Zusammenfassung Einleitung In der COVID-19-Pandemie zählen Pflegebedürftige zu den besonders vulnerablen Bevölkerungsgruppen. Ambulante Pflegedienste befanden sich gerade zu Beginn der Pandemie in einer Ausnahmesituation. In dieser Arbeit sollen die Situation und die Probleme der Pflegedienste und der versorgten Pflegebedürftigen in den ersten beiden Wellen der Pandemie in Deutschland untersucht werden. Methoden Während der ersten beiden COVID-19-Wellen wurden zwei Querschnittstudien durchgeführt (erste Befragung: 28.04.–12.05.2020, zweite Befragung: 12.01.–07.02.2021). Insgesamt wurden Daten aus N = 1029 ambulanten Pflegediensten in die Analyse einbezogen. Die Analyse erfolgte anhand deskriptiver Maßzahlen. Ergebnisse Die Klient:innen von Pflegediensten waren in den ersten beiden Wellen der Pandemie stark belastet. Dies zeigt sich einerseits an einem erhöhten Erkrankungsrisiko und einer erhöhten Mortalität und andererseits am Wegfall verschiedener Versorgungs- und Unterstützungsangebote. Letzteres hat z. B. auch negative Auswirkungen auf die psychosoziale Verfassung der Pflegebedürftigen. Die Pflegedienste waren von hohen Personalausfällen und zusätzlicher Arbeit durch Schutzmaßnahmen betroffen. Diskussion Die COVID-19-Pandemie führte zu großen Belastungen von Pflegebedürftigen und ambulanten Pflegediensten und zu einer Reduzierung der Versorgungsangebote. Die Verschlechterung der Versorgung traf auf eine bereits angespannte Situation. Es zeigt sich, dass die Versorgung Pflegebedürftiger durch ambulante Pflegedienste nicht krisensicher gestaltet ist und dass zusätzliche Herausforderungen wie die einer Pandemie dramatische Folgen haben können. Zukünftig sollte es verlässliche Strukturen und schnell verfügbare Notfallpläne mit konkreten Handlungsanweisungen geben.

Zusammenfassung Hintergrund Die Folgen der COVID-19-Pandemie haben verschiedene Personengruppen vor große Herausforderungen gestellt; eine dieser Gruppen sind pflegende Angehörige. Die vorliegende Studie untersucht, welche Veränderungen die Pandemie für pflegende Angehörige mit sich gebracht hat und in welchem Ausmaß sich Lebensqualität und Pflegebelastung subgruppenspezifisch verändert haben. Methode Die Datenerhebung erfolgte im Sommer 2020 in einer Querschnittsstudie mit pflegenden Angehörigen im erwerbsfähigen Alter (N = 1143). Neben soziodemografischen Daten wurden Angaben zu Versorgungssituation, Vereinbarkeit von Pflege und Beruf sowie Belastung und Lebensqualität in einer Onlinebefragung erhoben. Versorgungssituation und Vereinbarkeit von Pflege und Beruf wurden deskriptiv analysiert. Für die Analysen der Veränderung der Lebensqualität und der Belastung wurden logistische Regressionsmodelle verwendet. Ergebnisse Die Versorgungssituation hat sich für viele Befragte (54,7 %) während der Pandemie geändert und ist zeitlich aufwendiger geworden. Für 70,8 % ist die Vereinbarkeit von Pflege und Beruf schwieriger geworden. Mit dem Pandemiemanagement der eigenen Arbeitgeber:innen zeigt sich die Mehrheit zufrieden (65,9 %). Die Lebensqualität hat ab- und die Belastung zugenommen, besonders deutlich für jüngere Pflegende, Frauen und Pflegende von Personen mit hohem Pflegebedarf. Diskussion Die Ergebnisse weisen darauf hin, dass sich die Lebenssituationen pflegender Angehöriger während der COVID-19-Pandemie verschlechtert haben. Entscheidungsträger:innen sollten dies anerkennen und besonders betroffene Subgruppen pflegender Angehöriger unterstützen. Zukünftig ist es wichtig, die informelle häusliche Pflege ebenso wie Versorgungssettings der professionellen (Langzeit‑)Pflege in gesundheits- und sozialpolitische Pandemiekonzepte einzubeziehen.

Background and aim While artificial intelligence (AI) is being adapted for various life domains and applications related to medicine and healthcare, the use of AI in nursing practice is still scarce. The German Ministry for Education and Research funded a study in order to explore needs, application scenarios, requirements, facilitators and barriers for research and development projects in the context of AI in nursing care. Method A sequential explorative mixed methods study including a stakeholder and expert workshop (N = 21), expert interviews (N = 14), an online survey (N = 53) and a Datathon (N = 80) was conducted with an emphasis on qualitative data. Results Needs and application scenarios encompassed the micro- and meso-level of care and derived from typical phenomena inherent to nursing care as well as from skill- and staff mix and consequences arising from staff shortages, from the extend of informal care and an associated need for information and education of informal caregivers and nursing assistants. Requirements for and characteristics of successful research and development projects included regulatory, processual, technological, ethical and legal aspects and supportive eco-systems. Conclusion A key element in the design of research projects remains participatory and demand-driven development that aims to bring AI solutions out of the lab and into practice. However, influencing factors remain that are outside the sphere of influence of individual projects, in particular the creation of resilient legal foundations for data use and the use of AI in practice, standardization of data structures and the establishment of infrastructures for data exchange across institutions and projects.

