1. Effects of mechanical treatment and exchanged cation on the microporosity of vermiculite

Abstract

The influence of the interlayer cations, sonication and grinding on surface area of the vermiculite from Santa Olalla has been studied. For untreated samples the surface area increases with the size of the exchanged cation and only the samples with the largest cations (NH+ and K+) show some microporosity. Sonication produces an increase of the surface area but the amount of micropores remains almost unchanged. Grinding leads to a significant increase of the surface area and of the microporosity. In the ground samples the percentage of micropores contributing to the surface area is influenced by the interlayer cation and the extent of N2 penetration by the water kept in the sample at a given degassing temperature. From transmission electron microscopy, thermogravimetry and XRD (d00l diffraction peaks) it can be concluded that grinding produces imperfections in crystal ordering, some alteration in the crystal edges, and a significant release of loosely bound water at temperatures up to 450 8C. These effects are responsible for the high surface area and microporosity measured by the N2 adsorption.

Abstract

Abstract Small animal models have been a challenge for the study of SARS-CoV-2 transmission, with most investigators using golden hamsters or ferrets. Mice have the advantages of low cost, wide availability, less regulatory and husbandry challenges, and the existence of a versatile reagent and genetic toolbox. However, adult mice do not robustly transmit SARS-CoV-2. Here we establish a model based on neonatal mice that allows for transmission of clinical SARS-CoV-2 isolates. We characterize tropism, respiratory tract replication and transmission of ancestral WA-1 compared to variants Alpha (B.1.1.7), Beta (B.1.351), Gamma (P.1), Delta (B.1.617.2), Omicron BA.1 and Omicron BQ.1.1. We identify inter-variant differences in timing and magnitude of infectious particle shedding from index mice, both of which shape transmission to contact mice. Furthermore, we characterize two recombinant SARS-CoV-2 lacking either the ORF6 or ORF8 host antagonists. The removal of ORF8 shifts viral replication towards the lower respiratory tract, resulting in significantly delayed and reduced transmission in our model. Our results demonstrate the potential of our neonatal mouse model to characterize viral and host determinants of SARS-CoV-2 transmission, while revealing a role for an accessory protein in this context.

Abstract We discovered that knifefish (Apteronotus albifrons) during suction feeding can produce millimeter-sized cavitation bubbles and flow accelerations up to ~ 450 times the acceleration of gravity. Knifefish may use this powerful suction-induced cavitation to cause physical damage on prey hiding in narrow refuges, therefore facilitating capture.

10. Dantrolene Significantly Improves Cerebral Blood Flow in a Rat Model of Hemorrhagic Vasospasms

Abstract

Introduction: A cerebral vasospasm (CVSP) is a potent vasoconstriction of the cerebral vasculature and the primary cause of morbidity and mortality following a subarachnoid hemorrhage. The middle cerebral artery (MCA) is commonly affected by CVSPs. Concomitant administration of dantrolene and nimodipine synergistically reduces vasospasms in aortic rings from Sprague Dawley rats. To determine if the effects observed in the systemic vasculature extend to the cerebral circulation, we investigated the effect of intravenous administration of dantrolene (2.5 mg/kg) and nimodipine (1 mg/kg and 2 mg/kg) on MCA blood flow velocity (BFV) 7 days after the induction of CVSPs. Methods: Vasospasms were induced by bathing the left common carotid artery with autologous whole blood. Age-matched sham rats were used as controls. BFV, mean arterial pressure (MAP), and heart rate (HR) were measured with a PeriFlux 5000 Laser Doppler System, and a CODA non-invasive blood pressure system, before and after administering the drugs. Morphometric evaluations were also performed to assess vascular alterations. Results: BFV was reduced by 37% with dantrolene alone (n = 6, p ≤ 0.05) and by 27% with 2 mg/kg nimodipine (n = 6, p < 0.05), while it was not affected by 1 mg/kg nimodipine. The combination of 1 mg/kg nimodipine with dantrolene, however, decreased BFV by 35% (from 435.70 ± 21.53 to 284.30 ± 23.13 perfusion units, n = 7, p ≤ 0.05). A similar reduction (31%) was obtained with dantrolene and 2 mg/kg nimodipine (from 536.00 ± 32.61 to 367.80 ± 40.93 perfusion units, n = 6, p ≤ 0.05). Neither MAP nor HR was affected by dantrolene or nimodipine alone. The combination of dantrolene with 2 mg/kg nimodipine, however, decreased MAP and increased HR. Furthermore, 7 days after the induction of vasospasms, lumen area of the left common carotid artery decreased, whereas media thickness and the wall-to-lumen ratio increased when compared to contralateral controls. The latter finding suggests that vascular remodeling was present at this stage. Conclusion: Altogether, our results indicate that 2.5 mg/kg dantrolene significantly reduces BFV in the MCA without altering systemic hemodynamic parameters to a similar extent than the highest dose of nimodipine or the combination of dantrolene and the lowest dose of nimodipine. Therefore, dantrolene may provide a promising alternative to lower the risk, or partially revert, CVSP.

