Background It is well established that regular compliance during periodontal maintenance therapy (PMT) maintains the stability of periodontal clinical parameters obtained after active periodontal therapy (APT). However, compliance during PMT has not yet been related to subgingival bacterial levels. Thus, this study followed individuals in PMT over 6 years and longitudinally evaluated the effects of compliance on periodontitis-associated bacterial levels and its relation to periodontal status. Methods From a 6-year prospective cohort study with 212 individuals in PMT, 91 were determined to be eligible. From this total, 28 regular compliers (RC) were randomly selected and matched for age and sex with 28 irregular compliers (IC). Complete periodontal examination and microbiological samples were obtained 5 times: T1 (prior to APT), T2 (after APT), T3 (2 years), T4 (4 years), and T5 (6 years). Total bacteria counts and levels of Actinomyces naeslundii, Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola were evaluated through quantitative polymerase chain reaction. Results RC had less tooth loss and better clinical and microbiological conditions over time when compared with IC. IC had higher total bacterial counts and higher levels of T. denticola. Moreover, among IC, total bacterial counts were positively associated with plaque index and bleeding on probing, while levels of A. naeslundii, T. forsythia, and T. denticola were negatively associated with clinical attachment loss (4 to 5 mm) among RC. Conclusions Compliance positively influenced subgingival microbiota and contributed to stability of periodontal clinical status. Regular visits during PMT sustained microbiological benefits provided by APT over a 6-year period.

BACKGROUND AND OBJECTIVE Human beta-defensins (hBDs) contribute to innate immunity antimicrobial activity. They are also effective in the adaptive immune response and may play a crucial role in the susceptibility to diseases of the oral cavity. This study aimed to evaluate the levels of hBD-1 in the gingival crevicular fluid of individuals with and without chronic periodontitis. MATERIAL AND METHODS Twenty periodontally healthy individuals (H) and 20 individuals with chronic periodontitis were recruited. Gingival crevicular fluid samples were collected from: healthy sites (Hh) from periodontally healthy individuals; and healthy sites (Ph), sites with gingivitis (Pg), and sites with periodontitis (Pp) from individuals with periodontitis. The levels of hBD-1 (pg/mL) were measured using enzyme-linked immunosorbent assay. Comparisons of hBD-1 between individuals (H and chronic periodontitis) and among sites (Hh, Ph, Pg, Pp) were performed through hierarchical linear modeling. RESULTS Gingival crevicular fluid levels of hBD-1 were: Hh = 229.52 ± 138.96 (median 199.26), Ph = 53.88 ± 58.17 (median 35.75), Pg = 57.11 ± 40.18 (median 39.90) and Pp = 55.31 ± 37.28 (median 54.19). However, the literature offers an insufficient number of published reports regarding the effect of bariatric surgery on oral health. As such, the present study aims to determine the association between periodontal status and being over-weight/obese in prebariatric and postbariatric surgery populations of Brazil. Methods: Three hundred forty-five participants between 18 and 60 years of age, from both sexes, were grouped according to prebariatric or postbariatric surgery and underwent a complete periodontal examination. Biologic, demographic, and behavioral variables were analyzed in a Poisson regression model. Results: Considering the timing of bariatric surgery, the sample was divided into three groups: PRE-OP (preoperative, n = 133), POS-OP1 (postoperative 6 months, n = 140). Regardless of the type of surgery (PRE-OP, POS-OP1, POS-OP2), the prevalence of periodontitis proved to be high (81.45%). There was a statistically significant difference in the prevalence of periodontitis among the PRE-OP, POS-OP1, and POS-OP2 groups (P=0.040). Chronic periodontitis (CP) and aggressive periodontitis (AP) are inflammatory diseases and the main cause of dental loss in adults. We aimed to investigate the expression of adhesion molecules and the source of proinflammatory and anti-inflammatory cytokines in circulating mononuclear cells from patients with CP and AP.Peripheral blood mononuclear cells from healthy controls and CP or AP patients were collected. The expression of the cell adhesion molecules CD11a and CD11b, and the cellular sources of interleukin (IL)-4, IL-10, IL-12, interferon-γ, and tumor necrosis factor-α by distinct subpopulations of circulating leukocytes were determined using flow cytometry.The expression of CD11a, but not CD11b, was significantly higher within the CD4(+) and CD8(+) T cells in CP and AP than in healthy controls. The frequencies of tumor necrosis factor-α-expressing CD4(+) T cells and CD14(+) cells were higher in AP and CP, compared to healthy controls, respectively. Moreover, the frequency of IL-10 expressing CD14(+) cells was higher in CP, but not AP, compared to healthy controls CD4(+) T cells committed to IL-4 production was higher in CP than in healthy controls.These results suggest the participation of CD11a in the pathogenesis of periodontal lesions and show distinct cellular sources of immunoregulatory cytokines in AP versus CP.