Background Multimorbidity poses a challenge for high quality primary care provision for nursing care-dependent people with (PWD) and without (PWOD) dementia. Evidence on the association of primary care quality of multimorbid PWD and PWOD with the event of a nursing home admission (NHA) is missing. This study aimed to investigate the contribution of individual quality of primary care for chronic diseases in multimorbid care-dependent PWD and PWOD on the duration of ongoing residence at home before the occurrence of NHA. Methods We conducted a retrospective cohort study among elderly care-dependent PWD and PWOD in Germany for six combinations of chronic diseases using statutory health insurance claims data (2007–2016). Primary care quality was measured by 21 process and outcome indicators for hypertension, diabetes, depression, chronic obstructive pulmonary disease and heart failure. The primary outcome was time to NHA after initial onset of care-dependency. Multivariable Cox proportional hazard models were used to compare the time-to-event between PWD and PWOD. Results Among 5876 PWD and 12,837 PWOD 5130 NHA occurred. With the highest proportion of NHA for PWD with hypertension and depression and for PWOD with hypertension, diabetes and depression. Average duration until NHA ranged from 6.5 to 8.9 quarters for PWD and from 9.6 to 13.5 quarters for PWOD. Adjusted analyses show consistent associations of the quality of diabetes care with the duration of remaining in one’s own home regardless of the presence of dementia. Process indicators assessing guideline-fidelity are associated with remaining in one’s home longer, while indicators assessing complications, such as emergency inpatient treatment (HR = 2.67, 95% CI 1.99–3.60 PWD; HR = 2.81, 95% CI 2.28–3.47 PWOD) or lower-limb amputation (HR = 3.10, 95% CI 1.78–5.55 PWD; HR = 2.81, 95% CI 1.94–4.08 PWOD) in PWD and PWOD with hypertension and diabetes, increase the risk of NHA. Conclusions The quality of primary care provided to care-dependent multimorbid PWD and POWD, influences the time individuals spend living in their own homes after onset of care-dependency before a NHA. Health care professionals should consider possibilities and barriers of guideline-based, coordinated care for multimorbid care-dependent people. Further research on quality indicator sets that acknowledge the complexity of care for multimorbid elderly populations is needed.

The COVID-19 pandemic constitutes an exceptional risk to people living and working in nursing homes (NHs). There were numerous cases and deaths among NH residents, especially at the beginning of the pandemic when no vaccines had yet been developed. Besides regional differences, individual NHs showed vast differences in the number of cases and deaths: while in some, nobody was affected, in others, many people were infected or died. We examine the relationship between facility structures and their effect on infections and deaths of NH residents and infections of staff, while considering the influence of COVID-19 prevalence among the general population on the incidence of infection in NHs. Two nationwide German surveys were conducted during the first and second pandemic waves, comprising responses from n = 1067 NHs. Different hurdle models, with an assumed Bernoulli distribution for zero density and a negative binomial distribution for the count density, were fitted. It can be shown that the probability of an outbreak, and the number of cases/deaths among residents and staff, increased with an increasing number of staff and the general spread of the virus. Therefore, reverse isolation of NH residents was an inadequate form of protection, especially at the beginning of the pandemic.

Zusammenfassung Für den Wunsch vieler Menschen, auch bei Pflegebedürftigkeit lange im eigenen Zuhause zu leben, ist das Kennen von Einflussfaktoren für einen Heimeinzug von Bedeutung. Wissen zu diesen Faktoren kann zur Vermeidung, Verzögerung oder angemessene Gestaltung eines Heimeinzugs beitragen. Präventive Maßnahmen, die die kognitive und funktionale Gesundheit oder die Unterstützung von pflegenden Angehörigen adressieren, können den Verbleib in der eigenen Häuslichkeit unterstützen. Innovative Technologien bieten zudem neue Möglichkeiten der Prävention.

Zusammenfassung Ziel der Studie Ermittlung von Prädiktoren für den Eintritt in ein Pflegeheim mittels einer Sekundärdatenanalyse von Krankenkassen- und Pflegebegutachtungsdaten. Methodik Eine retrospektive Längsschnittanalyse betrachtet den Zeitraum von 2006–2016 unter Verwendung von Routinedaten. Krankenkassen- und Pflegebegutachtungsdaten von im Jahr 2006 pflegebedürftig gewordenen Menschen, die in der eigenen Häuslichkeit lebten, wurden zusammengeführt. Prädiktoren für einen Pflegeheimeintritt wurden auf dieser Basis und mit Hilfe von Cox Regressionsanalysen ermittelt. Ergebnisse Die Studienpopulation umfasste 48 892 Menschen. Demenz, Krebserkrankungen des Gehirns, kognitive Einschränkungen, die Verschreibung von Antipsychotika, Frakturen mit Krankenhausaufenthalten, Krankenhausaufenthalte über zehn Tage Länge und höheres Alter wiesen die höchsten Effektstärken unter den Prädiktoren auf. Schlussfolgerung Kenntnisse über die Prädiktoren dienen der Sensibilisierung von Akteuren in der Versorgung pflegebedürftiger Menschen. Sie erleichtern das Erkennen von Unterstützungsbedarf in der eigenen Häuslichkeit bei Menschen, die unter einem erhöhten Risiko eines Heimeintritts stehen.

Background People prefer to age in place and not move into a nursing home as long as possible. The prevention of cognitive and functional impairments is feasible to support this goal. Health services play a key role in providing support for underlying medical conditions. We examined differentials in nursing home admissions between patient sharing networks in Germany and whether potential variations can be attributed to indicators of health care provision. Methods We conducted an ecological study using data of patients of 65 years and above from all 11 AOK statutory health insurance companies in Germany. Nursing home admissions were observed in a cohort of persons becoming initially care-dependent in 2006 (n = 118,213) with a follow-up of up to 10 years. A patient sharing network was constructed and indicators for quality of health care were calculated based on data of up to 6.6 million patients per year. Community detection was applied to gain distinct patient populations. Analyses were conducted descriptively and through regression analyses to identify the variation explained by included quality indicators. Results The difference in the proportion of nursing home admissions between identified clusters shows an interquartile range (IQR) of 12.6% and the average time between onset of care-dependency and admission to a nursing home an IQR of 10,4 quarters. Included quality indicators attributed for 40% of these variations for the proportion of nursing home admissions and 49% for the time until nursing home admission, respectively. Indicators of process quality showed the single highest contribution. Effects of single indicators were inconclusive. Conclusions Health services can support persons in their preference to age in place. Research and discussion on adequate health care for care-dependent persons and on conditions, where nursing home admission may be beneficial, is necessary.

