Introduction L’asthme est une maladie inflammatoire chronique des voies aeriennes, ayant des repercussions sur la vie scolaire, psychologique et socioprofessionnelle. Ces difficultes sont plus frequentes chez l’adolescent asthmatique. Methodes Etude retrospective de janvier 2015 a decembre 2018, portant sur 282 patients asthmatiques dont 65 adolescents, suivis a la consultation d’allergologie du CHU Ibn-Rochd de Casablanca. Resultats Les adolescents representaient 23 % de l’ensemble des patients asthmatiques dont 25 % des cas sont adresses par la pediatrie. La moyenne d’âge etait de 17 ans avec des extremes de12 a 19 ans. Sans predominance de sexe. Un antecedent d’hospitalisation en reanimation pour une exacerbation severe d’asthme etait retrouve dans 14 % des cas. L’asthme est classe stade I dans 12 % des cas, stade II dans 34 % des cas, stade III dans 49 % des cas et stade IV dans 5 % des cas apres une prise en charge therapeutique bien adaptee. L’asthme etait controle dans 41 % des cas, partiellement controle dans 49 % des cas et non controle dans 10 % des cas. Le deni de la maladie et la non-observance du traitement sont les principales causes du non-controle. Conclusion A travers cette etude, il parait que l’asthme est plus difficile a controler chez les adolescents, d’ou l’interet d’une bonne education et d’une therapie psycho-comportementale et d’un suivi adequat.

3. Sarcoïdose thoracique atypique : aspect radio-clinique

Introduction La sarcoidose est une maladie granulomateuse systemique de cause inconnue ayant pour localisations privilegiees le poumon et le systeme lymphatique. Certaines presentations sont atypiques, rares et trompeuses. Methodes Nous rapportons 19 cas de sarcoidose thoracique a presentation radio-clinique et endoscopique atypique. Resultats La moyenne d’âge etait de 47,7 ans avec une predominance masculine de 54 % des cas. Le tableau clinique atypique etait represente par les hemoptysies dans trois cas, la douleur thoracique dans deux cas et l’alteration de l’etat general dans trois cas. L’aspect radiologique atypique etait des images en lâcher de ballons dans trois cas, des opacites alveolaires bilaterales et des opacites excavees dans deux cas chacune, des adenopathies mediastinales unilaterales calcifiees, une pleuresie et une opacite d’allure tumorale a projection hilaire dans un cas chacun. La presentation endoscopique atypique etait une stenose d’allure tumorale dans un cas. Le diagnostic etait confirme par les biopsies bronchiques etagees dans six cas, la biopsie ganglionnaire peripherique dans trois cas, la biopsie d’un nodule pulmonaire sous thoracoscopie dans deux cas, la biopsie transbronchique et la biopsie d’une granulation du cavum dans un cas chacun. Chez un patient, il s’agissait d’une association tuberculose-sarcoidose. Le traitement etait base sur la corticotherapie systemique dans huit cas, associee a la corticotherapie nasale et au traitement antibacillaire dans un cas chacun. Le traitement adjuvant etait associe dans tous les cas. L’evolution etait bonne dans la majorite des cas. Conclusion La sarcoidose thoracique peut prendre des formes atypiques qui peuvent etre meconnues par le praticien d’ou la necessite d’y penser. En cas de doute, la confirmation histologique s’impose.

5. Syndrome d’apnée du sommeil chez la femme

Introduction Le syndrome d’apnees obstructives au cours du sommeil (SAOS) est une affection qui peut atteindre 2 % des femmes et 4 % des hommes de la population generale d’âge compris entre 30 et 60 ans. Les complications cardiaques et cerebrovasculaires associees a cette affection sont a l’origine d’une importante morbi-mortalite accrue qui en fait un veritable probleme de sante publique. Methodes Etude retrospective etalee sur 16 ans portant sur 315 observations de patients dont 215 femmes. Colliges au service des maladies respiratoires du CHU Ibn Rochd de Casablanca. Duree De 2006 a 2016, score Epworth et Berlin, examen clinique minutieux et polygraphie ventilatoire. Tous les dossiers sont traites sur Excel. Resultats Nous rapportons une etude retrospective menee au service des maladies respiratoires du CHU Ibn Rochd de Casablanca, portant sur 315 cas adresses pour suspicion de SAOS dont 215 femmes. La moyenne d’âge etait de 54 ans avec des extremes de 13 a 88 ans. Dans les antecedents, on a note une hypertension arterielle dans 60 % des cas, une dyslipidemie dans 30 % des cas, un diabete dans 25 % des cas, un asthme et une rhinite dans 5 % des cas chacun, une acromegalie et une insuffisance corticosurrenalienne dans 3 cas chacun et une patiente etait trisomique. La symptomatologie clinique etait dominee par les ronflements nocturnes avec pauses respiratoires et hyper-somnolence diurne dans tous les cas. La nycturie est retrouvee dans 77 % des cas et l’enuresie dans 57 % des cas. L’examen clinique trouvait une obesite dans 70 % des cas et severe dans 43 %. La moyenne de l’indice apnee–hypopnee etait de 17 (extremes : 5–57). L’examen ORL retrouvait un retrognathisme dans 26 % des cas et une hypertrophie amygdalienne dans 32 % des cas. Le SAOS etait severe dans 60 % des cas. Une prise en charge nutritionnelle etait preconisee chez les patientes obeses et celles en surpoids, la pression positive continue etait instauree chez 23 % des patientes. Le traitement chirurgical etait preconise chez cinq patientes et l’orthese d’avancee mandibulaire chez sept patientes. Conclusion Les effets de l’obesite sont plus marques quand les graisses se repartissent au niveau abdominal ou se deposent au niveau des parois anterolaterales des voies aeriennes superieures. Chez la femme souffrante de SAOS, on note souvent une reduction de l’espace aerien posterieur, un allongement du palais mou et de la distance os hyoide–branche horizontale de la mandibule.

