Hypersomnia is a symptom which is pathognomonic of patients with Kleine-Levin syndrome (KLS), but the cause of this finding remains undefined. Given the pervasive association between obstructive sleep apnoea (OSA) and excessive daytime sleepiness, there exists the possibility that OSA might contribute to the sleepiness exhibited by these patients. Here we report a middle age man who had excessive daytime sleepiness and diagnosed as OSA after polysomnography. Prescribing of continuous positive airway pressure (CPAP) lead to unmasking of episodes of increased sleepiness and other behavioural features of KLS. [ABSTRACT FROM AUTHOR]

Abstract: Background: Kleine–Levin syndrome (KLS) is a rare disorder of unknown etiology. To date, only four familial cases have been described. The possible presence of genetic and autoimmune processes has been postulated recently. Our objective was to report for the first time a multiplex KLS Saudi family with 6 out of 12 family members affected. Methods: The demographic and clinical features of the six affected family members are described. KLS was diagnosed according to the International Classification of Sleep Disorders (ICSD). Human leukocyte antigen (HLA) typing was performed for both affected and unaffected family members and compared to previous studies. Results: The father and three male and two female children were affected. Age of onset ranged from 15 to 21years. Symptoms disappeared in four family members. HLA typing was identical in the father and two children (1F and 5M). All affected members shared one-half of HLA antigens. HLA typing revealed that four members out of the six affected members are homozygous at DQB1∗02 loci. Conclusions: This report provides a description of a multiplex KLS family with six members affected. HLA-DQB1∗02 homozygosity was present in 4/6 affected and 2/6 unaffected family members. The family studied presents an invaluable opportunity for further DNA and genetic studies, which may help in finding the mutation in the future. [Copyright &y& Elsevier]

Kleine-Levin syndrome (KLS) is a rare disorder of recurrent hypersomnolence. The pathophysiology continues to be poorly understood. Autoimmunity, genetic polymorphisms, dysfunction of the hypothalamic axis, and abnormalities in functional imaging have been proposed. Several triggers have been described, including infection, toxins, head trauma, sleep deprivation, lactation, and menses. We present the first case report in the medical literature of KLS triggered by pregnancy and the first case of KLS from Armenia. Our patient has a pattern of mostly pregnancy-related episodes of several day sleepiness occurring monthly. Background. Pediatric Kawasaki disease (KD) and human immunodeficiency virus (HIV)+ adult Kawasaki-like syndrome (KLS) are dramatic vasculitides with similar physical findings. Both syndromes include unusual arterial histopathology with immunoglobulin (Ig)A+ plasma cells, and both impressively respond to pooled Ig therapy. Their distinctive presentations, histopathology, and therapeutic response suggest a common etiology. Because blood is in immediate contact with inflamed arteries, we investigated whether KD and KLS share an inflammatory signature in serum. Methods. A custom multiplex enzyme-linked immunosorbent assay (ELISA) defined the serum cytokine milieu in 2 adults with KLS during acute and convalescent phases, with asymptomatic HIV+ subjects not taking antiretroviral therapy serving as controls. We then prospectively collected serum and plasma samples from children hospitalized with KD, unrelated febrile illnesses, and noninfectious conditions, analyzing them with a custom multiplex ELISA based on the KLS data. Results. Patients with KLS and KD subjects shared an inflammatory signature including acute-phase reactants reflecting tumor necrosis factor (TNF)-α biologic activity (soluble TNF receptor I/II) and endothelial/smooth muscle chemokines Ccl1 (Th2), Ccl2 (vascular inflammation), and Cxcl11 (plasma cell recruitment). Ccl1 was specifically elevated in KD versus febrile controls, suggesting a unique relationship between Ccl1 and KD/KLS pathogenesis. Conclusions. This study defines a KD/KLS inflammatory signature mirroring a dysfunctional response likely to a common etiologic agent. The KD/KLS inflammatory signature based on elevated acute-phase reactants and specific endothelial/smooth muscle chemokines was able to identify KD subjects versus febrile controls, and it may serve as a practicable diagnostic test for KD.

