Eğitim, temelleri ailede atılan ve okul eğitimi ile devam eden dinamik bir süreçtir. Bu sürecin en önemli kademesi şüphesiz ki ilkokulda verilen eğitimdir. Okuma ve yazma gibi en temel beceriler ilkokulda kazanılmaktadır. Fakat ilkokulda çeşitli sebeplerle en temel beceriler olan okuma yazma ve matematiksel işlemlere ait kazanımlara ulaşamayan öğrencilere de rastlanmaktadır. Akranlarına göre dezavantajlı konumda olan bu öğrencilere yönelik İlkokullarda Yetiştirme Programı (İYEP) geliştirilmiştir. Bu çalışmanın amacı, İYEP'in dezavantajlı öğrenciler için önemini araştırmak ve uygulanan programı bu öğrenci grupları açısından değerlendirmektir. Çalışma, nitel araştırma yöntemi ile yürütülmüştür. Durum çalışması desenlerinden tekli durum çalışması ile İYEP eğitimi alan bir öğrencinin İYEP süreci değerlendirilmiştir. Çalışmada İYEP eğitim süreci, doküman incelemesi yoluyla ayrıntılı betimleme yapılarak analiz edilmiştir. Elde edilen nitel verilere destek olması bakımından ek olarak araştırmacı tarafından geliştirilen yarı yapılandırılmış görüşme formlarıyla program hakkında öğretmen görüşlerine de başvurulmuştur. Çalışmanın örneklemi, Çorum ili Mecitözü ilçesi Doğu İlkokulu'nda İYEP eğitimi alan 1 öğrenci ve yine Çorum ilinde İYEP eğitimi veren 14 öğretmenden oluşmaktadır. Tekli durum çalışmasından elde edilen verilere göre; İYEP eğitimine katılan öğrencinin akademik başarısının eğitim öncesine göre çok daha iyi bir duruma geldiği ortaya konulmuştur. Fakat matematik dersi çarpma ve bölme işlemlerine ait kazanımlarda katılımcı öğrencinin henüz istenilen düzeye ulaşamadığı tespit edilmiştir. Öğretmen görüşlerine göre; İYEP'in dezavantajlı öğrenciler açısından son derece önemli olduğu, programda kullanılan etkinlik kitaplarının daha da zenginleştirilmesi gerektiği, İYEP grupları oluşturulurken öğretmenlerin de fikirlerine başvurulmasının gerekliliği ve programın öğrencilere yönelik psikososyal destek yönüne ağırlık verilmesinin önemi belirtilmiştir. Gerek tekli durum çalışması gerekse öğretmen görüşlerinden elde edile nitel verilere göre; İYEP 'in dezavantajlı öğrenciler açısından hayati öneme sahip bir program olduğu ortaya konulmuş ve araştırma sonunda araştırmacı tarafından programla ilgili bir takım önerilere yer verilerek çalışma tamamlanmıştır. Education is a dynamic process, the foundations of which are laid in the family and continue with school education. The most important stage of this process is undoubtedly the education given in primary school. The most basic skills such as reading and writing are acquired in primary school. However, there are also students who cannot reach the basic skills of literacy and mathematical operations due to various reasons in primary school. A Training Program in Primary Schools (IYEP) has been developed for these students who are at a disadvantage compared to their peers. The aim of this study is to investigate the importance of IYEP for disadvantaged students and to evaluate the applied program in terms of these student groups. The study was carried out with the qualitative research method. The IYEP process of a student who received IYEP training was evaluated with a single case study, one of the case study designs. In the study, the IYEP education process was analyzed by making detailed descriptions through document review. In addition to supporting the qualitative data obtained, teachers' opinions about the program were also consulted with semi-structured interview forms developed by the researcher. The sample of the study consists of 1 student receiving IYEP education in the Eastern Primary School in Mecitözü district of Çorum and 14 teachers giving IYEP education in Çorum. According to the data obtained from the single case study; It has been revealed that the academic success of the student participating in the IYEP education is much better than before the education. However, it has been determined that the participant student has not yet reached the desired level in the achievements of multiplication and division operations in mathematics. According to teacher opinions; It has been stated that İYEP is extremely important for disadvantaged students, the activity books used in the program should be enriched even more, the need to consult teachers' ideas while forming İYEP groups, and the importance of focusing on the psychosocial support aspect of the program for students. According to the qualitative data obtained from both single case study and teacher opinions; It has been revealed that İYEP is a program of vital importance for disadvantaged students and at the end of the research, the study was completed by giving some suggestions about the program by the researcher.