6. Clinical and radiologic profile of the pseudotumoral silicosis

The silicosis is the most frequent pneumoconiosis. The pseudo-tumor shape is rare. We bring back a retrospective study concerning 45 cases of pseudo-tumor silicosis brought together in the service of the respiratory diseases of the CHU IBN ROCHD of Casablanca over a period going from January,1997 till December,2014. It is about 45 men. The average age is 50 years. The smoking is found in 31 case (68, 8 %), with an average consumption of 24 Packet per year. A history of tuberculosis is found in 9 cases (20 %). Thirty one patients were well diggers (68, 8 %), ten were miners (22 %) and four were bricklayers (9 %). The clinical chart is dominated by the dyspnoea in 37 cases (82 %), the bronchial syndrome in 34 cases (75 %) and hemoptysy in 15 cases (33 %). The imaging found pseudo-tumor opacities in every case, with micronodule opacities in 30 cases (67 %), pneumothorax and pleurisy in a case each and mediastinal lymphadenopathy in 8 cases. The bronchoscopy showed spots of anthracoses in 31 cases (68, 9 %). The respiratory functional exploration showed a mixed ventilatory disorder in 28 cases (62 %) and restrictive likely in 6 cases (13 %). Bacilloscopy is positive for 5 patient9s (11 %). The transparietal draining biopsy shows a tuberculoid granuloma without caseous necrosis in 1 case. The treatment is based on the long-duration of bronchodilators in 37 cases (82 %), corticoids inhaled in 34 cases (75, 5 %), and Diuretics in 7 cases. The anti bacillary treatment is started in 5 cases of silico-tuberculosis. The evolution is good in 18 cases, still in 18 cases. Eight patient were lost sight and one patient died. The therapeutic care of this particular shape is essentially symptomatic, proving the importance of prevention.

Described in 1819 by Laennec, the dilation of bronchi is defined by the permanent and irreversible increase in bronchial caliber with destruction of the skeleton fibrocartilaginous .It is a frequent affection, that always raises a problem of care. We report a retrospective study of 294 cases of dilation of bronchi,brought together in the service of the University Hospital of respiratory diseases Ibn Rochd of Casablanca between January 2002 and January 2015. The mean age was 43 years (14-91 years), with female predominantly in 78% of the cases. The etiological investigation identified a medical history of pulmonary tuberculosis in 104 cases of respiratory infection and a medical history of childhood respiratory disease in 43 cases. The average deadline of consultation is 6 months. The clinical symptomatology were dominated by bronchorrhea in 267cas, dyspnea in 143 cases and hemoptysis in 112 cases. The thoracic imaging objectified dilation of bronchi in 189 cases, located in 100 of the cases and bilateral in 89of the cases. The dilations are cylindrical in 13% of the cases, cystic in 51% of the cases and varicose in 36% of the cases .using a Bronchoscopy, bronchial inflammation was found in 218 cases, as well as a foreign body and a budding tumor in two cases each. Probabilistic antibiotherapy and respiratory physiotherapy are indicated for all our patients. Only 21 patients were operated. The evolution was favorable in 98% of the cases. The dilation of bronchi represents a frequent pathology and a delicate management for the pulmonologist. The causes remains dominated, in our context, by the tuberculosis and the infantile respiratory disease, in spite of the vaccination and the antituberculous control program.

Introduction La tuberculose demeure encore un probleme de sante publique majeur au Maroc. La localisation medullaire de la tuberculose est rare, survenant dans un contexte de tuberculose multifocale ou miliaire tuberculeuse. Methodes (1) Etude retrospective : 42 cas de miliaire tuberculeuse colliges au service des Maladies Respiratoires du CHU Ibn Rochd. (2) Duree : septembre 2003–septembre 2015. Resultats Nous rapportons une etude retrospective portant sur 42 cas de miliaire tuberculeuse et qui ont tous beneficie d’une biopsie osteomedullaire (BOM), colliges au service des maladies respiratoires CHU Ibn Rochd de Casablanca entre septembre 2003 et septembre 2015. On a note une predominance feminine (62 % des cas). La moyenne d’âge etait de 38 ans. L’antecedent de tuberculose est retrouve dans six cas, un diabete et le post-partum dans deux cas chacun et une trisomie 21 dans un cas. Le tableau clinique est domine par l’alteration de l’etat general notee dans tous les cas, et la fievre dans 33 cas (82 %). Le telethorax a retrouve un aspect de miliaire dans tous les cas. Le bilan de dissemination a objective une atteinte cerebromeningee dans trois cas avec des tuberculomes cerebraux dans un cas, une pericardite dans deux cas, des adenopathies mediastinales et peripheriques dans six cas, et une atteinte retinienne dans cinq cas. L’hemogramme a montre une anemie dans cinq cas et une thrombopenie dans deux cas. La BOM, faite chez tous nos patients, a retrouve un granulome epithelio-gigantocellulaire avec necrose caseeuse dans huit cas, et sans necrose dans quatre cas. Le traitement antibacillaire entrepris etait a base de 2SRHZ/7RH dans 29 cas, de 2RHZE/4RH dans sept cas et de 2SRHZE/1RHZE/7RHE dans six cas. L’evolution etait bonne dans 32 cas, imprecise dans sept cas et fatale dans trois cas. La tuberculose medullaire est rare, sa prevalence au cours de la miliaire tuberculeuse est de 25 %. Conclusion Urgence diagnostique et therapeutique. Diagnostic : radiologique. Traitement : demarre generalement sur des elements de presomption. Pronostic : depend de la precocite de prise en charge.

