Purpose: To compare the anatomical, morphological, and functional outcomes of the conventional internal limiting membrane (ILM) peeling versus temporal inverted internal limiting membrane flap technique for large full-thickness macular holes (FTMHs). Subjects and Methods: Forty-six eyes of 44 patients with a minimum base diameter > 600 µm were included in this retrospective interventional study. The patients were divided into Group 1 (conventional ILM peeling) and Group 2 (temporal inverted ILM flap). The hole closure rate, best-corrected visual acuity (BCVA), ellipsoid zone (EZ), and external limiting membrane (ELM) defects were analyzed at baseline and 6 months after surgery.Results: Hole closure was achieved in 17/25 (68%) cases of Group 1 and 20/21 (95.2%) cases of Group 2. The hole closure rate was significantly higher in the temporal inverted ILM flap group (p = 0.022). The mean BCVA (logMAR) changed from 1.12 ± 0.43 to 0.72 ± 0.31 in Group 1 and from 1.07 ± 0.34 to 0.51 ± 0.26 in Group 2 at six months (p < 0.001 in both cases). U-shaped closure was observed in 3 (12%) eyes in Group 1 and 15 (71.4%) eyes in Group 2 (p < 0.001). The total restoration rates of ELM and EZ were significantly higher in the temporal inverted ILM flap group (p = 0.009, p = 0.001, respectively).Conclusion: The temporal inverted ILM flap technique is more effective than conventional ILM peeling for larger than 600 µm macular holes and improves anatomical, morphological, and functional outcomes.

The purpose of this study was to determine the phenotypic antibiotic susceptibility patterns, extended-spectrum ?-lactamase (ESBL) genes and genotypic profiles of ESBL-positive Escherichia coli strains isolated from urine samples obtained from outpatients with urinary tract infection in Turkey. A total of 120 E. coli strains during 2017, 2018, and 2019 (40 patients per year) were examined for antibiotic susceptibility patterns by disc diffusion method, for ESBL genes using PCR and sequencing and for molecular typing by pulsed-field gel electrophoresis (PFGE) method. The isolates were evaluated for their sensitivity to 21 different antibiotics. Four different antimicrobial resistance patterns were determined according to antibiotic susceptibility status of the isolates. The ?-lactamase genes detected in the isolates were CTX-M-15 + OXA-1 (n= 14), CTX-M-15 (n= 24), TEM-1 + CTX-M-15 (n= 52), TEM-1 + SHV-12 (n=6), SHV-12 1 (n= 6), TEM-1 + CTX-M-1 (n= 6), TEM-1 + CTX-M-16 (n= 6) and TEM-1 + CTX-M-9 (n= 6). The CTX-M-15 was the most prevalent ESBL enzyme in the isolates. As a result of PFGE analysis performed by XbaI enzyme restriction process, one major PFGE profile and three main groups (Group I-II-III) were observed. While antibiotic resistance profiles of the strains showed four groups (RI-RII-RIII-RIV), PFGE band profiles showed a major group (90% similarity ratio). High ESBL production and decreased susceptibility to broad-spectrum cephalosporins were observed in E. coli strains. Aim of the Study: Analyzing the effectiveness of half-dose single treatment Photodynamic Therapy (PDT) which was implemented by using verteporfin in chronic central serous chorioretinopathy (CSCR) is the aim of this study. Material and Method: In this study, 34 patients'who have chronic CSCR 37 eyes were examined prospectively. Patients were given 3mg/m2 verteporfin and PTD was implemented to the areas where were identified in FFA and/or ICG by using the spot which is 1000 micron bigger than the active lesion's total diameter. Before and after the photodynamic therapy, in 1 week, 1 month and 6 month Central foveal thickness were measured by using full ophthalmoscopy and optical coherence tomography. The variation in Best Corrected Visual Acuity and Central Foveal Thickness is accepted as the criteria for effectiveness. Findings: This study focuses on 37 eyes of 34 patients who are 25 male and 9 female. The average age of patients is 48.5±8.4. The average follow-up time is 6.4±1.5 months. As a result of the Photodynamic therapy, it is observed that 51 %of patients had 2 rows or more visual recovery according to the Snellen visual acuity chart. While Best corrected visual acuity level were same for 27% of patients', 1 or more rows decrease were identified for 10% of patients. Before the photodynamic therapy, while the Central foveal thickness was 372.4±107.5 ?, after the photodynamic therapy the average central macular thickness was 220.97±76.4 ? (p