ObjectivesWe perform and evaluate record linkage of German Care Needs Assessment (CNA) data to Statutory Health Insurance (SHI) claims data. The resulting dataset should enable the identification of factors in healthcare predicting the time between the onset of long-term care dependency and the admission to a nursing home in Germany in subsequent analyses.DesignA deterministic record linkage was conducted using the key variables region, sex, date of birth and care level. In further steps, the underlying cause of care dependency (International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10)) was added for a higher level of distinction. Before linkage, the suitability of the two datasets for these procedures was assessed. After linkage, the results of each stage were analysed and the resulting dataset was evaluated cross-sectionally with respect to bias generated through this process.SettingThe study comprises data from the German SHI and Statutory Long-Term Care Insurance.ParticipantsThe study cohort comprised 158 069 individuals who became care dependent in 2006. We obtained CNA data for the year 2006 including 188 935 individuals.ResultsWe could link CNAs to 66 310 individuals of the original study cohort, corresponding to 42.0%. Records from two federal states could not be matched due to missing data. Linkage rates were lower where more people shared the same attributes. The resulting dataset showed minor differences regarding age, sex and care level compared to the original cohort.ConclusionsData linkage between German SHI claims data and CNA data is feasible. Failure to link was mostly attributable to a lack of distinction between individuals using available identifiers. The resulting dataset contains relevant information from both health services provision and functional status of care dependent people and is suitable for further analyses with critical reflection of representativity.

Zusammenfassung Hintergrund/Zielsetzung Ambulant versorgte Pflegebedurftige sind als vulnerable Personengruppe fur Defizite in der Versorgung der Gesetzlichen Krankenversicherung (GKV) anzusehen. Qualitatsindikatoren (QI) konnten Hinweise auf Defizite und Optimierungspotentiale der GKV-Versorgung pflegebedurftiger Menschen geben. Um QI hinsichtlich ihrer Diskriminierungsfahigkeit bei alteren Menschen mit und ohne Pflegebedarf in unterschiedlichen Altersklassen zu untersuchen, sind Erkenntnisse zur Darstellbarkeit von QI auf Basis von GKV-Routinedaten von Interesse. Ziel der Studie ist eine explorative Annaherung an routinedatenbasierte Qualitatskennzahlen fur die ambulante Versorgung alterer Menschen mit Pflegebedarf, die dazu genutzt werden konnen, das Versorgungsgeschehen weiterzuentwickeln. Methode Datengrundlage dieser Querschnittstudie sind anonymisierte, versichertenbezogene Abrechnungsdaten von AOK-Versicherten aus dem gesamten Bundesgebiet des Jahres 2016. Die Stichprobe umfasst 5.934.986 Personen, die 65 Jahre oder alter waren und mit (n=785.135) oder ohne Pflegebedarf (n=4.799.369) in der eigenen Hauslichkeit oder in einem Pflegeheim (n=350.482) lebten. 47 Qualitatsindikatoren (QI) in den Versorgungsschwerpunkten Ambulant-sensitive Krankenhausfalle, Asthma, chronisch obstruktive Lungenerkrankung, Herzkreislauferkrankungen, Diabetes Mellitus Typ 2 und Depression wurden quartalsweise ermittelt und deskriptiv als Mittel aus vier Quartalen stratifiziert nach Lebensort und Altersgruppen dargestellt. Ergebnisse Die QI variieren in ihren Werten mehrheitlich zwischen den betrachteten Personengruppen, wobei ein gruppenubergreifender Trend zu Ungunsten alterer Personen vorliegt. Obgleich Personen mit Pflegebedarf bis auf den Versorgungsschwerpunkt Asthma hohere Anteile der den sechs untersuchten Versorgungsschwerpunkten zugrunde liegenden Diagnosen aufweisen, erzielen sie meist nachteiligere QI-Auspragungen als Personen ohne Pflegebedarf. Dies halt auch einer Stratifizierung nach Altersgruppen und damit der unterschiedlichen Altersstruktur der Gruppen stand. Diskussion Die Studie stellt Unterschiede in der Versorgung alterer Menschen dar. Angesichts der bislang vorliegenden empirischen Evidenz zu Versorgungsdefiziten sind Faktoren zu untersuchen, die eine leitliniengerechte Versorgung chronischer Erkrankungen bei Pflegebedurftigkeit erschweren oder begunstigen. Inwiefern individuelle Patientenpraferenzen, Zugang zum medizinischen Versorgungssystem oder eine Prioritatensetzung bei der Initiierung von diagnostischen oder therapeutischen Prozessen seitens der medizinischen und pflegerischen Leistungserbringer eine Rolle spielen, bleibt offen. Methodische Limitationen ergeben sich vor allem aus der Selektion der Untersuchungsgruppen unter Berucksichtigung von zwei Stratifizierungsmerkmalen. Fur eine Weiterentwicklung der QI sind neben individuellen auch populationsbezogene Einflussfaktoren auf den Indikatorwert zu ermitteln und deren Beeinflussbarkeit durch die Leistungserbringer zu prufen, um nicht beeinflussbare Faktoren in einer risikoadjustierten Darstellung der QI zu berucksichtigen. Schlussfolgerung Die Messung der Qualitat der ambulanten GKV-Versorgung alterer Menschen auf Basis von Routinedaten unter Berucksichtigung des Lebensorts und des Pflegestatus ist machbar und weist auf Unterschiede in den Versorgungsprozessen alterer und pflegebedurftiger Menschen hin. Fur die Weiterentwicklung der explorierten QI bedarf es einer methodischen Scharfung und Fundierung, insbesondere im Hinblick auf eine Risikoadjustierung. Mit Blick auf die Personengruppe ambulant versorgter pflegebedurftiger Menschen und ihrem moglichst langen Verbleib in der eigenen Hauslichkeit bleibt zu untersuchen, inwiefern neben personenbezogenen Merkmalen auch Kontext- bzw. Umgebungsfaktoren das individuelle Indikatorergebnis beeinflussen.

