No head-to-head trials have compared the efficacy of tisagenlecleucel vs historical treatments for adults with relapsed or refractory diffuse large B-cell lymphoma (r/r DLBCL). This study indirectly compared the overall survival (OS) and overall response rate (ORR) associated with tisagenlecleucel, using data from the JULIET study (Study of Efficacy and Safety of CTL019 in Adult DLBCL Patients; #NCT02445248), vs historical treatments assessed in the CORAL (Collaborative Trial in Relapsed Aggressive Lymphoma) study follow-up population. To assess treatment effects in the treated (full analysis set [FAS]) and enrolled (intention-to-treat [ITT]) study populations, the JULIET FAS vs the CORAL follow-up FAS and JULIET ITT vs CORAL follow-up ITT populations were separately compared. Propensity score weighting using standardized mortality ratio weight (SMRW) and fine stratification weight (FSW) was used to compare OS and ORR, adjusting for baseline confounders. The results indicated that tisagenlecleucel was associated with a lower hazard of death among the FAS (adjusted hazard ratio [95% confidence interval], both FSW and SMRW, 0.44 [0.32, 0.59]) and ITT populations (FSW, 0.60 [0.44, 0.77]; SMRW, 0.57 [0.44, 0.73]; all, P < .001). Median OS was 12.48 months (JULIET) vs 4.34 to 4.40 months (CORAL) for the FAS, and 8.25 (JULIET) months vs 4.04 to 4.86 (CORAL) months for the ITT populations. Tisagenlecleucel was associated with a significantly higher ORR compared with historical treatments among the FAS (adjusted response rate difference [95% confidence interval], both FSW and SMRW, 36% [22%, 0.48%]; P < .001) and among the ITT populations after SMRW adjustment (11% [0%, 22%]; P = .043). This analysis supports that improved response and OS are achieved in patients with r/r DLBCL treated with tisagenlecleucel compared with those treated with alternative historical treatments.

Crispleaf lettuce, which is a light green leaf lettuce, is the most popular leafy vegetable in Brazil. However, it is relevant to evaluate agronomic traits of lettuce with more intense green color leaves considering the increase in consumer demand. Thus, the aim of this study was to evaluate lettuce genotypes about the different chlorophyll contents and correlate with green mass production in a hydroponic system. Ten lettuce inbred lines and two cultivars (Vanda and Luiza) were evaluated for agronomic traits and chlorophyll contents in leaves. A randomized block design with four replicates was used. The chlorophyll content was variable among the genotypes, ranging from 43.3 (Luiza) to 20.8 (Vanda). The genotypes with the highest chlorophyll content were those who showed the lowest mass production, but no correlations were found between these variables, though.

