1. Patient-reported outcomes after robot-assisted radical prostatectomy and institutional learning curve for functional outcomes

Abstract: Purpose: The study was performed to examine patient-reported outcomes (PROs) in the 1st year after surgery and the institutional learning curve after the introduction of robot-assisted radical prostatectomy (RARP). Materials and Methods: The subjects were 320 consecutive patients who underwent RARP from 2014 to 2018. These cases were divided into three groups treated in the early, middle, and late periods, with about 100 cases in each. PROs were recorded using the Expanded Prostate Cancer Index Composite (EPIC). Results: There were no significant differences among the early, middle, and late periods based on EPIC scores. Urinary function and bother decreased in the 1st month after surgery, and gradually recovered thereafter. However, urinary function was significantly worse in the 1st year after surgery than at baseline. Urinary function and bother were better in patients treated with nerve-sparing surgery, and in nerve-sparing cases, urinary function and bother were best in the early period and worst in the late period. These cases also had the best score for sexual function in the early period, but sexual bother was worst in the early period. In contrast, in cases treated without nerve-sparing surgery, urinary function and bother were best in the late period and worst in the early period, although without significant differences. Conclusion: The functional results of this study based on PROs are useful for providing information for patients. Interestingly, the institutional learning curves for RARP differed in cases that did and did not undergo a nerve-sparing procedure.

Abstract: We report 850 μ m continuum polarization observations toward the filamentary high-mass star-forming region NGC 2264, taken as part of the B -fields In STar forming Regions Observations large program on the James Clerk Maxwell Telescope. These data reveal a well-structured nonuniform magnetic field in the NGC 2264C and 2264D regions with a prevailing orientation around 30° from north to east. Field strength estimates and a virial analysis of the major clumps indicate that NGC 2264C is globally dominated by gravity, while in 2264D, magnetic, gravitational, and kinetic energies are roughly balanced. We present an analysis scheme that utilizes the locally resolved magnetic field structures, together with the locally measured gravitational vector field and the extracted filamentary network. From this, we infer statistical trends showing that this network consists of two main groups of filaments oriented approximately perpendicular to one another. Additionally, gravity shows one dominating converging direction that is roughly perpendicular to one of the filament orientations, which is suggestive of mass accretion along this direction. Beyond these statistical trends, we identify two types of filaments. The type I filament is perpendicular to the magnetic field with local gravity transitioning from parallel to perpendicular to the magnetic field from the outside to the filament ridge. The type II filament is parallel to the magnetic field and local gravity. We interpret these two types of filaments as originating from the competition between radial collapsing, driven by filament self-gravity, and longitudinal collapsing, driven by the region's global gravity.

Abstract: The Galactic global magnetic field is thought to play a vital role in shaping Galactic structures such as spiral arms and giant molecular clouds. However, our knowledge of magnetic field structures in the Galactic plane at different distances is limited, as measurements used to map the magnetic field are the integrated effect along the line of sight. In this study, we present the first ever tomographic imaging of magnetic field structures in a Galactic spiral arm. Using optical stellar polarimetry over a $17^{\prime} \times 10^{\prime} $ field of view, we probe the Sagittarius spiral arm. Combining these data with stellar distances from the Gaia mission, we can isolate the contributions of five individual clouds along the line of sight by analyzing the polarimetry data as a function of distance. The observed clouds include a foreground cloud ( d < 200 pc) and four clouds in the Sagittarius arm at 1.23, 1.47, 1.63, and 2.23 kpc. The column densities of these clouds range from 0.5 to 2.8 × 10 ^21 cm ^−2 . The magnetic fields associated with each cloud show smooth spatial distributions within their observed regions on scales smaller than 10 pc and display distinct orientations. The position angles projected on the plane of the sky, measured from the Galactic north to the east, for the clouds in increasing order of distance are 135°, 46°, 58°, 150°, and 40°, with uncertainties of a few degrees. Notably, these position angles deviate significantly from the direction parallel to the Galactic plane.

Abstract: Abstract Background Currently, immunotherapy is indicated for patients with metastatic RCC or unresectable RCC, but there is no indication for immunotherapy in the neoadjuvant setting. We report a case in which the combined use of nivolumab and ipilimumab and sequential TKI therapy enabled surgical treatment. Case presentation A 71-year-old female was diagnosed with a metastatic clear-cell renal cell carcinoma with a level IV tumor thrombus. She was started on nivolumab-ipilimumab therapy, and was switched to pazopanib monotherapy because the tumor thrombus progressed within the right atrium. The tumor shrank to resectable status with sequential therapy. She then underwent right nephrectomy and thrombectomy. Pathological analysis showed 10–20% residual tumor in the primary tumor, but no viable cells in tumor thrombus. She remains clinically disease-free 1 year after surgery. Conclusion This case suggests the utility of sequential immune-targeted therapy as neoadjuvant therapy in advanced renal cell carcinoma.