10. Les différents aspects radiologiques de l’aspergillose pulmonaire

Introduction L’aspergillus est a l’origine de diverses atteintes respiratoires. L’imagerie joue un role essentiel dans la distinction entre ces differentes atteintes ( Fig. 1 ). Methodes Nous proposons une etude retrospective concernant 89 cas d’aspergillose pulmonaire hospitalises au service des maladies respiratoires du CHU Ibn Rochd de Casablanca entre janvier 2003 et aout 2015. Resultats La moyenne d’âge est 48 ans avec predominance masculine (59 %). On trouve le tabagisme chez 80 % des cas, le cannabisme dans 20 % des cas et un antecedent de tuberculose pulmonaire dans 96 % des cas. Les maitres symptomes sont le syndrome bronchique present dans 100 % des cas et les hemoptysies dans 90 % des cas. L’examen montre une denutrition chez 40 % des cas. La radiographie thoracique a montre une image en grelot dans 31 % des cas, un aspect de lobe ou de poumon detruit dans 29 % des cas, des images cavitaires dans 31 % des cas et des opacites rondes dans 17 % des cas. La TDM thoracique montre des images de mycetomes dans 47 % des cas, des cavitations dans 50 % des cas, un epaississement septal et pleural dans 30 % des cas, une bronchiolite diffuse dans 20 % des cas, des DDB proximales avec impactions mucoides dans 3 % des cas. Les diagnostics retenus sont des aspergillomes simples dans 50 % des cas, complexes dans 30 % des cas, d’aspergillose pulmonaire chronique necrosante dans 17 % des cas et aspergillose bronchopulmonaire allergique dans 3 % des cas. Conclusion Une bonne lecture de la TDM thoracique est essentielle pour differentier les formes de l’aspergillose respiratoires et guider la conduite therapeutique.

Introduction Les pneumopathies infiltrantes diffuses (PID) sont de mecanismes tres varies. La recherche d’une exposition professionnelle ou domestique est une etape essentielle de l’interrogatoire. Methodes Nous proposons une etude retrospective sur 54 cas de PID hospitalises au service des maladies respiratoires du CHU Ibn Rochd de Casablanca entre janvier 2013 et janvier 2016. Resultats La moyenne d’âge est 63 ans avec une predominance feminine. Les expositions les plus rencontrees sont celle a la poussiere de maison, chez 24 femmes aux foyers, cinq cas aux dejections d’oiseaux, quatre cas a l’amiante et quatre autres a la fumee de bois. La dyspnee et la toux seche ont domine la symptomatologie retrouvees dans 91 % et 58 % des cas respectivement. On a trouve des râles crepitants dans 71 % des cas. L’imagerie montre un aspect en rayon de miel dans 45 % des cas et un aspect en verre depoli dans 29 % des cas. Les biopsies bronchiques ont revele un granulome epithelio-gigantocellulaire dans 8 % des cas. Les biopsies pulmonaires ont revele un seul aspect de fibrose pulmonaire idiopathique. Une biopsie salivaire montre un granulome dans un seul cas. Le lavage broncho-alveolaire etait macrophagique dans 80 % des cas. L’echographie du cœur retrouve 2 cas d’HTAP. Le diagnostic d’asbestose a ete retenu chez 8 % des patients et dans 44 % des cas. Conclusion On a trouve des expositions au foin moisi, aux dejections d’oiseaux, a la fumee de bois, aux hydrocarbures et a la poussiere de maison sans pouvoir confirmer le diagnostic, ce qui montre le role considerable de l’exposition professionnelle et domestique dans la survenue des pneumopathies infiltrantes diffuses et donc l’interet de leur recherche, etape essentielle dans la prise en charge.

Introduction Le pneumothorax est une pathologie frequente. Plusieurs facteurs favorisants sont incrimines dans sa physiopathologie. L’interet de notre etude est de determiner l’origine professionnelle du pneumothorax. Methodes Nous proposons une etude retrospective etalee de janvier 2013 a janvier 2016 et concernant 128 cas hospitalises au service des maladies respiratoires du CHU Ibn Rochd de Casablanca. Resultats La moyenne d’âge etait de 49 ans avec predominance masculine. Le tabagisme actif est retrouve dans 90 % des cas. Le metier de macon etait le plus retrouve, dans 8 % cas, suivi du metier de porteurs de charge et d’agriculteur qui representent 5,4 % des patients chacun, de peintre de facade et de soudeur et de chanteur dans 2,7 % des cas chacun. Une exposition a l’amiante est retrouvee chez 16,2 % des cas, la silice dans 8 % cas, les particules de metaux et l’isocyanate dans 2,7 %. Une douleur en coup de poignard etait presente dans 87 % des cas. Un syndrome d’epanchement aerique etait present dans 85 % des cas. La radio-thorax a confirme le diagnostic dans tous les cas. Cinquante et un pour cent des patients ont beneficie d’exsufflation et 62 % d’un drainage (dont 22 % apres echec d’exsufflation) et le repos seul chez 9 %. L’evolution est bonne dans 89 % des cas et 5 % des patients ont eu recours a la chirurgie. Nous remarquons le nombre eleve de certains metiers comme celui du macon, des porteurs de charge, du peintre, du soudeur et de l’agriculteur avec un taux eleve d’exposition a l’amiante et a la silice. Conclusion Des etudes plus approfondies doivent s’interesser a ce sujet afin etablir un lien physiopathologie entre la survenue des pneumothorax et chaque agent suspecte afin de developper des mesures de prevention et d’integrer cette pathologie dans la liste d’indemnisation.