Abstract

Enterolobium cyclocarpum (Ec) is a native legume that grows from southern Mexico to South America; its seeds are traditionally used for feeding ruminants. The objective of this study was to evaluate the addition of Ec pods in diets of hair lambs and their effect on the productive variables, ruminal fermentation and in vitro production of methane (CH4) and carbon dioxide. Thirty male lambs fed diets containing 0% (Ec0), 20% (Ec20) and 40% (Ec40) based on dry matter were evaluated. The addition of Ec40 in the integral diet of the lambs did not affect the growth performance. The pH of the ruminal fluid did not show differences between the treatments. The addition of Ec40 increased propionate and decreased acetate. Protozoa decreased in 47 and 59% with Ec20 and Ec40, respectively; while the population of cellulolytic bacteria decreased with Ec40. In vitro degradability of dry matter was lower in 8.8% with the inclusion of Ec40 in diets. The novelty of the study was that Ec40 diet decreased the ruminal methane production.

12. Natural barriers: waterfall transit by small flying animals

Abstract

Waterfalls are conspicuous geomorphological features with heterogeneous structure, complex dynamics and multiphase flows. Swifts, dippers and starlings are well-known to nest behind waterfalls, and have been reported to fly through them. For smaller fliers, by contrast, waterfalls seem to represent impenetrable barriers, but associated physical constraints and the kinematic responses of volant animals during transit are unknown. Here, we describe the flight behaviour of hummingbirds (the sister group to the swifts) and of various insect taxa as they fly through an artificial sheet waterfall. We additionally launched plastic balls at different speeds at the waterfall so as to assess the inertial dependence of sheet penetration. Hummingbirds were able to penetrate the waterfall with reductions in both their translational speed, and stroke amplitude. The body tilted more vertically and exhibited greater rotations in roll, pitch and yaw, along with increases in tail spread and pitch. The much smaller plastic balls and some flies moving at speeds greater than 2.3 m s−1 and 1.6 m s−1, respectively, also overcame effects of surface tension and water momentum and passed through the waterfall; objects with lower momentum, by contrast, entered the sheet but then fell along with the moving water. Waterfalls can thus represent impenetrable physical barriers for small and slow animal fliers, and may also serve to exclude both predators and parasites from nests of some avian taxa.

Abstract

This article presents Hecatonquiros, a complete open-source ecosystem for low cost and lightweight robotic manipulators. It has been released to focus on aerial manipulation applications but can be used in any other robotic application that requires the use of manipulators. The proposed framework provides the control system, a simulation environment, and a set of back ends to allow reusing the algorithms with different hardware setups. Additionally, it is released with a set of tools to ease its usage and various examples to teach the users. Several manipulators models and end-effectors are available for the users to adapt to their different requirements. All the hardware is designed to be three-dimensional printed and its components are low cost and available in common robotic stores, so anyone can reproduce and use them. The software is available in the GitHub repository https://github.com/Bardo91/hecatonquiros .