Amaç: Bu araştırmada, 9-11 yaş aralığındaki bireysel ve takım sporlarında faaliyet gösteren sporcuların motorik özelliklerinin karşılaştırılması ve branşlar arasında farklılık olup olmadığının ortaya konulması amaçlanmıştır. Yöntem: Araştırmaya 2019 yılında 23 Nisan Spor Kulübü’nde aktif olarak basketbol branşında faaliyet gösteren 12 erkek çocuk ve Kocaeli Mercan Spor Kulübü’nde aktif olarak yüzme branşında faaliyet gösteren 12 çocuk (6 kız / 6 erkek) toplam 24 çocuk araştırmaya katılmıştır. Araştırmaya katılan her iki branşa boy, vücut ağırlığı, vücut kitle indeksi, dikey sıçrama, flamingo denge, kol hareket sürati, otur eriş, durarak uzun atlama, mekik testleri uygulanmıştır. Bulgular: Elde edilen bulgular sonucunda, araştırma gruplarının parametreleri karşılaştırıldığında boy, vücut ağırlığı, dikey sıçrama, kol hareket sürati, durarak uzun atlama ve vücut kitle indeksi parametrelerinde istatistiksel anlamlı farklılık bulunamamıştır (p>0,05). Flamingo denge, otur eriş ve mekik parametrelerinde istatistiksel anlamlı farklılık bulunmuştur (p, Objective: In this study, it is aimed to compare the motoric characteristics of athletes in the 9-11 age group, who are active in individual and team sports, and to determine whether there is a difference between the branches.Methods: A total of 24 boys participated in the study in 2019 with 12 boys active in the basketball branch of the 23 April Sports Club and 12 children (6 girls / 6 boys) active in the swimming branch of the Kocaeli Mercan Sports Club. Aim: Placenta accreta spectrum (PAS) are major risk factor for obstetric hemorrhage, which is a major cause of fetomaternal mortality and morbidity especially in developing countries. It is aimed to investigate the characteristics, incidence, maternal and fetal outcomes of placenta previa cases with and without PAS. Additionally we intended to analyze the clinical features, risk factors of placenta previa cases presenting with PAS.Material and Methods: A retrospective cohort study was conducted to analyze the pregnancies of placenta previa with and without PAS. Written and electronic maternally-linked medical records between January 2005 and December 2014 were reviewed. Placenta previa diagnosis was confirmed ultrasonographically and PAS were defined clinically as placental adherence to the uterus without easy separationResults: A total of 11351 deliveries were analyzed between January 2005 and December 2014. 387 patients were diagnosed to have placenta previa. Multiple pregnancies were detected in 11 of 387 previa cases and those were excluded. The incidence of placenta previa was 3.41‰ in our institution. The number of gravida ≥4 increased the risk of PAS 1.56 folds, ≥2 previous cesarean section (C/S) 9.74 folds, ≥3 abortus 3.83 folds, gestational hypertension (GHT) by 29.72 folds and gestational diabetes (GDM) 2.49 folds. According to logistic regression analysis ≥3 abortus, ≥2 previous C/S, and GTH were statistically significant.Conclusion: Incases of placenta previa, ≥3 abortion, ≥2 previous C/S and GHT were the most important risk factors in terms of developing PAS. We should consider strict evaluation of placenta previa cases with these risk factors for PAS development during pregnancy may have a decreasing effect on maternal-neonatal morbidity and mortality., Amaç: Plasenta akreata spektrumu (PAS), özellikle gelişmekte olan ülkelerde fetomaternal morbidite ve mortalitenin ana sebebi olan obstetrik kanama için önemli bir risk faktörüdür. Ek olarak, PAS ile başvuran plasenta previa vakalarının klinik özellikleri ve risk faktörlerinin analiz edilmesi amaçlanmıştır.Gereç ve Yöntemler: Plasenta previa ile birlikte PAS olan ve olmayan gebelikleri analiz etmek amacıyla retrospektif kohort bir çalışma planlandı. Ocak 2005 ile Aralık 2014 arasında yazılı ve elektronik olarak tıbbi kayıtlar gözden geçirildi. Risk faktörlerinin lojistik regresyon analizinde ≥3 abort sayısı, ≥2 geçirilmiş sezaryen sayısı ve gestasyonel hipertansiyon varlığı anlamlı olarak değerlendirilmiştir.Sonuç: Plasenta previa vakalarında; ≥3 abort sayısı, geçirilmiş sezaryen sayısının ≥2 ve gestasyonel hipertansiyon varlığı PAS gelişimi açısından en önemli risk faktörleridir. Bu risk faktörlerine sahip plasenta previa olgularının gebeliği boyunca PAS gelişimi açısından sıkı takibi maternal-neonatal morbidite ve mortalite üzerine azalan bir etkiye sahip olabileceğini düşünmekteyiz.

Objective: We aimed to investigate the relationship between the difference betweenthe expected and detected Crown-Rump Length (CRL) values ​​at the 11thto 14th weeks of the gestation and the birth weight of the newborn.Material and Method: Our study was performed retrospectively with 1361 pregnancies whoapplied to our hospital for the first trimester screening test between 2013 and2016 at their 11th to 14th gestational weeks. Patientswere studied in three groups according to the difference between the expectedand the determined CRL size. Group A (n: 266) was conducted if the CRL wassmaller than expected (difference between expected and detected CRL size is 2to 6 days), Group B (n: 695) was conducted if the CRL was normal (difference is1 day less, equal or more than 1 day), Group C (n: 400) was conducted if theCRL was bigger than expected (the difference is 2-6 days less). The differencebetween the detected CRL values and the abnormal birth weight relation wasassessed.Results: The birth weight of the newborn was detected statistically differencebetween only two groups (between Group A and C, between Group B and C).According to this, the birth weight of Group C cases (3534,57 ± 461,76 g) wasstatistically higher than Group A (3271,37 ± 486,94 g) (p = 0.0001). The birthweight of Group C cases (3534,57 ± 461,76 g) was significantly higher thanGroup B (3307,56 ± 352,51 g) (p = 0.0001). There were statistically significantLGA fetuses in Group C (p = 0.0001). The highest LGA ratio was found in Group C(14.5%); and the lowest LGA rate in Group B (2.6%).Conclusion: Detection of expected large or small values in CRL size during earlygestation period should be a warning signal in terms of abnormal birth weightcomplications and these should be followed up more strictly by antenatal followup., Amaç: Çalışmamızda11-14. gebelik haftaları arasında beklenen ve saptanan BPM değerleri arasındakifark ile yenidoğanın doğum ağırlığı arasındaki ilişkiyi araştırmayı amaçladık.Gereç ve Yöntem:2013 - 2016 yılları arasında, 11-14. Sonuç:Erken fetal büyüme döneminde BPM boyutunda beklenenden büyük ya da küçükdeğerlerin saptanması makrozomi ve düşük doğum ağırlığının (LBW) öngörülmesiaçısından uyarıcı bir işaret olabilir ve bu gebeler daha sıkı antenatal takibealınmalıdır.