Summary Background In the primary analysis of the pivotal JULIET trial of tisagenlecleucel, an autologous anti-CD19 chimeric antigen receptor (CAR) T-cell therapy, the best overall response rate was 52% and the complete response rate was 40% in 93 evaluable adult patients with relapsed or refractory aggressive B-cell lymphomas. We aimed to do a long-term follow-up analysis of the clinical outcomes and correlative analyses of activity and safety in the full adult cohort. Methods In this multicentre, open-label, single-arm, phase 2 trial (JULIET) done at 27 treatment sites in ten countries (Australia, Austria, Canada, France, Germany, Italy, Japan, the Netherlands, Norway, and the USA), adult patients (≥18 years) with histologically confirmed relapsed or refractory large B-cell lymphomas who were ineligible for, did not consent to, or had disease progression after autologous haematopoietic stem-cell transplantation, with an Eastern Cooperative Oncology Group performance status of 0–1 at screening, were enrolled. Patients received a single intravenous infusion of tisagenlecleucel (target dose 5 × 108 viable transduced CAR T cells). The primary endpoint was overall response rate (ie, the proportion of patients with a best overall disease response of a complete response or partial response using the Lugano classification, as assessed by an independent review committee) at any time post-infusion and was analysed in all patients who received tisagenlecleucel (the full analysis set). Safety was analysed in all patients who received tisagenlecleucel. JULIET is registered with ClinialTrials.gov , NCT02445248 , and is ongoing. Findings Between July 29, 2015, and Nov 2, 2017, 167 patients were enrolled. As of Feb 20, 2020, 115 patients had received tisagenlecleucel infusion and were included in the full analysis set. At a median follow-up of 40·3 months (IQR 37·8–43·8), the overall response rate was 53·0% (95% CI 43·5–62·4; 61 of 115 patients), with 45 (39%) patients having a complete response as their best overall response. The most common grade 3–4 adverse events were anaemia (45 [39%]), decreased neutrophil count (39 [34%]), decreased white blood cell count (37 [32%]), decreased platelet count (32 [28%]), cytokine release syndrome (26 [23%]), neutropenia (23 [20%]), febrile neutropenia (19 [17%]), hypophosphataemia (15 [13%]), and thrombocytopenia (14 [12%]). The most common treatment-related serious adverse events were cytokine release syndrome (31 [27%]), febrile neutropenia (seven [6%]), pyrexia (six [5%]), pancytopenia (three [3%]), and pneumonia (three [3%]). No treatment-related deaths were reported. Interpretation Tisagenlecleucel shows durable activity and manageable safety profiles in adult patients with relapsed or refractory aggressive B-cell lymphomas. For patients with large B-cell lymphomas that are refractory to chemoimmunotherapy or relapsing after second-line therapies, tisagenlecleucel compares favourably with respect to risk–benefit relative to conventional therapeutic approaches (eg, salvage chemotherapy). Este documento procura reflexionar críticamente sobre el papel de la educación jurídica alternativa como piedra angular del método clínico. A partir de una breve lectura contextual del fenómeno global del COVID-19 y de sus impactos a nivel nacional, se devela la incapacidad real del estado social de derecho para extender la justicia social por todo el corpus popular. En particular, esta aseveración confirma la necesidad de descolonizar el derecho como campo reproductor de la colonialidad del ser, del saber y del poder. La metodología de este trabajo es la siguiente. Primero, se propone como antesala de la discusión una invitación a superar las lecturas legalocéntricas o tradicionalistas del derecho, que no se han cuestionado críticamente la función reproductora de la crisis de justicia social por parte del Estado moderno/colonial/capitalista. La categoría vertebradora de pueblo oculto permite entender que la discriminación y la exclusión social hoy agravadas por el COVID-19 están atravesadas por los derroteros del racismo implantado por la colonialidad. Estas dos últimas categorías vertebradoras (racismo y colonialidad) se constituyen como pilares explicativos del fenómeno del doble estado de excepción que emerge en las zonas periféricas del país. A partir de este escenario, se intenta esclarecer la importancia del método clínico sociojurídico y los objetivos fundamentales de su implementación desde un punto de vista integral, social y solidario. Se trata, por tanto, de plantear como horizonte la necesidad de repensar la metodología clínica, a partir de la integración de saberes otros que enriquezcan la educación jurídica en contexto y que permitan usar las tradicionales herramientas del derecho de interés público como insumos de lucha contrahegemónica, capaces de fracturar a nivel estructural la matriz de la colonialidad.

