Suicid är ett betydande folkhälsoproblem. De flesta som utför ett suicidförsök lider av en psykisk sjukdom och har oftast haft kontakt med hälso- och sjukvården innan händelsen. Antalet utalarmeringar till personer som mår psykiskt dåligt har ökat, därmed utgör suicidnära patienter en betydande patientgrupp prehospitalt. Trots det råder det en osäkerhet i omhändertagandet då ambulanssjuksköterskorna känner sig oförberedda och okunniga i vårdmötet. Syftet med studien var att belysa ambulanssjuksköterskors upplevelser av att vårda suicidnära patienter prehospitalt för att öka kunskapen och eventuellt åskådliggöra förbättringsområden som i förlängningen kan stödja den prehospitala vården av suicidnära patienter. Studien har genomförts som en kvalitativ intervjustudie med induktiv ansats. Resultatet mynnade ut i en huvudkategori; Inre och yttre faktorer påverkar det vårdande mötet samt tre kategorier; emotionell påverkan, ambulanssjuksköterskans yrkesroll samt organisationens betydelse. Med hjälp av nio subkategorier har struktur skapats. I resultatet framkom att vården av den suicidnära patienten berörde ambulanssjuksköterskorna på flera emotionella plan, bland annat uppdagades känslosamma skildringar, tragedi och medlidande i vårdmötet. En upplevelse av dränage av emotionell energi framkom också. Bristande kompetens i vårdmötet med den suicidnära patienten erfors emellanåt och det vårdande samtalet ansågs utgöra en viktig aspekt för att uppnå god vård. Ambulanssjuksköterskan ville finnas till för patienten och såg det som sin plikt. Slutligen upplevde ambulanssjuksköterskorna att prekära omständigheter kunde försvåra vården och göra den otillräcklig samt att psykiatrin stundom misslyckades med att ge adekvat vård. I slutet av examensarbetet diskuterades suicid i relation till hållbar utveckling och en hållbar ambulanssjukvård, både ur en ekonomisk- och social kontext., Suicide is a major public health concern. Most individuals who attempt suicide suffer from a mental disorder and have usually been in contact with healthcare professionals prior to the event. The number of mental health callouts have increased, making suicidal patients a considerable patient group in prehospital care. Despite this, some ambulance nurses express feelings of unpreparedness and ignorance in meeting and caring for suicidal patients. The aim of this study was to investigate ambulance nurses’ experiences of providing prehospital care to suicidal patients, to increase the knowledge and potentially identify areas of improvement in the care of suicidal patients. The study was conducted as a qualitative interview study with an inductive approach. The result was composed of one main category: Internal and external determinants influence the caring encounter and three categories: emotional stress, the role of the ambulance nurse and the importance of the organisation. Nine subcategories were created for the purpose of structure. The results suggested that caring for suicidal patients influenced the ambulance nurses emotionally, for example through emotional stories, tragedies and compassion for the patient. The work was further described as emotionally draining. The nurses occasionally experienced a lack of competence in caring encounter for suicidal patients and the communication with the patient was considered important to deliver high-quality care. A desire and duty to offer support to the patient was further expressed. Lastly, ambulance nurses described that difficult conditions could complicate the care, rendering it insufficient, and that the psychiatric care occasionally failed to provide adequate care. The thesis finally discussed suicide in relation to sustainable development and sustainable ambulance care, from an economical as well as a social perspective.

I föreliggande examensarbete presenteras en studie om förskollärare och barnskötares syn och tankar kring sina yrkesroller, och de konflikter som kan uppstå, då två yrkesgrupper arbetar sida vid sida, men med olika formellt ansvar. Vad anser de är det viktigaste i sin yrkesroll, och vilka tankar har man om hur yrkesrollerna kommer att se ut i framtiden?Genom en kvalitativ studie med intervjuer av både förskollärare och barnskötare på en förskola, så samlas de material in som utgör underlag för studien. Utifrån forskningsfrågorna, så kategoriserades materialet från intervjuerna, för att sedan presenteras i en löpande text.Vissa skillnader i informanternas uppfattning om yrkesrollerna kunde urskiljas men samtidigt så lyfts det fram att det är viktigt att ta tillvara på allas kompetens oavsett utbildning. I det vardagliga arbetet på förskolan, så menade alla informanter att personligheten är viktigare än själva yrkesrollen och utbildningen, och det är det som gör dig till den pedagog du är. Det framkommer tydligt att man ser positivt på att arbeta i arbetslag med olika yrkesgrupper, men ser samtidigt att barnskötarrollen på sikt är hotad, då många kommuner byter ut barnskötare mot förskollärare. Inom båda yrkesgrupperna ser man en viss fara, då det gäller det ökade kravet på dokumentation. Båda grupperna uttalar en oro för att man glömmer vad som är viktigast för barnen, och att kontakten och bemötandet med barnen kommer i skymundan

A high cell density perfusion process of monoclonal antibody (MAb) producing Chinese hamster ovary (CHO) cells was developed in disposable WAVE Bioreactor using external hollow fiber (HF) filter as cell separation device. Tangential flow filtration (TFF) and alternating tangential flow (ATF) systems were compared and process applications of high cell density perfusion were studied here: MAb production and cryopreservation. Operations by perfusion using microfiltration (MF) or ultrafiltration (UF) with ATF or TFF and by fed-batch were compared. Cell densities higher than 108 cells/mL were obtained using UF TFF or UF ATF. The cells produced comparable amounts of MAb in perfusion by ATF or TFF, MF or UF. MAbs were partially retained by the MF using ATF or TFF but more severely using TFF. Consequently, MAbs were lost when cell broth was discarded from the bioreactor in the daily bleeds. The MAb cell-specific productivity was comparable at cell densities up to 1.3 x 108 cells/mL in perfusion and was comparable or lower in fed-batch. After 12 days, six times more MAbs were harvested using perfusion by ATF or TFF with MF or UF, compared to fed-batch and 28x more in a 1-month perfusion at 108 cells/mL density. Pumping at a recirculation rate up to 2.75 L/min did not damage the cells with the present TFF settings with HF short circuited. Cell cryopreservation at 0.5 x 108 and 108 cells/mL was performed using cells from a perfusion run at 108 cells/mL density. Major advantages of perfusion are high cell numbers and high total production in a relatively small size bioreactor. Moreover, perfusion is optimal when the product of interest is unstable or if the product yield is low. On the other hand, disadvantages are for example technical challenges originating from non-robust cell separation devices as well as sterility concerns from the more complex set-up needed. In the present work, the use of a WAVE Bioreactor™ system 20/50 in perfusion mode with10 L disposable Cellbag™ bioreactors customized with two dip tubes in combination with disposable hollow fiber filters as external cell separating devices were investigated. Perfusion of an IgG producing CHO cell line was performed in a WAVE Bioreactor™ using either Alternating Tangential Flow or Tangential Flow Filtration. The properties and performances obtained in this bioreactor with both filtration systems were studied. Very high cell densities were achieved and could be stably maintained at high viability indicating of a healthy process suitable for instance for efficient cell banking.
