Background and Purpose: Pathogenic variants of the glycyl‐tRNA synthetase 1 (GARS1) gene have been described as a cause of Charcot–Marie–Tooth disease type 2D, motor axonal neuropathy with upper limb predominance (distal hereditary motor neuropathy [dHMN] type V), and infantile spinal muscular atrophy. Methods: This cross‐sectional, retrospective, observational study was carried out on 12 patients harboring the c.794C>T (p.Ser265Phe) missense pathogenic variant in GARS1. The patients' clinical data, nerve conduction studies, magnetic resonance imaging (MRI), and intraepidermal nerve fiber density in skin biopsies were reviewed. Results: The mean age at onset was 9.5 years; the intrinsic hand muscles were affected before or at the same time as the distal leg musculature. The clinical examination revealed greater weakness of the distal muscles, with a more pronounced involvement of the thenar complex and the first dorsal interosseous in upper limbs. Electrophysiological studies were concordant with an exclusively motor axonal neuropathy. A pathologic split hand index was found in six patients. Muscle MRI showed predominant fatty infiltration and atrophy of the anterolateral and superficial posterior compartment of the legs. Most patients reported distal pinprick sensory loss. A reduced intraepidermal nerve fiber density was evident in skin biopsies from proximal and distal sites in nine patients. Conclusions: GARS1 variants may produce a dHMN phenotype with "split hand" and sensory disturbances, even when sensory nerve conduction studies are normal. This could be explained by a dysfunction of sensory neurons in the dorsal ganglion that is reflected as a reduction of dermal nerve endings in skin biopsies without a distal gradient. [ES] La integración del vehículo eléctrico (EV) es una pieza clave en la transición energética. Sin embargo, una alta penetración del EV supone un incremento de la demanda eléctrica con posibles consecuencias negativas sobre las redes de distribución. Para evitarlo, es necesario digitalizar las infraestructuras de recarga para ofrecer mecanismos de carga controlada con el objetivo de minimizar el impacto en la red y gestionar de manera activa y eficiente los recursos. En este artículo se presenta un piloto real de recarga con mecanismos de monitorización y de control de carga, en el que se muestran las ventajas de tener una infraestructura monitorizada, digitalizada y gestionada, así como los efectos que esto tiene sobre la red., La elaboración de este artículo se llevó a cabo en el marco de la red de Centros de Excelencia HySGrid+, con el título Desarrollo de comunidades locales energéticamente positivas con sistemas híbridos de generación renovable y almacenamiento financiado por el Ministerio de Ciencia, Innovación y Universidades a través del organismo CDTI, en el programa CER-20191019. Purpose: Several hundred genetic muscle diseases have been described, all of which are rare. Their clinical and genetic heterogeneity means that a genetic diagnosis is challenging. We established an international consortium, MYO-SEQ, to aid the work-ups of muscle disease patients and to better understand disease etiology. Methods: Exome sequencing was applied to 1001 undiagnosed patients recruited from more than 40 neuromuscular disease referral centers ; standardized phenotypic information was collected for each patient. Exomes were examined for variants in 429 genes associated with muscle conditions. Results: We identified suspected pathogenic variants in 52% of patients across 87 genes. We detected 401 novel variants, 116 of which were recurrent. Variants in CAPN3, DYSF, ANO5, DMD, RYR1, TTN, COL6A2, and SGCA collectively accounted for over half of the solved cases ; while variants in newer disease genes, such as BVES and POGLUT1, were also found. The remaining well-characterized unsolved patients (48%) need further investigation. Conclusion: Using our unique infrastructure, we developed a pathway to expedite muscle disease diagnoses. Our data suggest that exome sequencing should be used for pathogenic variant detection in patients with suspected genetic muscle diseases, focusing first on the most common disease genes described here, and subsequently in rarer and newly characterized disease genes. .

