Objective: Healthcare professionals will be key to the success of COVID-19 vaccination programs because they are in the primary vaccination group. This study aims to measure and evaluate the acceptance, attitudes, and behaviors of healthcare professionals towards the COVID-19 vaccine. Methods: A questionnaire was applied to healthcare professionals via face-to-face interviews between 15-25 December 2020. Results: 507 participants were included in the study; the mean age was 31.6 ± 8.2 (19-71), and 52.3% were women. 52.3% of healthcare professionals said they wanted to get the COVID-19 vaccine. Men were more likely to have the COVID-19 vaccine than women (p <0.001). While the willingness for the COVID-19 vaccine was highest in doctors (83%), it was the least in nurses (37.7%) (p <0.001). The willingness to be vaccinated was higher in those with a higher education level (p <0.001). Conclusions: We observed that healthcare professionals' willingness to be vaccinated against COVID-19 differed according to their roles in the hospital, and doctors were the most widely group to accept vaccination. We believe our findings can guide public health campaigns, particularly for healthcare professionals, to increase acceptance of the COVID-19 vaccine. [ABSTRACT FROM AUTHOR]

12. 2024 ile Gündeme Gelen Yeni Aşılar: 9 Valanlı HPV.

İnsan papilloma virüsü (HPV), erkeklerde ve kadınlarda anogenital ve orofaringeal hastalığa neden olan cinsel yolla bulaşan bir patojendir. Yüksek riskli HPV genotipleriyle gelişen kalıcı viral enfeksiyon serviks kanserine neden olur. Yüksek riskli HPV genotipleri 16 ve 18, serviks ağzı kanserlerinin yaklaşık %70'ine neden olur ve 31, 33, 45, 52 ve 58 türleri de ilave olarak %20'sine neden olur. HPV tipleri 16 ve 18 ayrıca anal kanserlerin yaklaşık %90'ına ve orofaringeal kanser, vulvar, vajinal ve penis kanserlerinin önemli bir oranına neden olmaktadır. HPV tip 6 ve 11 anogenital siğillerin yaklaşık %90'ına neden olur. Aşılar, HPV enfeksiyonunun ve daha sonra HPV ile ilişkili hastalıkların gelişmesine karşı koruma sağlamak için geliştirilmiştir. HPV 9 değerlikli aşı (Gardasil 9), 6, 11, 16, 31, 33, 45, 52 ve 58 tiplerini hedefler. Amerika Birleşik Devletleri'nde (ABD) yalnızca 9 değerlikli aşı mevcuttur. ABD'de 9 değerlikli aşı, kadınlarda servikal, vulvar, vajinal, anal, orofaringeal ve diğer baş ve boyun kanserlerinin, anogenital prekanseröz ve displastik lezyonların ve genital siğillerin, erkeklerde ise anal, orofaringeal ve diğer baş ve boyun kanserlerinin, anal prekanseröz ve displastik lezyonların ve genital siğillerin önlenmesi için özel olarak onaylanmıştır. ABD'de Aşılama Uygulamaları Danışma Komitesi tarafından önerilen aşılama programı, Dünya Sağlık Örgütü'nün aşılama uzmanlarından oluşan Stratejik Danışma Grubu tarafından önerilenle aynıdır. Dokuz ila 14 yaş arasında; 0. ve 6. veya 12. ay arasında iki doz, 15 yaş üzerinde ise; 0., 1. veya 2. ve 6. aylarda üç doz HPV aşısı yapılmalıdır. Önerilen yaş aralığında, HPV aşısı için en uygun zaman, kişinin cinsel ilişkiye girmesinden önceki zamandır. Elde edilen veriler aşının güvenli olduğunu enjeksiyon bölgesinde gelişen hafif lokal reksiyon dışında iyi tolere edildiğini göstermektedir. HPV aşılamasının HPV enfeksiyonunu, genital siğilleri veya halihazırda mevcut olan anogenital intraepitelyal neoplaziyi tedavi etmemektedir. 13. Erişkin Yaş Grubunda Görülen Bir Kabakulak Olgusu.

