- Abstract
Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation. - Abstract
Background Reoperation rates are high after surgery for hip fractures. We investigated the effect of a sliding hip screw versus cancellous screws on the risk of reoperation and other key outcomes. Methods For this international, multicentre, allocation concealed randomised controlled trial, we enrolled patients aged 50 years or older with a low-energy hip fracture requiring fracture fixation from 81 clinical centres in eight countries. Patients were assigned by minimisation with a centralised computer system to receive a single large-diameter screw with a side-plate (sliding hip screw) or the present standard of care, multiple small-diameter cancellous screws. Surgeons and patients were not blinded but the data analyst, while doing the analyses, remained blinded to treatment groups. The primary outcome was hip reoperation within 24 months after initial surgery to promote fracture healing, relieve pain, treat infection, or improve function. An ultraviolet (UV) disinfection system was operated under field conditions using both unfiltered and filtered secondary wastewater effluents to assess the feasibility of using UV light to meet the stringent criterion specified in the California Wastewater Reclamation Criteria (CWRC). In the unfiltered wastewater, the CWRC criterion that the 7-day median not exceed 23 total coliform/100 mL was met consistently in effluent exposed to an average UV dose of 60 mW.s/cm 2 or greater. In the filtered wastewater, the CWRC criterion that the 7-day median not exceed 23 total coliform/100 mL was met consistently in effluent exposed to an average UV dose of 48 mW.s/cm 2 or greater.

- Abstract
Rats 2-, 7- and 12- mo. old were exposed to supine body restraint plus cold (5°C) for 3 hr. The mean cumulative length of lesions for the 2-, 7-, and 12 mo. old rats was 22.0, 43.0 and 16.0 mm. respectively. The same experimental design was used in a second study, but the pylorus was ligated prior to restraint. Total acid output/hr. was 134.6, 178.2, and 64.7 μEq/60 min. respectively for the three age groups. Older rats were not more susceptible to stress-ulcer and gastric acid was not significantly related to degree of ulceration. [ABSTRACT FROM PUBLISHER]

Chipmunks were allowed access to a running wheel and fed 1½ h each day. After 13 days of restricted feeding, all activity animals had died and postmortem pathology revealed extensive lesions of the glandular stomach. Food-yoked control animals, not allowed to exercise, very rarely died and showed minimal signs of stomach pathology when autopsied. Circadian running patterns of activity animals indicated a marked increase in diurnal running during restricted feeding. Results indicate that the activity-stress procedure is capable of producing stomach ulcers in a wild animal species.

In Experiment 1 rats exposed to signaled and unsignaled grid shock secreted more gastric acid after shock stress (chronic stress) for 8 days compared to the first 12 hr. of shock stress (acute stress). However, Experiment 2 indicated that the higher gastric acid values under chronic stress were not significantly greater than prestress baseline values. The results are interpreted to reflect an inhibition of gastric acid secretion as a function of acute stress. During chronic stress this inhibition is followed by a habituation of gastric secretory processes which is observed as a return of secretion volume to baseline levels.

Rats housed in running-wheel activity cages and fed 1 h daily exhibited excessive running and subsequently died revealing large stomach ulcers. However, prior experience with a 1-h feeding schedule for 15 days before exposure to the activity-stress procedure did significantly extend survival time. A second study illustrated that feeding schedules of.5, 1.0, 1.5, or 2.0 h daily did not yield differences in terms of stomach ulcers or survival time, but a 3-h daily feeding did significantly reduce ulcer incidence and increase survival time.

- Abstract
Female rats housed in running-wheel activity cages and fed 1 hr. daily ran significantly more than similarly housed and fed male rats. Male rats survived an average of 10 days, whereas mean survival time for female rats was 7.3 days. Experimental activity rats developed glandular stomach ulcers. Pair fed controls housed in cages without activity wheels were ulcer-free. Among experimental activity rats, there were no sex differences with respect to the number or severity of ulcers.

Rats fed 1 hr daily and housed in running-wheel activity cages exhibited excessive running and developed stomach ulcers as compared to food control, body weight control and home cage control rats. In addition to the observed gastric disease, experimental animals had increased bilirubins, decreased glycogen and decreased serum proteins suggesting that hepatic disease played a role in the lethal consequence of exposing rats to the activity-stress procedure. The decreases in liver glycogen and serum glucose suggested that the terminal problem was related to incipient exhaustion of metabolic substrates.

Female rats housed in running-wheel activity cages and fed 1 hr. daily ran significantly more than similarly housed and fed male rats. Male rats survived an average of 10 days, whereas mean survival time for female rats was 7.3 days. Experimental activity rats developed glandular stomach ulcers. Pair fed controls housed in cages without activity wheels were ulcer-free. Among experimental activity rats, there were no sex differences with respect to the number or severity of ulcers.

1. 1. The effects of glucagon alone and glucagon plus stress on lipid levels in various organs were studied. 2. 2. Stress added to glucagon produced no significant change in cholesterol levels of the kidney, liver, and aorta. There were also no significant changes in the triglyceride levels of liver, aorta and serum. 3. 3. Stress did raise the level of diaphragm and serum cholesterol. 4. 4. Stress lowered kidney and diaphragmatic triglyceride in glucagon treated animals. 5. 5. The significance of these results are discussed.