52 years female patient came to our clinic complaints with blurred vision and vision loss for the last few months. Visual acuity was detected 0.4 and 0.1 snellen scale at her right and left eye respectively. Slitlamp evaluation revealed normal anterior segments. Fundoscopic examination showed spotted atrophic areas and beaten egg-like appearance at the lower border of the round sharp macular lesion. Hyporeflective area was detected under the neurosensorial retina at RPE level with OCT in both eyes. With these clinical findings patient was diagnosed as adult onset vitelliform dystrophy. Purpose: To determine the incidence of retinopathy of prematurity (ROP) in premature infants and to find out the risk factors that play a role in its development. Material and Materials and Method: 223 infants who were scanned and followed for retinopathy of prematurity at Ondokuz Mayis University, Faculty of Medicine between the dates January 2007 and August 2014 were included in the study. The data of the examinations were obtained from our clinic's archive while the patients' birth weights and comorbid systemic diseases were taken from patient files. Results: 446 eyes of 223 premature infants, 129 of whom were male (57.8%) and 94 of whom were female (42.2%), were examined in the study. Of the 223 infants, 138 (61.9%) were found to have ROP at any stage while ROP was not found in 85 (38.1%) infants. Of the risk factors examined in our study, birth weight, birth week, multiple pregnancy, respiratory distress syndrome (RDS), blood transfusion, sepsis, intraventricular hemorrhage (IVH), surfactant treatment, bronchopulmonary dysplasia (BPD) and patent ductus arteriosus (PDA) were found to be the risk factors that affected the incidence of ROP. In the multivariate regression analysis that was conducted, birth week, birth weight and intraventricular hemorrhage were found to play a role in the development of ROP as independent risk factors. Conclusion: Retinopathy of prematurity is a multifactorial disease. In our study, low birth weight, low gestation age and intraventricular hemorrhage were found to be the risk factors effective in the development of retinopathy of prematurity. © 2015 Gazi Eye Foundation. We aimed to present a patient with bilateral choroidal metastasis following bladder carcinoma due to its rarely occurence. A 56 years old male patient was consulted to our clinic complaining of blurred vision and vision loss. Best corrected vision acuity was measured as 0.3 in right eye and 0.6 in left eye. Fundus examination showed serous detachment in superior temporal quadrant including temporal edge of macula in right eye and in the inferior temporal quadrant of the left eye. Optic cohorence tomography showed a choridal mass elavating retina. In orbital MRIG an ellipsoidal mass with 10×5 mm size was detected in the posteriolateral part of the right globe. Methastasis in the lung and scalp tissue were detected with the patient who had been following for 3 years for bladder carcinoma. We speculated that choroidal mass and serous detachment could be owing to the metastasis of bladder carcinoma. Two weeks after the first dose of chemotherapy and radiotherapy patients died due to systemic metastases. Choroidal metastases are the most common intraocular tumours. Choroidal metastasis, secondary to urothelial carcinoma, is rarely seen and mostly effects individuals that are old and male. © 2016 Gazi Eye Foundation. All rights reserved.

35-year-old female patient came to our clinic with complaints of blurred vision and low vision. Her examination showed that optic disc of the left eye was normal and there were prevalent white lesions located in the deep layers of retina with more lesions at the macula temporal. Fundus fluorescein angiography (FFA) showed hyperfluorescence lesions in the early period and coloration in the later period. The findings brought multiple evanescent white dot syndrome to mind. No treatment was started and the patient's control a week later showed her vision at the level of 0.6 snellen scale. İnterferon (İNF) kronik hepatit B, C, metastatik renal karsinom, kutanöz melanom,kaposi sarkomu ve yeni doğanlardaki hemanjiomlarda kullanılan antiviral, antiproliferatif ve immünomodülatuar aktiviteyi düzenleyen bir ilaçtır. İNF alan hastalardaretinopati gelişme insidansı %18-86 arasındadır. Retinopati gelişmesi için başlıca riskfaktörleri yüksek doz INF tedavisi, diabetes mellitus ve hipertansiyondur. INFtedavisine bağlı gelişmiş retinopati genellikle iyi seyirlidir, görme kaybı ve diğer gözsemptomlarına yol açmaz. Fakat bizim olgumuzda retinopati büyük bir görmekaybına yol açmış ve 3 aylık takip sonrasında düzelmemiştir. Makalemizde 56yaşında, erkek, delta hepatiti nedeniyle yüksek doz interferon-alfa 2b tedavisi(Haftada 3 gün 10 milyon ünite) sırasında 17. ayda retinopati gelişen hastamızısunduk. Sonuç olarak; retinopati İNF tedavisi süresince gelişebilen akılda olmasıgereken bir komplikasyondur. Özellikle, yüksek risk grubundaki hastalar İNF tedavisiöncesinde mevcut olabilecek retinopati açısından değerlendirilmeli ve tedavi süresince retinopati gelişimi riski açısından üç aylık aralıklarla düzenli takip edilmelidir

