PURPOSE: To describe the design and methodological approach of a multicenter, retrospective study to externally validate a clinical and imaging-based model for predicting the risk of late adverse events in patients with initially uncomplicated type B aortic dissection (uTBAD).MATERIALS AND METHODS: The Registry of Aortic Diseases to Model Adverse Events and Progression (ROADMAP) is a collaboration between 10 academic aortic centers in North America and Europe. Two centers have previously developed and internally validated a recently developed risk prediction model. Clinical and imaging data from eight ROADMAP centers will be used for external validation. Patients with uTBAD who survived the initial hospitalization between January 1, 2001, and December 31, 2013, with follow-up until 2020, will be retrospectively identified. Clinical and imaging data from the index hospitalization and all follow-up encounters will be collected at each center and transferred to the coordinating center for analysis. Baseline and follow-up CT scans will be evaluated by cardiovascular imaging experts using a standardized technique.RESULTS: The primary end point is the occurrence of late adverse events, defined as aneurysm formation (≥6 cm), rapid expansion of the aorta (≥1 cm/y), fatal or nonfatal aortic rupture, new refractory pain, uncontrollable hypertension, and organ or limb malperfusion. Objectives The authors explored the development and validation of machine-learning models for augmenting the echocardiographic grading of aortic stenosis (AS) severity. Background In AS, symptoms and adverse events develop secondarily to valvular obstruction and left ventricular decompensation. The current echocardiographic grading of AS severity focuses on the valve and is limited by diagnostic uncertainty. Methods Using echocardiography (ECHO) measurements (ECHO cohort, n = 1,052), we performed patient similarity analysis to derive high-severity and low-severity phenogroups of AS. We subsequently developed a supervised machine-learning classifier and validated its performance with independent markers of disease severity obtained using computed tomography (CT) (CT cohort, n = 752) and cardiovascular magnetic resonance (CMR) imaging (CMR cohort, n = 160). The classifier’s prognostic value was further validated using clinical outcomes (aortic valve replacement [AVR] and death) observed in the ECHO and CMR cohorts. Results In 1,964 patients from the 3 multi-institutional cohorts, 1,346 (68%) subjects had either nonsevere or discordant AS severity. Machine learning identified 1,117 (57%) patients as having high-severity and 847 (43%) as having low-severity AS. High-severity patients in CT and CMR cohorts had higher valve calcium scores and left ventricular mass and fibrosis, respectively than the low-severity group. In the ECHO cohort, progression to AVR and progression to death in patients who did not receive AVR was faster in the high-severity group. Compared with the conventional classification of disease severity, machine-learning–based severity classification improved discrimination (integrated discrimination improvement: 0.07; 95% confidence interval: 0.02 to 0.12) and reclassification (net reclassification improvement: 0.17; 95% confidence interval: 0.11 to 0.23) for the outcome of AVR at 5 years. For both ECHO and CMR cohorts, we observed prognostic value of the machine-learning classifications for subgroups with asymptomatic, nonsevere or discordant AS. Conclusions Machine learning can integrate ECHO measurements to augment the classification of disease severity in most patients with AS, with major potential to optimize the timing of AVR.

Establishing the reproducibility of expert-derived measurements on CTA exams of aortic dissection is clinically important and paramount for ground-truth determination for machine learning.Four independent observers retrospectively evaluated CTA exams of 72 patients with uncomplicated Stanford type B aortic dissection and assessed the reproducibility of a recently proposed combination of four morphologic risk predictors (maximum aortic diameter, false lumen circumferential angle, false lumen outflow, and intercostal arteries). For the first inter-observer variability assessment, 47 CTA scans from one aortic center were evaluated by expert-observer 1 in an unconstrained clinical assessment without a standardized workflow and compared to a composite of three expert-observers (observers 2-4) using a standardized workflow. A second inter-observer variability assessment on 30 out of the 47 CTA scans compared observers 3 and 4 with a constrained, standardized workflow. A third inter-observer variability assessment was done after specialized training and tested between observers 3 and 4 in an external population of 25 CTA scans. Inter-observer agreement was assessed with intraclass correlation coefficients (ICCs) and Bland-Altman plots.Pre-training ICCs of the four morphologic features ranged from 0.04 (-0.05 to 0.13) to 0.68 (0.49-0.81) between observer 1 and observers 2-4 and from 0.50 (0.32-0.69) to 0.89 (0.78-0.95) between observers 3 and 4. ICCs improved after training ranging from 0.69 (0.52-0.87) to 0.97 (0.94-0.99), and Bland-Altman analysis showed decreased bias and limits of agreement.Manual morphologic feature measurements on CTA images can be optimized resulting in improved inter-observer reliability. This is essential for robust ground-truth determination for machine learning models.• Clinical fashion manual measurements of aortic CTA imaging features showed poor inter-observer reproducibility. • A standardized workflow with standardized training resulted in substantial improvements with excellent inter-observer reproducibility. • Robust ground truth labels obtained manually with excellent inter-observer reproducibility are key to develop reliable machine learning models.

The increasing disease burden caused by atrial fibrillation has led to new interventions aimed at restoring normal sinus rhythm and preventing embolic complications. Cardiac CT can help identify suitable candidates, plan the intervention, and diagnose complications. In some institutions, this application has become the most common indication for the clinical use of cardiac CT. Consequently, expertise in making the pertinent observations outlined in this chapter is of critical importance for the success of any cardiac CT program and offers a tremendous opportunity for interdisciplinary collaboration.

