1. Disposal of animal healthcare services waste in southern Brazil: One Health at risk

Abstract

ABSTRACT Brazilian animal livestock is in full expansion but linked to the use of veterinary drugs which may be considered as emerging contaminants. The study aimed to characterize the disposal of Animal Healthcare Service Waste (AHSW), especially of veterinary drugs, besides identifying the most used drugs in the city of São Joaquim municipality, southern Brazil. Eighty-four cattle ranchers were interviewed through a structured questionnaire with closed questions about drugs use and disposal of the AHSW. The active ingredients of drugs most used were the antiparasitic ivermectin (68% of properties) and the antimicrobial oxytetracycline (48%). After the use, the disposal of the AHSW, including expired drugs, leftovers and wrapper, was the disposal next to the domestic trash, burned and deposited in the soil. The Chi square test showed association between the age of the participants and the disposal of sharp objects (X2 = 36,36, p= 0,020), showing that farmers aged above 60 years usually reuse, the ones who are 20 years old return to the place where they acquired the material and the rest adopts improper practices. The disposal practices of the AHSW adopted in the properties are still in disagreement with the current Brazilian legislation and may cause adverse effects on human, animal and environmental health.

Abstract

Alzheimer's disease (AD), including its mild cognitive impairment (MCI) phase that may or may not progress into the AD, is the most ordinary form of dementia. It is extremely important to correctly identify patients during the MCI stage because this is the phase where AD may or may not develop. Thus, it is crucial to predict outcomes during this phase. Thus far, many researchers have worked on only using a single modality of a biomarker for the diagnosis of AD or MCI. Although recent studies show that a combination of one or more different biomarkers may provide complementary information for the diagnosis, it also increases the classification accuracy distinguishing between different groups. In this paper, we propose a novel machine learning-based framework to discriminate subjects with AD or MCI utilizing a combination of four different biomarkers: fluorodeoxyglucose positron emission tomography (FDG-PET), structural magnetic resonance imaging (sMRI), cerebrospinal fluid (CSF) protein levels, and Apolipoprotein-E (APOE) genotype. The Alzheimer's Disease Neuroimaging Initiative (ADNI) baseline dataset was used in this study. In total, there were 158 subjects for whom all four modalities of biomarker were available. Of the 158 subjects, 38 subjects were in the AD group, 82 subjects were in MCI groups (including 46 in MCIc [MCI converted; conversion to AD within 24 months of time period], and 36 in MCIs [MCI stable; no conversion to AD within 24 months of time period]), and the remaining 38 subjects were in the healthy control (HC) group. For each image, we extracted 246 regions of interest (as features) using the Brainnetome template image and NiftyReg toolbox, and later we combined these features with three CSF and two APOE genotype features obtained from the ADNI website for each subject using early fusion technique. Here, a different kernel-based multiclass support vector machine (SVM) classifier with a grid-search method was applied. Before passing the obtained features to the classifier, we have used truncated singular value decomposition (Truncated SVD) dimensionality reduction technique to reduce high dimensional features into a lower-dimensional feature. As a result, our combined method achieved an area under the receiver operating characteristic (AU-ROC) curve of 98.33, 93.59, 96.83, 94.64, 96.43, and 95.24% for AD vs. HC, MCIs vs. MCIc, AD vs. MCIs, AD vs. MCIc, HC vs. MCIc, and HC vs. MCIs subjects which are high relative to single modality results and other state-of-the-art approaches. Moreover, combined multimodal methods have improved the classification performance over the unimodal classification.

