Bu tezde Gaziantep Atık Su Arıtma Tesisinde çalışmakta olan biyogaz motorlu bir kojenerasyon sisteminin genetik algoritma temelli termodinamik optimizasyonu sunulmaktadır. Bu amaçla, Matlab programı kullanılarak kendi sistem kodlarımız geliştirilecektir. Kojenerasyon sistemi 1000 kW elektrik üretmekte ve havasız çürütme tankına ısı sağlamaktadır. Optimizasyon amacı olarak sistemin ekserji verimliliği seçilmiş ve sistem bileşenlerinin ekserji verimi optimize edilmiştir. Optimizasyon değişkenleri hava-yakıt karışım oranı, motor girişindeki hava-yakıt karışım basıncı ve motor soğutma suyunun motor çıkış sıcaklığı olarak seçilmiştir. Optimizasyonda elitlik ve rulet çarkı seçilim yöntemleri ayrı ayrı uygulanmıştır. Gaz motoru ekserji verimi, birinci yaklaşım için sadece yakıt girişi ve güç üretimi göz önüne alınarak, ikinci yaklaşım için ise birinci yaklaşıma ısıl etkilerin eklenmesi göz önüne alınarak belirlenmiştir. Gaz motorunun 1. ve 2. yaklaşıma göre ekserji verimleri elitlik yöntemiyle sırasıyla %23.5 ve %45.1 ve rulet çarkı yöntemiyle %26.7 ve %46.7 olarak bulunmuştur. Egzoz gazı ısı değiştiricisi ekserji verimi elitlik yöntemiyle %46.5 ve rulet çarkı yöntemiyle %43 olarak belirlenmiştir. Elitlik yöntemi ile 1 ve 2 numaralı ısı değiştiricilerinin ekserji verimleri sırasıyla %59 ve %56, rulet çarkı yönteminde %58.2 ve %56.5 olarak belirlenmiştir. Sistemin toplam ekserji verimi ise elitlik yöntemi ile %33.2 ve rulet çarkı yöntemiyle %33.5 olarak bulunmuştur. In this thesis, a genetic algorithm based thermodynamic optimization of biogas-powered cogeneration system which is active in GASKİ WWTP is presented. In this aim, our self-adaptive codes will be developed by using Matlab software. Cogeneration system produces 1000 kW electricity and supplies heat for anaerobic digestion. The objective of optimization is selected as exergy efficiency of the overall system and also exergy efficiencies of other components are optimized. Optimization variables are selected as air-fuel mixture ratio, the pressure of air-fuel mixture at the inlet of the gas engine and temperature of jacket cooling water at the outlet of the gas engine. Optimization is applied by using elitism and roulette wheel, separately. Gas engine exergy efficiency is determined by taking into account only the fuel input and power generation for the first approach, and by taking into account the addition of thermal effects to the first approach for the second approach. Exergy efficiency of the gas engine for 1st and 2nd approaches are found as 23.5% and 45.1% in elitism method and 26.7% and 46.7% in the roulette wheel method. Exergy efficiency of exhaust gas heat exchanger is determined 46.5% in elitism and 43% in roulette wheel. Exergy efficiencies of heat exchanger-1 and heat exchanger-2 are found to be 59% and 56% in elitism and 58.2% and 56.5% in roulette wheel, respectively. Overall exergy efficiency of the system is determined 33.2% in elitism and 33.5% in roulette wheel. 88