[ES] El presente estudio tiene como objetivo la optimización energética en el proceso de producción de agua caliente en la empresa valenciana Mercavalencia S.A., logrando con ello una reducción de consumos y costes. Se procederá pues a la propuesta y valoración de diferentes alternativas de mejora de la eficiencia energética. El proceso actual de producción de agua caliente en la empresa se realiza mediante una caldera cuyo combustible es el gas natural. Es por ello que las distintas medidas se centrarán en la disminución del consumo de este combustible, conllevando a un ahorro económico y una menor contribución al efecto invernadero. - Abstract
El presente trabajo tiene como objetivo el realizar una revisión acerca del tratamiento del dolor, la optimización de la sedación y la presencia del delirium en la Unidad de Cuidados Intensivos. Está basada tanto en la experiencia profesional de los autores como en la revisión de la literatura y las últimas guías de recomendaciones de práctica clínica. La finalidad última de esta revisión es dar a conocer la importancia que tiene la optimización del tratamiento del dolor y la sedación en el pronóstico y evolución del paciente crítico, así como sensibilizar en la importancia de la identificación, prevención y tratamiento del delirium en este tipo de pacientes. - Abstract
[ES] La integración del vehículo eléctrico (EV) es una pieza clave en la transición energética. Sin embargo, una alta penetración del EV supone un incremento de la demanda eléctrica con posibles consecuencias negativas sobre las redes de distribución. Para evitarlo, es necesario digitalizar las infraestructuras de recarga para ofrecer mecanismos de carga controlada con el objetivo de minimizar el impacto en la red y gestionar de manera activa y eficiente los recursos. En este artículo se presenta un piloto real de recarga con mecanismos de monitorización y de control de carga, en el que se muestran las ventajas de tener una infraestructura monitorizada, digitalizada y gestionada, así como los efectos que esto tiene sobre la red.

BACKGROUND: Olive‐derived antioxidants have been shown to affect the oxidative status of meat and have also been associated with greater consumption of glucose, which might affect glycogen stores and muscle characteristics. This study evaluated the effect of oleuropein extract supplementation (OLE) versus vitamin E + Se (VE), and their combination (VEOLE), in pig diets, on pH, drip loss, the proportion of free fatty acids, and meat stability, and their prediction by blood oxidative status markers. RESULTS: The drip loss of muscle was lower in antioxidant‐supplemented groups when compared with controls. α‐Tocopherol concentration and total fatty acids profile were not affected by dietary oleuropein supplementation. However, OLE and VEOLE had lower free n‐3 polyunsaturated fatty acid (PUFA) levels when compared with VE and tended to have higher free monounsaturated fatty acid (MUFA) levels. Furthermore, the VEOLE group had lower free n‐6 PUFA levels when compared with controls or VE, whereas the OLE group had intermediated values. Muscle samples from pigs subjected to the antioxidant‐mixed supplementation (VEOLE) had lower malondialdehyde concentration when compared with the others. The VE and OLE groups showed intermediate malondialdehyde values. Chilled meat stability was highly correlated with antioxidant status in vivo. CONCLUSION: The administration of 96 mg oleuropein kg−1 feed produced similar meat quality characteristics as the use of 100 mg kg−1α‐tocopheryl acetate +0.26 mg kg−1 sodium selenite and it would be an interesting alternative in Mediterranean countries. The VEOLE group was the most effective for reducing lipid oxidation and for the production of polyunsaturated free fatty acids in meat, which would result in lower rancidity formation and better aroma development in products. © 2020 Society of Chemical Industry [ABSTRACT FROM AUTHOR]

Background: Laing myopathy is characterized by broad clinical and pathological variability. They are limited in number and protocol of study. We aimed to delineate muscle imaging profiles and validate imaging analysis as an outcome measure. Methods: This was a cross‐sectional and longitudinal cohort study. Data from clinical, functional and semi‐quantitative muscle imaging (60 magnetic resonance imaging [MRI] and six computed tomography scans) were studied. Hierarchical analysis, graphic heatmap representation and correlation between imaging and clinical data using Bayesian statistics were carried out. Results: The study cohort comprised 42 patients from 13 families harbouring five MYH7 mutations. The cohort had a wide range of ages, age at onset, disease duration, and myopathy extension and Gardner‐Medwin and Walton (GMW) functional scores. Intramuscular fat was evident in all but two asymptomatic/pauci‐symptomatic patients. Anterior leg compartment muscles were the only affected muscles in 12% of the patients. Widespread extension to the thigh, hip, paravertebral and calf muscles and, less frequently, the scapulohumeral muscles was commonly observed, depicting distinct patterns and rates of progression. Foot muscles were involved in 40% of patients, evolving in parallel to other regions with absence of a disto‐proximal gradient. Whole cumulative imaging score, ranging from 0 to 2.9 out of 4, was associated with disease duration and with myopathy extension and GMW scales. Follow‐up MRI studies in 24 patients showed significant score progression at a variable rate. Conclusions: We confirmed that the anterior leg compartment is systematically affected in Laing myopathy and may represent the only manifestation of this disorder. However, widespread muscle involvement in preferential but variable and not distance‐dependent patterns was frequently observed. Imaging score analysis is useful to categorize patients and to follow disease progression over time. [ABSTRACT FROM AUTHOR]

En els darrers anys la Seguretat del Pacient (SP) s’ha convertit en una de les pedres angulars de la Qualitat Assistencial. La Medicina inclou processos assistencials (PA) cada vegada més complexos, que juntament amb la tecnologia i la interacció humana, augmenten el risc de produir Esdeveniments Adversos (EA). El Servei de Medicina Intensiva (SMI) és una de les àrees de l’hospital on la possibilitat que es produeixin EA és més elevada. La Qualitat Assistencial compta amb recursos per la gestió del risc dels PA, que contribueixen a la seguretat del pacient. L’ Anàlisi Modal de Fallades i Efectes (AMFE) és una eina prospectiva i sistemàtica, que permet identificar, analitzar, avaluar i prevenir els EA que es podrien donar en els PA, definint 3 coeficients de puntuació: gravetat, la probabilitat que es produeixi una fallada i probabilitat de detectar-la. El producte dels 3 coeficients dona l’índex de prioritat de riscos (IPR), que permet establir la prioritat de les accions de millora. Un altre recurs és Six Sigma, eina que permet el càlcul de la probabilitat d’aparició de defectes d’un procés, anomenats defectes per milió d’oportunitats (DPMO), permetent quantificar en els processos redissenyats, la disminució de la probabilitat de presentar defectes. L’objectiu general proposat va ser demostrar que l’AMFE, dintre d’un procés de reenginyeria de processos, era una eina útil pel redisseny de processos assistencials de risc del SMI. Per això es van seleccionar quatre PA de risc amb Tècnica de Grup Nominal. Aquests van ser: control estricte de la glucèmia en pacients postoperats de cirurgia cardíaca, posició del capçal en pacients en ventilació mecànica invasiva, sedació i analgèsia en el pacient crític en ventilació mecànica invasiva i notificació i registre d’esdeveniments adversos en el SMI. Els tres primers eren PA de suport i el 4rt era un PA estratègic. Els objectius específics per cada procés redissenyat després de l’AMFE van ser: demostrar la disminució del risc d’hipoglucèmia en postoperats de cirurgia cardíaca; demostrar la millora del compliment de l’indicador posició semiincorporada; millora en el procés redissenyat de sedació i analgèsia dels indicadors monitorització de la sedació i sedació adequada, i en darrer lloc, dissenyar un procés nou, que fos la base del procés de Seguretat Clínica del SMI, dissenyant i validant un sistema de notificació i registre d’EA. Els resultats després de la implementació del procés redissenyat a partir de l’AMFE van assolir els objectius previstos. El procés control de la glucèmia va aconseguir reduir del 45% al 25% les determinacions en risc d’hipoglucèmia i un augment dels pacients en rang, passant del 36% al 56%, amb un augment del nivell sigma del procés d’ 1,6 al 2,1. El procés posició del capçal va passar d’un compliment de l’indicador del 20% al 90%, augmentant el seu nivell sigma de 0,6 a 2,8. El procés redissenyat de sedació i analgèsia, va aconseguir un nivell de compliment de l’IQ monitorització de la sedació del 100%; això va permetre quantificar l’IQ de sedació adequada que va ser del 87,5%. Finalment, l’aplicació de les accions de millora de l’AMFE van permetre dissenyar un sistema de notificació i registre d’esdeveniments adversos, validat en el seu primer any de funcionament, facilitant la monitorització i l’anàlisi dels EA. A la vista dels resultats, podem afirmar que l’AMFE és una eina vàlida en l’anàlisi del risc i redisseny dels Processos Assistencials, demostrat amb la millora de la mesura dels IQ i amb la disminució de la variabilitat dels PA estudiats. La reenginyeria de PA en el