7. Förlagsinsatser : möjligheter för ett kooperativt finansieringsinstrument

Studiens syfte är att utreda möjligheterna och begränsningarna för svenska lantbrukskooperativa företag att använda sig av förlagsinsatserna. Fyra företag är i fokus, nämligen Södra och KLS, vilka har använt sig av instrumentet, samt Swedish Meats och Svenska Lantmännen, vilka har aktuella överväganden att göra detsamma. Utgångspunkten i teorikapitlet är de tre roller, som medlemmen har med sin förening, nämligen att äga, nyttja och styra föreningen. Transaktionskostnadsteorin samt äganderätts- och agentteorierna används för att analysera förlagsinsatsinstrumentet. Det empiriska materialet insamlas genom kvalitativa personliga intervjuer med förtroendevalda och högre tjänstemän i de fyra fallföretagen. Möjligheterna för förlagsinsatserna är: · En individualisering av kapitalet sker, varvid investeraren får ett incitament i att engagera sig i föreningens ekonomiska utveckling. Det tas ett steg mot en bättre balans mellan ägar- och nyttjarrollen. · Den kooperativa särprägeln behålls, eftersom rösträtt saknas, varvid externa investerare inte kan ta kontroll och skifta mot ett avkastningsmål. · De agent- och äganderättsproblem, som uppstår på grund av att den kooperativa äganderätten är vagt definierad, reduceras men försvinner inte helt. · Villkoren formuleras av föreningen, varvid instrumentet kan användas för olika syften – såsom en generellt riktad emission eller riktade emissioner till systerföreningar för att stärka soliditeten; innehållen efterlikvid/slutbetalning varvid kapitalet behålls i föreningen under minst fem år; riktade emissioner till specifika projekt där endast de berörda investerar och på så vis får man en klart avgränsad och engarerad grupp. · Medlemmarna kan bli mer öppna för nya finansieringsformer och organsiationsstrukturer i framtiden, om förlagsinsatser emitteras och en utbildning angående instrumentet sker. Begränsningarna för förlagsinsatser är: · Marknadsplats saknas och pappershanteringen är omständig. · Medlemmarna upplever sin nyttjarrol, The purpose of this study is to identify the potentials and constraints of a Swedish cooperative financial instrument translated as non-voting B-shares. Four cooperatives are studied; Södra (forestry) and KLS (meat) because they have issued non-voting B-shares, and Swedish Meats (meat) and Svenska Lantmännen (grain) because they consider issuing such. Non-voting B -shares are allocated member equity. Dividends are paid annually if funds are available. Dividends to non-voting B-shares have a higher priority than pa tronage refunds. Nonvoting B-shares can be issued to members and non-members, but capital from non-members must not exceed the common stock. The currency of the instrument must be at least five years. Redemption can be done after six months, if claimed by the issuer, and after two years, if claimed by shareholders. The dividend conditions are set by the cooperative and based on its performance, for example 5 %. There may also be a bonus system. Redemption must be at nominal value. Since no second-hand market exists, the instrument reminds of a bond that is redeemable after five years. The shares are paper-based but it may potentially become traded electronically. Today the volume of the instrument is low and there are high transaction costs when the shares are traded. The three roles that a member has with his cooperative (user, owner and controller) are used to structure the theoretical analysis. Transaction costs theory, property rights theory, and agency theory are used to analyze the instrument. The collection of empirical data was conducted through qualitative interviews with management and directors of the cooperatives. The potentials for the non-voting B-shares are: · The capital is individualized which gives the members an incentive to take an active interest in the cooperative’s economic performance. · The cooperative identity is preserved. Investors cannot take control. · B-shares can be used for different purposes. They can be issued to members