In this study a genetic algorithm based thermodynamicoptimization of biogas engine powered cogeneration system that is active inGASKİ WWTP and produce electricity from biogas which is produced in plant willbe presented.In this purpose, we will develop our own code that is suitable tocharacteristics of system, by using MATLAB software. Biogas engine powered cogeneration system produces 1000 kW electricity andsupplies heat for anaerobic digestion. The focus of optimization will be onexergetic efficiencies of heat exchangers and overall system. Optimizationvariables are selected as output temperature of water that flow from digestionthrought engine-side heat exchanger, its mass flow rate and temperature ofwater that transfer heat from engineto heat exchanger. Optimization isapplied with using elitism and roulettewheel methods seperately, also single point mutation is applied. By usingelitism and roulette wheel, exergetic efficiency of heat exchanger-1 foundrespectively 32,2 % and 39,1 %. Exergetic efficiency of heat exchanger-2 forboth two methods found 59,5%. By using elitism and roulette wheel, exergeticefficiency of exhaust gas heat exchanger found respectively 47,7 % and 41%. Exergetic efficiency of overallcogeneration system obtained 26,4 % by using elitism and 26,1 % by usingroulette wheel. Elitlik ve rulet çarkı yöntemleriile ayrı ayrı optimizasyon yapılmış olup, tek nokta mutasyon yöntemiuygulanmıştır. Elitlik yöntemi ile optimizasyonda 1 numaralı ısı değiştiricininekserji verimi %32,5, rulet çarkı yönteminde ise %40 olarak bulunmuştur. Heriki yöntem için 2 numaralı ısı değiştiricinin ekserji verimi %59,5 olarakhesaplanmıştır. Egzoz gazı ısı değiştiricisinin ekserji verimi ise elitlik verulet çarkı yöntemlerine göre sırasıyla %44,7 ve %41,1 olarak bulunmuştur.Kojenerasyon sisteminin toplam ekserji verimi ise elitlik yöntemi ile %26,5 verulet çarkı yöntemiyle %26,1 olarak bulunmuştur. Yapılan analiz neticesindedeğişkenlerin etkileri grafikler halinde sunulmuş ve tartışılmıştır.

WOS: 000418781000003 PubMed ID: 28853103 Purpose: The first purpose of this study was to reveal factors affecting the postoperative development of systemic inflammatory response syndrome (SIRS) in patients undergoing standard percutaneous nephrolithotomy (PNL) for renal stones. The second purpose was to determine the role of the preoperative platelet-to-lymphocyte ratio (PLR) and the neutrophil-to-lymphocyte ratio (NLR) in the prediction of SIRS. Matarials and Methods: In total, 192 patients who had undergone conventional PNL for renal stones from 2013 to 2015 were included in the study. SIRS developed postoperatively in 41 (21.3%) patients. The patients were divided into SIRS and non-SIRS groups, and the effects of the PLR, NLR, and other demographic and operative data were investigated to predict the development of SIRS. Variables significant in the univariate analysis were evaluated using a multiple logistic regression model to determine the independent risk factors for developing SIRS after PNL. Results: Univariate analysis revealed significant differences in the preoperative PLR (P < .001), preoperative NLR (P = .018), number of access sites (P < .001), mean renal parenchymal thickness (P = .02), operative time (P < .001), decrease in hemoglobin (P = .016), length of hospital stay (P < .001), stone-free status (P = .023), and complication rate between the two groups of patients. However, multivariate analysis showed that only the PLR and the number of access sites were independent factors affecting the development of SIRS. When the PLR cut-off value was 114.1, development of SIRS was predicted with 80.4% sensitivity and 60.2% specificity. Conclusion: The preoperative PLR is an effective and inexpensive biomarker with which to predict SIRS after PNL. In particular, we recommend close monitoring of patients with a PLR of >114.1 because of the possible development of serious complications.

