Intracellular pathogens are responsible for an enormous amount of worldwide morbidity and mortality, and each has evolved specialized strategies to establish and maintain their replicative niche. Listeria monocytogenes is a facultative intracellular pathogen that secretes a pore-forming cytolysin called listeriolysin O (LLO), which disrupts the phagosomal membrane and, thereby, allows the bacteria access to their replicative niche in the cytosol. Nonsynonymous and synonymous mutations in a PEST-like domain near the LLO N terminus cause enhanced LLO translation during intracellular growth, leading to host cell death and loss of virulence. Here, we explore the mechanism of translational control and show that there is extensive codon restriction within the PEST-encoding region of the LLO messenger RNA (mRNA) (hly). This region has considerable complementarity with the 5' UTR and is predicted to form an extensive secondary structure that overlaps the ribosome binding site. Analysis of both 5' UTR and synonymous mutations in the PEST-like domain that are predicted to disrupt the secondary structure resulted in up to a 10,000-fold drop in virulence during mouse infection, while compensatory double mutants restored virulence to WT levels. We showed by dynamic protein radiolabeling that LLO synthesis was growth phase-dependent. These data provide a mechanism to explain how the bacteria regulate translation of LLO to promote translation during starvation in a phagosome while repressing it during growth in the cytosol. These studies also provide a molecular explanation for codon bias at the 5' end of this essential determinant of pathogenesis.

Perennial pepperweed, Lepidium latifolium L. (Brassicales: Brassicaceae), is an invasive weed that can form dense stands and displace native species. Bagrada hilaris Burmeister (Hemiptera: Pentatomidae) is a serious economic pest of Brassicaceae vegetable crops. Bagrada bug also feeds on L. latifolium and may interact with the plant fungal pathogen Albugo lepidii S.I. (Peronosporales: Albuginaceae) to affect biological control of L. latifolium. A series of laboratory experiments, including Y-tube olfactometer and host-choice tests, were conducted to investigate B. hilaris host-preference behavior. Adults were attracted to the odor of healthy L. latifolium compared with A. lepidii-infected leaves. Bagrada hilaris consistently preferred to feed on healthy L. latifolium when offered both healthy and A. lepidii-infected plants. Experiments were conducted to determine the effects of A. lepidii-infected L. latifolium on B. hilaris survival and development. Survival of all B. hilaris immature stages and adults was markedly reduced for those reared on A. lepidii-infected leaves. Total development time and stage-specific development were faster on healthy L. latifolium leaves compared with A. lepidii-infected leaves. In addition, the ability of B. hilaris adults to passively transmit the rust was studied. Our data demonstrated that B. hilaris could acquire the rust spores while feeding, but it did not passively transmit the pathogen to healthy plants.