Kooperativa företag har ständiga svårigheter att generera egenkapital, eftersom medlemmarna sällan erhåller ränta på sitt insatskapital. Dock behövs kapital till investeringar såsom företagsförvärv, FoU samt marknadslanseringar. KLS Livsmedel ek. för. (KLS) står nu inför sådana problem, när företaget måste stärka sin position på marknaden för förädlade köttprodukter. Inom den kooperativa sektorn har skett många fusioner. Företagen vill på detta vis förbättra sin konkurrenskraft, vinna skalfördelar och få starkare kapitalbas. För de företag, som väljer att fortsätta på egen hand, krävs andra strategiska åtgärder, bl a på kapitalsidan, eftersom den traditionella finansieringen med kapital endast från medlemmarna ofta inte är tillräcklig. Ägarstrukturen, dvs vilka ägarna är, hur ägandet är fördelat och vilka motiv ägarna har med sin kapitalplacering, påverkar hur riskkapital kan genereras till föreningen. Om externa intressenter erbjuds att satsa kapital i föreningen, ändras ägarstrukturen, vilket får betydelse för medlemmen. Syftet med uppsatsen är att analysera vilka konsekvenser olika finansieringsalternativ får för medlemmen och för dennes ägande och nyttjande av föreningen. Syftet är relaterat till följande problemställningar: • Hur påverkas KLS och den enskilde medlemmen av de olika alternativen? • Vilken betydelse har ägarstrukturen för medlemmen och dennes intresse för att satsa kapital? De finansieringsalternativ, som är aktuella för KLS, är (1) överföring av delar av verksamheten i bolag vars aktier delvis ägs av externa intressenter, (2) utgivande av konvertibla skuldebrev (som efter några år blir omvandlade till aktier i dotterföretag) samt (3) utgivande av förlagsinsatser. Dessa alternativ analyseras med hjälp av äganderättsteorin. De teoretiska slutsatserna från denna analys prövas med hjälp av fallstudier, genomförda såsom kvalitativa intervjuer med personer i KLS samt i ett antal företag, som använder samma eller liknande finansieringsinstrument. Arbete, Co-operative firms have generally difficulties to generate equity capital as members rarely obtain interest on their deposit. Still, capital is needed for investments such as acquisition of other firms, R&D and launching products on the market. KLS Livsmedel ekonomisk förening (KLS) is now facing such problems. KLS is a minor meat cooperative in the southern part of Sweden. As it has recently rejected an offer to merge with a nation-wide meat cooperative, it has decided to integrate vertically in the value chain in order thereby to strengthen its competitiveness. Mergers are frequent in the co-operative sector. The aim is to increase the competitiveness, to gain economies of scale in the production and to the capital basis. For cooperatives that prefer to operate as independent, other strategic measures are necessary, not the least in terms of capital, as the members are often not able to supply the required amount of funds. The structure of ownership, i. e., who the owners are, how the ownership is divided between different owners, and which motives the owners have, influence the firm’s financial basis. This ownership structure is affected if external investors are offered to invest capital in the cooperative, and thus the membership is affected. This thesis aims at analysing the effects that different financial instruments have for the members, governing and patronising their cooperative. It is related to following problems: • How are KLS and the member affected by the different alternatives? • Which importance has the structure of the ownership for the member and its interest to invest capital? The financial options considered by KLS are: (1) Parts of the business activities are transferred into subsidiaries, owned together with external partners, (2) Bonds are emitted to members and others, and these bonds will after a few years be converted into stock in some subsidiaries. Abstract: Kleine–Levin syndrome (KLS) is commonly described as a self-limiting disorder exhibiting episodes of hypersomnia and psychiatric symptoms but without any enduring disabilities. Recently, some authors have reported persistent or even progressive memory deficits associated with the disorder. Nevertheless, literature about cognitive disturbances in KLS is rare. Our report describes a patient with deficits of visual and verbal recall after remission of an episode, as well as selective deficits of visual recall 6 months later. Neuropsychological testing is necessary in all patients with KLS to further characterize the profile and impact of associated cognitive deficits. [Copyright &y& Elsevier]

- Abstract
Kleine-Levin syndrome (KLS) is a rare and disabling disorder characterized by recurrent episodes of hypersomnia as well as cognitive and behavioral abnormalities. Sadly, the treatment options are very limited and often ineffective. To our knowledge, there have been no reports in the literature for the use of acetazolamide as a treatment for this disorder. We report the case of a 26-year-old female patient with KLS in whom acetazolamide was used to treat recurrent hypersomnia and cognitive abnormalities. The patient had a dramatic improvement on acetazolamide in both the frequency and severity of her symptoms. The symptoms recurred when acetazolamide was stopped on two occasions and improved with resumption of the drug, leading us to conclude that its therapeutic effect was significant.