Amaç: Laparoskopik adrenalektomi, benign adrenal kitlelerin tedavisinde altın standart yöntem olarak kabul edilmektedir. Ancak birçok cerrah, benign adrenal kitle tedavisinde açık cerrahiyi tercih etmektedir. Bu tercihin nedeni, cerrahların laparoskopik adrenal cerrahinin ileri düzey laparoskopik cerrahi olduğunu ve öğrenme eğrisinin yüksek olduğunu düşünmeleri olabilir. Bu çalışmada, transperitoneal laparoskopik adrenalektomi cerrahisi sonuçlarımızı bildirmeyi amaçladık. Yöntemler: 2013 ile 2015 tarihleri arasında Namık Kemal Üniversitesi Tıp Fakültesi Hastanesin Üroloji Anabilim Dalında yapılmış olan laparoskopik adrenalektomi vakalarının verileri retrospektif olarak değerlendirildi. Hastaların demografik özellikleri, adrenal kitle patolojileri, kanama miktarı, hastanede kalış süreleri ve komplikasyonlar değerlendirildi. Bulgular: Toplam 21 hastanın verileri değerlendirildi. Bu hastaların 12’sine (%57,1) sağ adrenalektomi yapılırken, 9’una (%42,9) sol adrenalektomi uygulandı. Hastaların ortalama operasyon süresi 130,2±39, dk, ortalama kanama miktarı 197,6±72,4 ml ve ortalama hastanede kalış süresi 3,09±1,57 gün olarak tespit edildi. Patolojik değerlendirmede hastaların 4’ünde feokromasitoma, birinde miyeloma ve 16’sında adenoma olduğu tespit edildi. On the other hand, most surgeons still prefer to perform open surgery for adrenal tumors. This may be related with the surgeons opinions that laparoscopic adrenalectomy is an advanced laparoscopic surgery and has a high learning curve. In this article we present the results of our initial transperitoneal laparoscopic adrenalectomy cases. Methods: Lateral transperitoneal laparoscopic adrenal­ectomy cases that were performed between 2013 and 2015, were retrospectively analyzed. Patients demo­graphics, pathological types, operation time, blood loss, hospitalization time and complications were evaluated and compared with the literature. Results: A total of 21 patients were analyzed. Twelve (57.1%) patients had right and 9(42.9%) patients had left laparoscopic adrenalectomy. Mean operation time was 130.2±39.1 min, mean blood loss was 197.6±72.4 ml and mean hospitalization time was 3.09±1.57 days. Pathol­ogy reports of adrenal tumors were pheochromocytoma at 4 patients, myeloma at 1 patient and adenoma at 16 patients. Synchronous bilateral upper urinary tract urothelial cancer (UTUC) is a very rare form of urothelial cancer. In patients with high-risk unilateral UTUC, radical nephroureterectomy (RNU) is the gold standard treatment. However, there is no consensus on the treatment for synchronous bilateral UTUC. Evaluation of the patient and the tumor is recommended. Bilateral nephron-sparing surgery (NSS) was performed on a 53-year-old patient who presented with high-risk synchronous bilateral UTUC, and the outcome was reported.

It has a multifactorial etiology. It had been thought nocturia was a nonspecific symptom of lower urinary system dysfunction, but it has been determined that many diseases, related to different organ systems, might be reasons for this nonspecific symptom. Along with the importance of systemic diseases that cause nocturia, the symptom itself has adverse effects on patients' health and quality of life. There are several studies reporting a direct relationship between nocturia and depression, cognitive dysfunction, mood disturbances, falls, and fractures. For this reason, it is important to treat nocturia both to increase quality of life and to decrease related complications. Treatment opportunities have been under investigation for 20 years. Most of the studies in the literature have reported the results of single-drug medication on nocturia, which may be insufficient for a situation that has such a multifactorial etiology. Ömrü boyunca Kur'an'ın doğru anlaşılması ve hakikatlarının yaşanması için çalışmış, bu uğurda her türlü sıkıntı ve meşakkate razı olmuştur. Onun yazdığı eserler Kur'an'ın bir çeşit tefsiri niteliğindedir. Kur'an'ın mucizelik yönlerinden en parlağının belagat olduğunu göstermiştir. O, Kur'an'ın işaretiyle ahir zamanda bütün insanlığın ilim ve fenne yöneleceğini, ilim ve fennin de en parlağı olan belagat ve cezaletin, insanlar arasında çok rağbet göreceğini söylemiştir. Belagat, hitap ettiği kimselere göre uygun ve tam yerinde düzgün ve hakikatlı söz söyleme sanatıdır. Bediüzzaman Said Nursi, hayatı, yüksek gayeleri, manevî şahsiyeti, düşünceleri