Abstract

Mild cognitive impairment (MCI) is a transitional state between the cognitive changes in normal aging and Alzheimer’s disease (AD), which induces abnormalities in specific brain regions. Previous studies showed that paired helical filaments Tau (PHF-Tau) protein is a potential pathogenic protein which may cause abnormal brain function and structure in MCI and AD patients. However, the understanding of the PHF-Tau protein network in MCI patients is limited. In this study, 225 subjects with PHF-Tau Positron Emission Tomography (PET) images were divided into four groups based on whether they carried Apolipoprotein E ε4 (APOE 4) or abnormal cerebrospinal fluid Total-Tau (CSF T-Tau). They are two important pathogenic factors that might cause cognitive function impairment. The four groups were: individuals harboring CSF T-Tau pathology but no APOE 4 (APOE 4−T+); APOE 4 carriers with normal CSF T-Tau (APOE 4+T−); APOE 4 carriers with abnormal CSF T-Tau (APOE 4+T+); and APOE 4 noncarriers with abnormal CSF T-Tau (APOE 4−T−). We explored the topological organization of PHF-Tau networks in these four groups and calculated five kinds of network properties: clustering coefficient, shortest path length, Q value of modularity, nodal centrality and degree. Our findings showed that compared with APOE 4−T− group, the other three groups showed different alterations in the clustering coefficient, shortest path length, Q value of modularity, nodal centrality and degree. Simultaneously, voxel-level analysis was conducted and the results showed that compared with APOE 4−T− group, the other three groups were found increased PHF-Tau distribution in some brain regions. For APOE 4+T+ group, positive correlation was found between the value of PHF-Tau distribution in altered regions and Functional Assessment Questionnaire (FAQ) score. Our results indicated that the effects of APOE 4 and abnormal CSF T-Tau may induce abnormalities of PHF-Tau protein and APOE 4 has a greater impact on PHF-Tau than abnormal CSF T-Tau. Our results may be particularly helpful in uncovering the pathophysiology underlying the cognitive dysfunction in MCI patients.

Abstract

Collecting multiple modalities of neuroimaging data on the same subject is increasingly becoming the norm in clinical practice and research. Fusing multiple modalities to find related patterns is a challenge in neuroimaging analysis. Canonical correlation analysis (CCA) is commonly used as a symmetric data fusion technique to find related patterns among multiple modalities. In CCA-based data fusion, principal component analysis (PCA) is frequently applied as a preprocessing step to reduce data dimension followed by CCA on dimension-reduced data. PCA, however, does not differentiate between informative voxels from non-informative voxels in the dimension reduction step. Sparse PCA (sPCA) extends traditional PCA by adding sparse regularization that assigns zero weights to non-informative voxels. In this study, sPCA is incorporated into CCA-based fusion analysis and applied on neuroimaging data. A cross-validation method is developed and validated to optimize the parameters in sPCA. Different simulations are carried out to evaluate the improvement by introducing sparsity constraint to PCA. Four fusion methods including sPCA+CCA, PCA+CCA, parallel ICA and sparse CCA were applied on structural and functional magnetic resonance imaging data of mild cognitive impairment subjects and normal controls. Our results indicate that sPCA significantly can reduce the impact of non-informative voxels and lead to improved statistical power in uncovering disease-related patterns by a fusion analysis.