Abstract: We present observations of polarized dust emission at 850 μ m from the L43 molecular cloud, which sits in the Ophiuchus cloud complex. The data were taken using SCUBA-2/POL-2 on the James Clerk Maxwell Telescope as a part of the BISTRO large program. L43 is a dense ( ${N}_{{{\rm{H}}}_{2}}\sim {10}^{22}$ –10 ^23 cm ^−2 ) complex molecular cloud with a submillimeter-bright starless core and two protostellar sources. There appears to be an evolutionary gradient along the isolated filament that L43 is embedded within, with the most evolved source closest to the Sco OB2 association. One of the protostars drives a CO outflow that has created a cavity to the southeast. We see a magnetic field that appears to be aligned with the cavity walls of the outflow, suggesting interaction with the outflow. We also find a magnetic field strength of up to ∼160 ± 30 μ G in the main starless core and up to ∼90 ± 40 μ G in the more diffuse, extended region. These field strengths give magnetically super- and subcritical values, respectively, and both are found to be roughly trans-Alfvénic. We also present a new method of data reduction for these denser but fainter objects like starless cores.

Abstract: We study the H ii regions associated with the NGC 6334 molecular cloud observed in the submillimeter and taken as part of the B -fields In STar-forming Region Observations Survey. In particular, we investigate the polarization patterns and magnetic field morphologies associated with these H ii regions. Through polarization pattern and pressure calculation analyses, several of these bubbles indicate that the gas and magnetic field lines have been pushed away from the bubble, toward an almost tangential (to the bubble) magnetic field morphology. In the densest part of NGC 6334, where the magnetic field morphology is similar to an hourglass, the polarization observations do not exhibit observable impact from H ii regions. We detect two nested radial polarization patterns in a bubble to the south of NGC 6334 that correspond to the previously observed bipolar structure in this bubble. Finally, using the results of this study, we present steps (incorporating computer vision; circular Hough transform) that can be used in future studies to identify bubbles that have physically impacted magnetic field lines.

Abstract: We present BISTRO Survey 850 μ m dust emission polarization observations of the L1495A-B10 region of the Taurus molecular cloud, taken at the James Clerk Maxwell Telescope (JCMT). We observe a roughly triangular network of dense filaments. We detect nine of the dense starless cores embedded within these filaments in polarization, finding that the plane-of-sky orientation of the core-scale magnetic field lies roughly perpendicular to the filaments in almost all cases. We also find that the large-scale magnetic field orientation measured by Planck is not correlated with any of the core or filament structures, except in the case of the lowest-density core. We propose a scenario for early prestellar evolution that is both an extension to, and consistent with, previous models, introducing an additional evolutionary transitional stage between field-dominated and matter-dominated evolution, observed here for the first time. In this scenario, the cloud collapses first to a sheet-like structure. Uniquely, we appear to be seeing this sheet almost face on. The sheet fragments into filaments, which in turn form cores. However, the material must reach a certain critical density before the evolution changes from being field dominated to being matter dominated. We measure the sheet surface density and the magnetic field strength at that transition for the first time and show consistency with an analytical prediction that had previously gone untested for over 50 yr.

Abstract: We observed a stellar occultation by (3200) Phaethon, which occurred in western Japan on 2021 October 3 (UTC). This observation was requested by the DESTINY+ mission team, which plans to conduct a flyby of asteroid Phaethon in 2028. Overall, this research effort contributes towards a large-scale observation campaign with a total of 72 observers observing from western Japan to southern Korea. 36 stations were established, and stellar occultation by the asteroid Phaethon was detected in 18 of them. This is the first time that this many multiple chord observations have been made for such a small asteroid (it has a diameter of 5–6 km). Observational reductions show that the apparent cross-section of Phaethon at the time of the occultation could be approximated using an ellipse with a major diameter of 6.12 ± 0.07 km and a minor diameter of 4.14 ± 0.07 km, and a position angle of 117.°4 ± 1.°5. As can be seen from the small error bars of the fitted ellipse, we have succeeded in estimating the shape and size of the asteroid with an extremely high degree of accuracy. Our observation results, together with other observations, will be used to create a 3D model of Phaethon and to improve its orbit. The instruments that we used for this observation are commonly used by many amateur astronomers in Japan for occultation observations and are not difficult to obtain. This paper describes the method and results of our observations using a CMOS camera and a GPS module, so that many people can participate in occultation observations in the future.

Abstract: Among early stage prostate cancer patients, intraductal carcinoma of the prostate (IDC-P) and invasive cribriform are key prognostic factors; however, their presence and clinical significance following active surveillance (AS) are unknown. In men who opted for AS, we aimed to examine the presence and impact of IDC-P or cribriform, utilizing radical prostatectomy (RP) specimens.We re-reviewed 137 RP specimens available in the PRIAS-JAPAN prospective cohort between January 2010 and September 2020. We assessed the presence of IDC-P or cribriform, and compared the patients' characteristics and prostate-specific antigen (PSA) recurrence-free survival after RP between groups with and without IDC-P or cribriform. In addition, we examined the predictive factors associated with IDC-P or cribriform.The percentage of patients with IDC-P or cribriform presence was 34.3% (47 patients). IDC-P or cribriform pattern was more abundant in the higher Gleason grade group in RP specimens (P 0.001). The rates of PSA recurrence-free survival were significantly lower in the IDC-P or cribriform groups than in those without them (log rank P = 0.0211). There was no association between IDC-P or cribriform on RP with the Prostate Imaging-Reporting and Data System (PI-RADS) 4,5 score on magnetic resonance imaging (MRI) before RP even with adjustments for other covariates (OR, 1.43; 95% confidence interval [CI] 0.511-3.980, P = 0.497).IDC-P or cribriform comprised approximately one-third of all RP specimens in men who underwent RP following AS, confirming their prognostic significance.

