Abstract

International audience; Human beings are social animals. This ability to live together is ensured by cognitive functions, the neuroanatomical bases of which are starting to be unraveled by MRI-based studies. The regions and network engaged in this process are known as the "social brain ". The core of this network is the superior temporal sulcus (STS), which integrates sensory and emotional inputs. Modeling studies of healthy volunteers have shown the role of the STS.in recognizing others as biological beings, as well as facial and eye-gaze recognition, intentionality and emotions. This cognitive capacity has been described as the "theory of mind ". Pathological models such as autism, in which the main clinical abnormality is altered social abilities and communication, have confirmed the role of the STS in the social brain. Conceptualisation of this empathic capacity has been described as "meta cognition ", which forms the basis of human social organizationand culture.

Abstract

International audience; Schizophrenia is a common psychiatric illness (1% of the general population), characterized by the association of positive and negative symptoms and cognitive disorders. Antipsychotics, typical or atypical, are known to induce in patients with schizophrenia weight gain and abnormalities in glucose and lipid metabolisms. These modifications, in addition to metabolic risk factors, intrinsic to the psychiatric illness (physical inactivity, smoking, diabetes), increase the risk of cardiovascular complications. Some antipsychotics are associated with a higher risk of metabolic disorders. Before starting such a medication, all risk factors must be taken into account. In case of even effectiveness, one should consider the risk of inducing metabolic disorders, as well as the intrinsic risk factors of the patient, in order to prescribe the medication associated with the lower metabolic risk. Regarding iatrogenic diabetes, the risk of occurrence seems different, depending on the molecules, being more marked for clozapine, olanzapine, risperidone, quietapine then amisulpride, aripiprazole and finally ziprasidone. The physiopathology seems to involve both an increase in insulin resistance and an alteration of insulin secretion. Nevertheless, the benefit/risk often remains largely in favour of treatment, the atypical antipsychotics are at least equally effective and better tolerated on the cognitive and neurological functions than conventional antipsychotics being. They have particularly far fewer extrapyramidal effects. The reversibility of pathologies induced by atypical antipsychotics led to the formulation of guidelines, leading to regular clinical and biological follow-up.

Abstract

International audience; Enhancement of semantic priming effects (hyperpriming) has been well documented in schizophrenic patients (Minzenberg et al. 2002). In a previous study, we demonstrated that hyperpriming was related to a more pronounced requirement to inhibit unrelated information (Lecardeur et al. 2007). On the other hand, several authors have suggested that hyperpriming could reflect enhanced spreading of semantic activation in schizophrenia (Spitzer et al. 1993; Spitzer 1997; Moritz et al. 2001). So far, there are few reports that attempted to study the spreading of activation in schizophrenic patients compared to healthy controls, when the links between semantically related information were manipulated. Notably, authors stated that activation could consequently spread farther and further in the semantic networks of schizophrenic patients. Moreover, enhanced spreading of activation was hypothesized to be responsible of the appearance of thought and language disorders in schizophrenic patients. Consequently, we examined whether the manipulation of the type of connection and the semantic distance between prime and target can modulate semantic priming effects in schizophrenic patients compared to healthy controls.

Abstract

Very early onset schizophrenia is a psychiatric syndrome in children under 13 years of age that is characterized by a very poor prognosis, due to frequent resistance to treatment. We report here the case of a hospitalized 11-year-old child (YP), with a positive diagnosis of very early onset schizophrenia for the past 2 years (positive K-SADS-DSM-IV), without any previous sign of pervasive developmental disorder. YP experienced verbal auditory hallucinations, a delusion of alien control and heteroaggressive behaviour. Antipsychotic drugs were unsuccessful and furthermore, provoked severe acute dystonia. A 72-h EEG recording, an anatomical MRI brain scan and a complete set of metabolic analyses, were all negative. However, using data-driven analysis, we observed that YP’s functional MRI scan revealed bilateral neural activity in the auditory cortex during auditory verbal hallucinations (Figure 1). Ten sessions of fMRI-guided, low frequency (1 Hz), repeated trans-cranial magnetic stimulation (rTMS) over the left temporo-parietal cortex, were successful in stopping the verbal auditory hallucinations (Auditory Hallucination Rating Scale 47%). The improvement obtained with rTMS was maintained by repeating the sessions every 5 weeks. The clinical improvement was confirmed by a significant improvement of adaptative functions (Children’s Global Assessment Scale þ40%), and the fact that YP was able to go back home and attend school. Thereby, YP was able to receive education about his illness and to follow regular psychotherapeutic sessions. rTMS has been used for hallucinations in adults, and in children with attentional deficits and hyperactivity. To our knowledge, however, this is the first published case demonstrating the efficacy of fMRI-guided rTMS in the treatment of verbal auditory hallucinations in a child with schizophrenia. These results require replication.