Background Small hospitals in the US may lack access to infectious diseases (ID) expertise despite similar rates of antimicrobial use and drug-resistant bacteria as larger hospitals. A tele-antimicrobial stewardship program (TASP) is a force multiplier, expanding access to specialty care, training, and guidance on appropriate resource utilization. Data on the impact of TASPs in community or rural inpatient settings is limited. Methods We established a TASP at a 160-bed hospital in Armstrong County, PA (population < 5000) in September 2020. Tele-ID consult services were already being used (Figure 1). A non-local ID pharmacist or ID physician performed prospective audits and provided feedback with 1 local pharmacist on a 30-minute video conference call daily. At TASP implementation, all patients receiving intravenous (IV) fluoroquinolones, metronidazole, and azithromycin were reviewed. Figure 1 shows the additional support following TASP implementation, including addition of ceftriaxone, carbapenems, IV vancomycin, and tocilizumab to daily reviews. A patient monitoring form was developed to track interventions and the local pharmacists were trained in documentation. Table 1 lists other TASP features implemented. Figure 1. TASP Timeline Table 1. TASP Accomplishments Results From 09/01/2020 to 04/30/2021, 304 stewardship opportunities were identified and 77% of interventions were accepted. Recommending a duration of therapy was accepted most frequently (93.5%) and de-escalation of therapy least frequently (69.6%) (Table 2). Recommending an ID consultation or diagnostic testing was always accepted but only comprised 6.2% of all interventions. Daily calls involved an average of 5 patient reviews. Monthly antimicrobial use declined on average from 673 DOT (days of therapy)/1000 PD (patient days) to 638 DOT/1000 PD (Figure 2). Success is dependent on robust educational efforts, establishing strong relationships with local providers, and involvement of key stakeholders. Insects manifest phenotypic plasticity in their development and behavior in response to plant defenses, via molecular mechanisms that produce tissue-specific changes. Phenotypic changes might vary between species that differ in their preferred hosts and these effects could extend beyond larval stages. To test this, we manipulated the diet of southern armyworm (SAW; Spodoptera eridania) and fall armyworm (FAW; Spodoptera frugiperda) using a tomato mutant for jasmonic acid plant defense pathway (def1), and wild type plants, and then quantified gene expression of Troponin t (Tnt) and flight muscle metabolism of the adult insects. Differences in Tnt spliceform ratios in insect flight muscles correlate with changes to flight muscle metabolism and flight muscle output. We found that SAW adults reared on induced def1 plants had higher relative abundances (RA) of the A isoform of Troponin t (Tnt A) in their flight muscles; in contrast, FAW adults reared on induced def1 plants had lower RAs of Tnt A in their flight muscles compared to adults reared on def1 and controls. Although mass-adjusted flight metabolic rates showed no independent host plant effects in either species, higher flight metabolic rates in SAW correlated with increased RAs of Tnt A. Flight muscle metabolism also showed an interaction of host plants with Tnt A in both species, suggesting that host plants might be influencing flight muscle metabolic output by altering Tnt. This study illustrates how insects respond to variation in host plant's chemical defense by phenotypic modifications to their flight muscle proteins, with possible implications for dispersal.

For more than 100 yrs, the wheat stem sawfly (WSS) Cephus cinctus Norton (Hymenoptera: Cephidae), has been a destructive pest of cereal crops in the Northern Great Plains. WSS infestation levels >70% have been reported, and economic loss from crop damage caused by this insect can be as high as $80 million (USD) per year in the state of Montana alone. Contact insecticides are not effective against WSS because the larvae and pupae are protected inside the plant stem, and the development of solid-stem cultivars have not resulted in sufficient declines in WSS populations. Biological control offers the best hope for achieving economical and sustainable management of WSS. However, development and advancement of WSS biological control strategies has been limited in recent years. Presently, there are three main types of biologicals available for managing WSS: two native hymenopteran parasitoids, entomopathogenic nematodes, and entomopathogenic fungi. Here, we present the most current research regarding the use of these biologicals against WSS, and we discuss new strategies and research opportunities for improving their effectiveness for managing this persistent pest. It is unlikely that a single agent will produce the desired results, but by increasing pressure from all three sources, their combined impact may more reliably cause significant reductions in WSS infestations. Eradication of WSS, a native species, is not a realistic goal, but it is probable that WSS populations will decline more readily as biological control methods continue to advance.

Drought leading to water deficit stress is known to reduce performance of galling insects. The shoot tip-galling fly Parafreutreta regalis has been released for biological control of Cape-ivy (Delairea odorata) in California. Lack of moisture during the dry season causes wilting of Cape-ivy shoots, and subsequent reduced host quantity and quality could influence the fly’s ability to multiply and establish. We imposed water deficit stress on potted Cape-ivy plants, then measured the plant’s and insect’s response to water deficit compared to fully-watered plants. Water deficit stress was imposed during female oviposition (short-term), gall development (long-term), or full insect life cycle (long-term). Plants subjected to long-term water deficit showed declines in shoot growth, leaf area, and total biomass. In choice tests, 72% fewer galls were found on water deficit stressed plants, but there was no difference in the numbers of insects per gall. In no-choice tests, long-term water deficit reduced the number and size of galls by up to 51% and 39%, respectively, and puparia inside the galls were up to 35% smaller. Long-term water deficit also increased the time required for the insect to complete its life cycle (egg to adult) by about 10 days, and up to 80% fewer adults emerged, decreasing the fly’s rate of increase by up to five-fold. Our study shows that P. regalis will oviposit and develop successfully on water deficit-stressed plants, but declines in female preference, insect development and abundance could inhibit fly field establishment and/or population growth under climatic drought conditions.