Abstract: Background Currently, immunotherapy is indicated for patients with metastatic RCC or unresectable RCC, but there is no indication for immunotherapy in the neoadjuvant setting. We report a case in which the combined use of nivolumab and ipilimumab and sequential TKI therapy enabled surgical treatment. Case presentation A 71-year-old female was diagnosed with a metastatic clear-cell renal cell carcinoma with a level IV tumor thrombus. She was started on nivolumab-ipilimumab therapy, and was switched to pazopanib monotherapy because the tumor thrombus progressed within the right atrium. The tumor shrank to resectable status with sequential therapy. She then underwent right nephrectomy and thrombectomy. Pathological analysis showed 10–20% residual tumor in the primary tumor, but no viable cells in tumor thrombus. She remains clinically disease-free 1 year after surgery. Conclusion This case suggests the utility of sequential immune-targeted therapy as neoadjuvant therapy in advanced renal cell carcinoma.

Abstract: Objectives This study aimed to evaluate whether oncological outcomes of radical prostatectomy differ depending on adherence to the criteria in patients who opt for active surveillance. Materials and methods We retrospectively reviewed the data of 1035 patients enrolled in a prospective cohort of the PRIAS-JAPAN study. After applying the exclusion criteria, 136 of 162 patients were analyzed. Triggers for radical prostatectomy due to pathological reclassification on repeat biopsy were defined as on-criteria. Off-criteria triggers were defined as those other than on-criteria triggers. Unfavorable pathology on radical prostatectomy was defined as pathological ≥T3, ≥GS 4 + 3 and pathological N positivity. We compared the pathological findings on radical prostatectomy and prostate-specific antigen recurrence-free survival between the two groups. The off-criteria group included 35 patients (25.7%), half of whom received radical prostatectomy within 35 months. Results There were significant differences in median prostate-specific antigen before radical prostatectomy between the on-criteria and off-criteria groups (6.1 vs. 8.3 ng/ml, P = 0.007). The percentage of unfavorable pathologies on radical prostatectomy was lower in the off-criteria group than that in the on-criteria group (40.6 vs. 31.4%); however, the differences were not statistically significant (P = 0.421). No significant difference in prostate-specific antigen recurrence-free survival was observed between the groups during the postoperative follow-up period (median: 36 months) (log-rank P = 0.828). Conclusions Half of the off-criteria patients underwent radical prostatectomy within 3 years of beginning active surveillance, and their pathological findings were not worse than those of the on-criteria patients.

Abstract: To compare the medical costs of active surveillance with those of robot-assisted laparoscopic prostatectomy, brachytherapy, intensity-modulated radiation therapy, and hormone therapy for low-risk prostate cancer.The costs of protocol biopsies performed in the first year of surveillance (between January 2010 and June 2020) and those of brachytherapy and radiation therapy performed between May 2019 and June 2020 at the Kagawa University Hospital were analyzed. Hormone therapy costs were assumed to be the costs of luteinizing hormone-releasing hormone analogs for over 5 years. Active surveillance-eligible patients were defined based on the following: age74 years, ≤T2, Gleason score ≤6, prostate-specific antigen level ≤10 ng/ml, and 1-2 positive cores. We estimated the total number of active surveillance-eligible patients in Japan based on the Japan Study Group of Prostate Cancer (J-CAP) study and the 2017 cancer statistical data. We then calculated the 5-year treatment costs of active surveillance-eligible patients using the J-CAP and PRIAS-JAPAN study data.In 2017, number of active surveillance-eligible patients in Japan was estimated to be 2808. The 5-year total costs of surveillance, prostatectomy, brachytherapy, radiation therapy, and hormone therapy were 1.65, 14.0, 4.61, 4.04, and 5.87 million United States dollar (USD), respectively. If 50% and 100% of the patients in each treatment group had opted for active surveillance as the initial treatment, the total treatment cost would have been reduced by USD 6.89 million (JPY 889 million) and USD 13.8 million (JPY 1.78 billion), respectively.Expanding active surveillance to eligible patients with prostate cancer helps save medical costs.

