SUMMARY OBJECTIVE: The objective of this study was to examine the effects of orlistat use on metabolic control and weight loss in diabetic and nondiabetic patients. METHODS: A total of 119 patients with body mass index≥40 kg/m2 and receiving orlistat therapy, who applied to the Endocrinology polyclinic between January 2016 and October 2019, were included. The patients’ weight changes and biochemical values (i.e., fasting glucose, HbA1c, ALT, creatinine, and lipid parameters) were evaluated at the drug beginning and the last polyclinic control. The patients were divided into groups, whether they had diabetes or used metformin, and compared. RESULTS: The mean age of the 119 patients in the study was 45.3±11.5 years. A total of 94.1% of the patients were females and 5.9% were males. A total of 38.7% of the patients had diabetes and 29.4% had prediabetes. When the patients were compared to whether they had diabetes or used metformin, there was a statistically significant difference between the groups according to weight loss. The mean weight change of patients without diabetes and receiving metformin and orlistat was statistically significantly higher than that of patients with diabetes and receiving metformin and orlistat. DISCUSSION: It was determined that the weight loss effect of orlistat in obesity was seen in all groups, but this effect decreased in the diabetic group.

Amaç: Bu çalışmada D vitamini düzeyinin hemiplejik hastalarda denge ve düşme riski üzerine etkisinin araştırılması amaçlanmıştır. Yöntem: Tanımlayıcı tipteki çalışmaya Maçka Ömer Burhanoğlu Fizik Tedavi ve Rehabilitasyon Hastanesinde Temmuz 2020-Ocak 2021 tarihleri arasında Fiziksel Tıp ve Rehabilitasyon Polikliniğine hemipleji tanısı ile başvuran ve yatırılarak tedaviye alınan toplam 152 hasta alınmıştır. D vitamini eksikliği ya da yetersizliği saptanan hastalara Türk Endokrinoloji ve Metabolizma Derneği önerilerine uygun olarak damla formülünde D vitamini tedavisi verildi. Bulgular: Çalışmaya alınan 152 hastanın 57’si (%37,5) kadın, 95’i (%62,5) erkektir. Hastaların yaş ortalaması 62,0± 12,7 idi. Çalışmaya alınan hastaların sigara içenlerin sayısı 19 (%12,5), sigarayı bırakanların sayısı ise 35 (%23,0) idi. Hastaların hastaneye yattıkları ilk gün ölçülen D vitamini düzeyleri 20 ng/ml olan hasta sayısı 32 (%21,0) idi. Communication, which constitutes a large part of healthcare delivery, forms the basis of medical care. Good relationships can be established and individual’s needs can be understood by providing correct information exchange with effective communication. Being a foreigner in a country and not understanding the language spoken in that country poses a risk in accessing healthcare services. Migration, is an action caused by social change, affects the whole society in both the receiving and originating country. Firstly classic communication barriers are encountered when considering the adaptation process in terms of health communication. Although the concept of “mother tongue” is at the top, when viewed with the sub dimensions, spoken language, typefaces, terminology, visual symbols, the alphabet used and differences in dialect and accent also have an important place. Immigrants may have difficulties in describing their health problems due to differences in their mother tongue and culture, in this way the need for a interpreter may arise. Health professionals’ experiences with immigrants are also important. Mostly communication-related difficulties such as lack of communication, not getting informed consent, inability of empathizing, not effectively carrying out the diagnosis and treatment process can be experienced. In our country great steps have been taken to