Background: Oral activated factor XI (FXIa) inhibitors may modulate coagulation to prevent thromboembolic events without substantially increasing bleeding. We explored the pharmacodynamics, safety, and efficacy of the oral FXIa inhibitor asundexian for secondary prevention after acute myocardial infarction (MI). Methods: We randomized 1601 patients with recent acute MI to oral asundexian 10, 20, or 50 mg or placebo once daily for 6 to 12 months in a double-blind, placebo-controlled, phase 2, dose-ranging trial. Patients were randomized within 5 days of their qualifying MI and received dual antiplatelet therapy with aspirin plus a P2Y12 inhibitor. The effect of asundexian on FXIa inhibition was assessed at 4 weeks. The prespecified main safety outcome was Bleeding Academic Research Consortium type 2, 3, or 5 bleeding comparing all pooled asundexian doses with placebo. The prespecified efficacy outcome was a composite of cardiovascular death, MI, stroke, or stent thrombosis comparing pooled asundexian 20 and 50 mg doses with placebo. Results: The median age was 68 years, 23% of participants were women, 51% had ST-segment–elevation MI, 80% were treated with aspirin plus ticagrelor or prasugrel, and 99% underwent percutaneous coronary intervention before randomization. Asundexian caused dose-related inhibition of FXIa activity, with 50 mg resulting in >90% inhibition. Over a median follow-up of 368 days, the main safety outcome occurred in 30 (7.6%), 32 (8.1%), 42 (10.5%), and 36 (9.0%) patients receiving asundexian 10 mg, 20 mg, or 50 mg, or placebo, respectively (pooled asundexian versus placebo: hazard ratio, 0.98 [90% CI, 0.71–1.35]). The efficacy outcome occurred in 27 (6.8%), 24 (6.0%), 22 (5.5%), and 22 (5.5%) patients assigned asundexian 10 mg, 20 mg, or 50 mg, or placebo, respectively (pooled asundexian 20 and 50 mg versus placebo: hazard ratio, 1.05 [90% CI, 0.69–1.61]). Conclusions: In patients with recent acute MI, 3 doses of asundexian, when added to aspirin plus a P2Y12 inhibitor, resulted in dose-dependent, near-complete inhibition of FXIa activity without a significant increase in bleeding and a low rate of ischemic events. These data support the investigation of asundexian at a dose of 50 mg daily in an adequately powered clinical trial of patients who experienced acute MI. Background: Many postmortem studies address the cardiovascular effects of COVID-19 and provide valuable information, but are limited by their small sample size. Objectives: The aim of this systematic review is to better understand the various aspects of the cardio-vascular complications of COVID-19 by pooling data from a large number of autopsy studies. Data sources: We searched the online databases Ovid EBM Reviews, Ovid Embase, Ovid Medline, Scopus, and Web of Science for concepts of autopsy or histopathology combined with COVID-19, published be-tween database inception and February 2021. We also searched for unpublished manuscripts using the medRxiv services operated by Cold Spring Harbor Laboratory. Study eligibility criteria: Articles were considered eligible for inclusion if they reported human post-mortem cardiovascular findings among individuals with a confirmed SARS coronavirus type 2 (CoV-2) infection. Participants: Confirmed COVID-19 patients with post-mortem cardiovascular findings. Interventions: None. Methods: Studies were individually assessed for risk of selection, detection, and reporting biases. The median prevalence of different autopsy findings with associated interquartile ranges (IQRs). Results: This review cohort contained 50 studies including 548 hearts. The median age of the deceased was 69 years. The most prevalent acute cardiovascular findings were myocardial necrosis (median: 100.0%; IQR, 20%-10 0%; number of studies = 9; number of patients = 64) and myocardial oedema (median: 55.5%; IQR, 19.5%-92.5%; number of studies = 4; number of patients = 46). The median re-ported prevalence of extensive, focal active, and multifocal myocarditis were all 0.0%. The most prevalent chronic changes were myocyte hypertrophy (median: 69.0%; IQR, 46.8%-92.1%) and fibrosis (median: 35.0%; IQR, 35.0%-90.5%). SARS-CoV-2 was detected in the myocardium with median prevalence of 60.8% (IQR 40.4-95.6%). Conclusions: Our systematic review confirmed the high prevalence of acute and chronic cardiac pathologies in COVID-19 and SARS-CoV-2 cardiac tropism, as well as the low prevalence of myocarditis in COVID-19. Radiofrequency denervation has been established for many years as an important minimally invasive procedure for the treatment of chronic pain conditions. Positive experiences of many users for various indications are contrasted by a nonuniform evidence. With meticulous patient selection and correct assessment of the indications a longer term reduction of pain, a reduced need for analgesics and an improvement in the quality of life can be achieved. The aim of this interdisciplinary position paper is to present the value of radiofrequency denervation in the treatment of chronic pain. The summarized recommendations of the expert group are based on the available evidence and on the clinical experiences of Austrian centers that frequently implement the procedure. The position paper contains recommendations on patient selection and proven indications. We discribe safety aspects, complications, side effects and contraindications.