Abstract: We present 850 $��$m polarimetric observations toward the Serpens Main molecular cloud obtained using the POL-2 polarimeter on the James Clerk Maxwell Telescope (JCMT) as part of the B-fields In STar-forming Region Observations (BISTRO) survey. These observations probe the magnetic field morphology of the Serpens Main molecular cloud on about 6000 au scales, which consists of cores and six filaments with different physical properties such as density and star formation activity. Using the histogram of relative orientation (HRO) technique, we find that magnetic fields are parallel to filaments in less dense filamentary structures where $N_{H_2} < 0.93\times 10^{22}$ cm$^{-2}$ (magnetic fields perpendicular to density gradients), while being perpendicular to filaments (magnetic fields parallel to density gradients) in dense filamentary structures with star formation activity. Moreover, applying the HRO technique to denser core regions, we find that magnetic field orientations change to become perpendicular to density gradients again at $N_{H_2} \approx 4.6 \times 10^{22}$ cm$^{-2}$. This can be interpreted as a signature of core formation. At $N_{H_2} \approx 16 \times 10^{22}$ cm$^{-2}$ magnetic fields change back to being parallel to density gradients once again, which can be understood to be due to magnetic fields being dragged in by infalling material. In addition, we estimate the magnetic field strengths of the filaments ($B_{POS} = 60-300~��$G)) using the Davis-Chandrasekhar-Fermi method and discuss whether the filaments are gravitationally unstable based on magnetic field and turbulence energy densities., 18 pages, accepted for publication in ApJ

Abstract: Tao-Chung Ching et al., We present 850 μm dust polarization observations of the massive DR21 filament from the B-fields In STar-forming Region Observations (BISTRO) survey, using the POL-2 polarimeter and the SCUBA-2 camera on the James Clerk Maxwell Telescope. We detect ordered magnetic fields perpendicular to the parsec-scale ridge of the DR21 main filament. In the subfilaments, the magnetic fields are mainly parallel to the filamentary structures and smoothly connect to the magnetic fields of the main filament. We compare the POL-2 and Planck dust polarization observations to study the magnetic field structures of the DR21 filament on 0.1–10 pc scales. The magnetic fields revealed in the Planck data are well-aligned with those of the POL-2 data, indicating a smooth variation of magnetic fields from large to small scales. The plane-of-sky magnetic field strengths derived from angular dispersion functions of dust polarization are 0.6–1.0 mG in the DR21 filament and ∼0.1 mG in the surrounding ambient gas. The mass-to-flux ratios are found to be magnetically supercritical in the filament and slightly subcritical to nearly critical in the ambient gas. The alignment between column density structures and magnetic fields changes from random alignment in the low-density ambient gas probed by Planck to mostly perpendicular in the high-density main filament probed by James Clerk Maxwell Telescope. Abstract: Hwang et al., We present and analyze observations of polarized dust emission at 850 μm toward the central 1 × 1 pc hub-filament structure of Monoceros R2 (Mon R2). The data are obtained with SCUBA-2/POL-2 on the James Clerk Maxwell Telescope (JCMT) as part of the B-fields in Star-forming Region Observations survey. The orientations of the magnetic field follow the spiral structure of Mon R2, which are well described by an axisymmetric magnetic field model. We estimate the turbulent component of the magnetic field using the angle difference between our observations and the best-fit model of the underlying large-scale mean magnetic field. This estimate is used to calculate the magnetic field strength using the Davis–Chandrasekhar–Fermi method, for which we also obtain the distribution of volume density and velocity dispersion using a column density map derived from Herschel data and the C18O (J = 3 − 2) data taken with HARP on the JCMT, respectively. We make maps of magnetic field strengths and mass-to-flux ratios, finding that magnetic field strengths vary from 0.02 to 3.64 mG with a mean value of 1.0 ± 0.06 mG, and the mean critical mass-to-flux ratio is 0.47 ± 0.02. Additionally, the mean Alfvén Mach number is 0.35 ± 0.01. This suggests that, in Mon R2, the magnetic fields provide resistance against large-scale gravitational collapse, and the magnetic pressure exceeds the turbulent pressure. We also investigate the properties of each filament in Mon R2. Abstract: (Objectives) Investigation of patient-reported outcome (PRO) after ileal neobladder for bladder cancer. (Patients and methods) Forty patients who underwent radical cystectomy and ileal neobladder between July 2007 and December 2017 were asked to complete a questionnaire. PRO was evaluated using the Medical Outcomes Study Short Form-8 (SF-8) for general health-related quality of life (QOL) and the Bladder Cancer Index and the Japanese version of the Functional Assessment of Cancer Therapy-Bladder for disease specific QOL. Patients were divided into two groups according to the postoperative period (early and late), and the differences in questionnaire results between the groups were evaluated. (Results) All scores on the SF-8 were over 50 points, exceeding the Japanese national standard of 50 points for all items. The physical and mental summary scores were also favorable. For disease-specific QOL, only daytime incontinence was significantly higher in the late group, with daytime incontinence two or more times per week at 42.1%, compared to 20.0% in the early group (P=0.0403). A comparison of daytime urinary incontinence in four groups during the postoperative period showed that urinary incontinence was least frequent among patients between 64 and 101 months after surgery; patients in this group were also the youngest in age at the time of surgery. (Conclusions) Cross-sectional analysis of PRO revealed that patients with ileal neobladder had good general health-related QOL after surgery, but urinary incontinence was a significant problem.

