Abstract

International audience; Background The identification of patients at high risk of a disabling disease course would be invaluable in guiding initial therapy in Crohn’s disease (CD). Our objective was to evaluate a combination of clinical, serological, and genetic factors to predict complicated disease course in pediatric-onset CD. Methods Data for pediatric-onset CD patients, diagnosed before 17 years of age between 1988 and 2004 and followed more than 5 years, were extracted from the population-based EPIMAD registry. The main outcome was defined by the occurrence of complicated behavior (stricturing or penetrating) and/or intestinal resection within the 5 years following diagnosis. Lasso logistic regression models were used to build a predictive model based on clinical data at diagnosis, serological data (ASCA, pANCA, anti-OmpC, anti-Cbir1, anti-Fla2, anti-Flax), and 369 candidate single nucleotide polymorphisms. Results In total, 156 children with an inflammatory (B1) disease at diagnosis were included. Among them, 35% (n = 54) progressed to a complicated behavior or an intestinal resection within the 5 years following diagnosis. The best predictive model (PREDICT-EPIMAD) included the location at diagnosis, pANCA, and 6 single nucleotide polymorphisms. This model showed good discrimination and good calibration, with an area under the curve of 0.80 after correction for optimism bias (sensitivity, 79%, specificity, 74%, positive predictive value, 61%, negative predictive value, 87%). Decision curve analysis confirmed the clinical utility of the model. Conclusions A combination of clinical, serotypic, and genotypic variables can predict disease progression in this population-based pediatric-onset CD cohort. Independent validation is needed before it can be used in clinical practice.

Abstract

Background: The COVID-19 pandemic placed unprecedented strain on healthcare globally, which exacerbated factors leading to unplanned pregnancies. This study aimed to assess the complications of self-administration of misoprostol for miscarriage during the COVID-19 lockdown. Methods: This was a retrospective cohort study conducted at the Department of Obstetrics and Gynecology, Abbasi Shaheed Hospital, Karachi, from 1st March to 30th September 2020. Case records were compiled and analyzed. All women who reported after self-administration of misoprostol were included. The primary outcome measure was complications; anemia, blood transfusion, heavy bleeding, shock, sepsis, ruptured ectopic pregnancy, and ruptured uterus. A bivariable analysis was done to see the effect of gestational age at intake of misoprostol on the complications encountered, the chi-square test or Fischer’s exact test was also applied. Results: In this study, a total of 54 cases were studied. The most common complication was anemia (63%) followed by incomplete abortion (61.1%). Heavy bleeding (p˂0.001), blood transfusion (p=0.020), and incomplete abortion were significantly (p˂0.001) more common in women who used misoprostol at gestational ages greater than 6 weeks. Conclusion: Our results suggested that health complications were found in women who self-medicated misoprostol for induced abortions after gestational ages greater than 6 weeks during the COVID-19 lockdown.

13. Bt maize: a tool for improving food safety of grains at harvest

Abstract

A new EU (European Union) regulation came into force in 2007 with Regulation (EC) No. 1126/2007 which established maximum levels for fumonisins B₁ and B₂ (4000 ppb), deoxynivalenol (1750 ppb) and zearalenone (350 ppb) in maize and maize products. In order to evaluate French maize food safety, studies were carried out by the national Biological Risk Monitoring (BRM) Network. In this study, field trials involving 84 plots were conducted with Bt maize (MON 810) and its isogenic non-Bt counterpart in 2005 and 2006 in South-western France. Mycotoxin levels were determined in grain at harvest. Fumonisins B₁ and B₂, deoxynivalenol, and zearalenone were analyzed by LC-MS-MS and the results treated statistically using non parametric tests for mycotoxins and analysis of variance test for weather variables. As the climate was homogenous inside the experimental area, the transgenic event introduced into the maize was the only key parameter which differed between Bt and non-Bt maize plots. Our results showed that all mycotoxin families were not impacted in the same way. The efficacy of Bt maize reduced mycotoxins more than 90% for fumonisins and more than 50% for zearalenone although deoxynivalenol was lightly increased. Therefore a competition between the different Fusarium spp. which produced fumonisins or trichothecenes is hypothesized. According to Regulation (EC) No. 1126/2007, 93% of the maize of Bt maize plots were able to be commercialized compared to only 45% for non-Bt maize plots. The results of this work showed that Bt maize improved food safety and constituted an useful tool to reduce significantly mycotoxin levels in harvested and stored grains.