Amaç: Akut gastroenteritler, hastaneye başvuru yakınmaları arasında sıklıkla yeralmakta ve tedavilerdeki gelişmelere rağmen yüksek mortalite ve morbidite oranlarıile seyretmeye devam etmektedir. Bu çalışmada Dokuz Eylül Üniversitesi TıpFakültesi Hastanesi Çocuk Acil polikliniğine akut ishal yakınması ile başvuran çocukolguların başvuru özelliklerinin değerlendirilmesi amaçlandı.Gereç ve yöntem: Dokuz Eylül Üniversitesi Tıp Fakültesi Hastanesi Çocuk Acilpolikliniğine akut ishal yakınması ile başvuran 541 olgu [291 (%53,8) erkek]retrospektif olarak değerlendirildi. Hastalar yaş, cinsiyet, dehidratasyon varlığı vederecesi, tedavi şekli ve süresi, komplikasyonların varlığı açısından araştırıldı. Bulgular: Hastaların başvuru anındaki yaşı ortalama 4,7 ± 4,1 yıl (1 ay-16 yaş,ortanca: 3 yaş) idi. Otuzüç hasta (%6) kanlı dışkılama, 508 hasta sulu dışkılamayakınması ile başvurdu. İshal süresi ortalama 2,0 ± 1,6 gün (min:1- maks: 9 gün),günde yapılan dışkı sayısı ise ortalama 5,2 ± 2,9 (min: 3- maks:15) idi. Hastaların %70’inde (383 hasta) kusma yakınması da mevcuttu. Hastaların 206’sı (%38,4)hastaneye başvurmadan önce antibiyotik kullanmaya başlamıştı. Hastaların %84’ünde(450 hasta) dehidratasyon yok iken, %16’sında (91 hasta) değişik derecelerdedehidratasyon mevcuttu. Onsekiz hasta (%3,3) hastaneye yatırıldı. Yatış nedenlerielektrolit imbalansı, ağır dehidratasyon ve asidoz, ağır kusma, konvulzyon geçirme veailenin sosyal endikasyonu idi. Dehidratasyonu olan hastalara oral veya parenteralyolla rehidratasyon tedavisi verildi. Akut ishal yakınması ile başvuran hastalararasında kaybedilen hasta olmadı. Sonuç: Çocuklarda akut gastroenteritlerin büyük çoğunluğunu viral gastroenteritleroluşturmaktadır. İshal tedavisinin en önemli komponenti dehidratasyonun tespiti vetedavisidir. Bu nedenle tüm hastalarda biyokimyasal incelemeler yapılmamalı,antibiyotik tedavisi gerektirecek özellikleri olmayan hastalara ampirik antibiyotiktedavisi başlanmamalıdır

Dokuz yaşında erkek hasta, sağ gözde bulanık görme ve sinek uçuşması yakınmalarıyla başvurduğu dış merkezde tek taraflı retinoblastom tanısı aldıktan ve ilk kür kemoterapisi uygulandıktan sonra merkezimize başvurmuştu. Ailede retinoblastom öyküsü pozitifti. Oftalmolojik incelemede görme keskinliği her iki gözde 10/10, bilateral ön segment muayenesi doğal, göz içi basınçları normal idi. Fundus incelemesinde; sağ gözde optik disk ve makula olağan, retina nazalinde vitreusa protrüze olan kitle ve inferiorunda birkaç odak, vitreusta yaygın tohumlanma görüldü. Sağ gözdeki retinoblastom “Reese-Ellsworth Grup Vb”, “Uluslararası Retinoblastom Sınıflaması Grup E” olarak değerlendirildi. Sol göz normal bulundu. Hastanın başka merkezde aldığı ilk kür kemoterapisinden sonra merkezimizde dört kür VEC (vinkristin, etoposid, karboplatin) kemoterapisi kemoredüksiyon amacıyla uygulandı. Kemoterapi ile sağ gözdeki kitlede ve vitreus tohumlanmasında gerileme olmadığı görüldü. Retinoblastomun lokal ilerleme ve sistemik metastaz yapma riski de göz önüne alınarak, ileri evreli, kemoterapiye yanıtsız retinoblastomu olan sağ göze, 10/10 görmesine karşın enükleasyon uygulandı. Lokal tedavi ile görme keskinliği ve globun korunması planlanan olguda, tümörde kemoredüksiyon rejimi ile tümör regresyonu sağlanamadığından enükleasyon uygulanmıştır

