Purpose Structured reporting (SR) is increasingly used. So far, there is minimal experience with SR in whole-body computed tomography (WBCT). The aim of this study was to investigate the value of routine use of SR in WBCT in trauma with a focus on reporting time, reporting errors, and referrer satisfaction. Materials and Methods Reporting time and reporting errors of CT reports were prospectively quantified for residents and board-certified radiologists 3 months before and for 6 months after implementation of a structured report in the clinical routine. Referrer satisfaction was prospectively quantified by means of a survey before and after the implementation period of SR using a 5-point Likert scale. Before and after results were compared to determine the effect of structured reporting on WBCT in trauma at our institution. Results The mean reporting time was lower when using SR (65 ± 52 min. vs. 87 ± 124 min., p = .25). After 4 months, the median reporting time was significantly lower with SR (p = .02). Consequently, the rate of reports that were finished within one hour rose from 55.1 % to 68.3 %. Likewise, reporting errors decreased (12.6 % vs. 8.4 %, p = .48). Residents and board-certified radiologists reported fewer errors when using SR with 16.4 % vs. 12.6 % and 8.8 % vs. 2.7 %, respectively. General referrer satisfaction improved (1.7 ± 0.8 vs. 1.5 ± 1.1, p = .58). Referrers graded improvements for standardization of reports (2.2 ± 1.1 vs. 1.3 ± 1.1, p = .03), consistency of report structure (2.1 ± 1.1 vs. 1.4 ± 1.1, p = .09), and retrievability of relevant pathologies (2.1 ± 1.2 vs. 1.6 ± 1.1, p = .32). Conclusion SR has the potential to facilitate process improvement for WBCT in trauma in the daily routine with a reduction of reporting time and reporting mistakes while increasing referrer satisfaction. Zusammenfassung Hintergrund Im Rahmen von Erweiterungsbaumaßnahmen am Uniklinikum Dresden war die Abschaltung der zentralen medizinischen Gasversorgung in einem Gebäude mit 3 intensivmedizinischen Teilstationen mit 22 Betten, einem OP-Trakt mit 5 OP und 6 Normalstationen mit je 28 Betten im laufenden Betrieb erforderlich. Damit bestand der Bedarf für die betroffenen Funktionseinheiten, für die Baumaßnahme eine interimistische dezentrale Gasversorgung mit Nullfehlertoleranz zu schaffen. Methodik Nach etablierten Verfahren der Risiko- und Fehlermöglichkeitsanalyse wurde durch den Notfall- und Katastrophenschutzbeauftragten des Klinikums eine Projektgruppe ins Leben gerufen, die einen Projektplan, eine Bedarfsabschätzung und einen Kommunikationsplan erarbeitete. Ergebnisse Eine Vielzahl von Risikofaktoren, für die geeignete Gegenmaßnahmen zu konzipieren waren, wurde systematisch ermittelt. Die Bedarfsabschätzung auf Basis physiologischer Parameter für die maximal 22 zu belegenden Beatmungsplätze über 4 h ergab je 26.000 l Sauerstoff und Druckluft. Sieben Einspeisungspunkte wurden mit je zwei 50-l-Flaschen Sauerstoff und Druckluft bestückt, mit einer Gesamtverfügbarkeit von je 175.000 l der beiden Gase. Je 8 weitere Flaschen waren zusätzlich in Reserve. Die Maßnahme wurde an einem Samstag ohne Elektivoperationsprogramm durchgeführt, sodass die betroffenen OP gesperrt werden konnten. Der Zeitpunkt wurde so gewählt, dass während des Nachmittagsschichtwechsels die doppelte Besetzung des Intensivpflegepersonals verfügbar war. Im Vorfeld wurden möglichst viele der Beatmungspatienten klinikintern verlegt. Neun Beatmungspatienten mussten allerdings verbleiben. Der technische Eingriff in die Gasversorgung dauerte lediglich 2 h, ohne Beeinflussung des Patientenzustands. Während der 2‑stündigen Interimsversorgung wurden auf den High-Care-Stationen 16.500 l Druckluft und 8000 l Sauerstoff verbraucht. Pro Beatmungspatient ergab sich rechnerisch ein Stundenverbrauch von 917 l Luft (15 l/min) und von 444 l Sauerstoff (7 l/min). Die vorausberechnete Menge auf Basis intensivmedizinischer Erfahrungswerte war deutlich geringer. Die 10fache Vorhaltung der Gasmenge und die vorhersehbar geringere Anzahl von Beatmungspatienten als die zugrunde gelegte Maximalbelegung haben dies mehr als kompensiert. Schlussfolgerung Für technische Eingriffe in Hochrisikobereichen ist eine sorgfältige Planung und Durchführung in einem effektiven Team erforderlich. Etablierte Verfahren des Projektmanagements und der Risikobewertung helfen in der Fehlervermeidung.

Background: Data on institutional structures of sarcoma care in Germany are scarce. The utilization of an interdisciplinary tumor board (IDTB) is an essential part of modern cancer care. We investigated to which extent and when IDTB are used in sarcoma care. We hypothesized that IDTB before treatment initiation were used more often at certified cancer centers and at high-volume centers and that IDTB utilization increased over time. Methods: From 2017 to 2020 we conducted a prospective cohort study, undertaking major efforts to include the whole spectrum of sarcoma treatment facilities. To analyze potential predictors of IDTB utilization, we calculated multivariable logistic regressions. Results: Patients and survivors (n = 1,309) from 39 study centers (22 tertiary referral hospitals, 9 other hospitals, and 8 office-based practices) participated; 88.3% of the patients were discussed at some stage of their disease in an IDTB (56.1% before treatment, 78% after therapy, and 85.9% in metastatic disease). Hypotheses were confirmed regarding the utilization of IDTB in certified cancer centers (vs. all others: OR = 5.39; 95% CI 3.28–8.85) and the time of diagnosis (2018/2019 vs. until 2013: OR = 4.95; 95% CI 2.67–9.21). Conclusion:Our study adds to the evidence regarding the institutional structures of sarcoma care in Germany. Utilization of a tumor board before therapy seems to be in an implementation process that is making progress but is far from complete. Certification is a possible tool to accelerate this development.