Introduction Les pleuresies hemorragiques sont assez frequentes en pratique courante en pneumologie. Dans le but de decrire les aspects epidemiologiques, cliniques et etiologiques de ces pleuresies. Methodes Etude retrospective etalee sur 12 ans Portant sur 287 observations de patients. Resultats Il s’agissait de 186 hommes et 101 femmes. La moyenne d’âge de nos patients etait de 54 ans (19–92). Le delai moyen entre le debut de la symptomatologie et la consultation etait de 80 jours. Le tabagisme actif etait note dans 49 % des cas et un antecedent de neoplasme chez 21 % des cas. La douleur thoracique, la toux seche et la dyspnee sont retrouvees chez tous les patients. L’alteration de l’etat general est notee dans 87 % des cas. La radiographie thoracique a montre une pleuresie de grande abondance dans 72 % des cas, de moyenne abondance dans 21 % des cas et enkystee et bilaterale dans neuf cas chacune. Le liquide est sero-hematique dans 75 % des cas, franchement hemorragique et incoagulable dans 71 cas. La ponction-biopsie pleurale a confirme le diagnostic etiologique dans 91 % des cas, le cytodiagnostic du liquide pleural dans 16 % des cas et la biopsie sous thoracoscopie dans 24 cas (8 %). Les etiologies etaient dominees par l’origine maligne dans 203 cas (70 %), la tuberculose dans 15 % des cas, cardiaque dans 1 % des cas et renale dans 1 % des cas. Dans 12 % des cas, l’etiologie etait indeterminee. Conclusion Les pleuresies hemorragiques viennent en 2 e position parmi les pleuresies. L’origine maligne metastatique est la cause la plus frequente. La thoracoscopie diagnostique trouve une place preponderante dans la confirmation du diagnostic.

14. Évaluation de l’état nutritionnel chez les patients porteurs de bronchopneumopathie chronique obstructive

La denutrition est frequente chez les patients atteints de bronchopneumopathie chronique obstructive (BPCO), sa prevalence varie de 19 % a 45 %. Elle est de type proteino-calorique et resulte essentiellement d’un hyper-metabolisme de repos du a la consommation d’oxygene des muscles respiratoires. Nous avons mene une etude retrospective s’etalant d’aout 2011 jusqu’a fevrier 2014 portant sur 59 patients suivis au CHU Ibn Rochd de Casablanca pour bronchopneumopathie obstructive chronique (BPCO) Il s’agit de 57 hommes et de 2 femmes, dont la moyenne d’âge est de 58,5 ans. Tous les patients sont tabagiques (consommation moyenne : 38,7 PA). La dyspnee est retrouvee dans 94 % des cas, la toux avec expectoration dans 76 % des cas. L’examen clinique trouve un IMC moyen a 21 kg/m 2 (extremes : 12–27,5 kg/m 2 ). La denutrition est retrouvee dans 27 %, elle est severe avec un IMC 2 dans 30 % des cas. La NFS a retrouve une anemie hypochrome microcytaire dans 36 %et une polyglobulie dans 26 % des cas. Le taux moyen de l’albuminemie est de 32 g/L (extremes : 20–45 g/L). La BPCO est classee stade I dans 14 %, stade II dans 30 %, stade III dans 32 % et stade IV dans 24 % des cas. Le traitement s’est base sur une alimentation hypercalorique fractionnee chez les patients denutris, recours aux bronchodilatateurs a liberation prolongee seuls dans 14 % des cas, associes aux corticoides inhales et aux xanthiniques dans 86 % des cas. Trois patients sont sous oxygenotherapie de longue duree. Nous insistons a travers ce travail sur la frequence de la denutrition chez les patients BPCO, et nous rappelons la disponibilite de moyens simples et peu couteux pour son depistage.

Introduction La bronchopneumopathie chronique obstructive (BPCO) est desormais consideree comme une maladie systemique. Une de ses principales complications est l’apparition d’une hypertension pulmonaire (HTP). L’HTP est definie par une PAP moyenne superieure a 25 mmHg au repos. Methodes Etude retrospective de janvier 2008 a janvier 2015. Population de 335 patients porteurs de BPCO. Resultats Parmi ces patients, 97 avaient une hypertension pulmonaire confirmee a l’echocardiographie, soit une prevalence de 29 %. La moyenne d’âge etait 57,3 ans, avec une nette predominance masculine (97 %). L’intoxication tabagique etait notee dans 97 % des cas. L’examen clinique avait note des signes d’HTP dans 29 % des cas, et des signes d’insuffisance cardiaque droite dans 18 % des cas. La gazometrie realisee chez 25 % de nos malades avait note une hypoxie dans 9 % des cas, associee a une hypercapnie dans 6 % des cas. La PAP moyenne retrouvee chez les malades etudies etait de 42 mmHg avec un intervalle allant de 32 mmHg a 87 mmHg. L’HTP etait legere dans 7 % des cas, moderee dans 38 % des cas et severe dans 55 % des cas. L’evolution etait favorable dans 60 % des cas et marquee par la survenue d’exacerbations annuelles frequentes dans 18 % des cas. Conclusion L’HTP chez nos patients porteurs de BPCO est frequente et severe. Ceci peut etre explique par le diagnostic de BPCO souvent a un stade tardif et les difficultes de prise en charge dans notre contexte. Nul doute qu’une meilleure prise en charge de l’HTP ameliorerait le pronostic de nos patients BPCO.