Abstract

Recent developments of tau Positron Emission Tomography (PET) allows assessment of regional neurofibrillary tangles (NFTs) deposition in human brain. Among the tau PET molecular probes, 18F-AV1451 is characterized by high selectivity for pathologic tau aggregates over amyloid plaques, limited non-specific binding in white and gray matter, and confined off-target binding. The objectives of the study are (1) to quantitatively characterize regional brain tau deposition measured by 18F-AV1451 PET in cognitively normal older adults (CN), mild cognitive impairment (MCI), and AD participants; (2) to evaluate the correlations between cerebrospinal fluid (CSF) biomarkers or Mini-Mental State Examination (MMSE) and 18F-AV1451 PET standardized uptake value ratio (SUVR); and (3) to evaluate the partial volume effects on 18F-AV1451 brain uptake.Methods: The study included total 115 participants (CN = 49, MCI = 58, and AD = 8) from the Alzheimer's Disease Neuroimaging Initiative (ADNI). Preprocessed 18F-AV1451 PET images, structural MRIs, and demographic and clinical assessments were downloaded from the ADNI database. A reblurred Van Cittertiteration method was used for voxelwise partial volume correction (PVC) on PET images. Structural MRIs were used for PET spatial normalization and region of interest (ROI) definition in standard space. The parametric images of 18F-AV1451 SUVR relative to cerebellum were calculated. The ROI SUVR measurements from PVC and non-PVC SUVR images were compared. The correlation between ROI 18F-AV1451 SUVR and the measurements of MMSE, CSF total tau (t-tau), and phosphorylated tau (p-tau) were also assessed.Results:18F-AV1451 prominently specific binding was found in the amygdala, entorhinal cortex, parahippocampus, fusiform, posterior cingulate, temporal, parietal, and frontal brain regions. Most regional SUVRs showed significantly higher uptake of 18F-AV1451 in AD than MCI and CN participants. SUVRs of small regions like amygdala, entorhinal cortex and parahippocampus were statistically improved by PVC in all groups (p < 0.01). Although there was an increasing tendency of 18F-AV-1451 SUVRs in MCI group compared with CN group, no significant difference of 18F-AV1451 deposition was found between CN and MCI brains with or without PVC (p > 0.05). Declined MMSE score was observed with increasing 18F-AV1451 binding in amygdala, entorhinal cortex, parahippocampus, and fusiform. CSF p-tau was positively correlated with 18F-AV1451 deposition. PVC improved the results of 18F-AV-1451 tau deposition and correlation studies in small brain regions.Conclusion: The typical deposition of 18F-AV1451 tau PET imaging in AD brain was found in amygdala, entorhinal cortex, fusiform and parahippocampus, and these regions were strongly associated with cognitive impairment and CSF biomarkers. Although more deposition was observed in MCI group, the 18F-AV-1451 PET imaging could not differentiate the MCI patients from CN population. More tau deposition related to decreased MMSE score and increased level of CSF p-tau, especially in ROIs of amygdala, entorhinal cortex and parahippocampus. PVC did improve the results of tau deposition and correlation studies in small brain regions and suggest to be routinely used in 18F-AV1451 tau PET quantification.

6. CASOS DE INTOXICAÇÃO POR AGROTÓXICOS EM MALEICULTORES E ANÁLISE DOS RESPECTIVOS PRONTUÁRIOS DE ATENDIMENTO

Abstract

Atualmente, o Brasil é líder mundial no consumo de agrotóxicos. O aumento do uso e manuseio desses produtos químicos pode provocar o aumento nos casos de intoxicações humanas, oferecendo risco à saúde do trabalhador rural e ao próprio ambiente. A expansão crescente da agricultura na Região da Serra Catarinense provoca o consequente incremento do uso de agrotóxicos, que pode culminar com o aumento dos casos de intoxicações e colocar em risco à saúde do trabalhador rural. O objetivo desse estudo foi levantar possíveis casos de intoxicação por agrotóxicos em maleicultores de um município da Serra Catarinense e comparar as informações coletadas com seus respectivos prontuários de atendimento médico na Unidade Básica de Saúde (UBS) que atende a região estudada. Foram entrevistados 66 maleicultores dos quais 11 manifestaram intoxicação por agrotóxicos. Os resultados demonstraram que os indivíduos que informaram ter sofrido intoxicação por agrotóxicos apresentaram no mínimo dois sintomas, sendo que os mais comuns foram cefaleia e irritação da pele, relacionados principalmente, ao uso de Mancozebe, Fenitrotiona e Difenoconazol. Na UBS, Foram encontrados quatro prontuários referentes a esses indivíduos, entretanto em nenhum deles o diagnóstico foi descrito como intoxicação, apesar de alguns sintomas descritos no prontuário remetessem a sintomas de intoxicação por agrotóxicos. Conclui-se que há casos de intoxicação na região, entretanto, os respectivos prontuários de atendimento não associam o caso à intoxicação por agrotóxicos.

