14. Preliminary criteria for classification of adult Still's disease

Abstract

We have attempted to design classification criteria for adult Still's disease by analyzing the data obtained through a multicenter survey of 90 Japanese patients with this disease and of 267 control patients. The proposed criteria consisted of fever, arthralgia, typical rash, and leukocytosis as major, and sore throat, lymphadenopathy and/or splenomegaly, liver dysfunction, and the absence of rheumatoid factor and antinuclear antibody as minor criteria. Requiring 5 or more criteria including 2 or more major criteria yielded 96.2% sensitivity and 92.1% specificity. However, an exclusion process will be needed for an accurate classification, since this disease is relatively rare.

Abstract

The regulation of syngeneic erythrophagocytosis (EP) by macrophages (Mφ) harvested from young and old NZB mice was examined by spectro- photometric assay and morphological observation. Peritoneal exudate Mφ from young NZB mice weakly ingested syngeneic red blood cells (RBC). T cells derived from old NZB mice accelerated ingestion of RBC by young Mφ. On the contrary, T cells from young NZB mice suppressed EP by young Mφ. The suppressive activity of young T cells appeared clearly when they were added to Mφ derived from old mice, which ingested syngeneic RBC actively without help by old NZB T cells. Namely, such an active EP by old Mφ was completely suppressed when they were incubated with young T cells. Simultaneous addition of both young and old T coils to either young or old NZB Mφ with RBC suppressed the EP. Pretreatment of young T cells with anti-Lyt 1·2 antibody and complement (C) made the suppressive activity prominent, and preincubation with anti-Lyt 2·2 and C eliminated the suppressive activity, but gave rise to the enhancing activity. Young T-cell homogenates added to young or old Mφ together with RBC did not reveal suppressive activity for EP, and on the contrary facilitating activity appeared predominantly. Young and old T-cell homogenates added together to young Mφ did not suppress EP. The target of T-cell-factor accelerating EP was Mφ, but not RBC. 症例は35才主婦で,高校時代より手指,足趾,肩,肘関節に一過性の関節痛発作が出没. 35才時,右母指MP関節に関節炎が出現したため,当科受診. X線像上頸部,手指,肩などの関節周囲および両下肢血管壁に沿って石灰沈着像を認めた. CT scan,下肢動脈造影により下肢動脈壁に顆粒状と一部リング状石灰沈着を確認し, Monckeberg動脈硬化症と診断した.本例の関節周囲の石灰沈着は,その部位と形態,及び沈着物の性状分析より塩基性リン酸カルシウム沈着症によるものと考えられた. Monckeberg動脈硬化症と塩基牲リン酸カルシウム沈着症の合併した報告例はいまだない.

Abstract

An in vitro anti-NZB red blood cell (RBC) autoantibody-forming system was developed by culturing young (antiglobulin negative) or old (antiglobulin positive) NZB mouse bone marrow cells in the presence of young or old NZB thymocyte homogenates (or RNA thymus extracts) and young NZB-RBC. Using young NZB bone marrow cells, the greatest number of autoantibody forming cells were seen following stimulation with a mixture of young and old thymocyte homogenates (TH). A higher response was seen with old NZB bone marrow cells stimulated by old NZB thymocyte homogenates and RBC. Phenol-extracted thymic RNA retained activity when added to bone marrow culture containing NZB-RBC. Thymus RNA from 9 to 10 days old NZB mice had no activity although their bone marrow cells responded well to stimulation by young and old thymic RNA together with RBC. By analysing results obtained using T-cells enriched for helper and suppressor activities, we have concluded that abrogation of self-tolerance in mice is due to the appearance of functionally modulated helper T-cell subsets and to imbalance between helper and suppressor T cells.

Abstract

Human IgM that express the variable region genes of the VH3 family bind staphylococcal protein A (SPA). We previously reported that the SPA-binding IgM can be divided into two groups based on the differential binding avidities for solid-phase SPA. To study the molecular basis for these differences, we cloned B cells from human blood by EBV transformation. The nucleotide sequences of the expressed Ig heavy chain genes were determined on 20 B cell clones that produce SPA-binding IgM. The germ-line VH3 gene usage in IgM with high avidities for SPA were distinct from the germ-line VH3 genes used in IgM with low avidities for SPA. There was no correlation in the usage of D or JH genes or in the usage of light chains in IgM according to the SPA binding avidity. These results suggest that the differential binding avidities for SPA are at least partly due to specific germ-line VH3 gene usage. An investigation of direct binding of SPA to the synthetic peptides corresponding to the portions of the variable regions of SPA-binding and non-SPA-binding IgM showed that the peptides corresponding to the VH3 family specific framework region 3 sequences had significant SPA binding capacities, while the peptides corresponding to the other subdomains and those corresponding to framework region 3 of the reported VH3 sequences from non-SPA-binding IgM showed little or no binding. It is of interest that the Ig-framework region 3 subdomain corresponds to the fourth hypervariable region, which in the TCR-beta chain has been implicated as a critical site for T cell superantigen binding.