Cholesterol-dependent cytolysins (CDCs) represent a family of homologous pore-forming proteins secreted by many Gram-positive bacterial pathogens. CDCs mediate membrane binding partly through a conserved C-terminal undecapeptide, which contains a single cysteine residue. While mutational changes to other residues in the undecapeptide typically have severe effects, mutation of the cysteine residue to alanine has minor effects on overall protein function. Thus, the role of this highly conserved reactive cysteine residue remains largely unknown. We report here that the CDC listeriolysin O (LLO), secreted by the facultative intracellular pathogen Listeria monocytogenes , was posttranslationally modified by S-glutathionylation at this conserved cysteine residue and that either endogenously synthesized or exogenously added glutathione was sufficient to form this modification. When recapitulated with purified protein in vitro , this modification completely ablated the activity of LLO, and this inhibitory effect was fully reversible by treatment with reducing agents. A cysteine-to-alanine mutation in LLO rendered the protein completely resistant to inactivation by S-glutathionylation, and a mutant expressing this mutation retained full hemolytic activity. A mutant strain of L. monocytogenes expressing the cysteine-to-alanine variant of LLO was able to infect and replicate within bone marrow-derived macrophages indistinguishably from the wild type in vitro , yet it was attenuated 4- to 6-fold in a competitive murine infection model in vivo . This study suggests that S-glutathionylation may represent a mechanism by which CDC-family proteins are posttranslationally modified and regulated and help explain an evolutionary pressure to retain the highly conserved undecapeptide cysteine.

Summary A primary function of adult winged insects is dispersal. Limiting larval dietary intake (partial starvation) has been shown to affect the flight muscle metabolism of adult moths reared on artificial diet, but a more ecologically relevant question is whether natural variation in host plant quality can lead to differences in the flight capacity of adult insects. Recent studies have shown that inbreeding compromises plant antiherbivore defences. We created inbred and outbred progeny from locally collected horsenettle (Solanum carolinense L.) and examined how host plant inbreeding affects the growth, development, and flight muscle physiology of tobacco hornworm (Manduca sexta L.), a specialist herbivore on Solanaceae. We tested the hypothesis that within population genetic variation in host plant quality, resulting from inbreeding, can create significant changes to the larval development and flight physiology of an adult insect. We found that Manduca larvae reared on inbred horsenettle plants grew faster and developed into larger pupae compared to larvae reared on outbred plants. Adult flight metabolic rate was greater in adults reared on inbred plants compared to outbred plants, and this elevation was independent of body mass when we excluded one plant family that produced small, low metabolic rate moths regardless of breeding regime. Differences in mass-specific flight metabolism were associated with changes in alternative splicing of troponin t, a flight muscle protein that regulates muscle contraction. These results show that host plant inbreeding can create effects that cascade through larval and pupal development to affect dispersal-related traits of the adult stage. Hence, plant inbreeding may also impact herbivore population dynamics, particularly their ability to spread away from, and possibly into, isolated patches of inbred plants creating increased herbivore pressure on these plant populations. More generally, our findings reveal that changes in population biology at one trophic level can affect the metabolic physiology and flight capacity of an animal at a higher trophic level.

Larra bicolor F. (Hymenoptera: Crabronidae) is a specialist parasitoid of Scapteriscus (Orthoptera: Gryllotalpidae) mole crickets, attacking adults and medium to large nymphs. Reproductive systems were dissected from 10 female wasps collected in northern Florida. Each had 2 ovaries, each with 3 ovarioles. The maximal number of mature eggs (10) plus developing oocytes (83) was 93. Female wasps deposit an egg on the venter of the host's thorax, and the wasp larva develops as an ectoparasitoid. Twenty newly-emerged female wasps housed in small cages with at least 1 male and with 7 potential hosts replaced daily deposited a mean 2.44 eggs (range 0–10) per day for a total lifetime production averaging 56 eggs (range 17–91) during a lifespan averaging 23.5 d (range 8–40). Assuming 3 wasp generations with fecundity as shown to 1 host generation, per year, the wasp should easily be able to outreproduce its host mole crickets. A few of the hosts became superparasitized with 2 or even 3 eggs, but at most 1...