The aim of this study is to investigate the experience differences of eight and eleven grade students’ related to specializing, generalizing, conjecturing and proving stages of mathematical thinking. For that purpose, were performed 2 worksheet that indicated mathematical thinking stages to 14 eight and 11 eleven grade students. The data structure was analyzed with discriptive analysis. The results that acquired from worksheets and observation during the perform were indicated they are forced to express themselves both mathematically and verbally that either groups progressively throuhg to theproving stage, as well as further at eight grade students., Bu çalısmanın amacı sekizinci ve on birinci sınıf ögrencilerinin matematiksel düsünmenin özellestirme, genelleme, varsayımda bulunma ve ispatlama asamalarındaki yasantı farklılıklarını incelemektir. Bu amaçla 14 sekizinci sınıf ve 11 on birinci sınıf ögrencisine matematiksel düsüncenin üzerinde durulan asamalarını içeren 2 çalısma yapragı uygulanmıstır. Veriler betimsel analiz yöntemi ile analiz edilmistir. Çalısma yapraklarından ve uygulama sırasında yapılan gözlemlerden elde edilen sonuçlar; sekizinci sınıf ögrencilerinin daha fazla olmakla birlikte her iki grubun da ispat asamasına dogru ilerledikçe kendilerini hem matematiksel olarak hem de sözel olarak ifade etmek de zorlandıklarını göstermektedir.

AIM: The aim of the study was to evaluate multiple patient and adenoma variables associated with high-grade dyplasia (HGD) and invasive carcinoma. (which were termed as "advanced pathology (AP)").MATERIALS AND METHODS: In total 6250 processes there were 1379 (22.06 %) polyps from 894 patients. We evaluated 914 polpys which we could find patholojical results.RESULTS: A total of 699 adenomas were detected from 914 polyps. We found more higher percantage of AP features at the rigt-sided adenomas than the left-sided (18.5% vs 7.1%, p: 0.002). According to the logistic regression analysis; male gender, larger or equal 2 cm diameter, villous histology and non-dimunitive gross appearance were found to be the independent risk factors for the AP features of the adenomas.CONCLUSION: In our study, similar to other studies, male gender, larger than 2 cm adenom diameter, villous component and non-dimunitive gross appearance are the independent risk factors for the AP features of any adenomas. However in contrast to most of the other studies we found higher percentage of AP features at the right-sided adenomas than the left-sided ones., Amaç: Bu çalışmanın amacı high-grade displazi (HGD) ve invaziv kanser ( “ilerlemiş patoloji (İP)” olarak tanımlanmıştır) ile ilişkili multipl hasta ve adenom karakterlerini değerlendirmektir. Materyal ve Metod: Toplam 6250 işlem sonunda 894 hastada 1379 (% 22.06) polip mevcuttur. Biz patoloji sonuçlarına ulaşabildiğimiz 914 polipi değerlendirmeye aldık. Bulgular: 914 polipte toplam 699 adenom tespit edildi. A total of 699 adenomas were detected from 914 polyps. Sağ kolon yerleşimli poliplerde sola göre daha yüksek oranda İP özellikleri bulundu (% 18.5 vs % 7.1 p: 0.002). Lojistik regresyon analizi sonucunda; erkek cinsiyet, 2 cm’e eşit ya da büyük boyut, villöz histoloji ve non-dimünitif görünüm adenomlardaki İP özellikleri için bağımsız risk faktörleri olarak bulundu. Sonuç: Çalışmamızda, diğer çalışmalara benzer olarak, erkek cinsiyet, 2 cm’e eşit ya da büyük boyut, villöz histoloji ve non-dimünitif görünüm adenomlardaki İP özellikleri için bağımsız risk faktörleri olarak bulundu. Fakat diğer birçok çalışmanın aksine biz sağ kolon yerleşimli poliplerde sol taraf yerleşimli poliplere göre daha yüksek oranda İP özellikleri bulduk.

