2. Examining the Effects of Behavioral Skills Training on Social Praise Delivery in Malaysian Classrooms

Abstract

Malaysia launched the Inclusive Education Program (IEP) in 1997 to integrate students with special needs into mainstream classrooms and local researchers have recently asserted that more training is necessary for Malaysian teachers to effectively work with IEP students. The purposes of this research, therefore, were to (1) introduce the use of social praise via behavioral skills training (BST) to teachers of students in IEP, (2) increase the use of social praise by teachers of students in IEP via written feedback provided by the experimenter following each post-BST classroom session; and (3) increase the likelihood of individually selected target behavior in students who attend IEP. A multiple-baseline across four teacher-student dyads was conducted. Results indicated that BST and written feedback were effective in increasing the frequency (converted to responses per minute) of social praise issued by all four teachers and that collateral effects were seen by an increase in target behaviors for two out of four student participants. A posttreatment social validity survey completed by teacher participants indicated that the BST goals, procedures, and outcomes were practical and important.

Abstract

The surface-enhanced Raman spectroscopy (SERS) method exploits the plasmonic effect of nano-sized metallic materials to intensify the Raman scattering of the monochromatic light of analyte molecules. This promotes the sensitivity and specificity of the Raman spectroscopy analysis method. This study integrated SERS with machine learning (ML) to detect dengue fever, a disease infecting more than 40% of the world's population. Non-structural protein 1 (NS1), detected in the sera of infected dengue patients during the early infection stage, is currently recognised as a biomarker for the early diagnosis of DF. However, no attempts have been made to detect NS1 in the salivary Raman spectra. Given this situation, this study delves into the potential of SERS as an early, non-invasive DF detection technique using salivary NS1. The SERS spectra of saliva samples (n=289) were collected and subsequently classified as positive and negative for DF, using principal component analysis (PCA) integrated with support vector machine (SVM) models. The PCA-SVM model's performance was benchmarked against two clinical diagnostic NS1-enzyme-linked immunosorbent assay (ELISA) tests recommended by the World Health Organization (WHO). The PCA-SVM model outperformed both tests regarding radial basis function kernel (RBF) and cumulative percent variance (CPV; 83.22% accuracy, 88.27% sensitivity, 78.13% specificity). It is encouraging that the sensitivity level of the PCA-SVM model is above the benchmark set by the saliva-based NS1-ELISA tests proposed by the WHO, demonstrating the potential of SERS for the non-invasive detection of DF. 18. Survivability of patients admitted for stroke in a primary stroke center, Penang, Malaysia: a retrospective 5-year study

Abstract

Background Stroke is one of the most common noncommunicable diseases, with significant public health implications both globally and in Malaysia. The aim of this study was to evaluate post-stroke survivability as well as the major drug classes prescribed for hospitalized stroke patients. Methods A 5-year retrospective study was carried out on the survival of stroke patients admitted to Hospital Seberang Jaya, a main stroke center in the state of Penang, Malaysia. Patients admitted for stroke were first identified using the local stroke registry database, and their medical records were then accessed for data collection, which included demographic information, comorbid conditions, and medications prescribed during admission. Results The Kaplan-Meier overall survivability analysis performed indicated 50.5% survival for the duration of 10 days (p Conclusion The findings of the study are intended to encourage more non-stroke hospitals in Malaysia to increase their efforts in treating stroke patients, as early treatment can help reduce the severity of the stroke. With the incorporation of evidence-based data, this study also contributes to local data for comparison and improves the implementation of regularly prescribed stroke medication.

Abstract

Background and Objective:Distinguishing attributes of stroke subtypes is crucial to establish appropriate planning for patient care and preventive measures. This study aims to compare the associations among demographic characteristics, risk factors, clinical manifestations, and outcomes of different stroke subtypes in a Malaysian stroke-ready hospital. Methods:The study utilized data that were collected from the local hospital-based stroke database, which is part of the Perai Regional Integrated Stroke Intervention System. The database is representative of the population in mainland Penang. All confirmed local ischaemic stroke (IS) and haemorrhagic stroke (HS) cases aged 18 years and above admitted to Hospital Seberang Jaya from 1st January 2010 to 31st December 2019 were included. Descriptive and inferential statistics were employed. Results:There was a total of 1,805 patients with 1,572 (87.1%) IS patients and 233 (12.9%) HS patients. The mean (SD) age for IS patients was 62.75 (12.08) and 60.51 (13.65) for HS patients. Generally, there were more male than female patients: 957 (60.9%) male IS patients and 137 (58.8%) male HS patients. A significantly higher proportion of IS patients were aged ≥60 years old (59.9%, p=0.021), of Indian origin (15.5%, p=0.034), had diabetes (51.2%, p Conclusion:Significant differences were observed on risk factors between IS and HS. IS was linked mainly with hemiparesis and speech disturbances, whereas HS patients mainly exhibited headaches, nausea and vomiting, altered sensorium, and seizures, in addition to more severe stroke and poor outcomes.

