A substantial proportion of acute stroke patients fail to recover following successful endovascular therapy (EVT) and injury to the brain and vasculature secondary to reperfusion may be a contributor. Acute stroke patients were included with: i) large vessel occlusion of the anterior circulation, ii) successful recanalization, and iii) evaluable MRI early after EVT. Presence of hyperemia on MRI perfusion was assessed by consensus using a modified ASPECTS. Three different approaches were used to quantify relative cerebral blood flow (rCBF). Sixty-seven patients with median age of 66 [59–76], 57% female, met inclusion criteria. Hyperemia was present in 35/67 (52%) patients early post-EVT, in 32/65 (49%) patients at 24 hours, and in 19/48 (40%) patients at 5 days. The Central portion of the Cordillera Oriental of Colombia is currently reported to harbor two species of Caecilia distributed at comparable elevations on opposite versants of these Mountains. These are C. corpulenta, known from Virolín, Santander, at 1700–2000 m on the western slopes of the Cordillera Oriental and C. degenerata, known from Garagoa, Boyacá, and Choachí and Fomequé, Cundinamarca, at 1800–2100 m on the eastern slopes of the Cordillera Oriental. Both species have dermal scales and lack secondary grooves, and have been subjected to misidentifications by herpetologists studying the Gymnophiona of the Eastern Andes of Colombia. Our results indicate that only the latter species is found in Colombia and the former is restricted to Peru, leaving those populations previously referred to C. corpulenta and those distinct from C. degenerata pending names. We here present an account for C. degenerata based on material from Choachí and Fómeque, Cundinamarca, as well as descriptions of three new species from the Cordillera Oriental and adjacent Venezuela: C. atelolepis sp. nov., C. epicrionopsoides sp. nov., and C. macrodonta sp. nov. We also provide additional morphological information for the recently described C. pulchraserrana.

The aim of the study was to determine whether tibiofemoral contact stress predicts risk for worsening knee pain over 84 ms in adults aged 50-79 yrs with or at elevated risk for knee osteoarthritis.Baseline tibiofemoral contact stress was estimated using discrete element analysis. Other baseline measures included weight, height, hip-knee-ankle alignment, Kellgren-Lawrence grade, and Western Ontario and McMaster Universities Osteoarthritis Index pain subscale. Logistic regression models assessed the association between baseline contact stress and 84-mo worsening of Western Ontario and McMaster Universities Osteoarthritis Index pain subscale.Data from the dominant knee (72.6% Kellgren-Lawrence grade 0/1 and 27.4% Kellgren-Lawrence grade ≥ 2) of 208 participants (64.4% female, mean ± SD body mass index = 29.6 ± 5.1 kg/m 2 ) were analyzed. Baseline mean and peak contact stress were 3.3 ± 0.9 and 9.4 ± 4.3 MPa, respectively. Forty-seven knees met the criterion for worsening pain. The highest tertiles in comparison with the lowest tertiles of mean (odds ratio [95% confidence interval] = 2.47 [1.03-5.95], P = 0.04) and peak (2.49 [1.03-5.98], P = 0.04) contact stress were associated with worsening pain at 84 mos, after adjustment for age, sex, race, clinic site, and baseline pain. Post hoc sensitivity analyses including adjustment for body mass index and hip-knee-ankle alignment attenuated the effect.These findings suggest that elevated tibiofemoral contact stress can predict the development of worsening of knee pain.

We here provide the first Colombian records of Oscaecilia ochrocephala since 1942 based on material from Darién and Urabá Antioqueño. We discuss certain aspects of intraspecific variation of this species and denote its great morphological similarity to O. polyzona, which we consider as its potential sister species. Lastly we discuss the usefulness of some supposedly diagnostic characters previously found in the literature —that we consider to be of limited use— and instead propose that coloration and squamation be used in order to differentiate these two species.

To determine the optimal combination of imaging and biochemical biomarkers for use in the prediction of knee osteoarthritis (OA) progression.The present study was a nested case-control trial from the Foundation of the National Institutes of Health OA Biomarkers Consortium that assessed study participants with a Kellgren/Lawrence grade of 1-3 who had complete biomarker data available (n = 539 to 550). Cases were participants' knees that had radiographic and pain progression between 24 and 48 months compared to baseline. Radiographic progression only was assessed in secondary analyses. Biomarkers (baseline and 24-month changes) that had a P value of0.10 in univariate analysis were selected, including quantitative cartilage thickness and volume on magnetic resonance imaging (MRI), semiquantitative MRI markers, bone shape and area, quantitative meniscal volume, radiographic progression (trabecular bone texture [TBT]), and serum and/or urine biochemical markers. Multivariable logistic regression models were built using 3 different stepwise selection methods (complex models versus parsimonious models).Among baseline biomarkers, the number of locations affected by osteophytes (semiquantitative), quantitative central medial femoral and central lateral femoral cartilage thickness, patellar bone shape, and semiquantitative Hoffa-synovitis predicted OA progression in most models (C statistic 0.641-0.671). In most models, 24-month changes in semiquantitative MRI markers (effusion-synovitis, meniscal morphologic changes, and cartilage damage), quantitative central medial femoral cartilage thickness, quantitative medial tibial cartilage volume, quantitative lateral patellofemoral bone area, horizontal TBT (intercept term), and urine N-telopeptide of type I collagen predicted OA progression (C statistic 0.680-0.724). A different combination of imaging and biochemical biomarkers (baseline and 24-month change) predicted radiographic progression only, which had a higher C statistic of 0.716-0.832.The present study highlights the combination of biomarkers with potential prognostic utility in OA disease-modifying trials. Properly qualified, these biomarkers could be used to enrich future trials with participants likely to experience progression of knee OA.