Background International registries for acute type A aortic dissection (TAAD) demonstrate stagnant operative mortality rates in excess of 20% and stroke rates of 9% to 25%, with little global emphasis on stroke reduction or carotid involvement. Cerebral malperfusion with TAAD has been linked to poorer outcome. We hypothesize that concomitant carotid dissection or complex dissection flaps in the arch play a major role in stroke development and that aggressive reconstruction of the arch and carotid arteries can improve neurologic outcomes in TAAD. Methods A standardized protocol focused on expedient care, neurocerebral protection, and common carotid and total arch reconstruction was developed for 264 consecutive TAADs. Arch and complete carotid replacement was based on arch dissection anatomy, carotid involvement, or an intraarch tear. Neurocerebral monitoring with continuous electroencephalogram/somatosensory evoked potentials was used in all cases. Results The postoperative stroke and hospital mortality rates were 3.4% and 9.1%, and stroke rates by extent of arch replacement were 4%, 3%, and 0% for hemiarch, total arch, and total arch with complete carotid replacement, respectively. An intraoperative change in the electroencephalogram/somatosensory evoked potentials was strongly predictive of stroke and had a negative predictive value of 98.2%. Conclusions An algorithmic approach to TAAD including (1) rapid transport-to-incision-to-cardiopulmonary bypass established centrally, (2) neurocerebral monitoring, (3) liberal use of total arch replacement for clearly defined indications (and hemiarch for all others), and (4) common carotid arterial replacement for concomitant carotid dissections significantly improves outcomes.

Background— Computed tomography aortic valve calcium scoring (CT-AVC) holds promise for the assessment of patients with aortic stenosis (AS). We sought to establish the clinical utility of CT-AVC in an international multicenter cohort of patients. Methods and Results— Patients with AS who underwent ECG-gated CT-AVC within 3 months of echocardiography were entered into an international, multicenter, observational registry. Optimal CT-AVC thresholds for diagnosing severe AS were determined in patients with concordant echocardiographic assessments, before being used to arbitrate disease severity in those with discordant measurements. In patients with long-term follow-up, we assessed whether CT-AVC thresholds predicted aortic valve replacement and death. In 918 patients from 8 centers (age, 77±10 years; 60% men; peak velocity, 3.88±0.90 m/s), 708 (77%) patients had concordant echocardiographic assessments, in whom CT-AVC provided excellent discrimination for severe AS (C statistic: women 0.92, men 0.89). Our optimal sex-specific CT-AVC thresholds (women 1377 Agatston unit and men 2062 Agatston unit) were nearly identical to those previously reported (women 1274 Agatston unit and men 2065 Agatston unit). Clinical outcomes were available in 215 patients (follow-up 1029 [126–2251] days). Sex-specific CT-AVC thresholds independently predicted aortic valve replacement and death (hazard ratio, 3.90 [95% confidence interval, 2.19–6.78]; P P =0.010). Conclusions— Sex-specific CT-AVC thresholds accurately identify severe AS and provide powerful prognostic information. These findings support their integration into routine clinical practice. Background Catheter-directed interventions for the treatment of patients with submassive pulmonary embolism (sPE) have shown promise in rapidly improving right-sided heart strain and preventing decompensation to massive pulmonary embolism. Among various catheter interventions, ultrasound-assisted thrombolysis (USAT) has attracted interest as potentially having more efficient lytic effect that could achieve thrombolysis faster and with a reduced lytic dose. However, based on clinical evidence, it is unclear whether USAT is superior to standard catheter-directed thrombolysis (SCDT). We herein describe the study design of the Standard vs UltrasouNd-assiSted CathEter Thrombolysis for Submassive Pulmonary Embolism (SUNSET sPE) trial, an ongoing randomized clinical trial designed to address this question. Methods Adults with sPE presenting or referred to our institution are considered for enrollment in the trial. At the discretion of the treatment team, all patients undergo a catheter-directed intervention plus concomitant therapeutic anticoagulation. Participants are randomized 1:1 to a USAT catheter or an SCDT catheter. Study assessors are blinded to treatment group. The primary outcome is clearance of pulmonary thrombus burden, assessed by postprocedure computed tomography angiography. Secondary outcomes include resolution of right ventricular strain by echocardiography; improvement in pulmonary artery pressures; and 3- and 12-month improvement in echocardiographic, functional capacity, and quality of life measures. The study is powered to detect a 50% improvement in pulmonary artery thrombus clearance. Our enrollment target is 40 patients per treatment arm. Conclusions SUNSET sPE is an ongoing randomized, head-to-head, single-blinded clinical trial with the goal of assessing whether USAT results in superior thrombus clearance compared with SCDT in patients with sPE. We expect the results of our study to inform future guidelines on choice of thrombolysis modality in this population of challenging patients.

Background Preliminary clinical experience with a percutaneous endoventricular partitioning device (Parachute®, CardioKinetix Inc., Menlo Park, CA, USA) suggests that it ameliorates global LV dysfunction and heart failure symptoms in selected patients who have suffered previous anterior myocardial infarction. Less is known of its effect on regional LV function. Objective To gain insight into device effect on regional LV function by analysis of cardiac computed tomographic (CT) images obtained before and after device implantation. Methods Comparative analysis of pre and 6 months post-implantation contrast-enhanced CT images from 6 subjects enrolled in the phase 1 Parachute clinical trials, including regional LV volume and systolic excursion, as well as device motion. Results After implantation, a significant reduction in volume of the “dynamic” LV compartment (that which was not excluded by the device) was accompanied by a significant reduction in dykinetic motion and a trend toward an improved ejection fraction. Penetration of contrast into the excluded compartment was still present at 6 months, however the apical motion was significantly less diskinetic in 3 subjects and unchanged in the other 3. Overall device surface motion was inward in systole, a significant improvement relative to the overall dyskinetic LV apex pre-implantation. Device motion was spatially heterogeneous, which appeared to be dependent on the motility of the myocardium that anchored its individual splines. Conclusions Our data suggest that the Parachute device acts as a functional impediment to flow and stretch, effectively depressurizing the apical segment.