fedakarlıkları ve telif ettiği Nur Risâleleri ile insanlığa her zaman güzel bir örnek olmuştur. İşte böyle bir zatın eserlerinin incelenmesi ile meydana gelen bu tezimizde Bediüzzaman Said Nursi'nin Kur'an'ın belagatı ile alakadar kullandığı kavramların tesbit edilmesi ve bu kavramların izah edilmesi ve misallerle açıklanmasına çalıştık. Background/Aims: In our study, we aimed to assess the effect of vitamin E and C supplementation to triple and quadruple Helicobacter pylori eradication regimens. Materials and Methods: Four hundred patients with H. pylori infection were classified into four groups. Patients in group A (n=100) received amoxicillin, clarithromycin, and lansoprazole for 2 weeks. In group B, patients (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, and lansoprazole for 2 weeks. Patients in group C (n=100) received amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks, whereas those in group D (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks. H. pylori eradication was assessed with the C14 urea breath test 2 months after the end of the therapy. The eradication rate was assessed using per-protocol (PP) and intention-to-treat (ITT) analyses. Results: Three hundred forty-eight patients finished the study. The eradication of H. pylori was achieved in 63 of 84 patients (75%) by PP and 63 of 100 (63%) by ITT analysis in group A, 60 of 84 (71.4%) by PP and 60 of 100 (60%) by ITT analysis in group B, 72 of 89 (80.9 %) by PP and 72 of 100 (72%) by ITT analysis in group C, and 76 of 91 (83.5%) by PP and 76 of 100 (76%) by ITT analysis in group D. There was no remarkable change between groups A and B (p>0.05). Similar results were also found between groups D and C (p>0.05).Conclusion: This study revealed that supplementing vitamins C and E to either the triple or quadruple therapies did not provide an additional advantage for achieving significantly higher eradication rates for H. pylori

Escherichia coli, üriner sistem inf eksiyonlarından (ÜSİ) en sık izole edilen mikroorganizmadır. Son zamanlarda ülke- mizde üropatojen E.coli suşlarında, ÜSİ tedavisinde sık kullanılan antibiyotiklere karşı duyarlılıkta azalma görülmektedir. Bu çalışmada, ÜSİ ön tanısıyla üroloji polikliniğinden gönderilen idrar kültürlerinden izole edilen E.coli suşlarının siprof loksa- sin, nitrof urantoin ve f osf omisin duyarlılıkları, Clinical and Laboratory Standards Institute (CLSI) önerileri doğrultusunda disk dif üzyon yöntemi ile belirlenmiş; genişlemiş spektrumlu beta-laktamaz (ESBL) üretimi çif t disk sinerji yöntemiyle araş- tırılmıştır. İncelenen E.coli izolatlarında en yüksek direnç oranı % 32.1 ile siprof loksasine karşı saptanmış, bunu % 5.1 ile nitrof urantoin ve % 4.5 ile f osf omisin takip etmiştir. ESBL pozitif lik oranı % 22.5 olarak tespit edilmiştir. Direnç oranları ESBL pozitif ve negatif lerde sırasıyla siprof loksasine karşı % 85.3 ve % 17.3 nitrof urantoine karşı % 12.6 ve % 2.9, f osf omi- sine karşı ise % 13.9 ve % 1.9 olarak saptanmıştır. Bölgemizde siprof loksasine direnç oranında yükselme gözlenmesine karşın f osf omisin ve nitrof urantoin, ÜSİ tedavisinde iyi birer alternatif olmayı sürdürmektedir. Escherichia coli is the most commonly isolated microorganism in cases of urinary tract inf ection (UTI). Currently, in our country, susceptibility of uropathogenic E.coli strains to commonly used antibiotics in the treatment of UTIs has decrea- sed. In this study, the resistance rates of E.coli strains isolated f rom urine samples of patients with UTI f rom urology outpa- tient clinic were determined f or ciprof loxacin, nitrof urantoin and f osf omycin by disk diff usion method according to Clinical and Laboratory Standards Institute (CLSI) guidelines and extended-spectrum beta-lactamase (ESBL) production was tested by double-disk synergy method. In E.coli strains the highest resistance rate was f ound f or ciprof loxacin (32.1 %). This was f ollowed by nitrof urantoin (5.1 %) and f osf omycin (4.5 %). ESBL positivity rate was f ound as 22.5 %. Resistance rates f or ESBL positive and negative cultures were determined as, 85.3 % and 17.3 % f or ciprof loxacin, 12.6 % and 2.9% f or nitrof u- rantoin, 13.9 % and 1.9 % f or f osf omycin. Despite the increased resistance observed f or ciprof loxacin in our region, f osf omycin and nitrof urantoin continue to be a good alternative in the treatment of UTIs.