Rats fed a high lipid diet and also exposed to unavoidable and unpredictable grid shock for 2 and 8 days showed higher levels of accumulated chemical cholesterol in aorta, kidney, liver and serum as compared to control rats fed only the high lipid diet. Thirty days of shock stress produced similar results except that kidney tissue revealed lower and nonsignificant levels of accumulated cholesterol. A differential cholesterol clearing mechanism for the various tissues is postulated to explain the relative vulnerability of aorta, liver and kidney to stress-induced cholesterol deposition.

In a previous paper (1) it was found that stress had definite effects on lipid deposition in rats on high cholesterol diet. In that paper it was found that there was increased deposition of cholesterol in the aortas, livers, and sera of stressed animals as compared to the controls. There was only a transient elevation in the kidneys of the stressed antimals. Additionally, evidence was presented that the kidney had a better mechanism for clearing cholesterol than the aorta. Also, in other work it has been found that clofibrate seemed to have some effect in mobilizing cholesterol from tissues (2). In this paper an attempt was made to see if CPIB would have any effect on cholesterol deposition in the stressed rat.

- Abstract
Background: There is an epidemic of renal failure among Aborigines in the Australia's Northern Territory. The incidence is more than 1000 per million, and is doubling every three to four years. We evaluated the relationship of birthweight to renal disease in adults in one high-risk community., Methods: We screened more than 80% of people in the community for renal disease, using the urine albumin/creatinine ratio (ACR, g/mol) as the marker, and reviewed records for birthweights., Results: Birthweights were available with increasing frequency for people born after 1956. In 317 adults aged 20 to 38 years at screening, the mean birthweight (SD) was 2.712+/-0.4 kg, and 35% had been low birthweight (LBW, less than 2.5 kg). Birthweight was positively correlated with body mass index (BMI), blood pressure, and diabetes rates, but was inversely correlated with ACR. The odds ratio for overt albuminuria in LBW persons compared with those of higher birthweights was 2.82 (CI, 1.26 to 6.31) after adjusting for other factors, and LBW contributed to an estimated 27% (CI, 3 to 45%) of the population-based prevalence of overt albuminuria. Multivariate models suggest that increasing BMI and blood pressure and decreasing birthweight act in concert to amplify the increases in ACR that accompany increasing age., Conclusions: LBW contributes to renal disease in this high-risk population. The association might be mediated through impaired nephrogenesis caused by intrauterine malnutrition. The renal disease epidemic in Aborigines may partly be the legacy of greatly improved survival of LBW babies over the last four decades. Disease rates should eventually plateau as birthweights continue to improve, if postnatal risk factors can also be contained.

- Abstract
Background: An epidemic of end-stage renal disease (ESRD) is accompanying the rising rates of hypertension, type 2 diabetes and cardiovascular disease among Aborigines in the Northern Territory of Australia. Incidence rates are now 21 times those of nonAboriginal Australians and are doubling every four years. We describe the rates and associations of renal disease in one remote community, which has a current ESRD incidence of 2700 per million, and cardiovascular mortality among the highest in Australia., Methods: Between 1992 and 1995 a community-wide screening program was conducted, in which the urinary albumin/creatinine ratio (ACR) was used as the chief renal disease marker. More than 90% of the population ages five and older participated., Results: Albuminuria was evident in early childhood and increased dramatically with age; 26% of adults had microalbuminuria and 24% had overt albuminuria. All renal failure developed out of a background of overt albuminuria. ACR was significantly correlated with the presence of scabies at screening, with a history of poststreptococcal glomerulonephritis, which is epidemic and endemic in the community, with increasing body wt, blood pressure, glucose, insulin and lipid levels, and with evidence of heavy drinking. ACR was also significantly and inversely correlated with birth weight. As a result of its association with deteriorating hemodynamic and metabolic profiles, increasing ACR was also correlated with increasing cardiovascular risk score. Direct observations showed, and multivariate models predicted, progressive amplification of ACR when multiple risk factors were present simultaneously. Albuminuria also clustered in families., Conclusion: Renal disease in this population is multifactorial, with risk factors related to whole-of-life nutrition, metabolic and hemodynamic profiles, infections, health behaviors, and possibly a family predisposition. Its relationship to low birth weight, and its associations with deteriorating metabolic and hemodynamic profiles, suggest that renal disease is, in part, a component of Syndrome X, which explains the simultaneous increase in metabolic, cardiovascular and renal disease in Aboriginal people. The family clustering might have both environmental and genetic causes, and is under further investigation. Most of the identified risk factors arise out of poverty, disadvantage and accelerated lifestyle change, and the current epidemic can be explained by the confluence of many risk factors in the last few decades. The introduction of effective and sustained programs to address social, economic and educational inequities in all Aboriginal communities, and of screening and renal- and cardiovascular-protective treatment programs for those already afflicted are matters of great urgency.