Granulomatosis with polyangiitis is a systemic disease resulting in necrotizing vasculitis of small- and medium-sized vessels. Cardiac involvement is rare and when present usually manifests with pericarditis and coronary artery vasculitis. We report here a case of granulomatosis with polyangiitis involving the native coronary arteries, bypass graft, and pericardium with interesting imaging findings on contrast-enhanced CT and MRI. A 57-year-old man with a history of chronic headaches presented to the emergency room with syncope. Contrast-enhanced CT demonstrated extensive soft tissue attenuation around the native coronary arteries and bypass graft. Contrast-enhanced MRI demonstrated enhancing nodular soft tissue surrounding the coronary arteries, bypass graft, and pericardium. Pericardial biopsy revealed a necrotizing granulomatous pericarditis with vasculitis concerning for granulomatosis with polyangiitis. The patient demonstrated MPO-positive and PR-3 negative serologies. After being discharged on rituximab and prednisone, follow-up CT 3 years later showed significant improvement of the soft tissue thickening surrounding the coronary arteries, bypass graft, and pericardium.

This study investigated the relative efficiencies of a stereographic display and two monoscopic display schemes for detecting lung nodules in chest computed tomography (CT). The ultimate goal was to determine whether stereoscopic display provides advantages for visualization and interpretation of three-dimensional (3D) medical image datasets. A retrospective study that compared lung nodule detection performances achieved using three different schemes for displaying 3D CT data was conducted. The display modes included slice-by-slice, orthogonal maximum intensity projection (MIP), and stereoscopic display. One hundred lung-cancer screening CT examinations containing 647 nodules were interpreted by eight radiologists, in each of the display modes. Reading times and displayed slab thickness versus time were recorded, as well as the probability, location, and size for each detected nodule. Nodule detection performance was analyzed using the receiver operating characteristic method. The stereo display mode provided higher detection performance with a shorter interpretation time, as compared to the other display modes tested in the study, although the difference was not statistically significant. The analysis also showed that there was no difference in the patterns of displayed slab thickness versus time between the stereo and MIP display modes. Most radiologists preferred reading the 3D data at a slab thickness that corresponded to five CT slices. Our results indicate that stereo display has the potential to improve radiologists' performance for detecting lung nodules in CT datasets. The experience gained in conducting the study also strongly suggests that further benefits can be achieved through providing readers with additional functionality.

Background CT scan measures of emphysema and airway disease have been correlated with lung function in cohorts of subjects with a range of COPD severity. The contribution of CT scan-assessed airway disease to objective measures of lung function and respiratory symptoms such as dyspnea in severe emphysema is less clear. Methods Using data from 338 subjects in the National Emphysema Treatment Trial (NETT) Genetics Ancillary Study, densitometric measures of emphysema using a threshold of −950 Hounsfield units (%LAA-950) and airway wall phenotypes of the wall thickness (WT) and the square root of wall area (SRWA) of a 10-mm luminal perimeter airway were calculated for each subject. Linear regression analysis was performed for outcome variables FEV 1 and percent predicted value of FEV 1 with CT scan measures of emphysema and airway disease. Results In univariate analysis, there were significant negative correlations between %LAA-950 and both the WT ( r = −0.28, p = 0.0001) and SRWA ( r = −0.19, p = 0.0008). Airway wall thickness was weakly but significantly correlated with postbronchodilator FEV 1 % predicted (R = −0.12, p = 0.02). Multivariate analysis showed significant associations between either WT or SRWA (β = −5.2, p = 0.009; β = −2.6, p = 0.008, respectively) and %LAA-950 (β = −10.6, p = 0.03) with the postbronchodilator FEV 1 % predicted. Male subjects exhibited significantly thicker airway wall phenotypes (p = 0.007 for WT and p = 0.0006 for SRWA). Conclusions Airway disease and emphysema detected by CT scanning are inversely related in patients with severe COPD. Airway wall phenotypes were influenced by gender and associated with lung function in subjects with severe emphysema.

Right ventricular (RV) failure is associated with poor outcomes in pulmonary hypertension (PH). We sought to phenotype the RV in PH patients with compensated and decompensated RV function by quantifying regional and global RV structural and functional changes. Twenty-two patients (age 51 +/- 11, 14 females, mean pulmonary artery (PA) pressure range 13-79 mmHg) underwent right heart catheterization, echocardiography, and ECG-gated multislice computed tomography of the chest. Patients were divided into three groups: Normal, PH with hemodynamically compensated, and decompensated RV function (PH-C and PH-D, respectively). RV wall thickness (WT) was measured at end-diastole (ED) and end-systole (ES) in three regions: infundibulum, lateral free wall, and inferior free wall. Globally, RV volumes progressively increased from Normal to PH-C to PH-D and RV ejection fraction decreased. Regionally, WT increased and fractional wall thickening (FWT) decreased in a spatially heterogeneous manner. Infundibular wall stress was elevated and FWT was lower regardless of the status of global RV function. In PH, there are significant phenotypic abnormalities in the RV even in the absence of overt hemodynamic RV decompensation. Regional changes in RV structure and function may be early markers of patients at risk for developing RV failure.

PURPOSE The purpose of this study was to compare the measurements of the aortic root obtained from electrocardiographically (ECG)-gated computed tomography (CT) angiography (CTA) to the measurements obtained from transthoracic echocardiography (TTE). MATERIALS AND METHODS This was a retrospective study in a patient population scanned at our institution between December 2005 and January 2007 with retrospectively ECG-gated CTA. ECG-gated CTA was performed with a 64-section helical CT scanner (Light speed, VCT, GE, Milwaukee, WI). Sixty-eight patients; 51 men and 17 women were included in this study. Aortic root diameters were measured by using double oblique reconstruction from axial source images. The TTE measurements of the aortic root were obtained from the reports that were performed within 2 months of CTA. RESULTS The average aortic root diameter measured by TTE was 33+/-4.1 mm; on CTA it was 36.9+/-3.8 mm. The median difference between the 2 measurements was 3.9 mm which was significant (P

Adenomyoepithelioma of the breast is a rare tumor, having a bicellular pattern of proliferation which consists of epithelial and myoepithelial cells. Though generally considered a benign tumor, removal of the primary tumor may occasionally be followed by local recurrence and rarely distant metastasis. We describe a case of a 60-year-old woman with adenomyoepithelioma of the breast who developed lung, bone, and kidney metastases 5 years after removal of the primary tumor.

