2. Management of Foreign Bodies in the Aerodigestive Tract

Abstract

Foreign body (FB) aspiration and ingestion are frequently encountered by emergent otolaryngology services. The authors describe their experience in the management of FB cases in the aerodigestive tract. We carry a retrospective study about 626 patients who came or were referred to our department between 1996 and 2007 with a history or suspicion of a FB in the aerodigestive tract (except nasal and oropharyngeal FB). All of them have undergone rigid endoscopy under general anaesthesia. Children younger than 10 years were the most involved (36.9%) followed by patients between 71 and 80 years old (11.3%). The FB were visible on clinical examination in 39 cases. Chest and neck X-ray, showed radio-opaque FB in 302 cases (48.7%). A total of 626 rigid endoscopies were performed. FB were encountered in 549 patients (87.7%). The most involved sites were the oesophagus (51.9%) followed by the tracheobronchial tree (33.9%) and the hypopharynx (13.5%). Bones (22%) and coins (20.1%) were the most frequently encountered FB. Successful removal was achieved in 521 cases (94.9% of the FB found). The complication rate after rigid endoscopy was 1.3%. FB in the aerodigestive tract are frequent and may lead to severe complications. Removal through the rigid endoscope still has its place as the most reliable method. Prevention and public education for this serious problem should be considered.

3. Rhinocerebral mucormycosis: five cases and a literature review

Abstract

In this retrospective study, we describe our experience in the diagnosis and management of rhinocerebral mucormycosis (RCM), a rapidly lethal fungal infection.Between 1997 and 2007, five patients hospitalized for suspicion of RCM. Computed tomography was performed in all cases, and diagnosis was confirmed after anatomopathological or mycological examination. All patients underwent medical and surgical treatment. Follow-up was clinical and radiological with a mean period of 17 months.All patients were diabetic. Exophthalmia, rhinorrhea, and ophthalmoplegia were the most frequent symptoms observed. One patient had loss of visual acuity and another exhibited peripheral facial palsy. One patient had extensive hemifacial cutaneous necrosis. Nasal endoscopy revealed black necrotic lesions in one case, and another patient had a tumefaction localised in the left middle meatus. Necrotic lesions were most often found in the orbit, the maxillary and the ethmoidal sinuses on computed tomography (four cases for each site). One patient had thrombophlebitis of the cavernous sinus, and another had an intracranial extension. All patients were administered ordinary insulin and intravenous amphotericin B. Surgical debridement of the nasal cavity and the involved sinuses was performed through lateral rhinotomy (four cases) or endoscopy (one case). Unilateral orbital exenteration was associated in two cases. Progression was favourable in four cases; one patient died from sepsis despite aggressive treatment.Early diagnosis is crucial for the management of RCM. Treatment of underlying disorders, use of intravenous amphotericin B, and aggressive surgical intervention are key in reducing morbidity and mortality rates.

Abstract

Introduction : Acquired tracheal stenoses represent rare but serious disease. They are often secondary to inappropriate management of patients under artificial ventilation. The goal of this study is to evaluate our results in the management of these stenoses and to assess the benefits and the limits different therapeutic means. Materials and methods : We carry a retrospective study about 18 cases of acquired tracheal stenoses treated and followed in our department between 1999 and 2006. Initial endoscopic and radiological explorations have been performed in all cases. Treatment of the stenoses was medical, endoscopic and/or surgical. Follow-up was clinical and endoscopic with a mean period of 22 months. Results : All patients were victims of pathology needing intubation. Tracheotomy was performed after intubation in 50% of cases after a mean period of 12 days (5-20 days). Dyspnea and dysphonia were the major functional symptoms. Initial endoscopy showed a double tracheal stenosis in one case. Stenoses were initialy fibrous in 72.2% of cases and evolutive in 27.8% of cases. CT scan performed in 12 cases and MRI in 2 others allowed to better study stenosis characteristics. RFE was performed in 6 cases and showed an obstructive syndrome in all of them. All patients received medical treatment. Before a definitive treatment, dilatation was performed in 11 cases (61%) and stenting in one other (5.6%). Laser diode therapy was used in 11 patients including 2 cases having postoperative recurrence. Tracheal resection and reconstruction was performed in 11 cases (61%) having extensive and severe stenoses with involvement of tracheal cartilage. Conclusion : Acquired tracheal stenoses represent a serious complication with high morbidity. If tracheal resection and reconstruction remains the gold standard treatment, endoscopy represents now a major alternative in their management. Nevertheless, prevention should be considered, given that most stenoses are iatrogenic due to traumatic or prolonged intubations.