BACKGROUND A major reason for the low number of organ donors in Germany is a deficit in the recognition of patients who may have impending irreversible loss of brain function (ILBF) in hospitals capable of organ retrieval. METHODS We used anonymized data from the German Organ Procurement Organization (Deutsche Stiftung Organtransplantation, DSO) to compare two 12-month periods (a reference period and an evaluation period) before and after the implementation of an electronic screening tool (DETECT) at the University Hospital Dresden (UKD) with four other university hospitals without tool implementation (comparative cohort). DETECT is intended to aid in the recognition of potentially impending ILBF. The study endpoints encompassed patients with potentially unrecognized ILBF, patients with recognized ILBF, organ donations performed, and reports to the DSO. Changes in absolute risk were compared with Breslow-Day tests. RESULTS 309 patients who died with primary or secondary brain lesions were identified in the UKD in the reference and evaluation periods (164 and 145 patients, respectively), and 1060 (529, 531) in the comparative cohort. In the UKD, the number of unrecognized cases of possibly impending ILBF was 14/164 (8.54%) in the reference period and 1/145 (0.69%) in the evaluation period, yielding an absolute reduction of 7.85% (95% confidence interval [-3.36; -12.33]); by contrast, in the comparative cohort, there was a 0.55% absolute increase between the two periods ([-2.21; 3.30]; p = 0.002 for the comparison between the two cohorts). Only minor differences in absolute risk change were seen with regard to the probability of recognized ILBF (7.09% [0.29; 13.88] vs. 2.42% [1.18; 6.01]; p = 0.234), organ donation (4.70% [-0.89; 10.28] vs. 0.55% [-2.17; 3.26]; p = 0.214), or reporting to the DSO (4.17% [-1.77; 10.11] vs. 2.22% [-1.44; 5.89; p = 0.447); these changes may have arisen by chance. CONCLUSION These findings suggest that the use of DETECT can help to reduce the deficit in the recognition of patients with impending or manifest ILBF.

The primary objective of the IMPRESS study is to assess the causal effects of the IQM peer review on mortality in patients ventilated > 24 h. Secondary analyses are conducted for mortality in patients with myocardial infarction, stroke, COPD, pneumonia, and the procedural provision of a colorectal resection. This article provides a description of the study design and presents baseline results. Descriptive statistics for 231 included hospitals and patient characteristics. Due to randomization, the treatment/control group hospitals were similar with respect to the mortality in patients ventilated > 24 h and other patient and hospital characteristics at baseline. Mortality was highest (lowest) in patients ventilated > 24 h (with colorectal resection). The IMPRESS study provides a unique opportunity to assess the impact of the IQM peer review on the mortality in patients ventilated > 24 h. The secondary, exploratory, and qualitative analyses are expected to provide insights on determinants of in-hospital mortality, structure and process quality, and the robustness of different approaches to risk adjustment of quality indicators.

BackgroundAlthough clinical peer review is a well-established instrument for improving quality of care, clinical effectiveness is unclear.MethodsIn a pragmatic cluster randomised controlled trial, we randomly assigned 60 German Initiative Qualitätsmedizin member hospitals with the highest mortality rates in ventilated patients in 2016 to intervention and control groups. The primary outcome was hospital mortality rate in patients ventilated fore more than 24 hours. Clinical peer review was conducted in intervention group hospitals only. We assessed the impact of clinical peer review on mortality using a difference-in-difference approach by applying weighted least squares (WLS) regression to changes in age-adjusted and sex-adjusted standardised mortality ratios (SMRs) 1 year before and 1 year after treatment. Recommendations for improvement from clinical peer review and hospital survey data were used for impact and process analysis.ResultsWe analysed 12 058 and 13 016 patients ventilated fore more than 24 hours in the intervention and control hospitals within the 1-year observation period. In-hospital mortality rates and SMRs were 40.6% and 1.23 in intervention group and 41.9% and 1.28 in control group hospitals in the preintervention period, respectively. The groups showed similar hospital (bed size, ownership) and patient (age, sex, mortality, main indications) characteristics. WLS regression did not yield a significant difference between intervention and control groups regarding changes in SMRs (estimate=0.04, 95% CI= −0.05 to 0.13, p=0.38). Mortality remained high in both groups (intervention: 41.8%, control: 42.1%). Impact and process analysis indicated few perceived outcome improvements or implemented process improvements following the introduction of clinical peer review.ConclusionsThis study did not provide evidence for reductions in mortality in patients ventilated for more than 24 hours due to clinical peer review. A stronger focus on identification of structures and care processes related to mortality is required to improve the effectiveness of clinical peer review.

Background Relationships between in-hospital mortality and case volume were investigated for various patient groups in many empirical studies with mixed results. Typically, those studies relied on (semi-)parametric statistical models like logistic regression. Those models impose strong assumptions on the functional form of the relationship between outcome and case volume. The aim of this study was to determine associations between in-hospital mortality and hospital case volume using random forest as a flexible, nonparametric machine learning method. Methods We analyzed a sample of 753,895 hospital cases with stroke, myocardial infarction, ventilation > 24 h, COPD, pneumonia, and colorectal cancer undergoing colorectal resection treated in 233 German hospitals over the period 2016–2018. We derived partial dependence functions from random forest estimates capturing the relationship between the patient-specific probability of in-hospital death and hospital case volume for each of the six considered patient groups. Results Across all patient groups, the smallest hospital volumes were consistently related to the highest predicted probabilities of in-hospital death. We found strong relationships between in-hospital mortality and hospital case volume for hospitals treating a (very) small number of cases. Slightly higher case volumes were associated with substantially lower mortality. The estimated relationships between in-hospital mortality and case volume were nonlinear and nonmonotonic. Conclusion Our analysis revealed strong relationships between in-hospital mortality and hospital case volume in hospitals treating a small number of cases. The nonlinearity and nonmonotonicity of the estimated relationships indicate that studies applying conventional statistical approaches like logistic regression should consider these relationships adequately.

Background: Prolonged ventilation is associated with a high risk of death. This study investigated both patient-level and hospital-level risk factors for in-hospital mortality in patients ventilated for more than 24 hours. Methods: The analyses were conducted in the framework of a German national multicenter retrospective cohort study. Patient and hospital characteristics were examined using descriptive statistics. Risk factors of in-hospital mortality were analyzed using multilevel robust Poisson regressions for binary outcomes. Potential effect modifications were examined by stratified analyses. Results: The sample includes 95 672 cases of patients ventilated >24 hours in 163 hospitals covering the period 2016 to 2017. According to the results of multilevel Poisson regressions, main patient-level risk factors for in-hospital mortality were age (per year relative risk [RR] = 1.021, 95% CI = 1.020-1.023), stroke (RR = 1.459; 95% CI = 1.361-1.563), emergency case admission (RR = 1.273, 95% CI = 1.156-1.403), and transfer from another hospital (RR = 1.169, 95% CI = 1.084-1.261). The individual risk of in-hospital death was positively associated with hospital size (RR of hospitals with 600-799 beds vs Conclusion: By providing evidence on multiple patient-level and hospital-level risk factors for in-hospital mortality in patients ventilated for more than 24 hours, this large multicenter study has main implications for quality assessment of clinical care and the adequate specification of risk adjustment models. The revealed effect modifications indicate the relevance of stratified analyses.

