The development of next-generation antimalarials that are efficacious against the human liver and asexual blood stages is recognized as one of the worlds most pressing public health challenges. In recent years, aminoacyl-tRNA synthetases, including prolyl-tRNA synthetase, have emerged as attractive targets for malaria chemotherapy. We describe the development of a single-step biochemical assay for Plasmodium and human prolyl-tRNA synthetases that overcomes critical limitations of existing technologies and enables quantitative inhibitor profiling with high sensitivity and flexibility. Supported by this assay platform and co-crystal structures of representative inhibitor-target complexes, we develop a set of high-affinity prolyl-tRNA synthetase inhibitors, including previously elusive aminoacyl-tRNA synthetase triple-site ligands that simultaneously engage all three substrate-binding pockets. Several compounds exhibit potent dual-stage activity against Plasmodium parasites and display good cellular host selectivity. Our data inform the inhibitor requirements to overcome existing resistance mechanisms and establish a path for rational development of prolyl-tRNA synthetase-targeted anti-malarial therapies.

BackgroundCoronavirus disease 2019 (COVID-19)-related short-term mortality is high in dialysis patients, but longer-term outcomes are largely unknown. We therefore assessed patient recovery in a large cohort of dialysis patients 3 months after their COVID-19 diagnosis.MethodsWe analyzed data on dialysis patients diagnosed with COVID-19 from 1 February 2020 to 31 March 2021 from the European Renal Association COVID-19 Database (ERACODA). The outcomes studied were patient survival, residence and functional and mental health status (estimated by their treating physician) 3 months after COVID-19 diagnosis. Complete follow-up data were available for 854 surviving patients. Patient characteristics associated with recovery were analyzed using logistic regression.ResultsIn 2449 hemodialysis patients (mean ± SD age 67.5 ± 14.4 years, 62% male), survival probabilities at 3 months after COVID-19 diagnosis were 90% for nonhospitalized patients (n = 1087), 73% for patients admitted to the hospital but not to an intensive care unit (ICU) (n = 1165) and 40% for those admitted to an ICU (n = 197). Patient survival hardly decreased between 28 days and 3 months after COVID-19 diagnosis. At 3 months, 87% functioned at their pre-existent functional and 94% at their pre-existent mental level. Only few of the surviving patients were still admitted to the hospital (0.8-6.3%) or a nursing home (∼5%). A higher age and frailty score at presentation and ICU admission were associated with worse functional outcome.ConclusionsMortality between 28 days and 3 months after COVID-19 diagnosis was low and the majority of patients who survived COVID-19 recovered to their pre-existent functional and mental health level at 3 months after diagnosis.

BackgroundPatients on kidney replacement therapy (KRT) are at very high risk of coronavirus disease 2019 (COVID-19). The triage pathway for KRT patients presenting to hospitals with varying severity of COVID-19 illness remains ill-defined. We studied the clinical characteristics of patients at initial and subsequent hospital presentations and the impact on patient outcomes.MethodsThe European Renal Association COVID-19 Database (ERACODA) was analysed for clinical and laboratory features of 1423 KRT patients with COVID-19 either hospitalized or non-hospitalized at initial triage and those re-presenting a second time. Predictors of outcomes (hospitalization, 28-day mortality) were then determined for all those not hospitalized at initial triage.ResultsAmong 1423 KRT patients with COVID-19 [haemodialysis (HD), n = 1017; transplant, n = 406), 25% (n = 355) were not hospitalized at first presentation due to mild illness (30% HD, 13% transplant). Of the non-hospitalized patients, only 10% (n = 36) re-presented a second time, with a 5-day median interval between the two presentations (interquartile range 2-7 days). Patients who re-presented had worsening respiratory symptoms, a decrease in oxygen saturation (97% versus 90%) and an increase in C-reactive protein (26 versus 73 mg/L) and were older (72 vs 63 years) compared with those who did not return a second time. The 28-day mortality between early admission (at first presentation) and deferred admission (at second presentation) was not significantly different (29% versus 25%; P = 0.6). Older age, prior smoking history, higher clinical frailty score and self-reported shortness of breath at first presentation were identified as risk predictors of mortality when re-presenting after discharge at initial triage.ConclusionsThis study provides evidence that KRT patients with COVID-19 and mild illness can be managed effectively with supported outpatient care and with vigilance of respiratory symptoms, especially in those with risk factors for poor outcomes. Our findings support a risk-stratified clinical approach to admissions and discharges of KRT patients presenting with COVID-19 to aid clinical triage and optimize resource utilization during the ongoing pandemic.