Aims The human left atrial appendage (LAA) is a region of increasing interest as a target for intervention. We sought to improve insight into the anatomy of this region using computed tomography (CT). Methods and results Multidimensional cardiac reconstruction (whole heart and isolated left atrium) from CT images was performed in each of three groups: (i) patients without atrial fibrillation (AF, n ¼ 10); (ii) patients with intermittent (paroxysmal) AF (n ¼ 25); (iii) patients with continuous (persistent) AF (n ¼ 10). Indices included LAA morphology, anatomical relationships, dimensions, angulation, and motility. There was substantial interindividual variation in each index. LAA morphologic differences were associated with variations in anatomical relationships. LAA dimensions in AF patients exceeded those in patients without AF, but angulation and motility were similar. The LAA could be subdivided into proximal and distal portions, each of which had distinct morphology and anatomical relationships. Dimensions in men tended to exceed those in women. Conclusion Regardless of AF history, there is broad variation in LAA morphology, anatomical relationships, dimensions, angulation, and motility. These observations may have importance for the development of technologies for therapy delivery in this region.

BACKGROUND Recent reports suggest that the CartoMerge system is useful for guiding human posterior left atrial (PLA) endocardial ablation. OBJECTIVE To assess the accuracy of the CartoMerge system during PLA ablation. METHODS Sixteen patients undergoing PLA catheter ablation were studied. In each patient, registration of preoperative computed tomographic (CT) and intraoperative electroanatomic left atrial images was performed to create CartoMerge images. Encircling of right and left pulmonary venous vestibules with ablation points was then performed guided solely by intracardiac echocardiography, with point locations saved on a CartoMerge image to which the operator was blinded. The accuracy of the CartoMerge image was then assessed by measuring the distance from the location of each ablation point on the image to its actual anatomic location. In five patients, accuracy of registration of each of three left atrial CT images (just prior to mitral valve opening, at end-diastasis, at end-atrial contraction) with the electroanatomic image was compared. In two patients, accuracy of registration using left atrial image data alone was compared with that which used both left atrial and thoracic aorta image data. RESULTS In each patient, inaccuracy of the CartoMerge image was apparent, the magnitude of which was similar for right- and left-vestibule ablation points. Accuracy was significantly improved when the end-atrial contraction CT image was used for registration. The inclusion of thoracic aorta image data did not improve accuracy. CONCLUSIONS The CartoMerge system is inaccurate. Inaccuracy may be reduced by using CT and electroanatomic images obtained at the same point in the atrial mechanical cycle.

Atrial fibrillation (AF) is a common cardiac rhythm disturbance and its incidence is increasing. Radiofrequency catheter ablation (RFCA) is a highly successful therapy for treating AF, and its use is becoming more widespread; however, with its increasing use and evolving technique, known complications are better understood and new complications are emerging. Computed tomography (CT) of the pulmonary veins, or more correctly, the posterior left atrium (LA), has an established role in precisely defining the complex anatomy of the LA and pulmonary veins preablation and has an expanding role in identifying the myriad of possible complications postablation. The purposes of this article are: to review AF and RFCA; to discuss CT evaluation of the LA and pulmonary veins preablation; and to review the complications of RFCA focusing on the role of CT postablation.

ObjectivesWe sought to test the hypothesis that a novel 2-dimensional echocardiographic image analysis system using artificial intelligence-learned pattern recognition can rapidly and reproducibly calculate ejection fraction (EF).BackgroundEchocardiographic EF by manual tracing is time consuming, and visual assessment is inherently subjective.MethodsWe studied 218 patients (72 female), including 165 with abnormal left ventricular (LV) function. Auto EF incorporated a database trained on >10,000 human EF tracings to automatically locate and track the LV endocardium from routine grayscale digital cineloops and calculate EF in 15 s. Auto EF results were independently compared with manually traced biplane Simpson’s rule, visual EF, and magnetic resonance imaging (MRI) in a subset.ResultsAuto EF was possible in 200 (92%) of consecutive patients, of which 77% were completely automated and 23% required manual editing. Auto EF correlated well with manual EF (r = 0.98; 6% limits of agreement) and required less time per patient (48 ± 26 s vs. 102 ± 21 s; p < 0.01). Auto EF correlated well with visual EF by expert readers (r = 0.96; p < 0.001), but interobserver variability was greater (3.4 ± 2.9% vs. 9.8 ± 5.7%, respectively; p < 0.001). Visual EF was less accurate by novice readers (r = 0.82; 19% limits of agreement) and improved with trainee-operated Auto EF (r = 0.96; 7% limits of agreement). Auto EF also correlated with MRI EF (n = 21) (r = 0.95; 12% limits of agreement), but underestimated absolute volumes (r = 0.95; bias of −36 ± 27 ml overall).ConclusionsAuto EF can automatically calculate EF similarly to results by manual biplane Simpson’s rule and MRI, with less variability than visual EF, and has clinical potential.