Sevgi, nefret, bilgi, doğruluk, yalancılık, masumluk, saflık, dürüstlük, dostluk, hak, adalet, güven, güvensizlik, inanma söz verme, saygı, şeref, iyi - kötü gibi yüksek değerler; yarar, kuşku, çekememezlik, kıskançlık, haksızlık gibi ilgi ve çıkarın ortaya koyduğu araç değerler ile gelenekler, görgü kuralları, moda, zevk gibi kaynağında alışkanlıkların yer aldığı sosyal değerlerle kuşatılmış olan insan, kendisiyle ve toplumla her an çatışma halinde olmuştur. Bu çatışma, kendine özgü kuralları ve gerçekliği olan roman şahıslarının davranışlarına da yansımıştır. Bu tezde önce yapılan Felsefî araştırmalar ışığında antagonizm kavramı hakkında bilgi verilmiş; sonra da Türk romanının en önemli isimlerinden Halit Ziya Uşaklıgil'in mevcut sekiz romanındaki şahısların davranışları bu bağlamda incelenmeye çalışılmıştır. Amaç Çalışmamız soliter böbrek ile sağlıklı iki böbreğe sahip bireylerde böbrek boyut ölçümleri ve stereolojik yöntemle böbrek hacim hesaplamasının yapılması ve iki grup arasındaki morfolojik farklılıkların belirlenmesi amaçlandı. Materyal ve Metot Herhangi bir şikayet ile Namık Kemal Üniversitesi Araştırma ve Uygulama Merkezine başvurup, kontrast madde verilerek abdominal bilgisayarlı tomografisi (BT) çekilen hastalar çalışmaya dahil edildi. Hacim ölçümleri BT görüntüleri üzerinde Cavalieri prensibi ile yapıldı. Bulgular Çalışmamıza katılan bireylerin 22’si soliter böbrek, 30’u sağlıklı iki böbreğe sahiptir. Normal grup için böbrek hacim ortalaması 159 cm3, böbrek uzunluğu 10.7 cm, genişliği 6.6 cm ve kalınlığı 5 cm olarak bulundu. Soliter böbrek grubunda ise, böbrek hacmi 274 cm3, uzunluğu 12 cm, genişliği 7 cm ve kalınlığı 6 cm bulundu. Sonuç Soliter böbrek, normal böbreğe göre daha uzun, daha geniş, daha kalın ve hacimsel olarak da daha büyük olma eğilimindedir. Çalışmamızda soliter böbrek ile sağlıklı iki böbreğe sahip bireyler arasında, böbrek boyut ve hacim ölçümleri ile ilgili morfolojik farklılıklar ortaya konulmuştur. Elde edilen bu bulgular bazı böbrek hastalıklarının klinik tanı ve tedavisinde böbrek boyut ve hacminin doğru olarak değerlendirilmesi açısından literatüre katkıda bulunacağı düşüncesindeyiz. Aim This study was undertaken to compare the morphological differences between individuals with one functioning kidney and individuals with two functioning kidneys through the measurement of dimensions and stereological renal volume estimations. Materials and Methods The target sample consisted of patients who underwent an abdominal CT examination after attending Namık Kemal University’s Clinical Research and Application Center seeking medical attention. The volumetric estimations of CT images were based on the Cavalieri Principle. Results Of the overall study population, 22 had one healthy kidney and 30 had two healthy kidneys. Average renal dimensions were volume 159 cm3, length 10.7 cm, width 6.6 cm, and thickness 5 cm among anatomically normal individuals. The corresponding figures in the sole-functioning kidney group were volume 274 cm3, length 12 cm, width 7 cm, and thickness 6 cm. Conclusion Sole-functioning kidneys were longer, thicker, wider, and more voluminous compared with normal kidneys. We believe that the morphological differences between solitary and normal kidneys observed in this study in terms of dimensions and volume may represent a contribution to the existing literature that may assist in accurate volume and dimension estimations for clinical diagnosis and treatment of renal conditions.