SMI, amb l’AMFE per l’anàlisi del risc, permet redissenyar nous PA més segurs, millorant la Seguretat Clínica en el SMI., In the last years the Patient Safety (PS) has become one of the cornerstones of the Healthcare Quality. Medicine includes Healthcare Processes —they are increasingly more complex— that combined with technology and human interaction increase the risk of causing Adverse Events (AE). The Intensive Care Unit (ICU) is one of the hospital areas where the possibility of causing AE is higher. The Healthcare Quality has resources intended for the risk management of healthcare processes that contribute to the patient safety. The Failure Mode and Effects Analysis (FMEA) is a prospective and systematic tool that enable the identification, analysis, evaluation and prevention of the AE that can take place during the healthcare process. This tool uses three coefficients: severity, the probability of the failure occurring and the probability of detecting it. The product of this three coefficients results in the Risk Priority Number (RPN) that allows setting the priority of the corrective actions. There is another resource called Six Sigma, it is a tool that enables the calculation of the probability of a process defect to be occurred — called defect per million opportunities (DPMO)— allowing to quantify the probability of presenting defects in redesigned processes. The overall objective proposed was to prove that FMEA, within a reengineering process, was a useful tool for redesigning the risky healthcare processes of the ICU. For this reason, four risky healthcare processes with a Nominal Group Technique were selected: strict monitoring of glycemia levels in cardiac surgery patients during the postoperative period; the headboard position in patients with invasive respiratory support; sedation and analgesia in critical patients with invasive respiratory support; and the notification and the record of adverse events in the ICU. The first three processes were a supportive healthcare process and the last one a strategic healthcare process. The specifics aims for every redesigned process after the FMEA were: proving the hypoglycemia risk reduction in patient, that had a cardiac surgery, during the postoperative period; proving the improvement of the fulfillment of the semirecumbent position indicator; proving the improvement in the sedation and analgesia redesigned process of the monitoring indicators of sedation and the suitable sedation; and finally, designing a new process that were the basis of the Clinical Safety of the ICU, designing and validating a notification and recording system of AE. After the implementation of the redesign process from the FMEA, the results had achieved the intended objectives. The glycemia control process achieve to reduce from 45% to 25% the values under hypoglycemia risk and to raise the patients in the range, going from de 36% to the 56% with a raise of the sigma level of a process from 1.6 to 2.1. The headboard position process changes from 20% to 90%, increasing its sigma level from 0.6 to 2.8. The redesigned process of sedation and analgesia achieved a 100% compliance level of the quality indicator of sedation; this fact allowed quantifying the quality indicator of the suitable sedation —it was 87.5%. Finally, the application of the corrective actions of the FMEA enabled to design an adverse event notification and recording system — that was validated in the first year of operation— facilitating the monitoring and the analysis of AE. According to the results, it is possible to affirm that the FMEA is a useful tool for analyzing the risk and for redesigning the Healthcare Processes. It was proved through the improvement of the measurement of the quality indicators and the decrease of the variability of the healthcare processes that were studied. The reengineering of the healthcare process in the ICU — using the FMEA for the risk analysis— enables to redesign new safer healthcare processes, improving the clinical safety in the ICU.

Dins el TFC, s'implementa un prototipus de SIG que permet donar resposta a l'enunciat plantejat: la gestió de rutes en camins no cartografiats, realitzades pels veterinaris d'una empresa de sanitat animal en les visites a les granges associades. Dentro del TFC, se implementa un prototipo de SIG que permite dar respuesta al enunciado planteado: la gestión de rutas en caminos no cartografiados, realizadas por los veterinarios de una empresa de sanidad animal en las visitas a las granjas asociadas. In this Final Degree Project, a prototype GIS is implemented that permits us to respond to the statement set out: management of routes along unmapped roads, taken by veterinarians from an animal health company on their visits to associated farms.