Abstract

International audience; This paper describes the application of large deformation diffeomorphic metric mapping to cortical surfaces based on the shape and geometric properties of subregions of the superior temporal gyrus in the human brain. The anatomical surfaces of the cortex are represented as triangulated meshes. The diffeomorphic matching algorithm is implemented by defining a norm between the triangulated meshes, based on the algorithms of Vaillant and Glaunès. The diffeomorphic correspondence is defined as a flow of the extrinsic three dimensional coordinates containing the cortical surface that registers the initial and target geometry by minimizing the norm. The methods are demonstrated in 40 high-resolution MRI cortical surfaces of planum temporale (PT) constructed from subsets of the superior temporal gyrus (STG). The effectiveness of the algorithm is demonstrated via the Euclidean positional distance, distance of normal vectors, and curvature before and after the surface matching as well as the comparison with a landmark matching algorithm. The results demonstrate that both the positional and shape variability of the anatomical configurations are being represented by the diffeomorphic maps.

Abstract

International audience; Previous studies analyzing semantic priming in schizophrenic patients have reported conflicting results. In the present study, we explored semantic priming in a sample of schizophrenic patients with mild thought disorders. We wondered if distinct cognitive processes, such as facilitation and/or inhibition, underlie semantic hyperpriming and are variously impaired in schizophrenic patients. Using a lexical decision task, we evaluated semantic priming in 15 schizophrenic patients (DSM-IV) with mild thought disorders and 15 healthy controls matched for sex, age, and education level. The task was designed to divide semantic priming into two additive components, namely facilitation effect and inhibition effect. One-sample t-tests were performed to investigate differences in semantic priming, facilitation, and inhibition within each group. ANOVAs were performed to compare the effects of semantic priming, facilitation, and inhibition between groups. Patients displayed greater semantic priming than controls (i.e., hyperpriming), but this was not due to increased facilitation in processing semantically related pairs. On the contrary, hyperpriming was the result of prolonged response time to process semantically unrelated pairs, corresponding to a requirement to inhibit unrelated information. We demonstrated semantic hyperpriming in stabilized schizophrenic patients with mild severity of symptoms. Thus, semantic hyperpriming may be an intrinsic feature of schizophrenia that is not related to the clinical state of patients. Semantic hyperpriming was due to an inhibition effect involved in processing semantically unrelated information not to increased facilitatory effect for related pairs.

Abstract

International audience; Dysregulation of free radical metabolism has been supposed to be involved in schizophrenia etiopathogeny. Recently, Wang et al. showed a red blood cell super oxide dismutase increase in positive schizophrenia (Crow's type I), but neither in negative schizophrenia (Crow's type II) nor in controls. The study included 28 in-patients suffering from acute positive psychosis who were compared with 15 controls. We confirmed the results of Wang. We found a significantly red blood cell Super oxide dismutase increase in positive psychosis, in comparison to negative psychosis and controls (p = 0.0001). This SOD increase was in relationship with the degree of clinical psychomotor excitement. After 21 days of neuroleptic treatment, SOD activity decreased and reached standard values. These results support the hypothesis of striking relationships between catecholaminergic hyper-metabolism and SOD increase, in positive psychosis. These could account for psychotic positive symptoms improvement with neuroleptic treatment, which blocks dopamine pathways.

Abstract

International audience; Serum levels of haloperidol and reduced haloperidol as well as the reduced haloperidol/haloperidol ratios were determined in nine acute schizophrenics on oral haloperidol medication and correlated over 21 days with psycho pathology and extra-pyramidal symptom scores. We have investigated red blood cells haloperidol reductase activity in the group of patients. Significant correlations were found between haloperidol plasma levels and positive sub scale for each patient (r = 0.86 and p < 0.01; r = 0.70 and p < 0.05). We found a correlation between red blood cells reductase activity and the improvement of the psychotic anxiety scale (r = 0.64/and p < 0.05; r = 0.67 and p < 0.05), but not with reduced haloperidol/haloperidol ratios in plasma. The knowledge of reductase activity could predict the treatment response in acute schizophrenic patients. We suggest that the reported inter individual and inter ethnic differences in haloperidol and reduced haloperidol and in clinical response and adverse effects may be a reflection of genetic control of the two oxidative pathways mediated by cytochrome P450 isozyme and/or the reductase pathway mediated by haloperidol reductase in individual subject.