Accumulating evidence points to the central importance of the posterior left atrium (PLA) for atrial fibrillation (AF). Catheter ablation intended to cure AF is increasingly practiced; performance and assessment of this procedure is enhanced by accurate imaging of PLA anatomy. Prior reports have suggested that both computed tomographic (CT) and magnetic resonance (MR) imaging techniques provide accurate PLA images. These techniques have never been compared directly. Twenty patients referred for catheter ablation underwent preoperative imaging using both CT and MR. Each technique was used to create a multidimensional image of the PLA. Within patients, morphologic and dimensional PLA indices, including number of individual pulmonary venoatrial junctions, presence of ostial branches, circumference of each venoatrial junction, venoatrial junction “non-circularity”, and distance between ipsilateral superior and inferior venoatrial junctions, were well correlated. CT and MR-based images of the PLA appear comparable. Technique selection should involve considerations of toxicity, tolerance, and local resources.

Introduction: In living donor lung transplant, donor lobectomies from 2 donors provide right and left lower lobes for transplantation. In the past, routine evaluation of pulmonary anatomy was not performed preoperatively. Intraoperatively, surgeons were often forced to sacrifice the lingular artery or right middle lobe segmental artery to obtain an adequate arterial cuff for safe reimplantation. This study was performed to evaluate the utility of preoperative 3D-multidetector CT angiography (3D-MDCTA) as a noninvasive method of assessing pulmonary arteries to improve donor selection and surgical planning for living lung donors. Subjects and Methods: Five potential lung donors for 2 recipients were included in the study. CT scanning with 4 channel multidetector-row CT was performed, using a modified pulmonary embolism protocol. Post-processing was performed using volume rendering techniques on a commercially available workstation. Results: 3D-MDCT demonstrated that there are a number of variations in pulmonary arterial anatomy and that ideal anatomy was seldom encountered. Comparison of different donors determined which lower lobes were most favorable for transplantation. Surgery confirmed the accuracy of 3D-MDCTA. There were no pulmonary arterial complications, and no vessels were sacrificed. Conclusion: Safely explanting lower lobes from living donors for lung transplantation poses challenges not encountered in harvesting cadaveric donors or performing lobectomies for malignancy. 3D-MDCTA of pulmonary arteries can noninvasively delineate the often-complex pulmonary anatomy, which may assist in donor selection as well as reduce donor intraoperative and postoperative vascular complications.

This document is a revision of a previously published cardiothoracic curriculum for diagnostic radiology residency ( 1 ), and reflects interval changes in the clinical practice of cardiothoracic radiology and changes in the Accreditation Council for Graduate Medical Education (ACGME) requirements for diagnostic radiology training programs. The revised ACGME Program Requirements for Residency Education in Diagnostic Radiology ( 2 ) went into effect December 2003.

To measure observer performance at various levels of prevalence.A multiobserver multiabnormality receiver operating characteristic (ROC) study to assess the effect of prevalence on observer performance was conducted. Fourteen observers, including eight faculty members, two fellows, and four residents, interpreted 1,632 posteroanterior chest images with five prevalence levels by using a nested study design. Performance comparisons were accomplished by using a multireader multicase approach to assess the effect of prevalence from 28% (69 of 249) to 2% (31 of 1,577) on diagnostic accuracy. The mean times required to review and report a case were analyzed and compared for different levels of prevalence and readers' experience.Area under the ROC curve demonstrated that, with the study experimental conditions, no significant effect could be measured as a function of prevalence (P.05) for any abnormality, group of cases, or readers. There were no significant differences (P.05) in the mean times required to review and report cases at different prevalence levels and with different groups of readers.The consistency in the results and the size of this study suggest that with laboratory conditions, if a prevalence effect exists, it is quite small in magnitude; hence, it will not likely alter conclusions derived from such studies.

Objectives This study was designed to examine the dimensions and morphology of left atrial (LA) and distal pulmonary veins (PVs) using multidetector helical computed tomography (MDCT). Background Detailed knowledge of LA and PV anatomy will assist in the development of techniques for ablative intervention. Multidetector helical computed tomography is a method for multidimensional imaging of cardiac anatomy. Methods Multidetector helical computed tomography was used to image the LA and PVs in 70 subjects with and 47 subjects without atrial fibrillation (AF). Accuracy of the MDCT data was confirmed by correlation with echocardiography and endocardial electrogram recordings. Results Left atrial and PV dimensions were significantly larger in AF versus non-AF subjects, men versus women, and subjects with persistent versus paroxysmal AF. There were no differences between groups in morphologic detail. Conclusions Multidetector helical computed tomography images of the LA and PVs are accurate and provide detailed anatomic information. Significant differences in dimensions but not morphologic detail were apparent between groups.

To determine the performance of radiologists with differing levels of expertise in the diagnosis of the most common types of liver tumors with central scars (ie, focal nodular hyperplasia [FNH], fibrolamellar hepatocellular carcinoma [HCC], and large hepatic hemangioma) by using specific computed tomographic (CT) findings.Review of medical records at the University of Pittsburgh Medical Center identified patients with a total of 64 liver tumors that had central scars-including 29 cases of FNH, 20 fibrolamellar HCCs, and 15 large (3.5 cm in diameter) hemangiomas-and with CT scans available for review. Retrospective review of these scans was performed individually by six radiologists who were blinded to the diagnosis, including two faculty abdominal radiologists, one abdominal imaging fellow, and three radiology residents. Individual performance was evaluated by means of receiver operating characteristic analysis, and interobserver agreement was measured by using the Cronbach alpha. Individual CT findings that may allow differentiation of tumor types were identified with the Kruskal-Wallis test.CT allowed good to excellent interobserver agreement in the diagnosis of tumor type and in recognition of differential findings among the three types. The individual accuracy of diagnosis was very good, with the average area under the receiver operating characteristic curve ranging from 0.81 to 0.90. Although the faculty radiologists performed the best, the differences in performance between the subgroups of readers and the levels of confidence in diagnosis were not statistically significant. The diagnosis of fibrolamellar HCC was the most accurate and had the highest sensitivity, followed by FNH and large hemangioma. Clinical and CT findings that were found to be statistically significant in differentiating tumor types were patient age and sex, tumor size larger than 10 cm, width of tumor scars, invasion of vessels, nodular centripetal enhancement, marked hyperattenuation on arterial phase images, lymphadenopathy, heterogeneity, extrahepatic metastases, surface lobulation, calcification, and isoattenuation with liver tissue on portal venous phase images.CT allows accurate differentiation of the most common types of liver tumors with central scars, including FNH, fibrolamellar HCC, and large hemangioma.