Introduction Les DDB : augmentation permanente et irreversible du calibre bronchique, associee a une alteration de leurs fonctions. La tuberculose : domine l’etiologie dans notre contexte. Methodes Etude retrospective : 294 cas de DDB colliges au service des maladies respiratoires du CHU Ibn Rochd Casablanca, Maroc. Sur une periode de 13 ans etalee du janvier 2002 a janvier 2015. Resultats Decrite en 1819 par Laennec, la dilatation des bronches (DDB) se definit par l’augmentation permanente et irreversible du calibre des bronches avec destruction de la charpente fibrocartilagineuse. C’est une affection frequente, qui pose toujours un probleme de prise en charge. Nous rapportons une etude retrospective concernant 294 cas de dilatation des bronches. La moyenne d’âge etait de 43 ans, avec une predominance feminine dans 78 % des cas. L’interrogatoire a permis d’identifier des antecedents de tuberculose pulmonaire dans 35 % des cas et d’infections respiratoires dans l’enfance dans 14 % des cas. La symptomatologie clinique etait dominee par la bronchorrhee dans 90 % des cas, la dyspnee dans 48 % des cas et l’hemoptysie dans 38 % des cas. L’imagerie thoracique a objective des dilatations de bronches diffuses dans 64 % des cas, localisees dans 34 % des cas et bilaterales dans 30 % des cas. Les dilatations sont cylindriques dans 13 % des cas, kystiques dans 51 % et variqueuses dans 36 % des cas. L’antibiotherapie probabiliste et la kinesitherapie respiratoire sont indiquees chez tous nos patients. Seuls 7 % des patients sont operes. L’evolution etait favorable dans 98 % des cas. La dilatation des bronches represente une pathologie frequente. Ses etiologies restent dominees, dans notre contexte, par la tuberculose et les maladies respiratoires infantiles. Conclusion Interet de la bronchoscopie devant toute DDB localisee a la recherche d’une cause locale. La tuberculose : l’etiologie principale dans notre contexte. Les DDB secondaires a la rougeole ou la coqueluche : encore frequentes malgre la vaccination.

17. Carcinome bronchogénique et maladie thromboembolique

La maladie thromboembolique veineuse (MTEV) est la survenue d’une thrombose veineuse profonde et/ou d’une embolie pulmonaire, survient en moyenne chez 15 a 20 % des patients atteints de cancer et constitue une des principales causes de deces chez ces patients. A travers une etude retrospective etalee de janvier 2010 a aout 2014, nous avons evalue la survenue d’une MTEV chez 18 patients atteints d’un carcinome bronchogenique. La prevalence de la MTEV dans notre etude etait de 6,2 %, la moyenne d’âge de nos patients etait de 58,9 ans avec un sex-ratio H/F de 17/1. Le tabagisme etait note dans 16 cas. La symptomatologie clinique etait dominee par la dyspnee et la douleur thoracique. L’examen clinique avait trouve un syndrome cave superieur a sa phase d’etat dans 9 cas. La TDM thoracique a montre la presence d’un processus tumoral hilaire dans 9 cas, mediastinopulmonaire dans 6 cas, une condensation alveolaire avec des adenopathies mediastinales dans 2 cas et lobaire superieur droit dans 1 cas. La thrombose etait localisee au niveau de la veine humerale dans 5 cas, la veine basilique dans 3 cas, la veine cave inferieure, la veine poplitee, la veine jugulaire interne et la veine renale gauche dans un cas chacune. Le diagnostic de thrombose vasculaire a ete fait avant la confirmation histologique chez tous nos patients. Le type histologique etait domine par le carcinome a petites cellules et l’adenocarcinome. Soixante pour cent des patients etaient classes stade IV, six patients stade III B et un patient stade III A. Tous nos patients ont ete mis sous traitement anticoagulant. L’association entre maladie thromboembolique et cancer bronchogenique a un caractere pejoratif, vu le risque de mortalite elevee.

L’adenocarcinome bronchique primitif est une entite particuliere des tumeurs bronchiques caracterisee par son pronostic pejoratif et son taux d’incidence augmentant, devenant le type histologique le plus frequent. Nous rapportons une etude retrospective portant sur 71 cas d’adenocarcinome bronchique primitif colliges au service des maladies respiratoires du CHU Ibn Rochd de Casablanca sur une duree de quatre ans (2011–2014). La moyenne d’âge etait de 42 ans (40–80 ans), avec predominance masculine dans 86 % des cas, le tabagisme etait retrouve dans 89 % des cas. Le delai moyen de diagnostic etait de 18 jours. La symptomatologie etait faite de douleur thoracique dans 79 % des cas, dyspnee dans 61 % des cas et alteration de l’etat general dans 67 % des cas. Le scanner thoracique avait objective un processus du lobe superieur droit dans 35 % des cas, du lobe superieur gauche dans 21 % des cas, du lobe inferieur droit et du lobe inferieur gauche dans 4 % des cas chacun, et un processus mediastinopulmonaire dans 12 % des cas. La bronchoscopie avait retrouve une stenose bronchique dans 31 % des cas, un bourgeon endobronchique dans 29 % des cas, un epaississement des eperons dans 22 % des cas, une infiltration tumorale de la muqueuse bronchique dans 18 % des cas et une compression extrinseque dans 7 % des cas. Le diagnostic etait confirme par la ponction biopsie transparietale dans 54 % des cas et les biopsies bronchiques dans 46 % des cas. La plupart des patients etaient classes stade IV (78 %). Le traitement etait palliatif dans 78 % des cas. L’adenocarcinome est une forme histologique du carcinome bronchique primitif severe et de plus en plus frequente, vu l’elevation croissante de l’incidence du tabagisme.

