Since biochemical tests play an important role in the decisions of clinicians, it become necessary for each region andeach laboratory to have different biochemical results. When the reference population is obtained from hospital data, thehealthy and the diseased groups are also mixed. By using Bhattacharya procedure, it is possible to have a healthy subgroup out of the mixed data. The main objective of this study is to introduce Bhattacharya procedure that will providea healthy selection of population while calculating the reference interval by using hospital data, and to have a practicefor Aspartat Transaminase (AST) parameter. Big data set is required in order to use Bhattacharya method (n≈8000).Aspartat Transaminase (AST) data of 40.486 registered individuals applying only once to the polyclinic of Health,Research and Application Hospital of Mersin University, Clinic Biochemical Laboratory is used in this study. Referenceinterval is also calculated by using the nonparametric method and the same data. In the nonparametric method, thereference interval is calculated in the range of [12.28-49.00] with 95% confidence. According to Bhattacharya method,it is calculated as [7.10-32.26]. The comparison of the results shows that in Bhattacharya method, the reference intervalis narrower in comparison to the nonparametric method, Biyokimyasal testlerin, klinisyenlerin verdiği kararlarda önemli rol oynaması, her bölgenin ve her laboratuvarın farklı biyokimyasal sonuçlara sahip olmasını gerektirmiştir. Referans popülasyon, hastane verilerinden elde edildiği zaman sağlıklı ve hasta grupları birbirine karışır. Bhattacharya prosedürü kullanılarak bu karışık verilerden sağlıklı alt grup elde edilebilir. Bu çalışmanın amacı, hastane verileri ile referans aralık hesaplaması yapılırken, sağlıklı popülasyonun seçimini sağlayacak Bhattacharya prosedürlerini tanıtmak ve Aspartat Transaminaz (AST) parametresi için bir uygulama yapmaktır. Bhattacharya yönteminin kullanılabilmesi için büyük veri setlerine ihtiyaç vardır (n≈8000). Çalışmada Bhattacharya yönteminin adımlarını anlatmak için Mersin Üniversitesi Sağlık Araştırma ve Uygulama Hastanesi Klinik Biyokimya Laboratuvarında sadece bir kez polikliniğe başvuran hastaların kayıtlı 40.486 bireye ait ASpartatTransaminaz (AST) ölçümleri kullanılmıştır. Aynı veriler kullanılarak parametrik olmayan yöntemle de referans aralık hesabı yapılmıştır. Parametrik olmayan yöntemle referans aralığı %95 güvenle [12.28-49.00] aralığında hesaplanmıştır. Bhattacharya yöntemine göre [7.10-32.26] olarak hesaplanmıştır. Elde edilen sonuçlar karşılaştırıldığında parametrik olmayan yönteme göre Bhattacharya yönteminin referans aralığı daha dar bir aralıkta çıkmıştır.

The Comparison of Ion Exchange Chromatography and Immunoturbidimetric Methods of HemoglobinA1c Results Objective: Development of chronic complications of Diabetes Mellitus, a public health problem, is related with long term blood glucose levels of the patient. Today, measurement of blood hemoglobin A1c (HbA1c) levels were used as an indicator of the risk for development of DM related complications and average retrospective glisemic status. There exist numerous different analysis methods for the determination of blood HbA1c levels in routine test laboratories. Method: The relationship between the blood HbA1c data obtained with both immunoturbidimetric method by Cobas Integra 800 and ion exchange chromatography by Agilent 1100 HPLC system was evaluated. Results: The mean±standart deviation of blood HbA1c levels obtained by both Cobas Integra 800 and Agilent 1100 HPLC system are 6.06±1.83 and 6.29±1.59; respectively. Klinik laboratuvar testlerinin tıbbi karar amacıyla kullanılabilmesi için her testin referans aralıklarının bilinmesi gerekir. Referans aralık değerlerinin coğrafi bölge, ırk, cinsiyet, yaş ve beslenme alışkanlıkları gibi faktörlere bağlı olarak değişmesi nedeniyle her laboratuvar kendi referans aralık değerlerini belirlemelidir. Ancak referans aralıklarının hesaplanması oldukça