Ghana is undergoing a rapid and dramatic food system transition, driven by urbanization and globalization. The ways that people eat are changing, and rates of new diseases – diet-related illnesses like diabetes, hypertension and heart disease – are becoming much more common. While these changes are worrisome from a public health standpoint, the ways in which they are popularly articulated are concerning in a different way. The models that are used to describe these transitions risk imposing a deterministic trajectory modeled on Western history and values, which can inhibit other ways of imaging food systems change. Case studies on local food knowledges and movements reveal other ways of imagining. The first case study considers a Ghanaian local food movement in Accra which challenges assumptions of linear progress by looking to local “traditional” foods to solve “modern” problems. The movement expands the definition of what qualifies as local in a way that makes space for Ghana’s hybrid postcolonial context. The second case explores how everyday people in the neighboring city of Tema account for the rise in new diseases. People draw a connection between the increased use of chemical fertilizer and new disease, which can be understood as a metaphor for broader concerns of modernization, highlighting how the often-violent historical process of modernization still causes harm.

Despite decades of action to reduce global malnutrition, rates of undernutrition remain stubbornly high and rates of overweight, obesity and chronic disease are simultaneously on the rise. Moreover, while volumes of robust research on causes and solutions to malnutrition have been published, and calls for interdisciplinarity are on the rise, researchers taking different epistemological and methodological choices have largely remained disciplinarily siloed. This paper works to open a scholarly conversation between “mainstream” public health nutrition and “critical” nutrition studies. While critical nutrition scholars collectively question aspects of mainstream nutrition approaches, they also chart a different way to approach malnutrition research by focusing on politics, structural conditions, and the diverse ways people make sense of food and malnutrition. In this paper, we highlight the key research agendas and insights within both mainstream and critical nutrition in order to suggest spaces for their potential conversation. We ultimately argue that global public health nutrition interventions might achieve greater success in more equitable ways if they are informed by critical nutrition research. We aim for this intervention to facilitate more substantial crossing of disciplinary boundaries, critical to forging more socially and environmentally just dietary futures in the global South and beyond.

In the past 15 years, Senegal has made considerable progress in the fight against child undernutrition. To better understand how this was achieved, we reviewed 11 national policy documents published between 2001 and 2015, and interviewed 25 key-informants from the government, donor community, civil society, as well as 24 local service providers and 18 community members. We explored the commitment of actors and coherence between and within sectors. We linked this to changes in nutrition services experienced by communities . The key theme that emerged was the critical role of increased political commitment, materialized and maintained by a high-level national coordinating body for nutrition. The body actively facilitates multisectoral coherence in action for nutrition, overseeing the implementation of a national nutrition program, the effects of which are seen at the community level.

Changes in the immediate, underlying and basic determinants of nutritional status at the community- and household-level are a logical and empirical prerequisite to reducing high levels of undernutrition in high burden countries. This paper considers these factors directly from the perspective of community members and frontline workers interviewed in six countries in South Asia and sub-Saharan Africa. In each country, in-depth interviews were conducted with mothers, other community members and health workers to understand changes in health and nutrition practices, nutrition-specific interventions, underlying drivers and nutrition-sensitive interventions, and life conditions. Overall, the need for basic improvements in livelihood opportunities and infrastructure are solidly underscored. Nutrition-specific and -sensitive changes represented in most cases by deliberate government or NGO supported community interventions are rolling out at a mixed and uneven pace, but are having some significant impacts where solidly implemented. The synthesis presented here provides an invaluable source of information for understanding how community-level change occurred against a wider backdrop of national level progress.
