Abstract Background Though social determinants are the primary drivers of health, few studies of people living with HIV focus on non-clinical correlates of insecure and/or fragmented connections with the care system. Our team uses linked clinical and multisector non‐clinical data to study how residential mobility and connection to social services influence the HIV care continuum. We engage a diverse group of individuals living with HIV and other invested community members to guide and inform this research. Our objective is to generate consultant-informed, research-based interventions that are relevant to the community, and to share our engagement approach and findings so that other researchers can do the same. Methods Our research team partnered with the Indiana Clinical and Translational Sciences Institute’s Research Jam to develop and implement a human-centered design research plan to engage individuals with experience relevant to our research. We recruited a panel of consultants composed of people living with HIV and/or clinicians and individuals from agencies that provide medical and non-medical services to people living with HIV in Marion County, Indiana. To date, we have used a variety of human-centered design tools and activities to engage individuals during six sessions, with results informing our future engagement and research activities. Results Since the inception of the project, 48 consultants have joined the panel. Thirty-five continue to be actively engaged and have participated in one or more of the six sessions conducted to date. Consultants have helped guide and prioritize analyses, aided in identification of data missing from our ecosystem, helped interpret results, provided feedback on future interventions, and co-presented with us at a local health equity conference. Conclusions We utilize community engagement to expand the scope of our research and find that the process provides value to both consultants and the research team. Human-centered design enhances this partnership by keeping it person-centered, developing empathy and trust between consultants and researchers, increasing consultant retention, and empowering consultants to collaborate meaningfully with the research team. The use of these methods is essential to conduct relevant, impactful, and sustainable research. We anticipate that these methods will be important for academic and public health researchers wishing to engage with and integrate the ideas of community consultants.

In the Fall of 2021 we co-taught a graduate seminar that launched a year long Mellon Sawyer series. In this essay, we reflect on the contexts, paradoxes and processes that informed our multidisciplinary collaboration teaching a Sawyer Seminar on Race, Migration and White Supremacy in California. We believed that it was vital to being with the migration experiences of Native Americans from rural areas to urban California. We sought to position American Indian migration histories as foundational to cultural and historical understandings of migration to California. Our account of our pedagogical practices details the rewards and realities of collaborative teaching at a public research university. We identify the paradoxes and tensions that we encountered as we developed a syllabus that did not simply "add and stir" different methodologies, histories or fields, but instead, synthesized theoretical and pedagogical across film, art and media studies, history and sociology.

Objective: How we communicate about obesity is critical as treatment paradigms shift upstream. We previously identified parental perceptions, concerns, beliefs, and communication preferences about early life obesity risk. We engaged parents of children 0 to 24 months of age and pediatricians from Indianapolis, Indiana, USA in the co-design of messages and tools that can be used to facilitate parent/provider conversations about early life obesity prevention. Methods: From April to June 2021, we conducted a series of co-design workshops with parents of children ages 0 to 24 months and pediatricians to identify their preferences for communicating obesity prevention in the setting of a pediatric well visit. Human-centered design techniques, including affinity diagraming and model building, were used to inform key elements of a communication model and communication strategy messages. These elements were combined and refined to create prototype tools that were subsequently refined using stakeholder feedback. Results: Parent participants included 11 mothers and 2 fathers: 8 white, 4 black, and 1 Asian; median age 33 years with 38% reporting annual household incomes less than $50,000. Pediatricians included 7 female and 6 male providers; 69% white. Through an iterative process of co-design, we created an exam room poster that addresses common misconceptions about infant feeding, sleep and exercise, and a behavior change plan to foster parent/provider collaboration focused on achieving children’s healthy weight. Conclusions: Our hands-on, collaborative approach may ultimately improve uptake, acceptability and usability of early life obesity interventions by ensuring that parents remain at the center of prevention efforts.

OBJECTIVES:. To analyze the temporal trend in enrollment rates in a COVID-19 platform trial during the first three waves of the pandemic in the United States. DESIGN:. Secondary analysis of data from the I-SPY COVID randomized controlled trial (RCT). SETTING:. Thirty-one hospitals throughout the United States. PATIENTS:. Patients who were approached, either directly or via a legally authorized representative, for consent and enrollment into the I-SPY COVID RCT. INTERVENTIONS:. None. MEASUREMENTS AND MAIN RESULTS:. Among 1,338 patients approached for the I-SPY COVID trial from July 30, 2020, to February 17, 2022, the number of patients who enrolled (n = 1,063) versus declined participation (n = 275) was used to calculate monthly enrollment rates. Overall, demographic and baseline clinical characteristics were similar between those who enrolled versus declined. Enrollment rates fluctuated over the course of the COVID-19 pandemic, but there were no significant trends over time (Mann-Kendall test, p = 0.21). Enrollment rates were also comparable between vaccinated and unvaccinated patients. In multivariable logistic regression analysis, age, sex, region of residence, COVID-19 severity of illness, and vaccination status were not significantly associated with the decision to decline consent. CONCLUSIONS:. In this secondary analysis of the I-SPY COVID clinical trial, there was no significant association between the enrollment rate and time period or vaccination status among all eligible patients approached for clinical trial participation. Additional studies are needed to better understand whether the COVID-19 pandemic has altered clinical trial participation and to develop strategies for encouraging participation in future COVID-19 and critical care clinical trials.