The COVID-19 pandemic severely impacted the social health of nursing home residents with dementia due to social isolation. Consequently, the frequency of Behavioral and Psychological Symptoms in Dementia (BPSD) might increase. Technological solutions might help safeguard the social health of nursing home residents with dementia. This study investigates the impacts of the COVID-19 pandemic on clinical outcomes and the availability of social activities and technology to promote social participation in nursing home residents with dementia. The study analyzed cross-sectional data from a follow-up questionnaire nested in a larger national survey of care facilities in Germany. A mixed-methods approach integrated statistical analyses of closed-ended responses and thematic analysis of free-text responses. A total of 417 valid individual responses were received, showing an overall increase in observed BPSD—with anxiety and depression most frequently occurring. Many nursing homes canceled all social activities for residents with dementia, though a few had established procedures to facilitate social participation using technology. Requirements to promote social participation in this population using technology were identified at the micro-, meso-, and macro levels. Technology requirements permeated all three levels. During and beyond the COVID-19 pandemic, technology-driven solutions to promote social health among nursing home residents with dementia should be integrated into caregiving procedures.

Dominik Domhoff, Kathrin Seibert, Heinz Rothgang und Karin Wolf-Ostermann gehen in diesem Beitrag der Frage nach, welche Kommunikationstechnologien von ambulanten und stationären Pflegeeinrichtungen während der COVID-19-Pandemie genutzt wurden und welche Merkmale diese Nutzung beeinflussten. Dafür werteten sie Fragebögen von 1.525 Pflegeeinrichtungen aus. Viele Pflegeeinrichtungen führten in der Pandemie kurzfristig unterschiedliche digitale Kommunikationstechnologien ein oder verstärkten deren Einsatz. Zur Anwendung kamen vor allem Produkte, die auch außerhalb der Pflege weit verbreitet sind. Es verbleibt ein hoher Bedarf an Weiterentwicklung von technischer Infrastruktur und den Rahmenbedingungen zum Einsatz digitaler Technologien in der Pflege.

Betreute Wohngruppen oder Wohngemeinschaften sind als mogliches Wohnsetting zwischen Heim und Hauslichkeit fur pflegebedurftige Menschen von Bedeutung. Ziel der Arbeit war es, nachweisbare Unterschiede in gesundheitsbezogenen Endpunkten (Outcomes) zwischen Bewohnerinnen und Bewohnern von ambulant betreuten Wohngemeinschaften im Vergleich zu Bewohnerinnen und Bewohnern in traditionellen Einrichtungen der stationaren Langzeitpflege (Pflegeheim) zu erfassen. Ein Rapid Review wurde zur Identifikation von Versorgungsoutcomes in ambulant betreuten Wohngemeinschaften durchgefuhrt. Die Literaturrecherche erfolgte im September 2017 in den Datenbanken PubMed, CINAHL, Gerolit und LIVIVO. Deutsch- und englischsprachige, vergleichende Studien wurden einbezogen, wenn sie das Pflegeheim als Vergleichsgruppe untersuchten. Mehrheitlich beschreiben die eingeschlossenen 21 Studien einen begunstigenden Beitrag ambulant betreuter Wohngemeinschaften gegenuber Pflegeheimen. Dabei stellt Lebensqualitat ein haufig untersuchtes Outcome dar, das Hinweise auf einen Vorteil des Wohnens in ambulant betreuten Wohngemeinschaften, insbesondere fur Menschen mit Demenz, gibt. Eine Vielzahl von gesundheitsbezogenen Outcomes wurde in den Studien untersucht, welche Hinweise auf positive Effekte ambulant betreuter Wohngemeinschaften liefern. Zum grosen Teil liegt noch keine abschliesende Evidenz fur Outcomes auf Basis von Studien mit hohem Evidenzlevel vor.

Zusammenfassung Hintergrund An der gesundheitlichen Versorgung beteiligte Versorgungsnetzwerke konnen als formale oder informelle Netzwerke in Erscheinung treten. Fur eine sektorenubergreifende, populationsbasierte Qualitatsmessung in informellen Versorgungsnetzwerken, die zudem Aussagen uber die Kosteneffektivitat der Versorgung von pflegebedurftigen Personen in der eigenen Hauslichkeit erlaubt, fehlt ein umfassender evidenzbasierter Uberblick zu geeigneten Qualitatsindikatoren, die sich auf Grundlage von GKV-Routinedaten abbilden lassen. Fragestellung Welche populationsbasierten Indikatoren der Versorgungsqualitat in formalen und informellen Versorgungsnetzwerken sind in der internationalen Literatur beschrieben? Welche dieser Indikatoren werden als Ergebnisparameter in gesundheitsokonomischen Evaluationen mit welchen methodischen Zugangen eingesetzt? Sekundar werden verwendete Bewertungssystematiken und methodische Festlegungen von Schwellenwerten, Validitat und Eignung einzelner Indikatoren fur die Vorhersage der Versorgungsqualitat sowie die Abbildbarkeit von Qualitatsindikatoren auf Basis von GKV-Routinedaten untersucht. Suchstrategie Literaturrecherche in den Datenbanken PubMed, The Cochrane Library und NHS EED, CINAHL, GeroLit und EconLit im Mai 2017 und Juli 2018. Handsuche in den Literaturverzeichnissen sowie Sichtung der Projektdatenbank Versorgungsforschung Deutschland des Deutschen Netzwerks Versorgungsforschung e.V. Einschlusskriterien Quantitatives Design, in deutscher oder englischer Sprache. Jede Art eines formalen oder informellen Netzwerks, fur das die Mitglieder distinkt im Hinblick auf einzelne Leistungserbringer benannt sind und populationsbasierte Qualitatsindikatoren fur untersuchte Populationen von Erwachsenen (18 Jahre oder alter) beschrieben sind. Datenextraktion und Analyse Zwei Autorinnen (Cohens‘ Kappa = 0,64) sichteten Titel, Abstracts und Volltexte. Eine dritte Person wurde bei Unsicherheit bezuglich des Einschlusses von Studien hinzugezogen. Zur Bewertung der methodischen Gute kamen das AMSTAR-Instrument, das Chochrane Risk-of-Bias-Tool, die Newcastle-Ottawa-Skala (NOS), das Appraisal Tool for Cross-Sectional Studies (AXIS) sowie die Kriterien der Drummond-Checkliste zum Einsatz. Zentrale Ergebnisse Von 137 eingeschlossenen Studien untersuchten funf Studien informelle Netzwerke und nutzten Indikatoren fur Erkrankungen wie Diabetes mellitus, Herzinsuffizienz oder Ereignisse wie ambulant-sensitive Krankenhausfalle, die ebenso in formalen Netzwerken beschrieben werden. Von 14 gesundheitsokonomischen Evaluationen untersuchten funf Studien auch die Zusammenhange zwischen Kosten und Qualitat. Informationen zu Bewertungssystematiken und/oder der Festlegung von Schwellenwerten finden sich mehrheitlich nicht in den Studien. Obwohl die Reliabilitat und Validitat der in den Einzelstudien verwendeten Datengrundlage vielfach diskutiert wird, erfolgt eine Diskussion der Eignung der eingesetzten Indikatoren jedoch nur in knapp jeder vierten Studie. 121 Studien untersuchen Indikatoren, die sich in Ganze oder Teilen potentiell auf Basis von GKV-Routinedaten abbilden lassen und auf die Erkrankungen Arthrose, Asthma, chronische Schmerzen, COPD, Herz-Kreislauferkrankungen, Demenz, Diabetes mellitus, Osteoporose und auf psychische Gesundheit beziehen sowie die Versorgungsmerkmale ASK-Falle, angemessene medikamentose Versorgung alterer Menschen und Polypharmazie, Praventive Leistungen, Versorgungskontinuitat umfassen. Schlussfolgerung Diese systematische Ubersichtsarbeit identifizierte hauptsachlich in formalen Versorgungsnetzwerken eingesetzte Qualitatsindikatoren, die potentiell mit einem Spielraum der Erprobung und Weiterentwicklung fur die Messung der Versorgungsqualitat auch in informellen Netzwerke einsetzbar sein konnten. Der aufgezeigte Forschungsbedarf zu geeigneten Verfahren der Messung des Zusammenspiels von Versorgungsqualitat und Kosten sowie zu Validitat, Reliabilitat und pradiktiven Eignung einzelner Indikatoren, aber auch zu Bewertungssystematiken und Schwellenwerten sowie die Tatsache, dass die fur die ambulante Versorgung in Deutschland entwickelten Indikatoren-Sets kaum in den eingeschlossenen Studien genutzt wurden, stellt vielversprechende Ansatzpunkte fur intensivierte methodische Diskussionen und Auseinandersetzungen mit Fragestellungen der populationsbezogenen, sektorenubergreifenden Qualitatsmessung in der Versorgungsforschung dar.