AbstractPurposeMethodsResultsConclusionsThis scoping review aims to describe how Bobath concept is conceptualized, operationalized, and studied in adult neurorehabilitation.The Joanna Briggs Institute (JBI) and the Preferred Reporting Items for Scoping Reviews (PRISMA-ScR) guidelines were adopted. Non-scientific and research articles were searched in electronic databases PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Scopus, Web of Science, ScienceDirect, and Physiotherapy Evidence Database (PEDro), with the keywords “Bobath” or “Neurodevelopmental Treatment”, published in English, Spanish, and Portuguese, between 2013 and 2023.Of the 78 publications identified, 31 articles addressed the conceptual underpinnings of Bobath concept (seven theoretical papers, seven Delphi/surveys/mixed methods studies, four qualitative studies, one scoping review, 10 letters to the editor, and two editorials), comprising five themes: (a) theoretical principles; (b) clinical principles; (c) clinical reasoning; (d) conceptualizing movement; and, current (e) evidence debate. The revised definition and the Model of Bobath Clinical Practice provide a clarification of the unique aspects of Bobath concept. The current study extends research on the effects of institutionalization, most notably by examining whether-and how-both pre-institutional maltreatment in the family and the stability and consistency of institutional care interact to shape emotional and behavioral development. Fifty Portuguese children, placed in residential institutions when 8 days to 26 months of age, were evaluated using the Child Behavior Checklist when aged 18-31 months. Caregiver-rated internalizing and externalizing behavior problems proved to be unrelated to both early family and institutional experiences, as main effects, but the interaction of these factors significantly predicted externalizing problems: a history of maltreatment in the family coupled with unstable institutional caregiving arrangements predicted especially elevated levels of externalizing problems. Results are discussed in terms of the importance of more distal and proximate developmental experiences.

The current study extends research on the effects of institutionalization, most notably by evaluating the influence of proximal relationship processes over and above prenatal and pre-institutional family experiences. By focusing on current quality of institutional care and the child's early family background, it examines the influence of variations in the institutionalization experience on displays of indiscriminate social behavior, after taking into account potentially confounding pre-admission experiences. Seventy-four Portuguese children (11-30 months) placed in 17 residential institutions and their primary caregivers participated in the study. Children's displays of indiscriminate social behavior were assessed based on an observational measure and a semi-structured interview administered to the child's caregiver. Data on children's physical and mental development were also collected. Three contextual-risk composites of early family behavior - prenatal, family relational, and emotional-neglect--were created. The quality of institutional care was examined in terms of structural, relational characteristics, and, additionally, of the quality of child-caregiver relationship. Emergent literacy (EL) interventions are effective at fostering foundational skills for reading and writing. During the COVID-19 pandemic schools faced several challenges to maintain these interventions, namely changing face-to-face to remote modalities. In this study, the effects of a multi-tier emergent literacy intervention are examined, delivered both in face-to-face and remote modalities, during the second COVID-19 pandemic lockdown. The intervention was delivered to 102 preschool Portuguese children (aged 5–6 years) according to the Multi-Tier Systems of Support (MTSS) framework and evaluated using a pre and post-test design and a single group. The results showed significant gains after the intervention in all emergent literacy skills evaluated: phonological awareness, vocabulary, print concepts, alphabet knowledge, and emergent writing. These findings highlight the relevance of systematic interventions that implement universal screenings, progress monitoring and decision-making processes based on children's performance, and the potentialities of using mixed modalities (face-to-face and remote) to deliver such interventions in uncertain times. Eighty-five Portuguese children, aged 12 to 30 months, placed in residential institutions were assessed to investigate the influence of variations in the institutionalization experience of social withdrawal behavior, after taking into account potentially confounding individual characteristics and pre-admission experiences. In light of the limited attention in institutionalization research on social withdrawal, the determinants of the identified predictors of withdrawal symptoms also were examined. Current quality of care experienced in the institution, operationalized in terms of the absence (vs. presence) of a preferred attachment relationship, predicted social withdrawal, such that absence of such a relationship forecasted greater withdrawal. Moreover, existence of a preferred attachment relationship was itself predicted by better child socioemotional functioning, greater caregiver sensitive-responsiveness, and better quality of individualized care provided by the staff. © 2013 Michigan Association for Infant Mental Health.