Nonspecific respiratory symptoms overlap with coronavirus disease 2019 (COVID-19). Prompt diagnosis of COVID-19 in hospital employees is crucial to prevent nosocomial transmission. Rapid molecular SARS-CoV-2 testing was performed for 115 symptomatic employees. The case positivity rate was 2.6%. Employees with negative tests returned to work after 80 (±28) minutes.

Introduction: Cerebral vasculopathies (stenoses and aneurysms) are a major cause of stroke and disability in adults and children with sickle cell disease (SCD). HSCT has been shown to prevent or improve large vessel intracranial stenosis in children when compared to standard care (Verlhac 2020). We sought to determine the impact of HSCT on the development and progression of cerebral vasculopathies in adults with SCD. Methods: The study population included patients with SCD (HbSS, HbSC, and HbS/beta-thalassemia) that were enrolled in NIH studies (14-H-0077, 09-H-0225, 03-H-0170) and underwent HSCT from 2004-19 and received pre- and post-transplant MRI/MRA studies. MRA images were reviewed by two independent readers and scored for stenoses in eight intracranial arteries, Moyamoya collateralization, and presence and progression of intracranial aneurysms. Changes in vasculopathy were compared before and after transplantation. Results: Eighty-seven patients were included in the study, mean age 32.2 yrs. and 58% male. Median follow-up period was 3.3 yrs. (range: 0.5-17 yrs.), and 28% had evidence of vasculopathy at transplant; 17% with stenotic vasculopathy and 13% with aneurysm(s). None of the patients without vasculopathy developed stenoses or aneurysms after HSCT. MRA stenoses scores improved after HSCT in 62% of patients with vasculopathy, with a median change of 1.0 (range: 1-6; p=0.01). All intracranial aneurysms identified at transplant remained unchanged in follow-up. Conclusions: Hematopoietic stem cell transplantation improved or prevented the development of vasculopathy in adults with sickle cell disease.

Introduction: Perihematomal edema is a consequence of primary intracerebral hemorrhage. On diffusion MRI, a corresponding heterogenous pattern of hypo- and hyper-intensity is visible on apparent diffusion coefficient (ADC), suggesting both cytotoxic and vasogenic edema. Analogously, endovascular therapy (EVT) for ischemic stroke can result in hemorrhagic transformation (HT) and a concomitant ring-like pattern, or “Rings” of mixed intensity on ADC. The objective of this study is to describe this pattern of secondary injury early after EVT. Methods: Patients were included if they consented to the prospective Natural History of Stroke GUARDS study from April 2018 to February 2022, were treated with EVT for anterior circulation LVO, achieved TICI 2b/3 reperfusion, and had early MRI, ~2h post-EVT. Two independent readers measured early and 24h DWI lesion volumes and 5d FLAIR lesion volumes using a semi-automated validated approach. Presence of any HT on GRE and Rings on ADC (Figure) on early, 24h, and 5d MRI were read independently by multiple readers who then reached consensus. The relationship of Rings with HT and lesion volume was explored. Results: Eighty-nine patients were included with median age 64y, 55% female, median admit NIHSS 18, 64% M1 LVO, and 47% IV tPA. On early MRI, 14 (16%) patients had Rings, 12 (86%) of whom also had HT (p Conclusions: Rings on ADC was seen in half of patients 24h post-EVT and appears to reflect peri-lesion edema associated with hemorrhagic transformation. This imaging marker was associated with larger lesion volumes at 5 days and may be able to serve as a target for adjunctive therapy post-EVT to mitigate edema formation.