Germany was hit by the second wave of the pandemic much harder than by the first wave. The study aims to describe the extent to which nursing homes (NH) are affected by COVID-19. About 8,000 NHs were invited to participate in two waves of an online survey, with a share of 5-10% participating. The share of all deceased NH-residents with COVID-19 is about 50% (04/2020-02/2021). Factors that increase the risk of an outbreak in NH are the spread of the virus in the population, the size of the institution and staff-resident-ratio. The initial lack of protective equipment has decreased during the second wave, but the facilities have to cope with massive additional care needs with reduced staff. NHs have partly banned contacts between residents and relatives. As a conclusion the support of NH in their attempt to fight the impact of this and further pandemic situations requires highest attention.

BACKGROUND Artificial intelligence (AI) holds the promise of supporting nurses’ clinical decision-making in complex care situations or conducting tasks that are remote from direct patient interaction, such as documentation processes. There has been an increase in the research and development of AI applications for nursing care, but there is a persistent lack of an extensive overview covering the evidence base for promising application scenarios. OBJECTIVE This study synthesizes literature on application scenarios for AI in nursing care settings as well as highlights adjacent aspects in the ethical, legal, and social discourse surrounding the application of AI in nursing care. METHODS Following a rapid review design, PubMed, CINAHL, Association for Computing Machinery Digital Library, Institute of Electrical and Electronics Engineers Xplore, Digital Bibliography & Library Project, and Association for Information Systems Library, as well as the libraries of leading AI conferences, were searched in June 2020. Publications of original quantitative and qualitative research, systematic reviews, discussion papers, and essays on the ethical, legal, and social implications published in English were included. Eligible studies were analyzed on the basis of predetermined selection criteria. RESULTS The titles and abstracts of 7016 publications and 704 full texts were screened, and 292 publications were included. Hospitals were the most prominent study setting, followed by independent living at home; fewer application scenarios were identified for nursing homes or home care. Most studies used machine learning algorithms, whereas expert or hybrid systems were entailed in less than every 10th publication. The application context of focusing on image and signal processing with tracking, monitoring, or the classification of activity and health followed by care coordination and communication, as well as fall detection, was the main purpose of AI applications. Few studies have reported the effects of AI applications on clinical or organizational outcomes, lacking particularly in data gathered outside laboratory conditions. In addition to technological requirements, the reporting and inclusion of certain requirements capture more overarching topics, such as data privacy, safety, and technology acceptance. Ethical, legal, and social implications reflect the discourse on technology use in health care but have mostly not been discussed in meaningful and potentially encompassing detail. CONCLUSIONS The results highlight the potential for the application of AI systems in different nursing care settings. Considering the lack of findings on the effectiveness and application of AI systems in real-world scenarios, future research should reflect on a more nursing care–specific perspective toward objectives, outcomes, and benefits. We identify that, crucially, an advancement in technological-societal discourse that surrounds the ethical and legal implications of AI applications in nursing care is a necessary next step. Further, we outline the need for greater participation among all of the stakeholders involved.

ZusammenfassungDas Virus SARS-CoV‑2 und die damit einhergehende Erkrankung COVID-19 stellen die Gesundheitssysteme weltweit vor große Herausforderungen. Besonders die vulnerable Gruppe pflegebedürftiger Menschen in der Langzeitpflege ist gefährdet, schwere Krankheitsverläufe zu erleiden oder an der Infektion zu versterben.In einer bundesweiten Querschnittsstudie wurden mittels einer Onlinebefragung die Situation und die Bedarfe stationärer und ambulanter Langzeitpflegeeinrichtungen während der SARS-CoV-2-Pandemie erfasst und analysiert.Konkret forderten Teilnehmende aus 531 Einrichtungen einheitliche Handlungsempfehlungen zu SARS-CoV‑2, ausreichende und finanzierbare Schutz- und Hygienematerialien, Reihentestungen in den Einrichtungen, fundierte Beratung bei der Umsetzung von Maßnahmen, einen konkreten Pandemieplan und unterstützende Öffentlichkeitsarbeit in den Medien. Dazu wurden eine höhere Vergütung der Pflegenden, eine bessere Personalausstattung und stärkere Wertschätzung der pflegerischen Profession gefordert.Um die vulnerable Gruppe pflegebedürftiger Menschen vor einer SARS-CoV-2-Infektion zu schützen, muss die Langzeitpflege stärker in den Fokus der gesundheitspolitischen Maßnahmen während der Pandemie gerückt werden.