- Abstract
Giriş: Kabakulak enfeksiyonu, paramiksovirus grubundan Mumps virüsünün yol açtığı, sıklıkla çocukluk çağında görülen ancak tüm yaş gruplarında izlenebilen viral bir enfeksiyondur. Kabakulak aşı ile önlenebilen hastalıkların başında yer alır ve erişkin yaş grubunda görülmesi halinde komplikasyon ile seyreder. Biz burada erişkin yaşta gelişen bir Kabakulak olgusunu sunacağız. Olgu: Otuz bir yaş erkek hasta çiftçilikle uğraşıyor bir hafta önce başlayan sağ boyun ve sağ testiste ağrı ve şişlikle başvurdu. Ateş; 36,5 °C SO2; 96 TA; 120/70 mm Hg nabız;75/dk. Fizik muayenesinde sağ lateral servikalde yaklaşık 4x5 cm boyutlu ele gelen ağrılı şişlik ve sağ testiste elevasyon azalan ağrı ve şişlik mevcut. Laboratuvar: Amilaz: 352 U/l, Kabakulak IGM (+). Skrotal USG: Sağ testiste epididimi orşit ile uyumlu gözlendi. Skrotal soğuk uygulama ve elevasyon yapıldı. Sonuç: Kabakulak aşısı T.C. Sağlık Bakanlığı tarafından 2006 yılında çocukluk çağı bağışıklama kapsamına alınmıştır. Hastamızın aşı sorgulaması yapıldığında çocukluk yaş grubunda aşılarının yapılmış olduğu ifade edilmiştir. Ancak hastamızın çocukluk yaş grubunda kabakulak aşısının bağışıklama programında olmaması dolayısıyla kızamık kızamıkçık kabakulak aşısını özel olarak yaptırmadığını öğrenilmiştir. Aşı ile önlenebilen enfeksiyon hastalıkları erişkin yaşta komplikasyonlar ile seyredebilmektedir. Erişkin yaşta bireylerin serolojik taramalarının yapılması, duyarlı olmaları halinde aşı ile önlenebilen hastalıklara karşı aşılanmaları önem arz etmektedir. [ABSTRACT FROM AUTHOR]