Chronic periodontitis is a highly prevalent endogenous polymicrobial disease. To better understand the etiology of the disease a quantitative approach is mandatory and real-time PCR is the molecular technique currently preferred to achieve this purpose. Taking into account that such a kind of study is still scarce, we aimed to evaluate the association between periodontal microbiota and chronic periodontitis. A total of 60 low-income age-matched female adults, 30 with chronic periodontitis and 30 without periodontal disease, were enrolled. DNA obtained from subgingival specimens was used for quantification of Aggregatibacter actinomycetemcomitans , Eikenella corrodens , Fusobacterium nucleatum , Porphyromonas gingivalis , and Prevotella intermedia by real-time PCR. A. actinomycetemcomitans , E. corrodens , and F. nucleatum were detected in all subjects, P. gingivalis was observed in 70.0% and 46.6% and P. intermedia in 90.0% and 80.0% of chronic periodontitis patients and periodontally healthy subjects, respectively. P. gingivalis mean count was significantly higher in patients with chronic periodontitis than in periodontally healthy individuals. Accurate detection and quantification of five putative periodontal pathogens was feasible using a simple and fast real-time PCR protocol. Although P. gingivalis and P. intermedia have been found more commonly in chronic periodontitis patients, no statistical difference was observed between periodontally diseased and healthy groups. Quantitative data indicated association between P. gingivalis and chronic periodontitis. However, because of its uneven distribution, it should not be solely taken as a marker of periodontal status.

IL-10 and TNF-α are cytokines that have complex and opposing roles in the inflammatory responses. G/A polymorphisms at position –1082 ofIL10and –308 ofTNFAgenes have been reported to influence the expression of IL-10 and TNF-α, respectively. The aim of this study was to investigate the association between theIL10(-1082) andTNFA(- 308) gene polymorphisms with different clinical forms or severity of periodontitis in a sample of Brazilian individuals. DNA was obtained from oral swabs of 165 Brazilian individuals, which were divided into three groups: individuals with chronic periodontitis, aggressive periodontitis and individuals without clinical evidence of periodontitis. Evaluation ofIL10andTNFApolymorphisms was performed by RFLP analysis. Statistical analysis of data was performed using the χ2likelihood ratio and Fisher`s exact test. No significant differences in the genotype and allele distribution of eitherIL10orTNFAwere observed among individuals with different clinical forms or with different degrees of severity of periodontitis. Moreover, combined analysis ofIL10andTNFApolymorphisms did not show any association with periodontal status. As conclusion, theIL10andTNFAgene promoter polymorphisms investigated are not associated with different clinical forms of periodontitis or with severity of the disease in the Brazilian population polymorphisms.

Different periodontitis definitions have been used in periodontal research. This study assessed the impact of case definition on the prevalence and extent rates of periodontitis. A data set including 340 periodontal records, collected in Belo Horizonte, Brazil, was used. Periodontitis was defined as: 1) one site with probing depth (PD) >or= 4 mm; 2) clinical attachment level (CAL) >or= 5 mm in >or= 4 sites + one site with PD >or= 4 mm; 3) CAL >or= 6 mm in >or= 2 teeth + one site with PD >or= 5 mm; 4) >or= 4 teeth with >or= 1 sites with PD >or= 4 mm + CAL >or= 3 mm; 5a) interproximal CAL or PD >or= 4 mm at >or= 2 sites, not on the same tooth; and 5b) interproximal CAL of >or= 6 mm at >or= 2 sites, not on the same tooth + PD >or= 5 mm at >or= 1 proximal site. Definition 5 was determined to be the gold standard and the definitions were compared by means of agreement, sensitivity, specificity, and positive and negative predictive values. Prevalence and extent rates greatly varied, from 13.8% to 65.3% and from 9.7% to 55.6%, respectively. The use of different case definitions has a great impact on the prevalence and extent rates of periodontitis.