BACKGROUND Direct-acting oral anticoagulant use for stroke prevention in atrial fibrillation is limited by bleeding concerns. Asundexian, a novel, oral small molecule activated coagulation factor XIa (FXIa) inhibitor, might reduce thrombosis with minimal effect on haemostasis. We aimed to determine the optimal dose of asundexian and to compare the incidence of bleeding with that of apixaban in patients with atrial fibrillation. METHODS In this randomised, double-blind, phase 2 dose-finding study, we compared asundexian 20 mg or 50 mg once daily with apixaban 5 mg twice daily in patients aged 45 years or older with atrial fibrillation, a CHA2DS2-VASc score of at least 2 if male or at least 3 if female, and increased bleeding risk. The study was conducted at 93 sites in 14 countries, including 12 European countries, Canada, and Japan. Participants were randomly assigned (1:1:1) to a treatment group using an interactive web response system, with randomisation stratified by whether patients were receiving a direct-acting oral anticoagulant before the study start. Masking was achieved using a double-dummy design, with participants receiving both the assigned treatment and a placebo that resembled the non-assigned treatment. The primary endpoint was the composite of major or clinically relevant non-major bleeding according to International Society on Thrombosis and Haemostasis criteria, assessed in all patients who took at least one dose of study medication. This trial is registered with ClinicalTrials.gov, NCT04218266, and EudraCT, 2019-002365-35. FINDINGS Between Jan 30, 2020, and June 21, 2021, 862 patients were enrolled. 755 patients were randomly assigned to treatment. Two patients (assigned to asundexian 20 mg) never took any study medication, resulting in 753 patients being included in the analysis (249 received asundexian 20 mg, 254 received asundexian 50 g, and 250 received apixaban). The mean age of participants was 73·7 years (SD 8·3), 309 (41%) were women, 216 (29%) had chronic kidney disease, and mean CHA2DS2-VASc score was 3·9 (1·3). Asundexian 20 mg resulted in 81% inhibition of FXIa activity at trough concentrations and 90% inhibition at peak concentrations; asundexian 50 mg resulted in 92% inhibition at trough concentrations and 94% inhibition at peak concentrations. Ratios of incidence proportions for the primary endpoint were 0·50 (90% CI 0·14-1·68) for asundexian 20 mg (three events), 0·16 (0·01-0·99) for asundexian 50 mg (one event), and 0·33 (0·09-0·97) for pooled asundexian (four events) versus apixaban (six events). The rate of any adverse event occurring was similar in the three treatment groups: 118 (47%) with asundexian 20 mg, 120 (47%) with asundexian 50 mg, and 122 (49%) with apixaban. INTERPRETATION The FXIa inhibitor asundexian at doses of 20 mg and 50 mg once daily resulted in lower rates of bleeding compared with standard dosing of apixaban, with near-complete in-vivo FXIa inhibition, in patients with atrial fibrillation. The present population-based cohort study investigated long-term mortality after surgical aortic valve replacement (AVR) with bioprosthetic (B) or mechanical aortic valve prostheses (M) in a European social welfare state.We analysed patient data from health insurance records covering 98% of the Austrian population between 2010 and 2018. Subsequent patient-level record linkage with national health data provided patient characteristics and clinical outcomes. Further reoperation, myocardial infarction, heart failure and stroke were evaluated as secondary outcomes.A total of 13,993 patients were analysed and the following age groups were examined separately:50 years (727 patients: 57.77% M, 42.23% B), 50-65 years (2612 patients: 26.88% M, 73.12% B) and65 years (10,654 patients: 1.26% M, 98.74% B). Multivariable Cox regression revealed that the use of B-AVR was significantly associated with higher mortality in patients aged 50-65 years compared to M-AVR (HR = 1.676 [1.289-2.181], p 0.001). B-AVR also performed worse in a competing risk analysis regarding reoperation (HR = 3.483 [1.445-8.396], p = 0.005) and myocardial infarction (HR = 2.868 [1.255-6.555], p = 0.012). However, the risk of developing heart failure and stroke did not differ significantly after AVR in any age group.Patients aged 50-65 years who underwent M-AVR had better long-term survival, and a lower risk of reoperation and myocardial infarction. Even though anticoagulation is crucial in patients with M-AVR, we did not observe significantly increased stroke rates in patients with M-AVR. This evident survival benefit in recipients of mechanical aortic valve prostheses aged65 years critically questions current guideline recommendations.