ÖZETAmaç: Bruselloz ülkemizde endemik olarak görülen zoonotik bir hastalık olup, özellikle Güneydoğu,Doğu ve İç Anadolu bölgesinde yaygındır. Bu çalışmanın amacı brusellozlu hastalarınepidemiyolojik, klinik ve laboratuvar bulgularını, komplikasyonlarını ve tedavilerini değerlendirmektir.Metod: Bu çalışmada 2011 Ağustos ile 2016 Aralık tarihleri arasında poliklinikten veya yatırılarakizlenen 60 bruselloz olgusu geriye dönük olarak değerlendirildi. Bruselloz tanısı, klinikbulgularla beraber standard tüp aglütinasyon testi (titre >=1/160) ile konuldu.Bulgular: Olguların 42 (%70)’si erkek, 18 (%30)’i kadın olup, yaş ortalamaları 50.4±14.8 (yaşaralığı 20-80) idi. Meslek dağılımı açısından değerlendirildiğinde olguların %60’ının çiftçilik vehayvancılıkla uğraştığı ve 46 (%76.8)’sının kırsal kesimde yaşadığı saptandı. Klinik form açısındandeğerlendirildiğinde olguların %80’i akut, %16.7’si subakut ve %3.3’ü kronik idi. En sıkbulaşma yolları pastörize edilmemiş süt ürünlerinin tüketimi (%40) ve hayvancılık ile birliktepastörize edilmemiş süt ürünlerinin tüketimi (%56.7) idi. Olguların %50’si Mayıs ile Ağustosayları arasında başvurmuştu. Başlıca klinik semptomlar ateş (%73.3), halsizlik (%65), artralji(%61.7) ve terleme (%46.7) idi. Olguların %21.7’sinde tek eklem tutulumu, %10’unda poliartritbulguları ve %14.3 olguda ise orşit saptandı. Laboratuvar incelemede olguların %25’indeserum transaminaz yüksekliği, %25’inde anemi ve %8.3’ünde trombositopeni saptandı. İki(%3.3) olguda relaps görüldü.Sonuç: Endemik bölgelerde ateş, eklem ağrısı, halsizlik gibi özgün olmayan yakınmalarla gelenhastalarda bruselloz ayırıcı tanıda düşünülmelidir.Anahtar Sözcükler: Bruselloz; Epidemiyoloji; Tanı; TedaviABSTRACTAim: Brucellosis is a zoonotic disease that is endemic in our country, especially in Southeast,Eastern and Central Anatolia regions. The aim of this study is to evaluate the epidemiological,clinical and laboratory findings, complications and treatment of the patients with brucellosis.Method: In this study, 60 patients with brucellosis, followed between August 2011 andDecember 2016 were evaluated retrospectively. The diagnosis of brucellosis was made bystandard tube agglutination test (a titer of ≥1/160) with clinical features compatible withbrucellosis.Results: Forty two (70 %) of the patients were male and 18 (30 %) were female. The meanage of patients was 50.4+/-14.8 years (age range 20-80) . When evaluated in terms ofoccupational distribution, 60 % of cases were dealing with farming and livestock, and 46(76.8%) were living in rural areas. Rates of acute, subacute and chronic infection were 80,16.7, and 3,3 % respectively. The most common routes of transmission were consumption ofunpasteurized dairy products (40%) and consumption of unpasteurized dairy products withanimal husbandry (56.7%). Fifty percent of cases were admitted between May and AugustThe main symptoms were fever (73.3 %), malaise (65 %), arthralgia (61.7 %) and sweating(46.7 %). Single joint involvement was present in 21.7 % , polyarthritis in 10 % and orchitis in14.3 % of the cases. In laboratory studies 25 % of the cases had elevated serum transaminase,25 % had anemia and 8,3% had thrombocytopenia. AMAÇ:Hamilelik esnasında geçirilen aşı ile önlenebilen hastalıklar sadece anne için değil fetus için de risklidir.Bu çalışmanın amacı; Türkiye’nin Yozgat ilindeki hamile kadınlarda seçilmiş aşıile önlenebilen hastalık etkenleri olan kızamık, kızamıkçık, kabakulak,suçiçeği (KKKS) ve tetanoz hastalığı antikorlarının seroprevalansınıaraştırmaktır.METHOD:Kan örnekleri, hastanemiz kadın hastalıkları ve doğum polikliniğinebaşvuran 176 hamile kadından alındı.KKKS virüsleri ve tetanoza karşı spesifik antikor titreleri ELISA ilekantitatif olarak saptandı. Hastalık, aşılama geçmişi ve demografikbilgilerinin dahil olduğu anket formu dolduruldu. SONUÇLAR: Katılımcıların yaşortalaması 27.2±4.9 (18-45) idi. Tetanoz ,KKKS IgG seropozitifliği sırasıyla %90.34, % 97.16, % 98.30, % 99.4, % 100 idi. Son 5 yıl içerisinde tetanoz aşısıolanlarda bağışıklık oranı olmayanlara göre daha yüksek bulundu (93.7% ye 84.6% , p= 0.049). Tetanoza karşı bağışık olanların yaş ortalaması 26.9±5, bağışıkolmayanların yaş ortalaması 29.8±3.7 bulundu (p=0.0195). Yaş ile tetanozbağışıklık oranları arasında ters korelasyon tespit edildi. Yaş artıkça tetanozbağışıklık oranının azaldığı tespit edildi (p=0.002). TARTIŞMA:Çalışmamızdagebelerde yüksek oranda tetanoz ve KKKS bağışıklığı bulunmuştur. Ancak,hamilelik esnasında enfeksiyonun neden olduğu riskler göz önündebulundurulduğunda, aşı ile önlenebilen hastalıklara karşı antikorlar; daha önceaşılanmamış ya da hastalık geçirmemiş doğurganlık çağında olan kadınlardataranmalıdır ve seronegatif olanlar aşılanmalıdır., Background:Vaccine preventable diseases (VPD) duringpregnancy are risk not only for the mother, but also for the fetus. The aim ofthis study was to determine the seroprevalence of antibodies to tetanus andmeasles, mumps, rubella, varicella (MMRV) in pregnant women in Yozgat Turkey .Methods:Blood samples were taken from 176 pregnant women admitted to our hospital obstetricsand gynecology outpatient department . The specific IgG antibodies againsttetanus and MMRV viruses were determined quantitatively by ELISA and aself-recorded questionnaire was filled including their demographics, diseaseand vaccination history. Results:The mean age of the pregnant women participating in the study was 27.2±4.9 (18-45). Tetanus and MMRV IgG seropositivity were respectively 90.34%, 97.16%, 98.30%, 99.4%, 100%. Immunity to tetanus was higher in pregnants who were vaccinated withinlast 5 years compared to non-vaccinated ones (93.7% vs 84.6 % respectively).Mean age of individuals having immunity for tetanus was 26.9±5 years while that of individuals without immunity 29.8±3.7 years. There was a significant negativecorrelation between age and tetanusimmunity rate. It was found that immunity to tetanus decreased as ageincreased. Conclusion: In this study, high rate of tetanusand MMRV immunity wasfound among pregnant woman. However, considering of the risks due to theinfection during pregnancy, antibodies to VPD should be scanned in childbearingwomen who have not vaccinated previously or have not the disease, and seronegativeones should be vaccinated before pregnancy.