facilitate the provision of health services to immigrants. The interpreter support was provided in existing health centers. Separate health centers and call centers had been established, even foreign healthcare workers had been employed in these centers. However, the biggest barrier for immigrants to access healthcare services is still communication problems. The communication skills of existing interpereters and encouraging community translation services are need to be improved, especially in order to fill the gap of qualified interpereters., Sağlık hizmeti sunumunun büyük bir bölümünü oluşturan iletişim, tıbbi bakımın temelini oluşturur. Etkili iletişim ile doğru bilgi alışverişi sağlanarak iyi ilişkiler kurulabilir ve bireyin gereksinimleri anlaşılabilir. Bir ülkede yabancı olmak, o ülkede konuşulan dili anlamamak, sağlık hizmetlerine ulaşımda risk oluşturmaktadır. Göç, toplumsal değişimin neden olduğu bir eylem olup hem göç alan hem göç veren ülkedeki bütün toplumu etkiler. Uyum sürecine sağlık iletişimi açısından bakıldığında, öncelikle klasik iletişim engelleri ile karşılaşılmaktadır. Bunların başında “ana dil” kavramı olsa da alt boyutları ile bakıldığında konuşma dili, yazı karakterleri, terminoloji, görsel simgeler, kullanılan alfabe, ağız ve şive farklılıkları da önemli bir yer tutmaktadır. Göçmenler, ana dil ve kültür farklılıkları nedeniyle yaşadığı sağlık problemlerini anlatmada güçlük çekebilmekte ve bir çevirmen (tercüman) gerekliliği ortaya çıkabilmektedir. Sağlık çalışanlarının göçmenlerle ilgili yaşadığı deneyimler de önemlidir ve çoğunlukla iletişim eksikliği, bilgilendirilmiş onam alamama, empati kuramama, tanı ve tedavi sürecini etkin yürütememe gibi iletişim kaynaklı sıkıntılar yaşanabilmektedir. Ülkemizde de göçmenlere verilecek sağlık hizmetleri sunumunu kolaylaştırmaya yönelik büyük adımlar atılarak, ayrı sağlık merkezleri kurulmuş, mevcut sağlık merkezleri içinde tercüman desteği sağlanmış, çağrı merkezleri kurulmuş ve hatta yabancı sağlık personeli çalıştırılması yoluna gidilmiştir. Ancak hala yabancıların sağlık hizmetlerine ulaşmasının önündeki en büyük engel iletişim sorunlarıdır. Özellikle nitelikli tercüman açığının kapatılması için toplum çevirmenliği hizmetlerinin özendirilmesi, mevcut tercümanların sağlık ve iletişim becerilerinin geliştirilmesi gerekmektedir.

GİRİŞ ve AMAÇ: Araştırma, sağlıkla ilgili bölümlerde öğrenim gören üniversite öğrencilerinin sağlık okuryazarlığı ile sağlıklı yaşam biçimi davranışlarını değerlendirmek amacıyla yapılmıştır. YÖNTEM ve GEREÇLER: Tanımlayıcı ve kesitsel tipte olan araştırma Şubat- Mayıs 2020 tarihleri arasında yürütülmüştür. Araştırma, üç farklı devlet üniversitesinde sağlıkla ilgili bölümlerde öğrenim gören ve araştırmaya katılmayı kabul eden 1086 öğrenci ile tamamlanmıştır. Verilerin toplanmasında, Tanıtıcı Bilgi Formu, Türkiye Sağlık Okuryazarlığı Ölçeği ve Sağlıklı Yaşam Biçimi Davranışları Ölçeği II kullanılmıştır. BULGULAR: Öğrencilerin Türkiye Sağlık Okuryazarlığı Ölçeği’nden aldığı puanların ortalaması 36,2±8,3 ve %41,3’ü yeterli sağlık okuryazarlığına sahiptir. Öğrenciler Sağlıklı Yaşam Biçimi Davranışları Ölçeği II puan ortalaması ise 125,3±24,0’dir ve orta düzeydedir. Cinsiyet, gelir durumu, sağlık algısı, anne ve babanın eğitim durumu, öğrenim görülen fakülte ve bölüm gibi bazı özelliklerin öğrencilerin ölçeklerden aldıkları puanlar üzerinde etkili olduğu görülmüştür. Sağlık okuryazarlığı yüksek olan bireylerin daha fazla sağlıklı yaşam biçimi davranışlarına sahip oldukları belirlenmiştir. TARTIŞMA ve SONUÇ: Sağlık bilimlerinde okuyan öğrencilerin sağlık okuryazarlığı ve sağlıklı yaşam biçimi davranışları düzeyinin arttırılması büyük önem taşımaktadır.