Nuclear Experiment - Abstract
ALICE is a general-purpose heavy-ion experiment designed to study the physics of strongly interacting matter and the quark-gluon plasma in nucleus-nucleus collisions at the LHC. It currently involves more than 900 physicists and senior engineers, from both the nuclear and high-energy physics sectors, from over 90 institutions in about 30 countries. The ALICE detector is designed to cope with the highest particle multiplicities above those anticipated for Pb-Pb collisions (dN(ch)/dy up to 8000) and it will be operational at the start-up of the LHC. In addition to heavy systems, the ALICE Collaboration will study collisions of lower-mass ions, which are a means of varying the energy density, and protons (both pp and pA), which primarily provide reference data for the nucleus-nucleus collisions. In addition, the pp data will allow for a number of genuine pp physics studies. The detailed design of the different detector systems has been laid down in a number of Technical Design Reports issued between mid-1998 and the end of 2004. The experiment is currently under construction and will be ready for data taking with both proton and heavy-ion beams at the start-up of the LHC. Since the comprehensive information on detector and physics performance was last published in the ALICE Technical Proposal in 1996, the detector, as well as simulation, reconstruction and analysis software have undergone significant development. The Physics Performance Report (PPR) provides an updated and comprehensive summary of the performance of the various ALICE subsystems, including updates to the Technical Design Reports, as appropriate. The PPR is divided into two volumes. Volume I, published in 2004 (CERN/LHCC 2003-049, ALICE Collaboration 2004 J. Phys. G: Nucl. Part. Phys. 30 1517-1763), contains in four chapters a short theoretical overview and an extensive reference list concerning the physics topics of interest to ALICE, the experimental conditions at the LHC, a short summary and update of the subsystem designs, and a description of the offline framework and Monte Carlo event generators. The present volume, Volume II, contains the majority of the information relevant to the physics performance in proton-proton, proton-nucleus, and nucleus-nucleus collisions. Following an introductory overview, Chapter 5 describes the combined detector performance and the event reconstruction procedures, based on detailed simulations of the individual subsystems. Chapter 6 describes the analysis and physics reach for a representative sample of physics observables, from global event characteristics to hard processes.

Previously published data on low-pT π+/K+/p−p interactions at 250 GeV/c are used to analyze the rapidity charge distribution and hadron structure function for the projectile hadrons. It is shown that the rapidity charge distribution for projectile hadrons can be approximated by a Gauss distribution, and their structure functions are found.

We have studied the reactionspp→ppπ+π-,K+p→K+pπ+π−π, π+p→ π+,pπ+π− and π−p →π+π− at 147 GeV/c using the 30-inch Fermilab hybrid system. All four reactions were detected with the same apparatus and analyzed in the same way. The energy dependence of the channel cross section was found to beAp−0.6+B for thepp reaction andAp−1+B for the other three. About 90% of the cross section at 147 GeV/c can be accounted for by either beam or target diffraction. Some of the remaining cross section may come from double Pomeron exchange reactions which we tried to isolate. We have tested the hypothesis of a factorizable Pomeron and our data indicates a violation of this hypothesis. We show that the 3π mass enhancement in the mass region 1.2–1.4 GeV is diffractively produced in the π± beam reactions. Fourprong, four-constraint and six-prong, four-constraint cross sections are reported.

We have studied the inclusive production of K/sup asteriskplus-or-minus/(890) and Y/sup asteriskplus-or-minus/(1385) in pp, ..pi../sup +/p, and K/sup +/p interactions at 147 GeV/c. The experiment used the Fermilab 30-inch hydrogen bubble chamber with the hybrid spectrometer system. Results are based on a sample of 1916 observed K/sub S/ and 932 observed ..lambda... Inclusive cross sections are given for K/sup asteriskplus-or-minus/ and Y/sup asteriskplus-or-minus/ production from the three beams, and comparisons are made with experiments at other energies. Feynman-x and transverse-momentum-squared distributions are also calculated. The results suggest that the K/sup asterisk-/ is entirely produced in the central region, while the K/sup asterisk+/ includes a component from beam fragmentation. Comparisons are made with the additive quark model.

The gross features of the data are reviewed and the momentum tensor components, 〈S〉, and 〈T〉 are studied. Evidence for coplanarity effects is observed. A probabilistic method to search for jets is proposed. The jets found by this method are seen to be similar to highp T jets frompp interactionse + e − annihilations andvp interactions. The effect of subtracting the leading particles and the effect of lowp T diffractive and non diffractive components are investigated.

The Fermilab hybrid 30-in. bubble-chamber spectrometer was exposed to a tagged 147-GeV/c positive beam containing ..pi../sup +/, K/sup +/, and p. A sample of 3003 K/sup +/p, 19 410 pp, and 20 745 ..pi../sup +/p interactions is used to derive sigma/sub n/, , f/sub 2//sup tsc/c, and /D for each beam particle. These values are compared to values obtained at other, mostly lower, beam momenta. The overall dependence of on E/sub a/, the available center-of-mass energy, for these three reactions as well as ..pi../sup -/p and pp interactions has been determined.

Cross-section, angular distribution and polarization measurements are presented at six incident π+ energies for the reaction π+p → K+Σ+. Results at already studied energies are compatible with existing data and roughly agree with predictions of partial-waves analyses. However, new results around 1.920 GeV are in disagreement with such predictions, casting some doubt on the determination of decay branching ratios of the T = 3 2 isobars around this mass into K+Σ+.