18. Evaluation of mean platelet volume (MPV) levels in brucellosis patients

Aim. Brucellosis is a multisystemic infectious disease. An inflammatory process occurs in brucellosis which causes increase in acute phase reactants. Mean platelet volume (MPV) has been shown as an inflammatory marker in some diseases. In this study we searched the MPV levels of brucellosis patients. Method. MPV levels of forty-seven patients with brucellosis were evaluated. Results. We included a total of 47 brucellosis patients having an average age of 47.4±14.6 and sex distribution of 21 (44,7%) male and 26 (55,3%) female and also 47 control healthy subjects having an average age of 45,2±12,3 and sex distribution of 25 (53,2%) male and 22 (46,8%) female). Erythrocte sedimentation rate (ESR), MPV, red cell distribution width (RDW), Hemoglobin (Hgb) and white blood cell(WBC) values in pre-treatment stage were statistically different from after treatment period. MPV levels were significantly lower in the pretreatment group. Similarly RDW values were lower in pretreatment group. MPV levels of patients were not diffrent in the acute and subacute stage. There was not any significant difference in the MPV levels of patients with hepatomegaly and without hepatomegaly (MPV levels of patients with splenomegaly were lower than patients without splenomegaly Spearman’s correlation analysis showed a significant negative correlation between STAT (serum tube agglutination test) and MPV values. Conclusion. MPV levels may be useful in the follow-up of brucellosis patients. Keywords: Brucellosis, acute phase reactant, mean platelet volume Özet Amaç. Bruselloz multisistemik bir enfeksiyon hastalığıdır. Brusellozda akut faz reaktanlarının artmasına neden olan inflamatuar bir süreç izlenmektedir. Mean platelet volume (MPV)’ nin bazı hastalıklarda inflamasyonu gösteren bir değer olduğu gösterilmiştir. Bu çalışmada bruselloz hastalarının MPV düzeylerini araştırmayı amaçladık. Yöntem. Kırkyedi brusella hastasının MPV düzeyleri incelendi. Bulgular. Çalışmamıza yaş ortalaması 47,4±14,6 olan, 21 (%44,7) erkek, 26 (%55,3) kadın,toplam 47 bruselloz hastası ve yaş ortalaması 45,2±12,3 olan 25 (%53,2) erkek, 22 (%46,8) kadından oluşan 47 kişilik kontrol grubu dahil edildi. Tedavi öncesi ve tedavi sonrası eritrosit sedimentasyon hızı (ESR), MPV, red cell distribution width (RDW), hemoglobin (Hgb), beyaz küre (WBC) değerleri arasında istatistiksel olarak anlamlı farklılık mevcuttu. Tedavi öncesi MPV düzeyleri anlamlı olarak düşüktü. Benzer şekilde tedavi öncesi RDW değerlerinde de anlamlı düşüklük saptandı. Akut ve subakut faz arasında MPV düzeyleri açısından farklılık saptanmadı. Hepatomegali olan ve olmayan grupta da MPV düzeyleri açısından belirgin bir fark yoktu. Splenomegalisi olan hastaların MPV düzeyleri splenomegalisi olmayan hastalara göre daha düşük idi. Spearman korelasyonu, STAT (serum tube agglutination test) ve MPV arasında anlamlı negatif bir korelasyon gösterdi. Sonuç. Bruselloz multisistemik bir enfeksiyon hastalığıdır. Brusellozda akut faz reaktanlarının artmasına neden olan inflamatuar bir süreç izlenmektedir. Mean platelet volume (MPV)’ nin bazı hastalıklarda inflamasyonu gösteren bir değer olduğu gösterilmiştir. Bu çalışmada bruselloz hastalarının MPV düzeylerini araştırmayı amaçladık. Yöntem. Kırkyedi brusella hastasının MPV düzeyleri incelendi. Bulgular. Çalışmamıza yaş ortalaması 47,4±14,6 olan, 21 (%44,7) erkek, 26 (%55,3) kadın,toplam 47 bruselloz hastası ve yaş ortalaması 45,2±12,3 olan 25 (%53,2) erkek, 22 (%46,8) kadından oluşan 47 kişilik kontrol grubu dahil edildi. Tedavi öncesi ve tedavi sonrası eritrosit sedimentasyon hızı (ESR), MPV, red cell distribution width (RDW), hemoglobin (Hgb), beyaz küre (WBC) değerleri arasında istatistiksel olarak anlamlı farklılık mevcuttu. Tedavi öncesi MPV düzeyleri anlamlı olarak düşüktü. Benzer şekilde tedavi öncesi RDW değerlerinde de anlamlı düşüklük saptandı. Objective: The aim of our study was to evaluate pneumococcal and influenza vaccination rates in adults at risk who applied to our hospital in Yozgat province based on the patient's declaration. Methods: This report uses data from a single-center, non-interventional cross-sectional study that included patients who applied to Outpatient Clinics of Infectious Diseases and Clinical Microbiology, Internal Medicine, Cardiology, and Chest Diseases from May 2013 to May 2014. Information about pneumococcal and influenza vaccine coverage was collected during face-to-face interviews using a structured questionnaire with patients with diabetes mellitus, congestive heart failure, chronic obstructive pulmonary disease, chronic renal failure or patients who were ≥65 years of age. Results: A total of 2918 patients completed the interview. The mean age was 62.1±13 years (range 19-93) and 1255 (61.9%) were female. None of the patients had a vaccination card. Influenza vaccine coverage was 12.3%, and pneumococcal vaccine coverage was 3%. Only 2.8% had received both vaccinations. 92.9% and 98.9% of the patients who had not received influenza and pneumococcal vaccines respectively declared that they were not informed about these vaccines by their doctors. Conclusions: The rate of influenza and pneumococcal vaccinations in adults in Yozgat, Turkey fell far behind the recommendations. A strong recommendation from health-care providers may be helpful for increased uptake of vaccines. [ABSTRACT FROM AUTHOR]