Amaç: Bu araştırma, COVID-19 pandemisinin sağlık programlarında öğrenim gören öğrencilerde belirsizliğe tahammülsüzlüğe etkisini belirlemek amacıyla yapılmıştır. Gereç ve Yöntemler: Tanımlayıcı olarak planlanan araştırmanın evrenini Karadeniz bölgesinde bulunan iki devlet üniversitesinin meslek yüksekokulunda öğrenim gören sağlık bölümü (diyaliz, ilk ve acil yardım, fizyoterapi, yaşlı bakımı, çocuk gelişimi, tıbbi laboratuvar teknikleri, tıbbi dokümantasyon ve sekreterlik) 2. sınıf öğrencileri oluşturmuş olup çalışmaya katılmayı kabul eden 807 öğrenci örneklemi oluşturmuştur. Veriler Mayıs 2020 de birey tanıtım formu, belirsizliğe tahammülsüzlük ölçeği kullanılarak toplanmıştır. Verilerin istatistiksel analizinde; sayı, yüzde, ortalama ile verilmiş, karşılaştırmalarda Student-t test ve ki-kare testi kullanılmıştır.Bulgular: Araştırmaya katılan öğrencilerin %76,6’si kadın, %70,8’inin gelir durumu orta düzeyde, %47’sinin il merkezinde yaşadığı belirlenmiştir. Öğrencilerin %95,2 sosyal izolasyon önlemi almışken %18,3 kendisi veya yakınları karantinaya alınmıştır. Öğrencilerin pandemi döneminde ruh hali değişimi incelendiğinde %53,7’sinin kötüye gittiği, %63,2’sinin psikososyal destek hizmeti aldığı belirlenmiştir. Öğrencilerin COVID – 19’a ilişkin verileri ile belirsizliğe tahammülsüzlük ölçeğinden alınan puanlar karşılaştırıldığında, ruh hali değişimini kötü olarak niteleyenlerde, karantinaya alınanlarda, psikososyal destek hizmeti alanlarda ölçek puanlarında istatistiksel olarak farkın önemli olduğu belirlenmiştir (p, Aim: The study examines to determine the effect of COVID-19 pandemic on uncertainty and intolerance of students studying in health programs.Material and Methods: The population of the research, which is planned as a descriptive, is consisted of second-year students in the department of health studying at the regional vocational school of the two state universities in the Black Sea region and 807 students who have agreed to participate in the study sample. In the statistical analysis of data, the dual comparison of the number, percentage, average is performed with Student T test and Ki Square test.Results: It has been determined that 76.6% of the students participating in the study are women, 70.8% of them have a moderate-income level, 47% of them live in the city center. While 95.2% of the students took social isolation measures, 18.3% of them or their relatives were quarantined. When the mental health change of the students were questioned during the pandemic period, it was determined that 53.7%of them got worse and 63.2% of them received psychosocial healthcare. Amaç: Toksooplasma gondii, dünya çapında en yaygın zoonotik parazitlerden biridir. Bu çalışmada Trabzon ilinde ikamet eden 20 yaş ve üzeri bireylerde Toksoplazma gondii seroprevalansının tespit edilmesi ve seroprevalansla ilişkili olabilecek faktörlerin değerlendirilmesi amaçlanmaktadır. Gereç-Yöntem: Kesitsel tipteki araştırma, Eylül 2017 – Kasım 2018 tarihleri arasında Trabzon ilinde yapılmıştır. Araştırmanın evrenini Trabzon'da ikamet eden 20 yaş ve üzeri 567059 birey oluşturmaktadır. Hesaplanan örneklem büyüklüğü %50 bilinmeyen prevalans, %3 sapma, 1 desen etkisi ile 1066 kişidir. Çalışmada kırsal-kentsel, cinsiyet ve yaşa göre tabakalandırma yapıldığı için %40 fire payı eklenerek 1500 kişiye ulaşılması hedeflenmiş, 1502 kişiye ulaşılmıştır. İki aşamada gerçekleştirilen saha çalışmasının ilk aşamasında hane ziyaretleri gerçekleştirilmiş, Toksoplazma ile ilişkili olabilecek faktörlerin sorgulandığı anket formu yüz yüze görüşme yöntemiyle uygulanmıştır. İkinci aşamada katılımcılar Aile Sağlığı Merkezlerine davet edilmiş, Toksoplazma antikorlarını saptayabilmek amacıyla kan alımı işlemi gerçekleştirilmiştir. Serum örneklerinde elektrokemilüminesans immunoassay (ECLIA) yöntemi ile spesifik Toxoplasma IgG ve IgM antikorlarının varlığı ve düzeyi araştırılmıştır. Bulgular: Araştırma kapsamında katılımcıların %58,8'inde IgG, %2,3'ünde IgM seropozitifliği saptanmıştır. Doğurgan çağdaki kadınlarda (20-49 yaş grubunda) seropozitiflik %43,2, gebe olan 27 kadında ise bu oran %33,3'tür. Yaş ve vücut kitle indeksi arttıkça toksoplazma seroprevalansının arttığı, gelir arttıkça ise seroprevalansın düştüğü gözlenmiştir. Bahçe-tarla işleri ile uğraşanlarda, hayvancılık yapanlarda, kırsal ilçelerde ikamet edenlerde, medeni durumu evli olanlarda, eğitim düzeyi ilkokul ve altı olanlarda, riskli meslek sahiplerinde, diyabet hastalığı olanlarda ve psikiyatrik rahatsızlığı olanlarda seropozitiflik istatistiksel olarak önemli düzeyde yüksek bulunmuştur. Çiğ et, çiğ süt, yıkanmamış sebze-meyve tüketimi olanlarda, kedi besleyenlerde ve işlenmemiş su kullananlarda seropozitiflik açısından önemli bir fark saptanmamıştır. Lojistik regresyon modelinde yaş, eğitim