8. Predictors of Seeking Care for Influenza-Like Illness in a Novel Digital Study

Abstract

Background Previous research has estimated that >50% of individuals experiencing influenza-like illness (ILI) do not seek health care. Understanding factors influencing care-seeking behavior for viral respiratory infections may help inform policies to improve access to care and protect public health. We used person-generated health data (PGHD) to identify factors associated with seeking care for ILI. Methods Two observational studies (FluStudy2020, ISP) were conducted during the United States 2019–2020 influenza season. Participants self-reported ILI symptoms using the online Evidation platform. A log-binomial regression model was used to identify factors associated with seeking care. Results Of 1667 participants in FluStudy2020 and 47 480 participants in ISP eligible for analysis, 518 (31.1%) and 11 426 (24.1%), respectively, sought health care. Participants were mostly female (92.2% FluStudy2020, 80.6% ISP) and aged 18–49 years (89.6% FluStudy2020, 89.8% ISP). In FluStudy2020, factors associated with seeking care included having health insurance (risk ratio [RR], 2.14; 95% CI, 1.30–3.54), more severe respiratory symptoms (RR, 1.53; 95% CI, 1.37–1.71), and comorbidities (RR, 1.37; 95% CI, 1.20–1.58). In ISP, the strongest predictor of seeking care was high symptom number (RR for 6/7 symptoms, 2.14; 95% CI, 1.93–2.38). Conclusions Using PGHD, we confirmed low rates of health care–seeking behavior for ILI and show that having health insurance, comorbidities, and a high symptom burden were associated with seeking health care. Reducing barriers in access to care for viral respiratory infections may lead to better disease management and contribute to protecting public health.

Abstract

O Brasil é o maior consumidor mundial de agrotóxicos o que tem gerado preocupação sobre seus efeitos na saúde. O objetivo desse estudo foi caracterizar a exposição aos agrotóxicos de pomicultores no sul do Brasil e investigar sintomas e casos de intoxicação aguda. A pesquisa foi quantitativa e descritiva, com 82 agricultores entrevistados em domicílio, aplicando-se um questionário estruturado, abordando sobre manuseio dos agrotóxicos, uso de equipamentos de proteção individual (EPI) e queixas de sintomas e casos de intoxicação. A maioria dos participantes relatou sempre utilizar o EPI durante o preparo (93,9%) e aplicação (97,6%) dos agrotóxicos, porém a maioria não segue a sequência recomendada para vestir e retirar o EPI. Treze agricultores afirmaram que já haviam se intoxicado com agrotóxicos, mas apenas seis buscaram auxílio médico, quatro realizaram exame para confirmar o diagnóstico e um ficou hospitalizado. Porém, 40 agricultores relataram algum sintoma durante ou após a exposição aos agrotóxicos, 19 relataram três ou mais sintomas, o que caracteriza provável caso de intoxicação e 26 relataram dois ou mais sintomas, que representa caso possível de intoxicação aguda por agrotóxicos. É fundamental desenvolver ações educativas e de vigilância em saúde para minimizar os efeitos negativos das intoxicações aos agricultores expostos aos agrotóxicos nas suas atividades laborais.

Abstract

Brazilian animal livestock is in full expansion but linked to the use of veterinary drugs which may be considered as emerging contaminants. The study aimed to characterize the disposal of Animal Healthcare Service Waste (AHSW), especially of veterinary drugs, besides identifying the most used drugs in the city of São Joaquim municipality, southern Brazil. Eighty-four cattle ranchers were interviewed through a structured questionnaire with closed questions about drugs use and disposal of the AHSW. The active ingredients of drugs most used were the antiparasitic ivermectin (68% of properties) and the antimicrobial oxytetracycline (48%). After the use, the disposal of the AHSW, including expired drugs, leftovers and wrapper, was the disposal next to the domestic trash, burned and deposited in the soil. The Chi square test showed association between the age of the participants and the disposal of sharp objects (X2 = 36,36, p= 0,020), showing that farmers aged above 60 years usually reuse, the ones who are 20 years old return to the place where they acquired the material and the rest adopts improper practices. The disposal practices of the AHSW adopted in the properties are still in disagreement with the current Brazilian legislation and may cause adverse effects on human, animal and environmental health. RESUMO A pecuária brasileira está em franca expansão, porém associada ao uso de medicamentos veterinários que podem ser considerados contaminantes emergentes. Este estudo objetivou caracterizar o descarte dos Resíduos de Serviços de Saúde Animal (RSSA), especialmente dos medicamentos veterinários, além de identificar os medicamentos mais usados no município de São Joaquim, Sul do Brasil. Foram entrevistados 84 criadores de gado por meio de questionário estruturado com questões fechadas sobre uso de medicamentos e descarte dos RSSA. Os princípios ativos de medicamentos mais usados foram o antiparasitário ivermectina (68% das propriedades) e o antimicrobiano oxitetraciclina (48%). Após o uso, o descarte dos RSSA foi no resíduo domiciliar, queimado e a depositado no solo. O teste Qui quadrado mostrou associação entre a idade dos participantes e o descarte de perfurocortantes (X2 = 36,36, p= 0,020), evidenciando que agricultores com idade acima de 60 anos costumam reutilizar, aqueles com idade abaixo de 20 anos devolvem no local onde adquirem o material e os demais adotam práticas inadequadas para o descarte. As práticas de descarte dos RSSA adotadas nas propriedades rurais estão em desacordo com a legislação brasileira vigente e podem ocasionar efeitos adversos na saúde humana, animal e ambiental.