In an experiment with the 30-inch Hybrid Spectrometer at Fermilab we have obtained the inclusive and semi-inclusive production cross sections of the ϱ0 meson using a conventional background subtraction technique. Production cross sections for the ϱ0 are derived as a function of the Feynman scaling variablex, and the transverse variablespt2 andEt=(pt2+M2)1/2. The longitudinal distributions are compared with the (1−x) dependence of the proton and meson valence quark structure functions, using various forms of recombination and fragmentation models. The transverse distributions are compared with thermodynamic models. We give density matrix elements for the ϱ0 production from pions in the extreme forward region.

The ratio of hadronic π+/π− production has been studied in the target fragmentation region of π+ p, π− p,K + p andp p collisions at 147 GeV/c beam momentum. The difference between this ratio in the π+ p and π− p data is less than at lower energies but still present. No difference exists anymore for the π+/π− ratio at this energy betweenK + p andpp collisions. Exclusion of diffraction dissociation and resonance production does not change the main conclusions. The energy dependence of the ratio for π+ p and π− p collisions gives evidence for a universal asymptotic ratio for all beams. Thep t -dependence at low-p t is in agreement with the expectation from geometrical correlation of the valence quarks.

Eleven neutral and nine charged decays of charmed particles have been observed in a sample of 205 000 hadronic interactions in a 1.2-million-picture exposure of the SLAC Hybrid Facility bubble chamber to a 20-GeV/c backward-scattered laser beam. The charged and neutral lifetimes were determined to be ${8.2}_{\ensuremath{-}2.5}^{+4.5}$\ifmmode\times\else\texttimes\fi{}${10}^{\ensuremath{-}13}$ and ${6.7}_{\ensuremath{-}2.0}^{+3.5}$\ifmmode\times\else\texttimes\fi{}${10}^{\ensuremath{-}13}$ sec, respectively, with a ratio of ${1.2}_{\ensuremath{-}0.5}^{0.9}$.

Results are presented from a study of inclusive neutral strange particle production by a 147 GeV/c tagged π+/K+/p beam in the Fermilab 30-inch hydrogen bubble chamber. The experiment made use of the proportional hybrid spectrometer system. Results are based on 995 KS0, 485 Λ, and 83 Λ found in a sample of 132 000 pictures. Cross sections are given for inclusive production of these particles by each of the three beam particles, and comparisons are made with measurements at other energies. Topological cross sections are also calculated, and KNO multiplicity scaling is investigated. Distributions are presented of invariant cross sections as functions of the Feynman scaling variable x and c.m. rapidity y. The transverse momentum-squared distributions with their fitted slopes are also given. Comparisons are made of the production characteristics for the three beam types.

The energy dependence of the average of the charged multiplicity and its dispersion in π + /K + /p interaction on protons at 147 GeV/ c is found to be the same as in e + e − annihilations if an “effective energy” variable is used instead of the total energy. The effective energy S eff is defined as the invariant mass of all secondaries left after the two leading particles have been removed. Fitting the expression aS eff b to the average charge multiplicity 〈 n ch 〉, we find the power b to be in good agreement with the value of 0.25 predicted by Fermi's statistical model and by Landau's hydrodynamical model.

An analysis is presented of the production of jets in low-ptπ±p,k+p,pp collisions at 147 GeV/c collected at Fermilab in the rapid cycling hydrogen bubble chamber hybrid spectrometer. It is shown that selected events are more planar than predicted by a cylindrical phase space, but that 3-jet and 4-jet like structures seen in the momentum flows are reasonably reproduced by the cylindrical phase space events subjected to the same mathematical manipulation required to build momentum flows.

Cross sections for various channels in 3 prong + V 0 final states of K − n interactions are obtained at 8.25 GeV/ c . An energy dependence study of the quasi two-body reactions Σ − (1385) + vector meson and ΛB − seem to imply the presence of cuts in the Regge exchange formalism, whereas the reaction Δ(1236) K ∗ (890) does not require such cuts. Upper limits of 1 and 2 μb are found for the reactions K − n → Λ A 1 and K − n → Λ A 2 .

Forty-seven charm events have been observed in an exposure of the SLAC Hybrid Facility bubble chamber to a 20-GeV backward-scattered laser beam. Thirty-seven events survive all the necessary cuts imposed. Based on this number the total charm cross section is calculated to be ${63}_{\ensuremath{-}28}^{+33}$ nb.

39. Dose variations for biopsy, puncture and drainage under CT guidance: A national survey in 1709 patients.

- Abstract
Background: A nation-wide study recently published the dose reference levels for the main CT-guided interventional procedures in 5001 patients. We assessed the impact of patient's age, sex and targeted organ on the patient dose during thoracic and abdominopelvic biopsies and punctures/drainages., Patients and Methods: Data were extracted from the previous nationwide study. All biopsies, punctures and drainages for thoracic or abdominopelvic locations performed between January 2017 and June 2019 in all participating centers were included in the study. Multivariable analyses were carried out using a linear regression of the dose-length product (DLP) log, adjusted to age, sex, anatomical location, number of helical acquisitions and inclusion center., Results: Of the 5001 patients of the initial study, 2383 benefited from thoracic or abdominopelvic procedures, including 674 percutaneous destructions excluded. 1709 patients (44 centers), 1045 men, 664 women, median age 64.4 ± 14.0 years were included. The mean DLP was 751.2 ± 642.7mGy.cm. It was significantly higher in men than women ( p = 0.0005) and higher for abdominopelvic procedures than for thoracic locations ( p <0.0001)., Conclusion: Doses delivered to patients for abdominal and thoracic biopsies and punctures/drainages performed under CT guidance depend on gender and location. 40. Retrospective Analysis of Doses Delivered during Embolization Procedures over the Last 10 Years.