(DNVF) fördert seit Jahren die methodische Qualität von Versorgungsforschungsstudien auf der Basis von Memoranden und anderen Initiativen. Die Qualitäts- und Patientensicherheitsforschung (QPSF) gilt als Kerngebiet der Gesundheitsversorgungsforschung. Das vorliegende Memorandum erläutert wesentliche etablierte Fragestellungen und Methoden der QPSF. Vor dem Hintergrund der besonderen gesundheitspolitischen Bedeutung des Themas werden Methoden der Messgrößenentwicklung und -prüfung, die Risikoadjustierung, Methoden zur Erhebung von Patientensicherheitsdaten, Instrumente zur Analyse sicherheitsrelevanter Ereignisse und Methoden zur Evaluation der meist multiplen und komplexen QPSF-Interventionen behandelt. Zudem werden vordringliche Forschungsthemen benannt.

Background Inclusion of patient-reported outcomes (PROs) in routine cancer care is of key importance for individualized treatment, shared decision making and patient satisfaction. Objective To describe the implementation under routine conditions of an electronic self-administered PRO assessment and comparison of PROs before and after inpatient treatment in oncologic care. Methods In a tablet-based survey PROs on symptom burden, global health status/ quality of life (QoL) and health utility were collected twice (at hospital admission and discharge) in an inpatient oncological setting over a 17-month period using the EORTC QLQ-C30 and EQ-5D questionnaires. Data were linked to the hospital information system (HIS). Patient acceptability, recruitment rates, symptom burden, and clinically meaningful changes in PROs over time were analyzed. Results From a total of 384 hospitalized patients invited to participate at admission 371 (96.6 %) participated. At discharge, 195 patients were approached for a follow-up assessment, and 192 patients (98.5 %) participated. Despite strong acceptance among patients, recruitment rates were decreasing over time. During the hospital stay clinically meaningful improvements were observed for health utility (33.3 %, n = 64) and global health status/QoL (43.2 %, n = 83). Patients reported a variety of symptoms at admission and discharge. Conclusions Implementation of PRO assessment in routine care and data integration into the HIS provides valuable information for the entire medical staff as symptom burden is present during the entire hospital stay. Implications for Practice Long-term maintenance of PRO assessment in a clinical setting as a prerequisite of value-based healthcare requires continuous involvement of the nursing team, which can only be achieved by allocating resources to this task.

Zusammenfassung Hintergrund Sturze sind ein relevantes Risiko in der Gesundheitsversorgung. Bislang fehlen versorgungsepidemiologische Analysen zur Sturzhaufigkeit, Risikofaktoren und Gute eingesetzter Sturzrisikoassessments. Methode Im Rahmen einer Routinedaten-basierten Querschnittsstudie wurden alle stationaren Patienten des Universitatsklinikums Dresden der Jahre 2012 und 2013 hinsichtlich der Sturzinzidenz und Risikofaktoren (Items des Dresden-Sturzrisikoassessments, Alter, Geschlecht, Erkrankungsschwere, Verweildauer) untersucht. Das Sturzrisiko und assoziierte Risikofaktoren wurden anhand deskriptiver Verfahren und logistischer Regressionsmodelle ermittelt. Ebenfalls erfolgte die Beurteilung der Validitat des Dresden-Sturzrisikoassessments. Ergebnisse Die Sturzinzidenz bei stationarem Aufenthalt betrug 1,6% mit Unterschieden zwischen den Fachabteilungen (Sturzrisiko 0,1% bis 9,8%). Die Sturzinzidenz pro 1.000 Behandlungstage betrug 2,9 Sturze. Hoheres Patientenalter, langere Verweildauer und hohere Erkrankungsschwere waren signifikante Risikofaktoren fur ein Sturzereignis. Geringe Sturzfolgen (84%) dominierten vor solchen mit medizinischem Behandlungsbedarf (16%). Alle 9 Items des Sturzrisikoassessments waren signifikante Risikofaktoren fur ein Sturzereignis. Die Sensitivitat bzw. Spezifitat des Dresden-Sturzrisikoassessments betrug 69,9% bzw. 71,4%. Schlussfolgerung Altere und schwerer erkrankte Patienten haben ein hohes Sturzrisiko. Die valide Erfassung des Sturzrisikos von Patienten ist ein wichtiges Instrument des Qualitats- und medizinischen Risikomanagements im Hinblick auf die Ableitung praventiver Masnahmen. Die vorliegende Studie kann dazu beitragen, hinsichtlich der Gute von Risikoassessements zu sensibilisieren, das eingesetzte Sturzrisikoassessment weiter zu verbessern und die Patientensicherheit in der akutstationaren Versorgung zu erhohen.

Zusammenfassung Ziel Das Peer Review legt den Fokus auf die kritische (Selbst-)Reflexion primar arztlichen Handelns im Dialog mit Fachkollegen unterschiedlicher Fachdisziplinen und ggf. Berufsgruppen. Im Vordergrund steht das Prinzip der Selbstbestimmung, der Wille zu effektivem Lernen voneinander und das klare Engagement um der Sache, d. h. der Qualitatsverbesserung willen. In den vergangenen funf Jahren ist es vor allem der Initiative Qualitatsmedizin (IQM) gelungen, das Peer Review im deutschsprachigen Raum zu fordern. Ziel der vorliegenden Studie ist die erstmalige Analyse aller Protokolle der IQM Peer Reviews und die Darstellung der erzielten Ergebnisse im zeitlichen Verlauf. Im Fokus stehen dabei inhaltliche Schwerpunkte von identifizierten Verbesserungspotentialen, Informationen zum organisatorischen Ablauf bis hin zur Zufriedenheit der besuchten Kliniken. Methode Methodisch erfolgte eine systematische, deskriptive Analyse aller Protokolle der im Zeitraum 2010 bis 2014 durchgefuhrten Peer Reviews sowie der Feedbackbogen der IQM Mitgliedskliniken, bei denen ein entsprechendes Verfahren stattfand. Ergebnisse Im Ergebnis wurden 294 der 304 geplanten Peer Reviews berucksichtigt. Diese verteilten sich uber die verschiedenen Tragergruppen und Krankheitsbilder. Das Verbesserungspotential seitens des Peer Teams wurde hoher eingeschatzt als bei den Kliniken im Rahmen der Vorbereitung auf das Verfahren. Die inhaltliche Bewertung der zugrunde liegenden Analysekriterien zeigte unterschiedliche Trends auf. Positiv ist die Entwicklung bei der adaquaten und zeitgerechten Diagnostik und Behandlung, Verbesserungspotential hingegen besteht bei der umfassenden und schlussigen Dokumentation. Weiterhin konnten pro Krankheitsbild medizinische Auffalligkeiten identifiziert werden, die fur alle Mitgliedskliniken von Bedeutung sind. Bezuglich der Nachhaltigkeit des Verfahrens liegen erstmals Ergebnisse zu so genannten Re-Reviews vor, die Impulse in Richtung Struktur- und Prozessverbesserung gegeben haben. Erfreulich ist die kontinuierlich gestiegene Akzeptanz der besuchten Chefarzte innerhalb des untersuchten Zeitraumes. Schlussfolgerungen In der Zusammenschau aller vorliegenden Ergebnisse und Entwicklungen des Peer Review Verfahrens bei IQM hat sich dies in den letzten funf Jahren dank klar vorgegebener Rahmenbedingungen, Strukturen und Unterstutzungshilfen etabliert. Die Zusammenarbeit mit der Bundesarztekammer hat sich u. a mit der Veroffentlichung eines Curriculums Arztliches Peer Review bewahrt. Die Re-Reviews zeigen Impulse in Richtung Nachhaltigkeit des Verfahrens auf. Um diese Aussage mit Evidenz zu belegen, bedarf es einer Weiterentwicklung des Peer Review Verfahrens (nicht nur bei IQM) im Hinblick auf die wissenschaftliche Begleitforschung.