5. Place et limites de la chirurgie endonasale dans le traitement des mucoceles sinusiennes

Abstract

Objectif : Preciser les indications et les limites de la chirurgie endonasale dans le traitement des mucoceles sinusiennes. Materiel et methodes : Il s’agit d’une etude retrospective a propos de 15 patients colliges sur 13 ans (1994-2006). Tous les patients ont beneficie d’une endoscopie nasale et d’une TDM du massif facial. Une IRM a ete pratiquee dans 10 cas. Le traitement chirurgical comportait une marsupialisation ou une exerese de la mucocele. Les voies d’abord etaient endonasales, externes ou combinees, en fonction de l’extension ou de la topographie des lesions. La surveillance en postoperatoire a ete clinique et radiologique. Resultats : La TDM a montre une atteinte ethmoido-frontale (6 cas), maxillaire (5 cas), ethmoidale isolee (2 cas), frontale isolee (1 cas) et sphenoidale (1 cas). L’IRM a montre une extension orbitaire dans 7 cas et endocrânienne dans 3 cas. La voie d’abord chirurgicale etait endonasale exclusive dans 12 cas. Une voie combinee (endonasale et externe) a ete necessaire chez un patient ayant une atteinte frontale isolee et un autre ayant une atteinte ethmoido-frontale. Un patient ayant une mucocele maxillaire laterale a ete opere par voie de Caldwell-Luc. Aucune recidive n’a ete notee avec un recul moyen de 14 mois. Conclusion : La chirurgie endonasale represente la technique de choix dans le traitement des mucoceles sinusiennes et donne de bons resultats a long terme. Le siege et l’extension des mucoceles representent les principaux facteurs determinants dans le choix de la voie d’abord.

6. Place du prelevement ganglionnaire dans le traitement des cancers differencies de la thyroide n0 clinique

Abstract

Objectif : Mots-cles : Cancer differencie, thyroide, prelevement ganglionnaire Objectif : Demontrer l’interet du prelevement ganglionnaire sus-claviculaire et jugulaire inferieur dans la prise en charge des cancers differenties de la thyroide (CDT) N0 clinique. Materiel et methodes : Nous rapportons une etude retrospective a propos de 170 patients colliges sur 12 ans (1994- 2005). Tous les patients ont eu une thyroidectomie totale en 1 ou 2 temps. Le curage mediastino-recurrentiel a ete pratique soit systematiquement soit en cas de decouverte d’adenopathies en peroperatoire. Le curage lateral a soit de principe soit apres un prelevement ganglionnaire sus-claviculaire et jugulaire inferieur positif a l’examen extemporane. Des doses ablatives d’iode radioactif ont ete administrees en cas d’elevation du taux de la thyroglobuline ou d’une fixation d’iode lors du balayage cervico-thoracique. Resultats : L’examen anatomopathologique trouvait un carcinome papillaire (74,1%), vesiculaire (16,5%) ou papillaire a composante vesiculaire (9,4%). Trente patients ont eu un curage fonctionnel systematique uni ou bilateral, une metastase ganglionnaire a ete notee chez 9 d’entre eux. Cent quarante patients ont eu un prelevement ganglionnaire uni ou bilateral, un curage fonctionnel a ete pratique chez 6 d’entre eux devant un examen extemporane positif. Les taux de remission, de recidive et de metastases etaient respectivement de 83,3%, 6,7% et 10% dans le premier groupe, et de 89,3%, 4,3% et 6,4% dans le deuxieme groupe. Par ailleurs, un deces a ete observe chez deux patients du deuxieme groupe (1,4%) . Aucune difference statistiquement significative n’a ete retrouvee entre les differents taux. Conclusion : Les resultats de notre serie mettent en evidence l’interet du prelevement sus-claviculaire et jugulaire inferieur permettant d’eviter le curage fonctionnel systematique. 7. Evaluation et prise en charge de l’envahissement mandibulaire dans les carcinomes epidermoïdes de la cavite orale et de l’oropharynx