Symptomatic myomas can cause a diversity of symptoms, which can be divided into four categories: bleeding symptoms (irregular and/or heavy menstrual bleeding), pain, bulk-related symptoms and subfertility. Traditionally, symptomatic myomas have been treated with myomectomy or hysterectomy. UME has been re-ported to be an effective and safe alternative to treat myomas UME is a percutaneous transcatheter embolisation technique using embolisa-tion material to occlude the (end-) arteries supplying the myoma. UME blocks off the blood supply to myomas, causing them to shrink. Bu çalışmanın amacı Türk bayraklı ticari gemilerde çalışan farklı görevlerdeki gemiadamlarının tükenmişlik düzeylerini belirlemek; ayrıca, tükenmişlik durumlarının gemiadamlarının yaşlarına, denizde çalışma sürelerine ve gemideki görevlerine göre farklılıklarını ortaya koymaktır. Araştırmanın örneklem hacmini Türk ticaret gemilerinde çalışan 4’ü “Kaptan”, 8’i “Güverte Zabiti”, 8’i “Makine Zabiti”, 15’i “Güverte Personeli”, 8’i “Makine Personeli” olarak 43 Gemiadamı oluşturur. Çalışmada Maslach Tükenmişlik Envanteri’nden esinlenerek geliştirilmiş 23 maddelik Gemiadamı Tükenmişlik Anketi (GTA) kullanılmıştır. Ankette gemiadamlarının tükenmişlik durumu ‘Duygusal Tükenme (9 madde), Duyarsızlaşma (5 madde) ve Kişisel Başarı (9 madde) olmak üzere toplam üç alt ölçekte değerlendirilmiştir. Verilerin analizinde, SPSS 16.0 paket programı (Sosyal Bilimler için İstatistik Paket Programı) kullanılmıştır. Kullanılan verilere öncelikle Güvenilirlik Analizi uygulanmış, sonra Kolmogorov-Smirnov Normallik Testi, gemiadamlarının demografik özelliklerine göre farklılık analizleri, non-parametrik testlerden Ki-Kare analizi ve tükenmişlik alt boyutları arasında Korelasyon (ilişki) analizi yapılmıştır. Çalışmanın sonucunda; gemiadamlarının Duygusal Tükenmişlik alt boyutunda tükenmişlik yaşamadığı, buna karşın Duyarsızlaşma ve kişisel başarı alt boyutlarında ileri düzeyde tükenmişlik yaşadıkları belirlenmiştir. Ayrıca tükenmişlik alt boyutlarının Gemiadamları’nın yaş, denizde çalışma süreleri ve gemideki görevlerine göre anlamlı bir farklılık göstermediği de tespit edilmiştir. Bu durumun, gemiadamı sayısının yetersizliği, uzun çalışma saatleri, düşük kalitede uyku, beslenme yetersizlikleri, elektromanyetik kirlilik, gürültü ve olumsuz çevre koşullarından kaynaklandığı düşünülmektedir., THE NEGATIVE EFFECTS OF WORKING CONDITIONS UPON SEAMEN ON SHIPS SUMMARY Burnout can be defined as: individuals’ breaking from the very meaning and aim of their profession, their not being able to be concerned with the people they are serving or their withdrawing themselves from the job in reaction to their professional insatisfaction. The aim of this study is to determine the burnout levels of seamen of different status on commercial ships under Turkish flag. Besides, this study aims to discover the burnout level differences of seamen according to their age, the period they spent on sea, and their duty on the ship. The illustration number of this study consists of 43 seamen; 4 of them are ‘Captain’, 8 of them are ‘Deck Officer’, 8 of them are ‘Engine Officer’, 15 of them are ‘Deck Staff’, 8 of them are ‘Engine Staff’. In this study, Seamen Burnout Questionnaire (GTA), a questionnaire developed on Maslach Burnout Questionnaire, is used. In this questionnaire, burnout levels of seamen are analyzed under three sub-classes: ‘Emotional Exhaustion’(9 Items), ‘Depersonalization’(5 Items) and ‘Individual Success’(9 Items). Analyzing the data, SPSS 16.0 packet program (A statistical packet program for social sciences) is used. The mentioned data has undergone respectively, a Reliability