45. The Utility and Feasibility of Laparoscopic Surgery in Patients Diagnosed With Cervical Cystic Lesions.

Abstract

Introduction: This observation study aimed to differentiate between lobular endocervical glandular hyperplasia (LEGH) and gastric-type mucinous carcinoma (GAS) while evaluating the feasibility and efficacy of laparoscopic surgery in the preoperative diagnosis of cervical cystic lesions., Method and Material: A retrospective study was conducted to evaluate the diagnostic process and laparoscopic surgical management of cervical cystic lesions suspected to be LEGH or GAS. Preoperatively and postoperatively, MRI, cytology, histology, tumor marker analysis, and surgical outcomes (blood loss during surgery, operative time) were assessed. Six individuals were selected based on magnetic resonance imaging (MRI) results indicating a preoperative suspicion of LEGH or GAS. These patients underwent laparoscopic surgical treatment without indications of malignancy based on preoperative histology or cytology., Results: Initially, all individuals were suspected to have LEGH based on MRI findings. Postoperatively, two patients were diagnosed with LEGH, two with adenocarcinoma in situ (AIS) and minimal deviation adenocarcinoma (MDA), and two showed no notable findings on pathology (one diagnosed endometrioid carcinoma in endometrial tissue). Patients with malignancies exhibited longer surgical times and higher intraoperative blood loss. Preoperatively, no significant variation was observed in maximal lesion diameter between adenocarcinoma and LEGH. However, lesion diameter increased significantly over time in patients with GAS., Conclusion: Laparoscopic surgery demonstrated feasibility and provided crucial diagnostic and therapeutic outcomes, with no postoperative recurrence observed in cases of malignancy, despite the challenges associated with preoperative differentiation. These findings underscore the potential of laparoscopic surgery in enhancing both diagnostic accuracy and therapeutic efficacy for cervical cystic lesions, offering promise for improved patient outcomes and management strategies in clinical practice., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. 46. Immunomodulatory Effect of Proteasome Inhibitors via the Induction of Immunogenic Cell Death in Myeloma Cells.

Abstract

Several anti-cancer drugs are known to have immunomodulatory effects, including immunogenic cell death (ICD) of cancer cells. ICD is a form of apoptosis which is caused by the release of damage-associated molecular patterns (DAMPs), the uptake of cancer antigens by dendritic cells, and the activation of acquired immunity against cancer cells. ICD was originally reported in solid tumors, and there have been few reports on ICD in multiple myeloma (MM). Here, we showed that proteasome inhibitors, including carfilzomib, induce ICD in myeloma cells via an unfolded protein response pathway distinct from that in solid tumors. Additionally, we demonstrated the potential impact of ICD on the survival of patients with myeloma. ICD induced by proteasome inhibitors is expected to improve the prognosis of MM patients not only by its cytotoxic effects, but also by building strong immune memory response against MM cells in combination with other therapies, such as chimeric antigen receptor-T cell therapy.

Abstract

We report a new aspect of rapid (<10 seconds) light-induced cell membrane perforation for intracellular electrode by Hematoporphyrin (HP), which is an original photosensitizer for photodynamic therapy concept. A microelectrode insertion process to cell could be improved by pinpoint use of the photosensitizing effect to degenerate cell membranes prior to insertion. According to the concept, simple electrode coating with photosensitizer will enable intracellular electrode array for neural engineering.

Abstract

Reoperation in patients with patent coronary artery bypass grafts behind the sternum is associated with a high risk of graft injury that may be life-threatening. We recently performed mitral valve replacement in a patient with patent coronary artery bypass grafts and grade IV mitral regurgitation. Surgery was safely performed with minimal adhesion dissection and modified Port-Access Technique using an aortic balloon.

Abstract

Purpose: The aim of this study was to evaluate a new flow analyze device that can intraoperatively measure compliance and resistance of coronary artery bypass grafting (CABG) by the parameter identification method., Methods: Subjects were 95 patients who underwent CABG. Angiography soon after surgery confirmed patency in 90 grafts and graft failure in 5 grafts. Variables of aortic pressure and the graft flow were entered into this new device that includes a mathematical coronary circulation model to extrapolate other information the data; thus, we can estimate intraoperative flow rate, resistance, and compliance. Flow rate, resistance, and compliance were compared between patent and failed graft groups and among three different type grafts., Results: There were no differences in flow rate, resistance, and compliance among the three grafts types. Between the patent and failed graft groups, there were statistically significant differences in flow rate and resistance, and compliance tended to be lower for the failed graft group., Conclusions: By measuring graft resistance and compliance, this new device is useful for evaluating graft performance during CABG.

Abstract

A 71-year-old woman with idiopathic thrombocytopenic purpura (ITP), who had been treated with steroid and cyclosporine, was admitted in an emergency with fever and dyspnea. The diagnosis was mitral regurgitation due ton infective endocarditis. Although she received treatments for infection and cardiac failure, the cardiac failure could not be controlled. After high-dose γ-globulin therapy, an emergency operation was performed during the active phase of infective endocarditis. Rapid platelet transfusion was administered after weaning from extracorporeal circulation. She recovered and was discharged without postoperative bleeding and re-infection.The treatment course of elective cardiac surgery complicated with ITP has been established, but the course of emergency surgery has not been established because of the small number of cases reported. Since few patients have undergone emergency surgery for active infective endocarditis, we had difficulty in deciding the time of surgery and treatment for increasing the number of platelets before surgery, it was considered that the case provided us with useful suggestion for the future treatment for urgent surgery complicated with ITP.