Abstract: We present the results of simultaneous 450 $\mu$m and 850 $\mu$m polarization observations toward the massive star forming region NGC 2071IR, a target of the BISTRO (B-fields in Star-Forming Region Observations) Survey, using the POL-2 polarimeter and SCUBA-2 camera mounted on the James Clerk Maxwell Telescope. We find a pinched magnetic field morphology in the central dense core region, which could be due to a rotating toroidal disk-like structure and a bipolar outflow originating from the central young stellar object, IRS 3. Using the modified Davis-Chandrasekhar-Fermi method, we obtain a plane-of-sky magnetic field strength of 563$\pm$421 $\mu$G in the central $\sim$0.12 pc region from 850 $\mu$m polarization data. The corresponding magnetic energy density of 2.04$\times$10$^{-8}$ erg cm$^{-3}$ is comparable to the turbulent and gravitational energy densities in the region. We find that the magnetic field direction is very well aligned with the whole of the IRS 3 bipolar outflow structure. We find that the median value of polarization fractions, 3.0 \%, at 450 $\mu$m in the central 3 arcminute region, which is larger than the median value of 1.2 \% at 850 $\mu$m. The trend could be due to the better alignment of warmer dust in the strong radiation environment. We also find that polarization fractions decrease with intensity at both wavelengths, with slopes, determined by fitting a Rician noise model, of $0.59 \pm 0.03$ at 450 $\mu$m and $0.36 \pm 0.04$ at 850 $\mu$m, respectively. We think that the shallow slope at 850 $\mu$m is due to grain alignment at the center being assisted by strong radiation from the central young stellar objects., Comment: 17 pages, 11 figures, 4 tables

Abstract: We have obtained sensitive dust continuum polarization observations at 850 μm in the B213 region of Taurus using POL-2 on SCUBA-2 at the James Clerk Maxwell Telescope as part of the B-fields in STar-forming Region Observations (BISTRO) survey. These observations allow us to probe magnetic field (B-field) at high spatial resolution (∼2000 au or ∼0.01 pc at 140 pc) in two protostellar cores (K04166 and K04169) and one prestellar core (Miz-8b) that lie within the B213 filament. Using the Davis–Chandrasekhar–Fermi method, we estimate the B-field strengths in K04166, K04169, and Miz-8b to be 38 ± 14, 44 ± 16, and 12 ± 5 μG, respectively. These cores show distinct mean B-field orientations. The B-field in K04166 is well ordered and aligned parallel to the orientations of the core minor axis, outflows, core rotation axis, and large-scale uniform B-field, in accordance with magnetically regulated star formation via ambipolar diffusion taking place in K04166. The B-field in K04169 is found to be ordered but oriented nearly perpendicular to the core minor axis and large-scale B-field and not well correlated with other axes. In contrast, Miz-8b exhibits a disordered B-field that shows no preferred alignment with the core minor axis or large-scale field. We found that only one core, K04166, retains a memory of the large-scale uniform B-field. The other two cores, K04169 and Miz-8b, are decoupled from the large-scale field. Such a complex B-field configuration could be caused by gas inflow onto the filament, even in the presence of a substantial magnetic flux.

Abstract: Optical stellar polarimetry in the Perseus molecular cloud direction is known to show a fully mixed bi-modal distribution of position angles across the cloud (Goodman et al. 1990). We study the Gaia trigonometric distances to each of these stars and reveal that the two components in position angles trace two different dust clouds along the line of sight. One component, which shows a polarization angle of -37.6 deg +/- 35.2 deg and a higher polarization fraction of 2.0 +/- 1.7%, primarily traces the Perseus molecular cloud at a distance of 300 pc. The other component, which shows a polarization angle of +66.8 deg +/- 19.1 deg and a lower polarization fraction of 0.8 +/- 0.6%, traces a foreground cloud at a distance of 150 pc. The foreground cloud is faint, with a maximum visual extinction of < 1 mag. We identify that foreground cloud as the outer edge of the Taurus molecular cloud. Between the Perseus and Taurus molecular clouds, we identify a lower-density ellipsoidal dust cavity with a size of 100 -- 160 pc. This dust cavity locates at l = 170 deg, b = -20 deg, and d = 240 pc, which corresponds to an HI shell generally associated with the Per OB2 association. The two-component polarization signature observed toward the Perseus molecular cloud can therefore be explained by a combination of the plane-of-sky orientations of the magnetic field both at the front and at the back of this dust cavity., Accepted for publication in ApJ