The wheat stem sawfly, (Cephus cinctus Norton) Hymenoptera: Cephidae, has been a major pest of winter wheat and barley in the northern Great Plains for more than 100 years. The insect's cryptic nature and lack of safe chemical control options make the wheat stem sawfly (WSS) difficult to manage; thus, biological control offers the best hope for sustainable management of WSS. Entomopathogenic nematodes (EPNs) have been used successfully against other above-ground insect pests, and adding adjuvants to sprays containing EPNs has been shown to improve their effectiveness. We tested the hypothesis that adding chemical adjuvants to sprays containing EPNs will increase the ability of EPNs to enter wheat stems and kill diapausing WSS larvae. This is the first study to test the ability of EPNs to infect the WSS, C. cinctus, and test EPNs combined with adjuvants against C. cinctus in both the laboratory and the field. Infection assays showed that three different species of EPNs caused 60-100% mortality to WSS larvae. Adding Penterra, Silwet L-77, Sunspray 11N, or Syl-Tac to solutions containing EPNs resulted in higher WSS mortality than solutions made with water alone. Field tests showed that sprays containing S. feltiae added to 0.1% Penterra increased WSS mortality up to 29.1%. These results indicate a novel control method for WSS, and represent a significant advancement in the biological control of this persistent insect pest.

Electrical Submersible Pump (ESP) systems have been applied successfully for seabed hydrocarbon boosting for years. However, the design of ESPs for in-well applications presents several limitations for its use on the seafloor. The long slender body and limited gas handling capabilities have historically been important factors restricting the wider use of ESPs in subsea applications. In order to overcome the main design challenges, ESPs have been placed in caissons that enable gas/liquid separation and installation of the pump in a vertical position. This arrangement has resulted in high installation and work-over costs, limiting the acceptance in subsea applications. For low gas applications, skid based subsea ESP designs have also recently emerged, but these are large and heavy, which also leads to higher installation and work-over costs. The Subsea Production Alliance between Baker Hughes and Aker Solutions has developed an innovative new solution to these challenges in the form of an ESP system installed in a flow line jumper. This new solution utilizes the same well proven ESP and increases its gas tolerance by utilizing state-of-the art technology used in subsea separation and boosting applications. Fluid conditioning, gas/liquid mixing and liquid recirculation together with field proven control algorithms form the revolutionary flowline booster system. Installation and retrieval of the jumper-mounted ESPs are as simple as retrieving a standard flowline jumper by using a light installation vessel and field proven tie-in technology. This new system provides for a step-change in operational robustness and cost-benefit for subsea boosting solutions. This paper presents the concept and highlights the benefits of modularity, ease of installation and replacement, operability and client value. A simulation of the operational performance of the system applied in a brown field application is presented. A commercial comparison between the flowline booster system and a skid based ESP boosting system is also presented.

Inbreeding commonly occurs in flowering plants and often results in a decline in the plant's defense response. Insects prefer to feed and oviposit on inbred plants more than outbred plants – suggesting that selecting inbred host plants offers them fitness benefits. Until recently, no studies have examined the effects of host plant inbreeding on insect fitness traits such as growth and dispersal ability. In a recent article, we documented that tobacco hornworm (Manduca sexta L.) larvae that fed on inbred horsenettle (Solanum carolinense L.) plants exhibited accelerated larval growth and increased adult flight capacity compared to larvae that fed on outbred plants. Here we report that M. sexta mortality decreased by 38.2% when larvae were reared on inbred horsenettle plants compared to larvae reared on outbreds. Additionally, inbred plants showed a notable reduction in the average relative expression levels of LIPOXYGENEASE-D (LoxD) and 12-OXOPHYTODIENOATE REDUCTASE-3 (OPR3), two genes in the jasmonic acid signaling pathway that are upregulated in response to herbivore damage. Our study presents evidence that furthers our understanding of the biochemical mechanism responsible for differences in insect performance on inbred vs. outbred host plants.