BackgroundIn patients receiving high flow nasal oxygen (HFNO), predicting chance of response is crucial in order to avoid unnecessary delay of intubation. Whether commonly used predictors of non-invasive ventilation (NIV) response may be as applicable for HFNO, is unclear.MethodsWe conducted a retrospective bi-centric analysis of adults treated with HFNO in two Austrian medical intensive care units from 01/2014 until 09/2017. Predictive value of respiratory rate, PaO2, PaCO2, SpO2, ROX index, pH, heart rate, base excess, SOFA-Score and SAPS III was analysed by means of logistic regression with HFNO responding as dependent variable. Calculations were also done separately in patients receiving HFNO as post-extubation support (Group A) and all other indications (Group B). ResultsWe registered 127 patients (m:f = 70:57, median age: 67 [IQR 53-77] years). Forty-eight patients (37.8%) received HFNO as post-extubation support and were assigned to group A. Seventy-nine patients (62.2%) received HFNO due to any other indication and were assigned to group B. Criteria of HFNO response were fulfilled by 42 patients (87.5%) of group A and 53 (67.1%) of group B patients.Whereas respiratory rate, PaO2, PaCO2, SpO2, pH, heart rate, base excess, SOFA-Score and SAPS III did not differ between responders and non-responders, ROX index showed significant association with HFNO success in all patients. With increasing ROX index, the odds for response increased (OR [95% CI]: baseline: 1.21 [1.05, 1.38], p = 0.0069, after 2 hours: 1.19 [1.05; 1.34], p = 0.0061, after 6 hours: 1.23 [1.06; 1.42], p = 0.0049). Subgroup analyses revealed similar odds ratios for ROX index (baseline: group A: 1.17 [0.90; 1.52], p = 0.24; group B: 1.22 [1.04; 1.43], p = 0.015; after 2 hours: 1.30 [0.94; 1.82], p = 0.12, group B: 1.18 [1.03; 1.34], p = 0.014; after 6 hours: 1.76 [1.01; 3.06], p = 0.047, group B: 1.19 [1.03; 1.37], p = 0.022). ConclusionsCriteria of HFNO success deviate from criteria for response to NIV. ROX index indicated HFNO response in a wide range of indications.

Three recently published sham-controlled studies proved the efficacy of renal denervation (RDN) in hypertensive patients. The study presented here analyzed a nationwide multicentre registry database to clarify which patient subgroups benefit most from radiofrequency RDN.This is a post hoc analysis from the multicentre Austrian Transcatheter Renal Denervation Registry hosted by the Austrian Society of Hypertension. We correlated change of SBP after RDN to sex and presence/absence of comorbidities. Univariable correlation and multiple linear regression analyses were performed.Two hundred and ninety-one patients (43% women, median age 64 years) undergoing RDN between April 2011 and September 2014 were included in this analysis. Mean baseline ambulatory 24 h BP (systolic/diastolic) was 150 ± 18/89 ± 14 mmHg and mean baseline office BP was 170 ± 16/94 ± 14 mmHg.After RDN, mean ambulatory 24 h BP reduction was 9 ± 19/6 ± 16 mmHg. The following features were associated with a good response to RDN: high baseline systolic ambulatory BP, high baseline diastolic office BP, female sex, absence of diabetes mellitus, and absence of peripheral artery disease. Multivariable analysis identified female sex and absence of diabetes mellitus as strongest predictors for ambulatory BP reduction, although those groups had the lowest baseline ambulatory BP.Ambulatory BP reductions after RDN were substantially more pronounced in female and in nondiabetic patients despite lower baseline BP. It is concluded that in terms of efficacy female patients and nondiabetic patients might benefit more from RDN.

During hip replacement, acetabular and femur bone preparation may cause bone marrow extravasation. Paradoxical fat embolism occurs when emboli pass from the pulmonary to the systemic circulation by either a patent foramen ovale or pulmonary capillaries. Impaired consciousness in the immediate postoperative period of orthopedic surgery frequently results from hypovolemia, anemia, or residual anesthesia. A high index of suspicion is required to consider the possibility of paradoxical fat embolism.