Objective: In this study our aims was to detect the HBSAg, Anti HBs, Anti HCV, Anti HIV seropositivity and vaccination frequency among obstetrics and gynecology patients.Materyal And Method: 147 women who applied to our obstetrics and gynecology polyclinic were investigated for age, gravida, parity, pregnancy, abortus parameters. According these parameters patients were grouped at these groups. HBSAg, Anti HBS, Anti HCV, and Anti HIV seropositivity detected propectively Meantime questionarry about Hepatit B vaccination given to the patients.Results: Among 147 women, 78 women were pregnant (%53.1) and rest 69 women (%46.9) applied because of gynecologic reasons. Mean age was 32.88± 10.2, mean gravida was 2 ( 0-10) and mean parity was 1 (0-6).%17.9 women claimed that they had immunization, %80.8 women claimed that they hadn’t. %1,3 patient couldn’t remember whether she had or hadn’t vaccinated. 10.4% of our patients vaccinated Gynecology stated that the number of cases, 89.6% of patients who were referred to have not been immunized. Therefore, 1.4% of gynecological patients who were positive for HBsAg, HBsAg positive pregnant women were no.Conclusion: No HBsAg positivity detected among pregnant women. Low HbsAg positivity detected among gynecologic patients. These findings seem to be related with the low number of patients. Large prospective randomize trials are necessary for more precise evaluation., Amaç: Çalışmada amacımız polikliniğimize başvuran jinekolojik ve obstetrik hastalarda HBsAg, Anti-Hbs, Anti-HCV, ve Anti-HIV seropozitifliği ve aşılanma sıklığının belirlenme- sidir.Gereç ve Yöntemler: Polikliniğimize başvuran 147 kadın hastada, yaş, gebelik durumu, gravida, parite ve düşük/ küretaj öyküsü sorgulanarak, bu değişkenlere göre HBsAg, Anti-Hbs, Anti-HCV ve Anti-HIV seropozitifliği araştırıldı. Hastalara Hepatit-B aşılaması yaptırıp yaptırmadıklarına yönelik soruları içeren anket hastalara yüzyüze uygulandı.Bulgular: Olgularımızdan 69 (%46.9) hasta jinekolojik nedenlerle, 78 (%53.1) hasta ge- belik takibi için başvurmuştu. Yaş ortalaması 32,88 ±10.2 gravida ortalaması 2 ( 0-10) ve Parite ortalaması 1 ( 0-6) idi. Gebelerde aşılanma oranımız %17.9, aşılı olmayan gebe sa- yısı %80.8, hatırlamayan olgu sayısı %1.3 idi. Jinekolojik hastalarımızdan aşılı olduğunu ifade eden olgu sayısı %10.4, aşısız olduğunu ifade eden olgu sayısı %89.6 idi. Jinekolojik nedenle başvuran hastada %1.4 HBsAg pozitifliği saptandı, hiçbir gebede HBsAg pozitif- liği saptanmadı.Sonuç: Gebe hastalarda HbsAg pozitifliği hiç saptanmadı. Jinekolojik hastalarda HbsAng pozitifliği düşük olarak saptandı.Bu durum hasta sayısının az olması ile ilişkili olup bu konuda bölgemizde yapılacak daha fazla prospektif çalışmaya ihtiyaç bulunmaktadır.