Abstract Background Most analyses of gaps in human resources for health (HRH) do not consider training and the transition of graduates into the labour market. This study aims to explore the labour market for Peru’s recent medical, nursing, and midwifery graduates as well as their transition into employment in the Ministry of Health’s (MOH) system. Methods Data from four different datasets, covering 2007–2013, was used to characterize the patterns of recently trained physicians, nurses, midwives, and postgraduate-trained physicians that enter employment in the MOH system, and scenario analyses were used to describe how this rate of entry needs to adapt in order to fill current HRH shortages. Results HRH graduates have been increasing from 2007 to 2011, but the proportions that enter employment in the MOH system 2 years later range from 8 to 45% and less than 10% of newly trained medical specialists. Scenario analyses indicate that the gap for physicians and nurses will be met in 2027 and 2024, respectively, while midwives in 2017. However, if the number of HRH graduates entering the MOH system doubles, these gaps could be filled as early as 2020 for physicians and 2019 for nurses. In this latter scenario, the MOH system would still only utilize 56% of newly qualified physicians, 74% of nurses, and 66% of midwives available in the labour market. Conclusion At 2013 training rates, Peru has the number of physicians, nurses, and midwives it needs to address HRH shortages and meet estimated HRH gaps in the national MOH system during the next decade. However, a significant number of newly qualified health professionals do not work for the MOH system within 2 years of graduation. These analyses highlight the importance of building adequate incentive structures to improve the entry and retention of HRH into the public sector.

Abstract Aim: This study aimed to compare the effects of linear periodization (LP) and undulating periodization (UP) of strength training on acceleration in skater children. Methods: Twenty-nine girls (9.67±1.29 years-old, 34.47±8.06kg, 1.39±0.13m) were distributed into two groups: linear periodization group (LPG, n=14) and undulating periodization group (UPG, n=15). Six levels of progressive training were designed based on activities such as multi-jumps, plyometrics, sled towing, and facilitated exercises. The training lasted 16 weeks, with a 1-h session on three non-consecutive days per week. The acceleration was recorded by digital videography following the major trochanter of the femur, at four moments and under two conditions: static (SS) and dynamic start (DS) in a rectilinear path. Abstract

In this work, the feasibility of triplet fusion upconversion (TFU, also named triplet–triplet annihilation upconversion) technology for the functionalization (arylation) of furans and thiophenes has been successfully proven. Activation of aryl halides by TFU leads to generation of aryl radical intermediates; trapping of the latter by the corresponding heteroarenes, which act as nucleophiles, affords the final coupling products. Advantages of this photoredox catalytic method include the use of very mild conditions (visible light, standard conditions), employment of commercially available reactants and low-loading metal-free photocatalysts, absence of any sacrificial agent (additive) in the medium and short irradiation times. The involvement of the high energetic delayed fluorescence in the reaction mechanism has been evidenced by quenching studies, whereas the two-photon nature of this photoredox arylation of furans and thiophenes has been manifested by the dependence on the energy source power. Finally, the scaling-up conditions have been gratifyingly afforded by a continuous-flow device. Graphical abstract

Resumen Introducción: La colecistectomía laparoscópica es el tratamiento gold standard en la colecistitis aquda litiásica, si bien su manejo cuando existe coledocolitiasis concomitante es controvertido. Objetivos: Evaluar los reingresos por complicaciones biliares en pacientes con colecistitis aguda tratados de forma conservadora por sospecha de coledocolitiasis concomitante. Material y métodos: Estudio retrospectivo bicéntrico. Recogida de datos: Junio 2015 – Octubre 2017. Diagnóstico de colecistitis aguda según criterios de Guía de Tokyo y alto riesgo de coledocolitiasis según American Asociation for Gastroenterology. Resultados: Se incluyeron 137 pacientes de los cuales la colecistectomía se pospuso en 86 (62,8%) por sospecha de coledocolitiasis concomitante. Dicha obstrucción de la vía biliar se confirmó en el 42,3% de los casos. Cuando la vía biliar no está dilatada en la ecografía el valor de bilirrubina como predictor de coledocolitiasis fue 4,5mg/dL. La tasa de reingresos en paciente no intervenidos fue del 23,3%. Conclusiones: el tratamiento conservador de la colecistitis aguda por sospecha de una coledocolitiasis concomitante hace que se demore la colecistectomía. Dicha demora supone un aumento de los reingresos hospitalarios por complicaciones biliares.