The fluidization behavior of fumed silica, zirconia, and iron oxide nanopowders was studied at atmospheric and reduced pressures. Using a high-speed laser imaging system, the characteristics of fluidized aggregates of nanoparticles were studied in real time. The effect of different particle interactions such as London–van der Waals, liquid bridging and electrostatic on different fluidization parameters was studied at atmospheric pressure. The reduction of interparticle forces resulted in a reduced aggregate size and minimum fluidization velocity (Umf) and an increased bed expansion. Nanoparticles were also fluidized at reduced pressure (∼ 16 Pa) with vibration to study the effect of low pressure on the minimum fluidization velocity. Aggregate properties (size, density) instead of primary nanoparticle properties were found to govern the minimum fluidization velocity and expansion of the fluidized bed. An important consideration is the relative strength of intra-aggregate interparticle forces (forces within the aggregate holding nanoparticles together) to inter-aggregate interparticle forces (forces between aggregates). This relative strength may be inferred from the sphericity of the aggregates during fluidization.

The subunit composition of postsynaptic neurotransmitter receptors is a key determinant of synaptic physiology. Two glutamate receptor subunits,Drosophilaglutamate receptor IIA (DGluRIIA) and DGluRIIB, are expressed at theDrosophilaneuromuscular junction and are redundant for viability, yet differ in their physiological properties. We now identify a third glutamate receptor subunit at theDrosophilaneuromuscular junction, DGluRIII, which is essential for viability. DGluRIII is required for the synaptic localization of DGluRIIA and DGluRIIB and for synaptic transmission. Either DGluRIIA or DGluRIIB, but not both, is required for the synaptic localization of DGluRIII. DGluRIIA and DGluRIIB compete with each other for access to DGluRIII and subsequent localization to the synapse. These results are consistent with a model of a multimeric receptor in which DGluRIII is an essential component. At single postsynaptic cells that receive innervation from multiple motoneurons, DGluRIII is abundant at all synapses. However, DGluRIIA and DGluRIIB are differentially localized at the postsynaptic density opposite distinct motoneurons. Hence, innervating motoneurons may regulate the subunit composition of their receptor fields within a shared postsynaptic cell. The capacity of presynaptic inputs to shape the subunit composition of postsynaptic receptors could be an important mechanism for synapse-specific regulation of synaptic function and plasticity.

This article presents a structured psychoeducational group intervention for increasing social justice awareness, knowledge, and advocacy skills with upper elementary, middle school, or junior high aged students. The empowering students for social justice model is presented. A general framework that includes goals and activities for an eight-session group is described. Issues processed in the group revolved around the social justice themes of oppression, prejudice, and racism. Implications for school counselors are also provided.

Evolution has produced proteins with exquisite ligand binding specificity, and manipulating this effect has been the basis for much of modern rational drug design. However, there are general classes of proteins with broader ligand selectivity linked to function, the origin of which is poorly understood. The odorant binding proteins (OBPs) sequester volatile molecules for transportation to the olfactory receptors. Rat OBP3, which we characterize by X-ray crystallography and NMR, binds a homologous series of aliphatic gamma-lactones within its aromatic-rich hydrophobic pocket with remarkably little variation in affinity but extensive enthalpy/entropy compensation effects. We show that the binding energetics are modulated by two desolvation processes with quite different thermodynamic signatures. Ligand desolvation follows the classical hydrophobic effect; however, cavity desolvation is consistent with the liberation of "high energy" water molecules back into bulk solvent with a strong, but compensated, enthalpic contribution, which together. underpin the origins of broad ligand binding selectivity.