Background/aims: A tissue mass derived from mucosa or submucosa and extending into the lumen of the bowel is referred to as a polyp. Removal of polyps detected during colonoscopy and establishment of the pathological diagnosis are recommended since cancer has become the leading lesion and due to the potential cancer risk regardless of polyp size. In this study, we aimed to determine the prevalence of polyps and their histopathological results based on colonoscopies performed in our Unit. We aimed to reveal the polyp prevalence and histopathological results in our colonoscopy experience. Material and Methods: Colonoscopy reports between February 2005 and July 2010 were retrospectively reviewed from the endoscopy archive system in Uludag University, Endoscopy Unit, Department of Gastroenterology. Results: According to the colonoscopy reports, a total of 1379 polyps (22.06%) were detected in 894 patients; 914 of these polyps with attainable pathology results were analyzed. 530 of the patients (59.3%) were male and 364 (40.7%) were female. Of the polyps with neoplastic characteristics, 621 (68%) were tubular adenomas, 66 (7.2%) tubulovillous adenomas, and 5 (0.5%) villous adenomas. Conclusions: In this study, the prevalence of colon polyp types over an approximately six-year period was described in patients undergoing colonoscopy for various reasons in our Endoscopy Unit, which provides healthcare to an important part of the Eastern Marmara region. To determine the prevalence more clearly, especially for determination of prognosis in cases with dysplasia, additional wider and prospective studies are needed., Giriş ve Amaç: Mukoza veya submukozadan köken alan ve barsak lümeni içine uzanan doku kütlesine polip denir. Kolonoskopi sırasında saptanan poliplerin boyutu ne olursa olsun kolorektal kanserin öncü lezyonu ya da potansiyel kanser riski nedeniyle çıkartılması ve patolojik tanısının konması önerilmektedir. Biz bu çalışmamızda bölümümüzde yapılan kolonoskopi sonuçlarında saptanan polip prevelansı ve bu poliplerin histopatoloji sonuçlarını ortaya koymayı amaçladık. Gereç ve Yöntem: Uludağ Üniversitesi Tıp Fakültesi Gastroenteroloji Bilim Dalı Endoskopi Ünitesi'nde fiubat-2005 ile Temmuz-2010 arasındaki kolonoskopi raporları endoskopi arşiv sisteminden retrospektif olarak tarandı. Bulgular: Kolonoskopi raporlarına göre 894 hastada toplam 1379 adet polip (%22,06) tespit edildi. Bu poliplerden patoloji sonucuna ulaşılabilen 914 tanesi incelemeye alındı. Hastaların 530'u erkek (%59,3), 364'ü kadın (%40,7) idi. Neoplastik özellikte olan poliplerin 621 tanesi (%68) tubuler adenom, 66 tanesi (%7,2) tubulovillöz adenom, 5 tanesi de (%0,5) villöz adenom olarak saptandı. Sonuç: Bu çalışmada Doğu Marmara Bölgemizin önemli bir kesimine hizmet veren Endoskopi ünitemizde yaklaşık 6 sene boyunca çeşitli nedenlerle kolonoskopi yapılan hastalarımızdaki kolon polip tiplerinin prevelansı ortaya konmuştur. Prevelansın daha net belirlenmesi, özellikle displazi saptanan olgulardaki prognozun ortaya konması için daha geniş çapta ve prospektif çalışmalara ihtiyaç vardır.

Ektopik gebelik halen maternal mortalitenin önemli nedenlerinden biri olmaya devam etmektedir. Yardımla Üreme Teknikleri (YÜT)' nin yaygın ve sık kullanılması ile birlikte anormal yerleşimli gebeliklerin sıklığı da artmaktadır. Bu yazıda 39 yaşında sekonder infertil ve bilateral salpenjektomili hastaya uygulanan intrasitoplazmik sperm injeksiyonu embryo transferi sonrası sağ tubal stump ta meydana gelen ektopik gebelik vakasını sunduk. Maternal morbidite ve mortaliteyi azaltmak amacıyla erken tanı için İVF sikluslarında bilateral salpenjektomi yapılmış olsa da intrauterin gebelik izlenmediği durumlarda ayırıcı tanıda mutlaka stumpta ektopik gebelik düşünülmelidir., Ectopic pregnancy continues to be one of the important reasons for maternal mortality. With the widespread and frequent use of Assisted Reproductive Techniques (ART), the frequency of abnormally located pregnancies also rises. In this article, we presented a case of an ectopic pregnancy occurring in the right tubal stump after an intracytoplasmic sperm injection – embryo transfer performed on a 39 year-old patient with secondary infertility and bilateral salphingectomy. Although a bilateral salphingectomy was performed in IVF cycles for an early diagnosis in order to decrease maternal morbidity and mortality, an ectopic pregnancy in the stump should be definitely taken into consideration in cases where no intrauterine pregnancy is observed.

