AbstractPurposeMethodsResultsConclusionDelays in the diagnosis and treatment of infantile hypertrophic pyloric stenosis (IHPS) may lead to laboratory and clinical decompensation in infants. The aim of this study was to determine whether pyloric measurement parameters can be effectively used in the clinical prognosis of IHPS by evaluating the relationship between pyloric dimensions on ultrasonographic evaluation at the time of presentation and clinical and laboratory parameters in infants with pyloric stenosis.A retrospective observational study was conducted on 122 infants who were followed up with the diagnosis of IHPS in our neonatal clinic and operated on by a pediatric surgeon between January 2005 and December 2020.The mean pyloric dimensions of the infants were pyloric length 20 mm (17–22), pyloric thickness 5 mm (4–5.8), and pyloric index 96.5 (72.2–124). Babies with preoperative laboratory values of hypokalemia, hypochloremia and metabolic alkalosis had statistically higher pyloric thickness >5 mm (4.1–6) and pyloric index >100 (86–130) parameters.Pyloric thickness and pyloric index parameters can predict clinical prognosis in babies diagnosed with IHPS in the perioperative period. VITAMIN D , *NEONATAL intensive care units , *COVID-19 pandemic , *VITAMIN D deficiency , *QUARANTINE - Abstract
Vitamin D deficiency, which is considered a global pandemic, has become an even more important problem during the COVID-19 quarantine period. Our aim with this study is to investigate the change in vitamin D levels in babies of pregnant women who could not benefit from UV-B rays during the quarantine period. Retrospective study, and infants with hypocalcemia and low vitamin D levels (<20 ng/mL) hospitalized in the neonatal intensive care unit for any reason were included in the study. The patients were divided into two groups (study group and control group) and compared statistically. Study group of infants affected by the quarantine period (n:94); The control group was infants (38 infants) who were hospitalized in the neonatal intensive care unit for any reason before the quarantine. After the coronavirus quarantine period, annual hospitalization rates with the diagnosis of vitamin D deficiency increased from 1.4% to 3% (p:0.035). There was a significant decrease in serum vitamin D levels of the babies in the study group (p:0.001). Calcium treatment durations of the babies in the study group were higher than the babies in the control group (p:0.045). Although it is known that the application of vitamin D and calcium supplementation to women before pregnancy, during pregnancy, and lactation period leads to positive results, especially in newborns during the quarantine period, we should be alert to vitamin D deficiency especially, in newborns in this period. - Abstract
Objectives: The aim of this study was to evaluate long COVID patients with persistent respiratory symptoms through the application of the World Health Organization (WHO) International Classification of Functioning, Disability and Health (ICF) framework. Patients and methods: This national, prospective, multicenter, cross-sectional study was conducted with 213 patients (118 females, 95 males; median age 56 years; range, 20 to 85 years) with long COVID between February 2022 and November 2022. The ICF data were primarily collected through patient interviews and from the acute medical management records, physical examination findings, rehabilitation outcomes, and laboratory test results. Each parameter was linked to the Component Body Functions (CBF), the Component Body Structures (CBS), the Component Activities and Participation (CAP), the Component Environmental Factors (CEF), and Personal Factors according to the ICF linking rules. Analysis was made of the frequency of the problems encountered at each level of ICF category and by what percentage of the patient sample. Results: In the ICF, 21 categories for CBF, 1 category for CBS, and 18 categories of CAP were reported as a significant problem in a Turkish population of long COVID patients with persistent respiratory symptoms. Furthermore, eight categories for CEF were described as a facilitator, and four as a barrier. Conclusion: These results can be of guidance and provide insight into the identification of health and health-related conditions of long COVID patients with persistent respiratory symptoms beyond the pathophysiological aspects, organ involvement, and damage of COVID-19. The ICF can be used in patients with long COVID to describe the types and magnitude of impairments, restrictions, special needs, and complications. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the blood flow of the umbilical vein and right portal vein in macrosomic fetuses of diabetic mothers and investigate the effect of maternal insulin treatment on these blood flows. Material and Methods: This prospective case-control study was conducted between March 2019 and December 2019. Fetuses of the 49 pregestational and gestational diabetic mothers who had an abdominal circumference percentile above 97% were evaluated as macrosomic and formed the study group. The study group was divided into two subgroups: patients treated with insulin and those who did not. In the control group, 48 non-diabetic pregnant women with matched gestational weeks whose fetuses are at the 10-90% percentile were included. Time-averaged maximum blood velocity (TAMXV) values of the right portal vein and the free loop of the umbilical vein were measured. Results: The median right portal vein TAMXV value and umbilical vein TAMXV value were found to be significantly higher in diabetic pregnancies (16.25 cm/s, and 15.28 cm/s, respectively) than in the control group (12.76 cm/s, and 13.38 cm/s, respectively, p<0.001). Umbilical and right portal vein flows were similar in macrosomic fetuses of diabetic mothers who were treated with insulin or those who did not. While umbilical vein flow in macrosomic fetuses increased as the gestational age progressed (p=0.028), it was observed steadily in normally growing fetuses. Conclusion: The umbilical and right portal vein flows are higher in macrosomic fetuses of diabetic mothers than in appropriately grown fetuses. Maternal insulin treatment does not affect fetal umbilical vein and right portal vein blood flow in macrosomic fetuses. [ABSTRACT FROM AUTHOR]