The German Network for Health Services Research [Deutsches Netzwerk Versorgungsforschung e.V. (DNVF)] fosters the methodological quality of health services research studies by memoranda and other initiatives. Quality of care and patient safety research (QCPSR) form core areas of health services research. The present memorandum explicates principal QCPSR questions and methods. Based on the issues' particular relevance for health policy, the memorandum exemplifies methods for developing and testing indicators, risk adjustment techniques, methods for collecting patient safety data, tools to analyse patient safety incidents and methods for evaluating often complex and multicomponent QCPS interventions. Furthermore, we point out urgent research topics.Das Deutsche Netzwerk Versorgungsforschung e.V. Einleitung: Die Qualitatsbeurteilung der medizinischen Versorgung anhand von Qualitatsindikatoren spielt im deutschen Gesundheitssystem eine zunehmend wichtige Rolle. Gutekriterien von Qualitatsindikatoren sind eine hohe Validitat, Reliabilitat, Anderungssensitivitat, Interpretierbarkeit und Praktikabilitat. Aufgrund der zunehmenden Relevanz von Leitlinien kommt Qualitatsindikatoren, die durch Leitlinien empfohlen werden eine besondere Bedeutung zu. Methode: Mittels eines systematischen Reviews wurde auf der Basis aller zum Stichtag 30.11.2013 gultiger S3-Leitlinien untersucht, inwieweit Qualitatsindikatoren formuliert sind, welche Kategorien zur Strukturierung verwendet wurden und ob Qualitatsindikatoren in S3-Leitlinien anhand eines evidenzgeleiteten Vorgehens entwickelt wurden. Ergebnisse: In 34 (39%) der 87 aktuellen S3-Leitlinien wurden insgesamt 394 Qualitatsindikatoren formuliert. Dabei handelte es sich in der uberwiegenden Mehrzahl um Indikatoren der Prozessqualitat. Indikatoren der Ergebnisqualitat wurden in 9 S3-Leitlinien (10%) beschrieben. Die Gute der empfohlenen Qualitatsindikatoren geht aus keiner der analysierten Leitlinien hervor. Schlussfolgerung: Trotz der zunehmenden Relevanz der Qualitatsbeurteilung fur alle Interessensgruppen im Gesundheitswesen formulieren nur rund 40% der aktuellen S3-Leitlinien Qualitatsindikatoren. Empfehlungen zur Beurteilung der Ergebnisqualitat geben nur 10% der S3-Leitlinien. Der Prozess der Entwicklung und Empfehlung von Qualitatsindikatoren in S3-Leitlinien ist uneinheitlich und wird haufig nicht transparent dargestellt. Die derzeitigen Verfahren zur Empfehlung und Validierung von Qualitatsindikatoren in S3-Leitlinien genugen den Anforderungen der evidenzbasierten Gesundheitsversorgung nicht.

Summary Background The prevention of pressure ulcers (PU) is an important public health issue owing to their substantial clinical and economic burden. Objectives To investigate predictors of incident PU in hospitalized patients and the performance of the Braden Scale in intensive care units (ICU) and normal care units (NCU). Methods We conducted a prospective cohort study including all inpatients treated at the University Hospital Carl Gustav Carus Dresden, Germany, between 2007 and 2011. Documentation comprised patient characteristics, Braden Scale and clinical signs of PU. The primary outcome was incident PU during inpatient treatment. Predictors of PU were explored by using univariate and multivariate logistic regression models. To evaluate the performance of the Braden Scale a receiver operating characteristics (ROC) curve analysis was applied. Results The overall incidence of PU during inpatient treatment was 0·78%. A higher rate of PU was observed at ICU vs. NCU (4·77% vs. 0·59%). Multivariate analysis identified age [odds ratio (OR) 1·04, 95% confidence interval (CI) 1·035–1·041 per year], female sex (OR 1·11, 95% CI 1·01–1·22), length of stay (OR 17·79, 95% CI 15·46–20·48 for 30 or more days vs.

Zusammenfassung Prozessmanagement ist im klinischen Alltag eine wertvolle Methode zur systematischen Analyse und strukturierten Optimierung der Behandlungsqualitat und -sicherheit. Dazu bedarf es der Motivation, Bereitschaft und Einsicht bei den Mitarbeitern und der Leitung, Veranderungen umzusetzen. Um relevante Veranderungspotentiale zu identifizieren, ist die Definition konkreter (Qualitats-) Kennzahlen notwendig. Die Qualitatsindikatoren der externen stationaren Qualitatssicherung gemas § 137 SGB V sind eine Moglichkeit, vergleichbare Qualitatsergebnisse transparent darzustellen. Dieses Verfahren wird seit mehr als zehn Jahren durch den Gemeinsamen Bundesausschuss und den beauftragten Institutionen durchgefuhrt und konsequent weiterentwickelt. Fur 30 definierte Leistungsbereiche liegen bundesweit Daten zur Behandlungsqualitat der Krankenhauser vor. Qualitatsindikatoren sind fur das Prozessmanagement unverzichtbare Grosen, um die Ablaufe zeitnah zu messen, steuern und kommunizieren zu konnen. Die Zusammenfuhrung von definierten Prozessen mit Qualitatsindikatoren in einem Indikatorendashboard stellt fur die Information der Mitarbeiter uber die erzielten Qualitatsergebnisse einen wesentlichen Vorteil dar. Mit Hilfe eines prozessbasierten Indikatorendashboards werden wesentliche Informationen des Behandlungsprozesses gebundelt dargestellt bzw. visualisiert, die gegebenenfalls Anknupfungspunkte fur Prozessanalysen liefern. Damit konnen bei einer kontinuierlichen Berucksichtigung dieser Kennzahlen Vergleichswerte ermittelt, Verbesserungspotentiale zeitnah identifiziert und in die sich anschliesende Prozess(neu)gestaltung integriert werden.