Objective Prognosis in pulmonary hypertension (PH) is largely determined by RV function. However, uncertainty remains about what metrics of RV function might be most clinically relevant. The purpose of this study was to assess the clinical relevance of metrics of RV functional adaptation to increased afterload. Methods Patients referred for PH underwent right heart catheterisation and RV volumetric assessment within 48 h. A RV maximum pressure (Pmax) was calculated from the RV pressure curve. The adequacy of RV systolic functional adaptation to increased afterload was estimated either by a stroke volume (SV)/end-systolic volume (ESV) ratio, a Pmax/mean pulmonary artery pressure (mPAP) ratio, or by EF (RVEF). Diastolic function of the RV was estimated by a diastolic elastance coefficient β. Survival analysis was via Cox proportional HR, and Kaplan–Meier with the primary outcome of time to death or lung transplant. Results Patients (n=50; age 58±13 yrs) covered a range of mPAP (13–79 mm Hg) with an average RVEF of 39±17% and ESV of 143±89 mL. Average estimates of the ratio of end-systolic ventricular to arterial elastance were 0.79±0.67 (SV/ESV) and 2.3±0.65 (Pmax/mPAP-1). Transplantation-free survival was predicted by right atrial pressure, mPAP, pulmonary vascular resistance, β, SV, ESV, SV/ESV and RVEF, but after controlling for right atrial pressure, mPAP, and SV, SV/ESV was the only independent predictor. Conclusions The adequacy of RV functional adaptation to afterload predicts survival in patients referred for PH. Whether this can simply be evaluated using RV volumetric imaging will require additional confirmation.

Pulmonary cryptococcal infections occur in both immunocompetent and immunocompromised individuals, with a reported increased incidence of diffuse pulmonary disease in acquired immune deficiency syndrome (AIDS) patients. The authors observed no differences in the radiographic appearances of pulmonary cryptococcal disease between human immunodeficiency virus (HIV) patients and other immunocompromised individuals. Chest computed tomography (CT) contributes to a more comprehensive understanding of pulmonary cryptococcal infections.

To determine the usefulness of delayed post-equilibrium-phase contrast material-enhanced images in evaluation of intrahepatic cholangiocarcinoma.Review of surgical, pathologic, and radiologic records revealed 47 patients with proved cholangiocarcinoma with delayed (6-36 minutes) post-equilibrium-phase contrast-enhanced computed tomographic (CT) scans. Unenhanced, dynamic contrast-enhanced, and delayed images were retrospectively reviewed, and enhancement patterns for cholangiocarcinoma lesions were recorded. Degree of delayed enhancement was compared with that of surrounding liver parenchyma. Patterns of delayed enhancement were characterized as homogeneous or heterogeneous. Enhancement characteristics were correlated with histologic findings.Thirty-five (74%) of 47 patients had tumors with mild or marked hyperattenuating delayed contrast enhancement, of which 18 had tumors with homogeneous hyperattenuating enhancement. In three patients, the only evidence of tumor was on the delayed images. In one patient, the tumor was not definable at any imaging phase. Tumors with delayed enhancement tended to be fibrous; however, degree of contrast material retention did not always correlate with the fibrous content of tumors at histopathologic analysis.Delayed tumoral contrast enhancement is a typical feature of intrahepatic cholangiocarcinoma and may aid in the detection and characterization of such lesions at CT.

Objectives To determine if measurements of aortic wall attenuation can improve the CT diagnosis of acute aortic syndromes. Methods CT reports from a ten year period were searched for acute aortic syndromes (AAS). Studies with both an unenhanced and a contrast enhanced (CTA) series that had resulted in the diagnosis of intramural hematoma (IMH) were reviewed. Diagnoses were confirmed by medical records. The attenuation of aortic wall abnormalities was measured. The observed attenuation threshold was validated using studies from 39 new subjects with a variety of aortic conditions. Results The term “aortic dissection” was identified in 1206, and IMH in 124 patients’ reports. IMH was confirmed in 31 patients, 21 of whom had both unenhanced and contrast enhanced images. All 21 had pathologic CTA findings, and no CTA with IMH was normal. Attenuation of the aortic wall was greater than 45 HUs on the CTA images in all patients with IMH. When this threshold was applied to the new group, sensitivity for diagnosing AAS was 100% (19/19), and specificity 94% (16/17). Addition of unenhanced images did not improve accuracy. Conclusions Measurements of aortic wall attenuation in CTA have a high negative predictive value for the diagnosis of acute aortic syndromes.

Background— Routine clinical use of novel free-breathing, motion-corrected, averaged late-gadolinium-enhancement (moco-LGE) cardiovascular MR may have advantages over conventional breath-held LGE (bh-LGE), especially in vulnerable patients. Methods and Results— In 390 consecutive patients, we collected bh-LGE and moco-LGE with identical image matrix parameters. In 41 patients, bh-LGE was abandoned because of image quality issues, including 10 with myocardial infarction. When both were acquired, myocardial infarction detection was similar (McNemar test, P =0.4) with high agreement (κ=0.95). With artifact-free bh-LGE images, pixelwise myocardial infarction measures correlated highly ( R 2 =0.96) without bias. Moco-LGE was faster, and image quality and diagnostic confidence were higher on blinded review ( P P P P =0.056) because a significant number of vulnerable patients did not receive bh-LGE (because of arrhythmia or inability to hold breath). Conclusions— Myocardial infarction detection and quantification are similar between moco-LGE and bh-LGE when bh-LGE can be acquired well, but bh-LGE quality deteriorates with patient vulnerability. Acquisition time, image quality, diagnostic confidence, and the number of successfully scanned patients are superior with moco-LGE, which extends LGE-based risk stratification to include patients with vulnerability confirmed by outcomes. Moco-LGE may be suitable for routine clinical use.