Başkent Üniversitesi Ankara hastanesi üroloji kliniğine 2004-2013 yılları arasında alt üriner sistem semptomları ve PSA yüksekliği olan TRUS eşliğinde prostat iğne biyopsisi yapılan 563 hasta retrospektif olarak incelendi.Çalışmamıza dahil edilen hastaların tanı sırasındaki yaşları 42 ile 89 arasında olup bu hastaların yaş ortalaması 63,4 'dü. Hastalar yaş grubuna göre dört gruba ayrıldı .Bu yaş gruplarında serbest - total PSA, transizyonel zon hacmi, total prostat hacmi ve oranları incelendi. Ortaya çıkan bulgular ise ortalama PSA 10.08 ng/ml, ortalama serbest PSA 2.1 ng/ml, ortalama prostat hacmi 64.93 ml, ortalama transizyonel prostat hacmi 47.47, ortalama serbest total PSA oranının ortalaması % 20.14, transizyonel prostat hacminin tüm prostat hacmine oranının ortalaması % 71.32 olarak bulundu. Hastaların yaş ile prostat hacimleri arasında ileri derecede korelasyon olduğu görüldü. Yaş artışı ile özellikle serbest PSA arasında korelasyon olduğu görüldü. Ancak serbest/total psa ile transizyonel zon hacmi/total prostat hacmi arasındaki ilişki istatiksel olarak anlamlı ancak yüksek derecede korelasyon göstermemektedir. Bu sonuçlar ışığında prostat hacimlerini gösterebilmek için en uygun yaklaşım TRUS'dir. Ancak TRUS yapılma imkanı olmadığı zaman prostat volümünü göstermek için özellikle serbest PSA düzeyleri kullanılabilir The patients included in the study were between the ages of 42 and 89 at the time of diagnosis, and the mean age was 63,4. In these groups, free - total PSA, transition zone volume, total prostate volume and rate were analyzed.The clinical findings were as follows : Mean PSA 10.08 ng/ml, mean free PSA 2.1 ng/ml, mean prostate volume 64.93 ml, mean transitional zone volume 47.47, mean free to total PSA rate % 20.14, mean ratio of transitional prostate volume to the whole prostate volume % 71.32.The age and prostate volume of patients were significantly correlated with increasing age, especially free PSA is found to be correlated. However, the relation between Free/Total PSA and transition zone volume/ total prostate volume was statistically significant but not highly correlated.According to these results, the best approach to show the prostate volume is TRUS. However, when there is no possibility of applying TRUS, free PSA levels can be used to estimate the prostate volumes. 71

Kronik hepatit C'li hemodiyaliz hastalarında, hepatit C virus (HCV) enfeksiyonu prevalansı genel popülasyona göre belirgin derecede daha yüksektir. Bu hastalarda HCV enfeksiyonu önemli bir morbidite ve mortalite sebebidir. Aynı zamanda hemodiyalize giren kronik hepatit C hastalarında okült hepatit B virus (HBV) enfeksiyonu riski de artmıştır. Bu çalışmada hemodiyalize giren kronik hepatit C' li hastalarda pegile interferon alfa-2a monoterapisine hızlı ve erken virolojik yanıt oranlarının saptanması ve okült HBV enfeksiyonunun bu hasta grubunda tedaviye etkisinin olup olmadığının araştırılması amaçlanmıştır.Çalışmaya 11 hemodiyaliz (3 kez/hafta) hastası alındı. Tüm hastalar interferon naiv, anti-HCV pozitif, polimerize zincir reaksiyonu ile HCV RNA'sı pozitif ve karaciğer fibrozis skoru ?2 olan hastalar idi. Tüm hastalara pegile interferon alfa-2a, 135 µg dozunda, subkutan, haftada bir kez uygulandı. İlaveten, tedavi öncesi dönemde, çalışmaya alınan HbsAg'si negatif, anti- Hbs antikoru pozitif olan hastalara okült HBV enfeksiyonun saptanması için serum HBV DNA testi de yapıldı. Tedavinin 4. ve 12. haftasında hızlı virolojik yanıt (HVY) ve erken virolojik yanıt (EVY) oranları incelendi. Ayrıca okült HBV enfeksiyonu olan hastalarda tedavinin 12. haftasında HBV DNA testi tekrarlandı ve kronik hepatit C tedavisine etkisi incelendi.Sırasıyla, HVY ve EVY, 5 hastada (% 45.5) ve 10 hastada (%90.9) elde edildi. Tedavi öncesi prediktif faktörler olan kadın cinsiyet, 40 yaş altı olma, serum alanin aminotransferaz (ALT) düzeyi, viral yük, karaciğer fibrozis skoru ile HVY arasında ilişki bulunamadı. Hasta sayısı yetersiz olduğu için EVY ile prediktif faktörler arasındaki ilişki incelenmedi. Sadece 1 hastada (%9.1) EVY elde edilemedi ve bu hastada genotip 4 idi. Sırasıyla, tedavinin 4. ve 12. haftasında, 9 (%81.8) hastada ve 5 (%45.5) hastada ilaca bağlı yan etki gelişti. Sadece 1 (%9.1) hasta yan etki nedeniyle tedaviyi bıraktı. Okült HBV enfeksiyonu saptanması için 6 hastada HBV DNA çalışılabildi. Sadece 1 (% 16.7) hastada okült HBV enfeksiyonu saptandı. Bu hastada tedavinin 12. haftasında HBV DNA' sı negatif tespit edildi.Normal böbrek fonksiyonlu kronik hepatit C hastalarına göre hemodiyaliz hastalarında pegile interferon alfa-2a monoterapisi ile daha yüksek HVY ve EVY elde etmek mümkündür. Özellikle renal transplantasyon öncesi dönemde, bu hastaların yakın klinik ve laboratuvar takibi ile pegile inteferon alfa-2a monoterapisi güvenle kullanılabilir. Okült HBV enfeksiyonun hemodiyalize giren kronik hepatit C'li hastalar arasında daha sık olabileceği göz önünde bulundurulmalıdır. Prevalence of hepatitis C virus (HCV) infections are appearance significantly more than the normal population to heamodialysis patients with chronic hepatitis C. HCV infection is an important cause of morbidity and mortality among in this patients. At the same time, the risk factor of in heamodialysis patients with chronic HCV infection has been increased for occult hepatitis B virus (HBV) infection. The aim of this study has been evaluated the rapidly virologic response (RVR) and early virologic response (EVR) of pegylated interferon alpha-2a and whether occult HBV infection are impacted for treatment on this patients or not.Eleven heamodialysis patients (3 sesion/ weekly) were included into the study. All of the patients were interferon naive, anti-HCV antibodies positive, polymerase chain reaction HCV RNA positive and liver fibosis score ? 