Background In the general population with coronavirus disease 2019 (COVID-19), obesity is associated with an increased risk of mortality. Given the typically observed obesity paradox among patients on kidney function replacement therapy (KFRT), especially dialysis patients, we examined the association of obesity with mortality among dialysis patients or living with a kidney transplant with COVID-19. Methods Data from the European Renal Association COVID-19 Database (ERACODA) were used. KFRT patients diagnosed with COVID-19 between 1 February 2020 and 31 January 2021 were included. The association of Quetelet's body mass index (BMI) (kg/m2), divided into: Results In 3160 patients on KFRT (mean age: 65 years, male: 61%), 99 patients were lean, 1151 normal weight (reference), 1160 overweight, 525 obese I and 225 obese II/III. During follow-up of 3 months, 28, 20, 21, 23 and 27% of patients died in these categories, respectively. In the fully adjusted model, the hazard ratios (HRs) for 3-month mortality were 1.65 [95% confidence interval (CI): 1.10, 2.47], 1 (ref.), 1.07 (95% CI: 0.89, 1.28), 1.17 (95% CI: 0.93, 1.46) and 1.71 (95% CI: 1.27, 2.30), respectively. Results were similar among dialysis patients (N = 2343) and among those living with a kidney transplant (N = 817) (Pinteraction = 0.99), but differed by sex (Pinteraction = 0.019). In males, the HRs for the association of aforementioned BMI categories with 3-month mortality were 2.07 (95% CI: 1.22, 3.52), 1 (ref.), 0.97 (95% CI: 0.78. 1.21), 0.99 (95% CI: 0.74, 1.33) and 1.22 (95% CI: 0.78, 1.91), respectively, and in females corresponding HRs were 1.34 (95% CI: 0.70, 2.57), 1 (ref.), 1.31 (95% CI: 0.94, 1.85), 1.54 (95% CI: 1.05, 2.26) and 2.49 (95% CI: 1.62, 3.84), respectively. Conclusion In KFRT patients with COVID-19, on dialysis or a kidney transplant, obesity is associated with an increased risk of mortality at 3 months. This is in contrast to the obesity paradox generally observed in dialysis patients. Additional studies are required to corroborate the sex difference in the association of obesity with mortality.

- Abstract
Left ventricular hypertrophy (LVH) is a common cardiovascular complication in end-stage kidney disease (ESKD) patients. We aimed at studying the association of LVH with adiponectin and leptin levels, cardiovascular stress/injury biomarkers and nutritional status in these patients. We evaluated the LV mass (LVM) and calculated the LVM index (LVMI) in 196 ESKD patients on dialysis; the levels of hemoglobin, calcium, phosphorus, parathyroid hormone, albumin, adiponectin, leptin, N-terminal pro B-type natriuretic peptide (NT-proBNP) and growth differentiation factor (GDF)-15 were analyzed. ESKD patients with LVH (n = 131) presented higher NT-proBNP and GDF-15, lower hemoglobin and, after adjustment for gender, lower leptin levels compared with non-LVH patients. LVH females also showed lower leptin than the non-LVH female group. In the LVH group, LVMI presented a negative correlation with leptin and a positive correlation with NT-proBNP. Leptin emerged as an independent determinant of LVMI in both groups, and NT-proBNP in the LVH group. Low hemoglobin and leptin and increased calcium, NT-proBNP and dialysis vintage are associated with an increased risk of developing LVH. In ESKD patients on dialysis, LVH is associated with lower leptin values (especially in women), which are negatively correlated with LVMI, and with higher levels of biomarkers of myocardial stress/injury. Leptin and NT-proBNP appear as independent determinants of LVMI; dialysis vintage, hemoglobin, calcium, NT-proBNP and leptin emerged as predicting markers for LVH development. Further studies are needed to better understand the role of leptin in LVH in ESKD patients.