Abstract

Objectif : Le but de notre etude est l’evaluation de l’atteinte mandibulaire dans les carcinomes epidermoides de la cavite orale et de l’oropharynx. Materiel et methodes : Il s’agit d’une etude retrospective a propos de 34 patients colliges sur 6 ans (1999-2004) et ayant un carcinome epidermoide de la cavite orale et/ou de l’oropharynx avec envahissement mandibulaire confirme a l’anatomopathologie. L’atteinte mandibulaire a ete evaluee par l’examen clinique, l’orthopantomographie, la tomodensitometrie, et par les constatations peroperatoires du chirurgien. Tous les patients ont eu un evidement ganglionnaire, une exerese tumorale associee a une mandibulectomie segmentaire interruptrice ou conservatrice. Resultats : L’envahissement mandibulaire a ete suspecte cliniquement chez 70,5% des patients devant l’adherence de la tumeur a l’os. Apres examen clinique et imagerie, cette atteinte a ete diagnostiquee chez 88,2% des patients. Dans 11,8% des cas, l’atteinte n’a ete suspectee qu’en peroperatoire. L’examen histologique a confirme l’atteinte osseuse chez tous les patients. La mandibulectomie segmentaire a ete pratiquee chez 17 patients devant l’atteinte du canal mandibulaire. Vingt-huit patients ont eu une radiotherapie postoperatoire a la dose moyenne de 64 Gy. Les taux de recidive, de metastase et de deces etaient respectivement de 11,7%, 17,6% et 17,6% pour les patients qui ont eu une mandibulectomie segmentaire, et de 23,5%, 17,6% et 23,5% pour ceux qui ont eu une mandibulectomie conservatrice. Par ailleurs, 76,5% et 29,5% des patients ayant eu respectivement une mandibulectomie segmentaire et une mandibulectomie conservatrice avaient une gene fonctionnelle importante lors de l’alimentation. Conclusion : L’examen clinique, l’imagerie et l’examen peroperatoire sont d’un apport capital dans l’evaluation de l’atteinte mandibulaire. Une atteinte epargnant le canal mandibulaire justifie une mandibulectomie conservatrice permettant d’avoir de meilleurs resultats esthetiques et fonctionnels tout en assurant un controle carcinologiquement satisfaisant. 8. Les cancers de l’orbite etude retrospective à propos de 31 cas

Abstract

locoregionale rendant le traitement difficile et mutilant. Dans ce travail, nous rapportons notre experience dans la prise en charge diagnostique et therapeutique de ces tumeurs. Materiels et methodes : Notre etude retrospective a concerne 31 cas de cancers de l’orbite colliges sur 13 ans (1993- 2005). Tous les patients ont beneficie d’un examen clinique complet, d’une imagerie du massif facial (TDM et/ou IRM) et d’une biopsie de la tumeur. Le traitement a ete base sur la chirurgie, la radiotherapie et/ou la chimiotherapie. Resultats : La symptomatologie clinique etait dominee par les signes ophtalmologiques et les algies faciales. L’imagerie a montre dans tous les cas un processus expansif tissulaire a point de depart orbitaire, avec lyse osseuse orbitaire chez 16 patients (51,6%), une extension au massif facial chez 7 patients (22,6%), endocrâniennes chez 6 patients (19,4%) et des formes bilaterales atteignant les deux orbites dans 3 cas (9,7%) l’anatomopathologie montrait une predominance des lymphomes malins non hodgkiniens (32,3%) et des carcinomes epidermoides (32,3%), suivis des rhabdomyosarcomes embryonnaires (19,4 %). Douze patients ont ete traites par chirurgie et radiotherapie postoperatoire, dix patients par une association radio-chimiotherapie, et neuf autres par une chimiotherapie neo-adjuvante. La survie globale etait de 67,8% a 3 ans, 48,4% a 5 ans et 22,6% a 10 ans. Conclusion : Les cancers de l’orbite sont de mauvais pronostic. Un diagnostic precoce et un traitement radical et adapte au type histologique permet d’ameliorer la survie et la qualite de vie chez les patients atteints de ces tumeurs. 9. Prise en charge de l’hyperparathyroidie primaire : a propos de 25 cas et revue de la littérature