Abstract: We report the first high spatial resolution measurement of magnetic fields surrounding LkH$\alpha$ 101, a part of the Auriga-California molecular cloud. The observations were taken with the POL-2 polarimeter on the James Clerk Maxwell Telescope within the framework of the B-fields In Star-forming Region Observations (BISTRO) survey. Observed polarization of thermal dust emission at 850 $\mu$m is found to be mostly associated with the red-shifted gas component of the cloud. The magnetic field displays a relatively complex morphology. Two variants of the Davis-Chandrasekhar-Fermi method, unsharp masking and structure function, are used to calculate the strength of magnetic fields in the plane of the sky, yielding a similar result of $B_{\rm POS}\sim 115$ $\mathrm{\mu}$G. The mass-to-magnetic-flux ratio in critical value units, $\lambda\sim0.3$, is the smallest among the values obtained for other regions surveyed by POL-2. This implies that the LkH$\alpha$ 101 region is sub-critical and the magnetic field is strong enough to prevent gravitational collapse. The inferred $\delta B/B_0\sim 0.3$ implies that the large scale component of the magnetic field dominates the turbulent one. The variation of the polarization fraction with total emission intensity can be fitted by a power-law with an index of $\alpha=0.82\pm0.03$, which lies in the range previously reported for molecular clouds. We find that the polarization fraction decreases rapidly with proximity to the only early B star (LkH$\alpha$ 101) in the region. Abstract: We present new observations of the active star formation region NGC 1333 in the Perseus molecular cloud complex from the James Clerk Maxwell Telescope B -Fields In Star-forming Region Observations (BISTRO) survey with the POL-2 instrument. The BISTRO data cover the entire NGC 1333 complex (∼1.5 pc × 2 pc) at 0.02 pc resolution and spatially resolve the polarized emission from individual filamentary structures for the first time. The inferred magnetic field structure is complex as a whole, with each individual filament aligned at different position angles relative to the local field orientation. We combine the BISTRO data with low- and high- resolution data derived from Planck and interferometers to study the multiscale magnetic field structure in this region. The magnetic field morphology drastically changes below a scale of ∼1 pc and remains continuous from the scales of filaments (∼0.1 pc) to that of protostellar envelopes (∼0.005 pc or ∼1000 au). Finally, we construct simple models in which we assume that the magnetic field is always perpendicular to the long axis of the filaments. We demonstrate that the observed variation of the relative orientation between the filament axes and the magnetic field angles are well reproduced by this model, taking into account the projection effects of the magnetic field and filaments relative to the plane of the sky. These projection effects may explain the apparent complexity of the magnetic field structure observed at the resolution of BISTRO data toward the filament network., Erratum published in volume 899, issue 1, 5 p., April 10 2021. Abstract: Patients with favorable-risk prostate cancer on active surveillance (AS) are strictly followed for safer execution. Repeat protocol biopsy is essential for evaluating cancer aggressiveness. However, the acceptance rate of repeat biopsy is not high enough because of the burdens of biopsy. We assessed the impact of complications after the initial biopsy on repeat protocol biopsy at 1 year using data from the Prostate Cancer Research International: Active Surveillance (PRIAS)-JAPAN study. We performed a retrospective analysis using a prospective cohort in the PRIAS-JAPAN study. Patients with favorable-risk prostate cancer (n = 856) who consented to participate in the PRIAS-JAPAN study from 2010 to 2018 were enrolled. Follow-up evaluations included regular prostate-specific antigen, digital rectal examination and biopsy. Rates of complications after biopsies and repeat protocol biopsy non-acceptance rate at 1 year were reported. Logistic regression analysis explored the association between the complications after the initial biopsy and repeat protocol biopsy non-acceptance. Altogether, 759 patients (88.7%) actually proceeded to protocol at 1 year. Repeat protocol biopsy non-acceptance rate at 1 year was 14.9%. Abstract: [Abridged] Filaments and hubs have received special attention recently thanks to studies showing their role in star formation. While the column density and velocity structures of both filaments and hubs have been studied, their magnetic fields (B-field) are not yet characterized. We aim to understand the role of the B-field in the dynamical evolution of the NGC 6334 hub-filament network. We present new observations of the dust polarized emission at 850$\mu$m towards NGC 6334 obtained with the JCMT/POL-2. We study the distribution and dispersion of the polarized intensity ($PI$), the polarization fraction ($PF$), and the B-field angle ($\theta_{B}$). We derive the power spectrum of the intensity and $\theta_{B}$ along the ridge crest. Our analyses show a complex B-field structure when observed over the whole region ($\sim10$ pc), however, at smaller scales ($\sim1$ pc), $\theta_{B}$ varies coherently along the filaments. The observed power spectrum of $\theta_{B}$ can be well represented with a power law function with a slope $-1.33\pm0.23$, which is $\sim20\%$ shallower than that of $I$. This result is compatible with the properties of simulated filaments and may indicate the processes at play in the formation of filaments. $\theta_{B}$ rotates from being mostly perpendicular to the filament crests to mostly parallel as they merge with the hubs. This variation of $\theta_{B}$ may be tracing local velocity flows of matter in-falling onto the hubs. Our analysis suggests a variation of the energy balance along the crests of these filaments, from magnetically critical/supercritical at their far ends to magnetically subcritical near the hubs. We detect an increase of $PF$ towards the high-column density star cluster-forming hubs that may result from the increase of grain alignment efficiency due to stellar radiation from the newborn stars., Comment: Accepted for publication in Astronomy & Astrophysics