Bereits auf einer Intensivstation selbst gibt es viele apparative diagnostische Basismoglichkeiten, wie zum Beispiel Blutgasanalyse, Blutdruckuberwachung, Sonografien, hamodynamische Messungen, EKG und Rontgen, die sofort vor Ort durchgefuhrt werden konnen und bereits eine gute initiale Therapiesteuerung ermoglichen. Fur eine erweiterte Diagnostik muss jedoch unter entsprechenden Vorsichtsmasnahmen die Intensivstation verlassen werden. Dies sollte nur unter Abwagung von Risiko und Nutzen erfolgen.

Die Beurteilung des Volumenstatus und der Kreislaufsituation ist eine absolute Basismasnahme bei allen Intensivpatienten und kann nicht anhand eines einzelnen Parameters oder mit nur einem Ablesen von Zahlen und Kurven eines Gerates allein erfolgen, hierfur ist immer die Zusammenschau mehrerer Befunde notwendig. Anschliesend an die Feststellung einer unzureichenden Zirkulation und Sauerstoffversorgung der Gewebe sowie der Reagibilitat auf eine Volumenzufuhr mussen die Moglichkeiten zur medikamentosen Kreislaufunterstutzung evaluiert und angepasst eingesetzt werden.

Das Management der Atemwege beginnt nicht erst bei der endotrachealen Intubation, sondern schon wesentlich fruher mit Basismasnahmen. Im folgenden Teil werden die verschiedenen Moglichkeiten mit Indikation, Kontraindikation, Durchfuhrung und Problemen bzw. deren Losungen vorgestellt. Hierbei werden nicht nur die verwendeten Hilfsmittel, sondern auch einige wichtige Griffe zur Befreiung der Atemwege oder Offnung des Mundes besprochen.

Allergische Reaktionen kommen laut Literatur zunehmend haufiger vor. Eine Einteilung nach Art und Geschwindigkeit der Reaktion, aber auch in Stadien je nach Schwere der Symptome ist ublich. Sehr schwere Verlaufsformen kommen im Vergleich zu milden Reaktionen sehr selten vor. Zu den bekanntesten, meist schweren Verlaufen sind die Reaktionen nach Wespenstichen, nach dem Genuss von Meeresfruchten, aber auch nach Einnahme von Medikamenten wie Penicillin und nach dem Kontakt zu anderen chemischen Stoffen. Wegen des schnellen Fortschreitens bei schweren Verlaufen sollten diese Patienten prophylaktisch ein Adrenalin-Injektionsset mitfuhren.

Ein ARDS („acute respiratory distress syndrome“) ist eine gefurchtet Komplikation einer Entzundung in der Lunge. Diese kann nicht nur durch eine bakterielle Pneumonie, sondern auch durch Viren (wie SARS-COV-2), Pilze, Inhalationsschaden, Traumata und andere Ursachen ausgelost werden. Die Diagnose wird streng anhand von Kriterien gestellt, die Therapie muss immer auf einer Intensivstation mit Intubationsbereitschaft begonnen werden. Haufig ist eine Intubation mit nachfolgend schwieriger Beatmungssituation die Folge.

Die Lumbalpunktion ist die haufigste durchgefuhrte Intervention zur Gewinnung von Liquor cerebrospinalis im Erwachsenenalter. Als alternative Punktionsstelle gibt es die subokzipitale Punktion der Cisterna cerebellomedularis in der hinteren Schadelgrube. Die Liquorgewinnung dient der Abklarung vor allem bei Verdacht auf einen malignen Prozess oder eine Entzundung im Bereich des Zentralnervensystems. Gerade aufgrund entzundlicher Erkrankungen, wie einer bakteriellen Meningitis, mit akutem Handlungsbedarf sollte diese Intervention von jedem Intensivmediziner beherrscht werden.

Die korrekte Beurteilung einer Blutgasanalyse und der Parameter des Saure-Basen-Haushalts ist eine tagliche Routine auf einer Intensivstation und fur viele Therapieanpassungen essentiell. Dies ist bei der Beatmung und andererseits auch fur eine Korrektur der Storungen im Saure-Basen-Haushalt und bei Entgleisungen des Stoffwechsels von Bedeutung. Die Beurteilung muss, genauso wie jene eines EKG, EEG, einer Spirometrie und vieles mehr, immer Schritt fur Schritt erfolgen, so wird niemand durch einen sehr auffalligen Wert von anderen wichtigen Informationen abgelenkt.

