Abstract Aims There is limited data describing major adverse kidney events (MAKE) in patients supported with ventricular assist devices (VAD). We aim to describe the association between MAKE and survival, risk factors for MAKE, and renal trajectory in VAD supported patients. Methods and results We conducted a single‐centre retrospective analysis of consecutive VAD implants between 2010 and 2019. Baseline demographics, biochemistry, and adverse events were collected for the duration of VAD support. MAKE was defined as the first event to occur of sustained drop (>50%) in estimated glomerular filtration rate (eGFR), progression to stage V chronic kidney disease, initiation or continuation of renal replacement therapy beyond implant admission or death on renal replacement therapy at any time. One‐hundred and seventy‐three patients were included, median age 56.8 years, 18.5% female, INTERMACS profile 1 or 2 in 75.1%. Thirty‐seven patients experienced MAKE. On multivariate analysis, post‐implant clinical right ventricular failure and the presence of chronic haemolysis, defined by the presence of schistocytes on blood film analysis, were significantly associated with increased risk of MAKE (adjusted odds ratio 9.88, P

Takayasu arteritis (TAK) is a rare large-vessel vasculitis that is seen primarily in young females of Asian descent and is infrequently diagnosed in the United States. Pericardial effusion with or without pericarditis as a presenting feature of TAK is rare, with only about five percent of cases of pericarditis attributable to any autoimmune etiology. We present a case of a 22-year-old Caucasian woman who presented with a large, symptomatic pericardial effusion of unclear etiology, who after extensive laboratory workup and imaging to include whole-body positron emission tomography (PET) was diagnosed with TAK. In our patient, the use of whole-body PET showing characteristic hypermetabolism within the aortic arch helped secure our diagnosis while avoiding the need for pericardiocentesis. The patient had rapid symptomatic and radiographic improvement with the use of high-dose oral steroids in addition to colchicine and ibuprofen for her pericarditis and associated pericardial effusion. At follow-up just 1 week after initiation of steroids, only trace effusion was identified on transthoracic echocardiogram.

A number of researchers have called for additional investigations into cognitive ability and intelligence in recent years. This paper utilized a person-centered approach, multiple cognitive ability dimensions, and latent profile analysis to investigate multivariate relationships among cognitive ability dimensions in a sample of 1681 Army recruits. Six cognitive ability dimensions were assessed via the Armed Services Vocational Aptitude Battery. Performance measures were obtained from supervisor ratings of Effort, Discipline, and Peer Leadership. Using latent profile analysis, the results identified five distinct cognitive profiles or classes, which differed significantly across the three types of supervisor ratings.

A search for time-directional coincidences of ultra-high-energy (UHE) photons above 10 EeV with gravitational wave (GW) events from the LIGO/Virgo runs O1 to O3 is conducted with the Pierre Auger Observatory. Due to the distinctive properties of photon interactions and to the background expected from hadronic showers, a subset of the most interesting GW events is selected based on their localization quality and distance. Time periods of 1000 s around and 1 day after the GW events are analyzed. No coincidences are observed. Upper limits on the UHE photon fluence from a GW event are derived that are typically at ∼7 MeV cm ^−2 (time period 1000 s) and ∼35 MeV cm ^−2 (time period 1 day). Due to the proximity of the binary neutron star merger GW170817, the energy of the source transferred into UHE photons above 40 EeV is constrained to be less than 20% of its total GW energy. These are the first limits on UHE photons from GW sources.