We sought to investigate the gingival status of renal transplant recipients under SIR-based regimens with regard to the prevalence and potential risk variables associated with GO. Methods: A cross-sectional study was conducted at a public hospital in Belo Horizonte, MG, Brazil. Of 886 transplant recipients, an eligible sample of 144 subjects was selected. Medical, pharmacologic, and periodontal variables were recorded for each subject. GO was assessed through visual inspection and was evaluated in relation to variables of interest by univariate and multivariate analyses. Results: Although not clinically significant, 20.8% of the samples had GO, with a mean GO score of 5.0% 3.98% (range, 0% to 19%) and mean GO score per papilla of 0.378 0.361 (range, 0.1 to 1.9); the percentage of papilla affected by GO was 14.35% 9.86% (range, 5% to 45%). The multivariate final model, excluding periodontal variables, showed that the concomitant use of calcium channel blockers (P = 0.033) and the interaction between time since transplant and previous CNI use (P < 0.001) were associated with GO. Adding periodontal variables to the model improved its accuracy and retained papillary bleeding index as being strongly associated with GO (P < 0.001). Conclusions: GO was observed in a considerable number of subjects under SIR-based immunosuppressive regimens, although the relationship was not clinically significant. Susceptibility to and development of periodontal disease have been associated with psychological conditions. Previous studies have associated the presence of polymorphism in the promoter region of the serotonin transporter with several behavioral traits and psychological conditions such as depression, anxiety, and stress. The short allele S has a reduced transcriptional efficiency and is associated with lowered serotonin expression and uptake. The purpose of the present study was to investigate the association between 5-HTTLPR polymorphism and aggressive periodontitis in a sample of Brazilian individuals. This study involved 61 individuals affected by aggressive periodontitis and 71 without periodontitis. Genomic DNA was obtained from oral swabs, amplified by polymerase chain reaction (PCR), and genotyped at 5-HTTLPR. The Chi-square test and multivariate logistic regression were used for statistical analysis. The aggressive periodontitis group displayed a significantly higher occurrence of genotype SS (P < 0.01) and of allele S (P < 0.01). After adjustment for gender and age, it was observed that genotype SS occurred 8 times more frequently in this group. Our findings suggest that 5-HTTLPR polymorphism might be associated with aggressive periodontitis in the Brazilian population.

Summary Interleukin (IL)-6 is an inflammatory mediator involved in bone resorption. G/C polymorphism at position −174 of the IL-6 gene has been reported to influence IL-6 expression, with the G allele associated with higher expression levels. The aims of this study were to investigate the expression of IL-6 as well as the incidence of IL-6 (−174) gene polymorphism and their correlation to the severity of periodontitis in Brazilians. Peripheral blood mononuclear cells were collected from 12 non-smoker individuals with periodontitis for evaluation of IL-6 expression using flow cytometry. We observed a positive correlation between the mean clinical attachment loss and intensity of expression of IL-6, in which the greater the attachment loss, the higher the expression of IL-6 (P = 0·007, R2 = 0·52). Also, patients with severe periodontitis displayed a higher intensity of IL-6 expression compared to moderate periodontitis (P = 0·04). To determine the occurrence of IL-6 gene polymorphism, DNA was obtained from oral swabs of 209 Brazilian individuals with and without periodontitis. Polymerase chain reaction, restriction endonuclease digestion and electrophoresis were performed, allowing for detection of the IL-6 (−174) polymorphism. We observed that non-smokers with moderate periodontitis (P = 0·05) and control (P = 0·04) groups displayed a higher incidence of the G– genotype when compared to severe periodontitis. This suggests that the G– genotype may represent a protective role in severity of periodontitis. Thus, the increased expression of IL-6 and IL-6 (−174) polymorphism are associated with periodontal disease severity in Brazilian individuals.

The aim of the present study was to assess the incidence, severity, and extension of periodontal disease among young subjects.Three hundred sixty subjects from a public school in Belo Horizonte, Brazil were examined. Forty-four subjects with bone loss (BL) and/or clinical attachment loss (CAL)or = 4 mm, and no dental care during the following 52 months, were considered the reference sample and were recalled for a second examination. At both examinations, statistical analyses were performed for CAL and BL for subject, tooth, and site. Comparisons of CAL at subject, tooth, and site levels were performed with the Wilcoxon test for matched samples. Comparisons of BL at subject, tooth, and site levels were performed with the McNemar and Student t tests for dependent samples.Findings indicated an increase in the number of sites with CAL4 mm from baseline to follow-up. Sites that exhibited BL increased from 2.14% at baseline to 7.5% at follow-up (P0.001). The severity of periodontitis was characterized by increased CAL values at affected sites at both examinations (P0.001) and an increase in BL over time (mean BL at 35 affected sites was 2.6 mm at baseline and 3.5 mm at follow-up; P = 0.001). In addition, a 34% incidence of BL was verified.The incidence, severity, and extension of CAL and BL were significant findings. They indicate the importance of early detection of the disease and the need for preventive health care programs geared toward young subjects.