Background The field of digital technologies being developed or applied to support nursing care is very extensive. The aim of this scoping review is to provide an overview on technologies for which results on positive or negative effects on persons in need of care, caregivers or care institutions are available – and to appraise the reliability of these results. An additional focus is put on the question which care settings and target groups have been addressed by the research so far. Methods A scoping review design has been used to identify studies focussing on the effectiveness of digital technologies in nursing care for persons in need of care, caregivers or care institutions. The screening process included 19.510 scientific publications from 9 databases. Results A total of 123 single studies and 31 reviews were subjected to the analysis. The range of technologies that is researched to support nursing care is wide. The included technologies comprise nursing and health information technologies, such as assistive devices, information and communication technologies (including decision support systems, electronic health records, hospital and nursing home information systems), sensors and robotic technologies. The results show that there are many studies that demonstrate positive effects of the technologies, but the level of evidence is mostly low and study sizes are often small. Hardly any technology has been researched intensively enough to produce conclusive results. Studies on a high level of evidence (RCTs) lack for nearly all technological areas. Heterogeneous results in some areas indicate that effects may depend strongly on the mode and and specific context into which the technologies are introduced. Most studies are situated in inpatient care settings. Conclusion Due to the limited evidence on effectiveness of digital technologies in nursing care, it is not surprising that care institutions are reluctant to put innovative technologies into practice. The scoping review indicates technology areas that should be subject to future research with higher quality studies. Research on outpatient care settings, informal care arrangements and cross-sectoral care should be intensified to further exploit the potential of digital technologies to improve independence of care-recipients and unburden formal an informal carers.

Zusammenfassung. Hintergrund: Pflegebedürftige sind als hoch vulnerable Gruppe von der COVID-19-Pandemie besonders betroffen. Aufgrund ihrer Abhängigkeit von körpernahen Unterstützungsleistungen können Maßnahmen sozialer Distanzierung für sie aber nur sehr eingeschränkt durchgeführt werden. Zudem sind die sozialen und gesundheitlichen Folgen von Kontaktreduktionen für Pflegeheimbewohner_innen besonders hoch. Ziele: Die Studie erhebt die Verbreitung von COVID-19 in Pflegeheimen, die personellen und materiellen Ressourcen der Pflegeheime, den organisatorischen Umgang mit der Situation und die Unterstützungswünsche der Pflegeheime an die Pflegepolitik. Methode: Mehr als 7000 Pflegeheime wurden zur Teilnahme an einer Online-Befragung mit quantitativen und qualitativen Elementen eingeladen (Gelegenheitsstichprobe). Bei einem Rücklauf von 11,4 % liegen Fragebögen von 824 Heimen vor. Diese Daten wurden deskriptiv ausgewertet. Ergebnisse: 824 Pflegeheime nahmen an der Befragung teil. Jedes fünfte Heim hat mindestens einen laborbestätigten Fall von SARS-CoV-2 unter seinen Bewohner_innen und / oder Beschäftigten. Der anfänglich deutliche Mangel an Schutzausrüstung ist inzwischen zurückgegangen, allerdings müssen die Einrichtungen mit verringerten Personalressourcen einen Corona-bedingten Mehraufwand bewältigen. Pflegeheime haben Außenkontakte in einem Maße reduziert, das nun wieder zurückgefahren werden muss. Unterstützungswünsche der Pflegeheime richten sich auf die Bereitstellung und zusätzliche Finanzierung von Schutzausrüstung und Sachmitteln sowie die systematische und regelmäßige Testung der Pflegekräfte und die Bereitstellung bundeseinheitlicher Handlungsanweisungen. Schlussfolgerung: Pflegeheime bedürfen besonderer Aufmerksamkeit, da etwa die Hälfte aller mit COVID-19 Verstorbenen zuvor in Pflegeheimen gelebt haben. Notwendig ist insbesondere die Sicherstellung einer ausreichenden Versorgung mit Schutzausrüstung, die Schulung der Mitarbeitenden und die Durchführung von regelmäßigen Reihentests.

Zusammenfassung. Hintergrund: Pflegebedürftige sind aufgrund des Alters, bestehenden Vorerkrankungen und der Schwere von Krankheitsverläufen von der COVID-19-Pandemie besonders betroffen. Eine Vielzahl täglicher direkter Kontakte von Pflegekräften und Pflegebedürftigen erhöhen die Gefahr für eine Übertragung der Erkrankung. Ziel: Die Studie zielt darauf ab, (I) Fallzahlen ambulanter Pflegedienste und teilstationärer Pflegeeinrichtungen in Zusammenhang mit COVID-19, (II) personelle und materielle Ressourcen, (III) die Versorgungssituation von Pflegebedürftigen und (IV) den organisatorischen Umgang mit der Pandemiesituation zu beschreiben sowie Unterstützungswünsche an die Politik zu skizzieren. Methoden: Mehr als 12 000 ambulante und teilstationäre Pflegeeinrichtungen wurden zur Teilnahme an einer Online-Befragung eingeladen (Gelegenheitsstichprobe), 701 Pflegedienste (Rücklauf 7,3 %) und 96 teilstationäre Einrichtungen (Rücklauf 3,5 %) nahmen teil. Ergebnisse: Fast ein Drittel der befragten Pflegedienste (30,1 %, 189 / 627) sind durch (Verdachts-)Fälle von COVID-19 bei Klient_innen betroffen. Jeweils etwa die Hälfte der Dienste berichten von einer verringerten Inanspruchnahme von SGB V- (47,6 %, 288 / 605) bzw. SGB XI-Leistungen (59,7 %, 375 / 628). Zwei Drittel der teilstationären Einrichtungen (65,6 %, 63 / 96) geben an, aufgrund der Pandemie derzeit geschlossen zu sein. Ebenso berichteten fast die Hälfte der Dienste (45,8 %, 253 / 552), dass die Versorgung bei verringerter Leistungsinanspruchnahme von ambulanten Leistungen gefährdet / instabil oder sogar nicht sichergestellt sei. Schlussfolgerung: Die Ergebnisse zeigen, dass ambulant versorgten Pflegebedürftigen in der vorliegenden Pandemie – und auch mit Blick auf eine mögliche zweite Welle – eine erhöhte Aufmerksamkeit in Bezug auf prekäre Versorgungssituationen zukommen sollte.