AMAÇ: Bu çalıșmada RBPT ve/veya STAT testleri negatif olan klinik ve epidemiyolojik olarak bruselloz ile uyumlu hastalarda RBPT, STAT ve ELISA kombinasyonunun tanıdaki etkinliğinin değerlendirilmesi amaçlandı. YÖNTEM: Klinik ve epidemiyolojik olarak brusellozla uyumlu 66 olgunun serum örneği RBPT, STAT ve ELISA ile test edildi. RBT pozitif ve negatif kișilerde STAT ve/veya ELISA test sonuçları seropozitifl ik yö- nünden karșılaștırıldı. Veriler SPSS 16.0 bilgisayar programına girildi. Sonuçların istatistiksel analizi için Fischer Exact testi yapıldı. p, AIM: The aim of the study is to analyze the effi ciency of combination of RBPT, STAT and ELISA tests on patients with brucellosis where the diagnosis is dependent on the compatible clinical and epidemiological fi ndings with negative RBPT and/or STAT test results. METHODS: The serum samples of 66 patients having clinical and epidemiological fi ndings compatible with brucellosis were analyzed using the combination of RBPT, STAT and ELISA tests. Both positive and negative samples were re-analyzed for seropositivity by using the STAT or ELISA tests together. The results were statistically analyzed with the SPSS 16.0 computer program using the Fisher Exact test. The signifi cance level was defi ned as p< 0.05. RESULTS: Serum samples tested for brucellosis using the RBPT were found to be seropositive in 45.5% and seronegative in 54.5%. The seropositive group had a 73% seropositivity with the combination of STAT and ELISA tests. The seronegative group also had a 25% seropositivity with the combination of STAT and ELISA tests. Both groups’ seropositivity rate signifi cantly increased after the combination of the tests (p< 0.05). CONCLUSION: It seems that the combination of RBPT, STAT and ELISA tests increases the rate of seropositivity in patients with negative RBPT and /or STAT tests, particularly with a pre–diagnosis of brucellosis.