Background and Objective: It has been proposed that genotypes reflective of polymorphisms in cytokine genes can predispose individuals to disease by enhancing inflammatory processes. The C/T polymorphism at position −889 of the IL1A gene influences interleukin-1α expression, with the T allele inducing higher expression. The aim of this study was to evaluate the association of the IL1A (−889) gene polymorphism in Brazilian individuals with different clinical forms of periodontitis and severity of disease. Material and Methods: DNA was obtained from oral swabs of 163 Brazilian individuals and was amplified using the polymerase chain reaction (PCR). Products were submitted to digestion and were analyzed by electrophoresis to distinguish the C and T alleles. Results: A significant difference in the genotype distribution was observed when comparing the chronic periodontitis group with the control group, evaluating only nonsmokers (chi-squared analysis = 9.91; p = 0.007), as well as when smokers were included (chi-squared analysis = 6.36; p = 0.04). Moreover, we observed a higher incidence of the T allele in the chronic periodontitis group (37.8%) when compared with the control group (18.4%) in nonsmokers (p = 0.006, odds ratio = 2.69, confidence interval = 1.27–5.68) and also when smokers were included (p = 0.03, odds ratio = 1.87, confidence interval = 0.98–3.56). No statistical difference was observed when the aggressive periodontitis group was compared with the control group. With regard to severity of disease, no statistical difference was observed. Conclusion: These data show an association of the IL1A (−889) polymorphism with chronic periodontitis in Brazilian individuals.

Periodontal disease has been considered a systemic exposure implicated in a higher risk of adverse pregnancy outcomes. The aim of the present study was to determine whether maternal periodontitis is associated with an increased risk of preeclampsia.A case-control study was conducted in a public hospital in Belo Horizonte, Brazil. During the study period, 588 women, aged 14 to 46 years, were deemed eligible and had data available for analysis. Maternal demographic and medical data were collected from medical records. Preeclampsia was defined as blood pressure140/90 mm Hg andor =1+ proteinuria after 20 weeks of gestation. A periodontal examination was performed postpartum. Maternal periodontitis was defined as the presence of four or more teeth with one or more sites with a probing depthor =4 mm and clinical attachment lossor =3 mm at the same site. The effects of maternal age, chronic hypertension, primiparity, smoking, alcohol use, and number of prenatal visits were analyzed. Adjusted odds ratios (ORs) for preeclampsia were calculated using multivariate logistic regression.The prevalence of periodontitis was 63.9% and preeclampsia was 18.5%. Variables associated with preeclampsia were chronic hypertension (OR = 4.10; 95% confidence interval [CI] = 2.0 to 8.4; P = 0.001), primiparity (OR = 2.40; 95% CI = 1.5 to 3.9; P = 0.004), maternal age (OR = 1.07; 95% CI = 1.0 to 1.1; P = 0.001), and maternal periodontitis (OR = 1.88; 95% CI = 1.1 to 3.0; P = 0.001).Maternal periodontitis was determined to be associated with an increased risk of preeclampsia.

Immunological mechanisms participate in the pathogenesis of human chronic inflammatory periodontal disease (CIPD). Human CD4+lymphocytes express functionally heterogeneous profiles of cytokine production. CD26 is an integral membrane glycoprotein, that is, a marker of Th1-like cytokine development. The purpose of the present study was to compare the immuno-expression of CD26 receptor in periodontal sites with and without clinical attachment loss (CAL). Five patients with rapidly progressing periodontitis and one with juvenile periodontitis were investigated. Each patient presented at least one site with and without CAL. Ten sites with CAL and nine without any CAL were biopsied, followed by the immunohistochemical identification of the CD26 receptor using the MIB-DS2/7 antibody. The results demonstrated that the percentage of positive cells for this antigen in the periodontal sites with CAL was not significantly different from those without attachment loss. Therefore, Th1 cell impairment may not be directly involved with periodontal attachment loss.

Brain-derived neurotrophic factor (BDNF) is a member of the neurotrophic factor family. Outside the nervous system, BDNF has been shown to be expressed in various nonneural tissues, such as periodontal ligament, dental pulp, and odontoblasts. Although a role for BDNF in periodontal regeneration has been suggested, a function for BDNF in periodontal disease has not yet been studied. The aim of this study was to analyze the BDNF levels in periodontal tissues of patients with chronic periodontitis (CP) and periodontally healthy controls (HC). All subjects were genotyped for the rs4923463 and rs6265 BDNF polymorphisms. Periodontal tissues were collected for ELISA, myeloperoxidase (MPO), and microscopic analysis from 28 CP patients and 29 HC subjects. BDNF levels were increased in CP patients compared to HC subjects. A negative correlation was observed when analyzing concentration of BDNF and IL-10 in inflamed periodontium. No differences in frequencies of BDNF genotypes between CP and HC subjects were observed. However, BDNF genotype GG was associated with increased levels of BDNF, TNF-α, and CXCL10 in CP patients. In conclusion, BDNF seems to be associated with periodontal disease process, but the specific role of BDNF still needs to be clarified.