Dentro de los factores de estrés a los que puede estar expuesto el ganado lechero el relativo al calor es uno de los más conocidos y estudiados en vacas lecheras por el impacto que tiene sobre la producción láctea. Sin embargo, este nivel de conocimiento no se repite en relación a los terneros en crianza, por tal motivo el objetivo del presente trabajo fue el de estudiar el impacto del estrés por calor en esta categoría. Se monitorearon 246 terneros pertenecientes a 19 tambos en días con ITH ≥ 72 para la franja horaria de 12 a 15. Se categorizaron los registros obtenidos en: estrés ligero, moderado o severo. Se registró la posibilidad o no de acceder a sombra, el disponer o no de agua para consumo, la posición y la actividad principal que desarrollaba, también se evaluó la temperatura rectal (TR) y la frecuencia respiratoria (FR). Los datos fueron analizados mediante ANOVA. La comparación entre medias se realizó con el test Tukey (α=0,05) y el análisis de correlación con el test de Spearman. Se utilizó el programa InfoStat versión 2019p. En lo que refiere a actividad, no se observaron diferencias estadísticamente significativas. Sin embargo, sí se detectaron diferencias (p=0,0138) en la posición de aquellos animales expuestos a condiciones de estrés moderado vs. severo. La TR promedio fue e diferente (p=0,0063) entre los animales de la categoría estrés lijero respecto las otras dos categorías (> 39,2 °C). Los pacientes con enfermedad de Parkinson (EP) muestran sintomas no motores, como deterioro cognitivo, alteracion de las funciones ejecutivas y alteraciones del estado de animo. Dos procesos que se investigan actualmente en este tipo de pacientes son la Teoria de la Mente (ToM) y la toma de decisiones. La Teoria de la Mente es la capacidad de identificar los estados mentales (afectivos o cognitivos) en los demas, y es una habilidad necesaria para la comunicacion exitosa en situaciones sociales. El proceso de toma de decisiones se investiga en los pacientes con EP debido a las alteraciones en las vias dopaminergicas implicadas en los circuitos corticoestriatales, las cuales se han vinculado a las funciones cognitivas. Ambos procesos podrian estar alterados en los pacientes con EP despues de la terapia de estimulacion cerebral profunda (ECP). Con el objetivo de comparar el rendimiento de pacientes con EP (con y sin ECP) y controles sanos (HC) en teoria de la mente y tareas de toma de decisiones, aplicamos a tres grupos de pacientes (PD, n = 4; PD-DBS, n = 5 y HC, n = 5) la tarea de Yoni para identificar las caracteristicas afectivas y cognitivas en TM y la Tarea de Juego de Iowa (IGT) en la toma de decisiones. No hubo diferencias entre los grupos de EP en TM tanto en las caracteristicas afectivas como en las cognitivas. En cuanto a la toma de decisiones, obtuvimos resultados consistentes con los hallazgos anteriores, con pacientes con EP que mostraron impedimentos en este proceso. Algunos resultados sugieren que la terapia de ECP afecto el desempeno de los pacientes con EP en la toma de decisiones cuando se comparo con el grupo de control sano. Nuestros hallazgos describen algunos cambios no motores debidos a la ECP que se observan a menudo en los pacientes con EP.

We highlight in this dossier, among the issues that bring Latin American countries together, the political challenges surrounding the provision of education and the strengthening of democracy, and the reflection of the adoption of political measures that lead to a high percentage of indigence and poverty in most countries of the region. The political-economic agenda was built on the tension from two different perspectives, the implementation of neoliberal and conservative policies, bumps into collective organization to resist exclusion and propose different social projects. It is in this context, increasingly fierce in terms of conflicts and struggles, that reflections are needed on various aspects that shape our reality, such as education and the strengthening of democracy. It is along these lines that we are interested in the connection between education and other areas of social life, such as: education and work, education and citizenship, education and teaching work, education and evaluation, education and international organizations; among others. This is how we present reflections on different perspectives that problematize some aspects of educational policies and practices in the region. We invite you to get to know, debate and exchange experiences about the reality through the contributions on education and its alternatives, criticism and theoretical reflections present in this Dossier and that they help to build more democracy through education and social justice.