A profissão docente é uma das profissões com maior predisposição para a síndrome Burnout. Este estudo apresenta resultados de 606 professores portugueses que lecionam no 3.º Ciclo do Ensino Básico e Ensino Secundário, português. Esta investigação pretende responder à seguinte questão: Quais são os níveis de burnout dos professores portugueses, que lecionam a alunos que frequentam o 3.º Ciclo do Ensino Básico e/ ou o Ensino Secundário, e como esses níveis de burnout estão associadas com as variáveis sociodemográficas e profissionais? Este estudo teve como objetivos: 1) Identificar níveis de burnout de professores do 3.º Ciclo do Ensino Básico e do Ensino Secundário, que lecionam em Portugal; 2) Explorar a associação entre as variáveis sociodemográficas, profissionais e os níveis de burnout; 3) Explorar o perfil dos professores identificados com risco de burnout; 4) Refletir sobre as implicações dos resultados obtidos, nomeadamente ao nível da prática pedagógica dos professores e tecer recomendações, no âmbito da mesma. O instrumento utilizado nesta investigação foi o Cuestionario de Burnout do Profesorado (CBP-R) adaptado e validado para a população de professores portugueses. Relativamente aos resultados mais relevantes constatamos que os professores respondentes possuem risco de burnout moderado. Quanto ao 2º objetivo encontramos diferenças nas variáveis sociodemográficas e profissionais (por exemplo: o género, a idade, o tempo de serviço, em que zona do país leciona, entre outras). No 3º objetivo, conseguimos caraterizar os professores com risco de burnout, no entanto não foi possível identificar um perfil específico, indiciando que qualquer professor está sujeito ao risco de desenvolver burnout. Neste sentido, é determinante atender aos diversos fatores do contexto laboral que podem contribuir para a síndrome de burnout nas mais variadas fases profissionais dos professores, incluindo a formação inicial. Por fim, detetámos algumas problemáticas que poderão afetar as práticas pedagógicas. Sustentado nestes resultados são delineadas algumas recomendações, nomeadamente problematizar os planos de estudo da formação inicial de professores, no sentido de os prepara melhor para os desafios profissionais (exemplo investir numa maior diversificação dos contextos de prática de ensino supervisionada). The teaching profession is one of the professions with the greatest predisposition to burnout syndrome. This study presents results from 606 Portuguese teachers who teach in the 3rd Cycle of Basic Education and Secondary Education, Portuguese. This investigation intends to answer the following question: What are the burnout levels of Portuguese teachers, who teach students that attend the 3rd Cycle of Basic Education and/or Secondary Education, and how are these levels of burnout associated with the variables sociodemographic and professional? This study had the following objectives: 1) To identify levels of burnout among teachers in the 3rd Cycle of Basic Education and Secondary Education, that teach in Portugal; 2) Explore the association between sociodemographic and professional variables and levels of burnout; 3) Explore the profile of teachers identified as being at risk of burnout; 4) Reflect on the implications of the results obtained, namely in terms of the teachers' pedagogical practice and make recommendations, within the scope of the same. The instrument used in this investigation was the Cuestionario de Burnout do Profesorado (CBP-R), adapted and validated for the population of Portuguese teachers. Regarding the most relevant results, we found that the responding teachers have a moderate risk of burnout. As for the 2nd objective, we found differences in sociodemographic and professional variables (for example: gender, age, length of service, in which area of the country you teach, among others). In the 3rd objective, we were able to characterize teachers at risk of burnout, however it was not possible to identify a specific profile, indicating that any teacher is subject to the risk of developing burnout. In this sense, it is crucial to attend to the different factors in the work context that can contribute to the burnout syndrome in the most varied professional stages of teachers, including initial training. Finally, we detected some issues that could affect pedagogical practices. Supported by these results, some recommendations are outlined, namely to analize the study plans of initial teacher training, in order to better prepare them for