BACKGROUND: Acute appendicitis is the most frequent surgical emergency in the paediatric population. Complicated appendicitis accounts for 30% of cases and is inextricably linked to postoperative infectious complications. A study at our institution showed that amoxicillin-clavulanate resistant Escherichia coli in complicated appendicitis was significantly linked to postoperative infectious complications. These findings led to a change in the empirical antibiotic protocol (amoxicillin-clavulanate changed to ceftriaxone + metronidazole as of 2017), intending to reduce postoperative infectious complications in complicated appendicitis in our institution. AIM OF THE STUDY: This study aimed to analyse the microbiology and resistance profiles of pathogens of complicated appendicitis at our institution since implementing the new antibiotic protocol and the postoperative infectious complications rate. METHODS: We designed a retrospective comparative cohort study. During the defined study period (01 January 2017 to 31 July 2020), medical records were analysed for cases of acute appendicitis, complicated appendicitis and postoperative infectious complications, retaining only those who fulfilled inclusion criteria. Postoperative outcomes, microbiology and antibiotic resistance of peritoneal swabs were analysed. RESULTS: During the study period, 95 patients presented with a complicated appendicitis, and 11 (12%) developed postoperative infectious complications. The most frequent pathogens found in complicated appendicitis were E. coli (66%), Streptococcus anginosus (45%), and Bacteroides fragilis (22%). Pseudomonas aeruginosa was present in 17% of complicated appendicitis. Pathogens involved in postoperative infectious complications mirrored the distribution found in complicated appendicitis without postoperative infectious complications. Antibiotic susceptibility analysis showed that 10 (15%) of E. coli strains were resistant to amoxicillin-clavulanate but sensitive to ceftriaxone + metronidazole, with only one strain responsible for causing a postoperative infectious complication. Six additional strains of E. coli (9%) were resistant to amoxicillin-clavulanate and our empirical antibiotic regimen but were not associated with an increase in postoperative infectious complications. Compared with our previous study, there was a decrease in postoperative infectious complications from 16% to 12%. Postoperative infectious complications caused by amoxicillin-clavulanate-resistant E. coli decreased from 28% to 9%. CONCLUSION: This retrospective study demonstrated a decrease in the rate of postoperative infectious complications due to amoxicillin-clavulanate-resistant E. coli in complicated appendicitis. These findings accentuate the need to implement evidence-based treatment protocols based on local microbiology profiles and resistance rates to optimise post-operative antibiotics in complicated appendicitis.

We introduce the concept of chromatism compensation in nulling interferometry that enables a high rejection ratio in a wide spectral band. Therefore the achromaticity condition considered in most nulling interferometers can be relaxed. We show that this chromatism compensation cannot be applied to a two-beam nulling interferometer, and we make an analysis of the particular case of a three-telescope OCIS codes: 120.3180, 350.1260.

Extreme ultraviolet (EUV) lithography uses reflective ring-field projection systems. Geometrical obstruction limits the possible system configurations to small domains of the parameter space. We present an analysis, a search method, and a classification of these unobstructed domains. The exhaustive search method based on paraxial analysis provides an effective means for determining all possible design forms and for finding useful starting configurations for optimization. In this theoretical study we show that by removing or depositing additional multilayer (ML) periods of a thin-film interference coating, distortions in the reflected wave front induced by surface figure errors can be corrected. At [lambda] = 13.4 nm in the extreme-ultraviolet region the removal or deposition of a single period of the standard two-component molybdenum-silicon (Mo/Si) ML interference coating induces an effective phase change of magnitude 0.043 [pi] with respect to an identical optical thickness in vacuum. The magnitude of this wave-front shift can be enhanced with multicomponent MLs optimized for phase change on reflection. We briefly discuss the contributions of the shift in the effective reflection surface of the ML on the phase change. Direct detection of exoplanets is possible by use of a technique called nulling interferometry, which is based on destructive interference of light of the bright object and constructive interference of the faint object. In the infrared wavelength region, this implies that light of a star must be attenuated by a certain factor, the so-called rejection ratio, which typically equals [10.sup.6]. This can be achieved by use of phase shifters, which apply a phase shift of [pi] rad with an average error no greater than 2 mrad over a predefined wavelength region. For a 6-18-[micro]m wavelength interval, this is a tough constraint. We show that the 2-mrad constraint can be relaxed if more than two beams participate in the beam recombination. We focus our attention on dispersive phase shifters and show that rejection ratios beyond [10.sup.6] can be reached easily by use of a system of four or more apertures and simple dispersive phase shifters that consist of only one material. In a new type of illumination system for reflective liquid-crystal displays, the frontlight, unwanted shadows appear in certain viewing directions. It will be shown that for an accurate description of these shadows the geometrical optics approach is not satisfactory and that Fresnel diffraction has to be taken into account. A model for the diffraction effects was developed, and the predicted results correspond well to the measurements. In addition, some remarkable effects were explained from the theory of Fresnel zones. In the infrared wavelength region, a typical star is approximately a million times brighter than the planet that surrounds it, which is a major problem when we attempt to detect exoplanets in a direct manner. Nulling interferometry is a technique that one can use to solve this problem by attenuating the stellar light and enhancing that of the planet. Generally, deep nulling is achieved by use of achromatic phase shifters (APSs). Unfortunately, the technology needed to build these APSs is not yet fully developed. We show that deep nulling can also be achieved by using delay lines only. We investigate the nulling depth as a function of the width of the wavelength interval and the number of telescopes. We also show that we can obtain nulling depths of less than [10.sup.-6], which are required for exoplanet detection. Furthermore, we investigate the properties of the transmission map and make a comparison between our system and an APS system.