Even though the importance of nurses' participation in the process of technology development is frequently stressed by stakeholders, participation has been described as negligible and limited by nurses' abilities to identify and communicate their needs and ideas for application scenarios or the improvement of digital nursing technologies (DNT) in everyday care practice. Prior research often uses hypothetical scenarios or laboratory settings with little real-world relevance, and the number of studies investigating needs for technology development, application scenarios or requested technologies from the perspective of nurses with experience in technology use is exceedingly small. Against this background, this study aims to investigate needs, application scenarios and perspectives of nurses with practical experience in real-world DNT application in a range of different care settings.An explanatory sequential mixed methods design including an online survey (QUANT) and focus group (FG) discussions (qual) was used to explore nurses' perspectives and experiences. A convenience sample of participants was accessed via 19,000 e-mail contacts of directors of nursing (DONs) in care facilities and hospitals throughout Germany. Preliminary results of the online survey were discussed and elaborated in depth in three FGs. Quantitative results of the online survey were included in the development of the interview guideline for and data collection from the FGs. Descriptive, setting-specific analysis was conducted for quantitative data, and qualitative data was analysed by identifying key aspects.A total of 1,335 participants took part in the online survey, most of whom worked in ambulatory care institutions and held management positions such as DON or team leader. There were 14 FG participants. Ninety-five per cent of the participants of the online survey reported having experience in the use of DNT, predominantly with information and communication technologies (ICT). Overall, DNT were deemed to make work easier, and participants concurred on other positive effects such as increased efficiency or saved time and improved quality of care. Negative effects or concerns were reported less frequently. Reasons for non-adoption included technology-related (e.g. usability, functionality) and non-technology-related (e.g. competencies and context factors) issues, and facilitators for adoption were discussed in the FGs. Key aspects of application scenarios were enhanced technological support of direct nursing care tasks to reduce physical burden and mental stressors. Specifically, participants of the FGs expressed their wish for participative development and a general openness for nurses to be included in the development and testing of digital technologies.Although efforts in development, research and theory-building have been increasing over recent years, DNT that go beyond more traditional or common applications within the ICT category (such as electronic nursing records or process planning) are rare. There are already technologies available for many of the expressed needs for which, however, adoption fails or does not happen. The reported barriers and facilitators indicate issues that should be taken into account when developing DNT for application in nursing practice. The study reveals a distinct need for information, counselling, facilitation and organizational development, and brings to light broad opportunities for the collaborative development of guided DNT implementation and evaluation processes. Future development and research activities should preferably be conducted by interdisciplinary research groups.

Care-dependent community dwelling people are vulnerable to deficits in medical care provided by the German statutory health insurance (GKV). Quality of care indicators (QIs) contribute to the identification of deficits and of potential for the optimization of care. To investigate the discriminatory ability of QIs in a population of elderly people with and without care dependency and different age structures, insights into the feasibility of such QIs based on health claims data are of interest. The aim of this study is an explorative approach to health claims based QIs for the ambulatory medical care of care-dependent elderly which can be used to optimize health care processes.This cross-sectional study used anonymised health claims data of a sample of statutory health insurance members from all German federal states of the year 2016. The sample consisted of 5,934,986 insured persons aged 65 years or older, who were community dwelling with (n = 785,135) or without care-dependency (n = 4,799,369) or who were residents of a nursing home (n = 350,482). 47 QIs focusing on ambulatory care-sensitive conditions such as, asthma, chronic obstructive pulmonary disease, cardiovascular diseases, diabetes mellitus type 2 and depression were calculated on a quarterly basis and are reported descriptively as the arithmetic mean of four quarters stratified by place of residence and age group.The majority of QI values vary between the observed groups with an overarching trend to the disadvantage of older individuals. Even though care-dependent insured persons show higher prevalence rates of the underlying diagnoses of the QIs (except for asthma diagnoses), they score more detrimental QI values than those without care-dependency. This finding holds true after stratification by age group.This study describes differences in the quality of medical care for elderly people. Considering prior empirical evidence of deficits in medical care, factors that act as barriers or facilitators of guideline-oriented medical care need to be investigated. The contribution of patients' preferences, access to medical care or prioritisation by medical and nursing care providers when initiating diagnostic or therapeutic procedures remains unclear. Methodical limitations of this study notably derive from the selection process of the observed groups using two stratification variables. For the further development of QIs other influencing factors on both an individual and a population-related level as well as the providers' ability to influence these factors need to be taken into account and incorporated in a risk-adjusted description of QIs.Measurement of ambulatory medical care of an elderly population based on health claims data is feasible and shows differences in care processes of older and care-dependent individuals if place of residency and level of care-dependency are considered. Further development of the QIs explored should incorporate a thorough methodical foundation, particularly with regard to risk adjustment. In addition, the contribution of individual and contextual factors on QIs remains to be examined with a view to the community-dwelling care-dependent elderly and their ongoing residency in this setting.