Abstract

Leishmania-activated C-kinase antigen (LACK) is a highly conserved protein among Leishmania species and is considered a viable vaccine candidate for human leishmaniasis. In animal models, prime-boost vaccination with LACK-expressing plasmids plus attenuated vaccinia viruses (modified vaccinia Ankara [MVA] and mutant M65) expressing LACK, has been shown to protect against cutaneous leishmaniasis (CL). Further, LACK demonstrated to induce the production of protective cytokines in patients with active CL or cured visceral leishmaniasis, as well as in asymptomatic individuals from endemic areas. However, whether LACK is capable to trigger cytokine release by peripheral blood mononuclear cells from patients cured of CL due to Leishmania infantum (L. infantum) or induce protection in L. infantum-infected hamsters [visceral leishmaniasis (VL) model], has not yet been analyzed. The present work examines the ex vivo immunogenicity of LACK in cured VL and CL patients, and asymptomatic subjects from an L. infantum area. It also evaluates the vaccine potential of LACK against L. infantum infection in hamsters, in a protocol of priming with plasmid pCI-neo-LACK (DNA-LACK) followed by a booster with the poxvirus vectors MVA-LACK or M65-LACK. LACK-stimulated PBMC from both asymptomatic and cured subjects responded by producing IFN-γ, TNF-α, and granzyme B (Th1-type response). Further, 78% of PBMC samples that responded to soluble Leishmania antigen showed IFN-γ secretion following stimulation with LACK. In hamsters, the protocol of DNA-LACK prime/MVA-LACK or M65-LACK virus boost vaccination significantly reduced the amount of Leishmania DNA in the liver and bone marrow, with no differences recorded between the use of MVA or M65 virus vector options. In summary, the Th1-type and cytotoxic responses elicited by LACK in PBMC from human subjects infected with L. infantum, and the parasite protective effect of prime/boost vaccination in hamsters with DNA-LACK/MVA-LACK and DNA-LACK/M65-LACK, revealed the significance of LACK in activating human and hamster immune responses and support LACK to be a valuable candidate for inclusion in a vaccine against human VL.

Abstract

OBJECTIVE: Launching priority actions demand a comprehensive appraisal of the size effect that risk factors have on the burden of overweight and obesity. This study aimed to estimate the incidence of overweight and obesity among children and adolescents, with special emphasis on the role of specific risk factors.MATERIALS AND METHODS: Secondary analysis of data from the younger cohort of the Young Lives Study in Peru (2002-2017). The outcomes were: overweight and obesity, defined by the World Health Organization standards; whereas the exposures included caesarean birth, physical activity levels, sodas and sugar-sweetened beverages consumption, snacks consumption, and maternal body mass index (BMI). We used multilevel Poisson regression models, considering the repetitive nature of data, to estimate incidence rate ratios (IRRs) and population attributable fractions (PAFs).RESULTS: A total of 2052 children, mean age 1 (SD 0.4) year, and 50% girls, were enrolled at baseline. After 14 (SD 0.5) years of follow-up, the incidence of overweight and obesity were 7.9 (95% CI 7.6-8.2) and 2.2 (95% CI 2.0-2.4) per 100 person-year, respectively. Maternal BMI (IRRs 3.51; PAF 31.8%), low physical activity (IRR 1.64; PAF 27.4%), caesarean birth (IRR 1.63; PAF 11.4%), almost daily snack consumption (IRR 1.60; PAF 32.1%), and almost daily consumption of sweetened beverages (IRR 1.47; PAF 26.0%) increased the risk of developing obesity.CONCLUSIONS: This study provides evidence on the risk of overweight and obesity attributable to diet habits, physical activity and the obesogenic niche among children and adolescents in Peru, which may guide the implementation of evidence-based interventions.