Nötropenik hastalarda, ishal ve/veya karın ağrısı sık görülen semptomlar olup,infeksiyöz ve non-infeksiyöz nedenlerin birbirinden ayrılması tedavi planlamasındaönem tasımaktadır. Nötropenik hastalarda abdominal enfeksiyonlar sadece mukozayıtutan basit mukozitten %20-80 ölümle sonlanan nötropenik enterokolite veya diğeradı ile tiflite kadar değisen bir spektrumda karsımıza çıkabilmektedir.Bizim çalısmamızda Ocak 2007- Haziran 2007 tarihleri arasında 6 aylıkdönemde Ankara universitesi Tıp Fakültesi, Hematoloji bilim Dalı'nda AkutMiyelositer Lösemi, Akut Lenfoblastik Lösemi, Kronik Myelositer Lösemi,Miyelodisplastik Sendrom, Non-Hodgkin Lenfoma, Hodgkin Lenfoma, AplastikAnemi, Multipl Myeloma, Blastik Kriz nedeniyle kemoterapi uygulanan ve beklenennötropeni süresi 5 günden uzun olan 56 eriskin hastanın nötroepenik olduğudönemde gelisen ishal vakalarından alınan dıskı örnekleri incelendi. 2 örnekte (%3.6)Cryptosporidium ookistlerinin pozitifliği tespit edildi. 7 (%12.5) örnekte Entamoebahistolytica trofozoiti ile birlikte bol eritrosit görüldü. Enzim ?mmun Assayyöntemiyle dıskıda antijen aranması sonucunda bu 7 örnekte Entamoeba histolyticaantijeni pozitif saptandı. Salmonella, Shigella, Yersinia ve Campylobacter (SSYC),Vibrio ve E.coli O157:H7 için standart kültür teknikleri uygulanarak yapılan dıskıkültürü sonuçlarında herhangi bir üreme tespit edilmedi.10 hastada tiflitle uyumlu barsak duvar kalınlığı (>4mm) tespit edildi. - Abstract
Objective: Tularemia is a zoonosis caused by Francisella tularensis specific to the Northern hemisphere. In this study, we aimed to detect the general knowledge of familyphysicians in Yozgat about tularemia. Material and Method: Family physicians working in the province of Yozgat were contacted by e-mail regarding completing a questionnaire about tularemia. Results: All family physicians in Yozgat were attended to this research(n=129). Their mean age was 33.6 ± 5.4 years. One hundred and one (78.3%) of them were male and 28 (21.7%) were female. The duration of working years of the physicians was 6.4 ± 5. Forty (31%) of family physicians reported that they had at least a patient diagnosed as tularemia. Ninety six of the family physicians were confident in their knowledge about tularemia. However, 15 (15.6%) of them were stated that tularemia was notifiable diseases due to its transmission from human to human. Besides, 35 (36.5%) of them were stated that bleeding occurs in the clinical presentation of tularemia. In family physicians who think that tularemia has bleeding, there was no difference about their working years or duration who spent in the region from the others. While 26 (26.1%) of the family physicians who think they are knowledgeable about tularemia were selecting beta-lactam antibiotics for the treatment as a first choice, 11 (33%) of the other family physicians were using beta-lactam antibiotics (p=0.493). Conclusion: In endemic regions for tularemia, family physicians are mostly the first physicians form any patients. Thus, to have an adequate knowledge about the diagnosis and follow-up of tularemia is important. It is concluded that the knowledge of the family physicians in Yozgat about the clinical findings and treatments of tularemia should be improved. [ABSTRACT FROM AUTHOR]

- Abstract
A 16-year-old shepherd applied to our clinic with an ulcer on his right hand. On questioning him for possibility of zoonotic disease, the patient mentioned sudden consecutive deaths of his sheep and direct contact with these animals. In physical examination, he had a painless ulcer with vesicle and systemic examination was normal. The medical history and the clinical findings were suggesting cutaneous anthrax. The agent was not shown in the sample from the lesion. Seven-day oral penicillin V treatment was given to the patient. One week after the treatment, the typical eschar of anthrax appeared. Anthrax is still an endemic disease in some regions of our country where livestock is widespread, so considering anthrax in differential diagnosis is crucial for prompt treatment in endemic regions. 42. Pityriasis rosea: a natural history of pediatric cases in the Central Anatolia Region of Turkey.