Zusammenfassung Arztliches Handeln ist gepragt vom Messen von Zustanden und Zustandsanderungen individueller Patienten zur Diagnosestellung, Prognoseabschatzung und Erfassen des Therapieansprechens. Qualitatsanforderungen an Messinstrumente beinhalten Validitat, Reliabilitat, Anderungssensitivitat, Interpretierbarkeit und Praktikabilitat. Qualitativ hochwertige Messinstrumente sind eine Grundvoraussetzung der evidenzbasierten Medizin. Vor diesem Hintergrund wurden durch internationale Outcomes-Forschergruppen in den vergangenen Jahren Methoden zur Qualitatssicherung und Standardisierung von Messinstrumenten entwickelt. Bei Qualitatsindikatoren handelt es sich um Instrumente zur Messung der Versorgungsqualitat. Aufgrund der zunehmenden Relevanz der Qualitatsbeurteilung fur alle Interessensgruppen im Gesundheitswesen sowie des gesundheitspolitischen Willens zur Qualitatsbewertung und Ableitung entsprechender Konsequenzen mussen Qualitatsindikatoren mindestens den hohen Anspruchen genugen, die wir an Endpunkte in klinischen Studien stellen. Dennoch findet eine Betatigung von unabhangigen Wissenschaftlern und Klinikern auf dem Gebiet der Validierung und Standardisierung von Qualitatsindikatoren in Deutschland bisher praktisch nicht statt, so dass sich einzig das AQUA-Institut per Auftrag durch den GBA mit dieser Aufgabe beschaftigt. Die derzeitigen Verfahren und Aktivitaten zur Validierung von Qualitatsindikatoren genugen den Anforderungen der evidenzbasierten Gesundheitsversorgung nicht. Dies ist ein entscheidendes Hindernis fur die Erreichung der gesundheitspolitischen Ziele der Qualitatsbewertung. Die Autoren empfehlen daher einen mehrstufigen, multiprofessionellen, evidenzgeleiteten und evidenzbildenden Konsensusprozess in Anlehnung an etablierte Methoden der Outcomes Forschung zur Weiterentwicklung der Qualitatsmessung in Deutschland. Alle relevanten Interessensgruppen sollten dabei partizipieren.

Zusammenfassung Hintergrund Die externe Qualitatssicherung gemas § 137 SGB V (EQS) ist eine in Deutschland seit vielen Jahren etablierte Methode zur Qualitatssicherung der stationaren Patientenversorgung. Der flachendeckende Ansatz ist dabei einmalig im internationalen Vergleich. Neben dem gesetzgeberischen Auftrag fordern auch die Kostentrager diese Form externer Qualitatskontrolle, um einen qualitatsorientierten Wettbewerb zwischen den Kliniken zu erzielen. Seit der verbindlichen Einfuhrung im Jahr 2001 wird der Nutzen dieser Erhebung insbesondere durch die Leistungserbringer kritisch hinterfragt. Methode Die Untersuchung basiert auf allen Patienten des Universitatsklinikums Dresden (UKD), bei denen im Zeitraum 2003 bis 2011 ein Qualitatssicherungsbogen dokumentiert werden musste (n=45.639). Die Dokumentation dieser Bogen erfolgte durch speziell geschultes Personal. Fur jeden Leistungsbereich wird die Dokumentationsdauer der Qualitatssicherungsbogen separat erhoben. Es erfolgte eine deskriptive Analyse aller Qualitatssicherungsbogen und bei statistischer Auffalligkeit eine Analyse der sogenannten Strukturierten Dialoge. Ergebnisse Im gesamten Zeitraum sind 167 statistische Auffalligkeiten durch Uberschreitung der bestehenden Referenzbereiche aufgetreten. Davon wurden durch die Facharbeitsgruppen der Projektgeschaftsstelle Qualitatssicherung (PGSQS) an der Sachsischen Landesarztekammer neun als qualitativ auffallig eingestuft. Bei den verbleibenden 158 statistischen Auffalligkeiten handelt es sich bei 25 um Dokumentationsfehler, 96 wurden als lediglich statistisch auffallig eingestuft sowie wurden 37 mit einem Hinweis auf erneute Beobachtung seitens der PGSQS versehen. Der gesamte Dokumentationsbedarf der Qualitatssicherungsbogen umfasst ca. 1.420 Arbeitstage im Beobachtungszeitraum. Diskussion Die Ergebnisse konnen im Hinblick auf die Qualitat der Patientenversorgung als positiv gewertet werden, da sich nur eine geringe Anzahl an qualitativen Auffalligkeiten zeigt. Diese Aussage stutzt sich vor allem auf die Bewertung der Vergleichsgruppen Sachsen und Deutschland, die in den Qualitatsreports der EQS ausgewiesen sind. Die Mehrzahl der statistischen Auffalligkeiten beruht auf Dokumentationsfehlern. Weitere statistische Auffalligkeiten, welche medizinisch indiziert sind, treten trotz wiederholter Anfrage zur Uberarbeitung an das AQUA-Institut regelmasig bei bestimmten Qualitatsindikatoren auf. Beispielhaft sind an dieser Stelle die postoperativen Beweglichkeitsindikatoren der endoprothetischen Leistungsbereiche zu nennen, die nur eine bedingte Aussage uber das Outcome des Patienten treffen. Wichtig in diesem Kontext sind Informationen zur Lebensqualitat oder auch zum poststationaren Krankheitsverlauf der Patienten, die derzeit fehlen. Daruber hinaus wird die Nutzung der Daten der EQS fur eine wissenschaftlich fundierte Evaluationsforschung bisher eher restriktiv gehandhabt, sodass weitere Aussagen zur Behandlungsqualitat zumindest zum jetzigen Zeitpunkt nur eingeschrankt ableitbar sind.