2. Pegylated interferon alpha-2a, 135 µg, subcutaneous, weekly was applied to all of the patients. Additionally, the patients who had negative HbsAg and anti-HCV positive were tested polymerase chain reaction HBV DNA. In the 4th and 12th week of treatment RVR and EVR were investigated. Separately, HBV DNA test were repeated in the 12th week of treatment in patients with occult HBV infection and influences were investigated on the treatment of chronic hepatitis C.RVR and EVR were observed in 5 patients (45,5 %) and 10 patients (90,9 %), respectively. At the stage of pretreatment, predictive factors are such female sex, less than 40 years-old, serum alanine aminotransferase (ALT) level, viral load, liver fibrosis score were not associated with RVR. Since the number of patients were insufficient, it was not investigated between EVR and predictive factors. The only 1 patient (9.1 %) who did not have any EVR had genotype 4. In the 4th and 12th week of treatment, adverse effect was developed on 9 patients (81.9 %) and 5 patients (45.5, %), respectively. The only 1 patient (9.1 %) was taken out of treatment due to adverse effect. It was applied to 6 patients for HBV DNA test and it was detected on only 1 patient (16.7 %). In this patient, it was observed HBV DNA negative in 12th week.It was obtained with high rate for RVR and EVR on the heamodialysis patients with chronic hepatitis C rather than the normal renal function with chronic hepatitis C with pegylated interferon alpha-2a monotherapy. Especially, before the stage of renal transplantation, pegylated interferon alpha-2a monotherapy can be used on this patients by close to follow up clinically and laboratuary. It should be considered that occult HBV infections are among in heamodialysis patients with chronic hepatitis C. But course of occult HBV infection and impact of chronic hepatitis C on treatment are needed to be studied with high number of patients by prospectively.Key words: Heamodialysis, chronic hepatitis C, pegylated interferon alpha-2a, rapidly and early virological response, occult hepatitis B infection. 59

A single dose of infliximab administration for a patient with fistulating Crohn's disease led to a fatal course after disseminated tuberculosis and fungal sepsis. Tumor Necrosis Factor–α antagonists are known to have administrative indication in romatoid arthritis, Crohn's disease and psoriasis. However, many important complications may be seen during their usage, especially tuberculosis. In this report, the case was discussed, the literature was reviewed and main points with consensus were presented., Fistülizan tip Crohn hastalığı nedeni ile tek doz infliksimab kullanılan bir hastada gelişen yaygın tüberküloz enfeksiyonu ve ciddi enfeksiyonlar sonrası fatal seyir izlenmişti. Tümör Nekrozis Faktör-α antagonistlerinin romatoid artrit, Crohn hastalığı ve psöriaziste kullanım endikasyonları vardır. Ancak bu ajanların kullanımına bağlı başta tüberküloz olmak üzere birçok önemli komplikasyon görülebilmektedir. Sunduğumuz olgunun ışığında oldukça güncel olan bu konudaki literatür tekrar değerlendirilmiş ve konsensus sağlanan görüşler sunulmuştur.

The objective of this study was to obtain epidemiologic data about the patients who were admitted to Emergency Internal Medicine Unit, consultations that were made by internal medical clinics, their diagnosis and the ratio of hospitalization. The data of the patients who were admitted to Emergency Internal Medicine Unit of Uludag University Medical Faculty Hospital in a period of 12 months, between June 1, 2002 and May 31, 2003 were obtained from the Emergency Internal Medicine Unit Daily Patient Charts. Major symptoms of the patients, consultations of internal medical clinics, diagnosis and hospitalization ratios were analyzed retrospectively.In 12 months, 21934 patients were admitted to the Emergency Department and 6751 of them (30.7%) referred to Emergency Internal Medicine Unit. They were consulted 8732 times, 4748 of them (70.3%) consulted by only one clinic and 2003 of them consulted by two or more clinics. 2657 patients (39.4%) were hospitalized into different internal medical clinics, 107 patients (%1.6) were hospitalized into different surgery clinics, 506 patients (7.5%) were transferred to other hospitals, 35 patients (0.5%) died in Emergency Department and the other 3446 patients (%51.0) were discharged after their treatment were planned. The patients were consulted most frequently by Internal Medicine (2796), Cardiology (2268) and Neurology (1605) clinics and the most frequent major symptoms were chest pain (898), dyspnea (782) and altered level of consciousness (447), the most frequent diagnoses were various infectious diseases (785), acute coronary syndromes (462), intoxications (417) and emergencies related to oncologic-haematologic malignity (358)., Çalışmanın Amacı Acil Dahiliye Ünitesine (ADÜ) başvuran hastalara yapılan dahili bölüm konsültasyonları, konulan tanılar ve hospitalizasyon oranları hakkında epidemiyolojik bilgiler edinmektir. 1.6.2002 ile 31.5.2003 tarihleri arasındaki 12 aylık sürede Uludağ Üniversitesi Tıp Fakültesi ADÜ'ne başvuran hastaların verileri ADÜ Günlük Hasta Kayıt Formları incelenerek elde edildi. Hastaların major başvuru semptomları, konsülte eden bölümler, en sık konulan tanılar ve hospitalizasyon oranları retrospektif olarak araştırıldı. 12 aylık sürede acil servise 21934 kişi başvurmuş, bunlardan 6751'i (%30.7) ADÜ'ne devredilmiştir. Bu hastaların 4748'ine (%70.3) sadece bir, 2003'üne (%29.7) ise iki veya daha fazla bölüm tarafından olmak üzere toplam 8732 konsültasyon yapılmıştır. Hastaların 2657'si (%39.4) çeşitli dahili kliniklere yatırılmış, 107'si (%1.6) çeşitli cerrahi kliniklere yatırılmış, 506'sı (%7.5) başka hastanelere sevkedilmiş, 35'i (%0.5) acil serviste eksitus olmuş ve geri kalan 3446 (%51.0) hastanın ise tedavileri düzenlenip taburcu edilmiştir. En çok konsültasyon yapılan bölümler; İç Hastalıkları (n:2796), Kardiyoloji (n:2268) ve Nöroloji (n:1605), en sık majör başvuru semptomları; göğüs ağrısı (n:898), nefes darlığı (n.782) ve bilinç bozuklukları (n:447), en sık konulan tanılar ise; çeşitli enfeksiyonlar (n:785), akut koroner sendrom (n:462), intoksikasyonlar (n:417) ve onkolojik-hematolojik malignite acilleri (n:358) olmuştur.