Una encuesta transversal se llevó a cabo en 35 unidades productivas familiares (UPF) que criaban caprinos en el Municipio de Payogasta (Salta) con el objetivo de describir las prácticas productivas. Se recabaron datos del manejo, alimentación, reproducción y sanidad de los caprinos. Las UPF criaban en promedio 80,1±57,1 caprinos (sin contar cabritos). El biotipo predominante fue Anglo Nubian cruzados con Criollo. El sistema productivo fue extensivo con encierre nocturno y la alimentación se basaba en el pastoreo de los cerros, de alfalfa, rastrojos o residuos de cosecha. El 100% de las UPF practicaba el ordeñe manual destinado a elaborar quesos. En el 51,4% de las UPF el servicio era estacionado, mientras que en el resto continuo. En el 65,7 % de las UPF se desparasitaba rutinariamente. Aquellas UPF que estacionaban servicio y desparasitaban mostraron una tendencia a obtener mejores valores productivos. Estos resultados muestran limitantes productivas quecon prácticas sencillas sería posible incrementar la producción y elevar el bienestar de las familias. A cross-sectional survey was performed in 35 family farming units (FFU) in the Municipality of Payogasta (Salta), with the aim of describing production practices in their goat flocks. Data on management, feeding, reproduction and health practices were recorded. FFU had on average 80.1 ±57.1 goats (not including kids). The predominant biotypes was Anglo Nubian (60.5%) with Criollo crosses. Animals are extensively managed, with nocturnal enclosure, and flock feeding was based on grazing the hills, lucerne, crops or their residues. The 100% of the FFU produced milk by hand-milking to make cheeses. Breeding was stationed (51.4%), while in the rest it was continuous. The 65.7% of the FFU routinely dewormed. Those FFU that had stationed mating system and dewormed showed to obtain a tendency of higher production values. These results show important productive limitations that, with simple practices it would be possible to increase production and the well-being of producing families. Con el objetivo de conocer la influencia de las estaciones del año respecto de la composición y la calidad sanitaria de leche bovina de la cuenca del Valle de Lerma se llevó adelante el análisis de las muestras de leche remitidas por la industria al Laboratorio de Leche de la Universidad Nacional de Salta durante la primavera (Pr, n:104), el verano (Ve, n:131) y el invierno (In, n: 118) de los años 2017 y 2019. Se determinó el porcentaje de grasa (% G) y de proteína (% P), el punto de descenso crioscópico (DC) y el recuento de células somáticas/ml (RCS/ml). El análisis estadístico se realizó utilizando el programa InfoStat 2019p. El modelo respondió a un DCA. El % G y P fueron analizados mediante ANOVA. The SARS-CoV-2 variant, B.1.1.519, arose in North and Central America, circulating primarily in Mexico. We demonstrate that this variant peaked during the second wave of COVID-19 in Mexico City in the spring of 2021. This variant is likely more infectious, attributed to mutation in the RBD of the spike protein T478K also seen in the alpha variant (B.1.1.7). However the time dynamics of the spread of this variant drastically changed upon the introduction of delta (B.1.617.2) to the country in which we observe a shift from 0% in May 2021 to 55% delta in the span of one month. Since the delta variant has dominantly spread across the globe, we investigated the increasing frequency of the Mexico variant, B.1.1.519, in the public community within Mexico City. Once present, the delta variant was 78% of the Mexico City catchment in July 2021, a time which marked the commencement of Mexico’s third wave. Our data supports the growing concern that the delta variant is closely associated with the massive infection spread of the VOC in Central and South America. While the T478K mutation, also seen in the alpha variant, has evidence for increased transmissibility, these data suggest that the delta variant shows overall increased fitness seeing as it outcompeted the B.1.1519 this Mexico community.

Con el objetivo de investigar los efectos de los abortos bovinos en la cuenca lechera del Valle de Lerma se estudiaron los registros de datos de 128 vacas Holstein en ordeño obtenidos de software de 4 tambos. Los abortos se clasificaron en: vacas abortadas, vueltas a inseminar que continúan en lactancia (VAC, n= 36) y vacas que abortaron e iniciaron en una nueva lactancia (VAI, n=33), además se les sumaron los registros de vacas con partos normales (VPN, n= 59). Se registraron los litros de leche a los 305 días de lactancia (L/305), los días en lactancia (DEL), los intervalos entre partos (IPP), número de eventos de salud ocurridos pos aborto y el destino final de los animales (continúa en el tambo, descarte, muerte o sacrificio). Las variables continuas se sometieron a análisis de varianza en un modelo lineal mixto y las discretas a la prueba de Chi cuadrado y Odds Ratio (OR). Background: Obesity is a major public health concern worldwide. Latin America has experienced rapid growth in obesity incidence during the last few decades. Driven by confinement measures, a telemedicine program was implemented in March 2020 to give continuity to obese patients’ care through a weight loss program led by the endocrinology department in a tertiary care medical center in Latin America. Objective: This study aimed to describe the clinical experience of using digital health for monitoring and attention of obese patients and description of weight change outcomes of these patients followed via telemedicine during March 2020–December 2020. Methods: A retrospective cohort study was conducted including 202 patients. A Skillings-Mack test was performed to conduct a subgroup analysis of the medians of the weight over the follow-up period, and a mixed multiple linear regression model was performed to estimate the expected average change in weight over time Results: We observed good adherence to the program, represented by a weight loss of −4.1 kg at three months of follow-up, which was maintained even during the sixth month of follow-up. Conclusions: Digital Health strategies such as telemedicine can be a helpful tool for both patients and health care providers to support the continuity of care and showing satisfactory results in the management of obese patients.