professional challenges (eg investing in a greater diversification of contexts of supervised teaching practice). BACKGROUND AND AIMS: Studies examining kidney failure patients with COVID-19 reported higher mortality in hemodialysis patients than in kidney transplant recipients. However, hemodialysis patients are often older and have more comorbidities. This study investigated the association of type of kidney replacement therapy with COVID-19 severity adjusting for differences in characteristics. METHOD: Data were retrieved from the European Renal Association COVID-19 Database (ERACODA), which includes kidney replacement therapy patients diagnosed with COVID-19 from all over Europe. We included all kidney transplant recipients and hemodialysis patients who presented between February 1st and December 1st 2020 and had complete information reason for COVID-19 screening and vital status at day 28. The diagnosis of COVID-19 was made based on a PCR of a nasal or pharyngeal swab specimens and/or COVID-19 compatible findings on a lung CT scan. The association of kidney transplantation or hemodialysis with 28-day mortality was examined using Cox proportional-hazards regression models adjusted for age, sex, frailty and comorbidities. Additionally, this association was investigated in the subsets of patients that were screened because of symptoms or have had routine screening. RESULTS: A total of 1,670 patients (496 functional kidney transplant recipients and 1,174 hemodialysis patients) were examined. 16.9% of kidney transplant recipients and 23.9% of hemodialysis patients died within 28 days of presentation. In an unadjusted model, the risk of 28-day mortality was 33% lower in kidney transplant recipients compared with hemodialysis patients (hazard ratio (HR): 0.67, 95% CI: 0.52, 0.85). However, in an age, sex and frailty adjusted model, the risk of 28-day mortality was 29% higher in kidney transplant recipients (HR=1.29, 95% CI: 1.00, 1.68), whereas in a fully adjusted model the risk was even 43% higher (HR=1.43, 95% CI: 1.06, 1.93). This association in patients who were screened because of symptoms (n=1,145) was similar (fully adjusted model HR=1.46, 95% CI: 1.05, 2.04). Results were similar when other endpoints were studied (e.g. risk for hospitalization, ICU admission or mortality beyond 28 days) as well as across subgroups. Only age was found to interact significantly, suggesting that the increased mortality risk associated with kidney transplantation was especially present in elderly subjects. CONCLUSION: In this study, kidney transplant recipients had a greater risk of a more severe course of COVID-19 compared with hemodialysis patients when adjusted for age, sex and comorbidities.

In the general population with COVID-19, the male sex is an established risk factor for mortality, in part due to a more robust immune response to COVID-19 in women. Because patients on kidney function replacement therapy (KFRT) have an impaired immune response, especially kidney transplant recipients due to their use of immunosuppressants, we examined whether the male sex is still a risk factor for mortality among patients on KFRT with COVID-19. From the European Renal Association COVID-19 Database (ERACODA), we examined patients on KFRT with COVID-19 who presented between February 1st, 2020, and April 30th, 2021. 1204 kidney transplant recipients (male 62.0%, mean age 56.4 years) and 3206 dialysis patients (male 61.8%, mean age 67.7 years) were examined. Three-month mortality in kidney transplant recipients was 16.9% in males and 18.6% in females (p = 0.31) and in dialysis patients 27.1% in males and 21.9% in females (p = 0.001). The adjusted HR for the risk of 3-month mortality in males (vs females) was 0.89 (95% CI 65, 1.23, p = 0.49) in kidney transplant recipients and 1.33 (95% CI 1.13, 1.56, p = 0.001) in dialysis patients (pinteraction = 0.02). In a fully adjusted model, the aHR for the risk of 3-month mortality in kidney transplant recipients (vs. dialysis patients) was 1.39 (95% CI 1.02, 1.89, p = 0.04) in males and 2.04 (95% CI 1.40, 2.97, p interaction = 0.02). In patients on KFRT with COVID-19, the male sex is not a risk factor for mortality among kidney transplant recipients but remains a risk factor among dialysis patients. The use of immunosuppressants in kidney transplant recipients, among other factors, may have narrowed the difference in the immune response to COVID-19 between men and women, and therefore reduced the sex difference in COVID-19 mortality risk.