Zum Beginn der Planung des Diagnostisch-Internistisch-Neurologischen Zentrums (DINZ) am Universitatsklinikum Dresden (UKD) standen vor allem Pramissen baulicher Art im Vordergrund. So wurde der gesamte Komplex des DINZ barrierefrei geplant bzw. dem Altbaucharakter des UKD angepasst. Hierin einbezogen wurden die Planung und Sanierung fur den benachbarten Altbaukomplex Haus 19, der zugleich denkmalgeschutzt ist und zuvor chirurgischen Betten vorbehalten war.

Als Krankenhaus der Maximalversorgung bildet das Universitatsklinikum Carl Gustav Carus Dresden (UKD) das gesamte Spektrum moderner Medizin ab. Es vereint 21 Fachkliniken, 6 interdisziplinare Zentren und 3 Institute, die eng mit den klinischen und theoretischen Instituten der Medizinischen Fakultat zusammenarbeiten.

Falls are a relevant issue of inpatient treatment. Epidemiological analysis concerning incidence, risk factors for falls and the quality of risk assessments are missing.In a routine data-based cross-sectional study all patients hospitalized in the University Hospital Dresden, Germany, during 2012 and 2013 were analyzed according to fall incidence and risk factors (items of Dresden fall risk assessment [Dresden-FRA], age, sex, severity of disease, and length of stay. Fall risk and associated risk factors were determined using descriptive methods and logistic regression models. In addition, the quality of the fall risk assessment was evaluated.The risk of falling during the observation period was 1.6 %. Differences exist between the medical disciplines (risk of falling 0.1 % to 9.8 %). The fall rate was 2.9/1,000 days of treatment. Higher age, longer inpatient stay, and a higher level of disease severity were significant predictors of falls. Less serious consequences of falls were more frequently documented (84 %) than consequences of falls requiring treatment (16 %). All nine items of the Dresden-FRA were significant risk factors. The sensitivity and specificity of the Dresden-FRA were 69.9 % and 71.4 %, respectively.Patients at an older age and with more severe disease have a higher risk of falling. The validated assessment of the individual risk of falling is an important tool of medical quality management and risk management. The present study can help to raise awareness about the quality of risk assessments, to further improve fall risk assessment, and to increase patient safety in acute care.

Peer Review literally meaning "re-inspection by a peer" is a special form of external evaluation whose roots go back to Ancient Greece and which is widely employed in science to assess manuscripts submitted for publication. In the medical context the Peer Review process is defined as structured critical self-reflection through dialogue with colleagues. Its prime objective is to improve the quality of patient care by identifying potentials for improvement and by deriving an action plan. Amongst other things, this includes medical standards and guidelines, indications and their traceability, the monitoring of the treatment process as well as the interdisciplinary cooperation and teamwork between different professional groups. The Peer Review practice in Germany has received strong impetus from comprehensive hospital operator projects like IQM, the "Initiative Qualitatsmedizin", and the Peer Review practice in intensive care. This practice, which has primarily been developed by practitioners for practitioners of their own accord, offers the chance to integrate medical quality with little bureaucratic effort and direct transfer of knowledge back into daily clinical work. Another important approach to promote peer reviewer qualifications is the curriculum "Medical Peer Review", which has been published by the German Medical Association since 2011. The positive experiences regarding peer review practice go back to the 1990s and can be traced in the position paper of the German Medical Association concerning quality assurance in the field of pathology. This evolved into the initiative of pathologists in Saxony to implement the peer review practice within the remit of their State Medical Association. Since the 14(th) of May 1999 various institutes as well as medical group practices in pathology and cytology have joined forces to undertake voluntary inter-institutional quality control measures. The aim was to improve the process and especially the outcome quality by reviewing each other's quality (analysing samples) on the basis of defined criteria. A positive review outcome report, which will have to be unanimously agreed upon by all participants, will lead to a positive recommendation for a certificate issued by the Medical Association of Saxony. Between 1999 and 2011 a total of 56 peer review proceedings took place. The potentials for improvement concern the problem of "kitchen pathology" still being used in the context of macroscopic description, the quality of microscopic description including the use of correct nomenclature, the quality of tumour classification and immune histological investigations. Statements concerning the advantage of the peer review method in particular refer to the integration of resident pathologists. Due to the long period of implementation of the peer review system und the small number of proceedings, it is not possible to come to a clear conclusion about the improvement or deterioration of quality. During a period of more than 10 years three peer review proceedings were not successful. In this situation, it is appropriate to ask the question of how restrictive peer reviews should be. The three most important aspects for the pathologists in Saxony are: self-determination, learning from each other, and a commitment to quality improvement. So this method set an example of how a quality management method can persist for more than 10 years through voluntary engagement. And the initiative of pathologists in Saxony has given an important impetus to the development of other peer review systems in various health care organisations or disciplines in Germany.

Six Sigma is an innovative management- approach to reach practicable zero- defect quality in medical service processes. The Six Sigma principle utilizes strategies, which are based on quantitative measurements and which seek to optimize processes, limit deviations or dispersion from the target process. Hence, Six Sigma aims to eliminate errors or quality problems of all kinds. A pilot project to optimize the preparation for neurosurgery could now show that the Six Sigma method enhanced patient safety in medical care, while at the same time disturbances in the hospital processes and failure costs could be avoided. All six defined safety relevant quality indicators were significantly improved by changes in the workflow by using a standardized process- and patient- oriented approach. Certain defined quality standards such as a 100% complete surgical preparation at start of surgery and the required initial contact of the surgeon with the patient/ surgical record on the eve of surgery could be fulfilled within the range of practical zero- defect quality. Likewise, the degree of completion of the surgical record by 4 p.m. on the eve of surgery and their quality could be improved by a factor of 170 and 16, respectively, at sigma values of 4.43 and 4.38. The other two safety quality indicators "non-communicated changes in the OR- schedule" and the "completeness of the OR- schedule by 12:30 a.m. on the day before surgery" also show an impressive improvement by a factor of 2.8 and 7.7, respectively, corresponding with sigma values of 3.34 and 3.51. The results of this pilot project demonstrate that the Six Sigma method is eminently suitable for improving quality of medical processes. In our experience this methodology is suitable, even for complex clinical processes with a variety of stakeholders. In particular, in processes in which patient safety plays a key role, the objective of achieving a zero- defect quality is reasonable and should definitely be aspirated.