Abstract

Springtails (Collembola) have been traditionally portrayed as explosive jumpers with incipient directional takeoff and uncontrolled landing. However, for these collembolans who live near the water, such skills are crucial for evading a host of voracious aquatic and terrestrial predators. We discover that semiaquatic springtails Isotomurus retardatus can perform directional jumps, rapid aerial righting, and near-perfect landing on the water surface. They achieve these locomotive controls by adjusting their body attitude and impulse during takeoff, deforming their body in mid-air, and exploiting the hydrophilicity of their ventral tube, known as collophore. Experiments and mathematical modeling indicate that directional-impulse control during takeoff is driven by the collophores adhesion force, the body angle, and the stroke duration produced by their jumping organ, the furcula. In mid-air, springtails curve their bodies to form a U-shape pose, which leverages aerodynamic forces to right themselves in less than 20 ms, the fastest ever measured in animals. A stable equilibrium is facilitated by the water adhered to the collophore. Aerial righting was confirmed by placing springtails in a vertical wind tunnel and through physical models. Due to these aerial responses, springtails land on their ventral side 85% of the time while anchoring via the collophore on the water surface to avoid bouncing. We validated the springtail biophysical principles in a bioinspired jumping robot that reduces in-flight rotation and lands upright 75% of the time. Thus, contrary to common belief, these wingless hexapods can jump, skydive and land with outstanding control that can be fundamental for survival., 12 pages, 8 figures

Abstract

Small animal models have been a challenge for the study of SARS-CoV-2 transmission, with most investigators using golden hamsters or ferrets1, 2. Mice have the advantages of low cost, wide availability, less regulatory and husbandry challenges, and the existence of a versatile reagent and genetic toolbox. However, adult mice do not robustly transmit SARS-CoV-23. Here we establish a model based on neonatal mice that allows for transmission of clinical SARS-CoV-2 isolates. We characterize tropism, respiratory tract replication and transmission of ancestral WA-1 compared to variants Alpha (B.1.1.7), Beta (B.1.351), Gamma (P.1), Delta (B.1.617.2), Omicron BA.1 and Omicron BQ.1.1. We identify inter-variant differences in timing and magnitude of infectious particle shedding from index mice, both of which shape transmission to contact mice. Furthermore, we characterize two recombinant SARS-CoV-2 lacking either the ORF6 or ORF8 host antagonists. The removal of ORF8 shifts viral replication towards the lower respiratory tract, resulting in significantly delayed and reduced transmission in our model. Our results demonstrate the potential of our neonatal mouse model to characterize viral and host determinants of SARS-CoV-2 transmission, while revealing for the first time a role for an accessory protein in this context.

Abstract

Introduction and purpose Scientific production has become very important in the progression of the healthcare profession, and the studies made in this field reflect a gender gap in the authorship of such production. In the last decade, a review of the publications made over 35 years in 6 high impact factor journals has evidenced a lesser proportion of women among the first and last signing authors (1,2). Nevertheless, the presence of women as preferential authors is desproportionately low considering the number of female cardiologists in Spain in the year 2017 (40%) (3). The Revista Española de Cardiología (REC) is an international journal dedicated to cardiovascular diseases. Our study investigates the authorship gender differences from publications in the REC. Methods A cross-sectional study was made of the REC issues published between January 2011 and December 2020. For each article we recorded the doi identifier, the year of publication, the type of article, the number of authors, the number of female authors, and the number of women with preferential authorship. Preferential authorship was defined as first signing author, corresponding author or last author. A gender-based analysis of the number of authors, the type and year of publication and preferential authorship was carried out. Female participation was calculated from the female/male ratio, with an analysis of the trend observed over the years. Results Of 2859 articles documented, 4275 signing authors were women and 12,061 were men. 699, 548 and 481 women were the first, corresponding and the last author, respectively. Table 1 shows the percentage of articles in which a woman was a preferential author, as well as the percentage of women that would have to be present in the general population to cancel statistical significance. The number of articles in which a woman was the first or the corresponding author was seen to increase (p=0.008 and p=0.002, respectively), while no significant changes were observed in relation to last authorship (p=0.09). The percentage of articles in which at least one woman held at least one of the three preferential authorship positions tended to increase over time (p=0.056). In the year 2011, a total of 90 articles out of 252 (35.7%) had at least one woman among the three preferential authors, while in the year 2020 the figure was 123 out of 296 (41.6%) (Figure 1). Conclusions Although the participation of female authors in the scientific production of the REC has increased during the last decade, authorship gender inequalities persist. Abstract