zordur. Son yıllarda hastane verileri kullanılarak, referans aralıklarının indirekt yöntem ile daha kolay hesaplanabildiğine dair yayınların sayısı hızla artmaktadır.Bu çalışmanın amacı Mersin Üniversitesi Sağlık Araştırma ve Uygulama Hastanesi Klinik Biyokimya Laboratuvarı'nda 01.01.2005 ile 31.12.2006 tarihleri arasında (2 yıl) çalışılan testlerden, onsekizinin hasta sonuçlarının verilerini kullanarak indirekt yöntemle referans aralıklarının hesaplanmasıdır. Anılan tarihler arasında polikliniklerimize başvuran bireylerden 1.190.765 veri elde edilmiştir. Ancak bu çalışma kapsamı içerisine polikliniklerimize yalnızca bir kez başvuran 5 yaş ve üzeri hastalardan 480.937 veri dahil edilmiştir. Alt gruplara ayırmada ise MARS (Multivariate Adaptive Regression Splines; Çok Değişkenli Uyumlu Regresyon Uzanımları Tekniği) analizinden faydalanılmıştır.Çalışmanın sonucunda elde edilen bilgilere göre belirlediğimiz referans değerler ile üretici firmanın önerdiği değerler arasında benzerlikler vardır. Ancak tam bir örtüşme gözlenmemiştir. Glukoz, üre, kreatinin, total bilirubin, indirekt bilirubin ve aspartat transaminazın referans değerleri üretici firmanın değerleri ile benzer; lipit paneli, ürik asit, total protein, albumin, alanin transaminaz, gamma-glutamil transferaz, laktat dehidrogenaz ve kreatin kinaz değerleri ise farklı çıkmıştır.Bu çalışma hastane verileri kullanılarak indirekt yöntemle referans aralığı hesaplanabileceğini göstermiştir. Ancak gerekli veri sayısı, dışlanacak veriler, alt gruplara ayırma ve aşırı uç değerlerin atılması kriterleri ile referans aralığı hesaplarken seçilecek metodun ne olacağı hakkında literatürde henüz yeterli çalışma mevcut değildir. Bu yöntemin, bilgisayar informasyon sistemlerinin gelişmesi, bu konu hakkındaki çalışmaların artması ve standardizasyonun sağlanması ile ileride direkt yöntemin yerini alacağını düşünmekteyiz. The reference intervals of the clinical laboratory tests have to be known for accurate medical decision. As reference interval values vary due to factors such as geographical district, ethnicity, age and feeding, all laboratories have to determine their own values. However, it is quite difficult to estimate the reference intervals. Recently, publications are increasing which shows that estimating the reference intervals by indirect method is easier by using hospital data.The aim of this study was to analyze the reference intervals of eigthteen tests which were performed at the Clinical Biochemistry Laboratory of Mersin University Medical Hospital between 01.01.2005 and 31.12.2006 (2 years) by using patients? data with indirect method. Between those days 1.190.765 data were obtained from the patients who applied to our outpatient clinics. However, 480.937 data were included in this study from the patients who applied only once and who were at the age of 5 and over. MARS (Multivariate Adaptive Regression Splines) analyze was used in order to seperate the subgroups.As a conclusion, some of the estimated reference intervals were similar with the values recommended by the producer firm while some others were not. The reference values of glucose, urea, creatinin, total bilirubine, indirect bilirubine and aspartat transaminase were found similar with the values of the producer firm; the reference values of lipids, uric acid, total protein, albumine, alanine transaminase, gamma-glutamil transferase, laktat dehidrogenase and creatine kinase were not found similar with the values of the producer firm.The results of this study suggest that by using hospital data the reference intervals could be calculated by the indirect method. However, in the literature there does not exist enough studies about the necessary data number, seperating into subgroups, criteria of omitting extreme values and selecting the methods to estimate the reference intervals. We suggest that the indirect method will be using in future as studies on this issue increase, as computer information systems are developed and as the standardization is ensured. 111