Abstract

Objectif : Les auteurs rapportent leur experience dans la prise en charge de l’hyperparathyroidie primaire (HPTP) et comparent leurs resultats par rapport aux donnees de la litterature en discutant les avantages potentiels des differentes techniques chirurgicales. Materiel et methodes : Il s’agit d’une etude retrospective a propos de 25 patients colliges sur 8 ans (1998-2005). Tous les patients ont beneficie d’une echographie cervicale, d’un bilan renal et d’un dosage de la calcemie et de la parathormonemie (PTH). Une TDM cervicale a ete pratiquee dans 8 cas et une IRM dans 3 cas. Une scintigraphie a la tetrofosmine a ete realisee dans 10 cas, et a double isotope (thallium 201-technetium 99) dans 2 cas. Une cervicotomie avec exploration bilaterale systematique des 2 loges thyroidiennes a ete pratiquee dans tous les cas. En postoperatoire, la surveillance a ete clinique et biologique avec un recul moyen de 22 mois. Resultats : La calcemie etait elevee chez 23 patients et la PTH dans tous les cas. L’echographie a detecte une hypertrophie d’une glande parathyroide chez 14 patients (56%) et de 2 glandes chez 1 seul patient. La TDM cervicale a permis de localiser la lesion dans 5 cas. L’IRM etait normale dans les 3 cas ou elle a ete pratiquee. La scintigraphie a montre une hyperfixation dans 8 cas. Une exerese d’adenomes a ete pratiquee dans 92% et une parathyroidectomie subtotale dans 8% des cas. Le taux de guerison etait de 88% apres premiere intervention. Une reprise chirurgicale avec exerese d’adenome a ete pratiquee chez 2 patients (8%). Par ailleurs aucune complication n’a ete notee en postoperatoire. Conclusion : La chirurgie parathyroidienne classique donne de bons resultats avec des taux de guerison eleves et un taux de morbidite global faible. Ces resultats sont comparables avec ceux de la litterature. La chirurgie mini-invasive donne des resultats semblables mais impose des imperatifs techniques, une imagerie performante et des patients rigoureusement selectionnes. Abstract

American oil palm ( Elaeis oleifera ) is an important source of dietary oil that could fulfill the increasing worldwide demand for cooking oil. Therefore, improving its production is crucial and could be realized through breeding and genetic engineering approaches aiming to obtain high-yielding varieties with improved oil content and quality. The fatty acid composition and particularly the oleic/linoleic acid ratio are major factors influencing oil quality. Our work focused on a fatty acid desaturase (FAD) enzyme involved in the desaturation and conversion of oleic acid to linoleic acid. Following the in silico identification and annotation of Elaeis oleifera FAD2, its molecular and structural features characterization was performed to better understand the mechanistic bases of its enzymatic activity. EoFAD2 is 1173 nucleotides long and encodes a protein of 390 amino acids that shares similarities with other FADs. Interestingly, the phylogenetic study showed three distinguished groups where EoFAD2 clustered among monocotyledonous taxa. EoFAD2 is a membrane-bound protein with five transmembrane domains presumably located in the endoplasmic reticulum. The homodimer organization model of EoFAD2 enzyme and substrates and respective substrate-binding residues were predicted and described. Moreover, the comparison between 24 FAD2 sequences from different species generated two interesting single-nucleotide polymorphisms (SNPs) associated with the oleic/linoleic acid contents.