Knowledge about predictors of nursing home placement is highly relevant. If they are known, targeted interventions such as counselling can help prevent or delay relocation to a nursing home. Above all, preventive and structural measures can target influenceable predictors. The aim of the review was to map predictors of nursing home admission and thus the permanent stay in inpatient long-term care for the target group of people with a pre-existing need for care.A systematic literature search in the databases The Cochrane Library, PubMed, CINAHL, GeroLit and CareLit® was conducted in September 2017. Longitudinal studies with quantitative analyses were included. The methodological quality was assessed using the Newcastle-Ottawa Scale.45 cohort studies were included. 21 studies examined study populations with need for care, in 24 studies the care status was unclear. A variety of predictors of nursing home admission with at least one-and-a-half times higher risk for both study populations could be identified, including higher age, dementia, underweight, higher age of caregiver, psychiatric symptoms, smoking status, ethnicity, challenging behaviour in people with dementia, higher number of hospitalizations and lower level of life satisfaction.All the predictors identified are relevant to people in need of care. No predictors for the study population with need of care could be identified that are particularly important in contrast to persons with an unclear care status. However, the quality of studies among people with care and support needs is limited compared to studies targeting people with unclear care status.Many factors seem to predict the admission to a nursing facility. For the first time, these factors are listed in this systematic review for the target group of people in need of care. Interventions or preventive measures based on known influenceable predictors can help prevent or delay nursing home admission.

Shared-housing arrangements are relevant as a possible living arrangement alternative to living in a nursing home or residing in their own home for people in need of care. The aim was to record demonstrable differences in health-related outcomes between residents of shared-housing arrangements and residents of nursing homes.A rapid review was conducted to identify health-related outcomes in shared-housing arrangements. The literature search was carried out in September 2017 in the databases PubMed, CINAHL, Gerolit and LIVIVO. Comparative studies of German or English language were included when the nursing home was examined as a control group.The majority of the 21 studies included describes a favorable contribution of shared-housing arrangements compared to nursing homes. Quality of life is a frequently examined outcome that provides evidence of an advantage of shared-housing arrangements, especially for persons with dementia.A variety of health-related outcomes were examined. The results provide indications of favorable contributions of shared-housing arrangements. For the most part, there is still no conclusive evidence for outcomes based on studies of higher levels of evidence.

Im Auftrag des Senators fur Gesundheit im Bundesland Bremen fuhrte das Institut fur Public Health und Pflegeforschung (IPP), Universitat Bremen, eine Studie zur Erfassung von Problembereichen in der Versorgung alterer Menschen ab 65 Jahren sowie zur Identifikation von Ansatzpunkten zur Optimierung der Versorgung im Zeitraum von Mai bis Oktober 2014 im Lande Bremen durch. Die Datenerhebung erfolgte anhand des Verfahrens der Delphi-Methode. Hierzu wurden 87 ausgewahlte Expert_innen aus unterschiedlichen Bereichen des bremischen Gesundheitswesens zur Teilnahme an zwei aufeinander aufbauenden online-Befragungen eingeladen. Nach deskriptiv quantitativer sowie inhaltsanalytisch qualitativer Auswertung der erhobenen Daten wurden in einem Workshop die identifizierten Problembereiche vorgestellt und Ansatzpunkte fur Losungsstrategien hinsichtlich ihrer Praktikabilitat und Umsetzbarkeit von den Expert_innen diskutiert und einschlieslich der identifizierten Problemfelder abschliesend zu einer Expertise zusammengefasst. Elf Problembereiche liesen sich identifizieren, aus denen heraus die vier Schwerpunktthemen Schnittstellenmanagement, geriatrische Versorgung, Quartier sowie Qualifikation und Ausbildung abgeleitet und entsprechende Losungsvorschlage erarbeitet wurden.

Provider networks in healthcare can emerge as either formal or informal networks. For sector-encompassing population-based quality measurement in informal networks, which allows for conclusions about the cost-effectiveness of care for home-dwelling persons in need of care, a comprehensive review on suitable quality indicators that can be derived from German social health insurance claims data is still lacking.Primary review questions: Which population-based indicators of quality of care in formal and informal provider networks are described in the international literature? Which of these indicators are used as outcome parameters in health economic evaluations, and what are the methodological approaches in these evaluations? Rating approaches and methods for establishing thresholds as well as the validity and suitability of quality indicators to predict quality of care as well as the potential for the calculation of quality indicators based on German social health insurance claims data are included in the secondary review questions.Databases searched in May 2017 and July 2018 included PubMed, The Cochrane Library und NHS EED, CINAHL, GeroLit and EconLit. In addition, we hand-searched references of the studies identified and screened the project database Health Services Research Germany.Quantitative design, German or English language. Any kind of formal or informal network for which distinct members regarding single providers are named and population-based quality indicators for adults (18 years or older) are described.Two authors (Cohen's Kappa = 0.64) independently screened titles, abstracts and full texts. A third independent reviewer was consulted in cases of uncertainty regarding the inclusion of studies. Critical appraisal was conducted using AMSTAR, the Cochrane Risk-of-Bias Tool, the Newcastle-Ottawa Scale (NOS), the Appraisal Tool for Cross-Sectional Studies (AXIS) and the criteria of the Drummond Checklist.137 studies were included, five of which evaluated informal provider networks and applied indicators for medical conditions such as diabetes mellitus or heart failure or events like ambulatory care-sensitive hospitalisations, which were also utilized for formal networks. Five out of 14 health economic evaluations also assessed associations between costs and quality of care. The majority of studies did not include evidence on rating approaches and/or thresholds. Even though the validity and reliability of the used data in single studies is frequently discussed, only one in four of the included studies undertook a discussion of the suitability of the applied indicators. 121 studies explored indicators that can, in whole or in part, potentially be calculated on the basis of German social health insurance claims data and that target medical conditions such as osteoarthritis, asthma, chronic pain, chronic obstructive pulmonary disease, cardiovascular disease, dementia, diabetes mellitus, osteoporosis or mental health disorders as well as ambulatory care-sensitive events, appropriate medication of the elderly and polypharmacy, preventive care and continuity of care.This systematic review identified quality indicators that were predominantly used in formal provider networks and, with sufficient testing and further development, include the possibility of being used for measuring the quality of care in informal networks. The need for further research on suitable approaches to measure the interactions of quality of care and costs and on the validity, reliability and predictive suitability of single indicators as well as the finding that quality indicators especially developed for the German ambulatory sector were rarely used in the included studies constitute promising starting points for both an intensified methodological debate and the critical discussion of issues concerning population-based, sector-encompassing measurement of quality of care in health services research.