A hospital convenience sample of 594 eligible women was recruited from a women's health reference center of Belo Horizonte, Brazil. Four groups were formed considering BMI levels: BMI normal group ( = 352), overweight ( = 54), obesity level I ( = 48), obesity level II ( = 56), and obesity level III ( = 74). Full-mouth periodontal examination was performed and biological, demographic, and behavioral risk variables were evaluated. Obese and overweight women showed statistically significant differences in bleeding on probing, probing depth and clinical attachment level a parts per thousand yen4 mm, and frequency of periodontitis ( < 0.05) compared to women showing normal BMI. The final multivariate model for the occurrence of periodontitis revealed that obesity groups were significantly associated with periodontitis. In addition, age (25-45), smoking, diabetes, and hypertension remained significantly associated with the occurrence of periodontitis ( < 0.05). Periodontitis was positively associated with obesity, and this association was more evident as obesity levels increases. These findings indicate the need for early diagnosis and the inclusion of periodontal care in health care programs for obese women. Five years later, 80 individuals who had beendiagnosed with mucositis in the baseline examination were re-examined. Theseindividuals were divided into two groups: one group with preventive maintenanceduring the study period (GTP; n = 39), and another group without preventivemaintenance (GNTP; n = 41). The following parameters were clinically evaluated:plaque index, bleeding on periodontal and peri-implant probing, periodontal andperi-implant probing depth, suppuration and peri-implant bone loss. The influ-ence of biological and behavioural risk variables associated with the occurrenceof peri-implantitis was analysed using univariate and multivariate logistic regres-sion analyses.Results: The incidence of peri-implantitis in the global sample was 31.2%(GNTP = 43.9% and GTP = 18.0%).Conclusion: The absence of preventive maintenance in individuals with pre-exist-ing peri-implant mucositis was associated with a high incidence of peri-implanti-tis. Clinical parameters, such as bleeding on peri-implant probing, periodontalprobing depth and the presence of periodontitis were associated with a higher riskof developing peri-implantitis.

Methods: A total of 288 individuals diagnosed with chronic moderate-to-advanced periodontitis, who had finished active periodontal treatment, were evaluated in a public academic environment (AG) (n = 138), as well as in a private clinic (PG) (n = 150). A full-mouth periodontal examination was performed at baseline and at quadrimestral recalls, evaluating plaque index, probing depth, clinical attachment level, furcation involvement, bleeding on probing (BOP), and suppuration. Individuals' social, demographic, and biologic data, as well as compliance with recalls, were recorded. The effect of variables of interest and confounders were tested by univariate and multivariate analysis. Results: The PG demonstrated lower rates of periodontitis progression and tooth loss than did the AG. After adjusting for confounders, the risk variables of BOP (P=0.047), smoking (P = 0.003), and diabetes (P = 0.028) for the PG and smoking (P=0.047) for the AG showed a negative influence on periodontal status. Conclusions: In both groups, the periodontal maintenance therapy minimized the negative effect of the risk variables. To our knowledge, prospective studies (matched for sex, smoking, and diabetes) that investigated the influence of compliance in the progression of periodontitis and tooth loss in periodontal maintenance therapy (PMT) programs were not previously reported.A total of 58 regular complier (RC) and 58 erratic complier (EC) individuals were recruited from a prospective cohort with 238 patients under PMT and matched by sex, diabetes, and smoking habits. A full-mouth periodontal examination that included bleeding on probing (BOP), probing depths (PDs), clinical attachment levels, and number of teeth were determined at all PMT visits during a 3-year interval. The influence of variables of interest was tested through multivariate logistic regression.The progression of periodontitis and tooth loss was significantly lower among RC compared to EC patients. A higher progression of periodontitis was observed among EC patients who smoked. The final logistic model for the progression of periodontitis in the RC group included smoking (odds ratio [OR]: 4.2) and30% of sites with BOP (OR: 2.8), and the final logistic model for the progression of periodontitis in the EC group included smoking (OR: 7.3),30% of sites with BOP (OR: 3.2), PDs of 4 to 6 mm in 10% of sites (OR: 3.5), diabetes (OR: 1.9), and number of lost teeth (OR: 3.1).RC patients presented a lower progression of periodontitis and tooth loss compared to EC patients. This result highlighted the influence of the pattern of compliance in maintaining a good periodontal status. Moreover, important risk variables such as smoking and diabetes influenced the periodontal status and should be considered when determining the risk profile and interval time for PMT visits.