Background Echocardiography (echo) is a first line test to assess cardiac structure and function. It is not known if cardiovascular magnetic resonance (CMR) with late gadolinium enhancement (LGE) ordered during routine clinical practice in selected patients can add additional prognostic information after routine echo. We assessed whether CMR improves outcomes prediction after contemporaneous echo, which may have implications for efforts to optimize processes of care, assess effectiveness, and allocate limited health care resources. Methods and results We prospectively enrolled 1044 consecutive patients referred for CMR. There were 38 deaths and 3 cardiac transplants over a median follow-up of 1.0 years (IQR 0.4-1.5). We present a case of constrictive pericarditis resulting in an outpouching of the right ventricular free wall, simulating a right ventricular free wall aneurysm. The present case is, to the best of our knowledge, the first reported right ventricular free wall aneurysm-like outpouching adjacent to surrounding regions of thickened pericardium in a patient with constrictive pericarditis.

To investigate the prevalence of aortic root dilation in patients who underwent CT angiography of the thoracic aorta.In 95 patients, 64-slice multislice computed tomography was performed for evaluation of the thoracic aorta. Measurements of the annulus, sinuses of valsalva (SOV), sinotubular junction (STJ), and maximum ascending aorta (AAo) were made by double oblique multiplanar reformation (MPR). For the AAo, STJ, and SOV, dilation was defined as greater than 40 mm; for annulus, the dilation criterion was greater than 27 mm.Overall, 52 patients were diagnosed with a dilated AAo. Of those patients with dilated AAo, 28 patients had a dilated annulus, 27 patients had dilated SOV, and 11 patients had STJ dilation. Forty-three patients presented with normal AAo; 12 patients had annulus dilation; 12 patients had SOV dilation; and 4 patients had STJ dilation. In patients with dilated AAo, 38% also had a dilated annulus, 52% showed SOV dilation, and 21% presented with STJ dilation, compared to 28% annulus dilation, 28% SOV dilation, and 9% STJ dilation in patients with an AAo of normal caliber.Our data indicate a higher prevalence of aortic root dilation among patients with dilated AAo.

Lone atrial fibrillation (LAF) is considered by some to be a primary atrial electrophysiologic disorder. However, we have frequently observed evidence of "extraatrial" diseases - atherosclerosis and associated metabolic disorders - in our LAF patients. We sought to characterize and quantify extraatrial disease burden in LAF patients, and to correlate this burden with features of the arrhythmia including pattern (paroxysmal versus persistent) and response to catheter ablation.Forty-six consecutive patients with non-familial LAF underwent assessment for evidence of atherosclerosis (computed tomographic vascular calcification and elevated arterial pulse wave velocity) and associated metabolic diseases (dyslipidemia, insulin resistance and inflammation), and then catheter ablation. The cohort had a significant incidence of atherosclerosis (57%) and metabolic (70%) diseases. Patients with persistent AF tended to have a greater extraatrial disease burden than those with paroxysmal AF. A significant inverse relationship between the rate of ablation success and extraatrial disease burden was demonstrated.Extraatrial disease was common in this LAF cohort. Correlations between extraatrial disease burden and features of the arrhythmia would, if verified, challenge the notion that LAF is a "primary" electrophysiologic disorder.

The goal of this study was to assess whether radiologists’ search paths for lung nodule detection in chest computed tomography (CT) between different rendering and display schemes have reliable properties that can be exploited as an indicator of ergonomic efficiency for the purpose of comparing different display paradigms. Eight radiologists retrospectively viewed 30 lung cancer screening CT exams, containing a total of 91 nodules, in each of three display modes [i.e., slice-by-slice, orthogonal maximum intensity projection (MIP) and stereoscopic] for the purpose of detecting and classifying lung nodules. Radiologists’ search patterns in the axial direction were recorded and analyzed along with the location, size, and shape for each detected feature, and the likelihood that the feature is an actual nodule. Nodule detection performance was analyzed by employing free-response receiver operating characteristic methods. Search paths were clearly different between slice-by-slice displays and volumetric displays but, aside from training and novelty effects, not between MIP and stereographic displays. Novelty and training effects were associated with the stereographic display mode, as evidenced by differences between the beginning and end of the study. The stereo display provided higher detection and classification performance with less interpretation time compared to other display modes tested in the study; however, the differences were not statistically significant. Our preliminary results indicate a potential role for the use of radiologists’ search paths in evaluating the relative ergonomic efficiencies of different display paradigms, but systematic training and practice is necessary to eliminate training curve and novelty effects before search strategies can be meaningfully compared.

The purpose of this article is to describe the imaging features of patent ductus arteriosus (PDA) identified on chest MDCT performed for other indications and to describe the additional functional information that cardiac MRI can provide about these lesions.The daily use of MDCT studies for the evaluation of pulmonary embolic disease or aortic abnormalities can reveal incidental PDAs. Small incidental PDAs can be identified on chest MDCT angiography timed for either the pulmonary arteries or the aorta. Using multiplanar reformations, one can assess PDA location, caliber, length, and presence of calcifications. The presence of a "positive" or a "negative contrast jet" verifies a patent shunt. Cardiac MRI shows the detailed anatomic and morphologic features of a PDA. Hemodynamic information revealing the presence and severity of a significant shunt is obtainable using velocity-encoded MRI, allowing accurate shunt calculation. Using MDCT and MRI, information regarding the clinical significance of an incidental PDA can influence management decisions. The imaging information was used to determine that one PDA required intervention.