Since the beginning of the pandemic, many novel coronavirus disease 2019 (COVID-19) patients have experienced multisystem involvement or become critically ill and treated in intensive care units, and even died. Among these systemic effects, cardiac involvement may have very important consequences for the patient's prognosis and later life. Patients with COVID-19 may develop cardiac complications such as heart failure, myocarditis, pericarditis, vasculitis, acute coronary syndrome, and cardiac arrhythmias or trigger an accompanying cardiac disease. The ratio of COVID-19 cardiac involvement ranges between 7 and 28% in hospitalized patients with worse outcomes, longer stay in the intensive care unit, and a higher risk of death. Furthermore, deconditioning due to immobility and muscle involvement can be seen in post-COVID-19 patients and significant physical, cognitive and psychosocial impairments may be observed in some cases. Considering that the definition of health is "a state of complete physical, mental and social well-being", individuals with heart involvement due to COVID-19 should be rehabilitated by evaluating all these aspects of the disease effect. In the light of the rehabilitation perspective and given the increasing number of patients with cardiac manifestations of COVID-19, in this review, we discuss the rehabilitation principles in this group of patients. Objective: To evaluate the clinical features, prognostic factors and perinatal outcomes of fetuses with the diagnosis of congenital pulmonary airway malformation and bronchopulmonary sequestration. Method: The medical records of 59 fetuses with the diagnosis of congenital pulmonary airway malformation and bronchopulmonary sequestration were retrospectively analyzed. The characteristics and natural history of the thoracic mass, and the perinatal outcomes of cases were evaluated. Results: While 37 cases (71.2%) were diagnosed as congenital pulmonary airway malformation, 15 cases (28.8%) were diagnosed as bronchopulmonary sequestration. Associated structural anomalies were detected in 17.3 percent of cases and chromosomal anomalies were present in 7.7 percent of cases with associated anomalies. Live birth occured in 75 percent of cases. The presence of bronchopulmonary malformation was shown in 59 percent of cases in postnatal period, and 78.3 percent of these cases were operated in the first year of life. Conclusion: We observed that fetuses with the diagnosis of bronchopulmonary malformation had a good prognosis in the absence of associated anomaly and fetal hydrops. It is also observed that the cases with spontaneous regression of the thoracic mass did not need an operation. Thoracic transverse sections should be evaluated during basic fetal anatomic screening in order to increase the frequency of prenatal diagnosis of fetal thoracic anomalies.

- Abstract
Coronavirus disease 2019 (COVID-19) is a contagious infection disease, which may cause respiratory, physical, psychological, and generalized systemic dysfunction. The severity of disease ranges from an asymptomatic infection or mild illness to mild or severe pneumonia with respiratory failure and/or death. COVID-19 dramatically affects the pulmonary system. There is a lack of knowledge about the long-term outcomes of the disease and the possible sequelae and rehabilitation. This clinical practice guideline includes pulmonary rehabilitation (PR) recommendations for adult COVID-19 patients and has been developed in the light of the guidelines on the diagnosis and treatment of COVID-19 provided by the World Health Organization and Republic of Turkey, Ministry of Health, recently published scientific literature, and PR recommendations for COVID-19 regarding basic principles of PR. In this guideline, the contagiousness of COVID-19, recommendations on limited contact of patient with healthcare providers, and the evidence about possible benefits of PR were taken into consideration.