Introduction: Symptomatic and silent cerebral infarcts (SCIs) are common in children and adults with sickle cell disease (SCD). SCIs are associated with cognitive performance and risk of future symptomatic stroke. The incidence of overt stroke and SCI post HSCT is low in children, but there is limited information in adults. We sought to determine the impact of HSCT on the rate of new symptomatic and silent cerebral infarcts in adults with SCD. Methods: The study population included patients with SCD (HbSS, HbSC, and HbS/beta-thalassemia) that were enrolled in NIH studies (14-H-0077, 09-H-0225) and underwent HSCT from 2004-19 and received pre- and post-transplant MRI studies. MRI FLAIR, T1, and T2 images were reviewed by two independent readers and scored for presence of prior ischemic stroke and silent cerebral infarct (≥ 3mm on T2 or FLAIR images in 2 imaging planes). Changes in SCIs and ischemic strokes were compared before and after transplantation. Results: Eighty-four patients were included in the study, mean age 32.3 yrs. (range 10-65 yrs.) and 57% male. Median follow-up period was 3.3 yrs. (range: 6 months -17 yrs.) for a total of 412 patient-years. Overall, 60% had evidence of SCI and or prior ischemic stroke at transplant; 54% (45/84) with SCI and 23% with imaging evidence of a prior stroke. During the follow-up period, 13% (11/84) patients developed new ischemic injury, two patients developed a new stroke and nine patients developed new SCIs. The post-HSCT incidence rate of new stroke was 0.49 events/100 patient-years, and SCIs was 2.2 events/100 patient-years. Patients who progressed compared to those who did not were older at transplant (36.1 yrs. vs. 31.2 yrs.; p=0.09), had a prior history of stroke (27% vs. 22%, p=0.71), had a prior history of SCI (64% vs. 52%, p=0.47), and a prior history of stenotic vasculopathy (18% vs. 14%, p= 0.65). Conclusions: The incidence of new ischemic events after HSCT for SCD in adults is low. Our rates are comparable to incidence rates reported in children with SCD after HSCT. While rates of progression were higher in patients with prior stroke, SCI, and vasculopathy, we did not find a significant difference between groups.

Three different approaches were used to quantify relative cerebral blood flow (rCBF) on early post-EVT, 24 hours, and 5 days. Two independent trained readers measured lesion volumes using a semi-automated validated approach. Results: Sixty-seven patients with median age of 66 [59-76], 57% female, met inclusion criteria. Hyperemia was present in 35/67 (52%) patients early post-EVT (Figure), in 32/65 (49%) patients at 24 hours, and in 19/48 (40%) patients at 5 days. In comparison to control regions, rCBF was elevated by greater than 50% in hyperemic regions. Patients with hyperemia had larger ischemic lesion volumes and early lesion growth. A strong association (R2=0.81, p Conclusions: Hyperemia is frequently observed after successful EVT and is associated with lesion growth at 24 hours. Evidence of hyperemia may identify patients that may benefit from adjunctive therapy to prevent reperfusion injury, edema, and lesion growth.

Introduction: Lesion volume on MRI or CT provides evidence of tissue response to successful recanalization by endovascular therapy. The most appropriate time to estimate “infarct” volume remains unclear. Largely for convenience, 24-48 hours has been used in clinical trials and registry datasets which may be too early to determine the final infarct volume. The objective of this study was to quantify the comparability of lesion volume using MRI measured at 24 hours versus 5 days in acute stroke patients following successful recanalization. Methods: Patients were included if they consented to the prospective GUARDS study between April 2018 through February 2022, had LVO of the anterior circulation, were treated with EVT, achieved complete revascularization defined as TICI 2b/3, and imaged with MRI at 2 hours, 24 hours, and 5 days post EVT. Two independent readers measured lesion volumes using a semi-automated validated approach on DWI at 2 and 24 hours, and FLAIR at 5 days. Statistical comparisons of lesion volumes across time points were performed. Lesion growth was defined as change in lesion volume and percent change compared to lesion volume at 2 hours. Results: Sixty-nine patients met all study criteria and were included. Median age was 66 years, 55% female, median admit NIHSS of 18, 62% with M1 LVO, and 46% treated with IV thrombolysis. Post-EVT median lesion volumes were 24mL, 35mL, and 45mL respectively for 2-hour DWI, 24-hour DWI, and 5-day FLAIR. Lesion growth based on median percent change from the 2-hour DWI was 39% [21-96] and 52% [12-145] at 24 hours and 5 days. The difference in lesion growth based on percent change from 24 hours to 5 days was not significant (p=0.17). However, there was a significant difference based on change in lesion volume, 12mL [2-25] at 24 hours and 16mL at 5 days (p=0.029). Fifty-one (74%) patients had DWI volumes at 24 hours that were ≥80% of their FLAIR volumes at 5 days. However, 18 (26%) patients continued to have lesion growth beyond 24 hours, median lesion growth of 18mL,with 11 (61%) of those having lesion growth >20mL. Conclusions: Lesion growth >20mL continues in some patients beyond 24 hours, suggesting that infarct volume at 5 days should be used as the imaging outcome when assessing tissue response to recanalization.