Abstract

Cultivation of faba bean ( Vicia faba L.) in Tunisia is largely based on improved varieties of the crop. However, a few farmers continue to produce local cultivars or landraces. The National Gene Bank of Tunisia (NGBT) recently launched a collection project for faba bean landraces, with special focus on the regions of the North West, traditionally devoted to cultivating grain legumes, and where around 80% of the total national faba bean cultivation area is located. The seed phenotypic features of the collected samples were studied, and the genetic diversity and population structure analyzed using simple sequence repeat markers. The genetic constitution of the present samples was compared to that of faba bean samples collected by teams of the International Center for Agricultural Research in the Dry Areas (ICARDA) in the 1970s in the same region, and stored at the ICARDA gene bank. The results of the diversity analysis demonstrate that the recently collected samples and those stored at ICARDA largely overlap, thus demonstrating that over the past 50 years, little genetic change has occurred to the local faba bean populations examined. Abstract

Unlabelled: Extracranial head and neck neurogenic tumors are rare and usually revealed by histological examination. The aim of this study was to review the clinical, radiological and therapeutic particularities of these tumors., Methods: This retrospective study concerns 47 patients with neurogenic tumors of the head and neck, operated on between 1989 and 2011 (22 years period). All patients had complete physical examination and ultrasonography was performed when a cervical extension was found. CT scan and MRI were performed in 16 cases. Minimum follow up was 4 years., Results:   A sinusonasal tumor was found in 9 cases and a cervical mass was seen in 28 cases. Parapharyngeal extension was observed in two cases. Two patients had tympano-jugular glomic tumors and 8 of them had a cervical soft tissue tumor. Complete surgical resection was performed in 46 patients. Histological examination revealed a benign tumor in 91% of cases (n=43), 24 of them were schwannomas. Malignant tumors were seen in 4 cases: esthesioneuroblastoma (3 cases) and malignant schwannoma (1 case). These patients received post operative radiotherapy. After surgery, two patients had Claude Bernard Horner syndrome and one had a definitive facial nerve palsy., Conclusion: Extracranial head and neck neurogenic tumors may have several aspects depending on their localisation and their histological type. Surgery, when performed, should be complete with minimum complications.

Abstract

Faba bean (Vicia faba L.) is a globally important nitrogen-fixing legume, which is widely grown in a diverse range of environments. In this work, we mine and validate a set of 845 SNPs from the aligned transcriptomes of two contrasting inbred lines. Each V. faba SNP is assigned by BLAST analysis to a single Medicago orthologue. This set of syntenically anchored polymorphisms were then validated as individual KASP assays, classified according to their informativeness and performance on a panel of 37 inbred lines, and the best performing 757 markers used to genotype six mapping populations. The six resulting linkage maps were merged into a single consensus map on which 687 SNPs were placed on six linkage groups, each presumed to correspond to one of the six V. faba chromosomes. This sequence-based consensus map was used to explore synteny with the most closely related crop species, lentil and the most closely related fully sequenced genome, Medicago. Large tracts of uninterrupted colinearity were found between faba bean and Medicago, making it relatively straightforward to predict gene content and order in mapped genetic interval. As a demonstration of this, we mapped a flower colour gene to a 2-cM interval of Vf chromosome 2 which was highly colinear with Mt3. The obvious candidate gene from 78 gene models in the collinear Medicago chromosome segment was the previously characterized MtWD40-1 gene controlling anthocyanin production in Medicago and resequencing of the Vf orthologue showed a putative causative deletion of the entire 5' end of the gene., (© 2015 The Authors. 35. Ectopic lingual thyroid.

Abstract

Ectopy of the thyroid gland is an abnormal embryological development. Its occurrence in children is rare. In this study, we report the case of a 12-year-old girl that presented with dysphagia and nocturnal dyspnea. Magnetic resonance imaging confirmed the presence of a lingual thyroid. Thyroid scintigraphy showed intense and elective uptake of radiotracer at the base of the tongue. Hormonal tests revealed hypothyroidism. Treatment consisted of opotherapy based on levothyroxine. Evolution has been favourable and the patient showed significant improvement with reduction of the dyspnea and the dysphagia and normalization of thyroid hormone tests.