This study concerns the deployment of large numbers of container transporting vehicles on a network consisting of (dedicated) roads and logistic sites such as container terminals. Each site is managed autonomously by a site manager. The fleet of vehicles is deployed by a fleet manager. On request of the site managers, the fleet manager allocates vehicles to be hired by the site managers. Next, within these allocations, each site manager issues transportation jobs and assigns these jobs to the allocated vehicles. The objective of the fleet manager is to avoid unpaid empty driving and to keep the size of its fleet just large enough the serve the demand for vehicles. The objective of the site managers is to minimize the operational costs. In this context a new coordination system is proposed to allocate vehicle capacity, operating on the basis of a win-win situation for all actors. The coordination takes place at two levels. One level concerns the drawing up of an adaptive fleet deployment plan. The other level resides under control of the individual site managers and concerns the assignment of transportation jobs to vehicles, given the allocations of the fleet deployment plan. Both use repetitive linear programming procedures. Small size simulation experiments show that the approach might be effective.

We report on a way to shape surfaces with fluid jet polishing by adjusting the influence function (the shape of the footprint of the nozzle) instead of changing the dwell time of the nozzle on the surface. In that way, the surface is processed homogeneously, and no dip is generated in the center of the workpiece. - Abstract
Rationale: Despite oxygen's classification as an essential medication by the World Health Organization, it is inconsistently available in many resource-constrained settings. Hypoxemia is associated with increased mortality, and mounting evidence suggests that hyperoxia may also be associated with adverse outcomes. Objectives: To determine if overuse of oxygen for some patients in a Rwandan tertiary care hospital emergency department might coexist with oxygen shortages and underuse of oxygen for other patients, and whether an educational intervention coupled with provision of pulse oximeters could improve the distribution of limited oxygen resources. Methods: We screened all patients in the adult emergency department (ED) of the University Teaching Hospital of Kigali for hypoxemia and receipt of oxygen therapy for 5 weeks. After completing baseline data collection, we provided pulse oximeters and conducted a didactic training with pre- and posttests on oxygen titration, with a chosen target oxygen saturation (SpO2) of 90% to 95%. Four and 12 weeks after the intervention, we evaluated all patients in the ED again for SpO2and receipt of oxygen therapy for 4 weeks each period.We also recorded ED oxygen use and availability of reserve oxygen for the hospital during the three study periods. Results: During all data collection periods, 214 of 1,765 (12.1%) unique patients screened were hypoxemic. The proportion of patient-days with appropriately titrated oxygen therapy (SpO2, 90-95%) increased from 18.7% at baseline to 38.5% and 42.0% at 4 and 12 weeks postintervention (P<0.001). On a multiple-choice examination testing knowledge of appropriate oxygen titration, clinicians' scores improved from average 60% (interquartile range [IQR], 40-80%) correct to 80% (IQR, 60-80%) correct immediately after the educational intervention (P<0.001). We analytically calculate the focus setting for a beam with a high numerical aperture (NA) that optimizes its Strehl ratio in the case of small aberrations up to the ’just’ diffraction-limited value (Strehl ratio ≥0.80). The optimum focus setting deviates from the one that follows from a minimization of the wavefront aberration with the aid of the Zernike aberration coefficients. This deviation stems largely from the fact that the common quadratic approximation of the focus term becomes inadequate in the high-NA case. Fundamental high-NA amplitude nonuniformity in the exit pupil of an optical system in the case of a linearly polarized incident beam also influences the optimum focus setting. Results for spherical aberration and astigmatism are presented for an NA-value of 0.95.