Abstract

International audience; Scotch broom (Cytisus scoparius) is a very dominant species, often having undesirable environmental impacts. Browsing appears to be a powerful tool to control it, since it is targeted at the juvenile stage, which is the most sensitive in the demographic strategy of this species. A seven-year experiment on a natural Scotch broom shrubland in a French Pyrenean rangeland assessed the effects of three different browsing intensities (two levels of manual stem biomass removal simulating light and heavy browsing vs an untreated control) on juvenile survival, morphology and reproductive success at a young adult stage. These browsing strategies, representing pertinent management practices, were repeated over a five-year period. We also analysed the recovery of broom plants, i.e. surviving young adults after two years without browsing. Our results demonstrate that browsing the young plants is a promising strategy for controlling broom.

cited By 1; International audience; Crops of maize (Zea mays L.) were conducted in southwestern France with GMO (Genetic Modified Organism) vs isogenetic varieties in order to verify the control of European Corn Borer (ECB) Ostrinia nubilalis (Hübner) and the Corn Stalk Borer (CBS) Sesamia nonagrioides (Lefevbre) by GMO in field conditions. The bioassays were carried out in 1998 and 1999 before moratorium, then in 2005. Experiments involved respectively 18, 12 and 19 fields cultivated with Furio/Furio cb (GMO), Cecilia/ Elgina (GMO) and PR33P66/PR33P67 (GMO) varieties. These transgenic events expressed Cry1A(b) protein (Bt maize). Plants were noted for insect infestation assessment (number of larvae in stalks and ears per plant). Statistical tests used t-test on couple of plots. Results showed a significant difference in the density of both ECB and CBS between control and the two transgenic events. The two transgenic events acted differently. The control of the two Bt events on the two pests were differentiated and discussed. These experiments underlined the importance of field evaluation for testing real effects of transgenic events on crop according the environmental context.

Abstract

A simple density-dependent reinvasion model is described and used to estimate tsetse fly populations on the basis of removal trapping experiments. The model was tested on #Glossina fuscipes fuscipes$ Newstead in the Central African Republic and #G. palpalis palpalis$ (Robineau-Desvoidy) in the Republic of Congo (Brazzaville). The density-dependence is modelled by postulating that the inflow of flies each day is proportional to the deficit relative to the equilibrium population. Non-linear least square techniques were used to estimate the following parameters : the daily capture rate, the strength of the density-dependence, and the equilibrium fly population, at the beginning and at the end of the trapping experiment. The model ignores birth and death rates of flies and is applicable only when a rapid decrease in population occurs over a short period (between 10 and 20 days). Over longer periods one could not ignore the natural growth of the populations was well as other more complex density-dependent mechanisms. (Résumé d'auteur)

Abstract

La structure spatio-temporelle d'un peuplement de glossines (#Glossina palpalis$, #G. pallicera$ et #G. nigrofusca$) a été étudiée à partir de données de capture au piège biconique. Les dynamiques temporelles des deux espèces dominantes, #G. palpalis$ et #G. pallicera$ (67,6% et 30,1% des captures) sont en opposition de phase. Il apparaît également une structuration spatiale pour ces deux espèces. L'analyse triadique permet de préciser ce qui ressort d'une structuration permanente liée à l'habitat et ce qui est dû aux fluctuations saisonnières. Elle précise les sites à #G. nigrofusca$, espèce marginale qui ne représente que 2,3% des captures. Les deux espèces dominantes se suivent en alternance dans les zones à forte densité de capture (mosaïque forêt-plantation, savanes incluses, lignes de bas-fond avec présence de points d'eau). Cette "exclusion" relative pourrait être liée à l'homme (#G. palpalis$ est presque toujours associé à de gros campements, #G. pallicera$ aux petits campements ou à l'absence de l'homme), mais aussi à des interactions interspécifiques complexes. #G. nigrofusca$ apparaît dans cette zone comme une espèce évitant l'habitat humain. Cette approche plurispécifique souligne l'importance de #G. pallicera$ dont le rôle épidémiologique reste à élucider. (Résumé d'auteur)

Abstract

Morphological and functional abnormalities of the megakaryocytic series have been well described in myelodysplastic syndromes. Platelet peroxidase has always been demonstrated in abnormal megakaryocytes and early megakaryoblasts in such syndromes. We have studied a case of myelodysplastic syndrome with marked morphological abnormalities of megakaryocytes in which ultrastructural studies showed the coexistence of platelet peroxidase positive and platelet peroxidase negative megakaryocytes. This enzymatic deficiency was confirmed by the ultrastructural study of circulating platelets. This case appears to be the first report of a partial platelet peroxidase deficiency. It adds to the enzymatic abnormalities in myelodysplastic syndrome already described for the red cells and the granulocytic cells. [ABSTRACT FROM PUBLISHER]