Legionella pneumophila is an intracellular bacterial pathogen that is the cause of a severe pneumonia in humans called Legionnaires’ disease. A key feature of L. pneumophila pathogenesis is the rapid influx of neutrophils into the lungs, which occurs in response to signaling via the IL-1R. Two distinct cytokines, IL-1α and IL-1β, can stimulate the type I IL-1R. IL-1β is produced upon activation of cytosolic sensors called inflammasomes that detect L. pneumophila in vitro and in vivo. Surprisingly, we find no essential role for IL-1β in neutrophil recruitment to the lungs in response to L. pneumophila. Instead, we show that IL-1α is a critical initiator of neutrophil recruitment to the lungs of L. pneumophila–infected mice. We find that neutrophil recruitment in response to virulent L. pneumophila requires the production of IL-1α specifically by hematopoietic cells. In contrast to IL-1β, the innate signaling pathways that lead to the production of IL-1α in response to L. pneumophila remain poorly defined. In particular, although we confirm a role for inflammasomes for initiation of IL-1β signaling in vivo, we find no essential role for inflammasomes in production of IL-1α. Instead, we propose that a novel host pathway, perhaps involving inhibition of host protein synthesis, is responsible for IL-1α production in response to virulent L. pneumophila. Our results establish IL-1α as a critical initiator of the inflammatory response to L. pneumophila in vivo and point to an important role for IL-1α in providing an alternative to inflammasome-mediated immune responses in vivo.

Larra bicolor F. (Hymenoptera: Crabronidae) is a specialist parasitoid of Scapteriscus (Orthoptera: Gryllotalpidae) mole crickets, attacking adults and medium to large nymphs of the hosts. Adult wasps derive energy from plant nectars. In replicated trials in pastures in northern Florida, many more wasps fed on nectar of Spermacoce verticillata F. (Rubiaceae), a non-native plant, than on nectar of the native plants Spermacoce prostrata Aubl. or Spermacoce remota Lamarck. Few of them fed on the native plant Solidago fistulosa Michx. (Asteraceae). About as many fed on the native plant Chamaecrista fasciculata (Michx) Greene as on S. verticillata in the autumn months until the native plant ceased flowering by October. In contrast, S. verticillata continues flowering until the first hard freeze, which typically occurs in December, so overall, it is a more reliable nectar source in northern Florida and may be still more reliable in frost-free areas of southern Florida where it may flower year-round. The number of immature wasps (eggs and larvae) parasitizing mole crickets was positively related to host density but also declined with distance from a plot of S. verticillata out to 200 m, based on samples of pitfall trap-collected mole crickets. The occurrence of parasitized mole crickets at a 200-m distance suggests that female wasps recruited to a plot of S. verticillata forage for hosts out to at least 200 m. This in turn suggests that mole cricket populations might be diminished by planting plots of S. verticillata at least 400 m apart when L. bicolor wasps are present.

International audience; An in vitro system was developed to mimic the structural and flow conditions of the human olfactory epithelium and to measure the dynamics of odorant-binding to odor-binding protein (OBP). A hydrophilic fused silica capillary, coated internally with a thin (about 1.3 µm) aqueous film of recombinant rat-OBP3 mimicked the human olfactory epithelium. Isobutylthiazole in air was introduced into the capillary, and the outflow gas was monitored in real-time using on-line mass spectrometry. Time-dependent changes in the gas phase odorant concentration gave an indication of the rate and extent of mass transfer between the gas phase and the aqueous OBP layer during the initial uptake phase, during the steady state (when the OBP was fully loaded with odorant) and then during a release phase when clean air was introduced into the capillary. Control experiments showed no significant isobutylthiazole interaction with the system and measured the contribution of water to odorant uptake. Isobutylthiazole uptake was then measured under different flow and concentration regimes, from which the stoichiometry of binding was calculated. The measured flow and structural characteristics in the model system were comparable with the situation in vivo. Uptake and release of isobutylthiazole with OBP3 during a simulated tidal flow regime showed strong uptake but little release of odorant due to the strong binding characteristics of this particular compound. The model system (dynamic biomimetic odorant-binding system) allowed monitoring of the dynamic binding and release of airborne odorants to OBP, and the resulting kinetic data provide an insight into the way OBP functions in vivo.