ÖZET Selim Veya Habis Adneksiyal Tümörlerin Ayırımında UHrasonografik Görüntüleme ve Renkli Doppler Uhrasonograünin Yeri AMAÇ: Selim ve habis adneksiyal tümörlerin ayıncı tanısında ultrasonograph görüntüleme ve renkli doppler ultrasonogram ile postoperatif histopatolojik tanılan arasındaki ilişkiyi irdelemek ve adneksiyal kitlelerin ayırıcı tanısında bu iki yöntemin birbirlerine olan üstünlüklerini karşılaştırmaktır. MATERYAL ve METOD: Çalışma 1996-1997 yıllannda arasında I.Û. Cerrahpaşa Tıp Fakültesi Kadın Hastalıktan ve Doğum Anabilim Dalında yapıldı. Adneksiyal kitle nedeni ile ameliyat edilecek olan 100 olgu çalışma kapsamına alındı. Bu olgulara ameliyat öncesi ultrasonogram görüntüleme ve renkli doppler ultrasonogram uygulandı. UHrasonografik değerlendirmede adneksiyal kitleler basit kist, septalı kist, papiller kist, kompleks kist, solid küle olarak gruplara ayrıldı. Basit kistler selim, septalı, papiller, kompleks kist ve solid kitleler habis olarak değerlendirildi.Renkli kan akımı saptanan kitlelerde doppler çalışması yapıldı. Doppler ölçümlerinde Rl ve Pl hesaplandı. Ameliyat sırasında çıkanlan kitlelerin histopatolojik tanılan ile ultrasonogram görüntüsü ve renkli doppler bulguları karşılaştınldı. BULGULAR: Histopatolojik değerlendirmede ultrasonogram olarak basit kistlerin %95(42/44)rınin selim, kompleks kistlerin %9Q(19/21)'nmın habis adneksiyal kitleler olduğu saptandı. Adneksiyal kitlelerin %78'inde renkli kan akımı saptandı. Renkli dopplerde kan akımı saptanamayan olgulann tümünün histopatolojik olarak selim olduğu tespit edildi. Selim adneksiyal kitlelerin ortalama Rl değerleri 0.68 ± 0.16, Pl değerleri 1.41 ± 0.69, habis adneksiyal kitlelerin ortalama Rl değerleri 0.48 ±0.17 ve Pl değerleri 0.78 ± 0.46 olarak hesaplandı. Aralanndaki fan/ istatistiksel olarak anlamlı olarak bulundu. Rl için eşik değer 0.40 olarak tespit edildi. Ultrasonogram morfolojik değerlendirmenin duyarlılığı %94, özgüllüğü %63, renkli doppler ultrasonogram incelemesinin duyarlılığı %35, özgüllüğü %95 olarak saptandı. Her iki yöntem aynı yönde bulgu verdiği zaman duyarlılığın %92, özgüllüğün %95, yöntemler zıt yönde bulgu verdiğinde ise adneksiyal kitlenin selim ya da habis olma ihtimalinin %S0 olduğu görüldü. SONUÇ. Adneksiyal tümörlerin ayıncı tanısında ultrasonogram morfolojik değerlendirmenin duyarlılığının yüksek, renkli doppler ultrasonograph ise özgüllüğünün yüksek olduğu görüldü. Renkli doppler ultrasonogram yönteminin, selim ve habis adneksiyal kitlelerin Rl ve Pl değerlerinin üst üste çakışmasından dolayı duyarlılığının düşük olduğu tespit edildi. 42 ABSTRACT Diagnostic Value of Ultrasonography and Color Doppler Ultrasonography in Adnexal Tumors OBJECTIVE: The comparison of conventional ultrasonography and color doppler ultrasonography with their value and effectiveness in differential diagnosis of benign and malign adnexal tumors and their relation to the postoperative histopathological diagnosis is the main purpose of this study. SUBJECTS AND METHODS: The study was accomplished in l.U. Cerrahpaşa Medical Faculty Obstetrics and Gynecology Department in 1996-1997. One hundred cases mat would be operated with the diagnosis of adnexal mass were included in the study. Conventional ultrasonography and color doppler ultrasonography were performed for all of the cases preoperative^. In conventional ultrasonography, the adnexal masses were divided into groups of simple cyst, septated cyst, papillary cyst, complex cyst and solid mass. Simple cysts were assumed to be benign whereas the ather cysts and solid masses were accepted to be malign. When color now was determined, doppler studies were completed. Rl and PI values were calculated as doppler studies. The histopathological diagnosis of the masses that were extirpated in the operation and preoperative ultrasonograph^! morphology and color doppler results were compared. RESULTS: In the histopathological assesment, 95% (42/44) of the ultrasonographically simple cysts were benign, while 90% (19/21) of the complex cysts were malign. Color now was detected in 78% of the adnexal masses studied. All of the masses that detected had no color now in doppler ultrasonography, were histopathologically benign. Benign adnexal masses had a mean Rl value of 0.68±0. 