The present study aims to determine the oral health impacts among regular- and erratic-complier patients and their correlations with personality traits by means of the oral impacts on daily performance (OIDP) and the neuroticism extraversion openness five-factor inventory (NEO FFI-R) instruments. Methods: A total of 58 regular and 58 erratic compliers matched by sex, diabetes, and smoking were recruited from a cohort of 238 participants under periodontal maintenance. Periodontal examinations and questionnaire applications were performed in recalls during a 3-year interval. The influence of variables of interest on the OIDP was evaluated through univariate and multivariate linear regression analyses. The clinical evaluation of patient oral health records includedperiodontal parameters and the presence of tooth fracture. Cases and controls werecompared with demographic, behavioural and clinical variables of interest. Riskvariables for the occurrence of gingival recession were identified in multivariateregression models, with linear and logistic regressions.Results: The case group presented a higher prevalence and severity of gingivalrecession when compared with the control group. Individuals with tongue piercingspresented an 11 times greater chance for the occurrence of gingival recession in theanterior lingual mandibular region as compared with controls (OR511.0, 95% CI5.02–24.09). To evaluate the influence of examiner experience on the variability of periodontal probing depth measurements obtained by conventional manual probing.Thirty subjects with a diagnosis of chronic periodontitis were evaluated by an experienced examiner using an electronic probe and randomly assigned to 3 groups. Examiners with different levels of experience (undergraduate students, postgraduate students, and associate professors) evaluated each group with a manual probe. Electronic and conventional probing were repeated 45 days after cause-related periodontal therapy.A total of 8,127 periodontal sites were evaluated at the baseline examination and reassessment. Agreement between methods was satisfactory at the baseline examination (kappa = 0.45; P.001) and reassessment (kappa = 0.42; P.001). The best agreement between electronic and manual probing at the baseline examination was obtained by the postgraduate students (kappa = 0.66) and at reassessment by the associate professors (kappa = 0.60). Undergraduate students obtained the lowest agreement values in both examinations (kappa = 0.42 and 0.11, respectively).Examiner experience has direct influence on the accuracy of measurements. Dental schools must evaluate if the methodology employed to teach the use of conventional manual probing is effectively qualifying their students for dental practice.

Association studies between maternal periodontitis and an elevated risk for preeclampsia have shown conflicting results. The aims of the present case-control study were: 1) to evaluate the association between maternal periodontitis and preeclampsia before and after matching, assessing confounding and interaction; and 2) to evaluate the influence of the extent and severity of periodontal parameters, bleeding on probing (BOP), probing depth (PD), and clinical attachment loss (CAL), in association with preeclampsia.Initially, 1,206 Brazilian women were included and divided into a control group (1,042 non-preeclamptic women who gave birth to infants with adequate gestational age and birth weight) and a case group (164 preeclamptic women). Further, 125 preeclamptic women were matched according to age, chronic hypertension, and primiparity to 375 non-preeclamptic women randomly selected from the control group. Maternal periodontitis was defined as PDor =4 mm and CALor =3 mm at the same site in at least four teeth. The effect of variables of interest and confounding were assessed by univariate and multivariate analysis.After controlling for confounders, maternal periodontitis was included in the multivariate final model (odds ratio [OR] = 1.94; 95% confidence interval [CI]: 1.37 to 2.77; P0.001) and remained associated with preeclampsia after matching (OR = 1.52; 95% CI: 1.01 to 2.29; P = 0.045). The odds of preeclampsia were associated with an increase in the number of sites with BOP and PD and CALor =4 mm.Maternal periodontitis is a risk factor associated with preeclampsia, emphasizing the importance of periodontal care in prenatal programs.

It has been suggested that periodontitis is associated with systemic alterations such as adverse pregnancy outcomes. However, some conflicting results have been reported. This case-control study was conducted to determine the association between maternal periodontitis and preterm birth (PTB), low birth weight (LBW), and intrauterine growth restriction (IUGR).A total of 1,305 Brazilian women from multiethnic groups were enrolled and divided into four groups: control group (1,042 women who gave birth to infants with adequate gestational age and birth weight); PTB group (238 women, gestational age37 complete weeks); LBW group (235 women, birth weight2,500 g); and IUGR group (77 women who gave birth to infants with fetal growth retardation). Periodontitis was defined as the presence of at least four teeth with one or more sites with probing depthor =4 mm and clinical attachment lossor =3 mm. The effects of variables of interest, confounders, and interaction were tested through univariate and multivariate logistic regression tests.After adjusting for variables of interest, maternal periodontitis was retained in the final model for PTB (odds ratio [OR] = 1.77; 95% confidence interval [CI]: 1.12 to 2.59), LBW (OR = 1.67; 95% CI: 1.11 to 2.51), and IUGR (OR = 2.06; 95% CI: 1.07 to 4.19). When the interaction between periodontitis and previous PTB was tested, an OR of 5.94 (P0.001), 9.12 (P0.001), and 18.90 (P0.001) was observed for PTB, LBW, and IUGR, respectively.Maternal periodontitis is associated with an increased risk for PTB, LBW, and IUGR. Results emphasize the importance of periodontal care in prenatal health programs.