τ-Synopses is a system designed to provide a run-time environment for remote execution of various synopses. It enables easy registration of new synopses from remote platforms, after which the system can manage these synopses, including triggering their construction, rebuild and update, and invoking them for approximate query processing. The system captures and analyzes query workloads, enabling its registered synopses to significantly boost their effectiveness (efficiency, accuracy, confidence), by exploiting workload information for synopses construction and update. The system can also serve as a research platform for experimental evaluation and comparison of different synopses.

The covalent attachment of ubiquitin to cellular proteins is a powerful mechanism for controlling protein activity and localization1. Ubiquitination is a reversible modification promoted by ubiquitin ligases and antagonized by deubiquitinating proteases2. Ubiquitin-dependent mechanisms regulate many important processes including cell-cycle progression, apoptosis and transcriptional regulation3. Here we show that ubiquitin-dependent mechanisms regulate synaptic development at the Drosophila neuromuscular junction (NMJ). Neuronal overexpression of the deubiquitinating protease fat facets4 leads to a profound disruption of synaptic growth control; there is a large increase in the number of synaptic boutons, an elaboration of the synaptic branching pattern, and a disruption of synaptic function. Antagonizing the ubiquitination pathway in neurons by expression of the yeast deubiquitinating protease UBP2 (ref. 5) also produces synaptic overgrowth and dysfunction. Genetic interactions between fat facets and highwire6, a negative regulator of synaptic growth that has structural homology to a family of ubiquitin ligases, suggest that synaptic development may be controlled by the balance between positive and negative regulators of ubiquitination.

Inhaled adrenergics and steroids are the main agents used in acute asthma. Dosing recommendations for adrenergics, while generally becoming more aggressive, lack prospective validation. A double blind, randomized trial of two regimens of nebulized metaproterenol was conducted in patients presenting to an Emergency Department with an acute asthma exacerbation. Asthmatics age 16-55, with no other cardio-pulmonary disease, presenting with peak expiratory flow rate (PEFR)30% of predicted and greater than 80 L/m were enrolled. All patients received 125 mg of methylprednisolone and theophylline, if needed, to reach therapeutic levels. The experimental group received 0.3 cc metaproterenol in 2.5 cc of saline at times 0, 20", 40", 1', 2', 3', 4', 5', 6', and 7'. The control group received metaproterenol at times 0, 1 hr, and hours 3, 5, and 7. Placebo was given to control group patients at 20", 40", 2', 4', and 6'. PEFR and vital signs were measured 10 min after each treatment. Study end points included discharge upon reaching set criteria or admission if patients were not discharged following the hour 7 treatment.Seventy one patients were enrolled, 40 in experimental group and 31 in the control group. The group characteristics did not differ at entry in any significant way, and the groups began with mean expected PEFR of 23.4% and 24.5%, respectively. There were no significant differences at any point in PEFR outcomes, time to discharge, or admission rate. The experimental group showed a greater increase in pulse rate and a reduced diastolic blood pressure at 20, 40 and 60 min. The experimental group had a 12- and 8-fold increase in the risk of a pulse rate140 at 40 and 60 min, respectively. This group also had two moderate complications, both near the 60-minute mark. These were an induction of atrial fibrillation in one patient and ischemic electrocardiographic changes in another.Three treatments in the first hour, and hourly thereafter showed no benefit over treatments initially, at one hour, and every other hour in acute, moderate, or severe exacerbation of asthma. Side effects were markedly increased in the control group. Such dosing should not be recommended as routine therapy.

The biochemical pathway leading to secretion of thyroid hormones depends on iodide transport, iodide oxidation, and binding of tyrosyl residues present in thyroglobulin (Tg). This requires a normal thyroid peroxidase (TPO), T2O2 generating system, and supply of iodide and Tg. Abnormalities of this process will be reviewed. Patients with defects of hormone synthesis typically display a compensatory growth of the thyroid and hypothyroidism or cretinism depending on the severity and time of onset of the thyroid hormone deficiency. These disorders are probably all congenital, however, the underlying genetics is not fully understood.