Abstract: 253 Background: An international staging collaboration for cancer of the prostate (STAR-CAP) was recently established and used for predicting oncological outcomes for patients with prostate cancer. However, the validation of STAR-CAP has not yet been performed in the Japanese cohort. Our study aimed to assess the STAR-CAP prognostic model for Japanese patients with prostate cancer and compare it to the CAPRA score for the prediction of biochemical recurrence (BCR) after radical prostatectomy. Methods: We retrospectively analyzed data from a multi-institution database of 1,645 patients who underwent radical prostatectomy between 2010 and 2020. We excluded patients with inadequate follow-up duration ( < 1 year), missing or incomplete perioperative clinical parameters relevant for STAR-CAP, and patients who received neoadjuvant therapy. BCR was defined as two consecutive prostate-specific antigen values ≥ 0.2 ng/mL. The 3-year BCR-free survival was assessed by Kaplan-Meier analyses and log-rank test. The performance of both models to predict 3-year BCR-free survival was assessed using the area under the curve (AUC). Results: The 3-year BCR-free survival ranged from 47.3% (high risk) to 93.5% (low risk) in the STAR-CAP classification ( p< 0.001) and from 62.5% (high risk) to 94.6% (low risk) in the CAPRA classification ( p< 0.001). AUCs for the 3-year BCR prediction were 0.756 and 0.784 for STAR-CAP and CAPRA, respectively. Conclusions: In Japanese patients, both STAR-CAP and CAPRA prognostic systems provide sufficient stratification and their predictive ability for 3-year BCR-free survival is comparable, with a small advantage for CAPRA (2.8%).

Abstract: We present the 850 $\mu$m polarization observations toward the IC5146 filamentary cloud taken using the Submillimetre Common-User Bolometer Array 2 (SCUBA-2) and its associated polarimeter (POL-2), mounted on the James Clerk Maxwell Telescope (JCMT), as part of the B-fields In STar forming Regions Observations (BISTRO). This work is aimed at revealing the magnetic field morphology within a core-scale ($\lesssim 1.0$ pc) hub-filament structure (HFS) located at the end of a parsec-scale filament. To investigate whether or not the observed polarization traces the magnetic field in the HFS, we analyze the dependence between the observed polarization fraction and total intensity using a Bayesian approach with the polarization fraction described by the Rice likelihood function, which can correctly describe the probability density function (PDF) of the observed polarization fraction for low signal-to-noise ratio (SNR) data. We find a power-law dependence between the polarization fraction and total intensity with an index of 0.56 in $A_V\sim$ 20--300 mag regions, suggesting that the dust grains in these dense regions can still be aligned with magnetic fields in the IC5146 regions. Our polarization maps reveal a curved magnetic field, possibly dragged by the contraction along the parsec-scale filament. We further obtain a magnetic field strength of 0.5$\pm$0.2 mG toward the central hub using the Davis-Chandrasekhar-Fermi method, corresponding to a mass-to-flux criticality of $\sim$ $1.3\pm0.4$ and an Alfv\'{e}nic Mach number of $, Comment: 24 pages, 15 figures, accepted for publication in ApJ

Abstract: In the published article, we presented high-resolution polarimetry data obtained by using JCMT SCUBA-2/POL-2, and compared them with the larger-scale magnetic structure observed by Planck (Planck Collaboration et al. 2020). There was a miscalculation in the analysis of the Planck data for comparison, and the mean position angle of the Planck magnetic field should be corrected from -40° ± 7° to -48° ± 6°. Thus, we replace the descriptions in the published article as follows. In Section 4.2, paragraph 3, the second sentence should read: "The differences between the orientations of IRAS 4A, IRAS 4B, and IRAS 2A are not statistically significant, but they do show significantly different orientations from those of the global B-field observed by Planck (-48° ± 6°; Section 4.1)." In Section 4.1, paragraph 3, the second line should read: "The Planck B-field orientation shows a smoothly and slowly varying field distribution with a position angle of -48° ± 6° in our observed NGC 1333 area." Accordingly, we replace Figures 4, 5, 9, 10, and 17 to reflect the correct Planck data. As described above, this error of modest magnitude is related only to our derivation of the Planck polarization angle, and the JCMT observation results are unaffected. Therefore, all conclusions drawn in the published article are unchanged even after the above correction is applied. The 1 pc scale magnetic field observed by Planck shows a smooth distribution, and the interstellar magnetic field in molecular clouds increases the complexity significantly on the scale of less than 1 pc.