Abstract In the Tyrrhenian back‐arc basin, the Ventotene Volcanic Ridge (VR) represents a NW‐SE‐oriented morphological high, located 25 km north of Ventotene Island in the Eastern Pontine Islands. It is considered part of an alignment of volcanic edifices developed in the central‐eastern Tyrrhenian Sea, from the western Pontine Islands (WPI) to the Campanian Volcanic Region. The samples dredged along the VR are basalts to trachybasalts belonging to the transitional (TR) rock‐series, with a 40Ar‐39Ar age of 2.760 ± 0.039 Ma. Petrological and geochronological data link the VR basalts to the TR peralkaline rhyolites cropping out at the WPI (∼1.6‐1.1 Ma); here, during a time span of ∼4.5‐1.0 Ma highly differentiated products of high‐potassium calc‐alkaline (HKCA, ∼4.5‐2.9 Ma) and K‐alkaline (KA, ∼1 Ma) series were also erupted. Notably, the parental melts of the three rock‐series were recognized in the continental slope close to the WPI (TR, HKCA) and eastward, namely in eastern Pontine Islands (KA), at the VR (TR), and in the Campanian Plain (HKCA). All the parental melts show typical orogenic geochemical signatures being originated from a similar heterogeneous suprasubduction mantle source, without the involvement of anorogenic sources. Tectonically, the faults bordering the VR, associated to the Pliocene E‐W directed rifting stage of the Tyrrhenian back‐arc basin were characterized by a more intense activity during the time span ∼5.0‐2.0 Ma (the emplacement time of HKCA and TR series), while KA products are related to a later stage of extension deformation (

Previously, in Hermundstad et al., 2014, we showed that when sampling is limiting, the efficient coding principle leads to a ‘variance is salience’ hypothesis, and that this hypothesis accounts for visual sensitivity to binary image statistics. Here, using extensive new psychophysical data and image analysis, we show that this hypothesis accounts for visual sensitivity to a large set of grayscale image statistics at a striking level of detail, and also identify the limits of the prediction. We define a 66-dimensional space of local grayscale light-intensity correlations, and measure the relevance of each direction to natural scenes. The ‘variance is salience’ hypothesis predicts that two-point correlations are most salient, and predicts their relative salience. We tested these predictions in a texture-segregation task using un-natural, synthetic textures. Lo scopo del presente studio è stato quello di valutare lefficacia oncologica della laringectomia totale di salvataggio in pazienti precedentemente sottoposti a laringectomia subtotale open o microchirurgia laser transorale affetti da carcinoma squamocellulare laringeo. Abbiamo analizzato retrospettivamente le informazioni cliniche, chirurgiche e patologiche di 35 pazienti sottoposti a laringectomia totale di salvataggio dopo recidiva di carcinoma laringeo (laringectomia subtotale open o transorale). Le informazioni sono state analizzate tramite lutilizzo delle curve di Kaplan-Meier nonché tramite lanalisi univariata e multivariata dei fattori prognostici. Non sono emerse differenze statisticamente significative nel confronto tra il gruppo di pazienti precedentemente sottoposti a laringectomia subtotale ed il gruppo sottoposto a microchirurgia laser transorale sia in termini di overall survival (OS) e disease specific survival (DSS) a 3 anni (OS = 38% vs 52%, p = 0,16; DSS = 40% vs 61%, p = 0,057) che di controllo locoregionale (LRC) a 2 anni (LRC = 40% vs 54%, p = 0,056). È stata tuttavia messa in evidenza una tendenza che indica una sopravvivenza e controllo locoregionale peggiore nei pazienti sottoposti a laringectomia subtotale. La conservazione dello scheletro osteocartilagineo della microchirurgia laser transorale si traduce ipoteticamente in una maggiore probabilità di salvataggio delle recidive anteriori con diffusione extralaringea.

Spirulina platensis contains several compounds showing nutritional and therapeutic benefits. Recently, a series of peptides able to reduce the blood pressure level and to enhance the endothelial vasorelaxation was isolated from the hydrolyzed highly water-soluble Spirulina extract (HSE). However, HSE shows critical organoleptic characteristics also having poor intestinal permeability, limiting absorption when orally delivered. This research aims to overcome the critical issues through the encapsulation of HSE in Chitosan/Mannitol—(CM)-based microparticles by spray drying. The produced powders (CM-HSE) showed good process yield (≈70%) and encapsulation efficiency (≈100%) also having good derived flow properties as well as stability up to six months storage. The microparticles constituting the spray-dried powder resulted in an amorphous micrometric state (d50 ≈ 14 µm) able to retain dark colour and unpleasant smell of raw HSE. Moreover, the in vitro permeation study by Franz cell indicated that the engineered microparticles are able to enhance the permeation of HSE through an intestinal biomimetic barrier (551.13 μg/cm2 CM-HSE vs. 315.46 μg/cm2 HSE at 270 min).
