2. The differential impacts of the Affordable Care Act for employees of small versus large firms

Abstract

One concern about the Patient Protection and Affordable Care Act (ACA) was the possible negative impacts on small group employees. Small firms have fewer resources and their decision to offer employer-sponsored insurance (ESI) might be affected by new regulations in the ACA. On the other hand, the ACA established the individual exchange as well as expanded Medicaid in certain states, giving employees more choices for obtaining health insurance. Using data from the Medical Expenditure Panel Survey (MEPS) Household Component 2007-2014, we examined whether trends in ESI availability, insurance status, access to care, and financial burden differed for small group employees versus large group employees. Difference-in-difference models showed no significant differences between small group and large group employees.

5. Liberalizing Women’s Rights in Saudi Arabia: Powered by the Kingdom’s Economic Will

Abstract

Since 2017, women’s rights have increased significantly in Saudi Arabia, from being allowed to drive cars to no longer having to wear head coverings. Saudi women now have greater mobility, not only to travel freely, but also to participate in the political, economic, and social rights spheres. The question remains: Why did the Kingdom wait so long to advance women’s rights? The central question to be addressed by this thesis is, “What factors led to the liberalization of women’s rights in Saudi Arabia?” To answer it, I will argue that the Kingdom’s primary motivation is to increase labor participation and diversify its economy, thereby making the country more prosperous and reducing its reliance on oil revenue. To accomplish this, the Kingdom turned to women, who make up nearly half of its population, to help revitalize the country and its economy. Saudi Arabia’s economic rationale for greater inclusion of women has led the Kingdom to grant privileges and opportunities to women by way of making them valuable contributors to the country’s economy. The initiatives and policies impacting women have benefited both Saudi women and the country.

Abstract

Reversal of ischemia is mediated by neo-angiogenesis requiring endothelial cell (EC) and pericyte interactions to form stable microvascular networks. We describe an unrecognized role for tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) in potentiating neo-angiogenesis and vessel stabilization. We show that the endothelium is a major source of TRAIL in the healthy circulation compromised in peripheral artery disease (PAD). EC deletion of TRAIL in vivo or in vitro inhibited neo-angiogenesis, pericyte recruitment, and vessel stabilization, resulting in reduced lower-limb blood perfusion with ischemia. Activation of the TRAIL receptor (TRAIL-R) restored blood perfusion and stable blood vessel networks in mice. Proof-of-concept studies showed that Conatumumab, an agonistic TRAIL-R2 antibody, promoted vascular sprouts from explanted patient arteries. Single-cell RNA sequencing revealed heparin-binding EGF-like growth factor in mediating EC-pericyte communications dependent on TRAIL. These studies highlight unique TRAIL-dependent mechanisms mediating neo-angiogenesis and vessel stabilization and the potential of repurposing TRAIL-R2 agonists to stimulate stable and functional microvessel networks to treat ischemia in PAD.
