7. Risk of TIRADS-based inappropriate FNAC in autonomous thyroid nodules is clinically negligible

Abstract

Objective: Thyroid nodule (TN) is usually managed according to Thyroid Imaging And Reporting Data Systems (TIRADS) with the major aim to reduce as much as possible unnecessary fine-needle aspiration cytologies (UN-FNACs). Since the assessment of autonomously functioning thyroid nodule (AFTN) according to TIRADS is heterogeneous, that virtually benign entity may increase the rate of UN-FNAC. This study retrospectively analyzed the appropriateness of TIRADS-based FNAC indication in AFTNs, also looking at the impact of TSH and nodule size. Methods: Cases diagnosed with AFTN on scintigraphy were searched. Patients who had undergone AFTN treatment, were on medications or supplementation that could affect thyroid function, or had multiple AFTNs were excluded. The AFTNs were assessed according to ACR-TIRADS. Results: Forty-eight AFTNs were included of which 37.5% had FNAC indication according to TIRADS. The FNAC indication rate in the case of TSH lower than 0.4 mIU/L was significantly higher than in other cases (P = 0.0078). The most accurate TSH cut-off and AFTN size associated with UN-FNAC were ≤ 0.41 mIU/L and > 22 mm, respectively. The multivariate analysis showed that both TSH and nodule size were independent predictors of UN-FNAC with OR of 6.65 and 6.46, respectively. According to these data, the rate of FNAC indication dropped to 4.16%. Conclusion: Inappropriate FNACs in AFTNs are primarily observed in patients with low TSH and large AFTN. Since these cases typically undergo scintigraphy, the risk of TIRADS-based UN-FNAC is clinically negligible. There is no need for integrating other imaging procedures into the TIRADS model.

13. Gender Segregation: Analysis across Sectoral-Dominance in the UK Labour Market

Abstract

This paper aims to evaluate how changing patterns of sectoral gender segregation play a role in accounting for women's employment contracts and wages in the UK between 2005 and 2020. We then study wage differentials in gender-specific dominated sectors. We found that the propensity of women to be distributed differently across sectors is a major factor contributing to explaining the differences in wages and contract opportunities. Hence, the disproportion of women in female-dominated sectors implies contractual features and lower wages typical of that sector, on average, for all workers. This difference is primarily explained by "persistent discriminatory constraints", while human capital-related characteristics play a minor role. However, wage differentials would shrink if workers had the same potential and residual wages as men in male-dominated sectors. Abstract

The long-standing model-independent annual modulation effect measured by DAMA deep underground at Gran Sasso Laboratory with different experimental configurations is summarized and perspectives will be highlighted. DAMA/LIBRA-phase2 set-up, $\simeq$ 250 kg highly radio-pure NaI(Tl) confirms the evidence of a signal that meets all the requirements of the model independent Dark Matter annual modulation signature at high C.L.; the full exposure is 2.86 ton $\times$ yr over 22 annual cycles. The experiment is currently collecting data in the DAMA/LIBRA-phase2 empowered configuration with an even lower software energy threshold. Abstract

Full self-consistent stationary Vlasov-Maxwell solutions of magnetically confined plasmas are built for systems with cylindrical symmetries. The stationary solutions are thermodynamic equilibrium solutions. These are obtained by computing the equilibrium distribution function resulting from maximizing the entropy and closing the equations with source terms that are then computed by using the obtained distribution. This leads to a self-consistent problem corresponding to solving a set of two coupled second order non-linear differential equations. Relevant plasma parameters are introduced and a bifurcation leading to an improvement of plasma confinement is shown. Conversely in the improved confinement setting, we exhibit the emergence of a separatrix in the integrable motion of a charged particles.