Apart from a multidimensional quality definition and the understanding of quality as a company-wide challenge, a third essential element of quality management is prevention. Thus, company quality policy has to be prevention-oriented and requires both customer and process orientation as important prerequisites. Process orientation especially focuses on the critical analyses of work flows as a condition for identifying early intervention options which, in turn, may influence the result. Developing a business organisation requires the definition of criteria for space planning, room assignment and room integration in consideration of both medical and economic aspects and the architectural concept. Specific experiences will be demonstrated as a case study using the example of a new building in the midst of the Carl Gustav Carus University Hospital in Dresden, the Diagnostic Centre for Internal Medicine and Neurology. The hospital management placed an order to develop a sustainable as well as feasible business organisation for all the different departments. The idea was to create a medical centre where maximum use was made of all planned spaces and resources on the basis of target processes which had to be defined and agreed upon with all the persons concerned. In a next step all the personal, space and operational resources required were assigned. The success of management in all industries, including the health care sector, crucially depends on the translation of ideas into practice, among them the critical factor of sustainability. In this context, the support by the management as a role model, a formal frame for the respective project group and the definition of controlling via defined indicators have special importance. The example of the Diagnostic Centre for Internal Medicine and Neurology demonstrates that the result of changed processes may release a cultural change where competition can be replaced by cooperation step by step.

Zusammenfassung Eine grostmogliche Sicherheit im Bereich der medizinischen Behandlungsablaufe ist nur durch eine aktive Einbindung des medizinischen Personals im Rahmen eines proaktiven und praventiven Qualitatsmanagements zu erreichen. Aufgrund der bestehenden Wissensasymmetrie zwischen Patienten und dem medizinischen Personal konnen Patienten und Angehorige die Qualitat der medizinischen Behandlung sowie die Sicherheit der die Behandlung begleitenden Prozesse nicht zuverlassig beurteilen. Mithilfe von detaillierten prozessorientierten Befragungen des medizinischen Personals kann eine systematische Risikoanalyse aller medizinischen Leistungsprozesse erfolgen, mit dem Ziel latente Problemfelder zu identifizieren, welche die Sicherheit der Patientenbehandlung gefahrden. Durch Aufstellen von Risikoportfolios und Prioritatslisten kann nachfolgend ein sinnvoller Handlungsplan zur praventiven Qualitatssteuerung entwickelt werden. Im Gegensatz zu Fehlermeldesystemen, die sich zur kontinuierlichen Qualitatsuberwachung durch Erfassung von kritischen Einzelereignissen eignen, erlauben systematische Mitarbeiterbefragungen eine zeitpunktbezogene umfassende Risikoanalyse aller medizinischen Leistungsprozesse.

We wish to add three aspects of evaluating the quality of guidelines that were not mentioned in the article (1). Systematic research has shown for randomized controlled studies that unsatisfactory blinding of the study medication, randomization, or allocation can result in relevant systematic biases. This evidence is the basis of the validity of the Cochrane Collaboration’s risk of bias tools (2). However, systematic studies of the validity are lacking for the quality standards of other study types and for evaluation instruments for guidelines. It remains unclear what concrete conclusions users can draw if a guideline is given a lower quality rating according to the described evaluation instruments. We therefore think that a need exists to conduct appropriate studies to find out which negative effects result in poorer quality guidelines. The degree to which guideline adherence can be operationalized is a critical factor for success in implementing guideline recommendations. This aspect is not explicitly provided for in the instruments, but it is crucial for measuring guideline adherence. In the context of guideline development, it is important to develop appropriate instruments for measuring changes in the quality of care as a result of using guidelines. A systematic review showed that 34 out of 87 current S3 guidelines define quality indicators for measuring guideline recommendations, but that the applied methods for defining quality indicators are largely lacking, as are any conclusions about the quality of guideline indicators (3). Aspects of the quality with which quality indicators in guidelines are developed are also missing from the evaluation instruments.

„To err is human.“ Unter diesem Titel wies bereits im Jahr 2000 das Institute of Medicine darauf hin, dass zwischen 2,9 und 3,7 Prozent aller in Krankenhausern in den USA aufgenommenen Patienten Fehlern beziehungsweise Beinahe-Fehlern wahrend des Behandlungsprozesses ausgesetzt sind. Diese fuhrten in etwa sieben Prozent der Falle zu einer dauerhaften Schadigung des Patienten und bei 14 Prozent dieser Schadensfalle sogar zum Tod (vergleiche Kohn/Corrigan/Donaldson 2000).

Klinische Pfade konnen medizinische Behandlungen aus der Prozessperspektive organisieren. Die traditionellen Abteilungsgrenzen werden dabei uberschritten und die Behandlung des Patienten in den Mittelpunkt gestellt. Die Pfaderstellung erfordert einen mehrschrittigen und arbeitsaufwendigen Prozess unter Einbeziehung aller beteiligten Berufsgruppen. Die Nutzung von klinischen Pfaden in den Bereichen Patienteninformation, Ablauftransparenz sowie zur kontinuierlichen Qualitatssicherung bietet sich an. Die Steuerung von medizinischen und administrativen Prozessen wird durch die Analyse der Daten eines klinischen Pfades unterstutzt und sichert damit die arztliche Handlungsfreiheit.

Zusammenfassung Chronische Schmerzen bedingen angesichts ihrer bio-psycho-sozialen Komplexitat und der facherubergreifenden Prasenz eine interdisziplinare Zusammenarbeit. Diese erst ermoglicht eine umfassende Einordnung des Befundes und ist Vorraussetzung fur ein individuelles und ressourcenorientiertes Therapiekonzept mit dem Schwerpunkt der korperlichen und psychischen Aktivierung. Dieses Konzept ist Behandlungsschwerpunkt des UniversitatsSchmerzCentrums (USC), das im April 2004 im Universitatsklinikum Dresden gegrundet wurde und seitdem neben der bereits etablierten ambulanten Therapie auch teilstationare und stationare Versorgung ermoglicht. Ausgangspunkt der Grundungsinitiative fur das Schmerzzentrum war die Sensibilisierung der Kostentrager fur die komplexe Proble-matik des chronischen Schmerzes und ebenso fur die bestehenden regionalen Strukturdefizite. Gefolgt von der Vorlage eines schlussigen multimodalen, fachubergreifenden Behandlungskonzeptes mit ausfuhrlicher Kostenkalkulation wurde nach 1 1 2 -jahriger Arbeitsphase im Juni 2004 ein Vertrag zur integrierten Versorgung mit der AOK Sachsen und dem VdAK unterzeichnet. Nach nun zweijahriger Behandlungsphase sollen erste Erfahrungen und Ergebnisse, insbesondere der tagesklinischen Therapiesaule des Schmerzzentrums innerhalb des IV-Vertrages, dargestellt werden.