We report on what we believe to be a novel classification method for polishing processes that we apply in our laboratory on a regular basis. Two parameters are deduced from the in situ iTIRM (intensity-detecting total-internal-reflection microscopy) measurement method. Contrary to Preston's law, which gives the removal rate, the parameters of the iTIRM process are a measure of the change in surface quality (roughness, subsurface damage, and scratch and dig) and the duration of the polishing process. Future astronomical space missions will comprise a constellation of several optical telescopes to detect exo-planets by interferometric nulling of starlight. The Darwin mission of the European Space Agency (ESA) and NASAs Terrestrial Planet Finder Interferometer both consist of a free-flying collection of telescopes and a beam combiner. As such, the constellation provides a co-phased array of telescopes that can also be used for aperture synthesis imaging. This imaging technique relies on recording intensity interference patterns, in which the layout of the beam combination optics and the detector play a key role. Several designs for beam combination have been proposed in the literature. In this article, we compare these beam combiners by rigorously simulating the imaging process of a weak stellar source, taking into account the photon arrival statistics, an imperfect detection process and the image reconstruction from the recorded data. The results are presented as the to be expected reconstruction error in the luminous intensity distribution function of a wide-field stellar source versus the provided amount of photons. Using these results, the optimum design of the combination beam combiner and detector array can be identified.

- Abstract
Purpose The role of temporary ovarian suppression with gonadotropin-releasing hormone agonists (GnRHa) during chemotherapy as a strategy to preserve ovarian function and fertility in premenopausal women remains controversial. This systematic review and meta-analysis using individual patient–level data was conducted to better assess the efficacy and safety of this strategy in patients with early breast cancer. Methods The trials in which premenopausal women with early breast cancer were randomly assigned to receive (neo)adjuvant chemotherapy alone or with concurrent GnRHa were eligible for inclusion. Primary end points were premature ovarian insufficiency (POI) rate and post-treatment pregnancy rate. Disease-free survival and overall survival were secondary end points. Because each study represents a cluster, statistical analyses were performed using a random effects model. Results A total of 873 patients from five trials were included. POI rate was 14.1% in the GnRHa group and 30.9% in the control group (adjusted odds ratio, 0.38; 95% CI, 0.26 to 0.57; P, 001). A total of 37 (10.3%) patients had at least one post-treatment pregnancy in the GnRHa group and 20 (5.5%) in the control group (incidence rate ratio, 1.83; 95% CI, 1.06 to 3.15; P = .030). No significant differences in disease-free survival (adjusted hazard ratio, 1.01; 95% CI, 0.72 to 1.42; P = .999) and overall survival (adjusted hazard ratio, 0.67; 95% CI, 0.42 to 1.06; P = .083) were observed between groups. Conclusion Our findings provide evidence for the efficacy and safety of temporary ovarian suppression with GnRHa during chemotherapy as an available option to reduce the likelihood of chemotherapy-induced POI and potentially improve future fertility in premenopausal patients with early breast cancer., SCOPUS: re.j, info:eu-repo/semantics/published

Contains fulltext : 167177.pdf (publisher's version ) (Open Access), BACKGROUND: Regulatory T (Treg) cells, a subset of CD4+ T lymphocytes, are mediators of immunosuppression in cancer, and, thus, variants in genes encoding Treg cell immune molecules could be associated with ovarian cancer. METHODS: In a population of 15,596 epithelial ovarian cancer (EOC) cases and 23,236 controls, we measured genetic associations of 1,351 SNPs in Treg cell pathway genes with odds of ovarian cancer and tested pathway and gene-level associations, overall and by histotype, for the 25 genes, using the admixture likelihood (AML) method. The most significant single SNP associations were tested for correlation with expression levels in 44 ovarian cancer patients. RESULTS: The most significant global associations for all genes in the pathway were seen in endometrioid (p = 0.082) and clear cell (p = 0.083), with the most significant gene level association seen with TGFBR2 (p = 0.001) and clear cell EOC. Gene associations with histotypes at p < 0.05 included: IL12 (p = 0.005 and p = 0.008, serous and high-grade serous, respectively), IL8RA (p = 0.035, endometrioid and mucinous), LGALS1 (p = 0.03, mucinous), STAT5B (p = 0.022, clear cell), TGFBR1 (p = 0.021 endometrioid) and TGFBR2 (p = 0.017 and p = 0.025, endometrioid and mucinous, respectively). CONCLUSIONS: Common inherited gene variation in Treg cell pathways shows some evidence of germline genetic contribution to odds of EOC that varies by histologic subtype and may be associated with mRNA expression of immune-complex receptor in EOC patients.