19. Effectiveness of the DyeVert Power XT system during percutaneous coronary interventions

Abstract

ABSTRACT Introduction and objectives: Contrast-induced-acute kidney injury (CI-AKI) is a potential complication of angiographic procedures. The DyeVert Contrast Reduction system (Osprey Medical, United States) is a device to reduce the concentration of contrast medium (CM) in the kidneys by decreasing the amount of CM delivered to patients. Unlike manual systems, few data are available on the DyeVert Power XT system, which is used in conjunction with automated contrast injection. The main aim of our study was to evaluate its effectiveness during percutaneous coronary interventions (PCI). Methods: Between 2020 and 2022, 101 patients who underwent PCI with the DyeVert Power XT system (case group) were enrolled to evaluate the amount of CM saved through the use of this device, as well as the rate, severity, and predictors of CI-AKI. Patients who underwent PCI without the use of the device (control group) were enrolled to create a matched group allowing assessment of differences in CM and the CI-AKI rate. Results: : In the case group, the amount of CM saved was 114 ± 42 mL, representing an average of 32% of the total CM. Fourteen patients (13.9%) developed CI-AKI. The only independent predictors of CI-AKI were hematocrit (OR, 0.86; 95%CI, 0.74-0.99; P = .04) and ejection fraction (OR, 0.88; 95%CI, 0.82-0.95; P = .001). As a result of diversion by the device, the amount of CM delivered was lower in the case group than in controls (252 vs 267 mL; P = .42), but this difference was nonsignificant. Equally, the reduction in CI-AKI (14.3% vs 16.3%) was nonsignificant. Conclusions: Hematocrit and ejection fraction may be more important predictors of CI-AKI than the CM volume normally used during PCI in the general population. The net practical benefit of DyeVert Power XT was low.

RESUMEN Introducción y objetivos: La nefropatía inducida por contraste (NIC) es una potencial complicación de los procedimientos angiográficos. El sistema DyeVert Power (Osprey Medical, Estados Unidos) permite reducir la concentración renal del medio de contraste al disminuir la cantidad administrada a los pacientes. Al contrario que sobre los sistemas manuales, existen pocos datos disponibles sobre el sistema DyeVert, que se utiliza junto a la inyección automática de contraste. El objetivo principal de este estudio fue evaluar su eficacia en procedimientos de intervencionismo coronario percutáneo (ICP). Métodos: Entre 2020 y 2022 se incluyó a 101 pacientes a quienes se realizó ICP utilizando el sistema DyeVert Power XT (grupo de casos) para evaluar la cantidad ahorrada de medio de contraste, así como la tasa, la gravedad y los predictores de NIC. Además, se seleccionó un grupo control de pacientes a los que se había realizado ICP sin utilizar el sistema DyeVert para comparar la cantidad de medio de contraste administrado y la tasa de NIC. Resultados: En el grupo de casos se redujo la administración de medio de contraste en 114 ± 42 ml (una media del 32% del total). Desarrollaron NIC 14 pacientes (13,9%). Los predictores de NIC fueron el hematocrito (OR = 0,86; IC95%: 0,74-0,99; p = 0,04) y la fracción de eyección (OR = 0,88; IC95%: 0,82-0,95; p = 0,001). Como resultado de la utilización del sistema DyeVert, la cantidad administrada de medio de contraste fue menor, pero sin diferencias estadísticamente significativas (252 frente a 267 ml; p = 0,42). La tasa de NIC fue menor con el sistema DyeVert, pero sin alcanzar la significación estadística (14,3 frente a 16,3%; p = 1,0). Conclusiones: El hematocrito y la fracción de eyección, más que la cantidad de contraste administrada, pueden ser predictores de NIC en los pacientes que reciben ICP. El beneficio del sistema DyeVert fue bajo.

Abstract

Thyroid metastases from nonthyroidal malignancies (NTMs) represent a diagnostic challenge, often displaying heterogeneous clinical manifestations. These metastases are rare but significant, accounting for approximately 2% of thyroid malignancies. Distinguishing them from primary thyroid malignancies is challenging due to the lack of specific ultrasound features, and the ultrasound-based risk stratification systems offer limited utility in such cases. Fine needle aspiration cytology is crucial for definitive diagnosis, yet it may not always provide accurate results. In this case report, we describe a unique instance of thyroid metastases originating from renal cell carcinoma, emphasizing the complexities in diagnosis and the importance of considering oncological conditions when assessing thyroid masses. Awareness of thyroid metastasis from NTMs, particularly in cases of diffuse thyroid hypoechogenicity and hypothyroidism, is essential for clinicians in their diagnostic approach.

Abstract

In this paper, we introduce a codification of the paths towards synchronization for synchronizing flows defined over a network. The collection of paths toward synchronization defines a combinatorial structure: the transition diagram. We describe the transition diagram corresponding to the Laplacian flow over the completely connected graph. This applies to the Kuramoto flow over the same graph when initial conditions close to the diagonal are considered. We present as well some results concerning the Laplacian and Kuramoto flows over the complete bipartite graph.