An epicardial segment of a coronary artery that courses through the myocardium is termed "myocardial bridging". Generally, this is a benign condition but it may lead to angina, ischemia or infarction. The current diagnostic standard of reference is coronary catheter angiography. Intravascular ultrasound (IVUS) and intracoronary Doppler (ICD) have been recently introduced as well. These are all invasive imaging modalities. We describe the utilization of gated multidetector CT (MDCT) as a non-invasive alternative for diagnosis of this anomaly. Information regarding the tunneled coronary artery including its length, depth, precise location and surrounding myocardium is easily obtainable.

In addition to the conventional means of diagnosing pulmonary embolism, resting echocardiography has sometimes been useful. We describe the case of a patient with a normal resting transthoracic echocardiogram, but with a markedly abnormal posttreadmill exercise echocardiogram revealing acute right ventricular dilatation, marked ventricular septal shift, and acute pulmonary hypertension. Pulmonary embolism was suspected and subsequently confirmed by chest computed tomographic angiography.

Radio-frequency catheter ablation (RFCA) of the distal pulmonary veins and posterior left atrium is increasingly being used to treat recurrent or refractory atrial fibrillation that resists pharmacologic therapy or cardioversion. Successful RFCA of atrial fibrillation requires resolution of abnormal rhythms while minimizing complications and can be achieved with precise, preprocedural, three-dimensional (3D) anatomic delineation of the target, the atriopulmonary venous junction. Three-dimensional multi-detector row computed tomography (CT) of the pulmonary veins and left atrium provides the necessary anatomic information for successful RFCA, including (a) the number, location, and angulation of pulmonary veins and their ostial branches unobscured by adjacent cardiac and vascular anatomy, and (b) left atrial volume. The 3D multi-detector row CT scanning and postprocessing techniques used for pre-RFCA planning are straightforward. Radiologists must not only understand these techniques but must also be familiar with atrial fibrillation and the technical considerations and complications associated with RFCA of this condition. In addition, radiologists must be familiar with anatomic variants of the left atrium and distal pulmonary veins and understand the importance of these variants to the referring cardiac interventional electrophysiologist.

One way to increase the efficiency of large-scale observer performance studies that utilize receiver-operator- characteristic (ROC) analysis would be to identify a group of observers whose ROC results consistently reflect a strong sensitivity to imaging mode differences. We reviewed the data of four large ROC studies performed by our group. The primary index used to classify observers was defined as the absolute difference between readers' Azs for two modes divided by the standard deviation, appropriately corrected for correlation. Using this index, observers were ranked as to their ability to detect differences between modes. We performed four different analyses on data that included 119,550 observations in four studies and 14 different modes performed by 20 readers. With the exception of one data set, there was no correlation between the level of performance of observers, as measured by Az, and their ability to discriminate between modes. Analyses designed to identify observers who were consistent in their rankings as to their ability to discriminated between modes was also unsuccessful in showing differences. This study suggests that without the use of special procedures or training, it is not likely that most research facilities will be able to select a group of highly experienced observers who consistently detect small differences among modes.© (2002) COPYRIGHT SPIE--The International Society for Optical Engineering. A 67-year-old man underwent left atrial appendage (LAA) exclusion concomitant with mitral valve surgery and radiofrequency ablation maze procedure. On transoesophageal echocardiography anticipating ablation for left atrial tachycardia, an echodense thrombus was visualised in the LAA location with apparent intracavitary extension into the left atrium. Based on CT imaging findings, the echo represented thrombosis of a large left atrial appendage with probable extension into the left atrium.

Cryptococcus neoformans is a ubiquitous saprophytic fungus with worldwide distribution. It has been found in nature, primarily in association with bird droppings, but nonavian sources have been described as well (10). Although the lungs are thought to be the portal of entry for cryptococcus, pulmonary infection is uncommon. The organism is trophic to the central nervous system and the vast majority of recognized infections involve meningitis. Before the acquired immunodeficiency syndrome (AIDS) epidemic (1980), cryptococcosis was a rare infection, and pulmonary disease was described in 10%–45% of patients with cryptococcal meningitis (2). However, the true incidence of pulmonary cryptococcosis (PC) was not known, but was much lower than meningitis. Most PC cases were identified histologically and often at autopsy, with only about 20% diagnosed by culture (2, 3, 13). In recent years, many of the reports of cryptococcal infection have been in the human immunodeficiency virus (HIV)-positive population, and little is known of the disease in non-AIDS patients, particularly pulmonary involvement. At our institution, a tertiary care medical center with a large transplant program, sporadic cases of PC have occurred in solidorgan transplant recipients as well as in patients with a variety of different medical conditions. The purpose of our study was to investigate the epidemiology of PC in non-AIDS patients, and to determine predictors of outcome among these patients.

Purpose To determine the incidence of acute respiratory failure (ARF) in non-AIDS patients with pulmonary cryptococcosis (PC). Design Retrospective cohort study. Setting University of Pittsburgh Medical Center, Pittsburgh, PA. Subjects All patients in whom PC without HIV infection was diagnosed between February 1989 and March 1999. Results Thirty-three patients with PC were identified, and 11 of those patients (33%) developed ARF and comprised the study group. Underlying diseases included solid-organ transplant recipients (seven patients; 64%) and other underlying medical conditions (four patients; 36%). The most common symptoms were cough, shortness of breath, and temperature elevation. Extrapulmonary involvement was seen in six patients (meningitis, four patients; peritonitis, one patient; laryngeal mass, one patient). Six of the 11 patients (55%) died. Conclusion ARF may develop in one third of non-AIDS patients with PC. This clinical syndrome is associated with the dissemination to extrapulmonary sites and high mortality rates. PC should be recognized as a possible cause of respiratory failure in non-AIDS patients.