Background Transcatheter Aortic valve Replacement (TAVR) has emerged as an alternative to surgical aortic valve replacement. It is widely known the short and mid-term outcomes, however, is limited about long-term outcomes in according to age. The 2021 ESC/EACTS guidelines for the management of valvular heart disease recommended TAVR in older patients ≥75 years or in those who are high risk. The aim of this study was to determine the survival and the factors predicting mortality after TAVR in according to age. Methods From April 2008 to December 2020, a total of 991 patients with symptomatic severe aortic stenosis underwent TAVR with balloon-expandable or self-expanding valves were included. Abstract

The use of transcatheter aortic valve replacement (TAVR) has expanded as an alternative to surgical aortic valve replacement and has become a widely accepted treatment option for patients with severe aortic stenosis who are considered intermediate- and high-risk surgical candidates. The information concerning the long-term durability of TAV devices is currently limited. The interplay between estimated life expectancy and prosthetic heart valve durability is a key consideration in these discussions. The aim of this study was to determine the clinical outcomes, survival and the factors predicting mortality after TAVR Methods From April 2008 to December 2020, the auto-expandible and baloom-expandable prostheses were implanted in 991 patients with symptomatic severe aortic stenosis with intermediate, high and deemed risk. Results The mean age was 79.43±6.7 years. The logistic EuroSCORE II and STS score were 5.92±5% and 5.22±3.7%, respectively. The implantation success rate was 98.7%. In-hospital mortality was 3.3%, and the combined endpoint of death, vascular complications, myocardial infarction or stroke had a rate of 17.2%. The late mortality (beyond 30 days) was 43.2%. Survival at 1, 3, 5, 7, 9 and ten years were 88.2%, 68.2%, 50,1%, 38.1%, 26.3% and 16.4% respectively, after a mean follow-up of 44.8±31 months. The NYHA functional class improved from 3.3±0.7 to 1.76±0.5 in the follow-up. At 7 years, 9 patients had severe prosthetic valve dysfunction (severe stenosis and moderate transvalvular regurgitationI. In 7 patients were treated again with TAVR. The predictors of cumulative mortality were: Charlson index [HR 1.167 (95% CI 1.078–1.264), p=0.001], Readmision Heart Failure [HR 2.793 (95% CI 1.948–4.005), p=0.001], stroke post-TAVR [HR 2.919 (95% CI 1.965–4.336), p=0.001], Left ventricular ejection fraction [HR −1.012 (95% CI 1.002–1.022) p=0.021] and Karnosfky index [HR −0.984 (95% CI 0.975–0.993), p=0,001] but the requirement of pacemaker or residual aortic regurgitation after TAVR were not predictors [HR 0,979 (CI 95% 0.678–1.415), p 0.911] and [HR 1.087 (CI 95% 0.913–1.294), p=0.349]. Conclusions TAVR is associated with excellent clinical results at follow-up. Abstract

Introduction Type 2 diabetes mellitus (DM) is an important factor that is associated with increased cardiovascular morbidity, producing disorders in different devices and systems whose mechanisms are not entirely known. This study was conducted in order to understand the evolution of aerobic capacity and the different cardiovascular risk factors (CVRF) in diabetic patients compared to non-diabetic patients. Methodology Prospective observational study. A total of 301 patients were included consecutively in a conventional cardiac rehabilitation program from January 2017 to March 2020, after one month of having had acute coronary syndrome, phase II of which lasted 2 months, who had an analysis, ergometry with spire gas analysis, and anthropometric characteristics were included. Those who had signs of ischemia in ergometry were excluded from the study. The tests were conducted as recommended by the ATS/ACCP. Results 301 patients were included, with an average age of 57.6±9.1 years, diabetics 92 (30,6%), LVEF average 56.9±9.6%, women 13.6%. Among the basal characteristics, diabetic patients had the highest prevalence of dyslipemia (89.1% vs 68.4% p s Conclusions In our study, patients with DM undergoing an RHC program achieve better control of FRCV and aerobic capacity in a similar way to non-diabetic patients, which is maintained at 7 months of follow-up in phase III, although initially the aerobic capacity gain in diabetics is slower, achieve a higher increase in VO2 in Phase III. Abstract