Abstract: 153 Background: 44 Japanese institutions have joined the Prostate Cancer Research International: Active Surveillance (PRIAS) study as PRIAS-JAPAN. As an side study, PRIAS-JAPAN investigates the usefulness of [-2] proPSA (p2PSA), a precursor of PSA, as a predictive marker for the reclassification during active surveillance patients. In this study, we aimed to evaluate the usefulness of p2PSA related parameters to predict the reclassification of first year protocol biopsy (1yr PBx). Methods: 19 institutions participated in this side study which started in January 2013. All participants were eligible for PRAIS inclusion criteria. These patients were required to receive blood sampling on every visit and protocol biopsy after inclusion. PSA and PSA isoforms (free-PSA, p2PSA) were measured in serum samples and parameters (%free-PSA, %p2PSA, phi) were also calculated. Mann-Whitney U test was used to compare continuous variables between reclassification and non-reclassification groups. Fisher's exact test and Chi-squared test with yates' continuity correction were used for categorical variables. Multivariable logistic regression models were used to predict the risk of reclassification. To assess the predictive power for reclassification, we plotted ROC curves and calculated the AUC. Decision curve analysis (DCA) was used to evaluate whether adding variables to the baseline (age, PSA, prostate volume) model would yield net clinical benefit. Results: From January 2013 to July 2017, this study enrolled 237patients. 99 patients were excluded for the reason of discontinuation of AS, non-adherence of protocol blood sampling and rejection of biopsy. 138patients were able to analyzed PSA, PSA isoforms and its parameters at the time of inclusion and 1yr PBx. Patients who showed reclassification were significantly higher level of %p2PSA at inclusion and %p2PSA and phi just before 1yr PBx. Multivariate analyses showed %p2PSA at inclusion (p= 0.04), %p2PSA (p= 0.01) and Phi (p= 0.008) before 1yr PBx were statistically significant predictors of reclassification at 1yr PBx. ROC analysis revealed that %p2PSA before 1yr PBx (AUC 0.674) and phi before 1yr PBx (AUC 0.669) were a significant discriminant factor in predicting reclassification. DCA analysis showed that phi before 1yr PBx had a highest net benefit in the range of clinically plausible risk threshold (10–40%). Conclusions: %p2PSA and Phi before 1yr PBx have good prediction power and phi is a most useful indicator of clinical decision making about active surveillance. Various pathophysiologies can cause the deterioration of renal function; however, the immune system plays important roles in promoting and protecting renal tissues. Receptor activator of NFκB ligand (RANKL) is a member of the TNF superfamily and is produced by bone-forming osteoblasts; the receptor for RANKL is called RANK. In bone biology, RANKL-RANK signaling has been extensively studied, but its roles in the immune system are still obscure. We investigated the role of the RANK system in I/R injury of the kidney using an experimental mouse I/R model. The left renal pedicles of 10-week-old male mice were clamped for 60 min, and reperfusion and right nephrectomy were simultaneously performed. Separate groups were administered an anti-RANKL antibody and recombinant RANKL (rRANKL) 24 h prior to I/R. After reperfusion for a set period of time, the serum creatinine level was measured, and the left kidney was removed for histological examination and western blotting to evaluate the expression and localization of RANK-associated molecules and cytokines. The serum creatinine levels were significantly elevated after I/R injury. A time-dependent increase in RANKL was observed up to 24 h, whereas RANK was induced for 12 h after reperfusion. RANK was expressed in infiltrating inflammatory cells, which were positive for CD68, a marker of monocytes/macrophages. The pre-treatment with the anti-RANKL antibody significantly impaired renal function and increased the induction of inflammatory cytokines (TNFα and IL-6), Toll-like receptor (TLR4) and MyD88 (all p .05) compared to the levels in the I/R group. However, rRANKL significantly improved renal function and decreased the levels of inflammatory cytokines (TNFα and IL-6), TLR4 and MyD88 (p .05) compared to those in the I/R group. The present study is the first to evaluate the role of the RANK system in renal I/R injury. RANKL-RANK signaling affects macrophage function and results in the downregulation of TLR4 and reduction in TNFα and IL-6, leading to improved renal function following I/R injury.

Abstract: Disuse atrophy of swallowing-related organs due to an excessive decrease in swallowing frequency is suspected to occur in patients with poor oral intake, especially elderly people. However, swallowing frequency in daily life has not previously been examined in the elderly. This study examined swallowing frequency in elderly people and compared these findings to those in a younger population and differences in the degree of activity in daily life. (i) We compared swallowing frequency in 20 elderly people (82·0 ± 8·3 year) and 15 healthy young people (26·5 ± 3·5 year). (ii) 20 elderly people were divided into two groups according to the degree of activity in daily life: a semi-bedridden group and bedridden group; the swallowing frequency was compared between these groups. (i) The swallowing frequency in the elderly people was 2-19 times per hour and the mean was 9·4 ± 4·9, and that in the healthy young people was 16-76 times per hour and the mean was 40·7 ± 19·5. Swallowing frequency in elderly people was significantly lower than that in young healthy people (P < 0·0001). (ii) The swallowing frequency in bedridden group was 2-11 times per hour and the mean was 6·8 ± 3·3, and that in semi-bedridden group was 3-19 times per hour and the mean was 11·9 ± 5·1. Swallowing frequency in bedridden group was significantly lower than that in semi-bedridden group (P < 0·05). These results indicate that in daily life, elderly people tend to swallow less frequently than young people. In addition, swallowing frequency was lower in elderly subjects with a low degree of activity in daily life.