Abstract

Objective: In this retrospective study, we describe our experience in the diagnosis and management of rhinocerebral mucormycosis (RCM), a rapidly lethal fungal infection., Methodology: Between 1997 and 2007, five patients hospitalized for suspicion of RCM. Computed tomography was performed in all cases, and diagnosis was confirmed after anatomopathological or mycological examination. All patients underwent medical and surgical treatment. Follow-up was clinical and radiological with a mean period of 17 months., Results: All patients were diabetic. Exophthalmia, rhinorrhea, and ophthalmoplegia were the most frequent symptoms observed. One patient had loss of visual acuity and another exhibited peripheral facial palsy. One patient had extensive hemifacial cutaneous necrosis. Nasal endoscopy revealed black necrotic lesions in one case, and another patient had a tumefaction localised in the left middle meatus. Necrotic lesions were most often found in the orbit, the maxillary and the ethmoidal sinuses on computed tomography (four cases for each site). One patient had thrombophlebitis of the cavernous sinus, and another had an intracranial extension. All patients were administered ordinary insulin and intravenous amphotericin B. Surgical debridement of the nasal cavity and the involved sinuses was performed through lateral rhinotomy (four cases) or endoscopy (one case). Unilateral orbital exenteration was associated in two cases. Progression was favourable in four cases; one patient died from sepsis despite aggressive treatment., Conclusion: Early diagnosis is crucial for the management of RCM. Treatment of underlying disorders, use of intravenous amphotericin B, and aggressive surgical intervention are key in reducing morbidity and mortality rates.

Abstract

Objective: The aim of this review is to study natural history, presentation, treatment and prognosis of head and neck liposarcomas., Methods: We carry a retrospective study about 15 patients diagnosed and treated in our department between 1969 and 2001. Therapeutic modalities included surgery, radiotherapy and chemotherapy., Results: Tumors were mostly localized at the neck (4 cases) and the scalp (3 cases). Cervical lymphadenopathy was found in 1 case. Chest X-ray found showed metastases in 2 cases. Histological types of liposarcomas were the following: myxoid (5 cases), well differentiated (4 cases), pleomorphic (4 cases), round cell (1 case) and dedifferentiated (1 case). Twelve patients underwent surgery. Complete excision was performed in 7 cases. In the other 5 cases, exeresis was incomplete and 4 of them received postoperative radiotherapy. Three patients were not operated because of vascular involvement (1 case) or metastasis (2 cases), and they all received radiotherapy alone. After initial treatment, complete remission was achieved in 7 cases (46.7%). A recurrence occurred in 8 cases (53.3%). Two of these patients were reoperated and another one received chemotherapy. In the other 5 cases, the tumor was uncontrollable and no additional treatment has been proposed. Five-year survival rate was 87% for patients who underwent surgery alone, 75% for those who underwent surgery and postoperative radiotherapy, and 0% for those treated by radiotherapy alone., Conclusion: The mainstay of treatment of head and neck liposarcomas is surgical excision and the prognosis is largely determined by the histological grade and the clinical stage., (Copyright (c) 2009 Elsevier Ireland Ltd. Abstract

Background: Upper respiratory tract (URT) infections are common in children. The knowledge of their risk factors allows in some cases better management. The aim of our study is to evaluate the role of allergy in this pathology., Methods: Through a cross-sectional study about 100 children having recurrent URT infections, we evaluated the prevalence of allergy in order to find a relation between the two pathologies in general, and between allergy and each recurrent pathology. The results were compared with those of a reference group involving 164 children., Results: Our study confirms a statistically significant association between allergy and recurrent URT infections (p=0.01), also between allergy and rhinopharyngitis (p=0.02), rhinosinusitis (p=0.001) and acute otitis media (p=0.01)., Conclusion: Allergy represents a risk factor for recurrent URT infections, particularly for rhinopharyngitis, rhinosinusitis and acute otitis media. Adequate management of allergy may consequently be beneficial for the treatment and the prevention of these infections.