Introduction To determine the aerobic capacity and the cardiopulmonary response to exercise is an important tool to assess functional capacity and a pronostic factor in patients with coronary artery disease (CAD). Maximal oxygen uptake (VO2) is considered the gold standard for measuring aerobic capacity, and a maximal graded exercise test is required for its direct measurement. Nevertheless, it requires long duration sessions, qualified medical personal and expensive equipment in order to perform it. These are the main reasons why its use is not widely spread in daily clinical practice and more accessible tests are needed. The aim of this study is to determine if we can predict the VO2 in patients with CAD, depending on basal characteristics of the patient. Objective To develop an equation to predict VO2 based on the 6-minute walk test (6MWT), basal characteristics and the presence of cardiovascular risk factors in patients with CAD and preserved left ventricular ejection fraction (LVEF). Methods We performed a cross-sectional prospective study with 202 patients with CAD and preserved LVEF, who were consecutively included in a conventional cardiac rehabilitation programme from March of 2019 to March of 2020 and underwent a 6-minute walk distance test and an ergoespirometry. The presence of cardiovascular risk factors, comorbidities and anthropometric variables were also evaluated. The tests were performed following ATS recommendations. The predictive equation for VO2 was obtained with a multivariate regression analysis. To assess the fit of the predictive model, we used conventional linear regression models according to the coefficient of determination (R2). A P value Results A total of 202 patients were analysed. The mean age was 57,5±9,3 years, mean LVEF was 61,2±6,14%, 13,4% were female, 54,4% arterial hypertension, 30,2% with diabetes, 76,7% dyslipidemia, 36,1% obese, 6,4% with peripheric artery disease, 9,4% with pulmonary disease. Abstract

Background Poor self-care is associated with higher risk in heart failure (HF). Self-care may influence patient-reported outcomes (PROMs) such as health-realted quality of life (QoL). Functional and instrumental autonomy are determinants of QoL. Literature has shown instrumental and/or functional dependency are associated with worse QoL. Purpose Describe the effect of self-care on QoL across to different levels of dependency and explore the potencial interaction between self-care and autonomy. Methods 1120 patients with chronic HF from DAMOCLES study were analyzed. We used modified European Heart Failure Self-care Behavior Scale 9-item version (EHFSCBS-9) to measure self-care, scores were inverted. To assess QoL, we used Minnesota Living with Heart Failure Questionnaire (MLHFQ). Functional dependency for basic activities of daily living (BADL) was evaluated using Barthel index (BI). Dependency for instrumental activities (IA) was evaluated using Lawton and Brody questionnaire (LBQ). Regarding dependency, patients were grouped in 3 phenotypes: independent (normal BI and LBQ, phenotype 1), dependency for IA (abnormal LBQ, phenotype 2) and dependency for BADL and IA (abnormal BI ± abnormal LBT, phenotype 3). General linear models (GLM) were constructed to explore interaction between dependency phenotypes and poor self-care (defined as EHFSCBS-9 ≤lower tertile) on adjusted MLHFQ scores and to obtain the estimated marginal means of QoL scores across phenotypes and levels of self-care. All models were adjusted by determinants of HF severity. Results Mean age was 72±11 years, 43% were women and mean LVEF was 45±17%. Mean MLHFQ scores were 45±24. Mean EHFSCBS-9 scores were 69±28. There was a significant inverse correlation between EHFScBS-9 and MLHFQ scores (r=−0.158; p-value Conclusions Self-care behaviour is an independent predictor of health-related QoL in HF regardless of the level of patient's autonomy. Better self-care was associated with better QoL particularly in patients with dependendecy to IA and/or BADL. Impact of self-care on QoL was attenuated in patients with normal autonomy. Abstract

In this study we address the need to promote student engagement with school science and the need to measure a more comprehensive conception of engagement considering three dimensions of engagement: emotional, behavioural, and cognitive. We address the first issue by designing and implementing a model-based inquiry instructional sequence focused on fostering students’ engagement with science. The second issue is addressed by developing and testing a heuristic that measures student engagement holistically by focusing on all three dimensions mentioned. The results report that our short MBI instructional sequence develop students’ emotional engagement because most of them recognize to having felt interest, concentration, and satisfaction and few students reported boredom, shame, or rejection during the teaching. They spent most of the time working in small groups and sharing-discussing their hypothesis, planning and evaluating their experimental designs, and building models to explain acid-base phenomena together which highlight behavioural and cognitive dimension of students’ engagement in our instructional sequence about chewing gum and pH of the mouth. The heuristic is designed to gather data continuously during instruction with minimal disruption to the learning and teaching sequence. The heuristic can be used by teachers, researchers, and also by students, to gain multi-level understandings about engagement by focusing on individual, group-level, or class-level analysis of responses.

