Abstract

11 pages; La morphologie crânienne de 28 spécimens de douroucoulis (genre Aotus), provenant du Bassin amazonien, a été étudiée à l'aide de méthodes de morphométrie géométrique en trois dimensions. De nouveaux résultats concernant la distinction morphologique de populations de l'espèce Aotus infulatus, réparties de part et d'autre du rio Tocantins, sont proposés. Ces résultats morphologiques concordent avec la distinction génétique de ces populations publiée par Schneider – et Sampaio –, et indiquent probablement de récents et rapides changements évolutifs au sein de cette espèce. Les résultats de cette étude ont une application taxinomique, mais pourraient également ouvrir de nouvelles perspectives médicales, puisque la sensibilité des douroucoulis à la malaria semble varier selon les espèces.

Abstract

International audience; Since 2008, the French guidelines have promoted the systematic use of 30 mg/day of primaquine for the radical cure of Plasmodium vivax and Plamodium ovale infections. We observed three relapses in 10 patients with P vivax acquired in French Guiana. No relapses were seen in West African P ovale patients.

International audience; Abstract Of 110 French troops who spent 4 months in Côte d'Ivoire in 2008, 12 (10.9%) experienced 14 malaria attacks after returning to France. An anonymous survey (response rate 39%) showed a high rate of poor compliance with chemoprophylaxis, including premature discontinuation after return in over half of the respondents.

Abstract

International audience; We present a case of severe malaria due to Plasmodium malariae. Genetic testing showed that the patient was homozygous for five important gene polymorphisms previously shown to be associated with increased susceptibility to, and/or severity of, severe sepsis. Our case suggests that P. malariae may cause life-threatening disease, and that disease severity may be linked, at least in part, to multiple susceptibility genes.

Abstract

Background Malaria diagnosis is vital to efficient control programmes and the recent advent of malaria rapid diagnostic tests (RDTs) provides a reliable and simple diagnostic method. However a characterization of the efficiency of these tests and the proteins they detect is needed to maximize RDT sensitivity. Methods Plasmodial lactate dehydrogenase (pLDH) gene of wild isolates of the four human species of Plasmodium from a variety of malaria endemic settings were sequenced and analysed. Results No variation in nucleotide was found within Plasmodium falciparum, synonymous mutations were found for Plasmodium malariae and Plasmodium. vivax; and three different types of amino acid sequence were found for Plasmodium ovale. Conserved and variable regions were identified within each species. Conclusion The results indicate that antigen variability is unlikely to explain variability in performance of RDTs detecting pLDH from cases of P. falciparum, P. vivax or P. malariae malaria, but may contribute to poor detection of P. ovale.

Abstract

International audience; INTRODUCTION:Malaria has been eradicated in Tunisia since 1979. Although it continues to be evoked in the case of fever after travel to an endemic zone, its diagnosis is however difficult during relapses, notably when they are delayed.OBSERVATION:A 50 year-old man having lived in Mauritania from 1978 to 1982 was hospitalized for interstitial pneumopathy and urarthritis. In spite of treatment with broad spectrum antibiotics, the fever accompanied by abundant sweating persisted. A thick blood drop and blood smear was requested and led to the diagnosis of Plasmodium malariae malaria.DISCUSSION:This observation recalls the possibility of parasitic upsurge of some plasmodial species. It should prompt physicians to be careful and evoke malaria in the case of fever in subjects having stayed, even several years before, in an endemic zone. This would permit early diagnosis and treatment.

Abstract

International audience; BACKGROUND: Malaria is influenced by a web of individual and ecological factors, i.e. factors relating to people and relating to environment. For a long time analysing these factors concurrently has raised statistical problems. Multilevel modelling provides a new attractive solution, which is still uncommon in tropical medicine. METHODS: Using an actual data set of 3864 individuals from 38 villages of the Highland Madagascar, a two-level modelling process is presented. Individual malaria parasitaemia is modelled step by step according to age (individual factor), altitude, and DDT indoor house-spraying status (village factors). RESULTS: The hierarchical organization of a data set in levels, fixed and random effects, and cross-level interactions are considered. Accurate estimations of standard errors, impact of unknown or unmeasured variables quantified and accounted for through random effects, are the highlighted advantages of multilevel modelling. CONCLUSION: While not denying the importance of understanding an aetiological chain, the authors recommend an increased use of multilevel modelling, mainly to identify accurately ecological targets for public health policy.

Abstract

International audience; The Africa Child Survival Initiative-Combatting Childhood Communicable Diseases (ACSI-CCCD) Project is a primary health care activity that focuses on antenatal care, immunization, diarrhoeal disease control, and malaria control in children under 5 years of age. In order to gauge progress made in the project, a community-based health interview survey to measure simultaneously several prevention and treatment indicators was carried out in 1986 in Conakry, Guinea. A sample of 1415 caretakers and their 2048 children aged under 5 years was visited using a cluster sampling technique. The survey documented the levels of literacy and health education awareness of the caretakers, measured the vaccination coverage levels for children and women of childbearing age, and determined treatment practices for diarrhoea and malaria. Of the 637 women who reported having given birth in the previous 12 months, 96% had visited an antenatal clinic, but only 49% had had two or more doses of tetanus toxoid, and 13% took weekly chemoprophylaxis against malaria. The vaccination coverage for measles was 16% for children aged 12-23 months. Oral rehydration therapy (ORT) was given to 16% of children with diarrhoea; however, only 43% of those who were administered ORT at home were treated according to standard guidelines. Of children with diarrhoea, 51% were given antidiarrhoeal or antimicrobial drugs by caretakers. Fever was treated at home for 79% of the febrile children, and 43% of those with fever also visited health units. The use of injectable antimalarials and prolonged treatments with chloroquine were common. Combining findings from a population-based community study with an assessment of practices in health facilities can provide reliable information for the implementation and monitoring of selective components of primary health care.