Analysis, Kolmogorov-Smirov Normality Test, Difference Analyses according to seamen’s demographic characteristics, Ki-Kare analysis from non-parametric tests and Correlation Analysis among burnout sub-classes. As a result of this study, it was determined that seamen do not suffer from burnout in the sub-level of Emotional Exhaustion, but suffer from great levels of burnout in the sub-levels of Depersonalization and Individual Success. Furthermore, it was discovered that sub-levels of burnout do not differ very much according to age, the period they worked on sea and their duty on ship. It is thought that, this situation is a result of few number of seamen, long working periods, low quality sleep, malnutrition, electromagnetic pollution, noise and negative environmental conditions., Yüksek Lisans, M.Sc.

Katma değerli yeni servislerin sayısının artması ile beraber konuma dayalı servislerin önemi artmaktadır.Acil aramaların nereden yapıldığının belirli bir hata payı içinde tespit edilmesini şart koşan Uluslararası İletişim Komisyonu da konuma dayalı servisler üzerinde anahtar bir etken olmuştur. Bu nedenle konum belirleme anahtar bir araştırma sorunu olmuş ve birçok sayıda konum belirleme çözümü sunulmuştur.Bu çözümlerden bazıları hücre numarası, varış zamanı ve açısı, istatistiksel ve parmak izi yöntemleridir. Bu tez parmak izi yöntemine dayalı bir konum belirleme yöntemi öne sürmektedir.Parmak izine bağlı konum belirleme bir kullanıcının yerini bulabilmek için daha önce toplanmış sinyal ölçümlerinden yararlanır. Önceki sinyal ölçümleri sonradan sinyal izi eşleştirmede kullanılmak için bir veritabanına kaydedilir ve bu veritabanına sinyal izi veritabanı denir. Sinyal izine bağlı konumlandırma dört aşamadan oluşmaktadır, bunlar ölçüm toplama, sinyal izi filtreleme, sinyal izi eşleştirme ve konum hesaplamadır.Bu tezde konuma dayalı servislerin hassaslığını artırmak için yeni bir sinyal izi eşleştirme yöntemi önerilmektedir. Bu yeni yöntem en çok bilinen ve en çok kullanılan yöntemler ile kıyaslanmıştır ve performansı bu algoritmalar ile karşılaştırılmıştır. Sistem mobil kullanıcının sinyal değerlerini ve hücresel ağ tarafından yapılan ölçümleri kullanarak kullanıcının yerini tespit edebilmektedir. Location based services have attracted too much attention recently due to the increasing number of new value-added services. Also, the requirement set by Federal Communications Commission (FCC) to locate emergency calls according to certain accuracy criteria has been the key factor for the development of location methods. Hence, location positioning has been a key research problem and numerous localization solutions have been proposed. These include technologies such as Cell ID, angle and time of arrival methods, statistical methods and fingerprinting methods. This paper proposes a fingerprinting based positioning method.Location fingerprinting is a novel method that benefits from the previous signal measurements to find the location of a user. The previous signal measurements are stored into a database, called fingerprint database, to be used for fingerprint matching. Fingerprinting consists of four phases such as data collection, fingerprint filtering, fingerprint matching, and location estimation algorithms.In this thesis, a new fingerprint matching algorithm is proposed to increase the accuracy of the location based services. This new fingerprint matching algorithm is compared with the most frequently used and well-known algorithms. The proposed algorithm does not require any hardware modifications. It does not require any modifications in the software on the mobile devices, either. By exploiting the measurements that are already being performed by the cellular network, the system can estimate the location of the mobile station using the RSS values of the MS. It is shown that the proposed algorithm gives better results than the compared algorithms in terms of accuracy. 76