To evaluate and compare the possible morphological and histopathological changes in gingival tissue of rats under the administration of cyclosporine and tacrolimus. The present study was motivated by the high prevalence of gingival overgrowth observed in subjects under cyclosporine regimens and by studies reporting a significant decrease in gingival overgrowth after the substitution of tacrolimus.Five Sprague-Dawley rat groups were administered therapeutic and greater-than-therapeutic doses of cyclosporine and tacrolimus over 54 days. The control group of 10 animals received distilled water as a placebo. The cyclosporine group was divided into two subgroups of 10 animals each, one receiving 10 mg/kg/day (CsA1) and one receiving 30 mg/kg/day (CsA2). The tacrolimus group was also divided in two subgroups of 10 animals each, receiving 3.2 mg/kg/day (Tcr1) or 6.4 mg/kg/day (Tcr2).Gingival overgrowth was higher in the group that was administered the higher cyclosporine dosage (CsA2) than in the group that received the therapeutic dosage, showing a positive relation between dosage and severity of gingival overgrowth. Hypercellularity, vascular congestion and focal inflammatory exudation were observed in the CsA2 subgroup only. There were no morphological or histological alterations in gingival tissue in either tacrolimus subgroups.Cyclosporine can induce adverse morphological and histopathological changes in gingival tissue of rats, and these effects are dose-dependent. Tacrolimus induced no gingival alterations in this rat model.

No. of bitstreams: 0 Previous issue date: 2006 Objectives: The aim of this study was to verify the prevalence of peri-implant disease and analyse possible risk variables associated with peri-implant mucositis and peri-implantitis. The study group consisted of 212 partially edentulous subjects rehabilitated with osseointegrated implants. Material and Methods: The implants placed were examined clinically and radiographically to assess the peri-implant status. The degree of association between peri-implant disease and various independent variables was investigated using a multinomial regression analysis. Results: The prevalence of peri-implant mucositis and peri-implantitis were 64.6% and 8.9%, respectively. In univariate modelling, healthy peri-implant subjects presented lower plaque scores, less periodontal bleeding on probing, and less time elapsed since placement of supra-structures. In multivariate analyses, the risk variables associated with increased odds for having peri-implant disease included: gender, plaque scores, and periodontal bleeding on probing. Presence of periodontitis and diabetes were statistically associated with increased risk of peri-implantitis. The only two factors, which did not contribute to the presence of the disease, were the time elapsed since placement of supra-structures and the frequency of visits for maintenance care. Conclusion: Our data suggest that subjects with periodontitis, diabetes, and poor oral hygiene were more prone to develop peri-implantitis. Background: Interleukin-1 beta (IL-1β) is a potent inflammatory mediator and an important polymorphism in the locus +3954 (C/T) of the human IL1B gene has been shown to affect the levels of this cytokine. This functional polymorphism has been associated with the establishment of inflammatory diseases, including periodontal disease, in European, Asian and North American populations. Objective: The aim of this study was to investigate the association between the IL1B (+3954) gene polymorphism and the occurrence of different clinical forms of periodontitis in a sample of Brazilian individuals. Methods: This study employed a cross-sectional design involving individuals from the State of Minas Gerais in the south-eastern region of Brazil. Genomic DNA was obtained from oral swabs of 129 individuals and amplified using the polymerase chain reaction (PCR) with specific primers flanking the locus +3954 of IL1B. PCR products were submitted to restriction endonuclease digestion and analyzed by polyacrylamide gel electrophoresis, to distinguish alleles T and C of the IL1B gene, allowing for the determination of the genotypes and detection of the polymorphism. Results and Conclusions: The chronic periodontitis group displayed a higher percentage of the T allele (28%) when compared to the aggressive periodontitis group (10.7%, χ2 = 5.24, p = 0.02, OR = 0.31, CI = 0.11–0.88) and to control group (8.7%, χ2 = 7.11, p = 0.007, OR = 0.24, CI = 0.08–0.73). Our data suggested that the polymorphism in the locus +3954 of IL1B gene could be a risk factor for chronic periodontitis in a sample of Brazilian individuals.