Zusammenfassung Die Etablierung Medizinischer Zentren soll den hohen Anforderungen im Gesundheitswesen gerecht werden. Dabei sollen zahlreiche Probleme wie z.B. die strukturierte interdisziplinare Zusammenarbeit der verschiedenen Fachdisziplinen unter Wahrung ihrer Autonomie einer guten und angemessenen Losung zugefuhrt werden. Dies erinnert an die gesetzgeberische Forderung zur Einfuhrung und Weiterentwicklung eines Qualitatsmanagementsystems in (zunachst) stationaren Einrichtungen bei zugleich fehlenden klaren Vorstellungen zur Aufbau- und Ablauforganisation als integraler Bestandteil des Managementkonzeptes eines Krankenhauses. Ernsthaften Bemuhungen um die Implementierung und Weiterentwicklung von interdisziplinaren Medizinischen Zentren stehen Marketinginteressen als Etikettenmodelle gegenuber. Denn der Begriff ist bisher ungeschutzt und der Patient, seine Angehorigen, die Bevolkerung, auch zuweisende Arzte und Einrichtungen des Gesundheitswesens bleiben in der Beurteilung der tatsachlichen Leistung eines Medizinischen Zentrums sich selbst uberlassen. Dies gilt auch fur so genannte zertifizierte Zentren, denn auch hier wachst der Markt der Anbieter, nicht aber die Transparenz, inwieweit ein zertifiziertes Zentrum tatsachlich einen messbaren, nachvollziehbaren und verstandlichen Vorteil fur den Patienten erbringt. Deshalb gilt es, klare Anforderungen an ein zertifiziertes Zentrum unabhangig vom Verfahren zu stellen. Dies betrifft insbesondere die Umsetzung eines Zentrumskonzeptes mit interdisziplinarem Anspruch, prozessorientierten transparenten Strukturen bis hin zur Definition von (Qualitats)kennzahlen und (Qualitats)zielen. Eingeschlossen sind auch die Sicherstellung der Ressourcen zur kontinuierlichen Erfassung einschlieslich der Bewertung der erhobenen harten und weichen Fakten sowie die Ableitung von Verbesserungsmasnahmen. Die drei Zentren im Universitatsklinikum Dresden, das Universitats KrebsCentrum (UCC), das Universitats GefasCentrum (UGC) und das Universitats SchmerzCentrum (USC) folgen diesem hohen Anspruch und haben Grundlagen zur messbaren Verbesserung der Patientenversorgung geschaffen bzw. erste Ergebnisse vorgelegt.

Whenever the internal reorganization of hospitals demands the implementation of interdisciplinary healthcare concepts in terms of interdisciplinary competence centers, the structural conflicts caused by subordination of autonomous structural units into centralized functional entities will have to be considered and solved. The Dresden University Vascular Center is an example of an interdisciplinary medical center based on a statute setting up binding rules of standing cooperation. In this case, the positive effects of concentration have been utilized, while leaving all structural units involved autonomous. This requires strict separation and transparent allocation of responsibilities and continuous and committed efforts on management and operating levels as described in the statute. Of course, this kind of close collaboration cannot be prescribed but requires permanent commitment to the general objectives as laid down in the statute and their acceptance as personal work goals.

Peer review focuses on critical self-reflection, especially of physicians in direct contact and dialogue with other disciplines and professional groups. The main purpose of the peer review is the principle of self-determination, the intention to learn from each other more effectively, and a commitment to quality improvement. During the past five years the Quality in Medicine Initiative (IQM) has successfully promotes peer reviewing in German-speaking areas. The aim of this study is to investigate all records of IQM peer reviews and to outline the results in the course of development. The major focuses include medical aspects for improvement, information concerning organizational processes and the satisfaction of the visited hospitals.A systematic descriptive analysis of all records as well as feedback sheets was conducted between 2010 and 2014.294 of 304 planned peer reviews were considered, involving among different hospital owners and different clinical conditions. The identified potential for improvement of the peer team was higher than that of the physicians of the hospitals visited. The assessment of the medical review criteria illustrates the following different trends: a positive development occurred concerning the appropriate and timely diagnostic and treatment; a clear potential for improvement could be established concerning a complete and conclusive documentation. Regarding the clinical conditions, anomalies were identified which are important for all IQM hospitals. As regards sustainability, first results from so-called re-reviews were available and provided important information pointing in the direction of structure and process improvement. The satisfaction of the visited physicians increased continuously within these five years.In the light of all data records and satisfaction sheets, the IQM peer review has become well-established over the last five years. Possible reasons are a well-defined framework, clear structures and different kinds of support. The success of the cooperation of the German Medical Association and IQM is illustrated by the development of the curriculum peer review. Re-reviews provide impulses for sustainability. At any rate, there is a need for scientific evaluation of the peer review method in order to generate evidence for this important tool of quality improvement.

Ohne Einsatz effektiver Analyse- und Reportingwerkzeuge kann kein klinisches Risikomanagement (RM) erfolgen. Viele Methoden und Werkzeuge werden teils noch als Insellosungen eingesetzt oder sind in Erprobung, wobei die USA in Bezug auf den Umsetzungsgrad eine Vorreiterrolle einnehmen. Verfahren mit langer arztlicher Tradition wie M&M-Konferenzen und Peer-Reviews werden fur das RM wieder entdeckt. Der Einsatz von IT und RM-Software ermoglichen u. a. die automatische Auswertung von Kenngrosen (GTT, PSI). Methoden wie CIRS, Fehlerursachenanalysen (RCA, London-Protokoll), Befragungen, Audits und diverse Beurteilungsverfahren einzelner Mitarbeiter, erfassen und analysieren nicht nur Risiken, sondern entfalten bereits alleine durch ihren Einsatz grose Wirkung auf die Sicherheitskultur und ihren Auspragungen wie Kommunikation und Fuhrungsverhalten. Szenarioanalysen und ganzheitliche Bewertungen klinischer Risikomanagement-Systeme stehen erst am Anfang ihres flachendeckenden Einsatzes.

Process management (PM) is a valuable method for the systematic analysis and structural optimisation of the quality and safety of clinical treatment. PM requires a high motivation and willingness to implement changes of both employees and management. Definition of quality indicators is required to systematically measure the quality of the specified processes. One way to represent comparable quality results is the use of quality indicators of the external quality assurance in accordance with Sect. 137 SGB V—a method which the Federal Joint Committee (GBA) and the institutions commissioned by the GBA have employed and consistently enhanced for more than ten years. Information on the quality of inpatient treatment is available for 30 defined subjects throughout Germany. The combination of specified processes with quality indicators is beneficial for the information of employees. A process-based indicator dashboard provides essential information about the treatment process. These can be used for process analysis. In a continuous consideration of these indicator results values can be determined and errors will be remedied quickly. If due consideration is given to these indicators, they can be used for benchmarking to identify potential process improvements.