Mestrado Bolonha em Contabilidade, Fiscalidade e Finanças Empresariais Este trabalho tem como objetivo, estudar a complexidade do sistema fiscal português, através dos determinantes que influenciam as decisões efetuadas pelo Centro de Arbitragem Administrativa (CAAD), no âmbito dos Imposto sobre o Património. A abordagem à complexidade fiscal é feita no âmbito dos impostos do IMI, do IMT e do IS testando através dos dados recolhidos se o número de palavras é uma variável explicativa. A amostra é composta por 1797 decisões arbitrais, proferidas entre 2011 e 2021. Destas observações, foram analisadas como variáveis, a decisão do pedido de arbitragem, o imposto em que se enquadra cada decisão, o tipo de requerente, o ano da decisão, o valor do processo, o tempo de decisão, a competência do valor das custas, os custos do processo de arbitragem e o número de palavras de cada decisão. Para tal, foram utilizados modelos Probit para avaliar os fatores que influenciam as decisões do CAAD e Poisson para avaliar se o número de palavras explica a complexidade fiscal. Os resultados demonstram que em todos os modelos Probit, as variáveis estudadas, à exceção do tempo de decisão, têm influência na decisão do pedido de arbitragem. Já no modelo Poisson verifica-se que as variáveis, à exceção do custo do processo de arbitragem, são estatisticamente significativas explicando, por isso, o número de palavras composto por cada decisão. Este modelo mostra ainda que quando o tempo da decisão e o valor do processo aumentam, o número de palavras também aumenta, comprovando assim a complexidade fiscal. This paper aims to study the complexity of the Portuguese tax system, through the determinants that influence the decisions made by the Administrative Arbitration Center (CAAD), in the context of Wealth Taxes. The approach to tax complexity is made within the scope of IMI, IMT and IS taxes by testing through the collected data whether the number of words is an explanatory variable. The sample consists of 1797 decisions with a time period between 2011 and 2021. From these observations, were analyzed as variables, the decision of the request for arbitration, the chapter of the CIMI, the CIMT and the CIS that fits every decision, the type of applicant, the year of the decision, the value of the process, the decision time, the competence of the amount of costs, the costs of the arbitration process and the number of words of each decision. To this end, Probit models were used to assess the factors influencing CAAD decisions and Poisson models were used to assess whether the number of words explains tax complexity. The results demonstrate that in all Probit models, the variables studied, with the exception of the decision time, have an influence on the decision of the request for arbitration. The Poisson model, on the other hand, shows that the variables, except for the costs of the arbitration process, are statistically significant in explaining, therefore, the number of words composed by each decision. This model also shows that when the decision time and the value of the process increase, the number of words also increases, thus proving the tax complexity. info:eu-repo/semantics/publishedVersion

O documento encontra-se organizado em sete capítulos, ao longo dosquais consta uma introdução, uma contextualização do estágio, uma descrição da prática letiva,da participação nas estruturas de orientação pedagógica e educativa, das atividades desenvolvidase das atividades extracurriculares, terminando com uma conclusão. A prática letiva compreendea observação, planificação e lecionação de aulas, a elaboração, realização e correção de testesde avaliação. O acompanhamento de uma Direção de Turma e a participação nas reuniões, doDepartamento de Matemática, de Conselhos de Turma e de seminários pedagógicos, insere-se nasestruturas de orientação pedagógica e educativa. As atividades desenvolvidas na escola abrangemas atividades dinamizadas e organizadas pelo Núcleo de Estágio de Matemática, pela escola e pelaprofessora estagiária. E por fim, as atividades extracurriculares contam com as formações em que aprofessora estagiária participou. This Internship Report prepared within the scope of the Master in Mathematics Teaching in the 3rdCycle of Basic and Secondary Education, at the Faculty of Sciences and Technology of the Universityof Coimbra, aims to describe the activities developed during the Curricular Internship that took place atEscola Secundária de Tondela, in the district of Viseu, during the academic year 2021/2022, under theguidance of Professor Raquel Caseiro, Scientific Advisor, and Professor Luís Carmelo, CooperatingAdvisor.This report intends to report and reflect the lived experiences and the work developed duringthis internship year. The document is organized into seven chapters, throughout which there isan introduction, a contextualization of the internship, a description of the teaching practice, theparticipation in the pedagogical and educational guidance structures, the activities developed and theextracurricular activities, ending with a conclusion. The teaching practice comprises the observation,planning and teaching of classes, the elaboration, realization and correction of assessment tests. Theaccompaniment of a Class Direction and the participation in the meetings, of the Department ofMathematics, Class Councils and Pedagogical Seminars, is part of the structures of pedagogical andeducational guidance. The activities developed at the school cover activities promoted and organizedby the Mathematics Internship Center, the school and the intern teacher. And finally, the extracurricularactivities include the training in which the intern teacher participated.
