pea soup, raisins and vinegar. Participants were filmed and the facial expression analysis was performed at six critical points: student approach; report of the complaint; clinical evaluation; and patient occurrence, intervention and reevaluation based on what was proposed by the Circumplex model of emotions recognition. Results: a total of 83,215 emotions related to the six critical points were verified. At the critical point of the proposed scenario with exposure to unpleasant odors, the intervention group presented the basic emotion of sadness and the Control Group, anger. Conclusion: it is inferred that the inclusion of unpleasant odors in the simulated scenarios can broaden the emotional development of health students.

RESUMO OBJETIVO: identificar a implicação da aplicação de recursos de som e imagem na avaliação de debriefing. MÉTODO: estudo quantitativo e experimental que inclui estudantes de graduação em Enfermagem com 18 anos ou mais, alocados de forma randomizada por grupo-intervenção (participação em debriefing com o uso de som e imagem) ou grupo-controle (participação em debriefing sem o uso de som e imagem), realizado em uma escola de Enfermagem do interior de São Paulo durante um workshop simulado, onde os alunos participaram de um cenário simulado de alta fidelidade. Para a coleta de dados foi utilizado o instrumento de caracterização sociodemográfica, a Escala de Experiência de Debriefing e a Avaliação de Debriefing associada à Simulação. Para análise dos dados foi utilizada estatística exploratória, com análise de frequência, porcentagem, teste de confiabilidade, análise amostral e teste de comparação de médias. RESULTADOS: participaram deste estudo 100 estudantes de graduação em Enfermagem. Apuraram-se valores elevados para a avaliação de debriefing e ressaltou-se a importância do papel do facilitador. No entanto, não se observaram resultados significativos para a avaliação de debriefing ao adicionar uma revisão de vídeo. CONCLUSÃO: estes resultados destacam a importância da reflexão durante a educação com base na simulação clínica e sugerem que a educação eficiente com simulação pode ser alcançada no debriefing mesmo quando a tecnologia de vídeo não está disponível.

Abstract Objective: The objective of this study was to construct and validate a low-cost simulator to train diabetes mellitus patients and their caregivers on the administration of insulin. Method: Action research study with a qualitative approach, through a validation methodological study. Results: A low-fidelity simulator was constructed with the areas recommended for insulin administration adapted to allow skill training. Validation was done by nine experts on the subject. Conclusions: The prototype has low cost and greater anatomical functional fidelity than the models currently available in the market. Implications for practice: The simulator developed can be used by health professionals, caregivers or by the patients themselves as a tool in the training for insulin administration. Its use may favor the identification of critical points related to the application technique, allowing the planning of more directive and effective educational interventions.

Abstract Objective: To describe the construction of a high-fidelity clinical scenario simulation of "Pressure injury patient nursing care". Method: Report the experience of constructing a high-fidelity clinical scenario simulation based on the literature and on Bloom's taxonomy. Learning objectives, prior study material, fidelity, clinical case, necessary resources, problem solving, face and content validity, debriefing and evaluation were considered. Results: The scenario included cognitive, psychomotor and affective aspects; role-playing and the moulage technique were used, and it was positively evaluated by the students. Conclusion: The construction of planned, structured and validated simulated clinical scenarios brings learners closer to the clinical practice, allowing the development of the skills required for the prevention and treatment of pressure injuries. Implications for practice: Well-designed simulated clinical settings simulate nursing care training with fidelity to patients with pressure injury.