Hastanede açılan yeni bası yarası oranının sıfır olması, rehabilitasyon hastanesinin bası yarası ile ilgili gerekli farkındalığın ve hemşire, refakatçi eğitim düzeyinin oluştuğunu göstermektedir., Aim: Stroke is a brain vascular disease that affects millions of people in the world every year and causes disability. Complications following a stroke are known to increase hospital stay and patient care costs. A complication of stroke, pressure ulcer, can be avoided with education of the caregiver. That is why we aimed to investigate the rates of pressure ulcers of patients who were hospitalized in a public rehabilitation hospital because of stroke. Secondary aim of this study is to investigate the in-service trainings of the nurses about pressure ulcers. Methods: This is an observational study. The files of inpatient stroke cases were retrospectively analyzed for dates between 01/01/2016 and 12/31/2018 and the data were collected. Education service files are analyzed for in-service trainings about pressure ulcers. Percentages were calculated for statistical analysis. Results: Of the 3640 patients hospitalized in our hospital, 1776 had stroke (48.80%). The number of patients with acute stroke was 1216 (68.47%). The mean number of days of hospitalization was 27.6 days. When the demographic characteristics of the patients were examined, 1005 of the inpatients were male (56.59%), 771 were female (43.41%). The mean age of the male patients was 64.18 and the mean age of the female patients was 68.78 respectively. When the risk factors for stroke were examined, 59.74% of the hospitalized patients had hypertension (HT), 25.16% had diabetes mellitus (DM), 13.56% had hyperlipidemia (HL), 15.03% had cardiac disease, 6.41% had urinary infection, 3.71% had pulmonary infection, 0.5% had deep vein thrombosis, and 0.56% had malignancy. When the rate of pressure ulcers were examined, the number of stroke patients with pressure ulcers was 123 (6.92%) and no new pressure ulcers were observed during hospitalization. The rate of stroke patients with pressure ulcers among all pressure ulcer patients was calculated as 32.97%. The most common anatomic localization was sacrum 65 (52.84%) while the least common localization was elbow region with 1 patient (0.81%). The in-service trainings given to the nurses for pressure ulcers were 2 per year and the rate of nurses attending the training was determined as 71.5%. The number of trainings for medical companions was 12 per year and the participation rate to the training was 57.3%.Conclusion: The incidence of pressure ulcers in stroke patients was determined as 6.92% and the most frequent localization was sacrum. The fact that the rate of new pressure ulcer opened in the hospital is zero indicates that in-service training and awareness about pressure ulcers in the rehabilitation hospital is sufficient.

Objective: In this study, we aimed to assess the factors leading to the diagnosis of conotruncal heart anomaly in the prenatal period by reviewing the records of the cases which were diagnosed with the conotruncal heart anomaly in our perinatology unit, and to raise the awareness of the diagnosis of prenatal conotruncal heart anomaly. Methods: The referral reasons, the presence of concomitant non-cardiac anomaly and chromosomal anomaly, perinatal outcomes of the cases and the confirmation success of the diagnosis at postnatal period of the cases which were diagnosed with the conotruncal hearth anomaly at the Perinatology Unit of Kanuni Sultan Süleyman Training and Research Hospital between January 2015 and December 2016 were evaluated. Results: Among all congenital cardiac anomalies, the incidence of conotruncal heart anomaly was 20.4%. The termination of pregnancy was performed in 37 (36.6%) of 101 cases included in the study. Intrauterine fetal death was observed in 5 (5%) cases. Chromosomal anomaly and non-cardiac additional structural anomaly were found in 26.7% and 34.7% of the cases, respectively. Prenatal diagnosis was confirmed in 52 (88.1%) of 59 (58.4%) cases which born alive. It was found that four-chamber view was abnormal in only 27.7% of the cases. Conclusion: Three vessels (3V) and three vessels trachea (3VT) views should be displayed routinely in basic fetal cardiac screening in order to increase the prenatal diagnosis frequency of these anomalies as four-chamber view is mostly normal in conotruncal heart anomaly.

The zscores of total length, total width and cortex width of the fetal adrenal glands were higher in the study group than the control group which was statistically significant (p0.05). Conclusion: It is possible to consider that the changes observed in the fetal adrenal gland sizes of the cases diagnosed with intrauterine growth restriction is a result of chronic hypoxia associated with uteroplacental failure and a component of poor fetal programming associated with the long-term poor health outcomes.

There are many kinds of pekmez, which is a traditional food.Pekmez is produced by evaporating water contained in fruit juices in open orvacuum boilers. Pekmez varies as liquid and solid pekmez depending onappearance and consistency properties. In this study, nutrient-high foodstuffssuch as sesame and peanut were used to produce pekmez mixtures. The sensoryanalysis method was applied in determining the appropriate formulations for thepekmez mixtures. The fat contents of grape and carob pekmez mixtures were foundto be 24.88% and 22.62%, and the protein contents were 14.75% and 15.49%,respectively. The total amount of phenolic substance was determined as 118.97mg GAE 100 g-1 for grape pekmez mixture and 333.84 mg GAE 100 g-1for carob pekmez mixture. The carob pekmez mixture was found have higherantioxidant activity than the grape pekmez mixture (P, Geleneksel bir gıda olan pekmezin birçok çeşidi bulunmaktadır.Meyve sularının içerdiği suyun açık kazanlarda veya vakumlu kazanlardabuharlaştırılması ile pekmez üretimi gerçekleştirilmektedir.
This national guideline provides suggestions regarding the PR methods during the clinical stages of COVID-19 and post-COVID-19 with its possible benefits, contraindications, and disadvantages.