Summary: Background: An urgent need exists to rapidly screen potential therapeutics for severe COVID-19 or other emerging pathogens associated with high morbidity and mortality. Methods: Using an adaptive platform design created to rapidly evaluate investigational agents, hospitalised patients with severe COVID-19 requiring ≥6 L/min oxygen were randomised to either a backbone regimen of dexamethasone and remdesivir alone (controls) or backbone plus one open-label investigational agent. Introduction The COVID-19 pandemic brought an urgent need to discover novel effective therapeutics for patients hospitalised with severe COVID-19. The Investigation of Serial studies to Predict Your Therapeutic Response with Imaging And moLecular Analysis (ISPY COVID-19 trial) was designed and implemented in early 2020 to evaluate investigational agents rapidly and simultaneously on a phase 2 adaptive platform. This manuscript outlines the design, rationale, implementation and challenges of the ISPY COVID-19 trial during the first phase of trial activity from April 2020 until December 2021.Methods and analysis The ISPY COVID-19 Trial is a multicentre open-label phase 2 platform trial in the USA designed to evaluate therapeutics that may have a large effect on improving outcomes from severe COVID-19. The ISPY COVID-19 Trial network includes academic and community hospitals with significant geographical diversity across the country. Enrolled patients are randomised to receive one of up to four investigational agents or a control and are evaluated for a family of two primary outcomes—time to recovery and mortality. The statistical design uses a Bayesian model with ‘stopping’ and ‘graduation’ criteria designed to efficiently discard ineffective therapies and graduate promising agents for definitive efficacy trials. Each investigational agent arm enrols to a maximum of 125 patients per arm and is compared with concurrent controls. As of December 2021, 11 investigational agent arms had been activated, and 8 arms were complete. Enrolment and adaptation of the trial design are ongoing.Ethics and dissemination ISPY COVID-19 operates under a central institutional review board via Wake Forest School of Medicine IRB00066805. This article explores the labor of contemporary digital privacy advocates and their myriad efforts to protect and preserve public interests during the era of Big Tech companies. It is based on qualitative interviews with professional staff, lawyers, and policy analysts at multiple major advocacy organizations in Washington, DC. We have employed a grounded theory approach to address four labor-related themes that consistently emerged across our interviews: coalition building, agenda formulation, the art of navigating public- and private-sector relationships, and balancing a domestic and global policy landscape. In the current policy landscape, there is an intensifying degree of advocacy–industry coordination taking place, in part because of US regulatory roll-backs under the Trump administration and a gridlocked Congress. As a result, advocacy organization staff members often rely on companies for information to do their assessment and agenda-setting work. They also apply pressure to these companies and force them to think about how their technologies and operations impact users and publics around the world; they mount legal challenges to various media and tech initiatives to ensure public interests are protected; and some end up working with or for these companies in ways that may impart and integrate the values of advocacy organizations within profit-driven organizations. This article explores the multiple dimensions of advocacy labor which itself is often excluded from media policy and industry analysis.

This essay introduces a special stream of *Media+Environment* focused on “disaster media.” In the process, the authors conceptualize this term in relation to “natural” and other disasters, including the COVID-19 pandemic, and explore how understandings of “disaster media” are embedded within several areas of humanities-based film and media scholarship. Writing from the social ecological premise that consequences of disasters stem in large part from systemic actions, the introduction develops three general arguments about disaster media as an analytic. First, disasters cause people to rethink what “media” are and to contend with the fact that, especially during disasters, media are constantly changing and being updated; they also escape the screen and sculpt the environment (media are not only representational but also affective and infrastructural). Second, because they come to the fore in relation to crisis situations, disaster media help expose structural inequalities; practices of relief and reform need to happen and can be facilitated (or inhibited) by mediatic means. Finally, disaster media need to be considered in relation to the multiple temporalities of climate disruption (from the *longue durée* of glacial flow to uncertain and sudden extreme weather). Discussing these issues, the authors also introduce pieces in the stream that are focused on humanitarian drone interventions and glacier-melt artworks.

Abstract Online social networks are major hubs of communications that are used to share information by societies worldwide. However, the ability to freely communicate on these platforms is increasingly restricted in countries across the globe and existing technological solutions do not fully address the needs of affected communities. In this paper we explore the design process of SecurePost, a novel tool that allows verified group anonymity to those communicating publicly on online social networks. We present survey-based research and ethnographic interviews of communities vulnerable to censorship conducted in Zambia, Turkey, and Mongolia between 2013 to 2016. We use analysis of this data to ground our work. We explore needs and requirements of users such as modes of censorship, resistance to network disruption, and appropriate platform consideration. We outline our technological solution and expand on how on-the-ground research of user communities guides technological requirements.