It is unclear whether the benefits of this activity on the cognitive level are associated with brain functions that are necessary for their performance or recurrence of activity or type of activity itself. Glycated hemoglobin is currently the gold standard for assessment of long-term glycemic control and response to medical treatment in patients with diabetes. Glycated hemoglobin, however, does not address fluctuations in blood glucose. Glycemic variability (GV) refers to fluctuations in blood glucose levels. Recent clinical data indicate that GV is associated with increased risk of hypoglycemia, microvascular and macrovascular complications, and mortality in patients with diabetes, independently of glycated hemoglobin level. The use of continuous glucose monitoring devices has markedly improved the assessment of GV in clinical practice and facilitated the assessment of GV as well as hypoglycemia and hyperglycemia events in patients with diabetes. We review current concepts on the definition and assessment of GV and its association with cardiovascular complications in patients with type 2 diabetes.

Bovine rotavirus A (RVA) and bovine coronavirus (CoV) are the two main viral enteropathogens associated with neonatal calf diarrhea. The aim of the present work was to study the impact of group and individual housing systems in the epidemiology of RVA and CoV infection. Eleven calves reared in individual housing (FA) and nine calves in group housing (FB) were monitored during the first 7 weeks of life. Stool and serum samples were screened for RVA and CoV antigens by ELISA. IgG1 antibodies (Ab) to both antigens were also measured. From the 160 fecal samples collected, the proportion of positive samples to RVA and CoV was significantly higher in FB (23.6%) than in FA (9%) (p = 0.03). The geometric mean of colostral IgG1 Ab titers to CoV and RVA in FA (IgG1 anti-CoV 1024 and anti-RVA 1782.9) was lower than in FB (IgG1 anti-CoV 10,321.2 and anti-RVA 4096) at birth. Calves less than 2 weeks of life from FB had a higher risk of being infected by RVA (OR = 4.9; p = 0.01) and CoV (OR = 17.15; p = 0.01) than calves from FA. Capsicum chinense peppers are considered the most Brazilian of the species and have wide genetic diversity among cultivars. Its demand is increasing, requiring the development of new hybrid cultivars, with better agronomic and nutritional categories. The objective of the study was to carry out agronomic, physical-applied and sensory evaluations of five hybrids and an open pollination cultivar (OP) (BRS Moema) of C. chinense. The five hybrids obtained higher productivity, yield and biometry of fruits, being ideal for the processed and fresh market. The cultivar BRS Moema and two hybrids (F1 10 and F1 12), had better fruit properties (lipids and fibers), and the F1 12 hybrid stood out for containing the highest antioxidant activity and capsaicin content. From the sensory point of view, the hybrids had greater sums for the attributes of preference (size, color, pungency and sweet taste). Thus, it is concluded that the hybrids F1 12 and F1 23 have the potential to be new cultivars in terms of productivity and yield, combining good agronomic, nutritional and sensory characteristics for a C. chinense species.

This study compared overall survival (OS), progression-free survival (PFS), complete response rate (CRR), and overall response rate (ORR) of tisagenlecleucel (tisa-cel) and lisocabtagene maraleucel (liso-cel) in relapsed or refractory large B-cell lymphomas (r/r LBCL). Using matching-adjusted indirect comparison (MAIC), individual patient-level data from JULIET (tisa-cel) were weighted to match the patient population in TRANSCEND (liso-cel). The main analysis compared infused JULIET patients (N = 106) with the TRANSCEND efficacy-evaluable set (EES) (N = 256 [infused]). After adjustment, OS, PFS, and the CRR were comparable between tisa-cel and liso-cel EES patients. The estimated adjusted 2-year OS, 2-year PFS, ORR, and CRR were 45.6, 38.2, 62.9, and 47.7%, respectively, for tisa-cel vs. 43.8, 42.1, 72.7, and 53.1% for liso-cel. A scenario analysis compared JULIET patients to the TRANSCEND primary analysis set (PAS) (N = 133). ORR was significantly higher in the TRANSCEND PAS compared with matched JULIET patients, but no significant differences in CRR were observed.