Abstract

Background and aims: Numerous studies examined the association between factors FV, FVII, FXII, and FXIII-A gene polymorphisms and ischemic stroke, but conclusive evidence is yet to be obtained. Thus, this meta-analysis aimed to investigate the novel association of FV rs1800595, FVII rs5742910, FXII rs1801020, and FXIII-A rs5982 and rs3024477 polymorphisms with ischemic stroke risk. Methods: A systematic review was performed on articles retrieved before June 2018. Relevant data were extracted from eligible studies and meta-analyzed using RevMan version 5.3. The strength of association between studied polymorphisms and ischemic stroke risk was calculated as odds ratios and 95% confidence intervals, by applying both fixed- and random-effect models. Results: A total of 25 studies involving 6100 ischemic stroke patients and 9249 healthy controls were incorporated in the final meta-analysis model. Specifically, rs1800595, rs5742910, rs1801020, rs5982, and rs3024477 consisted of 673, 3668, 922, 433, and 404 cases, as well as 995, 4331, 1285, 1321, and 1317 controls, respectively. The pooled analysis indicated that there was no significant association of FV rs1800595, FVII rs5742910, FXII rs1801020, FXIII-A rs5982, and FXIII-A rs3024477 polymorphisms with ischemic stroke risk, under any genetic models (dominant, recessive, over-dominant, and allelic). Conclusions: The present meta-analysis concluded that FV rs1800595, FVII rs5742910, FXII rs1801020, and FXIII-A rs5982 and rs3024477 polymorphisms are not associated with ischemic stroke risk.

Abstract

Aim: nlmixr offers first‐order conditional estimation (FOCE), FOCE with interaction (FOCEi) and stochastic approximation estimation‐maximisation (SAEM) to fit nonlinear mixed‐effect models (NLMEM). We modelled metformin's pharmacokinetic data using nlmixr and investigated SAEM and FOCEi's performance with respect to bias and precision of parameter estimates, and robustness to initial estimates. Method: Compartmental models were fitted. The final model was determined based on the objective function value and inspection of goodness‐of‐fit plots. The bias and precision of parameter estimates were compared between SAEM and FOCEi using stochastic simulations and estimations. For robustness, parameters were re‐estimated as the initial estimates were perturbed 100 times and resultant changes evaluated. Results: The absorption kinetics of metformin depend significantly on food status. Under the fasted state, the first‐order absorption into the central compartment was preceded by zero‐order infusion into the depot compartment, whereas for the fed state, the absorption into the depot was instantaneous followed by first‐order absorption from depot into the central compartment. The means of relative mean estimation error (rMEE) (MEESAEMMEEFOCEi) and rRMSE (RMSESAEMRMSEFOCEi) were 0.48 and 0.35, respectively. All parameter estimates given by SAEM appeared to be narrowly distributed and were close to the true value used for simulation. In contrast, the distribution of estimates from FOCEi were skewed and more biased. When initial estimates were perturbed, FOCEi estimates were more biased and imprecise. Discussion: nlmixr is reliable for NLMEM. SAEM was superior to FOCEi in terms of bias and precision, and more robust against initial estimate perturbations. [ABSTRACT FROM AUTHOR]