Background/aim: This study aimed to evaluate the demographic, clinical, and epidemiological features of pityriasis rosea (PR) in a cohort of 46 children in Yozgat, a city in the Central Anatolia Region of Turkey. Materials and methods: Forty-six children with PR were monitored at regular intervals (1, 2, 4, and 12 weeks) for 3 months from the time of diagnosis. A complete evaluation of the patient was performed at each visit. Results: The average age of patients at time of diagnosis was 12 ± 3.9 years. Cases were most common in the winter (rainy, snowy months; n = 14, 31%). Fifteen patients had a medical history significant for the presence of upper respiratory tract infection, while skin PR manifestations were preceded by drug intake in a second group of 15 patients. The presence of a herald patch was observed in 78.3% of patients, most frequently on the trunk (n = 23). Pruritus occurred in 75% of patients. Median PR duration was 3 weeks (range: 1-20 weeks). Conclusion: The course of PR is similar in Turkish children and adults. The high prevalence of pruritus in children with PR in Turkey was also significant. Further evaluation of this finding comparing adults and children is now required. [ABSTRACT FROM AUTHOR]

Aim: This study was conducted to determine the knowledge levels of the nursing students working in an endemic region about Crimean-Congo hemorrhagic fever (CCHF). Methods: This descriptive study involved 290 baccalaureate nursing students. Data were collected by means of a questionnaire. Results: Half of the students indicated that following up a patient with CCHF makes nursing a highly risky profession. The participating students were found to be unwilling to deal with patients with CCHF because of the obscurities about the ways people contract the disease. Conclusions:To improve nursing students unwilling to care for CCHF, particular emphasis should be placed on the training of nursing students as skilled nursing staff with humane attitudes towards CCHF. 46. Ulceroglandular Tularemia: A Case Report.

- Abstract
A 58-year-old woman was admitted to our hospital with a two week history of fever, and left axillary lymphadenopathy. She was living in a village endemic for tularemia, but she had no history of any animal contact or a tick bite. She had a wound on the second finger of her left hand which was the probable port of entry for Francisella tularensis. The clinical diagnosis of tularemia was confirmed by a microagglutination test. As this case demonstrated, other forms of tularemia other than oropharyngeal form prevalent in Turkey can be encountered among patients with fever and lymphadenopathy in endemic regions. 48. Infectious Etiology of Diarrhea Occurring in Neutropenic Patients.

- Abstract
Objective: Tha aim of this study is to investigate the incidence, clinical importance and etiologic agents of infectious diarrhea in patients with hematological malignancies undergoing neutropenia after chemotherapy. Methods: One hundred patients who developed neutropenia after chemotherapy were followed and monitored for abdominal symptoms (abdominal pain and diarrhea) and fever, prospectively. Stool samples were taken from 56 patients who developed diarrhea, and clinical data were collected by a structured survey. Statistical analysis was performed by SPSS 11.5 software. Results: Two (3.6%) samples were positive for Cryptosporidium oocysts, seven (12.5%) samples for Entamoeba histolytica trophozoites and antigen. Stool cultures were negative for enteric pathogens. Abdominal ultrasound was performed in 16 patients and thickening (>4 mm) of the bowel wall was detected in 10 patients. However, no classic pathogen of neutropenic enterocolitis was detected in stool samples of these patients. Conclusions: Infectious agents are rarely reported as the cause of diarrhea in neutropenic patients. Thus, it is not necessary to perform complicated and expensive diagnostic procedures in order to identify the etiology of diarrhea in patients with neutropenia. [ABSTRACT FROM AUTHOR]

Objective: The objective is to obtain the seroprevalence of HBsAg, anti-HCV, anti-HIV, RPR and to assess their rates according to time, gender, age and other specifications in blood donors. Methods: HBsAg, anti-HCV, anti-HIV and RPR results of 16 362 blood donors from January 1, 2005 to December 31, 2009 have been evaluated retrospectively. Prevalence of HBsAg, anti-HCV, anti-HIV, and RPR were investigated according to gender, age and other specifications of the donor. Results: 15,479 (94.6%) of the 16,362 donors were male and 883 (5.4%) were female. The age range of the donors was between 18 and 64 years, and the mean age was 31.55±8.94. 611 (3.73%) of the donors were repeat donors and 15,751 (96.27%) were the relatives of patients. HBsAg was positive in 85 (0.52%), anti-HCV in 59 (0.36%), anti-HIV in 16 (0.1%) and RPR in 13 (0.08%) of the donors. Western Blot confirmation test was reported positive in one of the 16 anti-HIV-positive donors. HBsAg positivity was 0.79% in 2005 and it regressed to 0.37% in 2009 (p=0.024). Conclusions: Seropositivity rate of HBsAg was lower than that of Turkey in general and seroprevalence of HBsAg decreased over years. [ABSTRACT FROM AUTHOR]