A microbiota indígena gengival de cães não está totalmente descrita, sendo sua identificação uma etapa importante no estabelecimento da etiopatogenia e terapia da doença periodontal. O objetivo deste trabalho foi isolar e identificar a microbiota periodontal de cães da raça Pastor Alemão, considerando sítios saudáveis e com doença periodontal. Foram utilizados 29 cães, com idade variando de três a seis anos, sendo analisados espécimes clínicos de sítios periodontais saudáveis de 12 cães e sítios com periodontite de outros 17. Foram isoladas 672 amostras microbianas, com predomínio dos gêneros Pasteurella, Staphylococcus, Porphyromonas e Fusobacterium. A microbiota dos sítios saudáveis equiparou-se à dos sítios doentes, tratando-se de uma microbiota indígena. A microbiota dos sítios doentes apresentou-se aumentada em relação a dos sítios saudáveis, indicando mudança do ambiente do sítio periodontal. The indigenous gingival microbiota of dogs is not totally described, although such identification is an important step to establish the etiopathogenesis and adequate therapy for the periodontal disease. The aims of this study were to isolate and identify the periodontal microbiota of German Shepherd dogs from healthy and with periodontal desease sites. Twenty nine German Shepherd dogs from three to six years of age were used in this study. Clinical specimens were analysed from healthy periodontal sites of 12 dogs and sites presenting gingivitis of 17 dogs. A total amount of 672 microbial samples, were isolated where the predominant genera were Pasteurella spp., Staphylococcus spp., Porphyromonas spp. and Fusobacterium spp. The microbiological population of the affected sites was similar to the healthy sites, consisting on an indigenous microbiota. The microbiota on the affecteded sites was higher in number than on the healthy sites, showing change in the environment of the periodontal sites.

Background: Papillon-Lefevre syndrome (PLS) is a disorder that involves destruction of the periodontium and abnormal hyperkeratosis of the palms of the hands and soles of the feet. Mutations of the lysosomal protease cathepsin C gene (CTSC) have been associated with PLS. However, genotypic and phenotypic correlation has not been established. In the present study we investigated the CTSC gene in a Brazilian cohort affected by PLS. Methods: Eight consanguineous members of a kindred with PLS were studied. DNA was extracted and all exons of the gene amplified by the polymerase chain reaction (PCR) using specific primers. Mutations were identified by DNA sequencing of the coding region and introns of the CTSC gene. Results: Sequence analysis of CTSC from subjects affected by PLS identified a novel mutation (587T → C) in exon 4, predicted to cause a Leu196Pro amino acid substitution. Three of 3 subjects were homozygous for cathepsin C mutations inherited from a common ancestor. One patient was heterozygous and showed plantar hyperkeratosis without periodontal disease. Two other family members were also heterozygous but did not present palmoplantar hyperkeratosis and/or periodontal disease. Conclusions: This study describes a novel mutation of the cathepsin C gene in a Brazilian kindred with Papillon-Lefevre syndrome. J Periodontol 2002;73:307-312.

The ability of Actinobacillus actinomycetemcomitans to produce bacteriocin has rarely been reported. Antagonistic substance production may confer an important ecological advantage for the producer microorganisms, especially in a competitive ecosystem such as the oral cavity. In the present study, 75 A. actinomycetemcomitans strains isolated from the oral cavity of human patients with periodontal disease, periodontally healthy subjects and marmosets, as well as two reference strains (A. actinomycetemcomitans ATCC 29523 and FDC Y4) were evaluated for auto-, iso-, and heteroantagonistic activity. Fifty-one (68.00%) strains exhibited antagonistic activity; heteroantagonism was observed more often than isoantagonism. Isolated strains antagonized 17 different species of Gram-positive and Gram-negative bacteria from the oral and nonoral microbiota. Sensitivity to heat and to proteolytic enzymes constituted strong evidence that the antagonistic substance has a proteic nature. Taken together, our data enabled us to confirm that the antagonistic substance detected was a bacteriocin. The wide spectrum of activity indicates the possibility that more than one antagonistic substance is produced and that these substances play an important role in the ecological balance of the oral ecosystem.

Bacteriocin production has been studied in very few anaerobic bacteria, and no report is available for Fusobacterium species. In the present study a total of 167 Fusobacterium isolates were tested for bacteriocin production: 70 isolates were obtained from the oral cavity of patients with periodontal disease, 47 were recovered from healthy oral sites of human subjects and 50 from the oral cavity of Callithrix penicillata . Autoantagonism and isoantagonism were observed when the bacteriocin-producing isolates were tested against themselves. Heteroantagonism was detected by testing the Fusobacterium isolates against 14 reference strains and 2 strains of Actinobacillus actinomycetemcomitans from our laboratory collection. The auto-, iso- and hetero-antagonism phenomena observed in this comparative study suggest a possible ecological role for this (these) antagonistic substance(s) in the oral environment.