Abstract

BackgroundChronic pain affects approximately 50 million adults in the United States and impacts mood, everyday functioning, and quality of life. The challenges of analgesic clinical trials and, therefore, the approval of new non-opioid analgesics, are based in part on a fundamental lack of understanding of those outcomes that are relevant to an individual’s overall functioning.ObjectivesTo determine the behaviors and health outcomes associated with chronic pain. This manuscript presents an overview of the study design, baseline health and behavioral characteristics of our sample, and preliminary findings of how behavioral characteristics differ between individuals with and without chronic pain.MethodsThe study is a decentralized digital longitudinal cohort study of 10,036 individuals (5,832 with chronic pain [CP] and 4,204 with no chronic pain [NCP]), age 18 years or older, living in the United States. The study period was one year. Data were collected from wearable activity trackers and health or fitness mobile applications to capture passively collected behavioral data including steps, sleep, and heart rate. Patient-reported outcomes on mood and pain, including the BPI-SF, PHQ-9, and GAD-7, were collected at various timepoints during the study.ResultsThe data suggest greater levels of depression and anxiety, lower quality of life, less physical activity, more variable sleep, and higher resting heart rate are associated with CP.ConclusionsThe longitudinal data from the larger study will yield substantial contributions to the body of literature in chronic pain, particularly in delineating relational and causal factors relevant to the impact of chronic pain, and potential development of a digital biomarker to assess and monitor patients’ everyday experience with chronic pain.

Abstract

Ageism is a pervasive problem at an individual, cultural, and systemic level, arguably one of the more socially acceptable forms of prejudice and bias at this point in today’s culture. This chapter reviews the concept of ageism and highlights available literature on the topic as well as areas where more research is needed. The effect of ageism on behavioral and physical health outcomes, intersectionality between ageism and other forms of bias and prejudice, the problem of ageism within healthcare settings and training, and burgeoning areas of intervention to mitigate the problem are reviewed.

Abstract

This research aimed to verify how the disposal of pesticide wastes occurs in apple orchards (Malus domestica) in the south of Brazil and to verify its compliance with current legislation. This was a quantitative, descriptive and prospective field study. The research was carried out with 82 farmers from two rural areas belonging to the municipality of São Joaquim, the largest Brazilian apple producer, located in the Serrano Plateau of the State of Santa Catarina, southern Brazil. The farmers were interviewed at home and questioned about the disposal of empty pesticide containers. The results of this research showed that most farmers carry out the proper management of the empty pesticide containers and return them to the collection stations or centers (85.5%), the other 8.5% dispose empty containers inappropriately, keeping them in the open air or burning them. It should be noted that 20.7% of farmers store these containers in the open air until they are delivered to the collection stations without any protection and exposed to people and animals. In addition, it was observed that individuals without any level of education tend to store empty containers inadequately. Thus, despite the fact that Brazil is a world reference in terms of the proper disposal of empty pesticide containers, there are still cases of non-compliance with the current legislation in relation to these procedures, and there is a need for technical guidance to workers, especially those with lower levels of education, as well as greater rigor of the legislation of supervision by the competent body.