Abstract

BackgroundSleep is disturbed in Rett syndrome (RTT), a rare and progressive neurodevelopmental disorder primarily affecting female patients (prevalence 7.1/100,000 female patients) linked to pathogenic variations in the X-linked methyl-CpG-binding protein 2 (MECP2) gene. Autonomic nervous system dysfunction with a predominance of the sympathetic nervous system (SNS) over the parasympathetic nervous system (PSNS) is reported in RTT, along with exercise fatigue and increased sudden death risk. The aim of the present study was to test the feasibility of a continuous 24 h non-invasive home monitoring of the biological vitals (biovitals) by an innovative wearable sensor device in pediatric and adolescent/adult RTT patients.MethodsA total of 10 female patients (mean age 18.3 ± 9.4 years, range 4.7–35.5 years) with typical RTT and MECP2 pathogenic variations were enrolled. Clinical severity was assessed by validated scales. Heart rate (HR), respiratory rate (RR), and skin temperature (SkT) were monitored by the YouCare Wearable Medical Device (Accyourate Group SpA, L’Aquila, Italy). The average percentage of maximum HR (HRmax%) was calculated. Heart rate variability (HRV) was expressed by consolidated time-domain and frequency-domain parameters. The HR/LF (low frequency) ratio, indicating SNS activation under dynamic exercise, was calculated. Simultaneous continuous measurement of indoor air quality variables was performed and the patients’ contributions to the surrounding water vapor partial pressure [PH2O (pt)] and carbon dioxide [PCO2 (pt)] were indirectly estimated.ResultsOf the 6,559.79 h of biovital recordings, 5051.03 h (77%) were valid for data interpretation. Sleep and wake hours were 9.0 ± 1.1 h and 14.9 ± 1.1 h, respectively. HRmax % [median: 71.86% (interquartile range 61.03–82%)] and HR/LF [median: 3.75 (interquartile range 3.19–5.05)] were elevated, independent from the wake–sleep cycle. The majority of HRV time- and frequency-domain parameters were significantly higher in the pediatric patients (p ≤ 0.031). The HRV HR/LF ratio was associated with phenotype severity, disease progression, clinical sleep disorder, subclinical hypoxia, and electroencephalographic observations of multifocal epileptic activity and general background slowing.ConclusionOur findings indicate the feasibility of a continuous 24-h non-invasive home monitoring of biovital parameters in RTT. Moreover, for the first time, HRmax% and the HR/LF ratio were identified as potential objective markers of fatigue, illness severity, and disease progression.

Abstract

The impact of common and rare variants in COVID-19 host genetics has been widely studied. In particular, in Fallerini et al. (Human genetics, 2022, 141, 147–173), common and rare variants were used to define an interpretable machine learning model for predicting COVID-19 severity. First, variants were converted into sets of Boolean features, depending on the absence or the presence of variants in each gene. An ensemble of LASSO logistic regression models was used to identify the most informative Boolean features with respect to the genetic bases of severity. After that, the Boolean features, selected by these logistic models, were combined into an Integrated PolyGenic Score (IPGS), which offers a very simple description of the contribution of host genetics in COVID-19 severity.. IPGS leads to an accuracy of 55%–60% on different cohorts, and, after a logistic regression with both IPGS and age as inputs, it leads to an accuracy of 75%. The goal of this paper is to improve the previous results, using not only the most informative Boolean features with respect to the genetic bases of severity but also the information on host organs involved in the disease. In this study, we generalize the IPGS adding a statistical weight for each organ, through the transformation of Boolean features into “Boolean quantum features,” inspired by quantum mechanics. The organ coefficients were set via the application of the genetic algorithm PyGAD, and, after that, we defined two new integrated polygenic scores (IPGSph1 and IPGSph2). By applying a logistic regression with both IPGS, (IPGSph2 (or indifferently IPGSph1) and age as inputs, we reached an accuracy of 84%–86%, thus improving the results previously shown in Fallerini et al. (Human genetics, 2022, 141, 147–173) by a factor of 10%.