- Abstract
Background: This study aimed to retrospectively analyze dosimetric indicators recorded since 2012 for thoracic, abdominal or pelvic embolizations to evaluate the contribution of new tools and technologies in dose reduction. Methods: Dosimetric indicators (dose area product (DAP) and air kerma (AK)) from 1449 embolizations were retrospectively reviewed from August 2012 to March 2022. A total of 1089 embolizations were performed in an older fixed C-Arm system (A1), 222 in a newer fixed C-Arm system (A2) and 138 in a 4DCT system (A3). The embolization procedures were gathered to compare A1, A2 and A3. Results: DAP were significantly lower with A2 compared to A1 for all procedures (median −50% ± 5%, p < 0.05), except for uterine elective embolizations and gonadal vein embolization. The DAP values were significantly lower with A3 than with A1 (p < 0.001). CT scan was used for guidance in 90% of embolization procedures. Conclusions: The last C-Arm technology allowed a median reduction of 50% of the X-ray dose. The implementation of a CT scan inside the IR room allowed for more precise 3D-guidance with no increase of the dose delivered.

Purpose: The purpose of this study was to assess the impact of advanced modeled iterative reconstruction (ADMIRE) algorithm and dose levels on the accuracy of Hounsfield unit (HU) measurement, image noise and contrast-to-noise ratio (CNR) in virtual monochromatic images (VMIs) with low iodine concentrations, and the accuracy of iodine quantification., Materials and Methods: A CT phantom was scanned with dual-source CT using abdomen-pelvis examination parameters at four dose levels: 5, 8, 11 and 20 mGy. Images were reconstructed using filtered-back projection (FBP) and ADMIRE levels 3 and 5 (A3-A5). HU accuracy was assessed calculating the root-mean-square deviation (RMSD
HU ). Image noise and CNR were computed on VMIs at 40/50/60/70 keV for 4 iodine inserts with 0.5, 1, 2 and 5 mg/mL concentrations. Accuracy of iodine quantification was assessed by the RMSDiodine and iodine bias (IB)., Results: The RMSDHU decreased significantly as the dose levels increased compared to 5 mGy, except for 8 mGy with A3 (P = 0.380) and with A5 level (P = 0.945). Noise increased by 63.0 ± 3.0 (standard deviation [SD])% from 20 mGy to 5 mGy. Noise decreased significantly by -53.8 ± 0.9 (SD) % with A5 compared to FBP. The CNR decreased by -43.1 ± 6.5 (SD)% from 20 mGy to 5 mGy. It increased using ADMIRE, and as the ADMIRE levels increased. Purpose: To compare the organ-dose and effective-dose (E) delivered to the patient during percutaneous vertebroplasty (PVP) of one thoracic or lumbar vertebra performed under CT guidance or using a fixed C-arm., Methods: Consecutive adult patients undergoing PVP of one vertebra under CT-guidance, with optimized protocol and training of physicians, or using a fixed C-arm were retrospectively included from January 2016 to June 2017. Organ-doses were computed on 16 organs using CT Expo 2.4 software for the CT procedures and PCXMC 2.0 for the fixed C-arm procedures. E was also computed with both software. Dosimetric values per anatomic locations for all procedures were compared using the paired Mann-Whitney-Wilcoxon test., Results: In total, 73 patients were analysed (27 men and 46 women, mean age 78 ± 10 years) among whom 35 (48%) underwent PVP under CT guidance and 38 (52%) PVP using a fixed C-arm. The median E was 11.31 [6.54; 15.82] mSv for all PVPs performed under CT guidance and 5.58 [3.33; 8.71] mSv for fixed C-arm and the differences was significant (p<0.001). For lumbar PVP, the organ doses of stomach, liver and colon were significantly higher with CT-scan than with the fixed C-arm: 97% (p=0.02); 21% (p=0.099) and 375% (p=0.002), respectively. For thoracic PVP, the lung organ dose was significantly higher with CT-scan than with the fixed C-arm (127%; p<0.001) and the oesophagus organ doses were not significantly different (p = 0.626)., Conclusion: This study showed that the E and the organ dose on directly exposed organs were both higher for PVP performed under CT-guidance than with the fixed C-arm., (Copyright © 2021 Associazione Italiana di Fisica Medica. - Abstract
Background: Behavioral health care providers aim to see their clients achieve lifelong recovery from substance use disorders (SUDs). To do so, they must accurately assess incoming resources and barriers to recovery to identify areas of need for their clients. A recovery capital scale (RCS) is one way to identify these resources and barriers. However, there is limited evidence that RCSs are invariant across different demographics., Objectives: To evaluate whether a short 10-item RCS (SRCS-10) is invariant across gender (male versus female), ethnicity (white versus African American, Hispanic, and other racial identities) and sexual orientation (heterosexual versus non-heterosexual)., Methods: The SRCS-10 is a shortened version of White's original unpublished 35-item RCS scale that was matched on items similar to the Brief Assessment of Recovery Capital (BARC-10). We used confirmatory factor analysis (CFA) to confirm unidimensionality and then conducted measurement invariance tests., Results: This study included 2751 females (67%) and 1341 males (33%) totaling 4092 clients from six large residential treatment centers. Using both exploratory factor analysis (EFA) and CFA the SRCS-10 has a large first to second eigenvalue ratio and had a CFI and TLI close to and above .9 with an SRMR of below .05 and an omega of .85. Additionally, we saw limited changes in key indicators such as the CFI and RMSEA across gender, race, and sexual orientation., Conclusions: These findings provide evidence that the SRCS-10 is a valid and reliable assessment across gender, race, and sexual orientation.