Bei der Arbeit auf einer Intensivstation gibt es immer wieder Situationen, in denen alle Masnahmen nicht mehr helfen konnen und ein Patient sicher versterben wird. In diesen Momenten sollte neben der Enttauschung uber den gesichert negativen Ausgang der Behandlung auch immer daran gedacht werden, dass der im Sterben liegende Mensch eventuell noch anderen Menschen das Leben verlangern bzw. retten kann. Es sollte immer an die Moglichkeit einer Organentnahme zur Transplantation gedacht und mit einem Transplantationszentrum in moglichen Fallen, sofern es keinen gesetzlichen Hinderungsgrund gibt, rechtzeitig Kontakt aufgenommen werden. Vor einer Organentnahme mussen zwingend der Hirntod festgestellt und Masnahmen zur Erhaltung der Korperfunktion bis zur Organentnahme gewahrleistet werden.

Eine Atemunterstutzung ist auf einer Intensivstation sehr haufig notwendig. Es gibt viele Moglichkeiten, bei einer Hypoxie, einer Hyperkapnie oder einer insuffizienten Atmung auf einer Intensivstation Unterstutzung anzubieten. Die Intubation sollte immer nur die letzte Masnahme darstellen, wenn keine andere sinnvolle Option mehr besteht. Eine differenzierte Auswahl des optimalen Verfahrens ist fur den Verlauf und das Outcome des Patienten entscheidend. Seit mehreren Jahren ist die Anzahl der endotrachealen Intubationen dank nichtinvasiver Verfahren rucklaufig, die Verweildauer auf einer Intensivstation kurzer und die Mortalitat vor allem durch eine nichtinvasive Ventilation fallend.

Analgesie, Sedierung und Narkosefuhrung gehoren zu den wichtigsten Basismasnahmen der Intensivmedizin. Ohne diese Verfahren mit Anwendung entsprechender Medikamente waren viele Behandlungen und Interventionen sowie diagnostische Masnahmen in der Medizin unzumutbar oder schlicht unmoglich. Analgetika, Sedativa und Anwendungsstrategien fur viele Situationen werden stets weiterentwickelt. Dies geschieht vor allem, um den modernen Anspruchen an eine moglichst schmerzfreie und wenig traumatisierende Therapie und Diagnostik gerecht zu werden und auch auf diese Art und Weise die Arbeit der medizinischen Berufe zu erleichtern.

Das Arbeiten auf einer Intensivstation unterscheidet sich nicht nur durch die viel hohere Verantwortung und den meist auch hoheren Zeitdruck bei der Arbeit, sondern auch durch die vielen zusatzlichen Moglichkeiten fur eine Uberwachung und sofortige Intervention. Diese Optionen mussen aber verantwortungsvoll eingesetzt und gesteuert werden, denn nur, weil etwas prinzipiell moglich ist, heist es noch lange nicht, dass der Einsatz auch (noch) sinnvoll ist. Des Weiteren muss mit dem Stress im Team und den Sorgen der Patienten und Angehorigen richtig umgegangen werden konnen.

Die COPD ist nicht nur, aber vor allem wegen des steigenden Nikotinkonsums, als auch der Umweltbelastung insbesondere in grosen Stadten und Regionen mit dichter Industrieansiedelung ein zunehmendes medizinisches Problem. Trotz aller Abschreckungsmasnahmen und Aufklarungen nimmt die COPD aktuell weiterhin zu. In fortgeschrittenen Stadien treten haufiger sehr schwere Exazerbationen mit Notwendigkeit zur Aufnahme auf der Intensivstation und Atemunterstutzung bzw. immer wieder auch invasiver Beatmung auf.

Die Punktion eines Gefases gehort vor allem auf einer Intensivstation, aber auch auserhalb, wie bei Punktionen fur die Koronarangiografie oder fur andere Gefasdarstellungen mit Kontrastmittel sowie bei Gefasinterventionen, zu einer der am haufigsten benotigten Fertigkeiten in der Medizin. Auf einer Intensivstation wird sie vor allem fur die Anlage zentralvenoser Katheter, passagerer Dialysekatheter und fur zentrale, seltener periphere arterielle Zugange verwendet.

Eine kardiale Dekompensation kann akut durch plotzlich einsetzende Herzinsuffizienz, wie bei Myokardinfarkt, grose Klappendefekten, Pulmonalembolie etc. auftreten und so zu einer plotzlichen kardialen Dekompensation fuhren. Bei bekannter Herzinsuffizienz tritt eine kardiale Dekompensation hingegen meistens langsamer auf. Das Lungenodem kann als Folge einer Herzinsuffizienz auftreten, es wird dann als kardiogenes Lungenodem, alle anderen Formen hingegen als nicht kardiogene Lungenodeme bezeichnet.