AMAÇ: Sıçanlarda testis torsiyon süresine bağlı olarak Leydig hücrelerinin endokrin fonksiyonları hakkında veri elde edebilmek için, hipotalamo-hipofizer-gonadal aks hormonlarının düzeylerindeki değişiklikleri erken ve geç dönemde tespit etmeyi amaçladık.MATERYAL, METHOD: Sprague-Dawley soyu yetişkin erkek sıçanlar Cerrahpaşa Deney Hayvanları Üretim ve Bakım Laboratuarından elde edildi. Sıçanlar, her grupta 10 tane sıçan olmak üzere 4 gruba ayrıldı, 1. grup sham grubu idi. Anestezi altında bütün gruplardaki sıçanların batınları açıldı ve sol testislerine orşiektomi yapıldı. Sıçanların sağ testisleri sırasıyla 2. grupta 1 saat, 3. grupta 3 saat, 4.grupta 5 saat süreyle 7200 torsiyone edilip, daha sonra sıçanların sağ testisleri detorsiyone edildi. Birinci gruptaki sıçanların sağ testislerine herhangi bir işlem uygulanmadı. Bütün sıçanlardan torsiyon-detorsiyonun 3. ve 30. gününde kuyruk veninden 2 cc kan alınarak serum total testosteron, serbest testosteron, LH düzeyleri ELİSA yöntemiyle ölçüldü.BULGULAR: Tüm gruplardaki hayvanların hormon tetkikleri değerlendirildiğinde 30. gündeki total testosteron düzeylerinde sadece 1. grup (sham) ile 4. grup arasında istatistiksel anlamlı fark bulundu. Dördüncü grup ile diğer bütün gruplar 30.gün serbest testosteron düzeyi arasında istatistiksel anlamlı fark bulundu. Dördüncü grup ile 1. ve 2. grup 30. gün LH düzeyleri arasında bulunan istatistiksel anlamlı fark, total testosteron ve serbest testosteronda bulunan farklarla uyumlu olup, 5 saatlik torsiyon sonrası testisin testosteron üretimininde bozulma olduğunu destekliyordu.SONUÇ: Beş saatlik torsiyon uygulanan sıçanlarda 30. günde bakılan total ve serbest testosteron düzeyinin diğer gruplara göre anlamlı derecede düşük çıkması, sıçanlarda 5 saatlik torsiyondan sonra Leydig hücrelerinin fonksiyonunun bozulduğunu gösterdi. PURPOSE: We aimed to investigate the alterations in the hypotalamo-hypofiser-gonadal hormones? levels; to find out the endocrin functions of Leydig cells due to duration of testicular torsionMATERIAL, METHOD: Sprague Dawley adult male rats were obtained from Experimental Animal Labratory of Cerrahpasa School Of Medicine. Rats were classified in 4 groups containing 10 each. Orchiectomy was performed in left testicles under anesthesia in all groups. At right testicles, torsion and later detorsion was carried out for 1 hours in 2. group, 3 hours in 3. group, and 5 hours in 4. group, respectively. In 1. group (sham), any intervention was carried out in right testicles. At the 3th and 30th day after torsion-detorsion 2 cc venous blood was obtained and levels of LH, total and free testosterone were measured with ELISA.RESULTS: Significant statistical relation was found at total testosterone levels at 30th day between sham and 4. groups. Also, significant relation was found at free testosterone levels at 30th day between 4th group and other groups. The statistical significant relation at the levels of LH at 30th day between 4th and first and second groups was in harmony with the difference at total and free testoterone and supports the deterioration at the testosterone production of testis after a duration of 5 hour testicular torsion .CONCLUSION: Total and free testosterone at 30. day in the 4.group, was torsioned 5 hours, was found significantly low according to other groups. We demonstrate, Leydig cells continue endocrin functions until 5 hours torsion at rats. 45