Abstract

Introduction:Understanding infectious disease transmissibility in the context of “place” is crucial for implementing disease control and harm reduction strategies.This study was aimed to determine the dynamics of spatial dispersion of COVID-19 in Malaysia and its associated indicators. Methodology: This ecological study included all active COVID-19 cases between 22nd January 2021 and 4th February 2021 using secondary data from multiple sources. Incidence, Bayesian incidence, global and local Moran indices were calculated. A geographical weighted regression (GWR) analysis in addition to ordinary least squares regression (OLS) was executed to assess the relationships between incidence of COVID-19 with population level socio-economic and health coverage indicators. All analyses were conducted using SPSS and Geo Da software. Results: A total of 51,476 cases were included in the analysis. The global Moran's index of incidence rate was 0.499 (P=0.01), indicative of positive spatial autocorrelation with high standards between districts and regions. The Local Indicators of Spatial Association (LISA) maps identified spatial clusters, particularly the main high-high patterns in Central and Southern regions, and low-low concentrated districts in Northern and Borneo regions of Malaysia. The GWR yielded average income per capita and population density as two crucial population level indicators of COVID-19 dispersion in Malaysia (PDiscussion:Incidence cases were highly clustered within the Central Region of Malaysia, configuring that propagation occurs rapidly in highly populous places.1 The significantly positive indicator of population density yielded via GWR confirmed this association, consistent with a study from Brazil.2 ➢ Average income per capita was significantly associated with incidence cases. Within metropolitans with rapid economic activities, it could be anticipated higher income per capita to facilitate population access in carrying out greater number of diagnostic tests, thus reporting cases escalation. Whereas districts with socioeconomic vulnerabilities reverberates prevention and control measures. Abstract

Introduction Conventional methods through population-based surveys in measuring public perceptions and reactions during emergencies or pandemic times are costly, labor intensive and time consuming. To understand phenomenological experiences "now," it is crucial to complement measurement tools with real-time "infoveillance" approaches. We aimed to explore public perceptions and reactions towards the COVID-19 pandemic in Malaysia via content and thematic analyses using Twitter. Methodology Using time-series computational methods, we archived 18,467 tweets from 5932 Twitter users across Malaysia between May 10, 2020 till May 25, 2020 using R software (TwitteR package). Original tweets (excluding retweets) in English and Malay were retrieved using keywords "COVID-19," "coronavirus," "SARS-CoV-2," "2019nCoV," and/or "nCoV2019." An exploratory qualitative thematic and content analysis was performed on all eligible Tweets. Theme-wise favorite count and favorite ratios were yielded. Results Three major themes and nine sub-themes emerged from the thematic-content analyses. The major theme "personal experiences (favorite ratio=17.18)" identified direct and indirect people's experiences to being infected or observing close relatives to be infected with COVID-19, while the second major theme "risk perceptions (favorite ratio=14.97)" highlighted sub-domains of "seriousness" of infectivity and "downplayed risks" of COVID-19 infections. The third major theme "expressions and reactions (favorite ratio=12.82)" conveyed five sub-domain themes of people's emotions towards the pandemic; "relief," "frustrations," "hope," "appreciation," and "appeal." Conclusion This study highlights the need for real-time effective communication and accurate information to be disseminated to tackle psychological repercussions in unexpected times. Abstract

© 2021 American Academy of Neurology, Objective: To measure the global impact of COVID-19 pandemic on volumes of IV thrombolysis (IVT), IVT transfers, and stroke hospitalizations over 4 months at the height of the pandemic (March 1 to June 30, 2020) compared with 2 control 4-month periods. Methods: We conducted a cross-sectional, observational, retrospective study across 6 continents, 70 countries, and 457 stroke centers. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases. Results: There were 91,373 stroke admissions in the 4 months immediately before compared to 80,894 admissions during the pandemic months, representing an 11.5% (95% confidence interval [CI] -11.7 to -11.3, p < 0.0001) decline. There were 13,334 IVT therapies in the 4 months preceding compared to 11,570 procedures during the pandemic, representing a 13.2% (95% CI -13.8 to -12.7, p < 0.0001) drop. Interfacility IVT transfers decreased from 1,337 to 1,178, or an 11.9% decrease (95% CI -13.7 to -10.3, p = 0.001). Recovery of stroke hospitalization volume (9.5%, 95% CI 9.2-9.8, p < 0.0001) was noted over the 2 later (May, June) vs the 2 earlier (March, April) pandemic months. There was a 1.48% stroke rate across 119,967 COVID-19 hospitalizations. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was noted in 3.3% (1,722/52,026) of all stroke admissions. Conclusions: The COVID-19 pandemic was associated with a global decline in the volume of stroke hospitalizations, IVT, and interfacility IVT transfers. Primary stroke centers and centers with higher COVID-19 inpatient volumes experienced steeper declines. Recovery of stroke hospitalization was noted in the later pandemic months.