durumunun ilkokul ve altı olması ve bahçe-tarla işleriyle uğraşma bağımsız risk faktörü olarak tespit edilmiştir.Sonuç: Çalışmanın toplum tabanlı gerçekleştirilmesi, 20 yaş üstü her yaş grubundan kişilerin katılımının olması nedeniyle görece yüksek oranda seropozitiflik saptanmıştır. Eğitim durumu düşük olanlarda, bahçe-tarla işleriyle uğraşanlarda seropozitifliğin yüksek görülmesi toksoplazma paraziti hakkındaki eğitimin önemini ortaya koymaktadır. Gebe ve doğurgan çağdaki kadınlarda toksoplazma enfeksiyonunu geçirmemiş olan kişiler, dikkat edilmesi gereken bir diğer gruptur.Anahtar Kelimeler: Toksoplazma, toplum, seroprevalans, Trabzon Objective: Toxoplasma gondii is one of the most common zoonotic parasites in the worldwide. In this study, the purpose is the detection of Toxoplasma gondii seroprevalence and evaluation of factors related to seroprevalence in individuals aged 20 and over residing in the province of Trabzon.Material-Method: A cross-sectional study was conducted in the province of Trabzon between September 2017 and November 2018. The population of the study consists of 567059 individuals aged 20 and over residing in Trabzon. The calculated sample size with %50 unknown prevalence, %3 deviation and 1 pattern effect is 1066 people. In this study, it is aimed to reach 1500 people by adding %40 because of stratification according to rural-urban, gender and age, in the end reached out 1502 people. In the first phase of the fieldwork which performed in two stages, household visits were carried out and using face to face interview method, a survey which questioning the factors related to toxoplasma was performed. In the second phase, the participants were invited to Family Health Centers and to detect toxoplasma antibodies, blood collection was carried out. In the serum samples, the presence and level of specific toxoplasma IgG and IgM antibodies were investigated by electrochemiluminescence immunoassay (ECLIA) method.Results: In the scope of the research, IgG and IgM seropositivity was found respectively %58,8 and %2,3 of the participants. Seropositivity rate in childbearing age women (20-49 age group) is %43,2 and in 27 women which is pregnant, this rate is %33,3. It was observed that as age and mass body index increasing, toxoplasma seropositivity increased and as income increasing, seropositivity decreased. Seropositivity was found to be statistically significant in who works with animals, who lives in rural districts, whose marital status is married, whose educational status is primary school or less, who works at dangerous jobs, who have diabetes and psychiatric discomfort. There wasn't found any significant difference in terms of seropositivity in who eats raw meat, raw milk or unwashed vegetables-fruits, who owns cats and who uses unprocessed water. In the logistic regression model, the age, the educational status which is primary school or less and farming have been identified as independent risk factor.Conclusion: Because of the study carried out community-based and participation in all age groups over the age of 20, relatively high seropositivity was found. High seropositivity in farmers those with low educational status reveals the importance of education on toxoplasma parasites. Introduction. Water management is ontop of the agenda for all countries Aimof the study. The aim of this study was to determine the water usage in thecar wash facilities of Ortahisar district of Trabzon province, such as thefeatures of the water used in drinking and car washing, the disposal ofwastewater and case of water interruption situation in the facilities. Material and methods. This descriptivestudy was conducted in Ortahisar district of Trabzon province. The studypopulation of the research is not fully known. All the car wash facilities inthe district of Ortahisar in Trabzon province were tried to be reached. Aquestionnaire consisting of questions evaluating the water use characteristicsof the facilities was applied to 61 (85.9%) business owners who had acceptedthe interview by face to face interview method. Results. 20 (32.8%) fuel stations and 41 (67.2%) car carefacilities participated in the survey. 49 (80.3%) of the facilities were usingtap water as drinking water source. 37 (60.7%) of the facilities were using tapwater as car washing water source and 28 (45.9%) of them were using well water.The median monthly water consumption of facilities using only tap water for carwashing was found 43 (8.6-429.8) tons. The median number of vehicles washed perday was 16.5 (4-100). 4 of the facilities (6.6%) discharged waste water tosewerage without treatment. Conclusions.Car washing causes considerable amount of water usage which is prominentlydrinking water.Key words: Car wash, watermanagement, Trabzon, wastewater