26. Estimating the effective sampling area of an alcohol-baited trap for monitoring the coffee berry borer

Abstract

ABSTRACT Here, single-trap, multiple-release-recapture field experiments were used for the first time to estimate the trap sampling area and absolute population density from trap catches for the coffee berry borer (CBB) Hypothenemus hampei (Ferrari). Fluorescent-dusted CBBs were released at several distances at all four cardinal points from an ethanol-methanol-baited multifunnel trap. Only 2.6 % of released beetles were recaptured, and recaptures decreased significantly with increasing release distances. The recapture analyses revealed that CBB moves randomly in the field and disperses at a maximum of 22.2 m. Despite the short plume of the trap (1.3 m), the calculated trap sampling area was 0.17 ha, with an overall catch probability of 0.01. Therefore, capturing 100 CBBs trap –1 ha –1 at the early filling stage of coffee beans reflects a 20.2 million borers ha –1 population at harvest. This results in a projected bean loss of 60.3 kg ha –1 . The findings shed light on improvements in using semiochemical traps and interpretations of catch data to enhance the CBB integrated management. Future studies on the performance of attractants and trap designs in different coffee farmscapes are encouraged to adopt the present methodology.

Abstract

Zinc tungstate (ZnWO$_4$) crystal scintillators are promising detection material for the experiments searching for double beta decay, dark matter, and investigating rare alpha decays. An extended R&D was performed to develop advanced quality ZnWO$_4$ crystal scintillators. The R&D programme included the selection of the initial materials, the variation of the compound stoichiometry, the application of single and double crystallization, and the annealing of the crystal boules. The optical transmittance of the produced boules was measured, and the luminescence under X-ray excitation in the temperature region from 85 K to room temperature was studied (thermally stimulated luminescence was measured till 350 K). The energy resolution and the relative scintillation pulse amplitude were measured with gamma-sources demonstrating high scintillation properties of the samples produced by single crystallization from deeply purified zinc and tungsten oxides, with stoichiometric composition, annealed in air atmosphere., Comment: 14 pages; 10 figures

Abstract

We herein present an overview of the upcoming 5th edition of the World Health Organization Classification of Haematolymphoid Tumours focussing on lymphoid neoplasms. Myeloid and histiocytic neoplasms will be presented in a separate accompanying article. Besides listing the entities of the classification, we highlight and explain changes from the revised 4th edition. These include reorganization of entities by a hierarchical system as is adopted throughout the 5th edition of the WHO classification of tumours of all organ systems, modification of nomenclature for some entities, revision of diagnostic criteria or subtypes, deletion of certain entities, and introduction of new entities, as well as inclusion of tumour-like lesions, mesenchymal lesions specific to lymph node and spleen, and germline predisposition syndromes associated with the lymphoid neoplasms.

Abstract

We study the out-of-equilibrium properties of the antiferromagnetic Hamiltonian Mean-Field model at low energy. In this regime, the Hamiltonian dynamics exhibits the presence of a stationary state where the rotators are gathered in a bicluster. This state is not predicted by equilibrium statistical mechanics in the microcanonical ensemble. Performing a low kinetic energy approximation, we derive the explicit expression of the magnetization vector as a function of time. We find that the latter displays coherent oscillations, and we show numerically that the probability distribution for its phase is bimodal or quadrimodal. We then look at the individual rotator dynamics as a motion in an external time-dependent potential, given by the magnetization. This dynamics exhibits two distinct time scales, with the fast one associated to the oscillations of the global magnetization vector. Performing an average over the fast oscillations, we derive an expression for the effective force acting on the individual rotator. This force is always bimodal, and determines a low frequency oscillation of the rotators. Our approach leads to a self-consistent theory linking the time-dependence of the magnetization to the motion of the rotators, providing a heuristic explanation for the formation of the bicluster.

Abstract

An experiment to search for 2ε-, εβ+- and 2β+-decays of 106Cd, using a 215 g cadmium tungstate scintillation crystal enriched at 66 % by 106Cd (106CdWO4) is carried out at the Gran Sasso underground laboratory (Italy). Events in the 106CdWO4 detector are recorded in (anti)coincidences with two large-volume CdWO4 scintillation counters. The design of the detector system, calibration and background measurements, methods, and results of data analysis to determine key detector characteristics are described. The experimental data are compared with Monte Carlo simulation results, and a background model is constructed. The radioactive contamination of the setup components is studied. The sensitivity of the experiment approaches the level of theoretical predictions for the 2νεβ+-decay channel, while for other possible 2β-decay channels it is already on the level of lim T1/2 ∼ 1021-1022 years.