16 and PI value 1.41±0.69. Malign adnexal masses had a mean Rl value of 0.48±0.17 and PI value 0.78±0.46. The difference between Rl and PI values of benign and malign masses were statistically significant. The cut-off value of Rl were calculated as 0.40. The sensitivity and specificity of conventional ultrasonography in the differential diagnosis of adnexal masses were 94% and 63% respectively, and on the contrary these values were 35% and 95% for color doppler ultrasonography. When these two diagnostic methods were utilized together, the sensitivity was 92% and the specificity 95%. When the results of the methods were opposite on determining whether the mass was benign or malign, the ability of the methods in differential diagnosis decreased to 50%. Background/Aims: To investigate the relationship between gamma-glutamyl transpeptidase (GGT) levels and histopathological status determined by biopsy in patients with chronic hepatitis B and C. Materials and Methods: Patients with chronic hepatitis B and C who were referred to the Uludag University Faculty of Medicine Gastroenterology outpatient clinic between January 2005-January 2011 and underwent liver biopsy were included in the study. Overall, 246 patients with hepatitis B and 151 patients with hepatitis C were enrolled. According to the evaluation based on the Ishak score, patients with a histological activity index (HAI) between 0-12 were defined as low activity, and those with an HAI between 13-18 were defined as high activity. In addition, patients with a fibrosis score of 0-2 were defined as low fibrosis, and those with a score between 3-6 were defined as high fibrosis; comparisons were made accordingly. Wandering spleen is the displacement of the spleen from its normal location due to the loss or weakening of ligaments that hold the spleen in the left upper quadrant. The possibility of torsion of the spleen is high due to the long and mobile nature of the vascular pedicle. Generally, cases are asymptomatic. Under conditions of delayed diagnosis, symptoms of splenomegaly, left portal hypertension, gastric fundal varices, and hypersplenism. may present as a result of development of vascular congestion associated with chronic torsion. There are only a few cases in the literature reporting the association of wandering spleen and fundal vat-ices. We report herein the case of a 55-year-old female who admitted to our clinic with complaints of fatigue and epigastric pain. She was determined to have gastric fundal varices and hypersplenism secondary to the development of left portal hypertension due to chronic splenic torsion.

Background: It has been recently reported that serum paraoxonase (PON1) and arylesterase (ARE) activities may be significantly reduced in patients with chronic liver disease. The aim of the study was to investigate the relations between serum PON1 and ARE activities and the degree of liver damage in patients with chronic liver injury. Methods: We studied a total of 75 patients with chronic liver disease (50 patients with cirrhosis and 25 patients with chronic hepatitis) and 25 healthy comparison subjects. Baseline and salt-stimulated PON1 and ARE activities were determined in all study participants. Results: Baseline and stimulated PON1 and ARE activities were significantly lower in patients with chronic liver disease than in controls. Background: Caspase-cleaved cytokeratin 18 (CK18-Asp396) is released from hepatocytes during apoptosis. Recent studies have indicated that serum levels of CK18-Asp396 could be clinically useful biomarker of chronic liver disease. To shed more light on the rate of hepatocyte loss by apoptosis in chronic liver disease, serum levels of CK18-Asp396 were examined in patients with nonalcoholic steatohepatitis (NASH) and chronic hepatitis C. Helicobacter pylori which is one of the commonly seen chronic bacterial infections in the world, has been demonstrated to have a relationship with chronic gastritis, peptic ulcer disease and gastric cancer. Current management of H.pylori infection involves the use of a proton pump inhibitor (PPI) and any two of amoxicillin, clarithromycin and metronidazole in combination. Antibiotic resistance which is in an increasing trend in H.pylori since the recent years, is the main cause of treatment failure. This study was conducted to determine the susceptibility of 31 H.pylori strains to several antibiotics by using E-test method (AB Biodisk, Sweden) and also to detect clarithromycin resistance by fluorescent in situ hybridization (FISH; SedFAST (R), Hungary). The strains were isolated from the gastric biopsy specimens of patients who were admitted to Uludag University Hospital, Bursa, Turkey with dyspeptic complaints. Clarithromycin, amoxycillin, metronidazole, tetracycline and ciprofloxacin resistance rates were as 41.9%, 3.2%, 41.9%, 3.2% and 45.2%, respectively. Resistance to single antibiotic was detected in 32.2% of the isolates whereas multiresistance was seen in 45.2%. For the hybridization process one probe specific for 16S rRNA and labeled with a fluorescein dye and the other probe specific for the mutations in 23S rRNA and labeled with Cy3 stain were used. Green signalling denoted presence of H.pylori in the specimen and red signalling was associated with clarithromycin resistance. All of the isolates yielded green signalling and the 13 isolates found to be resistant to clarithromycin by E-test, gave red signalling. No difference was detected between the two methods in terms of clarithromycin resistance determination. This was a preliminary study reporting the H.pylori resistance rates in our region, however, further larger scale studies are required for obtaining countrywide data.