Our study aims to determine the efficacy of infectious disease consultations and the interrelations between doctors in this social laboratory. This study was conducted at 34 centers located in 22 cities across Turkey and contributed by 210 infectious disease specialists (IDSs) and 970 non-infectious disease specialists (NIDSs), totaling 1180 medical doctors. Infectious disease specialists and NIDSs have separately contributed by responding to questionnaires designed specifically for the consultation process. It appears that a satisfactory collaboration has been established between IDSs and NIDSs during the consultation practices. There are some discrepancies in the perceptions of some of the NIDSs. These are the evaluation of patients holistically, the expectation of NIDSs in critical infection cases to start the therapy immediately, losing the support of drug companies by NIDSs, and the restriction of NIDSs in routine medical practice. On the other hand, NIDSs seem to have real problems in the diagnosis or treatment of infectious diseases. The consultation service provided by the IDSs in Turkey is widely accepted among other clinicians and appears to be of a crucial importance. OBJECTIVE: To evaluate the effects of confounding factors on sexual function in women with minimal pelvic organ prolapse (POP). METHODS: A cross-sectional study was conducted at a cervical cancer screening center in Turkey between December 1, 2012, and March 31, 2013. Symptom-free women with stage I or II POP were enrolled to evaluate the association between sexual function and sociodemographic variables, POP, POP-related quality of life, stress incontinence, and overactive bladder. RESULTS: Of 243 volunteers (mean age 46.0±9.1 years), 188 (77.4%) had a low Female Sexual Function Index score. Women with better sexual function tended to be younger, have a higher salary, not be in the menopause, and have no chronic illness (P0.05), apart from transvaginal length (P=0.011). Overactive bladder was more common (P=0.005) and more severe (P=0.002) in women with sexual dysfunction, and their POP-related quality of life was worse (P

Objective: In this study, the influences of Mother Health and Child Care Training Program on expectant mothers' traditional belief and applications related to pregnancy, childbirth and puerperium have been researched. Material and Methods: In the research, both scanning and experimenting models have been used. While scanning model was used to define traditional belief and applications related to pregnancy, puerperium, the real experimental model with pre-test and post-test design was used to define the effects of traditional belief and applications of the training program. Among 64 pregnant individuals, 31 experimental from the sub-socio-cultural society and 33 from the control group, who could not take the benefits of the training sufficiently, were educated during 24-36 week-pregnancy period. In this study, the data were acquired first by applying "the Interview Form of Traditional Belief and Applications of Mother Health" (n= 196). Then, the "Scale of Traditional Belief and Applications in Mother Health" and "Success Test about Mother Health" were applied as pre-tests and post-tests. Results: 87.6 percent of the expectant mothers stated that they believed to become pregnant by drinking the mixture of different plants; 77% of mothers said by placing sugar, sugar, tar, and the cotton made out of herbs, spices and onions, etc., into the uterus, 78.2% said that they believed the baby would be beautiful if the pregnant lady stared at the beautiful people around. At the end of the training, a significant increase were recorded in the post test achievement scores of experimental group, and achievement test was found to be higher than the control group posttest mean scores (p=0.025). There was no significant difference among the mean scores of pretest/posttest AS-FOIU Attitude Scale in the control group (p=0.525). At the end of the training, it was observed that there were positive changes among mothers related to pregnancy, delivery, and puerperium, furthermore AS-FOIU posttest attitude scale means were found to be significantly higher than pretest scores (p

A web site or application can be described as social media when it allows user-based content, individual users/members and provide interaction among users. Compared to traditional media, social media promotes two-way communication among users. Also social media is cost-efficient when compared with the traditional media like newspapers and television in terms of information publishing and access. Also access to social media is open to everyone. Users, who aren't satisfied with the content offered by traditional media produce the basis of social media by creating their own content [2]. Usage of platforms including blogs; social networks like Facebook, Myspace and Friendfeed; professional networks like LinkedIn; content sharing communities like Youtube and Flickr; sites like Wikipedia; forums and comments on news sites are becoming widespread day by day and continues to find new fields of application. The interactive nature of social media can also have some advantages for distance education. Technology-based distance education applications that aim to reach large audience become more challenging and effective when the communication and interaction opportunities increase. When appropriately integrated into the online classroom, social media can offer innovative learning experiences. On the other hand, online collaboration becomes much easier when it is facilitated by social media technologies. With creative integration of social media tools in a class, students can participate more and be more engaged in the learning process. One of the most important advantages of social media usage in distance education is helping online learners feel less isolated. The diversity of the platforms accepted as social media entails different suggestions and discussions about which the platform can be used for what purpose within the educational process. This study aims to clarify the usage of social media in distance education considering the examples from the field.