Resumen Antecedentes y objetivo En Espana no existen estudios que hayan evaluado la prevalencia de la miocardiopatia hipertrofica en la poblacion general. El objetivo de este trabajo fue evaluar la prevalencia de la miocardiopatia hipertrofica en una muestra amplia de la poblacion laboral espanola. Materiales y metodos Se incluyo a 13.179 trabajadores (73% varones, con una edad media de 40 anos) de 5 regiones espanolas a los que, entre mayo de 2008 y noviembre de 2010, se les realizo un reconocimiento medico con un electrocardiograma. Se derivo a los trabajadores con alteraciones sugestivas en el electrocardiograma o con antecedentes medicos predisponentes (sincope de esfuerzo o muerte subita en familiar menor de 50 anos) para una evaluacion ecocardiografica. Se definio miocardiopatia hipertrofica a la presencia de un grosor parietal igual o mayor a 13 mm en cualquier segmento del ventriculo izquierdo. Se estimo la prevalencia de la miocardiopatia hipertrofica en toda la muestra y en los trabajadores no hipertensos. Resultados Se selecciono a 1.008 trabajadores para el ecocardiograma, aunque solo 496 (49,2% de los seleccionados) acudieron a la prueba. Tras el ecocardiograma se detectaron 16 casos de miocardiopatia hipertrofica y se estimo una prevalencia del 0,24% en el total de la muestra. En el subgrupo de trabajadores no hipertensos se objetivaron 10 casos de miocardiopatia hipertrofica, que se corresponden con una prevalencia estimada del 0,19%. Conclusiones En nuestra muestra de la poblacion laboral espanola la prevalencia estimada de miocardiopatia hipertrofica fue del 0,24%. En el subgrupo de pacientes no hipertensos la prevalencia estimada fue del 0,19%.

Abstract

It is well known that amyloid beta (Aβ) peptides are generated in blood vessels, released into the brain during thrombosis, and temporarily accumulate in this organ after injury. Here we demonstrate that 24 h after transient middle cerebral artery occlusion (tMCAO), one of the standard models of focal ischemic stroke, Aβ peptide accumulates in the brain, concentrating on the blood vessel walls. Because Aβ oligomers are known to induce significant damage to brain cells, they act as an additional damaging factor during ischemic stroke. Considering that they have been shown to form ion channels in cells, affecting osmotic balance, we used an Aβ peptide channel blocker, tromethamine (2-amino-2-(hydroxymethyl) propane-1,3-diol), to prevent this additional injury. Tromethamine injected 0.1 g/100 g body weight intraperitoneally at 5 min before tMCAO decreased water content in the damaged hemisphere, as measured by dry brain weight. Congo red staining, which binds only to Aβ oligomer plaques (amyloid), showed that there was no significant presence of plaques. Therefore, we suggest that Aβ peptide oligomers are responsible for some of the brain damage during stroke and that blockage of the ion channels that they form could be beneficial in treating this complex neurological syndrome.

Abstract

Small bowel diverticula are rare and often asymptomatic. Severe lower gastrointestinal bleeding from jejunal diverticula is rarely reported and, therefore, should be considered a differential diagnosis in all cases of lower gastrointestinal bleeding with nonconclusive gastroscopy and colonoscopy. In this case report, we discuss a case of a 75-year-old male with massive lower gastrointestinal bleeding from jejunal diverticula. Initial gastroscopy did not reveal the source of bleeding. Repeat upper endoscopy with a pediatric colonoscope identified jejunal diverticula as the likely source of bleeding. Angiography identified the site of extravasation, and successful angioembolization was done by interventional radiology.
